Claims (1)
- 【特許請求の範囲】 1.患者の身体の選択領域の管腔内治療を行なう装置において、 a)細長い可撓性カテーテルチューブを有し、該カテーテルチューブが、患 者の体外に留められる近位端部と、患者の体内の通路内の選択領域に管腔内配置 される遠位端部と、遠位端部および近位端部を備えかつ前記カテーテルチューブ の前記近位端部と前記遠位端部との間に延びている第1の管腔とを有し、 前記カテーテルチューブは管腔内導入が行なえる充分に小さな直径を有し、 b)前記近位端部で前記第1の管腔と連通しているポートと、 c)生体適合性液体を収容する源とを有し、該源が、加圧下の前記液体を前 記ポートに導入すべく前記ポートと連通しており、 d)前記第1の管腔内に配置可能な少なくとも1つの個別治療要素を有し、 前記第1の管腔の前記遠位端部は前記治療要素が外に出ることに対して閉じら れており、前記治療要素は、前記第1の管腔を通って流れる前記液体が発生する 駆動力により、前記第1の管腔の前記近位端部から前記管腔を通って遠位端部に 移動でき、 e)前記カテーテルチューブは、患者の体内の選択領域に遠位端部を配置す るためのガイドワイヤに従う充分な可撓性を有し、前記カテーテルチューブは、 ガイドワイヤを受け入れて挿通させる第2の管腔を更に有することを特徴とする 装置。 2.前記第1の管腔は遠位端部が開放しており、前記装置は、前記第1の管腔の 遠位端部で前記第1の管腔内に延びている保持突出部を更に有し、該突出部は前 記第1の管腔内に前記治療要素を保持できるようになっていることを特徴とする 請求の範囲第1項に記載の装置。 3.前記細長いチューブは、近位端部および遠位端部を備えかつ前記カテーテル チューブの前記近位端部と前記遠位端部との間に延びている第3の管腔を有し、 該第3の管腔は前記遠位端部で前記第1の管腔と連通し、前記第1のおよび第2 の管腔の遠位端部が流体が外に出ることに対して閉じられており、これにより流 体が前記第1の管腔の前記近位端部と遠位端部との間で移動しかつ前記第 2の管腔に沿って戻り、これにより、前記治療要素が、前記第2の管腔内に導入 される液体の駆動力により、前記細長いチューブの前記遠位端部と前記近位端部 との間で移動できることを特徴とする請求の範囲第1項に記載の装置。 5.前記治療要素が放射線源であることを特徴とする請求の範囲第1項に記載の 装置。 6.前記細長いチューブの遠位端部に配置された互いに間隔を隔てた1対の放射 線不透過性マーカを更に有し、該マーカはこれらの間の有効治療領域を形成する ことを特徴とする請求の範囲第5項に記載の装置。 7.前記治療要素は放射性物質から出るベータ粒子からなることを特徴とする請 求の範囲第5項に記載の装置。 8.前記治療要素を前記第1の管腔内で位置決めするハウジング部材を更に有し 、該ハウジング部材が第1の端部と、第2の端部と、前記ハウジング部材内で前 記第1の端部から延びている第1のボアとを備え、 前記細長いチューブは前記ハウジング部材の前記第1の端部に取り付けられ 、前記第1の管腔の前記近位端部は前記第1の端部で前記第1のボアと連通して いることを特徴とする請求の範囲第1項に記載の装置。 9.前記ハウジング部材はゲートを更に有し、前記ゲートは、第1の位置と、前 記治療要素が前記第1の管腔内に放出される第2の位置との間で移動できできる ことを特徴とする請求の範囲第8項に記載の装置。 10.前記第1管腔に連結される流体ポンプを更に有し、該流体ポンプは前記第1 の管腔を通る流体の流量を制御できることを特徴とする請求の範囲第1項に記載 の装置。 11.人の身体の選択領域を治療する方法において、 a)細長い可撓性カテーテルチューブを設け、該カテーテルチューブが、患 者の体内の通路内の選択領域に配置される遠位端部と、患者の体外に留められる 近位端部と、前記近位端部と前記遠位端部との間に延びている第1の管腔と、生 体適合性液体を前記第1の管腔内に導入するための、前記近位端部で前記第1の 管腔と連通しているポートとを有し、 b)細長いカテーテルチューブの遠位端部を、患者の体内の通路内に導入し 、 c)前記遠位端部を前記通路内の選択位置に前進させ、 d)少なくとも1つの個別治療要素を、前記細長いカテーテルチューブの前 記近位端部において前記チューブの第1の管腔内に導入し、 e)生体適合性液体を前記第1の管腔を通して前記チューブの近位端部から 遠位端部に流すことにより、前記治療要素を前記第1の管腔を通して移動させ、 前記液体の流れは前記治療要素に作用する駆動力を発生して、前記治療要素を前 記近位端部から前記管腔を通して前記遠位端部に移動させ、 f)前記選択領域の治療を行なうのに充分な時間、前記治療要素を前記遠位 端部に保持できるようにし、 g)前記細長いカテーテルチューブを患者の身体から引き抜くことからなる ことを特徴とする方法。 