Abstract

(57)【要約】 【課題】本発明は手動で制御可能で片手で操作し得る内
視鏡用外科器具を提供する。 【解決手段】基端にハンドルを有し、先端に末端作動器
を取着したチューブ体を具備してなる。連結部材がチュ
ーブ体を通って延び上記ハンドルと末端作動器とを連結
している。ハンドルはチューブ体の長手軸と整合し中指
を受ける固定指用ループを含む。ハンドルは親指用ルー
プレバー、フィンガーホイル制御部、回転制御部を含
む。親指用ループレバーは、上記指用ループと同じ親指
で操作可能であり、末端作動器を操作する。フィンガー
ホイル制御部は末端作動器を回動させる。回転制御部は
同じ人差し指で操作されチューブ体と末端作動器とを自
転させる。その他、電気メス、合体内視鏡、灌注、吸引

【発明の詳細な説明】
【０００１】
【発明が属する技術分野】本発明は、内視鏡用外科器具
に関する。
【０００２】
【従来の技術】内視鏡検査（例えば、腹腔鏡検査、胸部
鏡検査、関節鏡検査）は光学照明器具、すなわち内視鏡
を用いて体内を可視化することを伴う外科療法の一つの
形態である。この場合、内視鏡および他の外科器具が小
さな穿刺孔を介して体内に導入される。腹腔鏡検査にお
ける内視鏡はもっぱら婦人科外科療法に用いられてき
た。しかし、他の分野の専門医も内視鏡の診断、手術上
の価値を認識するようになった。内視鏡外科療法の利点
は、手法が外来患者ベース、すなわち入院を必要とせず
に行われること；外科医は腹壁切開、大きな腹壁切開を
行うことなしに腹腔内部の内臓器官を観察することがで
きること；大きな切開でなく小さな穿刺孔あるいは傷を
つけるだけで済み、外傷を小さくすることができるこ
と；患者および保険会社に対する医療費が入院の短縮に
より軽減されること；術後の患者の苦痛が軽減するとと
もに回復時間を減少させることができることなどであ
る。内視鏡による手法は通常、套管針と呼ばれる器具を
用いて開始される。この套管針はカニューレあるいはト
ロカール・スリーブ（中央ルーメンを有する中空シース
あるいはスリーブ）と、カニューレに挿入された鋭い閉
塞具とからなっている。この套管針は腹壁あるいは胸壁
を貫通させるのに用いられる。閉塞具は套管針の先端部
が腹腔内の所定部位まで挿入された後、カニューレから
引抜かれ、カニューレは外科手法が施されている間は腹
壁を貫通したままの状態に保たれ、このカニューレを介
して内視鏡や外科用器具が導入されるようになってい
る。套管針としては、カニューレと同様に、種々のサイ
ズのものがあり、種々のサイズ、数の外科用器具に対し
適用し得るようになっている。内視鏡手法のために特別
に設計された外科用器具は特殊な道具（以降、末端作動
器と呼ぶ）が細長いチューブ体の先端に取着されてい
る。また、ハンドルが細長いチューブ体の基端に取着さ
れていて、その制御手段を介して末端作動器を操作し得
るようになっており、さらに末端作動器の軸を回転させ
る第２の制御手段を有している場合もある。つまり、連
結部材がハンドル制御部からチューブ体を通って末端作
動器まで延びている。一般に小さな穿孔あるいは切開部
に適合させるために、この外科器具は単一の特別なデリ
ケートな目的を果たすように設計されている。理想的に
は外科医は好みおよびその時々の手法に応じて最適な器
具を選ぶことである。しかし、実際には別の器具を用意
するための費用、１つを取出して別のものを再度導入す
る手間などのため、外科医はより適当な別の器具の使用
が好ましいにも拘らず最初の器具で間に合わす傾向にあ
る。したがって、単一の器具が種々の作業をこなすこと
ができれば、好ましいことである。さらに、そのような
多機能器具が使用、制御が容易で、かつ一方の手で操作
できればより好ましいものとなる。

【発明が解決しようとする課題】したがって、本発明は
１つの目的は、外科医の能力、機敏性を向上させること
ができ、かつ、内視鏡ポートの数が少なくて済む内視鏡
用外科器具を提供することにある。本発明は他の目的
は、末端作動器の連結、回転および操作を正確、かつ直
観的に片手制御可能な改良された外科器具を提供するこ
とにある。本発明は他の目的は、直径が２〜３ミリで、
種々のサイズの套管針スリーブあるいは内視鏡ポートを
通過させることができ、生体組織の損傷を小さくて済む
ことができる小型の内視鏡器具を提供することにある。
本発明はさらに他の目的は、種々の作業を行うことがで
きる多機能の器具を提供することにある。電気焼灼、吸
引／灌注、可視化ポート、トリガーロックなどの付加的
機能により、外科医がより少ない器具でより早く作業を
行うことを可能にする。
【０００４】
【課題を解決するための手段】本発明は、概略的に述べ
れば、種々の外科的作業を行うことができ、手動制御を
介して真の片手操作が可能な内視鏡用外科器具を提供す
るものである。すなわち、本発明の外科器具は、基端に
ハンドルを有し、先端に末端作動器を取着したチューブ
体を具備してなる。また、連結部材が上記チューブ体を
通って延び上記ハンドルと末端作動器とを連結してい
る。上記末端作動器は２つの作動片からなっている。双
方の作動片はそれぞれチューブ体の先端に直接、枢着さ
れている。上記ハンドルはチューブ体の長手軸と整合し
て固定された固定部を含む。好ましい例としては、この
ハンドルは少なくとも中指（すなわち、多分人差し指も
一緒に）が入る固定指用ループからなるグリップ部を有
する。この指用ループはハンドルの固定部に設けられチ
ューブ体の固定部との関連において動くことはない（す
なわち、一体的である）。このハンドルは上記固定部に
取着された親指用ループレバーを含む。この親指用ルー
プレバーは、上記指用ループを握る同じ手の親指で操作
可能なように形づけられている。この親指用ループレバ
ーを親指で第１の方向に動かすと末端作動器の２つの作
動片間の角度が増大し、逆の方向に親指用ループレバー
を動かすとこの角度が減少する。すなわち、親指用ルー
プレバーの往復動により２つの作動片がハサミのように
近づいたり離れたりする動作が交互に生じる。この器具
のハンドルは親指用ループレバーをハンドルとの関係で
固定位置にロックする制御部を有する。すなわち、この
ロッキング制御部は末端作動器の２つの作動片間の角度
がそれ以上増大するのを防止する（例えば、末端作動器
が組織を掴んだ状態を維持するため）。好ましい形態と
して、このロッキング制御部はハンドルを握る同じ手
（好ましくはその手の人差し指）で操作されるロック・
アンド・リリース・レバーである。
【０００５】このハンドルはさらにフィンガーホイル・
