【発明の詳細な説明】
超音波診断用のガス含有微小粒子を製造するためのメチレンマロン酸ジエステル
誘導体の使用、及びこの粒子を含有する薬剤
この発明は、請求の範囲に特徴づけられた対象物、すなわち、ガスを含有する
微小粒子を製造するための、非対称又は対称型メチレンマロン酸ジエステル誘導
体の使用、及びこの粒子を含有する超音波診断用のコントラスト剤に関する。
超音波診断は、無理がなく簡易な処理を行えるために医学において非常に広く
応用されてきた。超音波は種類の異なる組織の界面に達すると反射する。ここで
発生するエコー信号は電子により増幅されて目視できるようになる。血管と内臓
を超音波により表示することができても、一般的にその中の血流は表示されない
。液体、特に血液は、密度と圧縮率に周辺との差があるときだけ超音波コントラ
ストを生じる。医用超音波診断においては、例えば、ガスを含有する物質又はガ
スを発生する物質をコントラスト剤として使用するが、これは、液体又は固体と
血液との間にでるインピーダンスの差よりも、ガスとその周辺の血液の間にでる
インピーダンスの差のほうが、実質的に大きいからである(Levine R.A.,J.Am
.Coll.Cardiol.3: 28,1989; Machi I.J.,CU 11: 3,1983)。
微小気泡を含む溶液を末梢に注射すると、心臓のエコーコントラストが得られ
ることは知られている(Roelandt J,Ultrasound Med.Biol.8: 471-492,1982
)。これらの気泡は、例えば、振とう、別の方法の攪拌又は二酸化炭素の添加に
よって、生理的に許容される溶液中に得られる。しかしながら、気泡は数とサイ
ズについての標準化がなく、再現性も不十分にしか得られていない。気泡は、通
常、安定化されていないので寿命が短い。その平均直径はたいてい赤血球を超え
る大きさであるために、左心室、肝臓、腎臓又は脾臓などの器官は通過できても
、肺の毛細血管を通過し、その後器官ににコントラストを与えることはできない
。
そのほかに、気泡から発生する超音波エコーは、気泡生成、凝集および溶解な
どの、多数の互いに切り離せないプロセスから生成されるので、気泡は定量化に
向いていない。従って、例えば、超音波コントラスト剤の助けをかりて心筋層に
おけるコントラスト経過を測定して通過時間内における情報を得ることはできな
い。そのため、散乱体が十分な安定性を示すコントラスト剤が必要である。
欧州特許第0 131 540 号明細書には、糖類による気泡の安定化が記載されてい
る。これによってコントラスト効果の再現性と均一性は改善されるが、しかしな
がら、この気泡は肺を通過することができない。
欧州特許第0 122 624 号及び同第0 123 235 号明細書の記述によれば、糖、糖
アルコール及び塩による気泡の安定化効果は、界面活性物質を添加することによ
って改善される。肺の毛細血管通過性、並びに大腿動脈血管及び肝臓又は脾臓な
どの種々の器官を表示する可能性が、この超音波コントラスト剤を使用すると得
られる。しかし、このとき組織細胞に気泡が入れないので、コントラスト効果は
血管の内径によって制限されることになる。
記載された超音波コントラスト剤の中には、長時間に渡り体内に不変に留まる
ものはない。静脈投与後に選択的に蓄積することによって又は定量化によって、
十分なシグナル強度で器官を画像表示することは、これらの薬剤にはできない。
例えば、空気などのガスをカプセル化して超音波コントラスト剤として用いる
ことは、欧州特許第 0 224 934号明細書に記載されて
いる。ここで使用されるバリヤー物質は、タンパク質、とくに周知のアレルゲン
性をもつヒト血清アルブミンから成り、このために変性によって細胞障害効果が
現れることがある。
欧州特許第0 327 490 号及び同第0 458 745 号明細書には、生物分解性合成物
質を基にしたガスを含有する超音波診断用微小粒子が記載されている。これらの
