Description
本開示は、一般に、人工弁に関し、そしてより詳細には、可撓性リーフレットタイプの人工心臓弁デバイスに関する。
個々のリーフレットをフレーム(生物学的及び合成)に縫い合わせること、及び、合成リーフレットの場合にのみ、フレーム上へポリマーを射出成形及びディップコーティングすることを含む、リーフレットをフレームにカップリングさせるための多くの製造技術は使用されてきた。多くの場合に、得られたリーフレットはフレーム上で支持され、そしてリーフレットがフレームにカップリングされている取り付け端及び自由端を有するフラップを画定しており、該自由端はフラップを動かすことを可能にする。フラップは流体圧力の影響下に動く。操作時に、リーフレットは上流流体圧が下流流体圧を超えるときに開放し、そして下流流体圧が上流流体圧を超えるときに閉止する。リーフレットの自由端は下流流体圧の影響下にきつく適合し、弁を閉止して、下流血液が弁を通して逆流することを防止する。
記載される実施形態は心臓弁置換などの弁置換のための機器、システム及び方法に関する。より詳細には、記載される実施形態は、生物学的又は合成リーフレット材料及びフレームを有する可撓性リーフレット弁デバイス、ならびに、弁デバイスの製造方法及びインプラント方法に関する。

リーフレットフレーム200はリーフレット構造体300を保持しそして支持するように操作可能である。リーフレットフレーム200は環状であり、すなわち、軸X、及び、該軸に平行に延在して、互いに間隔を開けている複数の交連ポスト210を有するシリンダを画定しており、各交連ポスト210はポストスロット217を画定しており、それを通して軸Xに平行に位置合わせされている。交連ポスト210の間には、リーフレットウィンドー222があり、該リーフレットウィンドーはリーフレット自由端312を除くリーフレット310の周囲でリーフレット310にカップリングしそしてそれを支持するように操作可能である。

リーフレット構造体300は生体適合性ポリマーなどの、十分に追従性でかつ可撓性である任意の生体適合性材料を含むことができる。リーフレット構造体300はエラストマーと組み合わせた膜を含み、複合材料を形成することができる。リーフレット構造体300は、ある実施形態によると、複合材料を含むことができ、該複合材料はフィブリルのマトリックス内に複数の空間を含む延伸フルオロポリマー膜及びエラストマー材料を含む。本開示の範囲内にありながら、複数のタイプのフルオロポリマー膜及び複数のタイプのエラストマー材料を組み合わせて複合材料を形成することができることは理解されるべきである。本開示の範囲内にありながら、エラストマー材料は、複数のエラストマー、無機フィラー、治療剤、放射線不透過性マーカーなどの複数のタイプの非エラストマー成分を含むことができることも理解されるべきである。
保持要素400はリーフレット構造体300のブリッジ領域330により形成されるブリッジループ338内に配置されるように操作可能である要素であり、ブリッジループ338がポストスロット217を通して通過することを有効に防止し、それゆえ、リーフレット構造体300はポスト外側面で交連ポストに機械的にカップリングされる。保持要素400はポストスロット217の幅より大きい幅を有する。保持要素400がブリッジループ338内に配置されると、ブリッジループ338はポストスロット217を通して通過するのが防止されるであろう。ブリッジループ338のサイズは保持要素400のサイズに密接に対応し、それにより、縫合糸が弛緩し又は破損した場合に、ブリッジ領域330の一部分が弁オリフィス150までポストスロット217を通して延在することを防止するようなものであるべきである。

