キメラ抗原受容体(CAR)、CAR発現免疫エフェクター細胞ならびにCAR及びCAR T細胞を作製する及び使用する方法を本明細書に記載する。向上した治療特性を有する免疫エフェクター細胞組成物及び抗原提示細胞も提供する。
2.関連技術の説明
臨床グレードのT細胞の効力は、遺伝子療法と免疫療法を組み合わせて、より優れた(i)腫瘍関連抗原(TAA)の認識、(ii)注入後の持続性、(iii)腫瘍部位への移入可能性、及び(iv)腫瘍微小環境内でエフェクター機能を再循環させる能力の可能性を有する生物学的産物を遺伝子操作することにより改善することができる。T細胞のかかる遺伝子工学を使用して、細胞の特異性を再指向し、抗原を標的とする細胞毒性活性を有する治療用組成物を提供することができる。これらの遺伝子操作T細胞組成物は、例えば、癌患者における治療的介入に有効であると示されている(Jena et al.,2010;Till et al.,2008;Porter et al.,2011;Brentjens et al.,2011;Cooper and Bollard,2012;Kalos et al.,2011;Kochenderfer et al.,2010;Kochenderfer et al.,2012;Brentjens et al.,2013)。しかしながら、改善されたT細胞及びin vivo持続性の制御を可能にするCAR構築物に対する必要性が依然として残っている。
本明細書で用いるとき、「a」または「an」は、1つまたは複数を意味し得る。特許請求の範囲(複数可)で用いるとき、「含む」という語句と併せて使用される時、「a」または「an」という語句は、1つ以上を意味し得る。
キメラ抗原受容体分子は、組換え融合タンパク質であり、抗原(例えば、HER1/HER3)と結合する能力及びそれらの細胞質尾に存在する免疫受容体活性化モチーフ(ITAM)を介して活性化シグナルを伝達するという両方の能力により識別される。抗原結合性部分(例えば、一本鎖抗体(scFv)から生成される、を利用する受容体構築物は、それらが、HLAに依存しない様式で標的細胞表面上の未変性抗原と結合するという点で「汎用」であるという追加の利点を有している。

ある特定の態様では、本実施形態のCARポリペプチドは、抗原結合性ドメインと膜貫通ドメインの間に位置するヒンジドメインを含むことができる。いくつかの場合では、ヒンジドメインを、CARポリペプチド内に含んで、抗原結合性ドメインと細胞表面の間に十分な距離を提供するまたはCAR遺伝子改変T細胞の抗原結合性またはエフェクター機能に有害な影響を与え得る潜在的な立体障害を軽減し得る。

ある特定の態様では、本明細書に記載の実施形態は、抗原特異的再指向免疫エフェクター細胞(例えば、T細胞、NK細胞またはNK T細胞)を作製する及び/または拡大する方法であって、細胞を、CARをコードするDNA(またはRNA)構築物を含有する発現ベクターでトランスフェクトして、次いで、任意で細胞をフィーダー細胞、組換え抗原、または受容体に対する抗体で刺激して、細胞を増殖させることを含む、前記方法を含む。ある特定の態様では、CARを発現するように遺伝子操作された細胞(または細胞集団)は、幹細胞、iPS細胞、免疫細胞またはこれらの細胞の前駆体である。以下に記載する方法は、CAR発現のためのT細胞(または他の免疫細胞)遺伝子操作の特定の例に取り組む。
いくつかの場合では、本実施形態の免疫エフェクター細胞(例えば、T細胞)は、活性化及び増殖細胞(AaPC)と共培養されて、細胞拡大を助ける。例えば、抗原提示細胞(APC)は、本実施形態の治療用組成物及び細胞療法生成物の調製に有用である。抗原提示システムの調製及び使用に関する一般的なガイダンスについては、例えば、米国特許第6,225,042号、同第6,355,479号、同第6,362,001号及び同第6,790,662号;米国特許出願公開第2009/0017000号及び同第2009/0004142号;ならびに国際公開番号WO2007/103009を参照されたく、このそれぞれは参照により本明細書に組み込まれる。
