Description
本願は、米国特許法第119条(e)項に基づき、2015年4月20日に出願された米国仮特許出願第62/150,138号の利益を主張し、この出願は参照によりその全体が本明細書に組み込まれる。
本開示は、アルボシジブを含むレジメンに応答する可能性の高い患者に、アルボシジブを含むレジメンを施すことによって、がんを処置する方法に一般的に関する。いくつかの実施態様において、本方法は、アルボシジブを含む処置レジメンに対する患者の応答を予測することを含む。
がんの標的治療と組み合わせる予測および予後バイオマーカーの使用は、薬剤の開発期間を減らし、薬剤の効能を改良し、および臨床的判断を導くための鍵を握り得る。がんの処置において進歩はあるが、化学療法の大部分は未だ非効率的で、効果のないままである。化学療法の性能が概して不十分である一つの理由は、選択される処置が大抵、個々の患者の疾患に厳密には合致しないことである。正確な診断と治療とを連結する個別化医療の手法は、この問題を緩和しうる。

一つの態様において、本発明は患者のがん処置を決定する方法であって、骨髄のみから収集された患者の腫瘍またはがん細胞標本におけるMCL1依存性の程度を決定し、患者の一以上の臨床学的因子を決定し、一以上のがん処置に対する臨床応答の可能性によって患者を分類することを含み、ここで一以上の臨床学的因子は、臨床応答との関連についての、MCL1に特異的なBH3プロファイリングの読み出しの特異度および感度を向上するように選択される、方法を提供する。

一つの実施態様において、臨床学的因子は性別である。一つの実施態様において、その性別は男性である。他の実施態様において、その性別は女性である。
明の典型的なキットは、様々な試薬(例えば、BH3ペプチドを検出する1以上の薬剤)を含む。キットはまた、様々な検出方法において有用な試薬(例えば抗体など)を含む、検出のための1以上の試薬を含んでもよい。このキットはさらに、ウェルプレート(welled plate)、シリンジなどを含む、評価に必要な器具を含んでもよい。このキットはさらに、記載される試薬の使用方法を指示するラベルまたは印刷された説明書を含んでもよい。このキットはさらに、検査のための処置剤を含んでもよい。
ジョンズ・ホプキンズ大学病院(Johns Hopkins University Hospitals and Clinics)は、その組織貯蔵所に、以前に行った2回の臨床試験からの生存できる状態の凍結したAML標本を有する。ジョンズ・ホプキンズ大学のプロトコルNCT00795002(J0856)、NCT00407966(J0669)またはNCT01349972(J1101)に登録し、新たに診断されたAMLまたはMDS患者からの63個の末梢血および骨髄試料を得て、分析した。患者をFLAM(アルボシジブ(フラボピリドール)、Ara−Cおよびミトキサントロン(n=54))または7+3(Ara−Cおよびダウノルビシン、n=9)で処置した。完全な応答は、全ての細胞株の正常な成熟を伴う5%未満の骨髄芽球、1000/μLのANCおよび100,000/μLの血小板数、末梢血中に芽球が存在しないこと、髄中に白血病細胞が存在しないこと、疾患に関連する細胞遺伝学の解消、過去の髄外の疾患の解消、に特徴付けられる。BH3プロファイリングを完了した後、患者全体の特徴はジョンズ・ホプキンズ大学によって提供され、表1に要約され、ここで表1は、患者の年齢、細胞遺伝学的なリスク、FLT−3変異、NPM1変異、MDS/髄障害歴、過去の化学療法歴、BM芽球%、診断時のWBC数、および治療に対する応答を含む。個々の患者の特徴を、表2に記載する。
簡潔に述べると、抽出し、凍結した白血球試料を急速に解凍し、細胞の生存率をトリパンブルー色素排除法で決定した。細胞をFACS緩衝液(2%FBSを含む1x PBS)で洗浄し、蛍光標識されたCD45、CD3およびCD20モノクローナル抗体を用いて免疫表現型を決定した。次いで、細胞を、摂動の工程のためにニューマイヤー(Newmeyer)緩衝液(10mMトレハロース、10mM HEPES、80mM KCl、20μM EGTA、20μM EDTA、5mMコハク酸、pH7.4)中に再懸濁した。BH3ペプチドをニューマイヤー緩衝液中に希釈し、希釈標準溶液はBIM(100μM)、BIM(0.1μM)、NOXA(100μM)、Puma(10μM)、HRK(100μM)、BAD(100μM)およびBID(1.0μM)の終濃度となった。DMSOおよびCCCPをペプチドの負および正の対照として用いた。ジギトニンおよびオリゴマイシンを個々のFACSチューブに添加し、次いでBH3ペプチドを添加した。そして、細胞の透過処理、ペプチドまたは化合物の送達およびミトコンドリアの脱分極を生じさせるため、細胞をFACSチューブに添加し、室温で2時間15分インキュベートした。インキュベーション後、JC−1色素をニューマイヤー緩衝液中に調製し、処理された細胞に直接添加した。