本発明を次に、以下の例に関して、単なる例示によるものを含め、より詳しく記載する。
以下、本発明の実施形態を示す。
(1)対象における癌を処置するための方法であって、対象に対する全身経路による免疫賦活剤の同時投与と併用して、腫瘍に対する直接注射又は対象に対する全身投与により、腫瘍崩壊ウイルス又は腫瘍崩壊ウイルスのRNAを送達することを含む、前記方法。
(2)対象における膀胱癌を処置するための方法であって、対象に対する全身経路による免疫賦活剤の同時投与と併用して、腫瘍に対する直接注射、又は対象に対する全身投与若しくは膀胱内投与により、腫瘍崩壊ウイルス又は腫瘍崩壊ウイルスのRNAを送達することを含む、前記方法。
(3)腫瘍崩壊ウイルス又は腫瘍崩壊ウイルスのRNAが、ピコルナウイルス科から成る群から選択される、(1)又は(2)記載の方法。
(4)腫瘍崩壊ウイルス又は腫瘍崩壊ウイルスのRNAが、腫瘍細胞表面上の細胞間接着分子-1(ICAM-1)及び/又は崩壊促進因子(DAF)に結合するピコルナウイルス科のウイルスから成る群から選択される、(1)又は(2)記載の方法。

(22)対象がヒトである、(1)又は(2)記載の方法。
本試験は、悪性メラノーマのB16-ICAM-1マウスモデルにおける、免疫賦活抗体抗PD-1と併用のCVA21腫瘍崩壊ウイルス療法の有効性を調査したものである。マウスに腫瘍を、皮内に埋め込み(2×105細胞)、12日間確立させた後、治療を開始した。12、15、18、及び21日目、マウスを、マウス抗PD-1抗体又はマッチさせた対照抗体のいずれかと併用して、食塩水、CVA21、又はUV不活化CVA21のいずれかで処置した。食塩水、CVA21、又はUV不活化CVA21処置は腫瘍内投与し、抗PD-1及び対照抗体は腹腔内投与した。抗PD-1抗体と併用してCVA21で処置した動物は、対照群(食塩水+対照抗体)の動物と比較して、腫瘍の潰瘍化のエンドポイント、又は10%を超える体重の減少に基づいて、わずかな延命効果を示した。12日目から21日目の間の処置期間の間、CVA21+抗PD-1の併用は腫瘍抑制(tumoristatic)効果を示したが、治療を停止すると、腫瘍の体積は徐々に増大した。これにもかかわらず、この群の動物は、食塩水+対照抗体、UV-CVA21+対照抗体、及びUV-CVA21+抗PD-1抗体処置群と比較して、統計学的に有意な生存効果を示した。本試験により、抗CVA21中和抗体レベルはCVA21+抗PD-1対CVA21+対照抗体を投与したマウスにおいてより高レベルで検出されたため、抗PD-1抗体の免疫賦活性の性質も確認された。まとめると、本試験の主な知見は、CVA21+抗PD-1で処置した担癌マウスは、腫瘍成長の減速及び腫瘍の潰瘍化の低減に関する全体的な延命効果を示した。
試験物品であるコクサッキーウイルスA21(CVA21)[商品名:CAVATAK(商標)]はViralytics Ltdによって提供された。in vitro使用のための研究用株(research stock)は、調製された市販のCAVATAK(商標)のバイアルから作製した。バッチ:CCVA2115、濃度:1×109TCID50/ml (1.1mlバイアル)、再試験日:実施例2及び3に対して2008年12月、2014年1月、貯蔵の推奨:-70℃未満で貯蔵する。
対照抗体
実施例1及び2における対照抗体として、BioXCellからの抗体InVivoMAb Rat IgG2aを用いた。クローン: 2A3。カタログ番号: BE0089。ロット: 4807/0713。内毒素: <0.35EU/mg。製剤: PBS、pH6.5。滅菌: 0.2umろ過。純度: >95%。貯蔵: 希釈せずに暗所で4℃。実施例3における対照抗体として、BioXCellからの抗体InVivoMAb Rat IgG2bを用いた。クローンMPC-11。カタログ番号BE0086。ロット4700-2/0414。内毒素: <2.0EU/mg。製剤: PBS、pH7。滅菌: 0.2umろ過。純度: > 95%。貯蔵: 希釈せずに暗所で4℃。
