Abstract
본 발명은 (1) 열 쇼크 단백질 또는 이의 면역자극성 단편과, (2) 인간 파필로마 바이러스 단백질 또는 이의 항원성 단편을 포함하는 조성물을 개체에 투여함으로써 개체에서 사마귀를 치료하는 방법에 관한 것이다. 본 발명은 또한 (1) 열 쇼크 단백질 또는 이의 항원성 단편과, (2) 인간 파필로마 바이러스 단백질 또는 이의 항원성 단편을 함유하는 조성물을 개체에 투여함으로써 제1 유형의 인간 파필로마 바이러스로 감염되었거나 또는 감염된 것으로 의심되는 개체에서 인간 파필로마 바이러스를 치료하는 방법으로서, 여기서 제1 유형 및 제2 유형은 상이한 것인 방법에 관한 것이다.
Description
인간 파필로마 바이러스(HPV)에 의한 감염은 일반적인 것이다. HPV는 성적으로 이전될 수 있는 것으로서, 성생활을 하고 있는 성인들의 대략 20∼80%가 감염되는 것으로 파악된다. 대다수의 감염은 무증상이지만, 일단 감염되면 생식기에 사마귀(성인들중 약 1∼5%에 퍼져있음)와 항문성 기관에 암을 발생시킨다. 암의 다른 유형인, 자궁 경부암은 HPV와 매우 밀접하게 관련되어 있다[Frazer, Genitourin, Med. 72:398-403, 1996]. HPV 제6형, 제11형, 제16형, 제18형, 제31형 및 제33형은 종종 발암 위험을 증가시키는 것과 관련이 있는데, 제16형 및/또는 제18형은 90% 이상의 자궁 경부암에서 관찰된다[van Driel외 다수, Ann. 면역계를 표적화하여 전술한 바와 같은 사마귀 또는 질병을 치료 또는 예방하는데에 안전하고 효과적인 방법은 없다. 이러한 치료법을 개발하려는 노력들은 몇가지 이유에 의하여 지장을 받았는데, 이러한 이유들중 하나는 단일 HPV형으로부터 생성된 항원들은 한정적이고, 형 특이적인 면역 반응을 유도한다는 정설이다. 결과적으로, 몇개의 상이한 HPV형으로부터 생성된 항원들을 함유하는 혼합액(cocktail)은 광범위하게 효과적인 HPV 치료법에 있어서 필수적이라는 사실을 제시하고 있다[Caine 외 다수, Science 288:1753, 2000].
발명의 개요
본 발명은 부분적으로는 하나의 HPV형으로부터 생성된 단백질을 함유하는 융합 단백질은 다른 HPV형으로 인한 감염에 의하여 유발된 질병 또는 증상을 치료하는데에 사용될 수 있다는 발견에 기초하는 것이다. 예를 들어, 세균 열 쇼크 단백질(hsp)에 융합된 HPV 제16형 항원은 제16형 이외의 HPV형(예, HPV 제6형 및 제11형)에 의하여 유발된 인간의 항문성기 사마귀를 치료하는데에 효과적이었다. 이러한 결과는 다음의 두가지 사실들 즉,

따라서, 본 발명은 개체에 (1) hsp 또는 이의 면역자극성 단편 및 (2) HPV 단백질(예, 항원성 단백질 예컨대, HPV 제16형의 E7 단백질) 또는 이의 항원성 단편을 함유하는 조성물을 투여함으로써 개체중 사마귀를 치료하는 방법에 관한 것이다. 본원에서는 이들 성분을 각각 "성분(1)" 및 "성분(2)"라 칭한다. 융합 단백질의 제조

HPV 단백질 항원
임의의 HPV 항원은 본 발명의 조성물(예, 본원에 기술된 혼합물, 접합물 및 융합 단백질)에 사용하기에 적합하다. 그러나, HPV 감염 세포의 표면상에 존재하는 인지가능한 에피토프를 발현하는 HPV 항원이 특히 유용하다. HPV는 6개 또는 7개의 비구조 단백질 및 2개의 구조 단백질을 발현하며, 이들 각각은 본원에 기술된 면역 예방학적 또는 면역 치료학적 연구법에서 표적으로 사용될 수 있다.

