組織学/細胞学の方法を使用するバイオマーカー値の定量
[00255] 肺癌の評価では、多様な組織試料を組織学的又は細胞学的な方法において使用することができる。試料選択は、原発腫瘍の位置と転移の部位に依存する。例えば、組織学には、気管支内生検及び経気管支生検、微細針吸引、角針、及びコア針生検を使用することができる。細胞学には、気管支洗浄及び擦過、胸膜吸引、及び喀痰を使用することができる。肺癌の診断には、細胞学的分析が依然として使用されているが、組織学的方法は、癌の検出のためにより良好な感度を提供することが知られている。肺癌のある個体において上方調節されていることが示された、本発明で同定されたバイオマーカー(表19を参照のこと)のいずれを使用しても、組織学的標本を疾患の指標として染色することができる。

(1999) British Journal of Cancer 79(2):363-369; Imotoet al. (1998) J. Thorac. Carciovasc. Surg 115:1007-1011; Ohtaet al. (2006) Ann. Thorac. Surg. 82:1180-1184; Iizasa et al. (Aug. 2004) Clinical Cancer Research 10:5361-5366)。NSCLC腫瘍組織では、TSP−1と関連のトロンボスポンジン−2(TSP−2)が実質的に上方調節されていた。TSP−1とTSP−2は、細胞表面受容体、成長因子、サイトカイン、マトリックスメタロプロテイナーゼ、及び他の分子との様々な相互作用を介して変調される、複雑で状況依存的な効果がある細胞外マトリックスタンパク質である。モデル系において、原型的には、TSP−1とTSP−2は、CD47受容体を介して内皮細胞増殖を阻害することと、CD36受容体を介して内皮細胞アポトーシスを誘導することによって血管新生を阻害する。TSP−1とTSP−2については、好血管新生性の影響への証拠もある(Bornstein (2009) J. (2010) Lung Cancer (doi:1016/j.lungcan.2010.10.015)は、NSCLC腫瘍組織中で上方調節されていることが見出された。別の注目すべき例は、NSCLC組織中で劇的に下方調節されていることが報告されたRAGEである(Jing et al. (2010) Neoplasma. 57:55-61, Bartling et al. (2005) Carcinogenesis 26:293-301)。この知見は、非悪性腫瘍組織中より腫瘍中でより低いタンパク質について観測された最大の変化をsRAGEが有した、本明細書に開示される測定値に一致している。理論によって制限されずに言えば、1つの仮説は、RAGEが上皮の組織化においてある役割を担っていて、肺腫瘍中でのRAGEのレベルの低下が上皮組織構造の損失の原因となって、悪性の形質転換を潜在的にもたらす可能性がある、ということである(Bartling et al. (2005) Carcinogenesis 26(2):293-301)。BCA−1、CXCL16、IL−8、及びNAP−2のようないくつかのサイトカインも変化していて(表18)、先天性及び適応性の免疫系アームに由来する細胞を伴う腫瘍の浸潤が増殖性及び血管新生性のシグナルに影響を及ぼす生理活性分子をもたらすとする仮説と一致している(Hanahan & Weinberg (2011) Cell 144:646-74)。

(2010) The Journal of Thoracic and Cardiovascular Surgery 139(4):984-990)。MMP−12レベルは、局所再発と転移疾患に相関していた(Hofmann et al. (2005) Clin. Cancer Res. 11:1086-92, Hoffman et al. (2006) Oncol. Rep. 16: 587-95)。研究された8名の被検者のうち2名は、正常レベルのMMP−12を有したが、他の6名は、非腫瘍組織に比べて腫瘍組織中でMMP−12が15〜50倍上昇していた。

