Abstract
본 발명은 경피 흡수에 의해 생체에 펩티드성 약물을 유입시킬 수 있는 국소 도포 약제 제제에 관한 것이다. 이러한 제제는 약제학적 국소 제제에 통상적으로 사용되는 담체 및/또는 희석제와 혼합되는 (ⅰ) 펩티드성 약물, (ⅱ) 캅사이신, 히스타민 및/또는 칸타리스 추출물 및 (ⅲ) 상피화제를 포함하며, 임의로, 피부에 부착가능한 지지물상에 도포된다.
Description
본 발명은 경피 흡수에 의해 생체에 펩티드성 약물을 전달할 수 있는 국소용 약제 제제에 관한 것이다. 본 발명에 따른 국소용 약제 제제는 예를 들어, 연고, 크림, 겔, 로션, 용액, 현탁액 및 국소 치료에 일정하게 사용되는 이와 유사한 제형일 수 있으며, 이는 임의로 지지된 형태로 피부 표면에 적용될 수 있다. 이들 중 특히 바람직한 대표적인 것은 국소용 제형이 피부에 부착될 수 있는 지지체, 즉, 플라스터(plaster)(패취(patch))상으로 도포된 것이며, 이는 안전한 투여 및 용이한 조작을 가능하게 한다.
환자의 편안한 치료에 있어서, 아주 바람직한 제형은 국소용 약제 제제이다. 주사액과 같은 비경구 투여용 제형의 주입은 고통 또는 적어도 불쾌함을 유발하며, 의사 또는 간호사의 도움이 필요하거나, 스스로 투여할 경우, 환자의 연습 및 기술이 요구되는 반면, 이와 달리, 국소 제형은 심지어 환자에 의해 용이하고 고통 없이 치료될 부위에 적용될 수 있다. 국소 제형은 또한, 부전 실어증을 앓고 있거나, 약의 맛을 싫어하는 환자에 대해 경구 투여용 제형보다 더욱 바람직하다. 국소 제형의 특이적 이점은 환자가 처음에는 시각으로 또는 그의 피부를 만져봄으로써 약이 도포됐는지의 여부를 쉽게 확인하여, 부주의한 과잉 투여 또는 과소량 투여의 위험을 현저하게 감소시킬 수 있다는 점이다.
이러한 이점에도 불구하고, 국소용 제형은 다소 좁은 범위, 일반적으로 외상 및 국소 염증 치료를 위한 의료 수행에만 사용되었다. 그 이유는 주로 활성제 흡수의 생리학적 특징에 있다. 국소 도포 활성제가 표적 유기체에서 적당한 농도에 도달하기 위해서는, 활성제가 피부를 통해서 뿐만 아니라 피하 혈관벽을 통해서도 침투되어야 한다. 그러나, 피부 및 혈관벽은 이들의 물리적 구조(다공성 및 반투과성 막 성질) 때문에 아주 제한된 투과성을 갖는다. 또한, 피부와 접촉하는 이종 물질로부터 생체를 보호하기 위해 실질적으로 제공된 소위 각질층의 방해 작용에 의해 현저한 장애가 발생한다.
관련 활성제의 유입을 위해 경구 투여용 제형을 성공적으로 대신할 수 있는 몇몇 국소 제형이 이미 고안되어 있다. 이들 중 하나는 상품명 모토(MOTTO(R))로서 헝가리에서 약 15년 동안 시판되는 중인 항류마티즘 패취이며, 이는 연고 에멀션화제(Unguentum emulsificans)(비이온성 텐시드(tenside)를 포함하는 크림, Ph.Hg.V.)로 형성된 에멀션중에 포름산(항류마티즘 제(agent)로서), 살리실산 및 선택적으로 통증완화제를 포함하며, 이는 피부에 부착될 수 있는 지지물 상에 도포된다. 시바-게이지 컴패니(Ciba-Geigy Co. German Federal Republic)로부터 시판되는 조성물인 상품명 니트로데름(NITRODERM(R))은 메틸 셀룰로오스 또는 카르복시메틸 셀룰로오스로 형성된 겔중에 니트로글리세롤(강심제로서)을 포함하며, 이는 피부에 부착될 수 있는 작은 폴리우레탄 스폰지에 도포된다. 히사미트수 파마슈티칼스 컴패니 인코포레이티드(Hisamitsu Pharmaceutical Co. Inc. Japan)로부터 시판되는 조성물인 상품명 살론파스(SALONPAS(R))는 폴리비닐 알코올로 형성된 진한 부형제중에 메틸 살리실레이트(항염증제로서) 및 멘톨을 포함하며, 이는 피부에 부착될 수 있는 작은 폴리우레탄 스폰지에 도포된다. 이러한 공지된 조성물 중의 활성제는 비교적 저분자량의 강한 극성 물질이며, 따라서, 이들은 고분자량의 덜 극성인 물질보다 흡수되는 경향이 크다.
