本発明は、全体として、免疫療法、より具体的には、改変された免疫細胞、例えば、T細胞を用いた免疫療法に関する。
標的化免疫療法は、癌、感染性障害、及び自己免疫障害を含む、種々の疾患を治療するために、免疫細胞又は免疫細胞と結合する分子の使用に頼っている(Miller及びSadelainの文献、Cancer Cell. 27(4):439-49(2015); Sabatos-Peytonらの文献、Curr. Opin. Immunol. 22(5):609-615(2010); McLeod及びAndertonの文献、Curr. Opin. Pharmacol. 23:1-108(2015))。最近、腫瘍抗原を標的とするキメラ抗原受容体(CAR)を発現させるT細胞の遺伝子修飾は、ヒトにおける白血病細胞の根絶の成功を可能にした(Brentjensらの文献、Sci. Transl. Med. 5(177):177ra38. doi: 10.1126/scitranslmed.3005930(2013))。後者の手法において、CARをコードする相補的DNA(cDNA)は、組込み可能なγ-レトロウイルス又はレンチウイルスを介してT細胞に送達される。これらの組換えウイルスベクターは、半ランダムな様式でのヒトゲノムへのウイルスDNAの組込みを触媒する、ウイルスインテグラーゼ酵素を必要とする(Schroderらの文献、Cell 110(4):521-529(2002); Wuらの文献、Science 300(5626):1749-1751(2003))。第三の手法は、その成分を、ウイルス粒子を必要とせずに細胞内に送達することができる、DNA転位機構を利用している。この場合、DNAトランスポザーゼは、これも半ランダムな様式で、ヒトゲノムへのDNAトランスポゾン(対象となる遺伝子を含む)の組込みを行う(Yantらの文献、Mol. 本発明は、本明細書において、下記のように提示される特許請求の範囲によって反映される。本発明は、トランスジーンの発現が内在性プロモーターの制御下にあるように、トランスジーンがT細胞のゲノム内に組み込まれるT細胞、及びそのような細胞の使用方法に関する。
本発明は、免疫療法、及び具体的には、所望の条件下で治療的トランスジーンを発現するようにT細胞を遺伝子改変することに基づく標的化細胞療法に関する。本明細書に記載されるのは、治療的トランスジーンが内在性プロモーターの制御下に置かれるように、T細胞のゲノムへの該トランスジーンの組込みを標的化することにより、免疫療法用のT細胞を作製する方法である。文脈からそうでないことが示されない限り、本明細書に記載されるトランスジーン(単数形)への言及は1以上のトランスジーン(複数形)にも適用されることが理解されるであろう。本発明は、ゲノム編集を用いて、1つ又はいくつかの治療的トランスジーンを1以上の内在性プロモーターの制御下に置き、治療的T細胞における空間的・時間的発現の制御をもたらすT細胞療法の戦略を提供する。本発明は、治療的トランスジーン又は種々の治療的トランスジーンを発現するように改変されることになるT細胞を提供し、その場合、該トランスジーンの発現は、相応に発現をもたらす内在性プロモーターを用いて、T細胞の位置(例えば、腫瘍に近接しているときのみのトランスジーンの発現)、又は規定の時点(例えば、腫瘍細胞と結合する前又は結合した後)に依存するようにすることができる。したがって、本発明の細胞及び方法を用いて、治療的T細胞の有効性及び安全性を高めることができる。
一実施態様において、本発明は、T細胞であって、治療的トランスジーンの発現がT細胞の内在性プロモーターの制御下にあるように、該トランスジーンが該細胞のゲノム内の部位で組み込まれる、T細胞を提供する。好ましい実施態様において、本発明は、T細胞であって、キメラ抗原受容体(CAR)をコードする組換え核酸配列が該細胞によって該細胞の表面で発現されるように、該CARが該細胞のゲノム内の部位で組み込まれ、かつ該CARをコードする核酸の該部位での組込みが該細胞の表面での機能的T細胞受容体(TCR)複合体の発現を低下させ又は妨げる、T細胞を提供する。好ましい実施態様において、該組換え細胞を用いて、所望の標的に対する免疫応答を増強又は提供することができる。別の実施態様において、該組換え細胞を用いて、望ましくない免疫応答を抑制することができる。好ましくは、該細胞はヒトに由来する(組換え体にされる前はヒト起源である)(ヒト由来細胞は、本発明の治療方法におけるヒトへの投与に特に好ましい)。
内在性T細胞プロモーターの制御下でトランスジーンを組換え発現する細胞の作製に関して、トランスジーンは、T細胞のゲノムに導入される。好ましい実施態様において、CARを組換え発現する細胞の作製に関して、CARをコードする核酸は、T細胞に導入される。従来、そのような方法では、好適な発現ベクターが利用されており、その場合、T細胞に、トランスジーン、例えば、CARをコードする核酸が形質導入される。本発明では、トランスジーンは、ゲノム内の部位でのトランスジーンの標的化組込みをもたらす標的化コンストラクトにクローニングされる。好ましい実施態様において、CARをコードする核酸は、ゲノム内の部位、特定の実施態様において、細胞内での機能的TCR複合体の発現に必要とされるタンパク質をコードする遺伝子の発現を妨害する部位でのCARをコードする核酸配列の標的化組込みをもたらす標的化コンストラクトにクローニングされる。例えば、本発明のトランスジーン、例えば、CARをコードするポリヌクレオチドを、周知の分子生物学の技法を用いて、好適な標的化コンストラクト、又はレトロウイルスベクターなどの好適なベクターにクローニングし、T細胞に導入することができる(Ausubelらの文献、分子生物学の最新プロトコル(Current Protocols in Molecular Biology)、John Wiley and Sons, Baltimore, MD(1999)を参照)。