活性抗マウスPD-1抗体をBioXCellから得た。製品名: InVivoMAb抗m PD-1。クローン: RMP1-14。カタログ番号: BE0416。ロット: 4781/0813。内毒素: <0.61EU/mg。製剤: PBS、pH7。滅菌: 0.2 umろ過。純度: >95%。貯蔵: 希釈せずに暗所で4℃。
活性抗マウスCTLA-4抗体をBioXCellから得た。製品名: InVivoMAb抗m CTLA-4。クローン: 9D9。カタログ番号: BE0164。ロット: 5159/0414。内毒素: <2.0EU/mg。製剤: PBS、pH7。滅菌: 0.2um ろ過。純度: >95%。貯蔵: 希釈せずに暗所で4℃。
本試験では、ヒトメラノーマ細胞株SK-Mel-28、並びにマウスメラノーマ細胞株B16及びB16-ICAM-1を用いた。メラノーマ細胞株SK-Mel-28は、ATCC (アメリカ培養細胞系統保存機関)から得た。Mel-RMは、P. Hersey博士(University of Newcastle、New South Wales、Australia)より贈られた。B16マウスメラノーマ細胞は、当初A. Shurbier博士(Queensland Institute for Medical Research、Brisbane、Queensland、Australia)から得、次いで、ヒトICAM-1遺伝子を安定にトランスフェクトしてB16-ICAM-1細胞株を作製した。細胞株は全て、10%ウシ胎児血清(FCS) (SAFC Biosciences(商標)、Australia)、10mM滅菌N-2-ヒドロキシエチルピペラジンN'-2-エタンスルホン酸(HEPES) (ThermoScientific、Australia)、2mM L-グルタミン(ThermoScientific、Australia)、ピルビン酸ナトリウム(Invitrogen、Australia)、及び100IU/mlペニシリン-ストレプトマイシン(Invitrogen、Auckland、NZ)を含むDMEM (Thermo Scientific)中に維持した。細胞は全て、5%CO2環境中37℃で培養した。
動物実験は全て、ニューキャッスル大学動物飼育倫理委員会(ACEC)により承認番号: A-2013-327の下に承認された。6から8週齢のC57BL/6メスマウス(n=52)を、ニューキャッスル大学動物サービスユニット(Animal Services Unit)によって得た。マウスを、動物供給施設内の、特定の無病原菌領域における、個別換気ケージ内に維持した。マウスを、PC2実験室内のTechni-Plast Slim Line空気調節システムに接続した、HEPA-filtered Techni-Plast Cages (1145 IVC)内、4匹の群で、12/12時間の明/暗周期で収容した。マウスに、Specialty Feeds、WA、Australia製造のマウスキューブ/ペレットを自由に摂取させた。標準マウス食は、低脂肪含量(およそ5%)に調合され、肉を含んでいない。室内の気流は、1時間あたり12から15回空気交換を行ったが、IVCケージ内の気流は1時間あたり70回交換する割合であった。マウスの尾にマジックで印をつけることにより識別した。動物試験は全て、動物倫理委員会によって承認されたプロトコールにしたがって行った。試験終結時、又はマウスが人道的なエンドポイントに到達した場合、CO2窒息によりマウスを安楽死させた。
ウイルスTCID50アッセイ
96ウエル組織培養プレート中、コンフルエント単層のSK-Mel-28細胞を、CVA21の10倍の段階希釈(100μL/ウエルを繰返し4つずつ)に接種し、5% CO2環境中37℃で72時間インキュベートした。マウス血清を、2%ウシ胎児血清(FCS)を含むDMEM中、1:102から1:108までの範囲で10倍段階希釈した。ウエルを、倒立顕微鏡下、肉眼で細胞変性効果(CPE)に対してスコア付けした。検出可能なCPEを有したウエルを陽性とスコア付けし、ウイルスのエンドポイント力価の50%を、Karber法(Dougherty、1964年)を用いて算出した。