ローラを含む。このフィンガーホイル・ローラはハンド
ルの固定部に設けられる。すなわち、このフィンガーホ
イル・ローラの位置は上記指用ループを介してこの器具
を握る同じ手の人差し指により回転可能なように設けら
れている。人差し指によりこのフィンガーホイル・ロー
ラを第１の方向に回転させると２つの作動片が同時にチ
ューブ体の長手軸との整合から外れる方向に回動する
（傾く）。また、このフィンガーホイル・ローラを第１
の方向と逆の方向に回転させると２つの作動片が同時に
チューブ体の長手軸と整合する方向に回動する。すなわ
ち、外科医はフィンガーホイル・ローラを回転させるこ
とにより２つの作動片を１１０度の円弧の範囲で回動さ
せることができ、これにより末端作動器を外科治療部に
正確に位置させることができる。好ましい形態におい
て、上記ハンドルはさらに回転ノブを含む。この回転ノ
ブはチューブ体の基端の周りの配置される。この回転ノ
ブはフィンガーホイル・ローラを回転させる同じ人差し
指で回転可能となっている。この回転ノブはチューブ体
に取付けられ、回転ノブを回転させると、チューブ体お
よびそれに固定された末端作動器がチューブ体の軸を中
心として回転するようになっている。したがって、本発
明の上記構成からなる外科器具は片手操作により末端作
動器を３つの動作（すなわちハサミ、回動、自転）をさ
せることができる。親指用ループの移動による末端作動
器のハサミ動作およびフィンガーホイル・ローラの回転
による末端作動器の回動は、末端作動器に対する連結部
の相互作用により行うことができる。つぎに、この連結
部の相互作用について説明する。好ましい形態におい
て、この外科器具の連結部は第１および第２の長尺部材
を含む。この第１および第２の長尺部材は第１および第
２の作動片にそれぞれ連結されている。親指用ループレ
バーの動作は第１の長尺部材を静止した第２の長尺部
材、ハンドルおよびチューブ体との関連において移動さ
せる。この相関移動により第１の作動片を第２の作動片
との関連で回動させることができ、これにより末端作動
器の２つの作動片間の角度を変化させることができる。
すなわち、親指用ループレバーの往復動により末端作動
器の２つの作動片間のハサミ作用がもたらされる。
【０００６】さらに、フィンガーホイル・ローラの回転
によりチューブ体がハンドルの固定部材との関係で移動
し、２つの長尺部材はハンドルの固定部材との関係で静
止したままの状態に保たれる。このチューブ体の双方の
長尺部材に対する相関移動により末端作動器が同時に同
一方向に回動する。フィンガーホイル・ローラの回転が
２つの長尺部材間の実質的相関移動を生じさせないた
め、２つの作動片間の角度はこの回動においても実質的
に一定に保たれる。このように回動において２つの作動
片間の角度が実質的に一定に保たれるということは、外
科医にとって非常に有利である。なぜならば、これによ
り回動において末端作動器の予定部位への正確な位置決
めが可能となるからである。この正確な位置決めは特に
末端作動器が対象物あるいは組織を掴みながら回動を行
う場合に有利となる。フィンガーホイル・ローラは応力
倍増機構を介してチューブ体に連結している。好ましい
形態として、この応力倍増機構は、チューブ体基端周壁
面を構成するハブのラック部と歯合する一連のギア部を
含む。この応力倍増機構の採用は本発明において特に有
利である。なぜならば、末端作動器の回動位置の微妙か
つ正確な制御が可能となり、かつ、フィンガーホイル・
ローラに対し比較的軽い指圧で操作が可能となるからで
ある。
【０００７】上述の特徴、機能の他に、本発明の器具は
以下の機能を含むように形成することができる。すなわ
ち、（ｉ）電気焼灼機能；（ii）集積内視鏡；(iii) 吸
引／灌注機能である。電気焼灼機能に関し、好ましい形
態として、ハンドルには電気外科発電機などの電源と外
科器具とを接続するプラグが設けられる。このプラグか
らチューブ体に沿って末端作動器まで延びた通電路が形
成され、これにより電流を末端作動器を介して目的部位
に与えるようにする。集積内視鏡の形態としては、チュ
ーブ体に別のルーメンをチューブ体の全長に沿って設
け、このルーメンに内視鏡を収容するようにしてもよ
い。このルーメンの入口としては基端ポートを設け、ル
ーメンの出口としては先端ポートを設ける。この先端ポ
ートは末端作動器の近傍に設け、かつ、傾斜させ、内視
鏡がこの先端ポートから突出させ、末端作動器全体が内
視鏡の視野にあるようにしてもよい。集積内視鏡の他の
形態として、末端作動器がルーメン内に挿入された内視
鏡の先端部を掴み操作し得るように末端作動器を設計し
てもよい。例えば、末端作動器に、ルーメンの先端ポー
トと整合し得るループ、その他の器具からなる片を設け
てもよい。すなわち、内視鏡をルーメンを通して押込ん
だとき、それが上記ループにより案内されるようにす
る。ついで、末端作動器をハンドルの制御装置を介して
操作して内視鏡の方向を適宜変更させるようにする。し
たがって、外科医が望む任意の角度からも目的部位を観
察し得るよう内視鏡を位置決めするのに末端作動器を使
用することができる。
【０００８】最後に、本発明で吸引／灌注機能を設けた
形態として、チューブ体の全長に沿って複数以上のルー
メンを設けるようにしてもよい。この多重ルーメンを採
用することにより、ルーメンの１またはそれ以上を目的
部位へ灌注流体を流すのに用い、ルーメンの１またはそ
れ以上を目的部位から吸入流体を吸引するのに用いるこ
とができる。

【発明の実施の形態】以下に、本発明のを図示の実施例
を参照して説明する。ここで、図１は本発明に係わる内
視鏡用外科器具１を示している。内視鏡用外科器具１は
長尺のチューブ体１０と；チューブ体１０の基端に取着
されたハンドル２と；チューブ体１０の先端に取着され
た末端作動器３とを具備してなる。好ましい形態とし
て、ハンドル２はプラスチック成型品からなり、チュー
ブ体１０は繊維強化プラスチックから作られる。もちろ
ん、ハンドル２およびチューブ体１０を他の種々の材料
から作ることもできる。末端作動器３は好ましくはステ
ンレス鋼から作られたものであり外科療法を行うために
有用な種々の器具の１例を示している。図１において末
端作動器３はハサミとして表されている。この外科器具
１において適用可能な他の末端作動器としては、解剖ジ
ョー、グラスパー、イクスラクター、クランプ、鉗子、
開創器、バイオプシー器具、その他外科手術の補助をな
す同様の器具などがある。図１の末端作動器３は２つの