ある特定の実施形態では、遺伝子操作抗原提示細胞(APC)をさらに提供する。かかる細胞は、例えば、上記のように、免疫エフェクター細胞をex vivoで増殖させるために使用され得る。さらなる態様では、遺伝子操作ACPは、それら自体が患者に投与されて、それにより免疫エフェクター細胞の拡大をin vivoで刺激してよい。本実施形態の遺伝子操作APCは、それら自体が治療剤として使用されてよい。いくつかの実施形態では、遺伝子操作APCは、標的抗原に特異的な内因性免疫エフェクター細胞の活性化を刺激することができる治療剤として使用することができる、及び/または標的抗原に特異的な養子移入された免疫エフェクター細胞の活性もしくは持続性を増加させるために使用することができる。
いくつかの態様では、本実施形態のキメラ抗原受容体構築物及び細胞は、癌を有するまたは有すると疑われる対象において、これらの対象の腫瘍のサイズを低下させるまたは腫瘍の成長もしくは再成長を防止することにより適用される。従って、本明細書に提供する実施形態は、本発明の実施形態のキメラ抗原受容体構築物を対象の単離されたT細胞へと導入し、形質転換したT細胞を対象に再導入し、それにより抗腫瘍応答に影響をあたえて対象における腫瘍を減少させるまたは除去することによる、対象において成長を減少させるまたは腫瘍形成を防止するための方法にさらに関する。使用することができる好適なT細胞には、細胞毒性リンパ球(CTL)または破壊を必要とするT細胞受容体を有する任意の細胞が含まれる。当業者に周知であるように、対象からこれらの細胞を単離するための様々な方法が容易に利用可能である。例えば、細胞表面マーカー発現を使用することまたは市販のキット(例えば、Pierce製のISOCELL(商標)、イリノイ州、ロックフォード)を使用することである。
本明細書に記載の組成物のいずれもが、キットに含まれてよい。いくつかの実施形態では、アロジェネイックなCAR T細胞がキット内に提供され、これは、細胞の拡大に好適な試薬、例えば、培地、APC、遺伝子操作APC、成長因子、抗体(例えば、CAR T細胞を選別するまたは特徴付けるため)及び/または導入遺伝子をコードするプラスミド、例えば、標的抗原、HLA、ミトコンドリアのレポーター、CARまたはトランスポゼースも含んでよい。
本発明の実施形態を、以下の実施例を参照することにより詳細にさらに記載する。これらの実施例は、例示の目的でのみ与えられ、別段の指定がない限り、限定することを意図しない。ゆえに、本発明は、決して以下の実施例に限定されると解釈されるべきではなく、むしろ、本明細書に提供する教示の結果として明らかになる任意及び全ての変形を包含すると解釈されるべきである。
CAR T細胞持続性におけるCARポリペプチドに結合するFc受容体の役割を調査した。最初の試験については、CD19受容体を標的とするCAR構築物を試験した。CAR T細胞は、CD19RCD28構築物を発現し、これはCD19結合ドメイン(VL/VH);ヒンジドメイン(IgG4−Fc);膜貫通ドメイン(CD28 TM)及び細胞内シグナル伝達ドメイン(CD28−CD3ζ)を含む(図1を参照されたい)。CAR構築物を、細胞内にエレクトロポレーションを介して導入し、これには、スリーピングビューティーをベースとするトランスポゾンシステムを使用して構築物の遺伝子組み込みを媒介した(例えば、参照により本明細書に組み込まれる国際(PCT)出願番号PCT/US14/38005を参照されたい)。産生されたCAR T細胞を、次いで、フローサイトメトリーにより分析してCARポリペプチド発現を評価した。CARポリペプチドは、トランスフェクション後に効果的に発現すると見いだされ、結果得られた細胞集団の高い割合が、K562 aAPCとの共培養の28日後にCAR発現について陽性であった(図2A)。フローサイトメトリー試験は、CAR T細胞のある割合がin vitroでFcγR2aにより結合されることも示し、このことは、Fc受容体の結合が、in vivoでCASR T細胞に影響する可能性が高く、持続性CAR T細胞において役割を担う可能性があることを示す。