Hsp
다수의 hsp가 다양한 유기체들로부터 분리, 클로닝 및 특성규명되었다[Mizzen, Biotherapy 10:173-189, 1998 ; 본원에 사용된 "열 쇼크 단백질(들)" 또는 이의 약어(hsp(s))는 "스트레스 단백질"로서 칭하여지는 단백질과 동의어이거나 또는 이를 포함하는 용어이다]. 면역자극성 hsp, 또는 이의 면역자극성 단편들(예, 융합 폴리펩티드의 일부)은 본원에 기술된 조성물에 사용하는데에 적합하다. Hsp70, hsp60, hsp20∼30 및 hsp10은 미코박테리움 투베르큘로시스(Mycobacterium tuberculosis)및 미코박테리움 레프라에(Mycobacteriumleprae)에 의한 감염에 대한 숙주 면역 반응에 의하여 인지되는 주요 결정기이다. 뿐만아니라, 바실레 칼메트 구에린(Bacille Calmette Guerin ; BCG), 미코박테리움 보비스(Mycobacterium bovis) 균주의 hsp65는 이하 실시예에 기술된 바와 같이 효과적인 면역자극제인 것으로 밝혀졌다.

GrpE는 스트레스로 손상받은 단백질의 회복 및 손상받은 단백질의 분해에 관여하는 약 20 kDa의 이.콜라이 단백질이다. GrpE는 이.콜라이에서 스트레스 유전자 발현을 조절하는 역할을 한다.
Hsp10의 예로서는 GroES 및 Cpn10을 포함한다. Hsp10은 이.콜라이 및 진핵 세포의 미토콘드리아 및 엽록체에서 발견된다. Hsp10은 Hsp60 올리고머와 결합하는 7원 환을 형성한다. Hsp10은 또한 단백질 폴딩에도 관여한다.
유비퀴틴은 ATP 의존성 시토졸 프로테아제에 의하여 단백질을 단백 분해적으로 제거하며 단백질과 결합하는 것으로 공지되었다.