12.前記細長いチューブが更に、該細長いチューブの前記近位端部と遠位端部と の間に延びた第2の管腔を有し、 a)近位端部および該近位端部とは反対側の遠位端部を備えた細長いガイド ワイヤを、前記第1のの細長いチューブを導入する前に、身体の選択領域内に導 入するステップを有し、これにより前記ガイドワイヤの前記遠位端部が選択領域 まで延び、 b)前記細長いチューブの前記第2の管腔を前記ガイドワイヤ上で前進させ て、前記細長いチューブの遠位端部を身体の選択領域まで移動させるステップを 更に有することを特徴とする請求の範囲第11項に記載の方法。 13.a)第1の端部および該第1の端部とは反対側の第2の端部を備えた細長い イドワイヤ上で前進させることにより前記細長いガイドワイヤを導入した後に、 患者の体内に挿入し、 c)前記ガイドワイヤを前記ガイドチューブの遠位端部から取り出し、 d)前記細長いチューブの遠位端部が体内で前記ガイドチューブの管腔内に 位置決めされるまで、前記細長いチューブを前記ガイドチューブの管腔内に前進 させるステップを更に有することを特徴とする請求の範囲第11項に記載の方法 。 14.前記治療要素が放射性物質であり、前記放射性治療要素が、身体の選択領域 を治療的に有効な放射線量に被曝させるのに充分な時間、前記細長いチューブの 第1の管腔の遠位端部に留まることができるようにするステップを有することを 特徴とする請求の範囲第11項に記載の方法。 15.前記治療的に有効な放射線量は、100〜10,000ラドであることを特徴とす る請求の範囲第14項に記載の方法。 16.前記細長いチューブを患者の身体から引き抜く前に、前記放射線源を前記細 長いチューブの遠位端部から回収するステップを更に有することを特徴とする請 求の範囲第11項に記載の方法。 17.患者の身体の選択領域の管腔内治療を行なう装置において、 近位端部と、該近位端部とは反対側の遠位端部と、これらの近位端部と遠位 端部との間で延びている管腔とを備えた細長いチューブと、 近位端部および該元側端部とは反対側の遠位端部を備えた膨張可能なバルー ンとを有し、該バルーンの前記端部が前記細長いチューブの遠位端部に取り付け られ、 前記バルーン内で前記細長いチューブの遠位端部に配置される治療要素を更 に有することを特徴とする装置。 18.前記治療要素は近位端部および遠位端部を有し、 前記放射性要素の遠位端部に隣接して配置された第1の放射線不透過性マー カと、 前記治療要素の前記近位端部に隣接して配置された第2の放射線不透過性マ ーカとを更に有することを特徴とする請求の範囲第17項に記載の装置。 19.前記細長いチューブが前記チューブの近位端部と遠位端部との間で延びてい る第2の管腔を有し、該第2の管腔は前記バルーンと連通していて、加圧流体 が第2の管腔を通って導入されると前記バルーンを膨張できることを特徴とする 請求の範囲第17項に記載の装置。 20.バルーン血管形成術を受けている冠動脈の選択領域を照射する方法において 、 細長い可撓性カテーテルチューブを設け、該カテーテルチューブが、遠位端 部と、近位端部と、これらの遠位端部と近位端部との間に延びている第1の管腔 と、液体を前記第1の管腔内に導入するための、前記近位端部で前記第1の管腔 と連通しているポートとを有し、 細長いカテーテルチューブの遠位端部を、患者の血管系内に導入しかつ前記 遠位端部を、バルーン血管形成術が行なわれている冠動脈の選択領域まで前進さ せ、 少なくとも1つの個別放射性治療要素を、前記細長いカテーテルチューブの 前記近位端部において前記第1の管腔内に導入し、 加圧液体を前記チューブの近位端部で前記第1の管腔内に導入して放射性治 療要素に作用する駆動力を発生させることにより、治療要素を前記細長いカテー テルチューブの前記近位端部から前記遠位端部に移動させ、 前記放射性治療要素が、冠動脈の選択領域を治療的に有効な放射線量に被曝 させるのに充分な時間、前記細長いカテーテルチューブの遠位端部に留まること ができるようにし、 放射性治療要素を冠動脈内の選択領域から取り出すことからなることを特徴 とする方法。 