薬剤は生体内で十分に長い寿命を示し、静脈投与により細網内皮細胞系の細胞内
に、それと同時に肝臓又は脾臓内にも蓄積される。
この発明の課題は、現在の技術水準の欠点を克服した、すなわち微小粒子を基
にした超音波コントラスト剤を開発して、超音波診断用コントラスト剤を提供す
ることにあり、このコントラスト剤は、
●周辺の組織に対して明らかなコントラストを示し、
●サイズが小さく安定であるため、実質上のガス損失がなく且つ静脈投与後に実
質的に定量的に左心室に到達し、
●良好な適合性を示し且つアレルゲン性がなく、
●水又は血液中で互いに塊状化することがなく、そして
●製造が迅速にして簡易である。
この課題は、次の発明により解決される。
一般式Iの重合した非対称又は対称型のメチレンマロン酸エステル:

基R1及びR2は、同じであるか又は異なってもよく、そして飽和又は不飽和の
、1〜8個の炭素原子を含む基を意味し、この基は場合により、酸素原子(エー
テル基)及びカルボキシル基(エステ
ル)を含有する〕
から成るガス封入粒子が超音波診断用のコントラスト剤として適切であることを
発見した。
基R1及びR2としては、例えば、CH3−、C2H5−、n−C3H7−、イソ−
C3H7−、n−C4H9−、イソ−C4H9−、n−C5H11−、n−C6H13−、C
H2CO2C2H5−、CH2−CH=CH2−、CH2−C≡CH−、CH2−O−C
H3−、C2H4OC2H5−、CH2CO2C2H5−、CH2CH2CH2CO2C2H5
−及びアリル基が挙げられ、ここでR1及びR2は、互いに独立に同じであるか又
は異なることができる。
好ましくは、メチレンマロン酸エステルとして
が使用される。
粒子中に封入されるガスとして、空気、窒素、酸素、希ガス、二酸化炭素及び
フルオロ炭化水素が考慮の対象となる。
粒子は500nmから7μmの範囲にある平均サイズを有する。
相当する薬剤が示す利点は、特にそれらが生体内でより速やかに分解すること
、そして分解生成物に毒性の危険性がないことである。
壁の厚さは製造工程によって影響されるので、音域を超えて振動モードを励起
させる粒子を製造することができ、従ってコントラスト生成はもう1つ別の追加
成分にかかっている。
本発明の別の観点は、本発明の粒子の製造方法に関する。
非対称又は対称型のメチレンマロン酸エステルを基にした本発明
の粒子を製造するには、それぞれ所望のメチレンマロン酸エステルのモノマーを
、ガスで飽和された緩衝化水溶液中で、必要ならば1又は複数の界面活性物質を
入れて、スターラーを使って分散させ、続いて重合(約2〜6時間)させた後、
得られた粒子を分離し、必要ならば水で洗浄し、次いで薬学的に許容される懸濁
媒中に取り出して凍結乾燥を行う。
必要な場合に反応工程に供給される界面活性物質として考慮の対象になる好ま
しい物質は、Poloxamere(商標)、多糖類、ポリソルベート、サッカロースのモ
ノ又はジエステル、ポリエチレングリコールアルキルエーテル及びそれらの混合
物の群より選ばれた1又は複数の物質である。
ガスで飽和された緩衝液のpH値は、好ましくは5〜8である。粒子の分離は
、浮上分離法(Floatation)を用いて行う。
分離した粒子は、引き続いて望ましい場合には、洗浄し、懸濁媒中に再懸濁し
、そして凍結乾燥を行う。懸濁媒として注射用蒸留水が適し、必要ならばこれに
食塩及び/又はグルコース及び/又はマンニトール及び/又はラクトースを添加
し、必要ならば懸濁媒に界面活性物質を添加することができ、該物質としては多
糖類、ポリソルベート、Poloxamere(商標)、サッカロースのモノもしくはジエ
ステル又はポリエチレングリコールアルキルエーテル、並びにそれらの混合物が
挙げられる。
凍結乾燥粒子から注射にすぐに使える超音波コントラスト剤を製造するには、
凍結乾燥物を薬学的に許容される懸濁媒中に再懸濁するが、そのような懸濁媒と
しては、例えば注射用蒸留水、1又は複数の無機塩類の水溶液、つまり、生理的
電解質溶液、例えばタイロード液などの緩衝液、グルコース又はラクトースなど