別々の末端作動片４０、４４からなっている。これら末
端作動片４０、４４はチューブ体１０の先端にピン４８
を介して取着されている。すなわち、このピン４８はチ
ューブ体１０の長手軸に対し垂直に貫通するとともに、
末端作動片４０、４４の各基端部の孔（図１に示してい
ない）を貫通するようにして設けられている。したがっ
て、末端作動片４０、４４はそれぞれ独立してピン４８
の中心として回動し得るようになっている。この回動は
矢線Ｂ２で示すようにチューブ体１０の長手軸に対し離
れる方向および近付く方向の双方である。ハンドル２に
関連して、種々の外科療法を行うための末端作動器３を
操作するための制御部材が設けられている。第１に、末
端作動器３のハサミ作用を行うため、ハンドル２にトリ
ガーレバー４が設けられている。トリガーレバー４はハ
ンドル２の固定部材５にピン７を介して枢着されてい
る。矢線Ａ１で示すトリガーレバー４のいずれの方向へ
の回動も矢線Ａ２で示すような末端作動片４４の回動を
生じさせる。他方、トリガーレバー４の回動は末端作動
片４０を何ら回動させない。したがって、トリガーレバ
ー４を動かすことにより、末端作動片４４が他の静止し
た末端作動片４０に対し近付いたり、離れたりして、２
つの末端作動片４０、４４の間の操作角度θを変化させ
る。
【００１０】フィンガーホイル・ローラ８が末端作動片
４０、４４の双方を矢線Ｂ２で示すように同一方向に同
時に回動させるためにハンドル２に設けられている。こ
のフィンガーホイル・ローラ８はハンドル２の固定部材
５に取着され矢線Ｂ１で示すようにフィンガーホイル・
ローラ８の軸心を中心として双方向に回転可能となって
いる。外科器具１を上から見たとき、フィンガーホイル
・ローラ８の時計方向の回転は２つの末端作動片４０、
４４の双方をチューブ体１０の長手軸と整合する方向に
同時に回動させる（つまり、長手方向に延びた位置）。
同様に、フィンガーホイル・ローラ８を反時計方向に回
転させたときは、末端作動片４０、４４の双方はチュー
ブ体１０の長手軸から離れる方向に同時に回動する。フ
ィンガーホイル・ローラ８を回転させる間においてトリ
ガーレバー４を回動させないとすると、２つの末端作動
片４０、４４の間の操作角度θは実質的に一定に保たれ
たまま２つの末端作動片４０、４４が同時に同一方向に
回動することになる。２つの末端作動片４０、４４の間
の操作角度θをロックするための機構として、ハンドル
２にロック・アンド・リリース・レバー１２が設けられ
ている。このレバー１２はハンドル２の固定部材５に対
し回動自在に枢着されている。図１においてレバー１２
はロックされた状態が示されているが、これは矢線Ｄで
示すように解放位置（図示しない）に回動させることが
できる。ロックされた位置において、ラチェット・バー
１６によりトリガーレバー４がさらに開く方向に動くの
を防止し、これにより２つの末端作動片４０、４４の間
の開き動作が防止される。レバー１２を矢線Ｄで示す方
向に動かし、解放位置（図示しない）に移動させるとラ
チェット・バー１６がトリガーレバー４を解放させ、以
下に詳述するように２つの末端作動片４０、４４の間を
自由に開かせることができる。なお、外科医がレバー１
２をロック位置に移動させない限り、レバー１２は解放
位置にとどまる。
【００１１】ハンドル２にはさらにチューブ体１０全体
（したがって、取着されている末端作動器３も含めて）
を矢線Ｃ２で示すように自転させるための回転ノブ６が
設けられている。この回転ノブ６は矢線Ｃ１で示すよう
にハンドル２の固定部材５との関連で回転するようにな
っている。チューブ体１０は回転ノブ６に固着されてお
り、したがって、これら回転ノブ６およびチューブ体１
０は一体となって回転する。ハンドル２は外科医がいず
れの手でも外科器具を掴むことができるように設計され
ている。トリガーレバー４の親指用ループ９は外科医の
親指を受入れるようにデザインされており、固定部材５
に設けられた指用ループ１１は外科医の中指および薬指
を受入れるようにデザインされている。これにより外科
医の人差し指は自由となり、回転ノブ６、フィンガーホ
イル・ローラ８、ロック・アンド・リリース・レバー１
２を自由に操作することができる。この外科器具１、特
にハンドル２のデザインは、実際に片手操作を可能とし
ている。さらに、ハンドル２の固定部材５はチューブ体
１０の先端との位置関係で固定された状態に保たれるか
ら、この外科器具１は外科医にとって末端作動器３を極
めて正確に位置決めしたり、移動させたりすることを容
易にしている。
【００１２】図２〜５は末端作動器３の動作機構の先端
部を示している。末端作動片４０、４４はチューブ体１
０の先端に枢着されている。末端作動片４０、４４が回
動していないとき（すなわち、末端作動片４０、４４が
長手方向に延びている位置）、末端作動片４０、４４は
チューブ体１０の規定された直径の外側には突出してい
ない。好ましい形態として、このチューブ体１０の規定
された直径は約５ｍｍであり、外科器具１が、５ｍｍ程
の細い套管針スリーブ（図示しない）や同程度の細い腹
腔鏡用ポート（図示しない）を容易に通過させ得る程度
のものである。しかし、他の具体例として、このチュー
ブ体１０の規定された直径は２ないし３ｍｍ程度であっ
てもよい。図２に示すノッチ１５はチューブ体１０の１
側に設けられ、末端作動片４０、４４は長手方向に延び
た位置から離れて回動されている。図示の例ではチュー
ブ体１０の長手方向から約１１０度回動させることがで
きる。図４では末端作動片４４がチューブ体１０の長手
方向に延びた位置から約１１０度回動している状態を示
している。このように末端作動片４４が１１０度回動し
ていると、末端作動片４４がノッチ１５の近接のエッジ
部１９に当接してそれ以上は回動しないようになってい
る。各末端作動片４０、４４の基端周面には一連の歯４
２、４６が形成されている。末端作動片４０の歯４２は
図４、５に示されており、末端作動片４４の歯４６は図
３、５に示されている。チューブ体１０内部には第１の
長尺部材３２（図３、５）および第２の長尺部材３４
（図４、５）が摺動自在に挿通されている。これら長尺
部材３２、３４は好ましくは高強度ステンレス合金から
作られチャンネル１０６内に押し込められている（図
５）。このチャンネル１０６の寸法は長尺部材３２、３
４に対し摩擦を生じさせないような十分な大きさとする
と同時に、長尺部材３２、３４が横方向に捩じれが生じ
ない程度の十分に小さな大きさとする。末端作動片４
０、４４と同様に長尺部材３２、３４も別々で互いに独