ペプチドまたは化合物で処理されていない細胞の追加のチューブを、ヨウ化プロピジウム(PI)で染色し、細胞がジギトニンによって効率的に透過処理されたことを保証した。JC−1を伴う45分のインキュベーション後、細胞を3本のレーザーのBD FACSCanto IIで分析した。AML芽球を4つのパラメータに基づいてゲートした:1)透過処理(PI染色によって決定された)、2)SSCに基づくシングレット(singlet)の識別、3)CD45が弱く、CD3/CD20が陰性、および4)SSCが低い。次に、ゲートされた芽球の集団におけるJC−1の中央値の赤い蛍光を用いて、DMSO(負)およびCCCP(正)の対照と比較して、脱分極の割合(%)を計算した。細胞遺伝学的なリスクの状態を決定する細胞遺伝学的な分析は、ジョンズ・ホプキンズ大学によって提供された。個々の患者の細胞遺伝学的なリスクの分類を(有利、中間および不利)、がんおよび白血病のグループB(CALGB)のガイドラインから決定した:有利=inv16、t(8:21)、T(15;17)、中間=二倍体、不利=−5、−7、+8、t(6;9)、11q、PH1+、3つの関連しない細胞遺伝学的な異常など。
全63個の試料を受け取り、処理し、これらの試料を表1および2に要約する。完全なプロファイリングを43個の試料から得、追加の9個をプロファイルのサブセットで処理した(全体のアッセイを行うには細胞数が不十分であったため)。残った11個の試料は、悪い信号対雑音比(細胞がアッセイ前に既にアポトーシスしていた)または不十分な細胞数のいずれかであったため、いずれかのBH3プロファイリングを決定するにも質が不十分であった。全ての続く解析は、任意のBH3プロファイリングで処理が成功したこれらの試料のみで行った(全体でn=52)。
AML患者の骨髄または末梢血のBIM0.1プライミングは7+3レジメン(シタラビンプラスアントラサイクリン)に対する応答と相関した:Pierceall, et al. "BH3 Profiling Discriminates Response to Cytarabine-based Treatment of Acute Myelogenous Leukemia" Molecular Cancer Therapeutics参照。上述のように、NOXA骨髄プライミングはFLAM(アルボシジブ、ara−C、ミトキサントロン)レジメンに対する応答と相関する。我々は、FLAMまたは7+3のどちらを、未処置のAML患者を処置するために使用すべきかを識別するアルゴリズムを研究した。
AML細胞株であるMV−4−11はMCL−1を発現する。MCL−1は、MV−4−11細胞において重要な抗アポトーシスタンパク質である。CDK9阻害剤であるアルボシジブはMCL−1のmRNA発現を低下させ、このことは既に文献で良く立証されている。アルボシジブはMV−4−11細胞中でMCL−1のmRNAを用量依存的に下方制御する(図7A)。用量依存性を、細胞に0−8μMの濃度のアルボシジブを投与した際に、対照と比較したMCL−1のmRNAの相対的な発現量をモニターすることによって、観測した。MV−4−11細胞をインビトロ(in vitro)で2時間、アルボシジブ処理した。処理後、細胞を回収し、対照と比較した変化のプローブとしてのMCL−1のmRNAの検出のために、標準的なリアルタイムPCR(RT−PCR)アッセイを用いて調製した。
A549細胞株はヒト腺がん細胞株であり、MCL−1を発現する。アルボシジブはA549細胞中のMCL−1のmRNAを用量依存的に下方制御する(図8)。用量依存性を、細胞に0−8μMの濃度のアルボシジブを投与した際に、対照と比較したMCL−1のmRNAの相対的な発現量をモニターすることによって、観測した。A549細胞をインビトロで2時間、アルボシジブ処理した。処理後、細胞を回収し、対照と比較した変化のプローブとしてのMCL−1のmRNAの検出のために、標準的なRT−PCRアッセイを用いて調製した。
アルボシジブはMV−4−11細胞中のMCL−1を用量依存的に下方制御する(図10A)。用量依存性を、細胞に0−8μMの濃度のアルボシジブを投与した際に、β−アクチンと比較したMCL−1の相対的な発現量をモニターすることによって、観測した。MV4−11細胞をインビトロにおいて2時間、アルボシジブ処理した。処理後、細胞を回収し、β−アクチンと比較した変化のプローブとしてのMCL−1の検出のために、標準的な免疫ブロッティング法を用いて調製した。
SK−N−AS細胞株は、N−Mycを発現するヒト神経芽細胞腫の細胞株であり、N−Mycは重要な発がんタンパク質である。アルボシジブはSK−N−AS細胞中のN−Mycを時間依存的に下方制御する(図11)。時間依存性を、β−アクチンと比較したN−Mycの相対的な発現量を経時的にモニターすることによって、観測した。N−Mycの相対的な発現量を、300nMのアルボシジブ投与後1〜24時間の時点で測定した。