本発明は、治療的トランスジーンをT細胞の内在性プロモーターの制御下に置くように該トランスジーンをT細胞のゲノム内の部位で組み込むことにより、該トランスジーンをT細胞で発現させることに関する。内在性プロモーターを利用することにより、T細胞は、治療的トランスジーン又は種々の治療的トランスジーンを異なる内在性プロモーターの制御下で発現するように改変される。具体的な実施態様において、該トランスジーンの発現は、T細胞の微小環境に依存する。例えば、治療的トランスジーンの発現は、T細胞が特定の位置にあるときに誘導される内在性プロモーターを使用することにより(例えば、T細胞が腫瘍の位置にあり、腫瘍抗原への結合によって活性化され、それにより、内在性プロモーターを誘導するときに)、T細胞の位置に依存するようにすることができるか(例えば、腫瘍に近接しているときのみのトランスジーンの発現)、又は規定の時点におけるものである(例えば、規定の時点で、例えば、腫瘍細胞と遭遇したときのT細胞の活性化によって誘導される内在性プロモーターを使用することによる)ことができる。プロモーターは、例えば、それがT細胞と抗原との遭遇後にどのくらい早く活性化又は抑制されるか、それがどのくらい強く及びどのくらい長く発現されるかに基づいて選択される。プロモーターは、該プロモーターがその発現を調節するトランスジーンの薬理作用に適合するように選択される(例えば、あるトランスジーンは低いレベルでより効果的であり、あるトランスジーンは高い発現レベルでより効果的である、など)。T細胞におけるトランスジーンの発現を制御する内在性プロモーター(単数)の使用に関する本開示における記載は、文脈からそうでないことが示されない限り、T細胞におけるトランスジーン(他のトランスジーンと同じ又は異なるものであり得る)の発現を各々制御する複数の内在性プロモーターの使用に同等に適用されることが理解されるであろう。当業者は、T細胞療法において使用するためのT細胞の有効性を高める1以上のトランスジーンの所望の発現及び/又は調節をもたらすように、適切な内在性プロモーターを容易に選択することができる。
一実施態様において、治療的トランスジーンは、該トランスジーンの発現が、構成的である内在性プロモーターの制御下に置かれるように、T細胞のゲノム内の部位で組み込まれる。構成的プロモーターを用いて、トランスジーン、例えば、CAR又はCCRを発現させ、免疫応答を活性化することができる。PD1及び又はcTLA4 細胞内ドメインなどを含有する抑制性CAR(iCAR)の発現を制御する場合、構成的プロモーターを用いて、免疫応答を抑制することもできる。

一実施態様において、治療的トランスジーンは、該トランスジーンの発現が誘導性である内在性プロモーターの制御下に置かれるように、T細胞のゲノム内の部位で組み込まれる。誘導性プロモーターは、シグナルを核に伝達し、誘導性プロモーターの活性化をもたらす誘導因子に応答性であるプロモーターである(例えば、表1を参照)。一般に、誘導因子は、T細胞によって発現される分子の結合パートナーである。例えば、受容体の場合、結合パートナーは、その同族リガンドであることができ、又はCAR、CCR、もしくはTCRの場合、結合パートナーは、標的抗原であることができる。
本発明は、治療的トランスジーンの発現がT細胞の内在性プロモーターの制御下にあるように、該トランスジーンを該T細胞のゲノム内の部位で組み込むことにより、該T細胞で該トランスジーンを発現させることに関する。治療的トランスジーンは、治療的タンパク質又は治療的核酸をコードするヌクレオチド(例えば、DNA又はその修飾形態)配列である。該治療的タンパク質又は治療的核酸は、T細胞によって発現される場合、ヒト又は動物の疾患又は障害の治療に役に立つ。治療的核酸は、好ましくは、治療的RNAである。治療的タンパク質は、ペプチド又はポリペプチドであることができる。
キメラ抗原受容体(CAR)は、本発明の治療的トランスジーンによってコードされる例示的な産物である。本発明の細胞によって組換え発現されるCARは、抗原に結合する抗原結合ドメインである。抗原は、対象に存在するか、又はT細胞が投与される対象において予防されることが望ましい、疾患又は障害と関連する。

キメラ共刺激性受容体(CCR)は、本発明の治療的トランスジーンによってコードされる例示的な産物である。キメラ共刺激受容体(CCR)は、CARと同様に、抗原結合細胞外ドメイン、膜貫通ドメイン、及び細胞内シグナル伝達ドメインを含むキメラ受容体である(Sadelainらの文献、Cancer Discov. 3(4):388-398(2013))。CCRは、T細胞活性化ドメインを有さないが、共刺激ドメイン、例えば、CARについて上で記載されている共刺激ドメイン、例えば、CD28、4-1BB、OX40、ICOS、DAP10、2B4、CD70などのうちの1つを有する。CCRをT細胞受容体又はCARとともに用いて、二重抗原発現T細胞に対するT細胞反応性を増強することができる(Sadelainらの文献、上記、2013)。CCRを用いて、選択的腫瘍標的化を増強することもできる(Sadelainらの文献、上記、2013)。CCRは、その結合パートナー、すなわち、標的抗原に結合したときに、(CCRの共刺激ドメインに応じて)4-1BB、OX40、ICOS、又はCD70の効果を模倣する抗原特異的共刺激受容体である。

ドミナントネガティブ体は、本発明の治療的トランスジーンによってコードされる例示的な産物である。治療的トランスジーンによってコードされる場合に特に有用であるドミナントネガティブ体としては、本発明のT細胞の活性化を刺激又は持続するものが挙げられる。例示的なドミナントネガティブ体としては、抑制性キメラ抗原受容体(iCAR)、分泌可能な可溶性サイトカイン受容体(例えば、TGFβ、IL10の場合)、分泌可能な可溶性T細胞抑制性受容体(例えば、PD1、CTLA4、LAG3、又はTIM-3に由来するもの)などが挙げられるが、これらに限定されない。抑制性キメラ抗原受容体は、細胞外scFVドメイン(標的細胞で細胞表面分子に結合する)が抑制性T細胞受容体(例えば、PD1、CTL4)に由来する細胞内シグナル伝達ドメインに融合したものから構成される細胞表面受容体である。改変されたT細胞は、標的細胞と相互作用すると阻害される。