立して作動する（図５参照）。
【００１３】図３、４を参照してさらに説明すると、各
長尺部材３２、３４はその先端にラック３３、３５がそ
れぞれ形成されている。つまり、長尺部材３２に対する
ラック３３は図３に、また、長尺部材３４に対するラッ
ク３５は図４に示されている。長尺部材３２、３４先端
のラック３３、３５は外科療法の間に生体組織と絡まら
ないようにチューブ体１０先端の角型突出部２３により
囲まれている。長尺部材３２のラック３３は末端作動片
４４の歯４６と歯合し（図３）、長尺部材３４のラック
３５は末端作動片４０の歯４２と歯合している（図
４）。長尺部材３２、３４、特にラック３３、３５は、
長尺部材３２、３４が支えられているチューブ体１０の
背面部２７（すなわち、チャンネル１０６の内壁面）に
より末端作動片４０、４４の歯４２、４６との係合が保
たれている。長尺部材３２、３４とチューブ体１０との
相関的動作により、それぞれ係合した末端作動片４０、
４４の回動が行われる。図３に示すように、ハサミ作用
は長尺部材３２がチューブ体１０内部で移動したときに
奏せられる。つまり、この移動により末端作動片４４が
回動させられる。例えば、長尺部材３２が矢線Ａ３に示
すように基端方向に移動したとき、末端作動片４４が矢
線Ａ２に示すように長手方向延長位置に向けて回動す
る。長尺部材３２が逆に先端方向に移動したとき、末端
作動片４４は逆の方向に回動する。後述のように、外科
医はトリガーレバー４（図１）を操作することにより長
尺部材３２を移動させることができる。
【００１４】図４に示すように、末端作動片４０、４４
の同方向への同時回動はチューブ体１０を長尺部材３
２、３４との関係で移動させることにより行われる。す
なわち、チューブ体１０をハンドル２（図４に示されて
いない）に対し引いたり、押したりし、その間、長尺部
材３２、３４をハンドル２に対し静止した状態に保つこ
とにより行われる。チューブ体１０を矢線Ｂ３に示すよ
うに基端方向に移動させると末端作動片４０、４４は矢
線Ｂ２に示すように回動する。逆にチューブ体１０を先
端方向に移動させると末端作動片４０、４４は逆方向に
回動する。後述するように外科医はフィンガーホイル・
ローラ８（図１）を操作することによりチューブ体１０
を移動させ、これにより末端作動片４０、４４の双方を
同時に回動させることができる。
【００１５】図６、７はトリガーレバー４により長尺部
材３２をチューブ体１０内で移動させ末端作動片４０、
４４のハサミ作用を生じさせる機構について説明するた
めのものである。図６に示すように、長尺部材３２の基
端はボール２０およびソケット２３を介してトリガーレ
バー４に固定されている。図７に示すように、長尺部材
３２はボール２０の孔８０を通って延び固定ネジ２１に
よりボール２０に固定されている。さらに図６に示すよ
うに、外科医がハンドル２を締付けトリガーレバー４を
矢線Ａ１に示すように回動中心１８を中心として回動さ
せると、長尺部材３２が基端方向に向けて引かれる。こ
のボール２０およびソケット２３の組合せにより長尺部
材３２がチューブ体１０内で回転することができ（これ
については後に詳述する）、長尺部材３２に対する応力
が長尺部材３２とトリガーレバー４との固定部近傍にて
解放される。次に、図６、７、図８を参照して、フィン
ガーホイル・ローラ８の回転により末端作動片４０、４
４の双方を同時に回動させる機構について説明する。な
お、図７においては、説明上、チューブ体１０、トリガ
ーレバー４および固定部材５は省略されている。まず、
ハブ２６が接着剤または機械的締付け部材を介してチュ
ーブ体１０の外側に固定されている。したがって、ハブ
２６とチューブ体１０は一体となって移動する。フィン
ガーホイル・ローラ８は図６に示すように軸３７に設け
られている。第１のピニオン３８も軸３７に設けられ、
フィンガーホイル・ローラ８とともに回転する。第１の
ピニオン３８は歯車２２と歯合し（図８）、第１のピニ
オン３８と歯車２２とは互いに反対方向に回転する。歯
車２２は第２のピニオン２４（図８参照）を有する第２
の軸３９に設けられ、したがって歯車２２が回転すると
第２のピニオン２４も回転することになる。この第２の
ピニオン２４はハブ２６の基端部に形成されたラック２
５と歯合している。したがって、第２のピニオン２４が
回転するとハブ２６およびチューブ体１０がそれぞれ軸
方向に向けて移動することになる。
【００１６】さらに図８を参照して説明すると、フィン
ガーホイル・ローラ８、したがって第１のピニオン３８
が矢線Ｂ１で示す反時計方向に回転すると、歯車２２お
よび第２のピニオン２４が時計方向に回転することにな
る。この第２のピニオン２４の時計方向の回転によりハ
ブ２６が引かれ、したがって矢線Ｂ３に示すようにチュ
ーブ体１０が基端方向に引かれる。上述のように、長尺
部材３２、３４がハンドル２との関連で静止したままの
状態でのチューブ体１０の基端方向への移動により、末
端作動片４０、４４の双方が同時に長手方向延長位置か
ら回動する。上記ピニオン３８、２４および歯車２２は
応力増幅器として働き、フィンガーホイル・ローラ８に
比較的軽く指圧を加えるだけでチューブ体１０を移動さ
せることができる。もちろん、他の応力増幅機構を利用
することも可能である。例えば、一連のレバー、リンク
を利用したり、偏心カム、クランクシャフトを利用する
こともできる。末端作動片４０、４４の双方を回動させ
る間において、触覚的フィードバックが外科医に与えら
れる。これはフィンガーホイル・ローラ８の上面に一連
の回り止め８１（すなわち、円筒状溝、図６に示されて
いない）を設けることにより行われる。この場合、図９
に示すようにバネ５１が回り止め８１中に圧入される。
この回り止め８１は末端作動片４０、４４がどの程度回
動されたかを触覚的に外科医に伝えるだけでなく、末端
作動片４０、４４の位置を安定化させる作用をも有す
る。すなわち、回り止め８１は末端作動片４０、４４に
対し外力が加えられフィンガーホイル・ローラ８を反転
駆動（すなわち、意図する方向と逆の方向に回転させる
応力）させるような場合でも、このような末端作動片４
０、４４に対する圧力に抵抗する作用を生じさせる。こ
れは特に作動刃が１１０度を超えて回動したり、小さな
開創器として使用される場合に有用となる。
【００１７】次に、図６、７を参照して、回転ノブ６に
より末端作動器をチューブ体１０の長手軸を中心として
回動させる機構について説明する。図６に示すようにハ
ンドル２はキャビティ３３を有し、これに回転ノブ６の
