Abstract
본 발명은 자가면역 질환의 치료 및/또는 예방을 위한 약품, 유기체에서 조절 T 세포(TReg)의 형성을 위한 약품 및 본 발명에 따른 약품이 사용되는 다양한 방법에 관한 것이다.
Description
본 발명은 자가면역 질환의 치료 및/또는 예방을 위한 약품, 유기체에서의 조절 T 세포(TReg)의 형성을 위한 약품 및 본 발명에 따른 상기 약품들이 사용되는 다양한 방법들에 관한 것이다.
자가면역 질환들은 내생성 조직(endogenous tissue)에 대한 면역계의 과도한 반응이 특징이다. 상기 면역계는 잘못된 정보에 의하여 내생성 조직을 막아야 하는 이물질로서 인식한다. 이것은 심각한 염증 반응을 일으키고, 그것에 의해 공격받은 내장 기관에 손상을 입힌다.
내생성 구조 및 외인성 구조를 구분하는 중요한 부분은 소위 MHC 분자라고 불리는 내생성 세포 표면 분자에만 도킹(dock)하고, 따라서 내생성 조직을 묵인하도록 흉선(thymus)에서 "훈련된" T림프구 또는 T 세포에 의해 수행된다. 이 과정들은 "클론 결손(clonal deletion)" 및 "클론 선택(clonal selection)"이라고 불리운다. 상기 흉선에서 초기 선택이 이루어지는 동안, 상기 결속은 상기 T 세포들을 활성화시킬 수 있을 정도로 강하지 않은 반면에, 상기 내생성 세포막에서 MHC 분자를 인식할 수 있는 오직 이 T 세포들만 살아남는다. 결코 내생성 MHC 분자들과 결속될 수 없거나 인식할 수 없는 T 세포들은 제거된다. 상기 흉선에서 또한 일어나는 상기 클론 결손에서, 이런 식으로 활성화되었을 것이고, 결국 내생성 세포의 파괴를 초래하는 내생성 MHC 분자들을 "틀림없이" 인식하고 강하게 결속할 수 있는 이 T 세포들은 제거된다. 이 과정은 상기 면역계가 "자신"을 보호할 수 있고, "외인성"을 방지할 수 있게 하기 위하여 취하는 조치 중 하나이다.
자가면역 질환에서, 상기 T 세포들의 군은 비정상적으로 행동한다. 여전히 기능하는 외인성 분자에 대한 방어뿐만 아니라, 그것들은 또한 내생성 구조를 공격한다. 내장 기관들 또는 조직들은 외인성으로서 감지된다. 다양한 결과가 있을 수 있다 : 생명유지와 관련된 조직이 공격받는다면 자기면역 질환은 치명적으로 진행될 것이다. 상기 면역계는 이 조직들에 대한 방어를 지휘하고, 세포 및 또한 체액성의 방어 반응은 움직여지고, 자기항체(autoantibody)가 형성되고, 그 결과 충분한 시간이 지나면 상기 공격받은 내장 기관들은 기능이 중단된다. 가장 흔히, 상기 면역계는 약해지고, 신체는 모든 종류의 질환에 민감해진다. 일부 환경 하에서, 상기 외인성에 대한 인식은 또한 방해받고, 그 결과 악화된 암 세포의 전이는 더 이상 효과적으로 막을 수 없고, 환자는 감염성 질환에 더 민감해진다. 상기 질환 중에, 상기 신체의 치료 메커니즘은 상기 손상된 내장 기관 부분을 재생하기 위하여 가능한 한 멀리 떨어지려고 하는 반면, 상기 면역계의 세포들은 상기 외인성 조직을 파괴한다. 일반적으로, 치료하지 않는다면 상기 방어계의 이 잘못된 공격은 평생 또는 상기 타겟 조직이 완전히 파괴될 때까지 계속된다.
집중적인 연구에도 불구하고, 자가면역 질환들의 정확한 원인은 아직 밝혀지지 않고 있다. 일반적으로 인정된 가설은 자가면역 질환들이 유전적인 소인, 예를 들어 외부 감염과 결합하여 어떤 MHC 분자형의 존재 때문에 얻어진다는 가정에 근거한다. 그러한 일반적으로 결정된 요인들은 환자의 신체에 존재하고, 또한 극도의 스트레스, 감염 및 임신과 같은 좋지 않은 환경적 요인들이 일어난다면, 이것은 자가면역 질환들의 시작을 초래할 수 있다.
상기 면역계는 신체에 침입한 감염성 병원체를 방어하기 위해 싸울 수 있는 다양한 세포로 구성된다. 상기 면역 반응의 메커니즘은 전문화된 세포들의 활성화 및 소위 CD8 트랜스멤브레인(transmembrane) 글리코프로틴으로 표현되고 CD8+T 세포로 칭해지는 약간의 T 세포의 세포 독성과 같은 반응기 기능의 취득을 포함한다.
앞서 억제하는 T 세포로서 설명된, 조절 T 세포(TReg)들은 상기 T 세포들의 전문화된 하위 그룹이다. 이 세포들은 상기 면역계의 활성화를 억제하는 기능을 갖고, 그것 때문에 상기 면역계의 자기면역관용(self-tolerance)을 통제한다. 그 결과, 건강한 유기체에서, 그것들은 자가면역 질환들의 발명을 예방한다. 다양한 TReg 집단은 단백질 CD4, CD25 및 Foxp3으로 표현되는 것들을 포함하는 것으로 설명되어 왔고, 따라서 CD4+CD25+Foxp3+ T 세포로 칭한다. 또한 TReg은 CD4 및 Foxp3로 표현되지만, CD25로 표현되지 않는, 소위 CD4+CD25-Foxp3+ T 세포로 설명되어 왔다.
Lan 외((2005), Regulatory T cells : development, function and role in autoimmunity, Autoimmum. Rev. 4(6), PP 351~363)는 CD4+CD25+ 조절 T 세포의 고갈은 자가면역 질환들의 자발적인 발달을 초래한다는 쥐 모델(murine model)을 설명한다.
