Description
본 발명은 대체로 아미노산을 포함하는 조성물에 관한 것이다. 더욱 특히, 본 발명은 의약에 사용하기 위한, 특히 미토콘드리아 기능 이상과 관련된 질병의 치료에 사용하기 위한 아미노산을 포함하는 조성물에 관한 것이다.
미토콘드리아는, 주요 기능이 산화적 인산화, 즉, 포도당 또는 지방산의 대사로부터 나온 에너지가 아데노신 트리포스페이트(ATP)로 전환되는 과정인 세포 소기관이다. 이어서, ATP는 다양한 에너지-요구 생합성 반응 및 다른 대사 활동을 구동하는 데 사용된다.
미토콘드리아 기능 이상은 임의의 조직에 영향을 줄 수 있으며, 다양한 조직이 관여하는 정도에 따라 많은 다양한 증상을 초래한다.

미토콘드리아 기능 이상은 비만, 및 제2형 당뇨병, 고혈압, 이상지질혈증, 심부전, 신장 질환 및 골다공증을 포함한 관련 장애에서 또한 발견되었다.

따라서, 전세계적인 비만의 유행과 인구 고령화의 증가를 고려해 볼 때, 다가올 미래에 가장 빈번한 환자는 근육 감소증이 있는 비만 노인 환자일 것이다.
미토콘드리아 생합성(mitochondrial biogenesis) 및 활성에서 연령 관련 결손을 완화시키는 전략은, 포유 동물에서 생존을 촉진하는, 칼로리 제한(CR) 및 적당한 육체 운동을 포함한다. CR은 염증성 장애, 심혈관 장애, 암 및 신경퇴행성 장애의 예방을 통해 건강한 상태에서의 기대 수명(건강 수명)을 연장시키는 것으로 밝혀졌다. CR은 미토콘드리아 생합성에 관여하는 유전자의 발현을 촉진하고, 노화 또는 손상된 세포 및 조직에서 미토콘드리아 기능을 개선시킨다(문헌[Nisoli et al., 2005]).
CR은 인간에게 유익한 효과가 있지만, 그러한 식이 요법이 노인들에게 널리 채택될 가능성은 없다.
칼로리 섭취의 감소없이 식이 제한의 이점을 제공하기 위한 대안적인 해결책으로서, 예를 들어 EP 2 196 203 B1에 개시된 바와 같은, 아미노산을 포함하는 특정 조성물이 포유 동물에게 투여되었다.

본원에 개시된 조성물은 활성 작용제를 포함하며, 활성 작용제는 아미노산으로 류신, 이소류신, 발린, 트레오닌, 라이신을 그리고 산으로 시트르산, 숙신산, 말산을 함유한다.
EP 2 196 203 B1에 개시된 바와 같은 아미노산을 포함하는 조성물이 칼로리 제한(CR)의 이점을 제공하기 위한 대안적인 해결책으로서 포유 동물에게 투여되었고; 그러한 아미노산 기반 조성물(본 개시에서는 "BCAAem"으로 지칭됨)은 심근 및 골격근 둘 모두에서 증가된 미토콘드리아 생합성을 가져오는 것으로 밝혀졌다(문헌[D'Antona et al., 2010]). 이러한 효과는 내피 산화 질소 합성 효소(eNOS)의 발현 및 결과적인 산화 질소(NO) 생성에 의해 매개되었다(문헌[D'Antona et al., 2010]).

본 발명자는 함유된 산이 상이한 다수의 조성물을 시험하였고, 류신, 이소류신, 발린, 트레오닌 및 라이신과 함께 시트르산, 숙신산 및 말산을 활성 작용제로서 포함하는 조성물이 표시된 목적에 매우 효과적임을 발견했다. 실제로, 상기 언급된 활성 작용제를 포함하는 조성물뿐만 아니라 추가의 특정 아미노산(하기 표 1에 열거됨)을 포함한 상기 언급된 활성 작용제를 포함하는 조성물은, 미토콘드리아 생합성 및 기능을 촉진하는 데에 있어서 이전에 시험된 아미노산 기반 조성물(BCAAm)보다 유의하게 더 효과적이다.
조성물은 심근 세포주(HL-1), 즉, 심장 기능의 시험관내 모델을 나타내는 세포에서 시험되었다. 이러한 심근 세포의 분석으로부터 도출된 결과는 심부전의 예방에서 신규 조성물의 효능을 검증하는 데 이용될 수 있다.