微小環境モジュレーターは、本発明の治療的トランスジーンによってコードされる例示的な産物である。微小環境モジュレーターは、治療的な改変されたT細胞の近くの細胞の活性を調節する分子を指す。治療的トランスジーンによってコードされる場合に特に有用である微小環境モジュレーターとしては、本発明のT細胞の活性化を刺激又は持続するものが挙げられる。例示的な微小環境モジュレーターとしては、ヘパラナーゼ、TNFRSF14とも呼ばれるヘルペスウイルス侵入メディエーター(HVEM)などが挙げられるが、これらに限定されない。
抗体は、本発明の治療的トランスジーンによってコードされる例示的な産物である。例示的な抗体としては、T細胞抑制性リガンド、例えば、PD1L、CD80、CD86、ガレクチン-9、Fasリガンドなどに対する抗体が挙げられるが、これらに限定されない。

溶質輸送体は、本発明の治療的トランスジーンによってコードされる例示的な産物である。例示的な溶質輸送体としては、Glut1又はGlut3などのグルコース輸送体が挙げられるが、これらに限定されない。エフェクターT細胞は、エネルギーを生み出すのにグルコースの取込みの増加を必要とすることが知られている。T細胞が、(通常、数多くの)腫瘍細胞がグルコースを消費している競合的環境にある場合、グルコース輸送体を発現する改変されたT細胞は、グルコース輸送体の数の増加による恩恵を受けることができる。
酵素は、本発明の治療的トランスジーンによってコードされる例示的な産物である。例示的な治療的酵素としては、PKM2が挙げられるが、これに限定されない。ピルビン酸キナーゼ筋肉アイソザイム2(PKM2)は、解糖速度が速くかつ生合成前駆体の需要が高い、エフェクターT細胞などの分裂細胞で必要とされる酵素である。PKM2の過剰発現は、生合成前駆体が増加するのを助け、それにより、改変されたT細胞の増殖を向上させる。
リボザイムは、本発明の治療的トランスジーンによってコードされる例示的な産物である。治療的トランスジーンの産物である例示的なリボザイムとしては、それぞれ、病原体ゲノム又はウイルスゲノムを切断する病原体特異的又はウイルス特異的リボザイムが挙げられるが、これらに限定されない。ウイルス性病原体の場合、リボザイムは、したがって、ウイルス侵入時のウイルスRNA逆転写とウイルス集合時のウイルスRNAゲノムパッケージングの両方を阻害することができる。標的化ウイルスゲノムを含む、様々な病原体に対するリボザイムは、治療的トランスジーンの産物として発現されることができることがすぐに分かる。具体的な実施態様において、該トランスジーンは、HIV RNAゲノムを切断するHIV特異的リボザイムをコードし、そのため、ウイルス侵入時のウイルスRNA逆転写とウイルス集合時のウイルスRNAゲノムパッケージングの両方を阻害する。

転写アクチベーター又はリプレッサー(転写因子)は、本発明の治療的トランスジーンによってコードされる例示的な産物である。転写アクチベーター又はリプレッサーは、天然に存在するものであるか又はキメラであることができる。場合により、ある遺伝子のアクチベーターが別の遺伝子のリプレッサーであることができ、又はある遺伝子のリプレッサーが別の遺伝子のアクチベーターであることができる。治療的トランスジーンによって発現されることができる例示的な転写因子としては、Foxp3、NFAT、HIF-1αなどが挙げられるが、これらに限定されない。
非コードRNAは、本発明の治療的トランスジーンによってコードされる例示的な産物である。例示的な非コードRNA(マイクロRNA又は低分子干渉RNA)としては、抑制性受容体遺伝子メッセンジャーRNA、例えば、PD1、TIM-3、LAG3、CTLA-4などを標的とするものが挙げられる。したがって、具体的な実施態様において、治療的トランスジーンは、非コードRNA、例えば、マイクロRNA(miRNA)、低分子干渉RNA(siRNA)、アンチセンスRNAなどをコードする。改変されたT細胞の活性の刺激が望ましい具体的な実施態様において、非コードRNAは、例えば、標的抑制性受容体遺伝子メッセンジャーRNA、例えば、PD1、TIM-3、LAG3、CTLA-4などを標的とすることができる。
本発明は、対象をT細胞療法で治療する方法であって、該対象がそのような治療を必要としている、方法にも関する。T細胞療法が免疫応答を促進することになる(すなわち、免疫応答の刺激を必要としている対象を治療する)実施態様において、例として、治療されている対象は、癌又は感染性疾患を有し得、本発明の組換えT細胞の投与は、それぞれ、癌又は感染性疾患を治療することになる。T細胞は、癌もしくは感染性疾患と関連する標的抗原に対する(治療的トランスジーンによってコードされ得る)結合パートナー(例えば、CARもしくは抗体もしくは受容体)を組換え発現することにより、又は癌もしくは感染性疾患と関連する標的抗原に対して感作することにより、該癌又は感染性疾患に標的化することができる。感作されたT細胞を使用する具体的な実施態様において、該T細胞は、それぞれ、癌又は感染性疾患の抗原に対して感作される。好ましい実施態様において、本発明は、対象をCAR療法で治療する方法であって、該対象がそのような療法を必要としている、方法にも関する。CAR療法が免疫応答を促進することになる実施態様において、例として、治療されている対象は、癌又は感染性疾患を有し得、本発明の組換えT細胞の投与は、癌又は感染性疾患を治療することになり、CARは、それぞれ、癌又は感染性疾患病原体の抗原に結合する。そのような実施態様において、該T細胞は、CD8+、CD4+、TSCM、TCM、エフェクター記憶T細胞、エフェクターT細胞、Th1細胞、Th2細胞、Th9細胞、Th17細胞、Th22細胞、Tfh(濾胞性ヘルパー)細胞、又は本明細書に開示される他のT細胞であることができる。