カラー部３０が回転自在に軸支されている。このカラー
部３０の内部にはピン２８が、ハブ２６およびチューブ
体１０を貫通して設けられたスロット３１を貫通するよ
うにして延びている。回転ノブ６を回転させると、ピン
２８を介してチューブ体１０、したがって末端作動器３
を図１中矢線Ｃ２で示す方向に回転させる。ピン２８は
スロット３１中を摺動自在に移動可能であるため、ハブ
２６およびチューブ体１０はピン２８に邪魔されること
なく移動することができる。さらに、ハブ２６上の円筒
状ラック２５はハブ２６の軸心を中心として円形で対称
的に（螺旋状ネジでなく）カットされているから、チュ
ーブ体１０は回転してもチューブ体１０は移動しない。
すなわち、チューブ体１０の回転により末端作動器３は
回転するが末端作動器３が回動（傾斜）することはな
い。図７を参照して説明すると、ピン２８の回転により
（すなわち、回転ノブ６の回転により、図６参照）、長
尺部材３２、３４はチューブ体１０と共に回転すること
になる。長尺部材３２はピン２８の一側に配置されボー
ル２０に連結されている。このボール２０はソケット２
３内にて回転自在となっているため、ピン２８が回転す
ると、長尺部材３２もチューブ体１０内で回転させられ
る（図６参照）。長尺部材３４のＵ字型屈曲部３６は長
尺部材３４を固定する役目を果たし、回転ノブ６のカラ
ー３０との関係、したがってハンドル２との関係で基端
あるいは先端方向に移動することはない。すなわち、ト
リガーレバー４により長尺部材３２が移動しても、ある
いはフィンガーホイル・ローラ８によりチューブ体１０
が移動しても長尺部材３４は静止状態に保たれる。上述
のように、末端作動器３のハサミ作用、回動および自転
は全て独立したハンドル制御により行われる。これら３
つの機能の独立性により外科医により制御性が向上す
る。さらに１以上の機能を同時に行うことも可能であ
る。したがって、外科医による操作上の柔軟性が向上す
る。
【００１８】図９、１０は、ロック機構を説明するもの
である。すなわち、外科的療法において生体組織を末端
作動片４０、４４で掴んだ状態または末端作動片４０、
４４が閉塞した状態をそのまま維持させるロック機構を
示すものである。このロック機構は主としてハンドル２
およびトリガーレバー４の内側に配置されており、三脚
ラチェット・バー１６を具備し、これはピン４９を中心
として回動自在になっている。その第１のアーム５５が
ピン５３を中心として回動するバネ５１と係合し、これ
により三脚ラチェット・バー１６の第２のアーム５６が
キャッチ５０に向けて付勢されるようになっている。図
９に示すようにロック・アンド・リリース・レバー１２
の基端のフラットカム１３がラチェット・バー１６の第
３のアーム１７の先端と適宜係合し、第３のアーム１７
を持ち上げ第２のアーム５６をキャッチ５０から離間さ
せるようになっている。すなわち、図９はロック・アン
ド・リリース・レバー１２がロック機構を解除した状態
を示している。図１０に示すようにロック・アンド・リ
リース・レバー１２が反時計方向に回動したとき（矢線
Ｄ１で示す）、フラットカム１３と第３のアーム１７と
の相互作用によりラチェット・バー１６は若干、時計方
向に回動する（矢線Ｄ２で示す）。これによりラチェッ
ト・バー１６の歯５９がキャッチ５０の歯５７と係合
し、トリガーレバー４のそれ以上の開きが阻止される。
ラチェット・バー１６およびキャッチ５０の歯５７、５
９はトリガーレバー４が再度、引かれるまで、トリガー
レバー４がハンドル２に対しロックされた状態に維持さ
れるよう設計されている。
【００１９】図１に示す外科器具１は単極電気メスを実
行し得るよう設計されている。この電気メスは外科手術
において止血を行うためにしばしば必要となる。すなわ
ち、図１、６に示すようにハンドル２の後側にプラグ１
４が設けられている。このプラグ１４は電気外科発電機
（図示しない）からの電気メスコード（図示しない）を
接続するためのものである。図１１に示すようにプラグ
１４から長尺部材３２に向けて導電路が形成され、これ
により末端作動器３まで電気的に接続されている。な
お、上述のように、ハンドル２はプラスチック成型品か
らなり、チューブ体１０も繊維強化プラスチックから作
られているから、これらは導電性がない。さらに詳述す
ると、プラグ１４は、ハンドル２の固定部材５内に配置
されたワイヤー５２の基端８４と電気的に接続してい
る。ワイヤー５２の先端８６はハブ２６の周りに形成さ
れた導電性リング５４に接続している。また、導電性リ
ング５４はピン２８と電気的に接続し、これを介して長
尺部材３２、３４に電気的に接続されている（図７）。
【００２０】電気メス用ワイヤー５２および導電性リン
グ５４は、外科医が回転ノブ６を回転させチューブ体１
０および末端作動器３を自転させる場合の触覚フィード
バックを外科医に与える付加的作用をも奏する。すなわ
ち、ワイヤー５２は片持ちバネとして作用するように形
成され、このワイヤー５２の先端部８６が導電性リング
５４の周縁に沿って形成されている半円形回り止め上に
支持されている。ワイヤー５２の先端部８６は垂直方向
に移動可能になっているが、横方向の動きはワイヤー５
２の両側に形成されているリブ（図示しない）により制
限を受けるようになっている。
【００２１】次に、図１２〜１６においては、外科器具
１がチューブ体１０内に沿って延出する１以上のチャン
ネル（すなわち、ルーメン）を更に具備してなる。すな
わち、図１２〜１６に示す外科器具１は小型可撓性光フ
ァイバースコープ６２を受入れるべく特に設計されたル
ーメン９０を有する。このルーメン９０はチューブ体１
０のほぼ全長に沿って延びている。好ましくは、このチ
ューブ体１０の直径は５ｍｍである。他の同様の例とし
てチューブ体１０の直径が２ないし３ｍｍのものとする
こともできる。このルーメン９０への入口ポートとして
３つの場合が可能である。第１の入口ポート９２は図１
２に示すようにハンドル２の固定部材５の頂部である。
この第１の入口ポート９２からルーメン９０が固定部材
５を経由してチューブ体１０内部に延びている。第２の
可能な入口ポート６０は図１３に示すように回転ノブ６
に設けるものである。ルーメン９０は入口ポート６０か
らチューブ体１０内部に延びている。第３の可能な入口
ポートは図１４に示すようにチューブ体１０の基端近傍
に設けるものである。これらの入口ポート９２、６０、
９４は不使用時には小さな栓、摺動スリーブ（いずれも
図示しない）で密封され、外科器具１を介して空気が漏