바람직한 구현예에서, 시트르산, 말산, 숙신산의 전체 몰량과, 메티오닌, 페닐알라닌, 히스티딘 및 트립토판의 전체 몰량의 비는 1.35를 초과한다.
하나 이상의 구현예에서, 시트르산, 말산, 숙신산의 합계와, 분지쇄 아미노산인 류신, 이소류신, 발린의 합계의 중량 비는 0.1 내지 0.4, 바람직하게는 0.15 내지 0.35이다.

또 다른 바람직한 구현예에서, 본원에 개시된 조성물은, 바람직하게는 알츠하이머병, 파킨슨병 및 헌팅턴병 중에서 선택된, 미토콘드리아 기능 이상-관련 신경퇴행성 질병의 치료 및/또는 예방에 사용될 수 있다.
추가의 구현예에서, 조성물은 죽상 경화증, 제2형 당뇨병, 신장 질환, COPD 및 골다공증으로 이루어진 군에서 선택된 미토콘드리아 기능 이상-관련 질병의 치료 및/또는 예방에 사용될 수 있다.
하나 이상의 구현예에서, 조성물은 영양실조 임상 상황의 치료 및/또는 예방, 특히 단백질 영양실조 및 악액질의 치료 및/또는 예방에 사용될 수 있다.
추가의 구현예에 따르면, 아미노산 조성물은, 예를 들어 단백질, 비타민, 탄수화물, 천연 및 인공 감미료 및/또는 착향 물질과 같은, 약학적으로 허용되는 부형제를 포함할 수 있다. 바람직한 구현예에서, 약학적으로 허용되는 부형제는 유청 단백질, 말토덱스트린, 과당, 카제인칼슘, 어유, 시트르산 또는 그의 염, 수크랄로스, 수크로스 에스테르, 비타민 D3, 비타민 B군으로부터 선택될 수 있다.

실시예
표 1은 하기에 개시된 바와 같은 HL-1 세포에 대해 시험된 다양한 아미노산 기반 조성물을 나타낸다.
구체적으로, BCAAem 조성물은 EP 2 196 203 B1에 개시된 조성물이다.
"B2"로 명명된 조성물은 BCAAem 조성물과 유사하지만 시트르산을 추가로 포함하는 아미노산의 조합물을 활성 작용제로서 갖는다. 그러한 조성물은 또한 비타민 B1 및 B6을 포함한다.
알파 5(α5), 알파 6(α6), 알파 7(α7)로 명명된 조성물은 아미노산, 및 시트르산, 숙신산 및 말산을 함유하는 활성 작용제를 포함한다. 또한, 알파 7 조성물의 활성 작용제는 OKG(오르니틴 L-α 케토글루타레이트) 및 아미노산인 아스파테이트(L-아스파르트산)를 또한 포함한다.
상기 표 1의 조성물은 먼저 모든 성분을 0.8 메쉬로 시프팅(sifting)함으로써 제조할 수 있다. 예비-혼합물을 얻기 위해, (총량의 10 중량% 미만의 양의) 각각의 성분을 L-라이신 HCl의 일부와 함께 폴리에틸렌 백에 넣어 조성물의 총중량의 10%를 얻는다. 이어서, 백을 5분 동안 수동으로 진탕시킨다. 이어서, 예비-혼합물을 나머지 성분과 함께 믹서(Planetaria)에 로딩하고, 120 rpm에서 15분 동안 혼합하여 균질한 최종 조성물을 얻는다.