アルボシジブはA549細胞中のMCL−1を時間依存的に下方制御する(図12)。時間依存性を、β−アクチンと比較したMCL−1、Bcl−xLおよびBcl−2の相対的な発現量を経時的にモニターすることによって、観測した。MCL−1、Bcl−xLおよびBcl−2の相対的な発現量を、350nMのアルボシジブ投与後0〜96時間の時点で測定した。
Panc−1細胞株は、MCL−1を発現するヒト膵臓がん細胞株である。アルボシジブはPanc−1細胞中のMCL−1を時間依存的に下方制御する。時間依存性を、β−アクチンと比較したMCL−1、Bcl−xLおよびBcl−2の相対的な発現量を経時的にモニターすることによって、観測した。MCL−1、Bcl−xLおよびBcl−2の相対的な発現量を、730nMのアルボシジブ投与後0〜96時間の時点で測定した。一つの群において、処理の24時間後に、培地を除去し、730nMアルボシジブを含む新たな培地に交換した(すなわち、洗浄の工程;図13A参照)。
アルボシジブは、反復投与のレジメンを用いると、Panc−1細胞中のMCL−1を時間依存的に下方制御する(図14)。時間依存性を、β−アクチンと比較したMCL−1、Bcl−xLおよびBcl−2の相対的な発現量を経時的にモニターすることによって、観測した。MCL−1、Bcl−xLおよびBcl−2の相対的な発現量を、730nMのアルボシジブ投与後0〜96時間の時点で測定した。
Yugen8細胞株は、MCL−1を発現する皮膚がん細胞株である。アルボシジブはYugen8細胞中のMCL−1を時間依存的に下方制御する。時間依存性を、β−アクチンと比較したMCL−1、Bcl−xLおよびBcl−2の相対的な発現量を経時的にモニターすることによって、観測した。MCL−1、Bcl−xLおよびBcl−2の相対的な発現量を、280nMのアルボシジブ投与後0〜96時間の時点で測定した。一つの群において、処理の24時間後に、培地を除去し、280nMアルボシジブを含む新たな培地に交換した(すなわち、洗浄の工程;図15A参照)。
アルボシジブは、反復投与のレジメンを用いると、Yugen8細胞中のMCL−1を時間依存的に下方制御する(図16)。時間依存性を、β−アクチンと比較したMCL−1、Bcl−xLおよびBcl−2の相対的な発現量を経時的にモニターすることによって、観測した。MCL−1、Bcl−xLおよびBcl−2の相対的な発現量を、280nMのアルボシジブ投与後0〜96時間の時点で測定した。

Claims (10)
- MCL−1に依存する急性骨髄性白血病(AML)の哺乳動物を処置する方法において用いるための、アルボシジブを含む医薬組成物であって、シタラビンおよびミトキサントロンと併用され、前記哺乳動物が、MCL−1プライミングが少なくとも15%である骨髄細胞を有し、前記MCL−1プライミングが、前記哺乳動物の骨髄由来の細胞をMCL−1に選択的なBH3ペプチドで処理し、MCL−1プライミングに対応する、生じるミトコンドリアの脱分極の割合(%)の変化を決定することにより、決定されたものであり、BH3ペプチドがNOXAである、医薬組成物。
- MCL−1に依存する急性骨髄性白血病(AML)の哺乳動物を処置する方法において用いるための、アルボシジブを含む医薬組成物であって、シタラビンおよびミトキサントロンと併用され、前記方法が、
前記哺乳動物から複数の骨髄細胞を得ること、
前記骨髄細胞をMCL−1に選択的なBH3ペプチドで処理し、MCL−1プライミングに対応する、生じるミトコンドリアの脱分極の割合(%)の変化を決定することにより、前記骨髄細胞のMCL−1プライミングを決定すること、ここで、BH3ペプチドはNOXAである、および
前記骨髄細胞のMCL−1プライミングが少なくとも15%である場合に、前記哺乳動物に、アルボシジブ、シタラビンおよびミトキサントロンを投与すること、
を含む、医薬組成物。 - MCL−1プライミングの決定が、前記骨髄細胞を透過処理することを含む、請求項1または2に記載の医薬組成物。
- AMLが抵抗性である、請求項1〜3のいずれかに記載の医薬組成物。
- AMLが抵抗性である、または前記哺乳動物が再発した哺乳動物である、請求項1〜3のいずれかに記載の医薬組成物。
- 前記哺乳動物が、MCL−1プライミングが少なくとも20%である骨髄細胞を有する、請求項1〜5のいずれかに記載の医薬組成物。
- 前記哺乳動物が、MCL−1プライミングが少なくとも25%である骨髄細胞を有する、請求項1〜5のいずれかに記載の医薬組成物。
- 前記哺乳動物が、MCL−1プライミングが少なくとも30%である骨髄細胞を有する、請求項1〜5のいずれかに記載の医薬組成物。
- 前記哺乳動物が、MCL−1プライミングが少なくとも35%である骨髄細胞を有する、請求項1〜5のいずれかに記載の医薬組成物。
- 前記哺乳動物が、MCL−1プライミングが少なくとも40%である骨髄細胞を有する、請求項1〜5のいずれかに記載の医薬組成物。