Chatila T.A.((2005), Role of regulatory T cells in human disease, 116(5), p 949~959)는 상기 단백질 Foxp3에 대해 부호화한 유전자에서 돌연변이 때문에 CD4+CD25+ 조절 T 세포의 선천적인 결핍은 자가면역 질병들의 발생의 원인이 된다고 보고한다.
2005년 3월에 발행된 저널 "Nature Immunology"에 조절 T 세포에 관한 리뷰가 실려있다.
자가면역 질환들은 환부인 내장기관에 따라 치료된다. 원인 요법의 기본 원리는 면역억압제(immunosuppressant), 예를 들어 코르티손(cortisone)의 투여에 의하여 상기 면역계의 활성화를 억제하는 것이다. 시도들은 질환 발생에 관련된 상기 메커니즘에 특히 영향을 주는 신약을 개발하도록 해 왔기 때문에 이 물질들은 다수의 침투성 부작용 및 상호반응이 특징이다. 이것의 예들은 나탈리주맙(natalizumab) 및 인플릭시맙(infliximab)이다. 나탈리주맙은 단일 클론 항체 및 백혈구의 표면에 위치한 접착 분자인 IgG4의 선택적인 억제제이다. 나탈리주맙은 염증성 병소로의 백혈구의 이동을 억제하고, 용균반(plaque) 진행성 다발성 경화증의 특히 공격적인 형태의 지표에 사용된다. 인플릭시맙은 자가면역 염증성 반응에서 중요한 역할을 하는 종양 괴사 요소 α(Tumour necrosis factor; TNFα)에 대한 키메릭(chimeric) 단일클론항체이다. 인플릭시맙은 류마티스 관절염, 크론병(Crohn's disease), 베크테레프병(Bechterew disease) 및 건선(psoriasis)에 사용된다.
Ehrenstein 외(2004), Compromised function of regulatory T cells in rheumatoid arthritis and several by anti-TNFα therapy, J. Exp. Med., Vol. 200, No. 3, p 277~285에, TNFα를 겨냥한 단일클론항체로서, 인플릭시맙은 류마티스 관절염의 치료를 개선시킨다고 보고되었다.
Nadkarni 외(2007), Anti-TNFα therapy induces a distinct regulatory T cell population in patients with rheumatoid arthritis via TGF-β, JEM Vol. 204, p 33~39에 똑같이 제안되어있다.
Bresson 외(2006)는 항-CD3ε 특성 항체 및 프로인슐린(proinsulin)의 복합적인 투여에 의하여 타입 I 당뇨병의 치료를 제안한다.
Vandenbark 외((2008), Therapeutic vaccination with a trivalent T-cell receptor(TCR) peptide vaccine restores deficient FoxP3 expression and TCR recognition in subjects with multiple sclerosis, Immunology Vol. 123, p 66~78)는 특정한 TCR 펩타이드를 가진 환자들의 백신 접종 후에 다발성 경화증에서의 자가반동(autoreactive) 반응의 조절에서의 개선을 설명한다.
비록 이 더 새로운 물질이 매우 분명히 작용한다 해도, 심각한 부작용, 예를 들어 진행성다병소성백질뇌증(progressive multifocal leukoencephalopathy)의 발명이 일어날 수 있다. 이런 이유로 미국에서 첫 등록 후에 오직 3개월만에, 나탈리주맙은 시장에서 회수되었다. 이 새로운 활성 물질들의 비용은 매우 높다. 현재, 나탈리주맙 300mg의 가격은 2,000.00 유로를 넘는다. 인플릭시맙 200mg의 가격은 약 1,700.00 유로이다.
본 발명자들의 발견은 특히 종래기술에서 상기 IL-2 돌연변이 단백질의 그러한 작용의 어떠한 시사점도 발견되지 않았다는 것은 놀라웠다.
따라서 상기 돌연변이 단백질 hIL-2-N88R에 대하여 WO99/60128에서, 이것은 자연살상세포와는 대조적으로 선택적으로 T 세포를 활성화할 수 있고, 폐에서 암세포 전이 형성을 줄일 수 있다는 것을 기재하고 있다.
WO02/00243에서, 돌연변이 단백질 hIL-2-N88R에 대하여 안정하고, 히스티딘을 함유하고, 알부민이 없는 제제가 기재되어 있다.
US2002/0164300에서, 상기 돌연변이 단백질 hIL-2-N88R의 글리코실화된 이형이 설명되어 있다.
자가면역 질환들의 대상이 되는 치료 및/또는 예방을 위하여, 또는 유기체에서 조절 T 세포를 선택적인 활성화를 위하여, 본 발명에 따른 상기 돌연변이 단백질 hIL-2의 용도는 종래 기술에서 설명되어 있거나, 분명히 설명되어 있지 않다.
인간 야생형 IL-2에 대해서도, 해당하는 발견이 없다.
Van der Vliet 외((2007), Effects of the administration of high-dose interleukin-2 on immunoregulatory cell subsets in patients with advanced melanoma and renal cell cancer, Clin. Cancer Res. Vol. 13, p2100~2108)는 종양의 치료를 위하여 많은 투여량의 투여에서 IL-2의 치료상의 효능은 감소한다고 보고한다.
Ahmadzadeh 및 Rosenberg((2006), IL-2 administration increase CD4+CD25hiFoxp3+ regulatory T cells in cancer patients, Blood, Vol. 107, p2409~2414)는 환자의 조절 T 세포를 제거하는 것으로 종양 환자에게서 인간 야생형 IL-2의 치료적 효과를 향상시킨다고 제안한다. 그러나 이 접근법은 유망하지 않는 것으로 밝혀졌다(Powell 외(2007), Inability to mediate prolonged reduction of regulatory T cells after transfer of autologous CD25-depleted PBMC and interleukin-2 after lymphodepleting chemotherapy, J. Immunother. Vol. 30, p438~447 참조).