本発明の代わりの具体的な実施態様において、T細胞は、そのゲノム中に(任意に、治療的トランスジーンの代わりに)レポータートランスジーンを組み込んでおり、ここで、該レポータートランスジーンの発現は、内在性プロモーターの制御下にあり、該内在性プロモーターは、本明細書に上で記載されるプロモーターのいずれかであることができる。レポータートランスジーンは、検出可能なマーカー(好ましくは、細胞表面)タンパク質をコードするトランスジーンである。具体的な実施態様において、レポータートランスジーンは、IL4も膜結合型のIL4もコードしない。別の具体的な実施態様において、レポータートランスジーンは、細胞自殺スイッチをコードする。具体的な実施態様において、細胞自殺スイッチは切断型EGF受容体(EGFRt)であり、これは、必要な場合、治療的T細胞の検出と除去の両方を可能にする。このレポーターを認識し、インビボで抗体媒介性標的細胞死を誘発する特異的抗体(例えば、セツキシマブ)が存在する(米国特許第8,802,374号を参照)。したがって、例えば、EGFRtをコードするトランスジーンは、内在性の構成的又は誘導性プロモーターの制御下にあることができ、改変されたT細胞を治療目的で対象に投与した後、該対象に、EGFRtを認識し、望ましい場合、治療後にT細胞活性を停止するように、改変されたT細胞の細胞死を誘発する抗体を投与することができる。
本発明は、以下の例示的な実施態様を提供する。
T細胞に:
(i)トランスジーン、及び
(ii)該細胞のゲノム内の部位における該トランスジーンの標的化組込みに好適な相同組換えシステムであって、該トランスジーンを該細胞のゲノム内の該部位で組み込み、ここで、該トランスジーンの発現が内在性プロモーターの制御下にあり、ここで、該トランスジーンが治療的タンパク質又は治療的核酸をコードする、相同組換えシステム
を導入することを含む、方法。
T細胞に:
(i)CARをコードする核酸配列、及び
(ii)該細胞のゲノム内の部位における該核酸配列の標的化組込みに好適な相同組換えシステムであって、該CARの該部位での組込みが該細胞の表面での機能的T細胞受容体複合体の発現を低下させ又は妨げるように、該CARをコードする核酸配列を該細胞のゲノム内の該部位で組み込む、相同組換えシステムを導入することを含み、それにより、該CARを発現し、かつ機能的TCR複合体を欠くT細胞を作製する、方法。
(8.1 実施例1:汎用CAR T細胞の1ステップ作製)
下記は、汎用CAR T細胞の1ステップ作製の戦略である。
TRAC遺伝子上の第一のエクソンを標的とするように、TALEN及びgRNAを設計した。標的とされる配列は、TCRαの膜貫通ドメインの上流に位置する。このドメインは、TCRα及びβの会合並びに細胞表面へのアドレッシングに必要とされる。この遺伝子座での非相同末端結合(NHEJ)又はHDRによるCARの組込みは、TCR複合体を効率的に破壊する。
TALENをコードするプラスミドをTransposagenによって合成し、AgeIで線状化した。TALEN mRNAを転写し、mMessage mMachine T7 Ultraキット(Life Technologies; Carlsbad, CA)を用いて、インビトロでポリアデニル化した。RNAをRNeasyカラム(Qiagen; Valencia, CA)で精製し、Nanodrop機器を用いて定量した。RNAの品質は、変性ホルムアルデヒド/MOPSアガロースゲル上で確認した。修飾されたガイドRNA(gRNA)及びCas9 mRNAは、TriLink Biotechnologiesによって合成された。gRNAは、1ug/uLでサイトポレーションTバッファー(Harvard Apparatus; Holliston, MA)中で再構成した。
TRAC標的化(図1Aを参照)のために、pAAV-GFP骨格(Cellbiolabs; San Diego, CA)に基づいて、TALEN及びgRNA標的化配列が両側に隣接している1.9kbのゲノムTRAC(PCRにより増幅)と、TRACの第一のエクソンとインフレームの自己切断型P2Aペプチドと、その後ろにある、臨床試験で使用されている1928z CARとを含有するpAAV-TRAC-P2A-1928zを設計し、クローニングした(Brentjensらの文献、Sci. Transl. Med. 5(177):177ra38. doi: 10.1126/scitranslmed.3005930(2013))。簡潔に述べると、CARは、その前にCD8aリーダーペプチドがあり、その後ろにCD28ヒンジ-膜貫通-細胞内領域及びCD3ζ細胞内ドメインがある、ヒトCD19に特異的な単鎖可変断片19scFVを含む。このカセットは、ウシ成長ホルモンポリAシグナル(BGHpA)で終わる。
健康ボランティアドナー由来のバフィーコートをNew York Blood Centerから入手した。末梢血単核細胞を密度勾配遠心分離によって単離し、その後、Tリンパ球を汎T細胞単離キット(Miltenyi Biotech; San Diego, CA)を用いて純化した。細胞を、106細胞/mlの密度で、200U/ml IL-2(Miltenyi Biotech)とともにヒト血清(Gemini Bioproducts; West Sacramento, CA)が補充されたX-vivo 15培地(Lonza; Basel, Switzerland)中のDynabeads(1:1ビーズ:細胞) Human T-Activator CD3/CD28(ThermoFisher; Carlsbad CA)で活性化した。培地を2日毎に交換し、細胞を106細胞/mlでプレーティングし直した。
