Claims (1)
- 【特許請求の範囲】 1. ドリルガイドを、大腿骨や脛骨などの長手破断骨内に設置された髄内爪部 材の単数または複数の横骨−ネジ孔と機械的に心合わせするためのドリル冶具で あって、前記髄内爪部材が、少なくとも1つの直線遠方延出部と、冶具取り付け 用の近端部と、前記直線遠方延出部に形成された単数または複数の骨−ネジ孔と を有し、前記単数または複数の孔と前記爪部材の軸芯とが第1幾何学対称平面を 形成し、前記冶具が、 長手直線状ガイド・バーと、 前記爪部材の前記近端部と選択的にキー接続してハンドルが前記爪部材の 横方向に延出する手段と、更に、前記ガイド・バーを前記爪部材に対して平行に 案内係止するための手段とを有する固定ハンドルと、 前記単数または複数の骨用ネジ孔とほぼ心合わせされた単数または複数の ドリル−ガイド孔とを有していて、 前記ドリル冶具は、前記ガイド・バーに取り外し可能に取り付けられると ともに、前記第1幾何学平面の側方外方に延出するアウトリガ構造体を有してい て、このアウトリガ構造体が、前記爪部材の軸芯を含むとともに前記第1幾何学 平面に対して垂直な第2幾何学 平面においてガイド孔を備えた端部を有し、前記冶具が、更に、前記アウトリガ 構造体の前記ガイド孔に対して、前記ガイド・バーの前記単数または複数のドリ ル・ガイド孔が前記爪部材の単数または複数の孔に対して正しく心合わせされた ときに、前記爪部材に接触するため有効突出長さを備えた前記アウトリガ構造体 の前記ガイド孔に取り付けるように構成されたスペーサ・ロッドとを有している 。 2. 請求項1のドリル冶具であって、前記ハンドルと前記ガイドバーとは、前 記第1幾何学対称平面内に延出して、ドリルガイドを、骨折した大腿骨内に設置 された髄内爪部材の単数または複数の横骨−ネジ孔と心合わせさせる。 3. 請求項1のドリル冶具であって、前記ハンドルと前記ガイドバーとは、前 記第2幾何学対称平面内に延出して、ドリルガイドを、骨折した脛骨内に設置さ れた髄内爪部材の単数または複数の横骨−ネジ孔と心合わせさせる。 4. 請求項1のドリル冶具であって、前記爪部材は、断面形状が円形であると ともに、既知の半径を有し、更に、前記スペーサロッドの前記ガイドバーに対す る取り付けが、前記爪部材の半径に応じて前記スペーサ の有効突出長を決定するための手段を有している。 5. 請求項2及び4に記載のドリル冶具であって、前記スペーサロッドの有効 突出長を決定するための手段が、前記ロッド上に形成された肩部と、前記アウト リガ構造体と前記肩部とに対して特定のオフセットを有した状態で選択的に係合 可能なスペーサとを有し、前記スペーサが、使用可能な種々の爪部材の種々の断 面半径に関連付けられた種々のオフセットを提供する複数のスペーサの1つであ る。 6. 請求項5のドリル冶具であって、前記スペーサが、前記アウトリガ構造体 に取り外し可能に係合するとともに、この構造体に対する位置決めを行うための 第1手段を備えた本体部を有し、この本体部が、前記肩部とともに、かつ、この 肩部に対して、オフセットを有した状態で前記第1解除可能係合手段に向けて延 出している。 7. 請求項6のドリル冶具であって、前記スペーサと、前記第1及び第2取り 外し可能係合手段とが、前記本体部に一体的に形成されている。 8. 請求項6のドリル冶具であって、前記肩部が、前記第2取り外し可能係合 手段との係合用の周部連続溝の一部である。 9. 請求項3及び4のドリル冶具であって、前記スペーサロッドの有効突出長 を決定するための前記手段が、前記第2幾何学対称平面内において前記スペーサ ロッドを挿入方向付けするためのガイド孔を有し、前記ロッドが、前記爪部材に 向かう前記ロッドの突出量を制限するための肩部を有し、更に前記手段が、前記 ガイドバーに対して選択的に適用可能で、かつ、前記肩部によって係合可能なシ ムを有し、このシムが、使用可能な複数の爪部材の種々の断面半径に関連づけら れた種々の厚みを有する複数のシムの内の1つである。 10.請求項1のドリル冶具であって、前記ガイドバーは、一定でかつ非円形の 断面を有し、更に、2つの長手方向に互いに離間した平行なガイドピン孔を有す る。 11.請求項10のドリル冶具であって、前記アウトリガ構造体は、このアウト リガ構造体を前記ガイドバーに対して、前記ガイドピン孔によって、組み付け方 向づけするための2つの固定平行ガイドピンを有する。 12.請求項1のドリル冶具であって、前記ガイドバーは、種々の髄内爪部材寸 法に関連づけられた特定の長手方向位置において前記ハンドルに調節可能に固定 可能で、ある特定の爪部材−長さ寸法に対して適正な位置を選択することによっ て、前記アウトリガ構造体の 前記ガイドバーに対する取り付けによって、前記髄内爪部材の前記単数または複 数の骨−ネジ孔と心合わせされた前記単数または複数のドリル−ガイド孔の正し い位置が保証される。 13.請求項1のドリル冶具であって、前記アウトリガ構造体は、このアウトリ ガ構造体を、前記爪部材の前記軸芯を含む前記幾何学対称平面のいずれかの側に おいて、前記ガイドバーに対して選択的かつ可逆的に固定可能な状態で、前記ア ウトリガ構造体を前記ガイドバーに固定するための解除可能手段を有している。 14.請求項3のドリル冶具であって、前記アウトリガ構造体は、前記ガイドバ ーと、更に、前記第1幾何学対称平面とに対して単一の側方下方オフセットを備 えて延出し、これによって、前記アウトリガ構造体の前記ドリル−ガイド孔が、 前記第1幾何学平面に対して垂直である。 15.請求項1のドリル冶具であって、前記爪部材は、既知の半径を有する円形 断面形状であり、更に、前記ガイドバーは、前記第2幾何学対称平面に前記スペ ーサロッドを挿入方向づけするためのガイド孔を有し、前記スペーサロッドが、 2つの離間した周溝と、前記ガイドバーの片側にフィットするべく構成されたU 字 状係合部とを備え、前記U字状係合部の対向する両脚部が、前記ガイドバーの断 面の上方および下方端部を取り囲み、前記両脚部が、前記スペーサロッドの離間 溝と係合してこれによって前記スペーサロッドの前記髄内爪部材に対する近接移 動を制限する互いに離間したフォーク部材を形成するべく溝形成されている。 16.請求項15のドリル冶具であって、前記U字状係合部の前記両脚部の一方 の厚みが、前記髄内爪部材の前記既知半径に対して、前記スペーサロッドが前記 髄内爪部材と接触係合するべく軸芯方向に付勢されたときに、その接触によって 、前記単数または複数のドリル−ガイド孔と、前記髄内爪部材の単数または複数 の骨−ネジ孔との正しい心合わせが判定されるような特定の関係を有する。