Antony 및 Restifo((2005), CD4+CD25+ T regulatory cells, immunotherapy of cancer, and interleukin-2, J. Immunother. Vol. 28, p120~128)는 면역 치료제로서 IL-2를 오히려 무가치하다고 판단하고, IL-2의 투여는 자기면역을 유도할 수 있다고 보고한다.
Knoechel 외((2005), Sequential development of interleukin 2-dependent effector and regulatory T cells in response to endogenous systemic antigen, JEM Vol. 202, p 1375-1386)는 같은 견해를 보이고 있고, 자가면역 질환들의 조기 단계 치료를 위하여 IL-2 길항작용, 즉 IL-2 메커니즘의 억제를 제안한다.
이런 이유로 종래 기술에는 본 발명에 따른 해결법을 제시하는 어떠한 증거도 없다.
본 발명의 목적은 가능한 한 종래이기 때문에 상기 단점을 방지하는 자가면역 질환의 치료 및/또는 예방을 위한 새로운 약제 조성물을 제공하는 것이다. 특히, 저항성이 좋고 저독(低毒)한 것이 특징인 약제 조성물을 제공할 것이다.
본 발명의 목적은 유기체에서 조절 T 세포(TReg)의 형성을 위한 약품을 더 제공하는 것이다.
이 문제들은 hIL-2 야생형을 따라 번호가 붙여지고 적어도 하나의 20, 88 또는 126 위치에서 아미노산 치환을 하는 인간 인터류킨 2(hIL-2)의 돌연변이 단백질(mutein) 또는 그것의 절편(section) 또는 단편(fragment)의 제공을 통하여 해결된다.
본 발명은 그러한 hIL-2 돌연변이 단백질 또는 그것들의 단편은 자가면역 질환들의 치료 및 예방에 활용될 수 있는 높은 치료법의 잠재성을 갖는다는 것을 발견해왔다. 예를 들어 상기 hIL-2 돌연변이 단백질은 선택적으로 유기체에서 CD4+CD25+Foxp3+ 및 CD4+CD25-Foxp3+와 같은 조절 T 세포의 형성을 유도하는 다양한 실험적 제조로 입증될 수 있었다.
놀랍게도 본 발명에 따른 상기 hIL-2 돌연변이 단백질은 hIL-2 야생형보다 상기 조절 T 세포들에서의 현저하게 더 높은 활성화를 나타낸다. 이것은 특히 높은 농도에서 현저하다.
본 발명에 따른 상기 hIL-2 돌연변이 단백질에 있어서, 이중사슬 IL-2 수용기(IL-2Rβγ)에서 보다 삼중사슬 IL-2 수용기(IL-2Rαβγ)에서 더 강하게 결합한다는 것이 WO99/60128에 기재되어 있다. 본 발명자들은 처음에 보여줄 수 있었던 것처럼, hIL-2 야생형과 비교되는 본 발명에 따른 상기 hIL-2 돌연변이 단백질은 상기 IL-2 수용기(CD25) (CD4+CD25-Foxp3+)의 상기 α아단위(亞單位)가 부족한 이 조절 T 세포들의 형성을 유도하지만, 놀랍게도 또한 강화한다. 또한 이 부차집단(subpopulation)은 상기 면역계의 활성화의 억제 및 상기 면역계의 자기 내성의 조절에 기여한다. 그 결과 본 발명에 따른 상기 hIL-2 돌연변이 단백질은 상기 hIL-2 야생형보다 자가면역 질환들의 치료에 유효한 물질로서 매우 더 높은 효능 나타낸다.
본 발명자들은 또한 hIL-2 돌연변이 단백질은 자가면역 질환들의 억제에서 결정적인 역할을 하는 CD3+CD4-CD25+Foxp3+ 및 CD3+CD4-CD25-Foxp3+ (데이터는 표시하지 않음)와 같은 CD8 양성 조절 T 세포들의 형성을 유도한다는 것을 입증할 수 있었다.
또한 본 발명에 따른 상기 hIL-2 돌연변이 단백질은 자연살상세포(natural killer cell, NK 세포)와는 대조적으로 선택적으로 T 세포를 활성화시키는 hIL-2 야생형과 비교했을 때 더 많은 장점을 가지고 있고, 그 결과, 감소하는 유독성 프로파일 및 증가하는 치료 지수(therapeutic index)를 나타낸다. 따라서 본 발명에 따른 상기 hIL-2 돌연변이 단백질은 상기 hIL-2 야생형 보다 더 상당히 용인될 수 있다(WO99/60128 참조)
상기 세포독성 CD3+CD8+CD45RO+ T 세포에 근거하여, 상기 hIL-2 야생형과 대조를 이루어 본 발명에 따른 상기 hIL-2 돌연변이 단백질은 "실험되지 않았고, 핵심기억(central memory), 조기 구분되는" 및 "늦게 구분되는" CD8 T 세포로서 또한 설명된 CD8 양성 세포독성 T 세포의 확산에 효과가 없거나 경미한 효과를 갖는다는 것을 처음으로 보일 것이다. 이것은 상기 CD8+-세포독성 T 세포가 자가면역 질환들에서 끊임없이 지속되고 만성적인 염증성 과정의 원인이 되는 한에 있어서 유리하다(Liu 외(2007), Multiole Sclerosis, 13, p149 및 Haegele 외(2007), Neuroimmunol, 183, p168과 비교). 따라서 상기 hIL-2 야생형과 비교했을 때, 본 발명에 따른 상기 hIL-2 돌연변이 단백질은 저항성 장점을 더 나타내는 상기 CD8+ T 세포에 의해 유도되는 이 염증성 반응의 강화를 더 예방한다.