48時間の活性化の後、CD3/CD28ビーズを磁気的に除去し、細胞をビーズの非存在下で12~16時間培養した。Tリンパ球を、AgilePulse MAXシステム(Harvard Apparatus)を用いるTALEN又はCas9/gRNA RNAの電気的導入によってトランスフェクトした。簡潔に述べると、細胞をサイトポレーション培地T(Harvard Apparatus)中で洗浄した。その後、細胞をペレット化し、サイトポレーション培地Tに30×106細胞/mlで再懸濁させた。3×106個の細胞をオーダーメイドのヌクレアーゼをコードする表示された用量の各々のmRNAと混合して、0.2cmキュベットに入れた。エレクトロポレーションは、600Vでの2回の0.1msパルスと、それに続く、100Vでの4回の0.2msパルスからなっていた。エレクトロポレーション後、細胞を希釈して、培養培地に入れ、37℃、5%CO2でインキュベートした。エレクトロポレーションから2~4時間後、AAVを培地に添加し、その後、30℃でさらに20時間インキュベートし続けた。AAVドナーを表示されたMOI(1e5~1e6 MOI)で添加した。その後、編集された細胞を、標準的な条件を用いて培養した(37℃、~1e6細胞/mlの密度を維持するように、必要に応じて、2~3日毎に補充された、T細胞成長培地中で増殖させた)。
SFG γ-レトロウイルスベクター(Riviereらの文献、Proc. Natl. Acad. Sci. USA 92(15):6733-6737(1995))をコードするプラスミドを標準的な分子生物学の技法を用いて調製した。SFG-1928z及びSFG-P28zの合成は、以前に記載されている(Brentjensらの文献、Nat Med. 9(3):279-286(2003), Brentjensらの文献、2007 Clin. Cancer Res. 13(18 Pt 1):5426-5435(2007); Maherらの文献、Nat. Biotechnol. 20(1):70-5(2002))。形質導入されたgpg29線維芽細胞(H29)から得られたVSV-Gシュードタイプ化レトロウイルス上清を用いて、以前に記載されている通りに、安定なレトロウイルス産生細胞株を構築した(Gongらの文献、Neoplasia 1:123-127(1999))。
Retronectin(Takara; Mountain View, CA)をコーティングした癌レトロウイルスベクター結合プレート上での遠心分離により、T細胞に連続2日間で形質導入した。
CARが形質導入されたT細胞の細胞傷害性を標準的なルシフェラーゼベースのアッセイによって決定した。簡潔に述べると、ホタルルシフェラーゼ-GFPを発現するNALM6は、標的細胞としての役割を果たした。エフェクター(E)細胞と腫瘍標的(T)細胞を、NALM6培地中に100μl/ウェルの全容量で、1×105個の標的細胞を含む黒壁の96ウェルプレートを用いて、表示されたE/T比で、3連で共培養した。標的細胞のみを同じ細胞密度でプレーティングして、最大ルシフェラーゼ発現(相対発光単位; RLUmax)を決定した。18時間後、100μlのルシフェラーゼ基質(Bright-Glo、Promega; Madison, WI)を各々のウェルに直接添加した。放射光を発光プレートリーダー又はXenogen IVISイメージングシステム(Xenogen; Alameda, CA)で検出し、Living Imageソフトウェア(Xenogen)を用いて定量した。溶解は、[1-(RLUsample)/(RLUmax)]×100として決定した。
使用されたマウスモデルは、MSKCC施設内動物管理使用委員会によって承認されたプロトコルに従って、8~12週齢のNOD.Cg-PrkdcscidIl2rgtm1Wjl/SzJ(NSG)マウス(Jackson Laboratory; Bar Harbor, ME)であった。マウスに0.5×106個のFFLuc-GFP NALM6細胞を尾静脈注射により接種し、続いて、4日後に、2×105個のCAR T細胞を注射した。NALM6は、極めて均一な腫瘍負荷をもたらし、処置前に除外されるマウスはいなかった。盲検法は使用しなかった。イメージングデータセットの獲得のために、生体発光イメージングは、Living Imageソフトウェア(Xenogen)とともにXenogen IVISイメージングシステム(Xenogen)を利用した。腫瘍負荷は、以前に記載されている通りに評価した(Gadeらの文献、Cancer Res. 65(19):9080-9088(2005))。
本実施例は、トランスジーンによってコードされるCARのT細胞による発現が実施され、ここで、該トランスジーンの発現は、内在性T細胞プロモーター、具体的には、ヒトT細胞受容体a鎖(TRAC)プロモーターの制御下にあったことを示している。以下に記載されているのは、CD19特異的CARをヒトT細胞受容体a鎖(TRAC)遺伝子座に向けると、ヒト末梢血T細胞における均一なCAR発現がもたらされるだけでなく、T細胞の効力も増強され、編集された細胞が急性リンパ芽球性白血病のマウスモデルにおいて従来作製されたCAR T細胞よりも大いに優れていることを示す実験である。CARをTRAC遺伝子座に標的化すると、持続的なCARシグナル伝達が回避され、抗原に対する1回又は繰り返しの曝露の後にCARの効果的な内在化及び再発現が確立され、エフェクターT細胞の分化及び疲弊が遅延することがさらに示されている。これらの研究結果は、CAR免疫生物学の諸側面を明らかにし、免疫療法を前進させるCRISPR/Cas9ゲノム編集の大きな可能性を強調する。
(ガイド-RNA:)
TCRα遺伝子の定常鎖(TRAC)の第一のエクソンを標的とするように、ガイドRNA(gRNA) gRNAを設計した。標的とされる配列は、TCRαの膜貫通ドメインの上流にある。このドメインは、TCRα及びβの会合並びに細胞表面へのアドレッシングに必要とされる。その後、この遺伝子座での非相同末端結合(NHEJ)とHDRによるCARの組込みの両方によって、TCR複合体が効率的に破壊される。