れるのを防止する。ルーメン９０は図１２あるいは図１
５に示すように出口ポート６４で終わる。この出口ポー
ト６４は末端作動器３の基端に位置するチューブ体１０
の箇所に設けられている。また、この出口ポート６４は
末端作動片４０、４４が回動する側のチューブ体１０の
壁面に形成されている。この出口ポート６４からチュー
ブ体１０外へ導出された光ファイバースコープ６２は、
外科医の視界５８内に（図１２に点線で示されている範
囲）、末端作動片４０、４４の双方をその回動角度の如
何を問わず含めることができると共に、解剖部位をも含
めることができる。図１６はチューブ体１０の断面を示
すもので、これには光ファイバースコープ６２を組込ん
だルーメン９０が示されている。もちろん、ルーメン９
０は他のカテーテル装置、例えばカメラ、レーザーファ
イバーを収容することができる。
【００２２】図１７は外科器具１の他の実施例を示すも
ので、この場合、多重ルーメンが設けられている。図１
７に断面で示すようにチューブ体１０は大きい中央ルー
メン６６、２つの小さいルーメン６８およびチャンネル
１０６を有する。好ましい例として、このチューブ体１
０は直径が５ｍｍである。しかし、直径が２ないし３ｍ
ｍのチューブ体１０の多重ルーメンを形成することもで
きる。この多重ルーメンは種々の用途に使用することが
できる。以下に述べる第１の適用においては、多重ルー
メンを末端作動器との組合せで光ファイバー・カテーテ
ルの多重配置が可能となる。第２の適用においては、さ
らに後述するように吸引および灌注のために多重ルーメ
ンが提供される。
【００２３】図１８〜１１は本発明の３つの異なる具体
例、すなわち末端作動器３を多重ルーメン６６、６８と
の関連で使用し、光ファイバースコープ６２などの光フ
ァイバー・カテーテルを種々の方向に向けて配置する例
を示している。図１８に示す第１のスコープ／末端作動
器の例において、光ファイバースコープ６２はサイド・
ルーメン６８（図１７参照）の１つを介してチューブ体
１０に導入され、末端作動器３の１側に配置された出口
ポート６９から導出している。この末端作動片４０、４
４はハサミ刃でなく湾曲したデセクター・ジョーであ
る。これらデセクター・ジョー４０、４４は末端作動器
３の１側に湾曲し、その湾曲した側に出口ポート６９が
配置されている。これにより出口ポート６９から突出し
た光ファイバースコープ６２をデセクター・ジョー４
０、４４で掴むことが可能となっている。デセクター・
ジョー４０、４４で光ファイバースコープ６２を掴んだ
後、外科医は末端作動器３を操作して光ファイバースコ
ープ６２を種々の方向に向けることができる。末端作動
器３で光ファイバースコープ６２の末端を損傷させる危
険性を避けるためデセクター・ジョー４０、４４を扇形
に撓ませ、その中に光ファイバースコープ６２を受入れ
るようにしてもよい。
【００２４】図１９に示す第２のスコープ／末端作動器
の例においても、光ファイバースコープ６２ｂが用いら
れ、サイド・ルーメン６８の１つを通って出口ポート６
９から導出されている。この実施例においては、末端作
動器３は出口ポート６９の近傍の末端作動片４０にルー
プ（すなわち、ベイル）７６が設けられている。光ファ
イバースコープ６２をサイド・ルーメン６８に押込み、
また末端作動片４０が長手方向延出位置にある場合、光
ファイバースコープ６２をループ７６内に挿通させるこ
とができる。ついで末端作動片４０、４４を回動させた
とき、光ファイバースコープ６２が大きな直径の円弧を
描いて湾曲させることができ、これにより光ファイバー
スコープ６２をチューブ体１０の長手軸に平行な方向か
らチューブ体１０の長手軸に垂直な方向に向けることが
できる。すなわち、このスコープ／末端作動器形態を有
する外科器具１は事実上関節スコープとしても機能させ
ることができる。
【００２５】図２０、２１は第３のスコープ／末端作動
器の例を示す。この実施例では光ファイバースコープ６
２は大きい中央ルーメン６６（図１７）を介して導入さ
れ、末端作動器３の基端直後に位置する出口ポート９６
を介してチューブ体１０から導出されている。この大き
いルーメン６６のため光ファイバースコープ６２は大き
くてもよい。末端作動片４０、４４はこの場合、“アヒ
ルのくちばし”型グラスパー・ジョーである。末端作動
片４４はほぼ全長に亘って延びたチャンネル１０８を有
する。光ファイバースコープ６２をルーメン６６内に押
込み末端作動片４４を僅かに長手方向延出位置から回動
させることにより光ファイバースコープ６２を末端作動
片４４のチャンネル１０８を貫通させることができる。
一旦、光ファイバースコープ６２をこのように導入させ
た後は、末端作動片４４を長手方向に向けたり、それか
ら１１０度回動させてもよい。このようにして関節スコ
ープとして機能させることができる。図２１に示すよう
な長手方向延出位置にある場合、光ファイバースコープ
６２は歯４２、４６（図３、４参照）の存在により符号
７８で示すように若干横に突出する。
【００２６】次に、図２２を参照して多重ルーメン６
６、６８の第２の適用例を説明する。この例において
は、２つのサイド・ルーメン６８が灌注液を供給するの
に使用され、中央ルーメン６６は吸引のために使用され
る。この例におけるルーメン６６、６８の形状は断面が
不規則であるが、これらの断面は円形、台形、その他の
任意の形状であってよい。灌注ルーメン３８を流れる水
流はチューブ体１０の先端の出口ポート６９（図２２に
は１個のみ示されている）から噴出される。なお、出口
ポート６９は末端作動器３の両側に配置されている。ル
ーメン６６に通じている吸引ポート９６もチューブ体１
０の先端で、かつ末端作動器３の基端直後に配置されて
いる。吸引のため、末端作動片４０、４４は最大限に回
動させてもよく、チューブ体１０の先端は吸引される流
体プール内に位置させる。この吸引作用を促進するた
め、吸引ルーメン６６と灌注ルーメン６８との間に数個
の小さなホール６７を穿設してもよく、あるいはチュー
ブ体１０を貫通して吸引ホールを設けてもよい。この形
態ではルーメン６６、６８の全てを吸引あるいは灌注ポ
ートとして使用できる。
【００２７】図２３はチューブ体１０に接続された吸引
／灌注装置１００を示している。この吸引／灌注装置１
００はチューブ体１０上をスライドするアダプター７０
を備えている。このアダプター７０は基端吸引ポート１
０２および基端灌注ポート１０４の周りをシールしてい
る。なお、これらポート１０２、１０４は図中破線で示