본 발명자들은 또한 보일 수 있었던 것과 같이, 본 발명에 따른 상기 hIL-2 돌연변이 단백질은 또한 상기 면역 세포들의 항원특이 활성화를 활발하게 한다. 이것은 상기 hIL-2 돌연변이 단백질에 의하여 질환-특정 면역 세포들은 선택적으로 활성화되고, 그것으로 인하여 상기 면역 치료의 전신작용은 제한된다는 이점을 갖는다. 그 결과, 상기 hIL-2 돌연변이 단백질의 투여 때문에 다른 질환들의 유도 또한 예방된다.
타입 I 당뇨병의 쥐 모델에 근거하여, 본 발명자들은 자가면적 질환의 발병은 본 발명에 따른 상기 hIL-2 돌연변이 단백질로 치료하는 것에 의하여 예방될 수 있다는 것을 발견할 수 있었다.
따라서 본 발명의 근본적인 문제점은 완전히 해결된다.
실시예에 기초하여 더 자세히 설명할 것이고, 이것은 본 발명은 대표적인 특성이고 본 발명의 범위에만 특별히 한정되지 않는다. 여기서, 참조는 다음에 나타낸 첨부 도면으로 이루어진다.

본 발명에 따르면, 인간 인터류킨 2(hIL-2)의 "야생형(wild type)"은 (20N-말단 아미노산으로 더 구성된 단일 펩타이드가 없는)자연 인간 IL-2에서 나타나는 133 아미노산의 아미노산 염기서열을 갖는 폴리펩타이드 또는 단백질을 의미한다고 이해된다. hIL-2 야생형은 자연형 및 재조합형 둘 다 발현될 수 있다. hIL-2 야생형의 상기 아미노산 염기서열은 Fujita 외, (1983), PNAS USA 80, p7437~7441에 상기 단백질이 세포 사이의 일부분으로서 대장균에서 발현될 때 필연적으로 나타나는 추가적인 N-말단 메티오닌(methionine)이 있고 없는 것이 설명되어 있다. 상기 hIL-2 야생형의 상기 아미노산 염기 배열은 SEQ ID No.1 하의 첨부된 염기서열 프로토콜에 기재되어 있다. hIL-2에 대하여 부호화한 cDNA의 뉴클레오티드 염기서열은 SEQ ID No.2 하의 첨부된 염기서열 프로토콜에 기재되어 있다.
본 발명에 따르면, 인간 인터류킨 2(hIL-2)의 "돌연변이 단백질(mutein)"은 hIL-2 야생형에 비해 특정한 치환이 이루어졌던 폴리펩타이드 또는 단백질을 의미한다고 이해된다. 치환들이 이루어지는 위치의 식별은 예를 들어 SEQ ID No.1으로부터 얻을 수 있는 상기 hIL-2 야생형에서 상기 아미노산의 위치에 근거한다. 따라서 알라닌(A)는 1위치, 프롤린(P)는 2위치, 트레오닌(T)은 133위치 등에 위치한다. 20위치("D20")에 있는 아스파르트산 잔기(residue)(D)는 예를 들어, hIL-2-D20I 및 hIL-2-D20H로 표현되는 IL-2 돌연변이 단백질이 각각 형성되도록, 이소류신 잔기(I) 또는 히스티딘(H)에 의해 대체될 수 있다.
자가면역 질환의 치료 또는 조절 T 세포의 유도를 위하여 특히 적절한 조합 돌연변이를 형성하도록, 본 발명에 따른 상기 hIL-2 돌연변이 단백질은 상기 언급된 20, 88 또는 126위치 각각에 치환될 수 있다.
본 발명에 따르면, hIL-2 돌연변이 단백질은 또한 개질된 폴리펩타이드, 예를 들어 글리코실화된 hIL-2 돌연변이 단백질을 포함한다. 글리코실화된 hIL-2 돌연변이 단백질은 예를 들어 참조로서 포함된 미국 출원 특허 09/310,026 및 10/051,657에 기대되어 있다.
본 발명에 따르면, hIL-2 돌연변이 단백질의 "절편(section)" 또는 "단편(fragment)"은 상기 hIL-2 돌연변이 단백질에 비해서 하나 이상의 아미노산은 상기 N- 및/또는 C-말단에서 누락된 폴리펩타이드를 의미한다고 이해되지만, 그렇다 하더라도 이것은 여전히 자가면역 질환들의 치료 및/또는 예방에 대하여 본 발명에 따라 사용된 상기 hIL-2 돌연변이 단백질의 충분한 생물학적인 활성화를 나타낸다. 이 활성화는 상기 절편 또는 단편이 조절 T 세포의 유도에 대하여 상기 hIL-2 돌연변이 단백질의 활성화의 적어도 50%, 바람직하게 60%, 더 바람직하게는 80%, 가장 바람직하게는 95%를 나타낸다면 충분하다고 여겨진다. 상기 hIL-2 돌연변이 단백질의 활성화는 당업자에게 알려진 방법에 의하여 용이하게 측정될 수 있다. 그러한 방법은 예를 들면 WO99/60128, 예 3 내지 5에 기재되어 있다. 참조에 의하여 이 출원은 현재 명세서에 포함되어 있다.
본 발명에 따르면, 상기 언급된 위치에서의 치환은 하나의 아미노산이 유사한 생화학적 특성을 갖는 다른 것으로 교환되는 보존적 치환(conservative substitutions)이 아니라면 바람직하다.
이 점에서는, 20위치에서의 상기 치환이 아스파르트산(D)이 글루탐산(E)으로 교환되는 것이 아니라면 바람직하다. 위치 88에서의 상기 치환은 상기 아스파라긴(N)이 알라닌(A), 프롤린(P), 글리신(G), 글루타민(Q), 세린(S) 또는 트레오닌(T)와 교환되는 것이 아니라면 바람직하다. 또한 위치 126에서의 상기 치환은 상기 글루타민(Q)이 알라닌(A), 프롤린(P), 글리신(G), 아스파라긴(N), 세린(S) 또는 트레오닌(T)과 교환되는 것이 아닌 것이 바람직하다. 이 치환들은 상기 hIL-2 야생형의 생물학적 활성화를 바꾸지 않거나 매우 적게 바꿀 것이다.