방법
세포 및 처리
HL-1 심근 세포(뉴올리언스 스쿨 오브 메디슨(New Orleans School of Medicine)의 W.C. Claycomb로부터 제공받은 것)을 피브로넥틴/젤라틴-코팅 플라스크에 플레이팅하고, 문헌[Claycomb et al., 1998]에 개시된 바와 같이 (30 mM L-아스코르브산[Sigma-Aldrich]에 용해된 10 mM 노르에피네프린[Sigma-Aldrich] 저장 용액으로부터의) 100 μM 노르에피네프린, 2 mM L-글루타민, 100 U/ml 페니실린, 100 μg/ml 스트렙토마이신 및 10% FBS(JRH Biosciences)가 보충된 클레이콤(Claycomb) 배지(JRH Biosciences)에서 70% 내지 80% 컨플루언스(confluence)로 성장시켰다.
세포를 표 1에 나타낸 (클레이콤 배지에 용해시킨) 1%(w/v)의 조성물로 24시간 또는 48시간 동안 처리하였다.
이러한 기간의 종료시에, 세포로부터 mRNA 및 DNA를 추출하거나 세포를 사용하여 산소 소비를 평가하였다. 대조 세포는 클레이콤 배지로만 처리하였다.
유전자 발현 및 미토콘드리아 생합성 방법
RNeasy Mini Kit(Qiagen)를 사용하여 HL-1 심근 세포로부터 총 RNA를 분리하였고; 문헌[D'Antona et al. (2010)]에 기재된 바와 같이 iScript cDNA Synthesis Kit(Bio-Rad Laboratories)를 사용하여 총 RNA의 1 마이크로그램을 cDNA로 역전사시켰다.
유전자 상대 수준을 2-DDCT로 계산하였고, 여기서 DDCT는 어느 한 처리의 DCT와 내부 대조군으로서 GAPDH를 사용한 미처리 그룹의 DCT 사이의 차이에 상응하였다. DDCT(Delta-Delta-CT) 알고리즘은 정량적 실시간 PCR(qRT-PCR) 실험으로 상대적 유전자 발현을 결정하기 위한 근사법이다(문헌[Livak and Schmittgen, 2001] 참조).
Beacon Designer 2.6 소프트웨어(Premier Biosoft International)를 사용하여 프라이머(하기 표 2에 보고된 서열)을 설계하였다. 값을 글리세르알데히드 3-포스페이트 탈수소효소(GAPDH)의 발현으로 정규화하였다.

GAPDH는 미토콘드리아 DNA를 정규화하는 데 사용되었음.
미토콘드리아 DNA(mtDNA) 분석을 위해, QIAamp DNA 추출 키트(QIAGEN)로 총 DNA를 추출하였다.
미토콘드리아 DNA(mtDNA) 유전자에 특이적인 프라이머를 사용하여 mtDNA를 증폭시키고, GAPDH 유전자 DNA의 증폭에 의해 게놈 DNA에 대해 정규화하였다. Beacon Designer 2.6 소프트웨어(Premier Biosoft International; Palo Alto, CA)를 사용하여 설계된 프라이머를 gDNA에 대하여 표 2에 나타낸다.
통계 분석
모든 유전자 발현 데이터에 대해, 양측 대응-표본 t 검정을 이용하여 대조 세포와 처리된 세포 사이의 값을 비교하였다. 0.05 미만의 p 값이 통계적으로 유의한 것으로 간주되었다.
산소 소비
표 1에 나타낸 조성물로 처리된 1 ml 양의 HL-1 심근 세포를 행크의 균형 염 용액(Hank's balanced salt solution)(Sigma)에 재현탁시키고, 펠렛 세포로 스피닝 다운(spinning down)시켰다. 다수의 HL-1 세포에 양성 대조군으로서 산화 질소(NO) 공여체, 구체적으로 DETA-NO로도 일컬어지는 디에틸렌트리아민-NO(Sigma-Aldrich, Milan, Italy)를 또한 보충하였다.
이어서, 세포를 3.0x106/ml의 밀도로 호흡 완충액(respiration buffer)(0.3 M 만니톨, 10 mM KCl, 5 mM MgCl2, 10 mM K2PO4, pH 7.4)에 재현탁시켰다.
차트 기록기에 연결된 클라크형 산소 전극(Rank Brothers Ltd.)이 장착된 기밀 용기 내에서 37℃에서 샘플을 분석하였다.
인큐베이션 배지 중의 산소 농도를 37℃에서 200 μmol/l로 가정하여 산소 전극을 보정하였다.
약 10분 동안 연속 혼합하면서 산소 소비를 평가하였다. 이어서, 트레이스 레코더(trace recorder)의 기울기를 이용하여 산소 소비를 계산하였다. 산소 함량은 세포의 양에 따라 달라질 수 있다. 따라서, 세포 함량과 직접적으로 상호 관련되는 단백질 함량을 사용하여 세포 샘플 중에서의 산소 소비를 정규화하였다. 비신코닌산 단백질(BCA) 검정을 이용하여 단백질 함량을 결정하였다.
결과
미토콘드리아 DNA(mtDNA)
미토콘드리아의 양에 대한 효과를 검증하기 위해 다양한 아미노산 조성물로 처리된 세포에서 먼저 미토콘드리아 DNA(mtDNA)를 평가하였다.
도 1에 도시된 바와 같이, α5 조성물로 처리된 HL-1 심근 세포는, 대조 세포 (CT)에서, B2-처리 세포에서, 그리고 매우 흥미롭게는 BCAAem 조성물로 처리된 세포에서 평가된 mtDNA에 비해 mtDNA의 가장 유의한 증가를 나타냈다.
PGC-1α, Tfam 및 Cyt c
다양한 아미노산 조성물의 효과를 미토콘드리아 생합성에 대해 또한 시험하였다. 구체적으로, HL-1 심근 세포에 의한 다음과 같은 마커의 발현을 평가하였다:
- 미토콘드리아 생합성의 마스터(master) 조절 인자인 퍼옥시좀 증식 인자-활성화 수용체 감마 보조활성제 1-알파(PGC-1α),
- mtDNA 복제를 조절하는 mtDNA 전사 인자인 미토콘드리아 DNA 전사 인자 A(Tfam),