異なるCAR T細胞構築物のアレイを産生して、CAR T細胞に対するFc受容体結合を調節した。被験構築物を図4に示す。具体的には、異なるヒンジ配列を、多くの異なる膜貫通及び細胞内/シグナル伝達ドメインに関連して検査した。図5に表すフローサイトメトリー試験は、全ての被検構築物が、トランスフェクトされたT細胞(トランスフェクションは、CAR受容体構築物の組込みのためにスリーピングビューティーをベースとするトランスポゾンシステムを使用するエレクトロポレーションにより上記のように完了した)にて効果的に発現されたことを示す。同様に、結果得られたT細胞の高い割合が、K562 aAPCとの共培養の28日後にCAR受容体陽性であった(図5)。K562 aAPCとの共培養物におけるCAR T細胞拡大の動態を図7に示し、全てのCAR T細胞が同様の有効性で拡大したことを実証する。
追加の試験を、CD19受容体を標的とするCAR構築物を使用して実施した。最初の試験と同様に、PBMCをCD19R*CD28またはCD19RCD8CD28トランスポゾン&SB11トランスポゼースでエレクトロポレートし、IL−2及びIL−21と共に照射したクローン1上で、28日間にわたって7日間の刺激サイクルで共培養した。各刺激サイクルの終了時に、細胞を数え上げ、表現型を決定した(CD3、CAR)(図13A−D)。
自己または同種異系マウスモデルのいずれかにおけるCAR T細胞注入の後、サイトカインIL−2、IL−4、IL−7、IL−9及びIL−15のレベルをモニターした。追加のシグナル伝達構築物を探索して持続性を改善した。図15は、膜結合型IL−15/IL−15受容体(mIL15)構築物を示す。mIL−15構築物は、T細胞においてCARと共発現した。図16のデータ(例えば、左パネルを参照されたい)は、エレクトロポレートした細胞の大部分でのCAR及びmIL15の共発現を実証する。さらなるデータを図16に示し、これは、CARと共発現したときに、mIL−15が、抗原除去後の培養物における減衰からCAR T細胞を保護することができたことを示す(図16、上右パネルを参照されたい)。
T細胞をex vivoで増殖させて抗原提示細胞(T−APC)として機能させて腫瘍関連抗原(TAA)NY−ESO−1を提示させた。スリーピングビューティー(SB)システム(Hackett et al.,2010)を適応させてT細胞を遺伝子改変した(図20)(Cooper et al.,2005)。これは、2つのステップを必然的に含む:(i)SBトランスポゾン[すなわち、T細胞特異性を再指向するためのキメラ抗原受容体(CAR)(Jin et al.,2011;Kebriaei et al.,2012)]及びSBトランスポゼースを発現するDNAプラスミドの電気的導入及び(ii)CD64(Fc受容体)、CD86、CD137L及びインターロイキン(IL)−15の膜結合型ムテイン(mIL−15、図21)を共発現するK562細胞株に由来するデザイナー人工抗原提示細胞(aAPC)上の構成要素を安定的に発現するT細胞の増殖及び拡大。NY−ESO−1免疫原の発現を強制するために、培養物中のT−APCを、ハイグロマイシンで選択した。これは、プラスミドSB発現カセットが、ハイグロマイシンホスホトランスフェラーゼ(Hy)とチミジンキナーゼ(HyTK)の融合体を共発現するためである(図20)。遺伝子改変T細胞を、細胞致死性濃度のハイグロマイシンBの存在下でγ−照射OKT3負荷aAPCによる連続的な刺激により増殖させた(図20)。T細胞は、in vitro及びin vivoでT−APCとして機能することができ、これにはウイルス抗原インフルエンザA MP1を使用する(Cooper et al.,2005)。この実施例では、T細胞を遺伝子操作して、NY−ESO−1(FLAGタグ付きHyTKに融合)を発現させ(図22)、NY−ESO−1+mIL−15+HLA−A2+T−APCを、人工抗原提示細胞(aAPC)の存在下で増殖させ(図26)これにはSBシステムを使用した(Hackett et al.,2010)。