Med. 5:177ra138(2013))。簡潔に述べると、CARは、その前にCD8aリーダーペプチドがあり、その後ろにCD28ヒンジ-膜貫通-細胞内領域及びCD3ζ細胞内ドメインがある、ヒトCD19に特異的な単鎖可変断片19scFVを含む。CAR cDNAの後ろに、ウシ成長ホルモンポリAシグナル(bGHpA)がある。B2M遺伝子座を標的とする場合、1928z-pA配列がB2M遺伝子のATGとインフレームで配置されているのでP2A配列が必要ないことを除き、同様の戦略に従った。外因性プロモーター(EF1α、LTR、PGK、又はPGK100)を使用する場合、プロモーター-1928z-pAカセットを同じTRAC又はB2M侵入地点で逆向きに配置した。

T細胞活性化を開始してから48時間後、CD3/CD28ビーズを磁気的に除去し、T細胞を、AgilePulse MAXシステム(Harvard Apparatus)を用いるCas9 mRNA及びgRNAの電気的導入によってトランスフェクトした。3×106個の細胞を5μgのCas9及び5μgのgRNAと混合して、0.2cmキュベットに入れた。エレクトロポレーション後、細胞を希釈して、培養培地に入れ、37℃/5%CO2でインキュベートした。エレクトロポレーションから2~4時間後、組換えAAV6ドナーベクター(SignaGenにより製造されたもの; Gaithersburg, MD)を表示されたMOI(1×105~1×106の範囲)で培地に添加した。その後、編集された細胞を、標準的な条件を用いて培養した(37℃、~1×106細胞/mlの密度を維持するように、必要に応じて、2~3日毎に補充された、T細胞成長培地中で増殖させた)。
SFG γ-レトロウイルス(RV)ベクター(Riviereらの文献、Proc. Natl. Acad. Sci. USA 92:6733-6737(1995))をコードするプラスミドを以前に記載されている通りに調製した(Brentjensらの文献、Nat. Med. 9, 279-286,(2003); Maherらの文献、Nat. Biotechnol. 20:70-75(2002))。形質導入されたgpg29線維芽細胞(H29)から得られたVSV-Gシュードタイプ化レトロウイルス上清を用いて、以前に記載されている通りに、安定なレトロウイルス産生細胞株を構築した(Gongらの文献、前立腺特異的膜抗原に遺伝子標的化された癌患者のT細胞は、前立腺癌細胞を特異的に溶解させ、前立腺特異的膜抗原に応答してサイトカインを放出する(Cancer patient T cells genetically targeted to prostate-specific membrane antigen specifically lyse prostate cancer cells and release cytokines in response to prostate-specific membrane antigen.)、Neoplasia 1:123-127(1999))。Retronectin(Takara)をコーティングしたプレート上での遠心分離により、T細胞に形質導入した。
NALM-6及びNIH/3T3をATCCから入手し、MycoAlertマイコプラズマ検出キット(Lonza)を用いて、マイコプラズマ汚染を定期的に検査した。NALM-6細胞に形質導入して、ホタルルシフェラーゼ-GFPを発現させ、NIH/3T3細胞に形質導入して、ヒトCD19を発現させた(Brentjensらの文献、Nat. Med. 9, 279-286,(2003); Zhaoらの文献、Cancer Cell 28:415-428(2015))。

ヒトCD19を発現するNIH/3T3を人工抗原提示細胞として使用した(Brentjensらの文献、Nat. Med. 9, 279-286,(2003))。週1回の刺激のために、3×105個の放射線照射CD19+ AAPCを24ウェルプレートにプレーティングし、12時間後、X-vivo 15+ヒト血清+50U IL-2/mL中の5×105個のCAR T細胞を添加した。2日毎に、細胞を計数し、1×106 T細胞/mLの濃度に達するように培地を添加した。繰り返しの近位刺激のために(図6D)、細胞を、24時間後に(2回の刺激)又は12時間毎に(4回の刺激)、3T3-CD19がプレーティングされた新しいウェルに移した。各々の条件について、12時間毎に、T細胞を計数し、CAR、表現型及び疲弊マーカー発現について、FACSにより解析した。
CARをヤギ抗マウスFab(Jackson ImmunoResearch, 115-606-003; West Grove, PA)で標識した。T細胞の表現型決定のために、以下の抗体を使用した: BD biosciences(San Jose, CA)製のマウス抗ヒトBUV-395CD4(563552)、APC-cy7-CD8(557834)、BV-421-CD62L(563862)、BV-510-CD279(PD1、563076); eBiosciences(Carslbad, CA)製のマウス抗ヒトAPC-CD25(17-0259-42)、FITC-CD45RA(11-0458-42)、PerCP-eFluor710 CD223(LAG-3、46-2239-42)、及びBiolegend(San Diego, CA)製のFITCマウス抗ヒトCD366(TIM-3、345032)。細胞内染色のために、T細胞を固定し、BD Cytofix/Cytoperm Plusキット(BD Biosciences)を製造元の推奨に従って用いて透過処理した。抗CD8-FITC(クローンHIT8a、ebiosciencce)及び抗CD4-BUV-395(クローンSK3、BD Horizon; BD Biosciences)を細胞外染色に使用した。抗TNF-Alexa Fluor 700(クローンMAb11、BD pharmingen; BD Biosciences)、抗IL2-BV421(クローン5344.111、BD Horizon)、及び抗IFNg-BV510(クローンB27、BD Horizon)を細胞内染色に使用した。