또한 분자간 교차 결합 또는 부적절한 이황화물 가교 결합을 위한 자리에 넣는 치환은 상기 언급한 위치에서 이루어지지 않는 것이 바람직하다. 이런 이유로 본 발명에 따른 hIL-2 돌연변이 단백질의 20위치에서의 상기 치환은 상기 아스파르트산(D)이 아르기닌(R), 아스파라긴(N), 아스파르트산(D), 이스테인(C), 글루탐산(E), 글리신(G), 류신(L), 라이신(K) 페닐알라닌(F), 프롤린(P), 트레오닌(T) 또는 트립토판(W)와 교환되는 것이 아닌 것이 바람직하다. 88위치에서의 상기 치환은 아스파라긴(N)이 아스파르트산(D), 시스테인(C), 글루타민(Q), 트립토판(W) 또는 프롤린(P)로 교환되는 것이 아닌 것이 바람직하다. 126위치에서의 상기 치환은 상기 글루타민(Q)이 알라닌(A), 히스티딘(H), 트리트판(W), 시스테인(C), 글루타민Q), 글루탐산(E) 또는 라이신(K)으로 교환되는 것이 아닌 것이 바람직하다.
본 발명에 따른 상기 hIL-2 돌연변이 단백질은 당업자에 알려진 적절한 방법에 의해 제조될 수 있다. 그러한 방법은 본 발명에 따른 상기 IL 돌연변이 단백질에 대해 부호화되고, 예를 들어 상기 뉴클레오티드 염기서열 SEQ ID No.2 및 적절한 숙주에서 이 염기서열의 표현형을 포함하는 DNA 염기서열의 구성을 포함한다. 이 방법은 재조합 형태로 본 발명에 따른 상기 돌연변이 단백질을 유도한다. 그러나 본 발명에 따른 상기 돌연변이 단백질은 또한 화학 합성 또는 화학 합성 및 재조합 DNA 기술의 조합에 의하여 제조될 수 있다. 본 발명에 따른 상기 돌연변이 단백질의 제조는 현재 명세서에 참조로서 포함되어 있는 WO99/60128 실시예 1 및 2에 상세히 설명되어 있다.
본 발명에 따라 특히 바람직한 88위치에서 상기 아스파라긴(N)은 아르기닌(R)과 교환(hIL-2-N88R)된 hIL-2 돌연변이 단백질은 BAY50-4789라는 이름으로 당업자가 이용가능 하다(Shanafelt 외(2000), A T-cell-selective interleukin 2 mutein exhibits potent antitumor activity and well tolerate in vivo, Nat. Biotechnol. Vol. 18, p1297~1202 참조). hIL-2N88R의 상기 아미노산 염기서열은 SEQ ID No.3에 첨부된 염기서열 프로토콜에 기재되어 있다.
본 발명자들은 hIL-2 돌연변이 단백질의 치료적 효과는 사용된 성분 및 농도에 따라 달라질 수 있다는 것 또한 발견해 왔다. 고농도의 hIL-2는 자가면역 질환들의 치료에 유익할 수 있지만, 종양 치료에 있어서는 반대의 효과를 나타낼 수 있다.
본 발명에 따른 용도에서, 88위치에서의 상기 치환을 통하여 아스파라긴이 아르기닌(hIL-2-N88R) 또는 글리신(hIL-2-N88G), 또는 이소류신(hIL-2-N88I)와 교환되고/교환되거나, 20위치에서의 상기 치환을 통하여 아스파르트산이 히스티딘(hIL-2-D20H) 또는 이소류신(hIL-2-D20I) 또는 티로신(hIL-2-D20Y)과 교환되거나, 126위치에서의 상기 치환을 통하여 글루타민이 류신(hIL-2-Q126L)과 교환된다면 바람직하다.
이 방법은 자연살상세포와는 대조적으로 T 세포를 선택적으로 활성화시키는 것에 성공한 본 발명에 따른 hIL-2 돌연변이 단백질은 높은 치료적 효능을 나타내고, 저독성이 사용된다는 장점을 갖는다. 본 발명에 따른 상기 바람직한 hIL-2 돌연변이 단백질들의 이런 특성들은 현재 명세서에 참조로서 포함되어 있는 WO99/60128에 설명되어 있다.
본 발명에 따르면, 상기 hIL-2 돌연변이 단백질 또는 그것의 단편은 더 치환된 hIL-2 돌연변이 단백질 또는 그것들의 치환된 절편 또는 단편은 상기 hIL-2 야생형과 비교했을 때 더 치환되지 않은 상기 hIL-2 돌연변이 단백질 또는 그것들의 절편 또는 단편의 아미노산 염기 서열과 적어도 80%, 85%, 90%, 95% 또는 99% 동일한 아미노산 염기서열을 갖도록, 20, 88 또는 126위치를 제외한 어느 위치에 적어도 하나의 아미노산 치환을 더 갖는다.
이 방법은 대체 가능한 기본구조들은 적어도 하나의 20, 88 또는 126위치 또는 그렇지 않으면 상기 hIL-2 야생형에 상응하는 것으로부터 떨어진 상기 hIL-2 돌연변이 단백질보다 합성하기 쉬운 일부 경우에 제공된다. 상기 hIL-2 돌연변이 단백질 또는 그것의 절편의 생물학적 활성을 갖는 폴리펩타이드 및 이에 따라 자가면역 질환의 치료 및/또는 예방을 위한 약물을 얻기 위하여, 본 발명에 따른 상기 hIL-2 돌연변이 단백질 또는 그것의 절편의 아미노산 염기서열을 갖는 100% 동일한 아미노산 염기서열을 갖는 폴리펩타이드를 제공할 필요가 전혀 없다. 오히려, 충분히 높은 동일성이 유지된다면 충분하고, 필요하다면 적절한 활성도 손실이 용인되기는 하지만 활성도의 적어도 50%, 60%, 70%, 80%, 90%, 95% 또는 99%는 유지되는 것이 바람직하다. 상기 언급한 동일성은 ≥10 아미노산인 본 발명에 따른 상기 hIL-2 돌연변이 단백질의 절편에 근거한다. 상동관계의 정도는 당업자에게 알려진 방법, 예를 들어 BLAST 분석 또는 DNAStar Inc.사의 Lasergene 프로그램의 MegAlign 모듈을 이용한 방법으로 용이하게 측정될 수 있다.