されている。灌注液を供給するホース７２、真空を供給
するホース７４は双方ともアダプター７０ならびに各ポ
ート１０２、１０４に接続されている。この装置に示さ
れているプッシュボタン７５は吸引および灌注のための
制御弁を調節するためのものである。これ以外に、ホー
スにボタン（図示しない）を設け、これにより吸引およ
び灌注を制御するようにしてもよい。クリップあるいは
バンド７７はホース７２、７４をハンドル２に取着する
ためのもので、吸引／灌注装置１００がチューブ体１０
において回転したり移動したりするのを防止するための
ものである。
【００２８】図２４は図１に示すハンドル２の変形例を
示すものである。フィンガーホイル・ローラの代りに第
２のトリガー１４０がハンドルに設けられ、これにより
双方の末端作動片４０、４４を回動させる。第２のトリ
ガー１４０はチューブ体１０の長手軸に垂直な軸上を回
動する。この第２のトリガー１４０の回動によりチュー
ブ体ハブ２６（図示しない）が上述の応力倍増連結部
材、あるいはラックおよび歯車機構を介して移動する。
その他、第２のトリガー１４０はチューブ体１０の長手
軸に平行あるいは角度をなした軸上を回動するようにし
てもよい。この実施例では第２のトリガー１４０の動き
は従来のトリガーの回動と言うよりは“フラッピング”
に近いものである。この動作は上記の他の実施例と同様
に人差し指で容易に操作し得る。なお、本発明の外科器
具を好ましい例を参照して説明したが、本発明の趣旨を
逸脱しない範囲で種々変更し得ることは明らかであろ
う。

【発明の効果】以上詳述した如く、本発明に係わる外科
器具によれば、末端作動器の連結、回転および操作を片
手で正確、かつ直観的に制御することが可能となる。さ
らに本発明に係わる外科器具によれば、１つの外科器具
で種々の作業を行うことができる多機能性にすぐれてお
り、電気焼灼、吸引／灌注などを素早く行うことができ
る。

Claims (20)

    【特許請求の範囲】
  1. 【請求項１】 連結部を有せず、基端と先端とを有する
    チューブ体と；該チューブ体の先端にそれぞれ回動自在
    に直接取着され、相互間に操作角度を形成し得るように
    して設けられた第１の作動片と第２の作動片とを具備し
    てなる末端作動器と；該チューブ体の基端に取着された
    ハンドルであって、該ハンドルが該チューブ体の長手軸
    と整合して固定され、片手で握持し得るグリップ部を有
    する固定部と；手動駆動制御部と；手動回動制御部と；
    を含み、該手動駆動制御部および手動回動制御部が上記
    グリップ部を握持した片手で操作可能となっているもの
    と；該チューブ体を通って延び、上記手動駆動制御部お
    よび手動回動制御部と上記第１の作動片と第２の作動片
    とを操作可能に連結する連結部材と；を具備してなり；
    該手動駆動制御部の操作に応答して該連結部材が該末端
    作動器に作用して操作角度を変更させ；該手動回動制御
    部の操作に応答して該連結部材が該第１の作動片と第２
    の作動片に作用し、これらの操作角度を実質的に一定に
    保ちつつ、これらを同時に同一角度に回動させることを
    特徴とする内視鏡用外科器具。
  2. 【請求項２】 該手動駆動制御部がトリガーレバーを具
    備してなり、該トリガーレバーの第１の方向への動きが
    該第１の作動片と第２の作動片との間の操作角度を縮小
    させ、逆方向への動きが該第１の作動片と第２の作動片
    との間の操作角度を増大させるようにしたことを特徴と
    する請求項１記載の内視鏡用外科器具。
  3. 【請求項３】 該ハンドルのグリップ部が少なくとも中
    指が入るループを有し、該トリガーレバーが親指が入る
    ループを有し、該手動回動制御部が人差し指で操作可能
    になっていることを特徴とする請求項２記載の内視鏡用
    外科器具。
  4. 【請求項４】 該手動回動制御部が該ハンドルの固定部
    に配置されたフィンガーホイル・ローラであり、該フィ
    ンガーホイル・ローラの第１の方向への回転が該第１の
    作動片と第２の作動片を長手方向延出位置から離れる方
    向に回動させ、逆方向への回転が該第１の作動片と第２
    の作動片を長手方向延出位置に向けて近付く方向に回動
    させものであることを特徴とする請求項１記載の内視鏡
    用外科器具。
  5. 【請求項５】 該フィンガーホイル・ローラが、フィン
    ガーホイル・ローラの回転を触覚的にフィードバックを
    与えるためのバネおよび回り止め機構を具備してなると
    ともに、該チューブ体の長手軸にほぼ直交する回転軸を
    有することを特徴とする請求項４記載の内視鏡用外科器
    具。
  6. 【請求項６】 該手動回動制御部が該ハンドルの固定部
    に配置された第２のレバーであり、該第２のレバーの第
    １の方向への回転が該第１の作動片と第２の作動片を長
    手方向延出位置から離れる方向に回動させ、逆方向への
    回転が該第１の作動片と第２の作動片を長手方向延出位
    置に向けて近付く方向に回動させものであり；該第２の
    レバーがそれを操作させるための人差し指が入るループ
    を有することを特徴とする請求項３記載の内視鏡用外科
    器具。
  7. 【請求項７】 該ハンドルがさらに該チューブ体および
    末端作動器を回転させるための手動回転制御部を有し、
    該手動回転制御部は上記手動駆動制御部および手動回動
    制御部から独立して操作され、該ハンドルのグリップ部
    を握持する同じ片手で操作可能となっていることを特徴
    とする請求項１記載の内視鏡用外科器具。
  8. 【請求項８】 該手動回転制御部が回転ノブを具備して
    なり、該回転ノブが上記チューブ体に回転自在に固着さ
    れ、該チューブ体とともに回転するようになってととも
    に、回転ノブの回転を触覚的にフィードバックを与える
    ためのバネおよび回り止め機構を具備してなることを特
    徴とする請求項７記載の内視鏡用外科器具。
  9. 【請求項９】 該ハンドルが、手動ロック手段を有し、
    この手動ロック手段により該第１の作動片と第２の作動
    片との間の操作角度を増大させる方向にトリガーレバー
    が移動するのを防止するようにしており、該ロック手段
    が該ハンドルのグリップ部を握持する同じ片手で操作可
