Description

구조적 손상, 골막자극, 및 신경 포착으로부터 유발된 뼈통증은 양성 및 전이성 골질환중 가장 보편적인 합병증이고, 병원 및 사회 관행적으로 심각한 문제로 제시되고 있다(Coleman, 1997, Cancer 80 ; 15881594).

약제학적으로 허용가능한 염중 매우 바람직한 그룹은 각 포스포산 그룹중 하나의 산성 수소 및 하나의 약제학적으로 허용가능한 양이온, 특히 나트륨을 갖는 것을 특징으로 한다.

비스포스포네이트 (이하 본 발명의 약제로서 언급함)는 전형적으로 에난티오머 또는 디아스테레오머 또는 기하 이성체, 전형적으로 시스-트랜스 이성체와 같은 광학 이성체 같은 이성체 또는 적절하게 이성체의 혼합물 형태로 사용될 수 있다. 광학 이성체는 순수한 안티포드 및/또는 라세미체 형태로 수득된다.
본 발명의 약제는 또한 그의 수화물 형태로 사용될 수 있거나 그의 결정화를 위해 사용되는 그의 용매를 포함할 수 있다.
본 발명의 약제(비스포스포네이트)는 바람직하게 임의로 투여에 적절한 무기 또는 유기, 고형 또는 액상의 약제학적으로 허용가능한 담체와 함께 또는 혼합되어 치료학적 유효량의 활성 성분을 포함하는 약제학적 조성물의 형태로 사용된다.
약제학적 조성물은 예를 들면, 소화관, 예로서 경구, 직장, 분무 흡입 또는 비강 투여용 조성물, 비경구, 예로서 정맥내 또는 피하 투여용 조성물, 또는 경피 투여용(예: 수동 또는 이온주입식) 조성물일 수 있다.
바람직하게, 약제학적 조성물은 경구 또는 비경구(특히 정맥내, 동맥내 또는 경피) 투여로 조정된다. 정맥내 및 경구, 가장 바람직하게 정맥 투여가 가장 중요 한 것으로 판단된다. 바람직하게 비스포스포테이트 활성 성분은 비경구적 형태, 가장 바람직하게 정맥내 형태로 존재한다.
투여 및 복용의 특정 모드는 환자의 특성, 특히 적절하게 연력, 체중, 생활 방식, 활동 수준, 호르몬 상태(예: 폐경기후) 및 뼈광물 밀도를 고려한 의삭의 주의에 따라 선택될 수 있다. 그러나 가장 바람직하게, 비스포스포네이트는 정맥내로 투여된다.
본 발명의 약제의 용량은 다양한 인자, 예로서 활성 성분의 유효성 및 작용 기간, 투여 모드, 온혈동물의 종류, 및/또는 온혈동물의 성별, 연력, 체중 및 각 개체의 상태따라 달라질 수 있다.
일반적으로 대략 75kg의 온혈동물에 0.002-20.0 mg/kg, 특히 0.01-10.0 mg/kg의 단일 투여량으로 비스포스포네이트 활성 성분이 투여된다. 바람직하게, 이 투여은 수회, 임의로 동일, 부분 투여로 취할 수 있다.

단일 투여 유니트 형태의 제제는 바람직하게 약 1 % 내지 약 90%의 활성 성분을 포함하고, 비단일 투여 유니트 형태의 제제는 바람직하게 약 0.1% 내지 약 20%의 활성 성분을 포함한다. 캡슐제, 정제 또는 당의정과 같은 단일 투여 유니트 제제는 약 1mg 내지 약 500mg의 활성 성분을 포함한다.
소화관 및 비경구 투여용 약제학적 제제는 예를 들면, 당의정, 정제 또는 캡슐제 및 또는 앰플과 같은 투여 유니트 형태의 것이다. 이는 그 자체로 공지된 방법, 예로서 통상의 믹싱, 과립화, 조제, 용해 또는 동결 공정에 의해 제조된다.

