Description
本発明は、例えば、医薬品又は食品の形態にある体温低下抑制剤、特に手術時の全身麻酔による体温低下を抑制する体温低下抑制剤に関する。
外科手術時等の患者においては、全身麻酔により体温調節機構が抑制されるため、周囲の環境温度の影響を受けやすい状態となり患者の体温(核心温)の低下が起こる。この体温低下により悪寒・不快感、シバリング、頻脈・虚血性心電図変化、覚醒遅延、創部感染、創傷治癒遅延、免疫力低下、血液凝固障害等の合併症が発症している。
このような体温低下を防止するため、外科手術時にアルミニウム保温材、循環式加温マット、加温輸液、温風式加温等による保温対策が知られている。
しかしながら、従来の循環式加温マットは患者の体位によっては保温部位が局部的になり保温効果が十分でなかった。また、温風式加温は有効な手段であったが患者を被うウォーミングカバー等の消耗品が必要であることからコストの面で問題となっている。体温低下が体温中枢の閾値を越えて低下した場合、従来の保温対策を行っても体温回復には長時間を必要とする。また、高用量の総合型のアミノ酸輸液製剤(総アミノ酸濃度10W/V%以上)が体温低下抑制に有効であるとの報告(Eva Sellden,et al:Anesth Analg 89:1551−1556,1999;藤原広明、他;外科と代謝・栄養 第36巻4号:215−220、2002;T.Kasai,et al:British Journal of Anaesthesia 90:58−61,2003)があるが、本発明の有効成分の開示はなく、また本発明と同じ総アミノ酸濃度のアミノ酸製剤は開示されていない。
本発明は、又、医薬品又は飲食品の形態にある体温低下抑制剤を提供することを目的とする。
本発明は、又、周術期合併症を予防および/あるいは改善する体温低下抑制剤を提供することを目的とする。
本発明は、又、末梢血単核球TNF−α産生低下抑制剤を提供することを目的とする。
本発明は、又、静脈投与用輸液を提供することを目的とする。
本発明者らは、上記目的を達成するため様々な種類のアミノ酸について鋭意研究した結果、全身麻酔時にこれまでに報告されていた用量より低い量のアミノ酸が体温低下抑制作用を有すること、更にはある種のアミノ酸においては単独でも体温低下抑制作用を有することを見出し、本発明を完成するに至った。
すなわち、本発明は、バリン、アスパラギン酸、グリシン及びシステインからなる群から選択される少なくとも1種のアミノ酸を有効成分として含有することを特徴とする体温低下抑制剤を提供する。
本発明は、又、バリン、アスパラギン酸、グリシン及びシステインからなる群から選択される少なくとも2種のアミノ酸を組み合わせてなるアミノ酸混合物を有効成分として含有することを特徴とする体温低下抑制剤を提供する。
本発明は、又、バリン、アスパラギン酸、グリシン及びシステインからなる群から選択される少なくとも1種のアミノ酸を有効成分として含有することを特徴とする末梢血単核球TNF−α産生低下抑制剤を提供する。
本発明は、又、バリン、アスパラギン酸、グリシン及びシステインからなる群から選択される少なくとも1種のアミノ酸からなる静脈投与用輸液を提供する。
本発明の体温低下抑制剤の投与方法としては、経口投与、静脈内投与などが可能であるが、特にこれらに限定されるものではない。静脈内投与の場合は末梢静脈、中心静脈等から持続投与することが好ましい。
有効成分とするバリン、アスパラギン酸、グリシン、システインの投与量は投与方法、患者の症状や程度、剤形の種類により適宜選択すれば良いが、経口投与の場合、1投与あたりそれぞれ10mg〜10000mgの範囲で投与すればよい。静脈内投与する場合は経口投与の1/20〜1/2程度の量を投与すればよい。これらの投与量は必要によっては数回に分割して投与しても良いし、また必要によっては1日数回投与しても良い。一方、静脈内に持続投与する場合には10mg〜1000mg/kg/hrの範囲の投与速度で少なくとも2時間程度投与することが好ましい。また投与スケジュールとしては、体温が低下する前あるいは麻酔開始前に投与することが好ましい。さらに持続的に投与あるいは麻酔中継続的に投与することも好ましい。
本発明の体温低下抑制剤の医薬品形態、例えば、医薬品製剤は、投与方法によって変更可能であり特に限定されるものではない。例えば、液剤、錠剤、カプセル剤、顆粒剤、細粒剤、粉末剤、散剤等が考えられる。このような剤型を調製するためには医薬上許容しうる液体または固体状の適当な賦形剤、充填剤、増量剤、溶剤、乳化剤、滑沢剤、風味補正剤、香料、染料、緩衝物質等の補助剤を加えて行うのが好ましい。