BCAAem 조성물, B2 조성물(즉, BCAAem 조성물과 유사하지만 시트르산을 또한 포함하는 조성물), α5(즉, 아미노산 이외에 산인 시트르산, 숙신산, 말산을 또한 활성 작용제로서 포함하는 조성물)의 투여 후 얻어진 결과들 사이의 비교는 α5 조성물이 HL-1 심근 세포에서 상기 마커의 발현을 촉진하는 데 가장 효과적임을 나타냈다.
더욱이, 시간-경과 효과가 관찰되었는데, 지시된 3가지 카르복실산과 함께 표 1에 열거된 아미노산을 포함하는 α5 조성물에 의한 48시간 보충 후, 실제로 증가는 기저 값에 비해 더 한층 뚜렷했다.
24시간 처리(도 2)와 비교할 때 Tfam에 대한 그리고 48시간 처리 후(도 3) PGC-1α 및 Cyt c에 대한 BCAAem 조성물의 값에 비하여 증가는 또한 통계적으로 유의하였다.
KFL15 및 PP2CM
크뤼펠-유사 인자 15(KFL15) 및 미토콘드리아 기질-표적화 단백질 포스파타제 2C 패밀리 구성원(PP2CM)은 분지쇄 아미노산(BCAA)의 이화 작용을 조절하는 단백질이다.
BCAA 이화 작용의 제1 단계는 3가지 BCAA에 공통이며, 미토콘드리아 효소인 BCAA 아미노트랜스퍼라제(BCAT) 및 분지쇄 α-케토산 탈수소효소 복합체(BCKDC)를 필요로 한다.
분해의 완전히 가역적인 제1 단계에서, 미토콘드리아 BCAT는 BCAA로부터의 아미노기를 α-케토글루타레이트로 이동시켜 상응하는 분지쇄 α-케토산(BCKA) 및 글루타메이트를 형성한다.
그 후, BCKDC는 BCKA의 카르복실기의 탈카르복실화를 촉매하여 상응하는 분지쇄 아실-CoA 에스테르를 형성한다.
이 반응은 비가역적이며, 그에 따라 BCAA를 분해시킨다.
BCKDC 활성은, NADH, α-케토이소카프로에이트 및 분지쇄 아실-CoA 에스테르에 의한 최종-생성물의 알로스테릭 저해에 의해 조절된다.
BCKDC 활성은 그의 조절 서브유닛인 E1a의 인산화 상태에 의해 결정된다.
BCAA 수준이 낮을 때, E1a는 BCKD 키나제에 의해 과인산화되어, BCKDC 활성을 저해하고 유리 BCAA를 보존시킨다.
BCAA 수준이 높을 때, E1a는 미토콘드리아 내의 PP2C로 명명된 미토콘드리아-표적화 2C형 Ser/Thr 단백질 포스파타제(PP2CM) 또는 단백질 포스파타제인 Mg2+/Mn2+ 의존성 1K(PPM1K)에 의해 탈인산화되어, BCKDC를 활성화시키고 총 BCAA를 감소시킨다(문헌[Bifari and Nisoli, 2016]).
또한, KLF15는 심장에서 BCAT, BCKDC, 및 PP2CM 유전자 발현을 증가시키는 것으로 밝혀졌다(문헌[Sun et al., 2016]).