の一糖類又は二糖類の水溶液、マンニトールなどの糖アルコールの水溶液が挙げ
られ、該水溶液は、必要ならば更に界面活性物質、例えば、ポリソルベートもし
くは多糖類もしくはポリビニルピロリドンもしくはポリエチレングリコリルエー
テル、サッカロースのモノ及びジエステル、又は Poloxamere(商標)もしくは
その混合物、及び/又はグリセリンなどの生理的適合性をもつ多価アルコールか
ら成る群の物質を含有する。好ましい懸濁媒は、注射に適した水である。必要な
場合に溶解した物質の全体濃度は0〜15重量%を有する。
注射にすぐに使える製剤を製造する別な方法においては、粒子を製造する本発
明の方法の最後の凍結乾燥が省略される。
投与の安全性を高めるために、注射の直前に懸濁液の濾過を行うとよい。
次に述べる実施例は本発明の対象を詳しく説明するものであるがそれらに限定
されるものではない。
出発物質として使われるメチレンマロン酸エステル誘導体の製造については文
献から公知であり、例えば、ドイツ国特許第27 34 082 号明細書、米国特許第4,
931,584 号明細書、J.Org.Chem.48,3603(1983)及び Makromulekulare Che
mie 107 ,4-5(1967)に記載されている。実施例1
メチリデンマロン酸ジエチル1mlを、1%デキストラン−8(Serva,Feinbio
chemica GmbH & Co.)を含む 0.01 Mリン酸塩緩衝液,pH 7.4 100 ml中でスタ
ーラー(Dispermat-FT,VMA-GetzmannGmbH)を用いて20℃において60分間、1000
0 rpm(回転/分)で分散させた。次いで反応混合物をスターラーで攪拌しなが
らフラスコに移し、そして室温で6時間攪拌(300 rpm )して重合させた。
ガスを充填したナノサイズ又はマイクロサイズの超音波活性粒子
を浮上分離法によって分離し、水又は 0.9% NaCl 溶液で数回洗浄し、そして1
%デキストラン−8水溶液 200 ml 中に取り出した。
粒子は800 nmの平均サイズを有し、且つ優れた超音波活性を示す。試験管内試
験で得られた後方散乱係数αsは、5MHz、C = 2.2×10-7 T/ml のもとでαs
=7.8 ×10-2 dB/cm である。実施例2
実施例(1)の方法において、緩衝液系のpH値を8.0 にし、デキストラン−
8をデキストラン−10により置き換えた。粒子は平均サイズ 700 nmを有した。0
.1 %デキストランを含む5%マンニトール溶液 150 ml 中に粒子を取り出した
。実施例3
実施例(1)の方法において、緩衝液系のpH値を7.4 にし、デキストラン−
8をポリビニルピロリドン Kollidon(商標)PF-17により置き換えた。粒子は平
均サイズ 1.3μmを有した。粒子を0.1 % Kollidon(商標)PF-17 を含む5%
グルコース溶液 150ml中に取り出した。実施例4
実施例(1)の方法において、デキストラン−8をBrij(商標)-35 により置
き換えた。粒子は平均サイズ 2.0μmを有した。1%Brij(商標)-35 を含む 0
.5%グルコース溶液 150 ml 中に粒子を取り出した。実施例5
実施例(1)の方法において、デキストラン−8をBrij(商標)-96 により置
き換えた。粒子は平均サイズ 2.0μmを有した。粒子を0.1 %Brij(商標)-96
溶液 150 ml 中に取り出した。実施例6
実施例(1)の方法において、デキストラン−8を2%Tween(
商標)-20 により置き換えた。0.1 % Tween(商標)-20を含む5%マンニトー
ル溶液 150 ml 中に粒子を取り出した。粒子は平均サイズ 1.0μmを有した。実施例7
“1−エトキシカルボニル−1−エトキシカルボニルメチレンオキシカルボニ
ルエタン”のモノマー1ml を、1%デキストラン−8(Serva,Feinbiochemica