본 발명에 따르면 20, 88 또는 126위치를 제외한 다른 위치에서의 아미노산 치환은 보존적 아미노산 치환이라면 더 바람직하다.
이 방법은 자가면역 질환들의 치료 및/또는 예방 또는 유기체에서 조절 T 세포의 유도를 위하여 충분히 높은 활성도를 나타내는 본 발명에 따른 상기 hIL-2 돌연변이 단백질의 변형이 제공된다는 이점을 갖는다. 보존적 치환은 상기 돌연변이 단백질의 제2 또는 제3 구조에서 효과가 없거나 최소의 효과만이 있다고 당업자에게 알려져 있다. 그러한 보존적 치환은 Dayhoff("The Atlas of Protein Sequence and Structure, Vol. 5", Natl. 유기체에서 조절 T 세포의 형성을 유도하기 위한 본 발명에 따른 약품은 특히 약제적으로 수용 가능한 캐리어를 포함하는 약제 조성물이다.
이 방법은 상기 약품이 상기 유기체, 특히 사람에게 직접 주입될 수 있는 형태로 이미 제공되는 이점을 갖는다.
약제적으로 수용 가능한 캐리어는 종래의 기술에 알기 쉽게 설명되어 있다(Row 외(2006), Handbook of Pharmaceutical Excipients, 5th Edition, Pharmaceutical Press and American Pharmacists' Association; Bauer 외(1999), Lehrbuch der pharmazeutischen Technologie, Wissenschaftliche Verlagsgesellschaft mbH Stutt-gart 참조). 특히 바람직한 제제는 본 발명의 참조로서 포함된 WO02/00243에 설명되어 있다. 이 제제 알부민이 없고, 상기 hIL-2 돌연변이 단백질 또는 그것의 절편의 안정화는 히스티딘으로 효과를 얻는다. 완료된 약은 다음과 같은 농도에서 다음과 같은 조성을 갖는다 : hIL-2 돌연변이 단백질 또는 그것의 절편 = 0.1~5mh/ml; 히스티딘 = 0.08~1.6wt%; NaCl = 0~0.9wt%; 자당 = 1~10wt%;, 글리신 = 0~0.3wt% 및 pH는 약 5 내지 6.5 범위의 값이다.
일실시예에 따르면 상기 약제 조성물은 또한 면역 억제제를 포함한다.
본 발명에 따른 상기 hIL-2 돌연변이 단백질의 특별한 효능 때문에, 상기 약제적인 조성물은 자가면역 질환들의 치료 및/또는 예방을 위한 단일 조제용물질(monopreparation)로서 이미 사용될 수 있다. 그러한 단일 조제용물질은 오직 활성 물질로서만 본 발명에 따른 상기 hIL-2 돌연변이 단백질을 포함한다. 이와 관련해서 약제적으로 수용가능한 캐리어들, 용매(완충제, 물 등), 첨가제 등은 활성 물질이 아니다.

이 방법은 면역 억제제가 자가면역 질환들에서 명백히 치료 활성을 갖고, 당업자들이 충분히 이용 가능하다는 이점을 갖는다.

본 발명의 목적은 (a) hIL-2 돌연변이 단백질 또는 그것의 단편을 공급하는 단계, (b) 제1 유기체로부터 유도된 말초의 단핵 혈구(PBMCs)와 상기 hIL-2 돌연변이 단백질 또는 그것의 단편이 접촉하는 단계, (c) 조절 T 세포(TReg)를 함유하는 세포군(cell population)을 수득하기 위하여 상기 PBMC로 hIL-2 돌연변이 단백질 또는 그것의 단편을 배양하는 단계 및 (d) 제2 유기체에 상기 세포군을 주입하는 단계를 포함하는 유기체에서의 자가면역 질환의 치료 및/또는 예방을 위한 방법에 관한 것이고, 여기서 상기 hIL-2 돌연변이 단백질 또는 그것의 단편은 본 발명에 따른 hIL-2 돌연변이 단백질 또는 그것의 단편이다.
상기 제1 유기체 및 제2 유기체는 동일한 혈액군을 갖는 것이 바람직하고, 상기 제1 유기체 및 제2 유기체는 동일한 유기체이거나 개체인 것이 더 바람직하다.
세포군의 주입 또는 재주입에서 세포에 대한 원하지 않는 면역 반응이 일어나지 않고, 따라서 상기 방법은 부작용이 특히 적다.
본 발명에 따른 용도와 관련하여 설명된 상기 특성, 이점 및 정의는 유기체에서의 면역 반응의 치료 및/또는 예방을 위한 본 발명에 따른 상기 방법에 똑같이 적용된다.
상기 언급한 특징 및 앞으로 설명할 것들은 언급된 특정 조합에 뿐만 아니라 본 발명의 범위 내에서 벗어나지 않는 한 다른 조합 또는 단독으로 사용될 수 있다는 것은 말할 필요도 없다.