펩티드성 화합물은 약제학적 활성 물질의 중요한 그룹을 형성한다. 본 명세서 및 특허청구의 범위에 사용된 용어 "펩티드성 화합물" 또는 "펩티드성 약물"은 일반적으로, 임의의 약제학적 활성 물질을 의미하며, 이들의 분자는 펩티드 결합으로 서로 결합된 2개 이상의 아미노산을 포함한다. 약제학적으로 아주 중요한 이들의 한정된 그룹은 폴리펩티드의 그룹이며, 이들 중에서 천연 호르몬, 이의 합성 유사체 및 유도체가 특히 관심의 대상이다. 공중 위생의 견지에서 보면 아주 중요한 호르몬 그룹중 하나는 인슐린이다. 생물체의 정교한 균형을 조절하는 호르몬 및 호르몬 유사체는 유해 물질이며, 이에 따라 이들 물질의 경우에는 부주의한 과잉 또는 과소량 처리의 위험성을 감소시키는 것이 특히 요구된다. 일부 대표적인 호르몬 및 호르몬 유사체, 특히, 인슐린은 오로지 비경구 투여에 의해서만 유입될 수 있다. 따라서, 이러한 활성제를 생체에 국소적으로 유입시키는 것은 아주 바람직할 수 있다. 그러나, 생체에 펩티드성 약물을 유입시키는데 적용될 수 있는 국소 제제는 아직 개시되어 있지 않다.
본 발명자의 연구를 기초로 하여, 본 발명자는 특정 보조제를 사용함으로써, 펩티드성 약물을 경피 흡수가능한 형태로 유도할 수 있음을 밝혀냈다. 이러한 인식은 펩티드성 약물을 포함하는 국소용 약제 제제를 제조할 수 있게 한다.
따라서, 본 발명은 생체에 펩티드성 약물을 유입시키기 위한 국소용 제제에 관한 것이다. 본 발명에 따른 제제는,
(ⅰ) 펩티드성 약물,
(ⅱ) 캅사이신, 히스타민 및/또는 칸타리스 추출물, 및
(ⅲ) 상피화제(epithelizing agent)를, 국소용 약제 제제에 통상적으로 사용되는 담체 및/또는 희석제와의 혼합물로서 포함하며, 임의로, 피부에 부착되는 지지물상에 도포된다.
본 발명에 따른 제제는 펩티드성 약물로서, 바람직하게는, 천연 호르몬, 이의 합성 유사체 또는 유도체, 특히 바람직하게는, 인슐린을 포함할 수 있다. 본 발명에 따른 제제의 단위 용량(즉, 1회 치료를 위한 용량)에 함유되는 펩티드성 약물의 양(예컨대, 단일 플라스터에서)은 통상적인 경구 또는 비경구 형태의 단위 용량중에 존재하는 양과 거의 동일한 양일 수 있다. 따라서, 예시된 바와 같이, 진성 당뇨병의 치료에 적합한 본 발명에 따른 플라스터는 하나의 플라스터 또는 단위 플라스터 단편(예를 들어, 플라스터 표면의 1㎠에 대해) 당 0.1 내지 50 IU의 인슐린을 포함할 수 있다.
본 발명에 따른 제제의 성분(ⅱ)을 형성하는 캅사이신, 히스타민 및 칸타리스 추출물(팅크 칸타리디스(Tinctura cantharidis)의 활성제)은 발적성 국소 혈관확장제이다. 이러한 물질은 예를 들어, 동상, 팔다리 통증, 류머티즘 등을 치료하기 위한 피하 순환부를 자극하기 위해 외부 마사지용으로서 오랫 동안 사용되어 왔다[참조: Kiray, Racz, Torok: Bor-es nemibetegsegek(Skin and Venereal Diseases, in Hungarian); 1927 and subsequent pages(Medicina Konyvkiado, Budqpest, 1986)]. 본 발명에 따른 제제의 성분(ⅲ)인 상피화제로는, 피부의 상부 상피층을 약화시키기 위해 치료에 적용되는 것으로 공지된 어떠한 작용제도 이용될 수 있다. 이러한 작용제의 특징적인 대표되는 것으로는 예를 들어, 상기에 언급된 문헌에 기재되어 있다. 살리실산, 염화나트륨, 우레아 및 레조르시놀이 본 발명에 따른 제제중에 사용되어, 양호한 결과를 유도해낼 수 있는 상피화제의 실례들이다.