中和抗体の存在に対して試験するために、熱不活化したマウス血清試料を最初に、DMEM (2% FCS)中1:32から1:2048に希釈した。各血清希釈物100マイクロリットルを、CVA21 (100TCID50) 100μLと、37℃で1時間インキュベートした。この血清/ウイルス混合物50マイクロリットルを、次いで、384ウエルフォーマットにおいてSK-Mel-28細胞上に3つずつプレーティングした。A+IgG (陽性対照)は、Commonwealth Serum Laboratories (Sandoglobulin NF Liquid、バッチ番号:4322800002)から購入した。ウエルを、倒立顕微鏡下、肉眼でウイルス中和の存在(CPEがない)に対してスコア付けした。次いで、中和抗体の力価を、Karber法(Dougherty、1964年)を用いて算出した。
6週齢から8週齢の間のC57BL/6メスマウス(Animal Services Unit、The University of Newcastle、Australia)52匹を、上記に記載したものと同じ条件下で収容した。腫瘍細胞を注射する3日前、マウスの後側腹を、電気バリカンを用いて剪毛して、十分な回復時間を与えた。B16-ICAM-1細胞をトリプシンで回収し、2回洗浄し、無菌PBS中に再懸濁した。調製した細胞の生存性を、トリパンブルー染色及びTC-10自動細胞計数機(Biorad、Hercules、California、USA)で分析することによって評価し、95%超の生存性を有する細胞調製物のみを異種移植に用いた。腫瘍移植前、動物を5%イソフルランで麻酔した。PBS体積50μL中B16-ICAM-1細胞2×105個を、マウスの後側腹に腫瘍を単回注射して皮内接種した。
データは全て、GraphPad Prism v6.0 (GraphPad Software Inc.)を用いて分析及びプロットした。動物データの分析には、GraphPad Prism v6.0 (GraphPad Software Inc.)を用いて、処置群間の腫瘍体積における差を、二元配置のANOVA(反復測定あり)及びボンフェローニの事後検定を用いて比較した。生存曲線の比較を、ログランク(マンテルコックス)検定を用いて行った。
免疫適格性マウスモデルにおける併用の腫瘍崩壊ウイルス療法及び抗PD-1免疫療法のin vivo評価
CVA21及び抗PD-1の併用治療の取組みが、メラノーマの免疫適格性マウスモデルにおいて有効であるか否かを評価するために、C57BL/6マウスの後側腹上にマウスB16-ICAM-1腫瘍細胞(2×105)を、皮内に埋め込んだ。腫瘍を確立させた後、腫瘍接種後12、15、18、及び21日目に治療を開始した(図1を参照されたい)。食塩水又はCVA21は示した時間点に腫瘍内投与し、対照抗体又は抗PD-1抗体は腹腔内投与した。マウスを毎日モニタリングし、1週間に最高3回体重測定し、腫瘍体積を週2回、電気ノギスによって測定した。毎週の間隔で、伏在静脈からの血液サンプリングを行った。腫瘍接種後45日目に試験を終了した。
1週間に最高3回、動物の体重を測り、結果を、FileMaker Pro及び記録保持用の自社開発の動物モニタリングデータベースを用いて電子的に記録した(Internal Ref: Experiment #53)。体重の生データを表2から10に見出すことができる。本発明者らの施設の動物飼育倫理委員会の要求を満たすために、体重を、また、動物モニタリングチェックリスト/記録に書き写した。いかなる時間点に処置群とNTCマウスとの間に、平均体重における統計学的な有意差は観察されなかった(Holm-Sidak法を用いて複数の比較用に補正した複数のt検定)が、31日目のUV-CVA21+抗PD-1群は例外であった(Prism 6 for Mac OS X Version 6.0c、GraphPad Software、La Jolla California USA、www.graphpad.com)。動物はCVA21及び抗PD-1療法に良好に耐容したと思われ、観察できる処置からの毒性はなかった。合計9匹の動物を、腫瘍の潰瘍化及び/又は10%を超える体重減少のため安楽死させた。体重の減少は、殆どの場合、腫瘍の潰瘍化及び関連する腫瘍負荷と関係していた。