8~12週齢のNOD/SCID/IL-2Rγ-ヌル(NSG)雄マウス(Jackson Laboratory)をMSKCC施設内動物管理使用委員会によって承認されたプロトコルに従って使用した。マウスに0.5×106個のFFLuc-GFP NALM6細胞を尾静脈注射により接種し、続いて、4日後に、2×105個、1×105個、又は5×104個のCAR T細胞を注射した。NALM6は、極めて均一な腫瘍負荷をもたらし、処置前に除外されるマウスはいなかった。無作為化も盲検法も使用しなかった。イメージングデータセットの獲得のために、生体発光イメージングは、Living Imageソフトウェア(Xenogen)とともにXenogen IVISイメージングシステム(Xenogen)を利用した。腫瘍負荷は、以前に記載されている通りに評価した(Gadeらの文献、Cancer Res. 65:9080-9088(2005))。
製造元の指示に従って、RNeasyキット(QIAGEN; Hilden, Germany)をQIAshredder(QIAGEN)と組み合わせて用いて、全RNAをT細胞から抽出した。RNAの濃度及び品質を、NanoDrop分光光度計(Thermo Fisher Scientific; Carslbad, CA)を用いるUV分光法 によって評価した。100~200ngの全RNAを用いて、1:1の容量比のランダムヘキサマーとオリゴdTとともにSuperScript III First-Strand Synthesis SuperMix(Invitrogen; Carlsbad, CA)を用いて、cDNAを調製した。終了したcDNA合成反応液を2UのRNアーゼHにより37℃で20分間処理した。ABsolute Blue qPCR SYBR Green Low ROX Mix(Thermo Fisher Scientific)、及び以下のプライマーセット:
実験データは全て、平均±s.e.m.として提示されている。標本の大きさを前もって決める統計法は使用しなかった。パラメトリックデータ(標本の大きさ>10)に対するウェルチの2標本t-検定又は非パラメトリックデータ(標本の大きさ<10)に対するマン・ホイットニー検定を用いて、群を比較した。分散が等しくなかったので、ウェルチ補正を使用した。CAR刺激時のCAR MFIとRNAレベルの比較のために、ANOVA F-検定を使用した。統計解析は、GraphPad Prism 7ソフトウェア(GraphPad Software; La Jolla, CA)で行った。
1つの例において、治療的キメラ抗原受容体(CAR)を(内在性プロモーター/エンハンサーエレメントの制御下の)TRAC遺伝子座で組み込み; NFAT応答性転写ユニットをT細胞受容体β鎖定常(TRBC)遺伝子座で組み込み、これらから、2つの治療的PD1L特異的及びCTLA4 scFvを発現させる。この設定を用いて、改変されたT細胞をCARによって活性化させ、これは、NFAT活性化、それに続いて、治療的scFvの発現をもたらす。或いは、これらのトランスジーンをNFAT応答性CD69遺伝子座で組み込むことができる。特定の実施態様において、キメラ細胞表面リガンド-転写因子は、CD19-NFATである。したがって、一実施態様において、CARは、第一のトランスジーンによってコードされ、PD1L scFv及びCTLA4 scFVは、2シストロン性である第二のトランスジーンによってコードされ、ここで、該第二のトランスジーンの発現は、NFATによって誘導される内在性TRBCプロモーターの制御下にある。代わりの実施態様において、PD1L及びCTLA4 scFvを別々のトランスジーン(すなわち、第二及び第三のトランスジーン)から発現させる。具体的な実施態様において、PD1L及びCTLA4 scFvを単一のポリシストロン性トランスジーンから発現させる。そのようなコンストラクトは、別々の分子としてのPD1L及びCTLA4 scFvの発現をもたらすために、P2A配列などの切断可能な配列を任意に含むことができる。
組換え非組み込み型(又は組込み欠損)γ-レトロウイルス(rNIgRV又はIDgRV)は、宿主細胞ゲノムへのウイルスDNAの組込みを触媒することができない突然変異体インテグラーゼタンパク質を含有するレトロウイルスベクターである。この突然変異体レトロウイルスベクターを作製するために、突然変異体インテグラーゼタンパク質(先に示された突然変異を有する)をコードするプラスミドDNAを、エンベロープをコードするプラスミドDNA及びレトロウイルスゲノムをコードするプラスミドDNAと組み合わせて使用した。これら3つのプラスミドをプロデューサー哺乳動物細胞にトランスフェクトし、組換え突然変異体ウイルスベクターを培地中に放出させ、これを後に回収し、ヒト末梢血T細胞を形質転換するために使用した。

ヒトT細胞であって、CARが内在性TCRα鎖プロモーターの制御下で発現されて、該細胞の表面で該CARを産生するように、該CARをコードするプロモーターレス組換え核酸配列が、TCRα鎖の第一のエクソンである該細胞のゲノム内の部位で組み込まれ、かつ該CARの該部位での組込みが機能的TCRα鎖の発現を低下させ又は妨げる、前記ヒトT細胞。

キメラ抗原受容体(CAR)を発現し、かつ機能的T細胞受容体(TCR)複合体を欠くT細胞を作製する方法であって:
T細胞に:
(i)CARをコードする核酸配列、及び
(ii)該細胞のゲノム内の部位における該核酸配列の標的化組込みに好適な相同組換えシステムであって、該CARの該部位での組込みが該細胞の表面での機能的T細胞受容体複合体の発現を低下させ又は妨げるように、該CARをコードする核酸配列を該細胞のゲノム内の該部位で組み込む、相同組換えシステムを導入することを含み、それにより、該CARを発現し、かつ機能的TCR複合体を欠くT細胞を作製する、前記方法。