다른 경구 투여가능한 약제학적 제제는 젤라틴으로 제조된 건성-충진된 캡슐제, 및 글리세롤 또는 소르비톨과 같은 젤라틴 및 가소화제로 제조된 연성, 실링된 캡슐제이다. 건성-충진된 캡슐제는 예를 들면 락토스와 같은 충진제, 전분과 같은 결합제, 및/또는 활석 또는 마그네슘 스테아레이트와 같은 활택제 및 적절하게는 안정화제와 혼합하여 과립 형태의 활성 성분을 포함할 수 있다. 연성 캡슐제에서 활성 성분을 지방산 오일, 파라핀 오일 또는 액상을 폴리에틸렌 글리콜중에 용해시키거나 현탁시키고, 또한 안정화제를 첨가할 수 있다.
비경구용 제제는 특히 예를 들면, 동맥내, 근육내, 복강내, 비강내, 진피내, 피하 또는 바람직하게 정맥내와 같은 다양한 방식에 유효한 주사용 액제이다. 상기 액제는 바람직하게 예를 들면, 활성 성분만 또는 약제학적으로 허용가능한 담체와 함께 포함하는 동결건조된 제제로부터 사용전 제조될 수 있는 등장성 수용액이다. 약제학적 제제는 멸균될 수 있고/거나 보조제, 예를 들면, 보존제, 안정화제, 습윤제 및/또는 유제, 가용화제, 삼투압 조절을 위한 염 및/또는 완충제를 포함할 수 있다.

제조법:
상기 성분을 롤러 기어 베트상에서 롤링하여 150 g의 특정의 비등점 백금 분획 100-125중에 함께 용해시켰다. 300mm 의사용 블레이드를 사용하여 스프레딩 장치에 의해 상기 용액을 폴리에스테르 필름(Hostaphan, Kalle)에 적용시켜 약 75 g/m2 코팅시켰다. 건조시킨 후(15분 60℃), 실리콘-처리된 폴리에스테르 필름 (두께 75 mm, Laufenberg)를 필-오프 필름으로서 적용시켰다. 완성된 시스템을 펀치 도구를 사용하여 5 내지 30cm2의 원하는 형태의 크기로 펀치하였다. 완성된 시스템 각각을 작은 봉지(sachet)의 알루미늄 처리된 종이로 밀봉하였다.
실시예 3: 1.0 mg의 건성, 동결건조된 1-하이드록시-2-(이미다졸-1-일) 에탄-1,1-디포스폰산 (그의 혼합된 나트륨 염)을 포함하는 바이알. 1 ml의 물로 희석시킨 후, i. v. 주입용 액제(농도 1 mg/ml)를 수득하였다.