단백질 발현 및 정제

교차 반응성 HPV 서열

조성물의 투여

추가의 연구 없이, 당업자는 본원에 개시된 사실 및 이하의 실시예를 기본으로 하여, 본 발명을 최대한 이용할 수 있을 것으로 생각된다. 다음의 실시예는 당업자가 융합 폴리펩티드를 분리 및 이용할 수 있는 방법에 대하여 단지 예시하는 것으로 추측되는 것으로서, 어떠한 방식으로도 본원에 개시된 사항중 나머지 부분을 제한하고자 하는 것은 아니다.
HPV 제16형의 E7 단백질과 커플링된 엠.보비스 BCG Hsp65 함유의 융합 폴리펩티드를 재조합하여 생성하고 이를 WO 99/07860에 기술된 바와 같이 제제화하였다. Hsp65는 스트레스 단백질인 Hsp60 과의 일원이다. 항문 고도 비늘형 상피내 병변(HSIL)의 치료에 있어서, 상기 융합 폴리펩티드의 효능을 테스트하기 위한 인간의 의학적 수술중에 다음의 사실들이 관찰되었다.
항문 HSIL의 치료에 있어서 HspE7에 대한 랜덤화되고, 이중 맹검형이며, 위약 조절된 다중심 실험에 22명의 환자들을 참여시켰다. 바람직한 환자들은 생검 확인된 항문 HSIL을 보유하였는데, 이들은 인간 면역결핍 바이러스(HIV)에 대하여 음성적이었다. 환자들의 항문으로부터 면봉을 사용하여 얻은 세포들을 사용하여 환자들을 HPV 유형에 따라 분류하였는데, 이때 HPV-16을 보유하고 있을 것을 필요로하지는 않았다. 각각의 병변들은 HPV로 분류되지 않았다. 환자들에게 1달 간격으로 100 ㎍ HspE7 또는 위약을 피하 주사하였다. 이들을 항문 면봉 추출물로 도말 표본화하고, 생검을 이용하는 고해상도 후각 감지법(HRA) 및 전체 내과적 평가 방법에 의하여 치료 반응에 대해서 평가하였다. 대조군 실험이 행하여진후 12주 또는 24주 경과시 무반응 개체(즉, 만성 항문 HSIL 환자)들을, 1달 간격으로 HspE7 500 ㎍을 개체에 3회 주사하는 개방 표지 실험(open-label trial) 대상 개체들과 교차시켰다. 치료 대상은 위약 조절 실험에 있어서 이중 맹검되었으며, 이때 맹검 증상은 계속 유지시켰다.
환자들을 감염시킨 HPV 유형(들)을 평가하기 위하여, 생검하기 직전에 랜덤화된 위약 조절 실험을 스크리닝할때 Darcon 면봉을 사용하여 환자의 항문으로부터 표본을 수집하였다. 샘플 운반 매체(Digene)로 운반한후에, DNA를 분리하여 HPV 유형을 평가하는데에 사용하였다. 간단히 말해서, 교감 프라이머 세트인 MY09/MY11을 사용하여 중합효소 연쇄 반응(PCR)에 의하여 HPV DNA를 증폭시켰다. 증폭 단계 이후에, 샘플들을 나일론 막에 블롯팅시키고 29개의 상이한 HPV 유형(6, 11, 16, 18, 26, 31, 32, 33, 35, 39, 40, 45, 51, 52, 53, 54, 55, 56, 58, 59, 61, 66, 68, 69, 70, 73, AE2, Pap155 및 Pap291)에 특이적인 바이오틴-라벨링된 올리고뉴클레오티드와, 10개의 HPV 유형(2, 13, 34, 42, 57, 62, 64, 67, 72 및 W13B)에 대한 프라이머를 함유하는 개체군 프로브로 프로빙시켰다. "점 플롯"을 형성시킨 샘플들을 5점의 눈금을 이용하여 표준화된 대조군과 비교함으로써 HPV 유형에 대하여 양또는 음으로 스코어를 메겼는데, 스코어 1 또는 그 이상은 양성이었다.
PCR이 성공적임을 확인하기 위하여, 각각의 샘플에 대해 베타 글로빈 대조군 증폭 및 프로브 검출을 수행하였다. 샘플이 베타 글로빈의 존재에 대하여 양성적이지 않은 경우, PCR 단계는 기술적으로 실패한 것으로 간주하였다. 교감 프로브의 스코어가 2 이상이 아니었을 경우, 샘플들은 "HPV 음성"인 것으로 간주하였다.
이들이 개방 표지 실험에 도입될 경우, 22명의 환자들중 14명(64%)은 이들이 HspE7 또는 위약 100 ㎍을 3개월 동안 매달 간격으로 투약시키는, 이전의 이중 맹검 실을 통하여 항문성기 사마귀를 지속적으로 보유하였다. 환자들이 개방형 표지 실험에 있어서 교차되는 시간까지, 14명의 환자들중, 8명의 환자들(57%)은 악화되었으며, 4명의 환자들(29%)은 아무런 변화가 없었고, 2명의 환자들(14%)은 (1명은 눈에띄게 그리고 다른 한명은 미약하게) 호전되었다. 추가의 환자에서 이중 맹검 실험(개방 표지 실험의 개시 이전에 결정됨)의 개시시에 습우(condyloma)가 발생하였으며, 둘중 어느 하나의 실험시에 검출 가능한 사마귀를 보유하지 않는 다른 7명의 환자들과 같이, 이 환자는 이 분석법으로부터 제외시켰다. 습우는 14명의 환자 모두(100%)에 있어서 항문 직장관내에 존재하였으며, 14명의 환자들중 6명(43%)에서는 항문 주위의 피부에 존재하였다. 개방 표지 실험의 개시시에 사마귀를 보유하였던 14명의 환자들에 있어서, 발병 위치 관찰자들은 11명의 환자들(79%)의 경우 외과적으로 절제술이 필요하고, 2명의 환자들(14%)의 경우 국소 절제(예, 액체 질소, 전기 소작)가 필요하고, 1명의 환자(7%)의 경우 국소 치료(즉, 이미퀴모드(Imiquimod))가 필요함을 파악하였다. 개방 표지 실험에서 1개월 간격으로 HspE7 500 ㎍을 3회 주입시키는 대신에, 발병 위치 관찰자들이 추천하는 치료법을 연기하기 위하여 환자들을 선별하였다.
HspE7 500 ㎍으로 최종 처리한 이후 1개월 경과시, 2명의 환자들(14%)에게서는 사마귀가 관찰되지 않았으며, 11명의 환자들(79%)에서는 (개방 표지 실험을 수행시켰을때의 이들의 증상에 비하여) 사마귀의 크기 또는 수가 감소하였고, 1명의 환자들(7%)은 사마귀 크기가 증가하였음을 알 수 있었다(표 1). 개방 표지 실험의 1차 평가 시점(최종 투여 이후 4개월)까지, 추가로 한명의 환자가 부분적 반응 내지 완전 반응(즉, 사마귀가 관찰되지 않았음)을 경험하였는데, 이 경우 총 3명의 환자들(21%)이 완전 반응을 나타내었다(표 1). 개방 표지 실험의 6개월간의 평가 기간 동안, 이들 반응자들중 어느 누구에서도 재발하지 않았다. 10명의 환자들(71%)은 계속하여 부분 반응을 개선(즉, 잔류하는 사마귀를 제거하는데에 필요한 처리의 강도를 계속해서 감소시켜 크기를 더욱 많이 감소)시켰다. 무반응자(7%)는 개방 표지 실험의 말기에 호전되지 않았다.