ベースフレーム500は、図3及び4に示されるとおり、概して環状部材であり、該環状部材はベースフレーム内側面502及びベースフレーム外側面504を有するベースフレーム管腔550を画定している。ベースフレーム500は構造上の荷重負荷支持をリーフレットフレーム200に提供することができる。さらに、ベースフレーム500は移植部位での受容組織に積極的な係合を提供するように構成されうる。
弁100は、図1A及び1Bに示されるように、ベースフレーム500に隣接した縫製カフ600を備えることができる。縫製カフ600は移植部位にカップリングするための縫合糸を受け入れる構造を提供するように操作可能である。縫製カフ600は任意の適切な材料を含むことができ、該材料としては、例えば、限定するわけではないが、ダブルベロアポリエステル及びシリコーンが挙げられる。縫製カフ600はベースフレーム500の周囲付近にあることができ、又はベースフレーム500により弁膜周囲にあることができる。縫製カフ600はフィラー材料を含むことができ、該フィラー材料としては、例えば、限定するわけではないが、シリコーンリング及び/又はPTFEフエルトなどが挙げられる。
人工弁の製造方法は、ある実施形態によると、1層以上の延伸PTFE複合材料を含むチューブを得ることを含む。チューブからブリッジ領域により各々分離された複数のリーフレットを含むリーフレット構造体を切断すること。リーフレットの折り返し部分において折り返し開口部及びブリッジ領域においてブリッジ開口部を提供すること。複数の保持要素を得ること、ここで、各保持要素は保持要素開口部を画定している。各ブリッジ領域をブリッジループへと折り曲げ、そして各ブリッジループ及び2つの隣接リーフレットとの間で接合ネックを画定すること、ここで、ブリッジループはチューブ軸から半径方向に離れるように延在している。各ブリッジループ中に保持要素を配置すること。ブリッジ開口部及びそれと位置合わせされた保持要素開口部を通して縫合糸を通過させて、各保持要素をそれぞれのブリッジループに縫合すること。金属チューブからリーフレットフレームを切断し、リーフレットフレームウィンドー及びその間に交連ポストを画定すること、ここで、各交連ポストはポストスロットを画定し、該ポストスロットはブリッジ領域の2倍の厚さを受け入れる寸法である。リーフレットウィンドーフレームにおいてリーフレットウィンドーフレーム開口部及び交連ポストにおいてポスト開口部を提供すること。保持要素をポスト外側面に隣接させて、各接合ネックをそれぞれのポストスロットに配置し、リーフレットフレームにリーフレットを配置すること。保持要素開口部をポスト開口部と位置合わせすること。保持要素開口部及びそれと位置合わせされたポスト開口部を通して縫合糸を通過させて、各保持要素をそれぞれの交連ポストに縫合すること。リーフレットフレームインフロー端に沿って、そしてリーフレットウィンドーフレーム開口部と折り返し開口部を位置合わせして、リーフレットフレーム外側面に対して各リーフレットの折り返し部分を折り曲げること。折り返し開口部及びそれと位置合わせされたリーフレットウィンドーフレーム開口部を通して縫合糸を通過させて、各折り返し部分をそれぞれのリーフレットウィンドーフレームに縫合すること。

Claims (10)
- 中央軸、内側面及び外側面を有するリーフレットフレームであって、前記リーフレットフレームは、複数の交連ポスト及び隣接する交連ポストの間にそれぞれ位置する複数のリーフレットウィンドウを含む、リーフレットフレーム;
複数のリーフレット及び前記リーフレットのそれぞれの間のブリッジ領域を含むリーフレット構造体であって、前記リーフレットは、前記リーフレットフレームから内側方向に延在しており、前記リーフレットのそれぞれは、複数の折り返し開口部を有する折り返し部分を画定しており、前記折り返し部分のそれぞれは、前記リーフレットフレームのインフロー端で折り返されている、リーフレット構造体、及び
前記複数の折り返し開口部を用いて前記リーフレットフレームに対して前記リーフレット構造体の前記折り返し部分を固定する複数の固定構造、
を含む、人工弁。 - 前記リーフレットフレームは、前記リーフレットウィンドウにおいて複数のフレーム開口部を含み、前記複数のリーフレットのそれぞれの前記複数の折り返し開口部のそれぞれは、前記複数のフレーム開口部の1つと位置合わせされており、前記複数の固定構造は、前記位置合わせされた複数のフレーム開口部と前記複数の折り返し開口部を通過して、前記折り返し部分を前記リーフレットフレームの前記リーフレットウィンドウにカップリングさせる、請求項1に記載の人工弁。
- 前記リーフレットのそれぞれの前記折り返し部分は、前記リーフレットフレームのインフロー端の周囲で折られており、前記リーフレットフレームの外側面にカップリングされている、請求項1に記載の人工弁。
- 前記複数の交連ポストのそれぞれは、第一のポストレッグ及び第二のポストレッグを含み、前記ポストレッグは、前記複数のリーフレットウィンドウの1つからそれぞれの交連ポストに結合される交連先端部に向けて延在しており、各交連ポストの前記第一のポストレッグ及び第二のポストレッグは互いに間隔を開けてポストスロットを画定している、請求項1に記載の人工弁。
- リーフレット構造体は、連続シリンダとして形成される、請求項1に記載の人工弁。
- 前記複数の固定構造は、複数の縫合糸を含む、請求項1に記載の人工弁。
- 前記複数の固定構造は、接着剤又は熱接着を含む、請求項1に記載の人工弁。
- 前記リーフレット構造体は、少なくとも1つのフルオロポリマー膜層を含む、請求項1に記載の人工弁。
- 前記少なくとも1つのフルオロポリマー膜層は、延伸フルオロポリマー膜層である、請求項8に記載の人工弁。
- エラストマーが、前記延伸フルオロポリマー膜層の多孔性構造内に含まれているか、前記延伸フルオロポリマー膜層の片面又は両面にコーティングされているか、あるいは、前記延伸フルオロポリマー膜層にコーティングされかつその内部に含まれている、請求項9に記載の人工弁。