[00322] 無癌対照に比べたNSCLC患者の血清中でのタンパク質の差示的な発現を、NSCLC組織試料のそれと比較すると、有用な洞察が得られる(図26)。最も衝撃的な観測は、タンパク質発現の相対的な変化が、血清中より組織中でより大きいことである。この結果は、腫瘍組織がタンパク質発現の変化の源泉であって、たとえ循環中へ完全に放出されたとしても、全量の血液中で何倍にも希釈されてしまうので、予測され得ることである。この傾向は、図26のx軸に沿ったデータ点の伸長された分布において明白である(この図では、両軸を同じスケールで引いて、この論点を明示する)。腫瘍組織中で変化するような、図23及び24に示した分析物のうち12種は、NSCLC患者由来の血清中でも、対照に対して差示的に発現されている(図26中の赤塗り円)。この一方的な変化のほとんどは、組織と血清の間で同じであるが、同じでないものも2、3種ある。明らかに、タンパク質の組織ホモジェネート中の局所濃度がそのタンパク質の循環レベルと相関する必要はないが、逆相関は、ある種のバイオマーカーの「疾病組織」対「正常組織」中での再分布に関するヒントを提供する可能性がある。

8.タグ付け
[00371] NHS−PEO4−ビオチンアリコートを37℃で6分間融かしてから、タグ付け緩衝液(SB17,pH=7.25で0.05% Tween−20)で100倍希釈した。NHS−PEO4−ビオチン試薬を無水DMSOに100mM濃度で溶かして、−20℃で凍結保存しておいた。ロボット起動時にすぐ、この希釈したNHS−PEO4−ビオチン試薬をデッキ槽へマニュアルで加えて、ロボットプログラムをマニュアルで再始動させて、100μLのNHS−PEO4−ビオチンをそれぞれのCatch1フィルタープレートの各ウェルへ分注した。この溶液を、オービタルシェーカー上で、800rpmで5分間振り混ぜて、そのままCatch1ビーズとともにインキュベートした。

[00407] 未知試料の対数(RFU)測定値が10個のバイオマーカーのそれぞれで与えられる場合。「対照」対「疾患」群の対数尤度比を含んでなる個々の構成要素を作表して、表15のパラメータとxの値より計算することができる。個々の対数尤度比の合計は3.47である、又は「疾患がないこと」対「その疾患を有すること」の尤度は、32:1(ここで尤度=e3.47=32)である。全5個のバイオマーカーは、いずれも一貫して対照群に有利である。尤度を一緒に掛けると、上記に示したのと同じ結果が得られる、即ち未知試料に疾患がない尤度は32:1である。事実、この試料は、訓練セットの対照集団に由来した。本実施例は、表15中のバイオマーカーを使用する血清試料の分類を実証するが、表21からのどのバイオマーカーのセットを用いて、どの組織タイプでも同じアプローチを使用することができる。

