Description

【発明の詳細な説明】 (技術外1Ilf) 本発明の技術分野は、拡張、閉灯1、LF人、除去又は
視覚化する為に、管の中に少数のカブーテ/Lを白装置
することに1刀XS t、で用いl;、 t+るカテー
テル備品に関すイ)。つりトリ、いくつかのカブーテ/
LをさL2込む手11ir+に基本的に関連して用いら
り、るものに関する。
さらに明碌゛にすると、本発明は、通常の装置では近づ
くことが困難か又は不1−1]能である部位に、2番目
のカブ−デルを導いてその位置に留、V)る為の第1番
目の、つまりカテーテルを偏向り、−C案内する装置及
びその方法に関する1、第2番11C)ツノシー−チル
には、主として拡張、注入、渭!流、診断又は閉塞除去
手術用のカテーテルが含まり、る、yt:;/こ、本発
明の第1のカテーテルは、第2のJノテープ゛ルの代わ
りにガイドワイヤを導いて所定の位bJNVC留めたり
することにも用いられろ。
(従来技術の説明) カブ−チルを偏向案内する装置に関すイ)イlη来4支
術は存在しない。
(発明の概要) ?n]記偏向案内カテカブノシ・、つまり第1のノyチ
ーチルは、第2のカテーテルが通過う゛るのに十分な案
内管眸を有する。管腔は案内カテーテルの長さの大部分
において、同カデーブルの軸線に沿−って延びている。
その管腔はカテーテルの末端部に近づくにつれて、軸線
から角度をなL5−〔滑カ・VC11,117J’って
出口で終端する。この出口は、1917角形成動ru葭
の入口に語口して容易に配置できる。青1f’1′−,
の末端部のこの上“)なり曲した形状によれば、第2カ
テ・−デルか1)il :il、Ilす41間にそのカ
ブ−デルが支持され、庄/へ、第2カテーブルを配置し
7、膨張し、かつ、又は、押出する間に第2カデーテル
が後退し得ないよ′)にさ4する。
このカブ−チル偏向案内装置は、分枝した一方の枝かに
)1(12力の枝にカテーテルを移動する場合に間:、

用できく)。

カー1−−〜−7−ル案内装置は、柔軟な管状の本体(
10)を含み、この本体(1U)には、丸めr〕れた末
端部(12)と、カブ−チル内にガイドワイヤ又は他の
カデーテルイし・容易に挿入する為の漏斗状の案内ノケ
ソト(15)をf]する、接近1り能な案内部(14)
とが形成さJドている。+1fJ記カテーテル(10)
は、管腔(18)をイIIGえて−13・す、この’R
IIO(18)はカブ−デルの長さ方向の大部分のR1
12に糾−って軸線に沿って延長し、その後、IIII