(態様36)
誘導多能性幹細胞であって、キメラ抗原受容体(CAR)が該細胞によって該細胞の表面で発現されるように、該CARをコードする組換え核酸配列が該細胞のゲノム内の第一の部位で組み込まれ、かつ該CARをコードする核酸の該第一の部位での組込みが該細胞の表面での機能的T細胞受容体(TCR)複合体の発現を低下させ又は妨げる、前記誘導多能性幹細胞。
Claims (25)
- T細胞であって、キメラ抗原受容体(CAR)が該T細胞によって該T細胞の表面で発現されるように、該CARをコードする組換え核酸配列が該T細胞のゲノム内の第一の部位で組み込まれ、該CARをコードする核酸の該第一の部位での組込みが該T細胞の表面での機能的T細胞受容体(TCR)複合体の発現を妨げ、かつ、該T細胞における組み込まれた核酸配列の発現が、TCR複合体の内在性プロモーターの制御下にある、前記T細胞。
- 前記CARをコードする核酸配列が、前記T細胞のゲノム内の単一の部位で組み込まれる、請求項1記載のT細胞。
- 前記CARをコードする核酸配列が、前記T細胞のゲノム内の2つの部位で組み込まれる、請求項1記載のT細胞。
- 前記第一の部位が、TCR複合体のタンパク質をコードする遺伝子のエクソンにある、請求項3記載のT細胞。
- 前記第一の部位における前記CARをコードする核酸配列の組込みが、T細胞受容体α鎖、T細胞受容体β鎖、CD3γ鎖、CD3δ鎖、CD3ε鎖、及びCD3ζ鎖からなる群から選択されるタンパク質の発現を妨げる、請求項1~4のいずれか一項記載のT細胞。
- 前記内在性プロモーターが、T細胞受容体α鎖、T細胞受容体β鎖、CD3γ鎖、CD3δ鎖、CD3ε鎖、又はCD3ζ鎖をコードする遺伝子のプロモーターである、請求項1記載のT細胞。
- 前記CARが、癌抗原に結合する、請求項1~6のいずれか一項記載のT細胞。
- ヒトに由来する、請求項1~7のいずれか一項記載のT細胞。
- 前記CARをコードする核酸配列が、標的化相同組換えによって前記第一の部位に組み込まれる、請求項1~8のいずれか一項記載のT細胞。
- 前記CARをコードする核酸配列が、前記T細胞のゲノム内の複数の部位で、該複数の部位における該CARをコードする核酸配列の発現が異なる内在性プロモーターの制御下にあるように組み込まれる、請求項9記載のT細胞。
- 前記CARが前記T細胞によって該T細胞の表面で発現されるように、前記CARをコードする核酸配列も該T細胞のゲノム内の第二の部位に組み込まれる、請求項1~10のいずれか一項記載のT細胞。
- 前記第二の部位での前記CARをコードする核酸の組込みも、前記T細胞の表面での機能的TCR複合体の発現を妨げ、ここで、前記第一の部位及び前記第二の部位が、異なる遺伝子中にある、請求項11記載のT細胞。
- 第二のCARが前記T細胞によって該T細胞の表面で発現されるように、かつ第二の核酸配列の発現が前記第二の部位で内在性プロモーターの制御下にあるように、該第二のCARをコードする第二の核酸配列が、該T細胞のゲノム内の該第二の部位で組み込まれ、ここで、前記第一の部位及び第二の部位が、異なる遺伝子中にある、請求項1~12のいずれか一項記載のT細胞。
- ヒトT細胞であって、CARが内在性TCRα鎖プロモーターの制御下で発現されて、該T細胞の表面で該CARを産生するように、該CARをコードするプロモーターレス組換え核酸配列が、該T細胞のゲノム内のT細胞受容体(TCR)α鎖の第一のエクソンである部位で組み込まれ、かつ該CARの該部位での組込みが機能的TCRα鎖の発現を妨げる、前記ヒトT細胞。
- 前記CARが、CD19に結合する、請求項14記載のヒトT細胞。
- 複数の請求項1~15のいずれか一項記載のT細胞を含む、単離されたT細胞集団。
- 請求項1~15のいずれか一項記載のT細胞を含む、医薬組成物。
- 複数の請求項1~15のいずれか一項記載のT細胞を含むT細胞集団を含む、医薬組成物。
- 癌を有する対象を治療する方法に使用するための請求項17記載の医薬組成物であって、前記CARが、該癌の癌抗原に結合する、前記医薬組成物。
- 癌を有する対象を治療する方法に使用するための請求項18記載の医薬組成物であって、前記CARが、該癌の癌抗原に結合する、前記医薬組成物。
- 癌を有する対象を治療する方法に使用するための請求項16記載の単離された集団であって、前記CARが、該癌の癌抗原に結合する、前記集団。
- 前記対象がヒトであり、前記T細胞がヒトに由来する、請求項19又は20記載の医薬組成物。
- 前記対象がヒトであり、前記T細胞がヒトに由来する、請求項21記載の単離された集団。
- キメラ抗原受容体(CAR)を発現し、かつ機能的T細胞受容体(TCR)複合体を欠くT細胞をインビトロで作製する方法であって、
T細胞に:
(i)CARをコードする核酸配列、及び
(ii)該核酸の発現がTCR複合体の内在性プロモーターの制御下にあるような該T細胞のゲノム内の部位における該核酸配列の標的化組込みに好適な相同組換えシステムであって、該CARの該部位での組込みが該細胞の表面での機能的T細胞受容体複合体の発現を妨げるように、該CARをコードする核酸配列を該T細胞のゲノム内の該部位で組み込む、該相同組換えシステムを導入することを含み、それにより、該CARを発現し、かつ機能的TCR複合体を欠くT細胞を作製する、前記方法。 - 誘導多能性幹細胞であって、キメラ抗原受容体(CAR)が該細胞によって該細胞の表面で発現されるように、該CARをコードする組換え核酸配列が該細胞のゲノム内の第一の部位で組み込まれ、かつ該CARをコードする核酸の該第一の部位での組込みが該細胞の表面での機能的T細胞受容体(TCR)複合体の発現を妨げ、かつ、該細胞における組み込まれた核酸配列の発現が、TCR複合体の内在性プロモーターの制御下にある、前記誘導多能性幹細胞。