[00408] 本実施例は、本明細書に記載の方法のいずれにおいても分類器として使用し得るパネルを形成するための表21からのバイオマーカーの選択について記載する。MMP−12と表21中のバイオマーカーのサブセットを含有するバイオマーカーのパネルを選択して、良好な性能のある分類器を構築した。また、この方法を使用して、実施例2のバイオマーカーとしてどの潜在的なマーカーを含めるかを決定した。
[00415] 表1で選択したバイオマーカーは、「非マーカー」(即ち、表1中の包含基準(実施例2に記載するような)に合致しないシグナルを有するタンパク質)で組み立てた分類器よりよく機能する分類器を生じた。
[00423] 上記の分類器の良好な性能をマーカーのコアサブセットで説明できるのかどうかを検定するために、このマーカーの半分を表38及び39のバイオマーカーのリストより無作為に落とした。感度+特異度によって測定される、良性結節を悪性結節より区別するための分類器の性能は、わずかに0.07低下して(1.74から1.67へ)、癌を有する喫煙者を有さない喫煙者から区別する分類器の性能も、わずかに0.06低下した(1.76から1.70へ)。このバイオマーカー表のサブセットの性能特性から示唆されることは、収載したバイオマーカーの複合サブセットが診断検査法を組み立てるのに有効であって、分類器の性能を支配する、マーカーの特別なコアサブセットはないということである。
[00429] マルチプレックスアッセイでの最終の読出しは、本アッセイでの連続捕捉工程の後で回収されるアプタマーの量に基づく。マルチプレックスアッセイは、アッセイの最後に回収されるアプタマーの量が元の複雑な混合物(例、血漿)中のタンパク質の量に比例するという前提に基づく。このシグナルが、本当に、血漿中で非特異的に結合したタンパク質に由来するのではなくて、企図される分析物に由来することを実証するために、我々は、血漿中のゲルベースのプルダウンアッセイを開発した。このアッセイを使用して、所望のタンパク質がアプタマーとの平衡状態の後で実際に血漿より引き出せることを視覚的に実証して、その企図されるタンパク質標的へ結合したアプタマーがこのアッセイ中の動的チャレンジ工程を通して複合体として存続し得ることを実証することができる。本実施例に記載の実験において、このプルダウンアッセイの最後でのタンパク質の回収には、平衡状態後ほぼ2時間の間、そのタンパク質がアプタマーへ非共有的に結合したままであることが求められる。重要にも、本実施例において、我々はまた、非特異的に結合したタンパク質が上記の工程の間に解離して、最終シグナルへ有意には寄与しないことの裏付けを提供する。本実施例において記載されるプルダウン手法には、上記に記載のマルチプレックスアッセイの重要工程がすべて含まれることに留意されたい。
[00430] 50μL EDTA−血漿を、0.05% Tween−20(SB18T)及び2μM Z−Blockを含むSB18において100μLへ希釈することによって血漿試料を調製した。この血漿溶液を、150μLの最終容量において、37℃で2時間、10ピコモルのPBDC−アプタマーと平衡化した。平衡状態の後で、Durapore フィルタープレートにおいて、室温で5分間振り混ぜながらインキュベートすることによって、複合体と非結合アプタマーを133μLの7.5%ストレプタビジン−アガロースビーズスラリーで捕捉した。実施例1に記載のように、ビーズへ結合した試料を真空下にビオチンで、そして緩衝液で洗浄した。洗浄後、結合したタンパク質を、振り混ぜながら室温で5分間、ビオチン希釈液中の0.5mM NHS−S−S−ビオチン、0.25mM NHS−Alexa647で標識した。この染色工程は、ストレプタビジンビーズ上でタンパク質を捕捉するためのビオチニル化だけでなく、ゲル上での検出のための高感度染色を可能にする。実施例1に記載のように、試料をグリシンで、そして緩衝液で洗浄した。Black Ray光源を振り混ぜながら室温で10分間使用する光切断によって、アプタマーをビーズより遊離させた。この時点で、室温で5分間振り混ぜることによって、ビオチニル化タンパク質を0.5mg MyOneストレプタビジンビーズ上で捕捉した。この工程により、アプタマーへ結合したタンパク質だけでなく、最初の平衡状態以後にアプタマーより解離した可能性があるタンパク質も捕捉される。実施例1に記載のようにビーズを洗浄した。SB17T中50mM DTTとともに振り混ぜながら37℃で25分間インキュベートすることによって、MyOneストレプタビジンビーズよりタンパク質を溶出させた。次いで、この溶出液を、そのアプタマーの3’一定領域へ相補的な配列でコートされたMyOneビーズへ移して、振り混ぜながら37℃で25分間インキュベートした。この工程により、残存するアプタマーのすべてが捕捉される。このビーズを100μL SB17Tで1分間(2回)、そして100μL SB19Tで1分間(1回)洗浄した。45μLの20mM NaOHとともに振り混ぜながら2分間インキュベートすることによって上記の最終ビーズからアプタマーを溶出させて、ハイブリダイズした鎖を壊した。この溶出液の40μLを、0.05% Tween−20を含有する80mM HClの10μLで中和した。第一のビーズセットからの溶出物(アプタマーへ結合したすべての血漿タンパク質を表す)の5%を表すアリコートと最終のビーズセットからの溶出物(我々の臨床アッセイの最後に結合したままであるすべての血漿タンパク質を表す)の20%を表すアリコートを、還元及び変性の条件下にNuPAGE 4〜12% Bis−Trisゲル(Invitrogen)上で泳動させた。Alpha Innotech FluorChem QスキャナーでCy5チャンネルにおいてゲルを造影して、タンパク質を造影した。
[00433] 組織由来の疾患関連バイオマーカーを同定する、本明細書に記載のプラットフォームベース技術の有用性を実証するために、8名のNSCLC患者より入手した、外科摘出由来の均質化した組織試料について分析した。NSCLC患者はいずれも47〜75歳の範囲に及ぶ喫煙者であって、NSCLC病期1A〜3Bが含まれた(表17)。外科的切除の後でその組織をOCT培地(10.24%ポリビニルアルコール、4.26%ポリエチレングリコール、及び85.5%非反応性成分)に入れてから5〜10分以内に該組織を凍結させることによってすべての組織試料を入手した。これらの試料は、それぞれの摘出より3種の試料:腫瘍組織、隣接健常組織(腫瘍の1cm以内)、及び遠位の非関与肺組織を入手した。この試料を常に凍結させながら、5つの10μm厚の切片を切り出し、その組織周囲の過剰なOCTをトリミングして、凍結した1.5mLミクロ遠心管の中へ入れた。200μLの均質化緩衝液(SB18緩衝液+PIカクテル(マグネシウムを含まない、Pierce HALTプロテアーゼ阻害剤))の添加の後で、この試料を氷上のミクロ遠心管において回転ペストルで30秒間ホモジェナイズすると、組織断片が見えなくなった。次いで、この試料を遠心分離機において21,000gで10分間スピンさせて、0.2μmのマルチウェルプレートフィルターに通して無菌のマルチウェルプレートの中へ濾過した。5μLのアリコートをBCAタンパク質アッセイ用に取って、試料の残りは96ウェルプレートにおいて−70℃で凍結保存して密封した。
Claims (25)
- 個体が肺癌を有するか又は有さないかの診断を補助するための、又は個体における肺癌に関する情報を提供するための、方法であって:
個体からの生体試料においてバイオマーカータンパク質MMP−12、及びC9、MMP−7、ERBB1及びSCF sRから選択される少なくとも一つのバイオマーカーを検出し、バイオマーカータンパク質にそれぞれ対応するバイオマーカー値を与えることを含んでなり、ここで前記バイオマーカー値は、個体が肺癌を有する又は有さない可能性について表示するか、又は前記バイオマーカー値は、該個体中の肺癌に関する情報を提供する、前記方法。 - バイオマーカー値を検出することが in vitro アッセイを実施することを含む、請求項1の方法。
- 前記 in vitro アッセイが前記バイオマーカーのそれぞれに対応する少なくとも1つの捕捉試薬を含み、そして前記少なくとも1つの捕捉試薬をアプタマー及び抗体からなる群より選択することをさらに含んでなる、請求項2の方法。
- 前記 in vitro アッセイが、イムノアッセイ、アプタマーベースのアッセイ、及び組織学的又は細胞学的アッセイからなる群より選択される、請求項2又は3の方法。