テル案内装置は、末端部(12)及び供給口(22)の
位置を示す為に、又は、この但給ト1(22)と分岐動
脈(26)の入口とを補助的に一致させる為に、信号を
通過させないマークがイτJされていて)。このマーク
は参照番号(24)で示◇れている。
第3図は、例えば、分枝(26)を具えた三核に分かれ
た冠状動脈(28)を示す。分か11、た枝の内部には
治療すべき狭窄病変部(口0)がある。
カテーテル偏向案内装置(10)は、案内カブ−チル(
32)を通って、動脈1で沿って揮ば1Lイ)が、案内
カテーテル(32)は、米国特許4,323.D l 
1号、;[2載の案内カテーテル(21)と同イ子のも
のである。
環状の案内マーク(24)は、枝分か”1,7”C動脈
(26)の入口に供給L:](22)が到達しまたこと
を知らせて)。
カテーテル(10)は、その時、その細軸を中心として
回転さh、分かれた動脈の人(二1に夕・」向トて供給
口(22)が配置されることになる。このjμ合には、
供給口(22)の長軸線に沿って延びるマーク(2,1
)か調整の為に用いられる。この結果、カテーテル偏向
案内装置(10)と、主動脈(2B)及び枝動脈(26
)との関係は第4図に示す形状になる。カデーテ4(1
0)の本体は、1i111i叶ノ1だ、つまり比較的堅
いが…目ずやずい構蹟L ’iつている。ぞ旧ば枝分か
Jul、た動脈に供給[、+(22)を向けさぜるのに
必要にして十分なトルクな伝える為である。しかし、カ
テーテル(10)の末端部(12)は、なめらかで、か
つ柔軟に造られている。
カテーテル(10)と主動脈及び枝動脈の関係が第4図
に示す状態になると、第5図に示すように、柔軟な管状
の本体(38)及び膨張端部(40)で構成さAtだ拡
張カテーテル(口6)は、カテーテル(10)の管11
やに沿ってカテーテル(10)の11I11面にある供
給m1(22)な曲過して、分枝1./こ動脈の狭2ρ
病変部(30)(\j近に供給される11.jJイドワ
イヤ回様に他の形式のカテーテルも、カテーテル(10
)に?F1<)て送ることかできる1、膨張端部(40
)は、反転型が好ましく、このタイプは最初の1)す張
の段階ではかなりの程度放射状に膨張することな(カテ
ーテルの外にめくり返る性質を41し、ま/こ、その後
の第2の膨張の段階では放射状に膨張する性質を何して
いる。従つて、このような膨張部は、病変部(30)の
中+=1”めくり返さね7、そし7て放射状に膨張させ
r)」7ると、病変部は押し広げられ動脈が拡張し7て
n+冒11の流ね、が増大する。
第6図の実施例においては、ノlテーブル偏向案内装置
は、分岐した動脈(26)を超えてさ1゛)に1削くな
った動脈(2B)の末yiM部に先端部を容易に供給す
ることができるように径が小さくさハ/こ先で1,1部
(112)を有している。
第7図に示すカテーテル偏向案内装置の実施例は、カテ
ーテルの端部に固定的にガイドワイヤ延長部(113)
が取付けられた状態な示す。このガイドワイヤ延長部は
、動脈(28)から分岐り、/、−小さな末端部にカテ
ーテルの端部な供給する為のフIjツビ′−カイl’ 
(floppy gulde )としで伸υく。
第8図及び第9図に示ずカテーテルの実施例は、管+r
=′?(18)の軸方向の延長部(+19)をhず”’
 o ’+’? l+りと延長部の結合点には、コト栓
(121)が抜げないように詰められでいる。この枠(
121)にr)イト−ツイヤ(123)を貫通させても
よい。従って、このカデーテルイ111)向:f5内装
置に係る実施例は、案内装置かカイ1ワイヤの囲いを被
うことによって案内装置を定位ii’Yに配置″6′す
ることかできる。カテーテル偏向装置i;4条内・、l

Claims (5)

    案内し7、かつ枝分かij、 12だ管路内に前記ガイ
    ドワイヤ及び仙の)J11記カブーーーj゛ルを1]i
    )向させて挿入するカブ−−デフ1偏向案内装置におい
    て、前記本体にば11’l jt−勾・イドワイヤ、及
    び前記力テーブルを収容案内する装置がF’t yl:
     h、、該収容案内装置は、主・及42間部分と副次空
    間部分とをイjする管腔を備え、前記十要空1ハ」部分
    は、tiiJ記本体の長さの大部を占める。Lうに前記
    本体の軸線方向に延長し7 :l’、・す、前記副次空

    通路を有するカブ・−チル案内用管を配置し、前記案内
    用管の前記出11部を!j’J ML分岐管路の入【」
    と整合するように配置し2、次に、カテーテルを上記通
    路に沿わせて上記案内用管を通過させ、上記分岐管路内
    に移111!+さぜる方法。
  5. (5) 拡張カテーテルの竹状に突出17人−1k1.

     N[−:分岐動脈内の狭窄病変部を治療する方法にお