腫瘍体積を週2回、電気ノギスを用いて測定した。31日目までに、食塩水+対照抗体で処置したマウスは全て、腫瘍体積腫瘍の潰瘍化によって証明される進行性疾患のため、安楽死させた。図4に示す通り、UV-CVA21+対照抗体で処置した腫瘍における抗腫瘍活性は殆ど観察されなかった。35日目までに、腫瘍体積は全て増大し、人道的な最大のエンドポイントに到達し、安楽死を必要とした。最初の腫瘍開始体積に高度の変動性が存在し、活性CVA21+抗PD-1に最も好適な対照は、食塩水+抗PD-1よりむしろUV-CVA21+抗PD-1処置群とみなされた(治療が完了する前に3匹が安楽死を必要とした)。UV-CVA21+抗PD-1とCVA21+抗PD-1処置との間の24日目の腫瘍体積の比較により、両側のt検定を用いて有意差があったことが明らかになった(p<0.0039)。処置の活動期の間、UV不活化CVA21と比較して、生のCVA21+抗PD-1群におけるマウス間の腫瘍の有意な低減が最も顕著であり、最終的に殆どの群で腫瘍が再発した。
12日目にウイルスを腫瘍内注射し、およそ45分後、ウイルス処置したマウスの血清中に感染性のCVA21が検出された。CVA21でさらに腫瘍内処置したにもかかわらず、血中を循環するウイルスは、第1週以内19日目までに排除された。生のCVA21で処置した動物は、最高レベルのCVA21中和抗体を生成し、26日目に最大に到達した。これらのマウスが機能的な免疫系を有していたことを考慮すると、血流からのCVA21の排出は予想外ではなかった。驚くべきことに、抗PD-1抗体の投与は抗ウイルス免疫応答を増強すると思われ、これらのマウスは1:228中和単位を超える抗CVA21抗体のレベルの増大を示し、45日目まで続いた。これは、より大きな度合いのウイルス複製及び後続の抗ウイルス抗体の生成を可能にする、抗PD-1抗体の作用の結果であり得る。この知見から、増強されたレベルの抗ウイルス抗体が生成されただけでなく、より高レベルの特異的な抗腫瘍抗体がこのプロセスにおいて産生された可能性が生じ、このような抗腫瘍抗体は、腫瘍負荷を低減する結果として臨床上の利点をもたらし得る(抗体依存的な細胞の細胞毒性)。
抗PD-1抗体と併用のCVA21は、食塩水+対照抗体群と比較して、全生存における統計学的に有意な改善を実証した(p=0.014ログランク[マンテルコックス]検定)(図7E)。食塩水+抗PD-1と食塩水+対照抗体群とを比較すると、統計学的な有意差はなかった。これは、対照抗体と抗PD-1抗体との間に、生存に対する差がなかったことを示唆する。食塩水+抗PD-1の生存曲線を、CVA21+抗PD-1処置群と比較した場合、統計学的な差はなかった(p=0.4901ログランク[マンテルコックス]検定)(図7B)。この知見は、生存の主な効果は優先的に抗PD-1ベースであったことを示唆するが、抗PD-1を伴った場合、UV不活化CVA21と活性CVA21群との間を比較すると、生のCVA21がさらなる有意な生存時間の利点をもたらしたことが明らかになったのは若干意外であった。開始の腫瘍の変動性が高度であり、初期に安楽死させたため、相対的な延命効果を厳密に分析することは極めて困難であった。しかし、食塩水+抗PD-1群と比較した、生のCVA21+抗PD-1処置群の注目すべき延命効果の傾向が、活性な処置期間の間に観察された。
生のCVA21を抗PD-1抗体と併用して使用すると、食塩水+対照抗体処置群と比較して最良の全生存をもたらすことを、結果は示した。CVA21を抗PD-1抗体と併用すると、食塩水+対照抗体群と比較して全生存の統計学的に有意な改善を実証した(p=0.014ログランク[マンテルコックス]検定)(図7E)。食塩水+抗PD-1と食塩水+対照抗体群とを比較すると、統計学的な有意差はなかった。これは、対照抗体と抗PD-1抗体との間に、生存に対する差がなかったことを示唆する。食塩水+抗PD-1の生存曲線を、CVA21+抗PD-1処置群と比較すると、有意差がなかった(p=0.4901ログランク[マンテルコックス]検定)(図7B)。