21.前記細長いカテーテルチューブが、前記近位端部と遠位端部との間で延びか つ前記遠位端部で前記第1の管腔と連通して放射性治療要素を遠位端部に移動さ せるのに使用された液体の戻り通路を形成する第2の管腔を有し、前記液体が第 2の管腔の近位端部から遠位端部へと、および前記第1の管腔の遠位端部から近 位端部へと流れ、放射性治療要素を細長いカテーテルチューブの近位端部に戻す 駆動力を発生するように、前記加圧液体により発生される駆動力の方向を反転さ せることにより、前記放射性治療要素が、冠動脈の選択領域に隣接する前記細長 いカテーテルの前記遠位端部から取り出されることを特徴とする請求の範囲第2 0項に記載の方法。 22.前記放射性治療要素が、ベータ線を発生する放射性物質要素からなることを 特徴とする請求の範囲第20項に記載の方法。 23.前記治療要素の別のキャリヤを更に有し、該キャリヤは前記ハウジングに連 結できるようになっていることを特徴とする請求の範囲第8項に記載の装置。 24.前記ハウジングは、治療要素を配給すべく液体と連通する配給通路と、ガイ ドワイヤを通すためのガイドワイヤ通路と、前記液体をハウジングを通して戻す ための第3の通路とを有することを特徴とする請求の範囲第23項に記載の装置 。 25.前記治療要素は、治療要素の列を形成すべく可撓的に直列に結合された複数 の治療要素からなることを特徴とする請求の範囲第1項に記載の装置。 26.前記第1の管腔の直径は、治療要素の直径の2倍より小さいことを特徴とす る請求の範囲第1項に記載の装置。 27.前記治療要素は、前記遠位端部に固定して配置されることを特徴とする請求 の範囲第17項に記載の装置。 28.前記チューブは、前記近位端部と遠位端部との間に延びている管腔を有し、 該管腔内で、前記治療要素が前記近位端部と遠位端部との間で移動できることを 特徴とする請求の範囲第17項に記載の装置。 29.前記細長い可撓性カテーテルチューブが更に、 加圧流体が導入されたときに膨張できるように、前記遠位端部で前記細長い 可撓性カテーテルチューブにより支持された可撓性バルーン膜を有することを特 徴とする請求の範囲第1項に記載の装置。 30.第2のの細長いカテーテルチューブを更に有し、該カテーテルチューブが、 近位端部と、 該近位端部とは反対側の遠位端部と、 前記遠位端部に支持された可撓性バルーン膜と、 前記近位端部と遠位端部との間に延びている第1の管腔とを備え、該第1の 管腔は前記バルーン膜と連通していて、加圧流体が第1の管腔を通って導入され るときに前記膜を膨張させ、 前記近位端部と遠位端部との間に延びている第2の管腔を更に備え、該第2 の管腔は細長い可撓性カテーテルチューブを受け入れることができるサイズを有 することを特徴とする請求の範囲第1項に記載の装置。 31.前記細長い可撓性カテーテルチューブが更に、該細長いチューブの前記遠位 端部に支持された可撓性バルーン膜を有し、 前記第1の管腔内に前記治療要素を導入するときまたは導入する前に、前記 膨張可能なバルーン膜を膨張させて血管系の選択領域を拡張するステップを更に 有することを特徴とする請求の範囲第11項に記載の方法。 32.a)第2のの細長いカテーテルチューブを設け、該カテーテルチューブが、 近位端部と、 該近位端部とは反対側の遠位端部と、 前記近位端部と遠位端部との間に延びている第1のおよび第2の管腔と、 前記遠位端部に支持された可撓性バルーン膜とを備え、 該バルーン膜は、前記第1の管腔と連通していて、加圧流体が第1の管腔を 通って導入されるときに膨張でき、 前記第2の管腔は前記細長い可撓性カテーテルチューブを受け入れることが できるサイズを有し、 b)前記細長い可撓性カテーテルチューブを前記第2のの細長いカテーテル チューブの前記第2の管腔を通して患者に導入する前に、第2のの細長いカテー テルチューブを経皮的に患者の血管系内に導入し、 c)前記第2のの細長いカテーテルチューブの前記遠位端部を血管系の選択 領域まで前進させ、 d)前記第2のの細長いカテーテルチューブの前記第1の管腔内に加圧流体 を導入して前記バルーン膜を膨張させることを特徴とする請求の範囲第11項に 記載の方法。 33.