험 마지막에, 발 위치 관찰자들은 3명의 완전 반응자에 대하여 더이상 치료하지 말것을 권고하였다. 표 2에 제시된 바와 같이, 부분 반응자의 위치 관찰자들의 추천된 치료법은 절제 치료법(14명중 6명, 43%) 또는 국소 제제 치료법(14명중 4명, 29%)이었으며 ; 무반응자(14명중 1명, 7%)에 대하여는 추가의 시술을 추천하였다. 후속 단계에서 장기간 동안 22명의 환자들 모두의 반응에 관하여 기록하였으며, 관찰자들이 추천하는 치료법은 연기하였다.

문성기 사마귀를 보유하는 14명의 환자들 모두에서, 처음의, 램덤화된, 조절된 실험에 대한 스크리닝시 항문 면봉 표본에서 다수의 HPV 유형의 HPV DNA가 검출되었다(표 3). HPV-6 및/또는 11은 12명의 환자(86%)에 존재하였다. 1명의 환자만이 HPV-16 및 관련 유형을 보유하였으며, 다른 환자들은 분류될 수 없었다. 14명의환자들중 3명(21%)은 HPV-16에 대하여 양성이었다. 사마귀가 호전된 대부분의 환자들(13명중 11명, 85%)은 HPV-16을 보유하지 않았다. 무반응자들도 HPV-16을 보유하지 않았다(표 3 참조).