この知見は、本発明者らの限られた群のサイズに基づいて、生存の主な効果は優先的に抗PD-1ベースであったが、CVA21を加えても、食塩水ベースと比較して有意に増強しなかったことを示唆する。興味深いことに、抗PD-1を伴った場合、UV不活化CVA21と活性CVA21群との間の比較は、生のCVA21がわずかに長い生存時間をもたらし、統計学的に有意であったことを示した。これは、積極的に複製するCVA21、及び腫瘍細胞の溶解は、不活化したウイルス粒子と比較して、抗腫瘍免疫性を、免疫賦活性抗PD-1抗体の存在下でより有効に刺激し得るという本発明者らの仮説と整合的である。UV不活化CVA21+抗PD-1抗体群の生存は、UV不活化CVA21+対照抗体、又はCVA21+対照抗体群の生存と統計学的に差がなかった。
本試験は、実施例1に記述した実験の延長であり、悪性メラノーマのB16-ICAM-1マウスモデルにおける、免疫賦活抗体抗PD-1と併用の、コクサッキーウイルスA21(CVA21)腫瘍崩壊ウイルス療法の有効性を調査するものである。実施例2のタイムライン表示を図8に示す。実施例1と異なり、実施例2の全体のタイムラインは66日であり、以下により詳しく記載する「腫瘍再負荷」が含まれた。実施例2で用いる材料と方法は、別の点では実施例1において記載した通りであったが、実施例2に用いるマウスは4週齢から6週齢の間であった。
個々の動物の体重を収集し、実施例1に記載した通りに分析した。あらゆる時間点で、処置群とNTCマウスとの間に、平均体重における統計学的な有意差は観察されなかった(結果は示さず、Holm-Sidak法を用いて複数の比較用に補正した複数のt検定)[Prism 6 for Mac OS X Version 6.0c、GraphPad Software、La Jolla California USA、www.graphpad.com]。動物はCVA21及び抗PD-1治療に良好に耐容したと思われ、処置からの観察できる毒性はなかった。体重の減少は大多数の場合、腫瘍の潰瘍化及び関連する腫瘍負荷と関係していた。
腫瘍体積を週3回、電気ノギスを用いて測定した。42日目までに、食塩水+対照抗体で処置したマウスは全て、腫瘍体積腫瘍の潰瘍化によって証明される進行性疾患のため、安楽死させた。図9に示す通り、食塩水+対照抗体処置した腫瘍における抗腫瘍活性は殆ど観察されなかった。腫瘍体積は全て増大し、人道的な最大のエンドポイントに到達し、安楽死を必要とした。CVA21+抗PD-1処置群は最良の応答を示し、腫瘍開始は顕著に遅延し、6匹だけが、試験の後半に向けて原発腫瘍の成長の徴候を示した。
抗PD-1処置と併用のCVA21治療後に、頑強な抗腫瘍の免疫応答が発生したか否かを確立するために、B16マウスメラノーマ細胞をマウスに再負荷した。B16細胞にはヒトICAM-1受容体がなく、したがってCVA21治療に抵抗性である。これらの細胞は、B16-ICAM-1細胞株を産生するのに用い、原発腫瘍の後続の腫瘍崩壊を生じた可能性がある抗腫瘍の免疫応答の存在を同定するのに用いたB16細胞に抗原性が似ている。図10に見られる通り、B16腫瘍は、B16細胞を再負荷した全てのマウスに最終的に発症したが、45日目、食塩水+対照抗体群の平均腫瘍体積は、食塩水+抗PD-1、CVA21+対照抗体、及びCVA21+抗PD-1抗体群より統計学的に大きかった(二元配置のANOVA)。
抗PD-1抗体と併用のCVA21は、食塩水+対照抗体群と比較して、全生存における統計学的に有意な改善を実証した(p<0.0001ログランク[マンテルコックス]検定)(図11)。CVA21+対照抗体及び食塩水+対照抗体群を比較すると、統計学的な有意差はなかった。食塩水+抗PD-1の生存曲線をCVA21+抗PD-1処置群と比較した場合、統計学的な差が存在した(p=0.0026ログランク[マンテルコックス]検定)(図11)。この知見は、CVA21は抗PD-1抗体と併用して使用すると、有意な生存効果をもたらしたことを示唆する(食塩水+抗PD-1及びCVA21+抗PD-1に対してそれぞれ生存の中間値は、45日対60日)。