상기에 언급된 문헌에서 나타난 바와 같이, 비록 본 발명에 따른 제제의 성분(ⅱ 및 ⅲ)이 국소 치료용 조성물에 이미 개별적으로 사용되었지만, 이들을 조합하여 사용하는 것과 예상되는 조합물의 효과에 대한 어떤 데이타도 상기 문헌에는 언급되어 있지 않다. 또한, 성분(ⅱ) 또는 성분(ⅲ)이 국소 치료의 목적을 위해 펩티드성 약물과 조합되어 사용될 수 있다는 것도 언급되어 있지 않다.
본 발명에 따른 제제에 있어서, 성분 (ⅰ) 및 (ⅱ)는 서로 상호작용을 하며, 이는 예를 들어, 캅사이신이 인슐린 현탁액(상품명 인슐린 렌테 MC(Insulin lente MC)의 이름으로 노보 노르디스크에서 시판되는 제제와 같은)에 첨가될 경우, 투명한 용액이 형성된다는 관찰에 의해 입증되었다. 이러한 상호작용의 정확한 특성은 아직 규명되지 않았지만, 가장 유력하게는 두 성분의 느슨한 수소 함유 기와 국부 수소 억셉터 중심 간에 수소 결합이 형성되기 때문인 것으로 여겨진다 (캅사이신의 HO- 및 HOOC-기와 인슐린의 -NH2기 간에서와 같이). 본 발명자의 견해로는, 본 발명에 따른 제제로부터 펩티드성 약물의 경피 흡수 능력은 기본적으로 이러한 상호작용에 기인할 수 있다.
본 발명에 따른 제제는 성분(ⅱ)를 적어도 상기 상호작용을 일으킬 수 있는 양으로 함유해야 하지만, 이러한 최소량보다 많은 양의 성분(ⅱ)를 함유하는 것이 바람직하다. 임의의 개별적인 펩티드성 약물/성분(ⅱ) 쌍에 있어서, 최소 비 및 적합한 비는 약물요법 분야의 업자에게 일반적으로 공지되어 있는 시험에 의해 용이하게 측정될 수 있다. 일례로서, 예를 들어, 본 발명에 따른 인슐린 함유 제제는 10 IU의 인슐린에 대해 0.001 내지 0.3g, 바람직하게는, 0.003 내지 0.1g, 특히 바람직하게는, 0.005 내지 0.05g의 성분(ⅱ)를 함유할 수 있다.
본 발명에 따른 제제는 성분(ⅱ) 및 성분(ⅲ)을 1:(0.01 내지 10)의 중량비, 바람직하게는, 1:(0.03 내지 3)의 중량비, 특히 바람직하게는, 1:(0.05 내지 2)의 중량비로 함유할 수 있다.
본 발명자는 본 발명에 따른 제제에 성분 (ⅱ)와 (ⅲ)의 양이 많을수록, 활성제의 흡수가 더 빨라짐을 관찰하였다.
본 발명에 따른 제제는 국소 제제용의 통상적인 담체 및/또는 희석제와 함께 혼합되는 성분(ⅰ) 내지 (ⅲ)을 포함하며, 통상적인 담체 및/또는 희석제의 예로는 물, 알코올(또한, 글리세롤을 포함), 글리콜, 겔화제(예컨대, 메틸 셀룰로오스, 카르복시메틸 셀룰로오스, 폴리비닐 알코올 및 폴리비닐 아세테이트), 연고 기재물(예컨대, 라놀린), 이온성 및 비이온성 텐지드(tenzid) 등이 포함된다.
본 발명에 따른 제제는 또한, 엔벨로핑(enveloped)된 형태(예를 들어, 시클릭 스타치(cyclic starch)에 넣어지거나 마이크로캡슐화된 형태)의 펩티드성 약물을 포함할 수 있다. 이러한 엔벨로프(envelope)의 사용은 예를 들어, 고찰된 펩티드성 약물이 선택된 담체 또는 희석제와 양립할 수 없는 경우, 약물 요법에 있어서 고려될 필요가 있다. 예를 들어, 친수성 약제가 소수성 표면을 갖는 엔벨로프에 넣어질 경우, 이는 오일 담체와 양립할 수 있게 된다. 이러한 실례에서, 성분 (ⅰ)와 (ⅱ)의 상호 작용은 엔벨로프로부터 성분 (ⅰ)의 유리 속도로 진행된다.