실시예 5 염증 및 신경병적 통증을 갖는 래트 모델에서 비스포스포네이트의 효능
방법
염증성
통각과민
염증성 통증을 갖는 래트 모델에서 기계적 통각과민을 조사하였다. 압력 자극 증가에 대한 발 회피 역치(Paw withdrawal thresholds)는 시험된 없는 동물(naive animal)에서 프로인트 완전 어쥬반트(Freund's complete adjuvant)(FCA)를 좌측 뒷다리에 발바닥내로 주입하기 앞서 진통측정기((Ugo Basile, Milan)를 사 용하여 Randal-Sellito 기술에 의해 측정하였다. 24시간 후, 약물 또는 담체 투여전 및 10분 내지 6시간 후 발 회피 역치를 다시 측정하였다. 같은쪽 발에서의 통각과민 역전율을 하기 식에 따라 계산하였다:
신경병적
통각과민
좌측 좌골신경의 부분 결찰에 의해 유도된 신경병적 통증을 갖는 래트 모델에서 기계적 통각과민을 조사하였다. 수술 후 대략 14일 후 결찰시킨(같은쪽) 결찰시키지 않은(반대쪽) 발의 회피 역치를 약물 또는 담체 투여전 및 10분 내지 6시간 후 발 회피 역치를 다시 측정하였다. 각 시점에서 통각과민의 역전율을 하기 식에 따라 측정하였다:
모든 시험은 6마릴의 동물 그룹으로 수행하였다. 저장 농도의 약물을 증류수에 용해시키고 피부 투여하기 위하여 0.9% 염수에서 4mL/kg의 용량으로 희석액을 제조하였다. 모든 약물을 플라스틱 바이알로 제조하고 암실에서 보존하였다.
반복된 측량값으로 ANOVA 이어서 투키(Tukey) HSD 테스트를 사용하여 회피 역치 해석법을 수행하여 통계학적으로 분석하였다. 효능은 사용된 투여량에서 관찰된 통각과민의 최대 역전율을 언급한다.
결과
1. 프로인트 완전 어쥬반트를 한쪽 뒷발에 주입하여 유도된 염증성 통각과민 모델에서 졸레드로네이트 (0.003-0.1 mg/kg s. c.)는 투여량에 의존적인 기계적 통각과민의 역전율을 나타내었다. 효능은 30분이내 100%의 최대 역전율로 신속하게 개시되었고 투여 3시간후에는 유의적인 작용이 없이 지속기간은 짧았다. 일부 반대측에서의 작용은 최대 투여량에서 관찰되었다.
2. 파미드로네이트 (0.03-1 mg/kg s. c.) 및 클로드로네이트 (0.3-10 mg/kg s. c.) 모두 염증성 기계적 통각과민을 역전시키는데 효능이 없었지만, 시험된 최대 투여량에서는 발 회피 역치를 미세하게 감소시켰다.
3. 동측 좌골신경을 일부 결찰시켜 유도된 만성 신경병적 통증 모델에서 졸레드로네이트 (0.003-0.1 mg/kg s. c.)는 투여 30분이내 최대인 40% 중간값의 역전율의 기계적 통각과민을 나타내었다. 그러나, 최대 투여량에서는 반대측 발 회피 역치가 유의적으로 감소되었다.
4. 파미드로네이트 (0.03-1 mg/kg s. c.)는 20%의 통각과민 역전율을 나타내면서 신경병적 통증 모델에서 단지 미세하게 활성인 반면, 클로드로네이트 (0.3-10 mg/kg s. c.)는 불활성이었다. 두개의 약물은 다시 반대측 발 회피 역치를 일부 감소시켰다.
5. 이들 데이타는 졸레드로네이트가 만성 염증성 및 신경병적 통증을 갖는 래트 모델에서 기계적 통각과민을 역전시킨다.

효능
성숙한 암컷 래트에 MRMZ-1 래트 젖샘 암종 세포 (3㎕, 107 세포/ml)를 경골내 주입하였다. 세포 주입후 12-14일째를 시작으로 이 동물에서 기계적 통각과민, 기계적 무해자극통증(무해 작극에 대항 피부 감수성) 및 뒷다리 보존이 발생되었다. 세포 주입한 날로부터 1주 3회 투여된 졸레드론산 (ZOL) (10 및 30μg/kg s. c.)이 뒷다리 보존 및 기계적 무해자극통증을 현저하게 저해하였다. 19일째까지 최대 뒷다리 보존을 나타낸 비히클-처리 대조군과 비교하여, 더 많은 투여량의 ZOL을 투여받은 래트에서는 경골내 세포 주입후 19일째동안 뒷다리 보존 징후를 발생시키지 않았다. 그러나, 19일째 단일 주입으로 투여된 경우(100μg/kg, s. c.) ZOL 의 급성 효능은 없었다. 반대로, 모르핀을 사용한 급성 치료(1-lOmg/kg, s. c.)는 투여량에 의존적으로 기계적 무해자극통증을 감소시켰고, 또한 뒷다리 보존은 최대 투여량에서만 현저하게 감소시켰다.

- 유효량의 졸레드론산, 또는 그의 약제학적으로 허용가능한 염, 또는 그의 수화물을 포함하며, 6개월마다 1회씩 투여되는, 골감소증, 파제트병, 골다공증, 류마티스 관절염, 골관절염 및 종양 형성과 관련되는 통증 치료를 위한 약제.