와같이 지속적 항문 HSIL 및 동시 발생 항문 사마귀를 보유하는 환자와 관련된 HspE7(3개월 간격, 500 ㎍)의 개방형 표지, 교차 실험에서, 기준선 수준의 사마귀를 보유하는 14명의 환자들중 3명(17%)은 최종 투여후 4개월 경과시 더이상 사마귀를 보유하지 않았다. 다른 10명의 환자들(71%)은 그 증상이 호전되었다(즉, 사마귀의 크기가 상당히 감소하여 잔류하는 사마귀를 제거하는데에 필요한 치료의 정도는 계속해서 감소되었다). 1명의 환자(7%)는 실험중 병세가 호전되지 않아서 위치 관찰자들에 의하여 추가의 수술을 하도록 권유받았다.
개방 표지 실험에 도입하기 이전에, 이 실험 단계에 있는 대부분의 환자들(14명중 11명, 79%)은 사마귀를 제거하기 위하여 때때로 수술을 수행할 수도 있다. 본 실험의 마지막 단계까지, 1명의 환자에게만 수술 치료법이 권고되었다. 국소 절제 치료법(예, 액체 질소, 전기 소작)이 6명의 환자들(43%)에게 권고되었으며, 4명의 환자들(29%)에게는 국소 제제(예, 이미퀴모드)를 사용한 치료법이 권고되었다. 3명의 환자들은 추가의 치료를 필요로하지 않았다.
6개월에 걸쳐서 반응이 진행되었으며 이 기간 동안 어떠한 반응자에서도 재발하지 않았다. 습우의 점차적이고 진행성인 결과는 HspE7에 의한 세포 매개 면역성이 유도된 이후 면역학적 숙주 반응으로부터 예측할 수 있는 사실과 거의 일치하였다.
개방 표지 실험에 도입하기 이전의 이중 맹검 실험에 있어서, 2명의 환자들에게서 습우가 약간 호전되었다. 이때까지, 맹검 증상을 유지시켰으며, 이 경우 환자들이 Hsp E7 또는 위약 100 ㎍을 투여받았는지의 여부는 알 수 없다. 그러나, 3개월 간격으로 500 ㎍의 HspE7을 주사하는 개방 표지 실험에서 관찰되는 반응을 기초로 하여, 투여량이 100 ㎍보다 많을수록 더욱 활성인 것으로 나타났다.
사마귀가 HspE7 치료 이후에 호전된 13명의 환자들중 3명의 환자(23%)에게서만 얻은 항문 면봉 표본에서 HPV-16 DNA가 검출되었다. HspE7을 사용한 치료법에 반응한 사마귀를 보유하는 환자들의 대부분에서, HPV-6, HPV-11 또는 이들 모두로부터 얻은 DNA를 검출하였다. 이 데이터를 통하여 HspE7에 대한 반응에서 HPV 유형들간 면역학적 교차 반응성이 나타났다는 사실을 알 수 있다.
요약하면, 본원에 제시된 결과들은 HspE7이 항문 사마귀에 광범위하게 활성을 나타내는 것을 나타낸다. 이러한 활성은 HPV-16 양성 환자들에게 한정되는 것으로 보이진 않았으나, 다수의 HPV 유형과 교차하였다. HspE7은 항문 부위의 HPV-유발성 질병의 치료에 활성일 것이며 이러한 활성은 HPV-16 양성 환자들에게 한정되는 것은 아닐 것이다.
본원에 제시된 관찰 결과들은 HspE7을 사용한 치료법이 신규의 간단하고 수술을 수반하지 않는 항문 사마귀의 치료법(최소한 사마귀 종양을 감소시킴)을 구성할 수 있으므로, 치료 및 사망율의 수준을 감소시킬 수 있다는 사실을 제시하는 것이다. 내부 항문 직장 질병은 종종 꽤 고통스럽고 몸을 허약하게 만드는 추가의 치료법을 필요로 한다. "수술" 또는 절제 치료법의 횟수와 규모를 감소시키거나 또는 줄이는 부분 반응을 제공하는 임의의 치료법은 사망율을 줄이고, 노동 시간의 허비를 감소시킬 뿐만아니라, 삶의 질을 향상시킨다.
이러한 결과들을 통하여, 열 쇼크 단백질/HPV 제16형 항원 조성물은 사마귀를 줄이거나 감소시키는데에 효과적인 것으로서, 주로 HPV 제6형 및 제11형에 의하여 유발되는 것으로 생각된다. (1) 사마귀는 HPV를 주성분으로 하는 모든 조성물로 치료될 수 있으며, (2) HPV 제16형 조성물은 제16형을 제외한 HPV에 의하여 유발될 수 있는 증상을 치료하는데에 효과적이라는 관찰 결과는 매우 중요하다. 후자의 교차 반응 결과는 전혀 기대하지 못하였던 것으로서, 이로써 유형 특이적 조성물은 유형 특이적 면역 반응만을 나타낸다는 일반적인 사실을 알 수 있다.
융합 폴리펩티드의 관찰된 교차 반응성에 대한 가능한 기전을 규명하기 위하여, 다양한 HLA 제Ⅰ군 분자 및 HPV 제16형, 제6형 및 제11형의 E7 펩티드에 관하여 이론적인 결합성을 평가하였다. Parker외 다수의 문헌[J.Immunol.152:163, 1994(또한 전술한 웹 사이트 참조)]에 기술된 알고리즘을 사용하여 해리의 T1/2을 계산하였다. 데이터를 표 4-1 ∼ 4-4에 요약하였다.
굵은 글씨로 표기한 펩티드 서열은 각각의 HLA 분자와, 각각의 HPV 유형으로부터 얻은 E7 단백질에 대한 2개의 상부 결합제를 나타내는 것이다.
표 4의 결과는 평가된 특이적 HLA 분자에 따라서, HPV 제16형 E7 항원은 기타 HPV 유형들의 E7 항원에 대한 세포 매개성 면역 반응을 추가항원 자극시킬 수 있다는 사실을 제시하는 것이다. 예를 들어, HLA B 2705에 있어서, 모든 3가지 유형의 HPV 에 대한 E7의 아미노산의 76번 위치로부터 시작되는 펩티드에 있어서는 결합 정도가 높을 것으로 예상된다. 그러므로, 이러한 HLA 분자를 발현하는 환자에 있어서, HPV 제16형 E7 조성물은 교차 반응할 수 있으며, HPV 제6형 및 제11형에 의한 감염을 치료 또는 예방하는데에 유용할 수 있다. 표 4에서 굵은 글씨로 나타낸 펩티드 단편들 각각은 완전 E7 바이러스 항원에 대한 대체물로서, 조성물(예, 본원에 기술된 융합 폴리펩티드)중에 포함될 수 있는 항원성 단편을 나타내는 것이다. 물론, 이러한 2 이상의 추정 HLA 에피토프들, 또는 다수의 추정 HLA 에피토프를 함유하는 긴 단편도 사용될 수 있다.
Claims (39)
- 개체에서 사마귀를 치료하는 방법으로서, 이 방법은 사마귀를 보유하거나 또는 보유하는 것으로 의심되는 개체를 식별하는 단계 ; 및 이 개체에 (1) 열 쇼크 단백질(hsp) 또는 이의 면역자극성 단편과, (2) 인간 파필로마 바이러스(HPV) 단백질 또는 이의 항원성 단편을 포함하는 융합 단백질을 함유하는 조성물을, 사마귀를 치료하는데에 효과적인 양으로 투여하는 단계를 포함하는 것인 방법.
- 제1항에 있어서, hsp는 미코박테리아 hsp인 것인 방법.
- 제2항에 있어서, 상기 미코박테리아 hsp는 미코박테리움 보비스(Mycobacterium bovis) hsp인 것인 방법.