実施例2の結果は、CVA21を抗PD-1抗体と併用して使用すると、食塩水+抗PD-1抗体処置を投与した動物群と比較して、担癌マウスの全生存を改善したことを示した。CVA21を抗PD-1抗体と併用すると、食塩水+対照抗体群と比較して、全生存における統計学的に有意な改善を実証した(p<0.0001ログランク[マンテルコックス]検定)(図11)。CVA21の抗PD-1との併用の処置レジメンは良好に耐容され、試験物品に起因する有害事象はなかった。本試験の主な知見は、CVA21+抗PD-1で処置した担癌マウスは、腫瘍成長の減速に関連する全体の延命効果を示したことであった。CVA21+抗PD-1抗体で処置した動物は、対照群に対して疾患の進行の緩徐化を示し、二次性B16腫瘍での再負荷に対して、より抵抗性であった。
本試験は、悪性メラノーマのB16-ICAM-1マウスモデルにおける免疫賦活抗体抗CTLA-4と併用の、コクサッキーウイルスA21(CVA21)腫瘍崩壊ウイルス療法の有効性を調査するものである。
個々の動物の体重を収集し、実施例1に記載した通りに分析した。あらゆる時間点で、処置群とNTCマウスとの間に、平均体重における統計学的な有意差は観察されなかった(結果は示さず、Holm-Sidak法を用いて複数の比較用に補正した複数のt検定) [Prism 6 for Mac OS X Version 6.0c、GraphPad Software、La Jolla California USA、www.graphpad.com]。動物はCVA21及び抗CTLA-4治療に良好に耐容したと思われ、処置からの観察できる毒性はなかった。体重の減少は大多数の場合、腫瘍の潰瘍化及び関連する腫瘍負荷と関係していた。
腫瘍体積を週2回、電気ノギスを用いて測定した。45日目までに、腫瘍体積及び腫瘍の潰瘍化によって証明される進行性疾患のため、食塩水+対照抗体処置したマウスは全て安楽死させた。図13に示す通り、食塩水+対照抗体処置した腫瘍における抗腫瘍活性は殆ど観察されなかった。腫瘍体積は全て増大し、人道的な最大のエンドポイントに到達し、安楽死を必要とした。CVA21+抗CTLA-4処置群は最良の応答を示し、腫瘍開始における注目すべき遅延があり、試験の後半の間原発腫瘍の成長の徴候を示した動物はなかった。完全な腫瘍の退行とそれに続く永続的な応答が、いずれかの単独療法で処置した動物の60%に観察された。より興味深いことに、CVA21+抗CTLA-4併用治療で処置した動物全てが、原発腫瘍に対する完全な拒絶反応を実証した。
抗CTLA-4処置と併用のCVA21治療後に、頑強な抗腫瘍の免疫応答が発生したか否かを確立するために、B16マウスメラノーマ細胞をマウスに再負荷した。B16細胞にはヒトICAM-1受容体がなく、したがってCVA21治療に抵抗性である。これらの細胞は、B16-ICAM-1細胞株を得るのに用い、原発腫瘍の後続の腫瘍崩壊を生じた可能性がある抗腫瘍の免疫応答の存在を同定するのに用いたB16細胞に抗原性が似ている。77日目、抗CTLA-4+CVA21併用治療で処置した動物は、腫瘍の再負荷に対して40%の保護を実証し、それに比べて単一薬剤の抗CTLA-4抗体で処置した動物では25%の保護だった(図14)。
試験中の動物の全生存。食塩水で処置した動物に対して比較した場合、全処置群が動物の全生存を有意に延長した。単一薬剤の抗CTLA-4(p=0.0068)、単一薬剤のCVA21(p=0.0067)、及びCVA21+抗CTLA-4+CVA21(p=<0.0001)。抗CTLA-4抗体と併用のCVA21は、食塩水+対照抗体群と比較して、全生存における統計学的に有意な改善を実証した(p<0.0001ログランク[マンテルコックス]検定)(図15)。CVA21+対照抗体と食塩水+対照抗体群とを比較すると、統計学的な有意差はなかった。食塩水+抗CTLA-4の生存曲線をCVA21+抗CTLA-4処置群と比較すると、統計学的な差があった(p=0.0026ログランク[マンテルコックス]検定)(図15)。この知見は、CVA21を、抗CTLA-4抗体と併用して使用すると、有意な延命効果をもたらしたことを示唆する(食塩水+抗CTLA-4及びCVA21+抗CTLA-4それぞれに対して生存の中間値45日対60日)。試験中の動物の生存の中間値を図15(B)に示す。CVA21+抗CTLA-4の併用治療を投与した動物は、生存の中間値が最長であった(72日)。