    能に設計されていることを特徴とする請求項２記載の内
    視鏡用外科器具。
  10. 【請求項１０】 該ハンドルが電源を接続をとるための
    プラグをさらに有し；該ハンドルがさらに該プラグとチ
    ューブ体に沿って延びる連結部とを接続させるための導
    電路を有し；該連結部と該末端作動器は導電性材料から
    作られ、電流が該末端作動器近傍の治療部位に与えられ
    るようになっており；該チューブ体が非導電性材料から
    作られていることを特徴とする請求項１記載の内視鏡用
    外科器具。
  11. 【請求項１１】 該手動回動制御部がさらに機械的応力
    倍増器を有し、該応力倍増器を介してフィンガーホイル
    ・ローラからの回転力が該チューブ体に伝達されるよう
    になっていることを特徴とする請求項７記載の内視鏡用
    外科器具。
  12. 【請求項１２】 該機械的応力倍増器が歯車とピニオン
    とを具備してなり；該チューブ体の基端部に円筒状ラッ
    クが設けられ、該円筒状ラックが該機械的応力倍増器の
    歯車とピニオンとに動作的に連結し、フィンガーホイル
    ・ローラの回転が該円筒状ラックを介して該チューブ体
    の軸方向の移動を生じさせ、他方、上記回転制御部によ
    る該チューブ体の回転は該チューブ体の軸方向の移動を
    生じさせないことを特徴とする請求項１１記載の内視鏡
    用外科器具。
  13. 【請求項１３】 基端と先端とを有するチューブ体と；
    該チューブ体の先端にそれぞれ回動自在に直接取着さ
    れ、相互間に操作角度を形成し得るようにして設けられ
    た第１の作動片と第２の作動片とを具備してなる末端作
    動器と；該チューブ体を通って延びた連結部材であっ
    て、該連結部材が該第１の作動片に操作可能に連結され
    た第１の長尺部材と該第２の作動片に操作可能に連結さ
    れた第２の長尺部材とを具備してなり、該第１の長尺部
    材を該チューブ体に対し相対的に移動させることにより
    該第１の作動片を回動させ、該第２の長尺部材を該チュ
    ーブ体に対し相対的に移動させることにより該第２の作
    動片を回動させるものと；該チューブ体の基端に取着さ
    れたハンドルであって、該ハンドルが該チューブ体の長
    手軸と整合して固定され、片手で握持し得るグリップ部
    を有する固定部と、該第１の作動片と該チューブ体との
    間の相対的移動を生じさせて該第１の作動片を回動させ
    てその操作角度を変化させるための手動駆動制御部と；
    該第１の長尺部材と該第２の作動片との間の相対的移動
    を実質的に生じさせることなしに、該チューブ体と該第
    １の長尺部材および該第２の作動片との間の相対的移動
    を生じさせ、操作角度を実質的に維持させたまま第１の
    作動片と第２の作動片とを同時、かつ同一方向に回動さ
    せるための手動回動制御部と；を含み、該手動駆動制御
    部および手動回動制御部が上記グリップ部を握持した片
    手で操作可能となっているものと；を具備してなること
    を特徴とする内視鏡用外科器具。
  14. 【請求項１４】 該チューブ体がその長手軸に沿って、
    かつハンドルの固定部材との関係で移動可能となってお
    り；該手動回動制御部が、該第１の長尺部材および該第
    ２の作動片の該ハンドルの固定部材との相対的移動を実
    質的に生じさせることなしに、該チューブ体を該ハンド
    ルの固定部材との関係で移動させ、該第１の長尺部材お
    よび該第２の長尺部材との間の相対的移動を実質的に生
    じさせることなしに、該チューブ体と該第１の長尺部材
    および第２の長尺部材との間の相対的移動を生じさせる
    ことを特徴とする請求項１３記載の内視鏡用外科器具。
  15. 【請求項１５】 該第１、第２の長尺部材が成形された
    金属ワイヤーから作られていることを特徴とする請求項
    １３記載の内視鏡用外科器具。
  16. 【請求項１６】 基端と先端とを有し、ほぼ全長に亘っ
    て断面が連結チャンネルと第１のルーメンを形成してい
    るチューブ体と；該チューブ体に通じ、該第１のルーメ
    ンの基端へのアクセスを提供する基端ポートと；該チュ
    ーブ体に通じ、該第１のルーメンの先端へのアクセスを
    提供する先端ポートと；該チューブ体の先端に取着さ
    れ、２通り以上の操作が可能な末端作動器と；該チュー
    ブ体の基端に取着されたハンドルであって、該ハンドル
    が該末端作動器を操作する制御部を有し、その制御部に
    より異なった方法で該末端作動器を操作することを可能
    としたものと；該チューブ体を通って延び、上記ハンド
    ルの制御部から該末端作動器に操作可能に接続された連
    結部材と；を具備してなることを特徴とする内視鏡用外
    科器具。
  17. 【請求項１７】 該連結チャンネルが該連結部材のゆが
    みの発生を防止し得る断面寸法のものであり；該第１の
    ルーメンが該チューブ体の長手軸中心に対し偏心してい
    ることを特徴とする請求項１６記載の内視鏡用外科器
    具。
  18. 【請求項１８】 該連結チャンネルがその内部を通って
    延びた一対の長尺部材を具備してなることを特徴とする
    請求項１７記載の内視鏡用外科器具。
  19. 【請求項１９】 該チューブ体が、基端から先端近傍に
    亘るほぼ全長に断面構造として１以上の第２ルーメンが
    形成されたものであり；それぞれの第２ルーメンには：
    該第２ルーメンの基端へのアクセスを提供する基端ポー
    トが該チューブ体内に向けて設けられ；該第２ルーメン
    の先端へのアクセスを提供する先端ポートが該チューブ
    体内に向けて設けられていることを特徴とする請求項１
    ６記載の内視鏡用外科器具。
  20. 【請求項２０】 灌注／吸引を行うための各ルーメンの
    基端ポートが該チューブ体の基端近傍の露出面に設けら
    れ、そこから各ルーメンが内部に向けて延びており、該
    チューブ体が灌注／吸引装置を受理し得るように作ら
    れ、該灌注／吸引装置が灌注／吸引を行うための各ルー
    メンの基端ポートと接続可能なホースを備えていること
    を特徴とする請求項１９記載の内視鏡用外科器具。