- 情報が、予後判定、癌分類、疾患リスクの予測、又は治療法の選択を含む、請求項1〜10のいずれか一項の方法。
- 肺癌の可能性を示すためのコンピュータで実行される方法であって:
コンピュータ上でバイオマーカー情報を個体のために検索すること、ここで該バイオマーカー情報は、バイオマーカータンパク質MMP−12、及びC9、MMP−7、ERBB1及びSCF sRから選択される少なくとも一つのバイオマーカーにそれぞれ対応するバイオマーカー値を含む;
前記バイオマーカー値のそれぞれの分類をコンピュータで実施すること;及び
複数の分類に基づいて、前記個体が肺癌を有する可能性を示すことを含んでなる、前記方法。 - 個体が肺癌を有するか又は有さないかの診断を補助するための、又は個体における肺癌に関する情報を提供するための、方法であって:
個体からの生体試料においてバイオマーカータンパク質MMP−12、及びC9、MMP−7、ERBB1及びSCF sRから選択される少なくとも一つのバイオマーカーを検出し、該バイオマーカータンパク質にそれぞれ対応するバイオマーカー値を与えることを含んでなり、ここで前記バイオマーカー値は、個体が肺癌を有する又は有さない可能性について表示する、前記方法。 - バイオマーカー値を検出することが in vitro アッセイを実施することを含む、請求項13の方法。
- 前記 in vitro アッセイが前記バイオマーカーのそれぞれに対応する少なくとも1つの捕捉試薬を含み、そして前記少なくとも1つの捕捉試薬をアプタマー及び抗体からなる群より選択することをさらに含んでなる、請求項14の方法。