実施例3の結果は、CVA21を抗CTLA-4抗体と併用して使用すると、CVA21+対照抗体処置を投与した動物群と比較して、担癌マウスの全生存を改善したことを示した。CVA21+抗CTLA-4処置したB16-ICAM-1腫瘍は全て、77日までに退行した。CVA21を抗CTLA-4抗体と併用すると、食塩水+対照抗体群と比較して、全生存における統計学的に有意な改善を実証した(p<0.0001ログランク[マンテルコックス]検定)(図15)。45日目までに、食塩水+対照抗体処置したマウスは全て、腫瘍の進行及び/又は潰瘍化のため、安楽死させた。抗CTLA-4抗体と併用のCVA21の処置レジメンは良好に耐容され、試験物品に起因する明らかな有害事象はなかった。
本試験は、悪性メラノーマのB16-ICAM-1マウスモデルにおける、免疫賦活抗体抗PD-1、抗CTLA-4、及び抗PD-1+抗CTLA-4と併用の、コクサッキーウイルスA21(CVA21)腫瘍崩壊ウイルス療法の有効性を調査する。
腫瘍体積を週2回、電気ノギスを用いて測定した。図16に示す通り、試験27日目、食塩水+対照抗体処置した腫瘍における抗腫瘍活性は殆ど観察されなかった。単一薬剤群は全て(CVA21、抗PD-1、及び抗CTLA-4)、食塩水群と比較して有意な腫瘍の低減を示した。併用処置群は全て、食塩水群と比較して腫瘍の低減を示したが、全般的に単一薬剤処置に関して有意にレベルが高かった。図16Bにおいて、試験の時間経過全体を通して平均腫瘍体積を分析することで、食塩水の対照動物と比較して、単一薬剤及び併用群の両方に有意な腫瘍の低減がやはり示された。注目すべき傾向が同定され、抗PD-1及びCVA21の併用は、単一薬剤のCVA21及び抗PD-1処置単独と比較して、腫瘍発生の動態の低減を示した。腫瘍発生の個々のスパイダープロットを図16Cに表す。データは、食塩水対照群と比較して、全処置群における触知できる腫瘍発生の発生率における有意な低減を示す。特に注目すべきは、単一薬剤の抗PD-1又は抗CTLA-4で処置した動物における検出できる腫瘍の存在と比較した、静脈内CVA21投与と併用した場合の抗PD-1及び抗CTLA-4両方における腫瘍の発生率の低下である。CVA21+抗PD-1+抗CTLA-4の併用治療で処置した動物は全て、試験27日目に原発腫瘍の発生に対する完全な拒絶を実証したという観察は、より驚くべきことである。
腫瘍内(i.t.)注射後、CVA21はICAM-1発現性腫瘍細胞に優先的に感染し、腫瘍細胞の溶解、及び全身性の免疫媒介性抗腫瘍応答をもたらす。進行メラノーマ患者における、i.t.送達されるCVA21のフェーズII試験は、注射した病変及び遠位の非注射の病変の両方における抗腫瘍活性を強調した(図17)。
Balb-C SCIDマウスの左側腹上にSK Mel28細胞を埋め込んだ(0日目)。尾静脈中への注射により、マウスにCVA21又は食塩水のいずれかを静脈内投与した(14日目)。処置後3時間、6時間、24時間、及び72時間にマウスを安楽死させ、ウイルス及び細胞の遺伝子プロファイリング用に腫瘍を切除した。CVA21で処置したマウスからの腫瘍細胞に、インターフェロンg誘導性タンパク質10(IP-10)及びPD-L1の上方制御が観察された(図19)。
Claims (37)
- コクサッキーウイルスA21又はそのウイルスRNAを含む、処置の必要な対象における細胞間接着分子−1(ICAM−1)の発現性の癌を処置するための医薬組成物であって、
ここで前記医薬組成物は、治療的有効量のコクサッキーウイルスA21又はそのウイルスRNAが、治療的有効量の抗PD−1抗体と組み合わせて対象に投与されるように用いられる、医薬組成物。 - コクサッキーウイルスA21又はそのウイルスRNAが腫瘍に対する直接注射により投与され、そして、抗PD−1抗体が前記対象に対する全身経路により投与される、請求項1に記載の医薬組成物。