실시예
1. 원료 및 방법
1.1 체외 실험에서 사용하기 위하여 전혈(whole blood)에서 PBMC 의 분리
건강한 실험 대상, 흑색증 환자 또는 다발성 경화증 환자로부터의 말초의 단핵 혈구(PBMCs)는 림프구 분리 배지에 의하여 혈액으로부터 분리된다(Histopaque, Sigma Aldrich). 같은 실험 대상 또는 환자로부터의 두 튜브의 혈액(7 또는 10mL)은 살균한 50mL 튜브에 옮겨지고, RPMI 1640(InVitrogen, # 14190-69)로 30mL까지 만들어진다. 다음으로 상기 희석된 혈액 30mL는 밀도구배 용액의 15mL에 겹쳐 쌓아진다(밀도=1.077; Histopaque, Sigma Aldrich, # 10771).
without braking 20℃에서 40분 동안 400g로 원심분리한 후에, 두 "백혈구 고리"가 얻어지고, 살균한 50mL 튜브에 옮겨진 후, 인산염으로 완충된 살린(Phosphate Buffered Saline, PBS; InVitrogen, # 14190-169)로 두 번 세정된다. 적혈구로 오염이 될 경우, RBC(적혈구, "Red Blood Cells") 용해가 수행된다. RBC 용해액 2mL는 상기 세포 알갱이에 첨가되고, 배양이 상온에서 서서히 혼합되면서 2분간 수행된 후, 많은 양의 완전 배지로 세척된다(10% 송아지 태아 혈청으로 RPMI 1640).
살아있는 백혈구의 수는 트리판 블루(trypan blue)(InVitrogen, # 15250-061) 및 해모시토미터(haemocytometer)(FicherBioblock A2759B)를 이용하여 착색 배제에 의하여 측정된다.
1.2 CFSE 라벨링
숫자를 센 후에 상기 세포는 PBS에 두 번 세척되고, 1x106 세포/ml의 농도로 PBS에 재분산된다. CFSE(InVitrogen # C1157)는 0.5μM의 최종 농도로 첨가된다. 37℃, 암실에서 10분간 배양한 후에, 상기 CFSE 라벨링한 세포들은 4℃에서 신선한 완전 배지로 세 번 세척되고, 도말(plating out)하기 위하여 1x106 세포/ml의 농도로 완전 배지에 재분산된다.
1.3 체외에서의 상기 PBMC 의 자극
상기 PBMC는 자극되지 않은 채 남아 있거나, hIL-2 야생형(프로류킨) 또는 최종 농도 2.5μM(흑색종 펩타이드) 또는 30μg/mL(다발성 경화증 펩타이드)에 각각 펩타이드가 첨가된 흑색종-특정 단백질 gp100, TRP-2, MART-1 및 티로시나제로부터 유도되거나 다발성경화증에 대한 단백질 MOG 특정으로부터 유도된 합성 펩타이드 풀(pool)이 있거나 없는 hil-2-N88R(bay 50-4798; Lot #PR312C008)로 자극된다.
자극제 및 펩타이드는 다음 23조건 하에 첨가된다.

다음으로 상기 세포들은 5% CO2가 포함된 대기에 37℃에서 6일 동안 배양되었다.
1.4 FC500 플로우 세포계산에서 증식 측정 및 표현형
세포 표면 분자에 형광 라벨링된 항체로 상기 세포의 착색은 림프구 특정 하위 군의 증식에 대한 연구를 가능하게 한다(기억 및 활성 표시자, 표 2 참조). 형광 색소로 라벨링된(PE: 피코에리트린(phycoerythrin), ECD: PE-텍사스 적색, APC: 알로피알코시아닌(allophycocyanin), PC7: PE-Cy7) 항체들로 이루어진 상기 면역염색은 CFSE(카르복시플루오레세인 디아세테이트 숙신이미딜 에스테르(carboxyfluorescein diacetate succinimidyl ester))로 라벨링되지 않은 세포로 수행된다. 상기 다른 착색은 CFSE로 라벨링된 세포에서 수행된다.

CD24, Foxp3-APC 및 쥐 IgG2a-APC는 ebioscience, CD45RA-APC 및 CD45RO-APC는 BD Bioscience 제품이다. 다른 모든 항체는 Beckman-Coulter(France) 제품이다.
1.5 타입 I 쥐 당뇨병 모델
생후 12주 NOD("비(非)비만형 당뇨병", Non-Obese diabetes) 쥐들은 hIL-2 돌연변이 단백질 또는 hIL-2 야생성으로 매일 치료된다. 음성 제어 동물들은 생리식염수(살린)로 비슷하게 치료되었다. 상기 치료군은 3~5 마리의 동물로 구성되었다. 0번째 날부터 15번째 날에, hIL-2 돌연변이 단백질 또는 hIL-2 야생형의 5K 또는 25K-단위의 양은 쥐들에게 투여되었다. 17번째 날부터, 5K 단위의 치료군에서 이것은 100K 단위(=6.112μg)으로 증가되었다. 25K 단위로 치료한 동물들은 바꾸지 않고 유지되었다. 31번째 날에 마지막 투여가 이루어졌다. 유사한 실험에서, 치료는 25K 단위의 고정된 투여량으로 0번째 날부터 31번째 날까지 수행되었다. 당뇨병은 소변에서 글루코오스 수준의 모니터링에 의하여 검출되었다. 혈액 시료는 17번째 날 및 31번째 날에 상기 쥐들로부터 취해졌다. 상기 시료들은 항CD4, 항CD25 및 항FoxP3 착색 및 상기 CD4+ T 세포 중에 FoxP3+의 비율을 이용한 FACS에서 분석되었고, 따라서 CD4+FoxP3+ 세포에서 CD25의 발현의 중간 형광 강도(Mean Fluorescence Intensity; MFI)가 결정되었다.
2. 결과
2.1 hIL -2- N88R 에 의한 조절 T 세포의 유도
본 발명에 따른 상기 돌연변이 단백질의 효과를 시험하기 위한 적절한 체외 실험 시스템으로서, 말초의 단핵 혈구(PBMCs)가 사용되었다. PBMC는 T 세포(~75% CD4- 및 CD8-양성) 및 B 및 NK세포(~25% 양성)으로 구성되고, 상기 면역계를 잘 대표하는 세포군으로 여겨진다.