필요에 따라, 본 발명에 따른 제제는 또한, 펩티드성 약물과 함께 유입될 다른 약제학적 활성제 또는 활성 증가제를 포함할 수 있다. 이러한 성분의 예로는 때때로 인슐린과 함께 첨가되는 아연 화합물이 있다.
필요에 따라, 본 발명에 따른 제제는 펩티드성 약물의 존재를 검출하기에 적합한 지시약을 포함할 수 있다. 고려되는 약물의 존재 여부에 따라 반응하여 시각적으로 관찰가능하게 변하는(대부분 적합하게는 색 반응) 물질이 지시약으로서 사용될 수 있다. 청색화됨으로써 펩티드성 물질의 존재를 나타내는 닌히드린은 펩티드성 물질의 존재를 나타내는데 매우 적합한 지시약이다. 이러한 지시약 함유 제제의 큰 이점은 환자가 적용된 조성물의 색 변화(예를 들어, 닌히드린의 청색의 소멸)로부터 흡수가 일어났는지를 용이하게 결론내릴 수 있으며, 필요에 따라 적절한 간격으로 이미 소모된 플라스터를 교체할 수 있다는 점이다. 본 발명에 따른 제제는 양호한 관찰 능력을 제공하는 양의 지시약을 포함할 수 있다. 이러한 양은 분석분야의 업자에게 널리 공지되어 있거나, 일정한 시험에 의해 용이하게 측정될 수 있다.
하기 실시에는 본 발명에 따른 제제의 조성 및 제조 방법을 설명하기 위해 제공한 것이다.
실시예 1
0.28ml의 팅크 캅사이시니(Tinctura capsaicini)(Ph. Hg. V., 캅사이신 함유 70% 알코올 용액) 및 0.05g의 살리실산을 40IU/ml의 인슐린(상품명 인슐린 라피타르드(Insulin rapitard)로 노보 노르디스크 (Novo Nordisk)에서 시판)을 포함하는 10ml의 근내 투여가능한 수성 현탁액에 첨가하였다. 캅사이신 첨가시, 인슐린 현탁액으로부터 투명한 용액이 형성되었다. 0.01 중량%의 닌히드린을 용액에 첨가하자, 용액의 색이 청색으로 변하였다. 생성된 용액을, 15 중량%의 메틸 셀룰로오스를 첨가하여 겔화시키고, 형성된 겔을 20㎠의 활성 표면을 갖는 폴리우레탄 스폰지 플라스터상에 도포시켰다. 도포된 겔을 알루미늄 호일로 덮었다.
실시예 2
10g의 비이온성 연고 에멀션화제(Unguentum nonionicum emulsificans)(Ph.Hg.V., 비이온성 텐지드로 형성된 오일 겔)를 10g(400IU)의 결정성 인슐린, 0.05g의 결정성 살리실산 및 0.28ml의 팅크 캅사이시니와 혼합하였다. 생성된 오일 연고를 20㎠의 활성 표면을 갖는 폴리우레판 스폰지 플라스터상에 도포하고, 도포된 연고를 알루미늄 호일로 덮었다.
Claims (12)
- 인슐린을 생물체에 유입시키기 위한 국소 도포용 제제로서,(ⅰ) 인슐린,(ⅱ) 캅사이신, 및(ⅲ) 피부의 상부 상피층을 약화시키는 작용제를, 국소용 약제 제제에 통상적으로 사용되는 담체, 희석제 또는 이 둘 모두와의 혼합물로서 포함하며, 그 자체로 사용되거나 피부에 부착될 수 있는 지지체 상에 도포되어 사용되는 제제.
- 제 1 항에 있어서, 성분(ⅲ)으로서 살리실산, 염화나트륨, 레조르시놀 또는 우레아를 포함함을 특징으로 하는 제제.
- 제 1 항에 있어서, 인슐린의 존재 여부에 따라 반응하여 시각적으로 관찰가능하게 변하는 지시약을 포함함을 특징으로 하는 제제.
- 제 3 항에 있어서, 지시약으로서 닌히드린을 포함함을 특징으로 하는 제제.