- コクサッキーウイルスA21又はそのウイルスRNAが腫瘍に対する直接注射により投与され、そして、抗PD−1抗体が静脈内注入により対象に投与される、請求項1に記載の医薬組成物。
- コクサッキーウイルスA21又はそのウイルスRNAが、前記対象に全身経路により投与され、そして、抗PD−1抗体が前記対象に全身経路により投与される、請求項1に記載の医薬組成物。
- コクサッキーウイルスA21又はそのウイルスRNAが静脈内投与により対象に投与され、そして、抗PD−1抗体が静脈内注入により対象に投与される、請求項1に記載の医薬組成物。
- 癌が、前立腺癌、乳癌、卵巣癌、白血病、脳の癌、肺癌、直腸結腸癌、甲状腺癌、腎臓癌、肝臓癌、胃癌、膀胱癌、多発性骨髄腫、非小細胞肺癌(NSCLC)、膵臓癌、神経膠芽腫及びメラノーマからなる群から選択される、請求項1から5のいずれかの請求項に記載の医薬組成物。
- 癌が、前立腺癌、乳癌、卵巣癌、白血病、脳の癌、肺癌、直腸結腸癌、甲状腺癌、腎臓癌、肝臓癌、胃癌及びメラノーマからなる群から選択される、請求項6に記載の医薬組成物。
- 癌がメラノーマである、請求項1〜5のいずれかの請求項に記載の医薬組成物。
- 癌が肝臓癌である、請求項1〜5のいずれかの請求項に記載の医薬組成物。
- 癌が乳癌である、請求項1〜5のいずれかの請求項に記載の医薬組成物。
- 癌が肺癌である、請求項1〜5のいずれかの請求項に記載の医薬組成物。
- 癌が膀胱癌である、請求項1〜5のいずれかの請求項に記載の医薬組成物。
- 癌が胃癌である、請求項1〜5のいずれかの請求項に記載の医薬組成物。
- 癌が前立腺癌である、請求項1〜5のいずれかの請求項に記載の医薬組成物。
- 対象における細胞間接着分子−1(ICAM−1)の発現性の膀胱癌を処置するための医薬組成物であって、ここで、前記医薬組成物は、コクサッキーウイルスA21又はそのウイルスRNAを含み、前記医薬組成物は、治療的有効量のコクサッキーウイルスA21又はそのウイルスRNAが治療的有効量の抗PD−1抗体と組み合わせて対象に投与されるように用いられ、
ここで、コクサッキーウイルスA21又はそのウイルスRNAが前記対象に対する膀胱内投与により投与され、そして、抗PD−1抗体が前記対象に対する全身経路により投与される、医薬組成物。 - コクサッキーウイルスA21又はそのウイルスRNAが膀胱内投与により対象に投与され、そして、抗PD−1抗体が静脈内注入により対象に投与される、請求項15に記載の医薬組成物。
- コクサッキーウイルスA21又はそのウイルスRNAの投与が抗PD−1抗体の投与前である、請求項1〜16のいずれかの請求項に記載の医薬組成物。
- 対象がヒトである、請求項1〜17のいずれかの請求項に記載の医薬組成物。
- コクサッキーウイルスA21又はそのウイルスRNAが、抗PD−1抗体及び抗CTLA−4抗体と組み合わせて対象に投与される、請求項1〜18のいずれかの請求項に記載の医薬組成物。
- 抗体がモノクローナル抗体である、請求項1〜19のいずれかの請求項に記載の医薬組成物。
- 抗PD−1抗体が、ラムブロリズマブ(MK−3475)である、請求項20に記載の医薬組成物。
- 抗PD−1抗体が、ニボルマブである、請求項20に記載の医薬組成物。
- 組み合わせ療法が、コクサッキーウイルスA21またはウイルスRNAのみの治療的有効量を投与された対象の生存時間と比較して、対象の生存時間の増加をもたらす、請求項1〜22のいずれかの請求項に記載の医薬組成物。

- コクサッキーウイルスA21の投与が抗PD−1抗体の投与前である、請求項29〜34のいずれかの請求項に記載の医薬組成物。
- 細胞間接着分子−1(ICAM−1)の発現性の膀胱癌を治療するための医薬組成物であって、ここで、前記医薬組成物は、コクサッキーウイルスA21を含み、ここで、前記医薬組成物は、治療的有効量のコクサッキーウイルスA21が、全身経路により投与される治療的有効量の抗PD−1抗体と組み合わせて、対象に膀胱内投与により投与されるように使用される、医薬組成物。
- 前記コクサッキーウイルスA21の投与が、抗PD−1抗体の投与とおよそ同時である、請求項36に記載の医薬組成物。