건강한 여섯 실험 대상으로부터의 PCMS(106 세포/mL)는 야생형 IL-2(프로류킨) 또는 IL-2-N88R[BAY 50-4798. Lot #PR312C008("BAY#C008")]로 자극되거나, 10-11 및 10-6 M사이의 농도에서 또는 5μg/mL의 농도에서 비특정 미토겐 식물성적혈구응집소(phytohemagglutinin, "PHA") 또는 배양 배지만("Med")으로의 양성제어에서 자극된다. 상기 자극 후 0번째 날 및 6번째 날에, 상기 CD3+ 림프구에서의 상기 조절 CD4+CD25+Foxp3+ T 세포의 함량은 측정되었다. 상기 결과는 도 1 및 표 3에 표시하였다.

이 실험으로부터 10-7 M 및 10-6 M의 농도에서 hIL-2-N88R이 상기 조절 T 세포 CD4+CD25+Foxp3+의 부차집단을 뚜렷하게 유도한다는 결과가 뒤따른다. 상기 유도는 hIL-2 야생형에 의한 상기 PBMC의 자극보다 뚜렷하게 더 크다.
두 번째 제조에서, hIL-2 야생형과는 다르게 hIL-2-N88R로 자극한 후 상기 조절 T 세포 CD4+CD25+Foxp3+의 부차집단에서의 상기 증가가 연구되었다. 상기 결과는 도 2 및 표 4에 표시하였다.

여기서도 마찬가지로, hIL-2-N88R로 인한 자극은 상기 조절 T 세포 CD4+CD25+Foxp3+의 부차집단에서 뚜렷한 증가를 유도하고, 이것은 10-6M의 농도에서 hIL-2 야생형으로 자극한 것보다 뚜렷하게 크다.
2.2 hIL -2- N88R 은 흑색종 환자에서 조절 T 세포를 유도한다
다음으로, 본 발명에 따른 상기 hIL-2 돌연변이 단백질 N88R은 또한 면역 세포의 항체-특정 활성을 자극하였는지에 대해 조사되었다. 세 흑색종 환자로부터의 PBMC(106 세포/mL)는 5μg/mL PHA 또는 배양 배지만이 있는 흑색종이 결합된 펩타이드 풀의 존재 또는 부재하에서, 10-11 및 10-6M의 농도에서 hIL-2-N88R(BAY 50-4798, Lot #PR312C008) 또는 hIL-2 야생형(프로류킨)으로 자극 되었다. 다음으로 상기 조절 T 세포 CD4+CD25+Foxp3+ 및 CD4+CD25-Foxp3+의 부차집단들은 각각 측정되었다. 상기 결과는 각각 도 3 및 표 5, 도 4 및 도 6에 표시하였다.

hIL-2-88R의 투여는 흑색종 환자에게서 상기 조절 T 세포의 뚜렷한 증가를 유도한다는 것이 발견되었다. 10-7 M 및 10-6 M의 농도에서 CD4+CD25+Foxp3+ 부차집단의 경우 및 10-6 M의 농도에서 CD4+CD25-Foxp3+ 부차집단에서, 이것은 야생형 IL-2(프로류킨)의 상응하는 농도로 자극된 것 보다 뚜렷하게 더 크다.
2.3 hIL -2- N88R 은 다발성 경화증 환자에서 조절 T 세포를 유도한다

hIL-2-88R의 투여는 다발성 경화증 환자에게서 상기 T 세포의 뚜렷한 증가를 유도한다는 것이 발견되었다. 10-8 M 및 10-7 M의 농도에서 CD4+CD25+Foxp3+ 부차집단의 경우 및 10-6 M의 농도에서 CD4+CD25-Foxp3+ 부차집단에서, 이것은 hIL-2 야생형(프로류킨)의 상응하는 농도로 자극된 것 보다 뚜렷하게 더 크다.
2.4 hIL -2- N88R 는 다발성 경화증 환자 및 건강한 실험 대상에서 세포독성 CD8+ T 세포의 최소한의 증식을 유도한다
세포독성 CD8+ 중앙 기억 T 세포의 자극이 연구되었다. 건강한 실험 대상 또는 다발성 경화증 환자로부터 채취한 PBMC는 2.3에 설명된 것과 같이 처리되었다. CFSElow/CD3+CD8+CD45RO+ T 세포의 비율이 분석되었다. 상기 결과는 도 7 및 표 9, 도 8 및 표 10에 표시하였다.

hIL-2 야생형과는 달리, 다발성 경화증 환자 및 건강한 실험 대상에서 hIL-2-N88R은 중앙 기억 CD8+ T 세포의 매우 적은 증식만을 유발했고, 이것은 매 농도마다 연구되었다.
2.5 hIL-2 돌연변이 단백질로 치료하는 것은 동물 모델에서 타입 I 당뇨병의 발생을 예방한다
hIL-2 야생형에 비하여 hIL-2-N88R로 NOD 쥐들의 치료하는 것은 CD4+ 세포 내에서 Foxp3+ 세포의 더 높은 비율 증가를 유도한다(도 9(A)). 또한 이 CD4+FoxP3+ 양성 세포는 더 높은 CD25의 발현을 나타낸다(도 9(B)). 도 10은 상기 hIL-2 야생형과는 달리, 상기 쥐 타입 I 당뇨병 모델에서의 hIL-2-N88R치료는 상기 치료군에서 모든 쥐들에게서 상기 당뇨병의 발생을 예방했다.
3. 결론
본 발명자들에 의해 수행된 상기 실험들은 조절 T 세포(TReg)의 유도에 대하여 그것들의 잠재 효능 때문에 본 발명에 따른 상기 hIL-2 돌연변이 단백질 및 그것들의 단편은 자가면역 질환의 치료 및/또는 예방 또는 유기체에서의 TReg의 유도 및 체외에서의 TReg의 형성에 대하여 적절한 물질이라는 것을 분명히 보여준다. 이것은 체외 실험뿐만 아니라 체내 실험에서도 본 발명자들에 의해 입증된다.