本明細書に記載の実施形態のmIL15+NY−ESO−1+T−APCは、PBMCと共培養されたとき、NY−ESO−1五量体陽性T細胞を生成することができ、in vitroで骨髄腫細胞を殺傷できるNY−ESO−1+CAR T細胞を拡大することができる(図23及び24)。しかしながら、T−APCからのCD137L活性化の不在ならびにLAG3発現からの阻害の増大は、反応の減少をもたらし得、これは3週間の共培養後に観察された(図25)。LAG3発現は、T−APCの各刺激で増加し、枯渇の代用となり得る。
全ての細胞(癌性及び一次の両方)は、利用可能な代謝産物に応答して、それらのミトコンドリアを正当な立体配置から凝縮状態へと切り替える能力を有し、逆もまた同様である(Krauss et al.,2001)。ミトコンドリアの形状は、類似するex vivo培養条件において成長させたモックをエレクトロポレートした対照T細胞と比較してCAR改変T細胞において異なる(図29A及び29B)。ある特定のCAR改変T細胞が、凝縮状態のミトコンドリアを有するというこの所見は、CD28シグナル伝達ドメインで改変したT細胞と、CD137 T細胞シグナル伝達ドメインで改変した細胞を共刺激の不在下で比較したときに顕著になった(図30)。ゆえに、ex vivoで成長した未改変及び改変T細胞の代謝性需要と消費パターンは異なり、細胞がex vivoで特異的共刺激の不在下で増殖したときに、顕著になる。これに並行して、マウスモデルにおいて、ミトコンドリアの機能及びSRC(予備呼吸能)をCD8+T細胞において維持することが、感染後の安定したCD8+メモリーT細胞形成にとって重要であることが示された(Pearce et al.,2013;van der Windt et al.,2012)。SRCは、作業またはストレスが増加した条件下でエネルギーを産生するための細胞において利用可能である追加的なミトコンドリアの能力として説明され、長期細胞生存及び機能にとって重要であると考えられる。一連の実験を、in vitroで行って、通常のミトコンドリア質量を有するT細胞と比較して、高いミトコンドリアの強さを有するT細胞がより良好に生存し得るかどうか、それにより、in vivoにおいてより有効であり得るかどうかを決定した。
CAR+T細胞の遺伝子改変及びex vivo増殖は、ミトコンドリアの代謝の変化を引き起こし、4−1BB媒介性T細胞シグナル伝達は、課題とする培養環境においてCAR+T細胞のより良好な生存を促進する。

Claims (35)
- 抗原結合性ドメイン;ヒンジドメイン;膜貫通ドメイン及び1つまたは複数の細胞内シグナル伝達ドメイン(複数可)を含むキメラ抗原受容体(CAR)ポリペプチドであって、前記ヒンジドメインがIgG4−Fc配列を含み、前記IgG4−Fc配列が、野生型IgG4−Fcに対して、Fc受容体結合を低下させる少なくとも1つの突然変異を含む、前記キメラ抗原受容体(CAR)ポリペプチド。
- 前記IgG4−Fc配列が、配列番号1に対して少なくとも約90%同一である、請求項1に記載のCARポリペプチド。
- 前記IgG4−Fc配列が、前記野生型配列に対して、突然変異を位置L235またはN297で含む、請求項1に記載のCARポリペプチド。
- 前記IgG4−Fc配列が、前記野生型配列に対して、突然変異を位置L235及びN297で含む、請求項1に記載のCARポリペプチド。
- 前記IgG4−Fc配列が、前記野生型配列に対して、L235EまたはN297Q突然変異を含む、請求項1に記載のCARポリペプチド。