GmbH & Co.)を含むリン酸塩緩衝化水溶液 100ml(KH2PO4/Na2HPO4,0.066N
,pH 5.5)中でスターラー(Disperment FT,Getzmann GmbH & Co.)を用いて20
℃において60分間、8000 rpmにて分散させた。次いで反応混合物を、スターラー
を取り付けたフラスコに移し、そして室温で6時間攪拌(300 rpm)してさらに
重合させた。ガスを充填したナノ又はマイクロサイズの超音波活性粒子を、浮上
分離により又は遠心により分離し、水で数回洗浄し、そして0.1 %デキストラン
−8を含む5%マンニトール溶液200 ml中に取り出した。
粒子は 1.5μmの平均サイズを有し、且つ優れた超音波活性を示す。試験管内
試験で測定された後方散乱係数αsは、5MHz、C = 1.0・10-7 T/mlのもとでαs
=1.5 ×10-1 dB/cm であった。実施例8
実施例(7)の方法において、リン酸塩緩衝液のpH値を6.0 にした。粒子は
平均サイズ1.0 μmを有した。実施例9
実施例(7)の方法において、リン酸塩緩衝液のpH値を6.5 にした。粒子は
平均サイズ1.2 μmを有した。実施例10
実施例(7)の方法において、リン酸塩緩衝液をpH値 5.5のクエン酸(0.1 M
)/Na2HPO4(0.2 M)緩衝液により置き換えた。粒子
は平均サイズ1.0 μmを有した。実施例11
実施例(7)の方法において、デキストラン−8をデキストラン−10により置
き換えた。粒子は平均サイズ 0.8μmを有した。粒子を5%デキストラン−10を
含む5%グルコース溶液200 ml 中に取り出した。実施例12
実施例(7)の方法において、デキストラン−8を3%ポリビニルピロリドン
PF-17により置き換えた。粒子は平均サイズ 1.5μmを有した。0.5 % Kollido
n(商標)PF-17 を含む5%マンニトール溶液 200 ml 中に粒子を取り出した。実施例13
実施例(7)の方法において、デキストラン−8を3% Tween(商標)-80 に
より置き換えた。粒子は平均サイズ 1.2μmを有した。粒子を5%グルコース溶
液 200 ml 中に取り出した。実施例14
実施例(7)の方法において、デキストラン−8を2% Tween(商標)-40 に
より置き換えた。粒子は平均サイズ 1.0μmを有した。粒子を5%マンニトール
溶液 150 ml 中に取り出した。実施例15
実施例(7)の方法において、デキストラン−8を3% Pluronic(商標)F 6
8により置き換えた。粒子は平均サイズ 1.8μmを有した。粒子を5%マンニト
ール溶液 150 ml 中に取り出した。
Claims (1)
- 【特許請求の範囲】 1.超音波診断用のガス含有粒子を製造するための、一般式Iのメチレンマロ ン酸ジエステル誘導体: 〔上式中、 基R1及びR2は、同じであるか又は異なってもよく、そして飽和又は不飽和の 、1〜8個の炭素原子を含む基を意味し、この基は場合により、酸素原子(エー テル基)及びカルボキシル基(エステル)を含むことがある〕 の使用。 2.基R1及びR2が、メチル基、エチル基、イソプロピル基、プロピル基、ブ チル基、ペンチル基、アリル基、プロピニル基、メトキシメチル基、エトキシエ チル基、エトキシカルボニルメチル基又はエトキシカルボニルプロピル基を意味 する、請求項1に記載の一般式Iのメチレンマロン酸ジエステル誘導体の使用。 3.一般式Iの重合した対称又は非対称型のメチレンマロン酸ジエステル誘導 体から成るガス含有粒子を含む、超音波診断用薬剤。 4.重合した非対称又は対称型のメチレンマロン酸エステルから成る粒子の製 造方法であって、メチレンマロン酸エステルモノマーを、ガスで飽和された水性 緩衝液中で、必要ならば1または複数の界面活性物質を含有させ、スターラーで 分散させ、続いて重合せしめた後、得られたガス含有粒子を分離し、必要ならば 水で洗浄し、次いで薬学的に許容される懸濁媒中に取り出し、そして凍結乾燥を 行うことを特徴とする方法。