患者の身体の選択領域の管腔内治療を行なう装置において、 a)患者の体内の通路内の管腔内導入が行なえる充分に小さな直径の細長い 可撓性カテーテルチューブを有し、該カテーテルチューブが、 患者の体外に留められる近位端部と、 患者の通路内の選択領域に管腔内配置される遠位端部と、 前記近位端部と前記遠位端部との間に延びている第1および第2の平行管腔 とを備え、 前記第1の管腔は前記細長いカテーテルチューブの前記遠位端部で前記第2 の管腔と連通しており、 b)前記細長い可撓性カテーテルチューブの前記近位端部に連結される搬送 /装填部材と、 c)該搬送/装填部材により支持された配給ポンプと、 d)前記搬送/装填部材により支持された回収ポンプと、 e)少なくとも1つの治療要素を受け入れることができるサイズを有する、 前記搬送/装填部材内に形成された治療要素ボアとを有し、該ボアの一端は前記 カテーテルチューブの前記第1の管腔と連通していて、治療要素が前記治療要素 ボアから前記第1の管腔を通って移動でき、 f)前記搬送/装填部材内に形成された配給流体通路を有し、該配給流体通 路は、前記配給ポンプと前記本体部材の前記治療要素ボアとの間に連通しており 、 g)前記搬送/装填部材内に形成された回収流体通路を有し、該回収流体通 路は前記回収ポンプと第2の管腔との間に連通しており、 h)生体適合性液体を収容する源を更に有することを特徴とする装置。 34.前記搬送/装填部材は、 装填チャンバと、 1つ以上の治療要素を収容する別の装填インサートとを有し、該装填インサ ートが前記装填チャンバ内に受け入れられて、前記治療要素が前記治療要素ボア および前記配給流体通路と連通することを特徴とする請求の範囲第33項に記載 の装置。 35.前記細長い可撓性カテーテルチューブが、前記近位端部と遠位端部との間に 延びている第3の管腔を更に有し、前記搬送/装填部材が、該部材を貫通して延 びかつ前記第3の管腔と連通しているガイドワイヤ通路を有することを特徴とす る請求の範囲第33項に記載の装置。 36.前記装填インサートにより支持されるゲートを更に有し、該ゲートは、前記 治療要素が装填インサートから移動することを防止する第1の位置と、前記治療 要素が装填インサートから移動できるようにする第2の位置との間で移動できる ことを特徴とする請求の範囲第34項に記載の装置。 37.前記配給ポンプおよび回収ポンプの各々が、ピストンを収容するシリンダか らなることを特徴とする請求の範囲第33項に記載の装置。 38.前記配給ポンプおよび回収ポンプの各々がベローズからなることを特徴とす る請求の範囲第33項に記載の装置。 39.患者の血管系のアテローム性動脈硬化閉塞部位を治療する方法において、 a)細長いカテーテルチューブを設け、該カテーテルチューブが、 近位端部と、 該近位端部とは反対側の遠位端部と、 前記近位端部と遠位端部との間に延びている管腔と、 前記細長いカテーテルチューブの前記遠位端部に支持された可撓性バルーン 膜とを備え、該バルーン膜は前記管腔と連通していて、加圧流体が前記管腔を通 って導入されるときに膨張でき、 前記バルーン膜の下に配置された少なくとも1つの放射性治療要素とを更に 備え、 b)前記細長いカテーテルチューブの前記遠位端部を、経皮的に患者の血管 系内に導入し、 c)前記遠位端部を血管系内でアテローム性動脈硬化閉塞部位まで前進させ 、 d)前記管腔内に加圧流体を導入して前記バルーンを膨張させ、 e)アテローム性動脈硬化閉塞部位を治療的に有効な放射線量に被曝させる ことを特徴とする方法。 40.前記放射性治療要素が、ベータ線を発生する放射性物質からなることを特徴 とする請求の範囲第39項に記載の方法。 41.前記治療的に有効な放射線量は、100〜10,000ラドであることを特徴とす る請求の範囲第39項に記載の方法。 42.前記細長いカテーテルチューブが、前記近位端部と遠位端部との間に延びて いる第2の管腔を更に有し、 a)近位端部と、該近位端部とは反対側の遠位端部とを備えた細長いガイド ワイヤを設け、 b)前記細長いガイドワイヤの前記遠位端部を患者の血管系内に導入しかつ 前記細長いカテーテルチューブを患者内に導入すべく、前記ガイドワイヤの遠位 端部をアテローム性動脈硬化閉塞部位まで前進させ、 c)前記ガイドワイヤ上で前記細長いカテーテルチューブの遠位端部を患者 内に導入して、前記ガイドワイヤを前記細長いカテーテルチューブの前記第2の 管腔内に配置し、 d)前記膨張可能なバルーンを膨張させる前に、前記ガイドワイヤ上で前記 細長いカテーテルチューブの前記遠位端部を、アテローム性動脈硬化閉塞部位ま で前進させることを特徴とする請求の範囲第39項に記載の方法。