- 前記IgG4−Fc配列が、前記野生型配列に対して、L235E及びN297Q突然変異を含む、請求項1に記載のCARポリペプチド。
- 前記IgG4−Fc配列が、配列番号1に対して同一である、請求項1に記載のCARポリペプチド。
- 抗原結合性ドメインが、scFvを含む、請求項1に記載のCARポリペプチド。
- 前記細胞内細胞シグナル伝達ドメインが、CD3ζからのドメインを含む、請求項1に記載のCARポリペプチド。
- 前記細胞内細胞シグナル伝達ドメインが、CD28またはCD137(4−1BB)からの細胞内ドメインをさらに含む、請求項9に記載のCARポリペプチド。
- 前記膜貫通ドメインが、CD28、CD8aまたはCD137の膜貫通ドメインを含む、請求項1に記載のCARポリペプチド。
- 前記抗原結合性ドメインが、感染性疾患抗原または癌細胞抗原に結合する、請求項1に記載のCARポリペプチド。
- 前記抗原結合性ドメインが、癌細胞抗原に結合する、請求項12に記載のCARポリペプチド。
- 前記癌細胞抗原が、CD19であり、前記CARが、CD19標的CARである、請求項13に記載のCARポリペプチド
- 請求項1〜14のいずれか一項に記載のCARポリペプチドをコードする核酸分子。
- 請求項1〜14のいずれか一項に記載のCARポリペプチドまたは請求項15に記載の核酸を含む、単離された免疫エフェクター細胞。
- 前記細胞が、T細胞である、請求項16に記載の細胞。
- 前記細胞が、ヒト細胞である、請求項16に記載の細胞。
- 薬学的に許容され得るキャリア中で請求項16に記載の細胞の集団を含む医薬組成物。
- 対象を処置する方法であって、請求項1〜14のいずれか一項に記載のCARポリペプチドを発現する有効量のキメラ抗原受容体(CAR)T細胞を投与することを含む、前記方法。
- 標的抗原をコードする第一の導入遺伝子及びヒト白血球抗原(HLA)をコードする第二の導入遺伝子を含む、遺伝子操作抗原提示細胞(APC)であって、前記HLAが、前記標的抗原のエピトープと複合体化して前記APCの表面上に発現されている、前記遺伝子操作抗原提示細胞。
- 前記APCが、不死化されていない、請求項21に記載の遺伝子操作APC。
- 前記APCが、一次細胞である、請求項21に記載の遺伝子操作APC。
- 前記APCが、T細胞またはT細胞前駆体である、請求項21に記載の遺伝子操作APC。
- 前記APCが、TCRαβまたはTCRγδを発現するT細胞である、請求項21に記載の遺伝子操作APC。
- 共刺激分子をコードする少なくとも第三の導入遺伝子をさらに含む、請求項21に記載の遺伝子操作APC。
- 前記標的抗原が、NY−ESO−1である、請求項21に記載の遺伝子操作APC。
- 疾患を有する対象を処置する方法であって、請求項21〜26のいずれか一項に記載の有効量の遺伝子操作APCを前記対象に投与することを含み、前記APCが前記疾患に関連する標的抗原を発現する、前記方法。
- 前記対象が、前記APCによりコードされる前記標的抗原を発現する癌を有する、請求項28に記載の方法。
- 標的抗原が、NY−ESO−1である、請求項28に記載の方法。
- ミトコンドリアの予備呼吸能が高いCAR改変T細胞を選択する方法であって、ミトコンドリアのレポーター遺伝子の発現レベルを検出すること及び前記ミトコンドリアのレポーター遺伝子の発現レベルが上昇したCAR改変T細胞を選択すること、それによりミトコンドリアの予備呼吸能を有するCAR改変T細胞を選択することを含む、前記方法。
- 前記ミトコンドリアのレポーター遺伝子が、内因性遺伝子である、請求項31に記載の方法。
- 前記ミトコンドリアのレポーター遺伝子が、外因性遺伝子である、請求項31に記載の方法。
- 前記外因性遺伝子が、蛍光レポータータンパク質をコードする、請求項33に記載の方法。
- 前記蛍光レポータータンパク質が、ミトコンドリア局在化配列を含む、請求項34に記載の方法。