PP2CM 및 KFL15의 mRNA 수준의 평가는 α5 조성물이 HL-1 심근 세포에서 기저 값에 비해 PP2CM 및 KLF15 mRNA 수준을 증가시킴을 나타냈다. 증가는 심지어 BCAAem 조성물에 비해 유의하였다(도 4).
이러한 결과는 활성 작용제를 포함하는 조성물로서, 활성 작용제가 류신, 이소류신, 발린, 트레오닌, 라이신, 및 시트르산, 숙신산 및 말산을 함유하는 조성물이 미토콘드리아 기능을 촉진하는 데 매우 효과적이며, 대사적으로 활성인 세포에서 심지어 BCAAem 조성물에 비해 미토콘드리아 생합성을 더욱 효율적으로 활성화시킴을 나타낸다.
산소 소비(OCR)
다양한 조성물이 보충된 HL-1 세포의 산소 소비를 시험하였다. 양성 대조군으로서 디에틸렌트리아민-NO(DETA-NO)가 보충된 세포를 또한 시험하였다. 세포 산소 소비를 증가시키는 것에 대한 DETA-NO의 효과는 밝혀진 바 있다(문헌[Nisoli et al., 2003]). NO는 갈색 지방 세포 및 3T3-L1, U937, 및 HeLa 세포와 같은 다양한 세포에서 미토콘드리아 생합성을 유발하는 것으로 밝혀졌다. 산화 질소의 이 효과는 사이클릭 구아노신 3',5'-모노포스페이트(cGMP)에 의존성이고, 미토콘드리아 생합성의 마스터 조절 인자인 PGC-1α의 유도에 의해 매개되었다(문헌[Nisoli et al., 2003]).
DETA-NO 처리 48시간 후, 예상대로 산소 소비의 증가가 관찰되었다.
가장 주목할 만하게는, HL-1 세포가 α5 조성물로 48 시간 동안 보충되었을 때 산소 소비의 현저한 증가가 관찰되었으며, 이는 미토콘드리아 활성의 증가를 나타낸다(도 5).
증가는 B2 및 BCAAem 조성물의 투여 후에 관찰된 것보다 유의하게 높다.
종합하면, 이러한 결과는 류신, 이소류신, 발린, 트레오닌, 라이신, 시트르산, 숙신산 및 말산의 조합물을 활성 작용제로서 포함하는 조성물이 HL-1 심근 세포에서 미토콘드리아 생합성, 미토콘드리아 기능 및 BCAA 이화 작용을 촉진하는 데 유의하게 더 활성임을 나타낸다.
BCAA, 트레오닌 및 라이신뿐만 아니라 시트르산, 숙신산 및 말산을 포함하는 활성 성분을 제공하는 알파 6(α6) 및 알파 7(α7) 조성물은 유사한 이점을 달성하는 것으로 여겨진다.
주목할 만하게는, 아세틸-CoA에 더하여 혼합물에 풍부한 BCAA의 이화 작용은 숙시닐-CoA를 제공한다. 이 후자는 숙시닐-CoA 합성 효소 반응을 활성화시킬 수 있으며, 이는 결국 숙시네이트 탈수소효소의 후속 반응을 위한 기질로서 숙시네이트를 생성한다.