本発明の体温低下抑制剤は、輸液製剤の態様にあるのが好ましいので、輸液製剤の態様について特に詳細に説明する。輸液製剤においてはバリン、アスパラギン酸、グリシン、システインいずれかまたはバリン、アスパラギン酸、グリシン、システインのうち任意の組合せを含有すればよい。またバリン、アスパラギン酸、グリシン、システイン以外のアミノ酸を含有しなくてもよく、有効成分であるアミノ酸の効果に影響を与える可能性を考慮するとむしろ他のアミノ酸は含有しない方が好ましい。
バリン、アスパラギン酸、グリシン、システイン以外のアミノ酸を含有しない輸液製剤の場合、バリン、アスパラギン酸、グリシン、システインの少なくともいずれか一つのアミノ酸を10%以下、好ましくは0.01〜9W/V%、より好ましくは0.03〜6W/V%、更に好ましくは0.05〜4W/V%、最も好ましくは0.1〜3.3%の濃度範囲に設定すればよい。
一方バリン、アスパラギン酸、グリシン、システイン以外のアミノ酸を含有する輸液製剤の場合、総アミノ酸濃度が10W/V%未満であることが好ましく、更には1〜9W/V%の範囲が好ましく、最も好ましくは1.5〜5W/V%の範囲である。例えば以下のような各アミノ酸の濃度範囲で、総アミノ酸濃度が10W/V%未満、好ましくは1〜9W/V%、更に好ましくは1.5〜5W/V%となるように配合した総合アミノ酸製剤としても良い。

尚、バリンの濃度を0.01〜0.3W/V%とするのがより好ましく、グリシンの濃度を0.05〜0.4W/V%とするのがより好ましい。又、システインの濃度を0.01〜0.05W/V%とするのがより好ましい。
上記輸液製剤には、輸液成分として通常用いられる電解質、糖、pH調整剤、微量元素、ビタミン等の成分を必要とされる量を任意に配合しても良い。特に電解質を配合していることが好ましい。ここでいう電解質とは水溶液中でイオンを生じる化合物であり、特にNa+、K+、Cl−などのイオンを水溶液中で生じさせる無機塩、有機塩などが好ましい。更に具体的には、以下の濃度範囲のイオンを生じさせる電解質を配合していることが好ましい。
Na+ 20.0〜160.0mEq/L
K+ 3.0〜40.0mEq/L
Cl− 15.0〜160mEq/L
Na+を生じさせる電解質としては、塩化ナトリウム、L−乳酸ナトリウム、クエン酸ナトリウム、炭酸水素ナトリウム、リン酸二水素ナトリウム、リン酸一水素ナトリウム、酢酸ナトリウムなどがある。K+を生じさせる電解質としては、塩化カリウムなどがある。Cl−15.0〜160mEq/Lを生じさせる電解質としては、塩化ナトリウム、塩化カリウム、塩化カルシウム、塩化マグネシウムなどがある。
このような輸液製剤はpHが3.0〜8.5に調製されていることが好ましい。
より簡便な輸液製剤としては、使用時にバリン、アスパラギン酸、グリシン、システインを直接既存の輸液製剤に配合してもよく、その際本発明の体温低下抑制剤は、必要とされる量を含有する粉剤、顆粒剤、錠剤、濃厚液剤、あるいはこれらの組合せの形態であっても良い。配合される輸液製剤としては、電解質液が好ましく、中でも周術期に使用されるような等張電解質輸液、低張電解質輸液が好ましく、例えば、生理食塩水、リンゲル液、乳酸リンゲル液、酢酸リンゲル液、開始液、脱水補給液、維持液、術後回復液が好ましい。またそれらに糖を配合した輸液でもよい。手術時に投与する際には、上記輸液製剤を手術2時間前から術中にかけて0.1〜60ml/kg/hr、好ましくは10〜40ml/kg/hrの速度で投与するのが好ましい。
本発明の体温低下抑制剤の輸液製剤は、容器に充填されることが好ましく、必要に応じて各成分を複数の室に分割して充填しても良い。糖として還元糖を配合した場合アミノ酸と還元糖が経時的にメイラード反応を生じる場合があるため、アミノ酸と糖は隔離収容される方が好ましい。例えば2室を有する輸液容器において、第1室に糖・電解質液を充填し、第2室にアミノ酸液を充填すればよい。2室に区画する方法としては、使用時に外部からの押圧で剥離可能なシール部で区画する方法が好ましい。輸液容器の本体を構成する材料としては、可撓性、透明性に優れ、且低温保存後に落下しても破袋し難い軟質の樹脂材料が好ましい。特に、通常医療用容器に用いられているポリオレフィン類からなるものを好適に挙げることができる。ポリオレフィン類は、例えば、ポリエチレン、ポリプロピレン、ポリ1−ブテン、ポリ4−メチル−1−ペンテン等の重合応体を挙げることができる。容器本体は、前記樹脂をブロー成形、インフレーションあるいはデフレーション成形したものいずれでも使用できる。また、2枚の樹脂シートの周縁部を溶着して形成したものでも良い。既存の輸液製剤に配合する形態の場合には、必要成分のみを必要量、粉剤、顆粒剤、錠剤、濃縮溶液、あるいはこれらの組合せの形態で含有できる容器でよく、バイアル瓶でも良い。この場合既存の輸液製剤とのキット製品としてもよい。なお重炭酸イオン含有輸液製剤の場合には、特許第3271650号公報に記載されている方法で製造することが好ましく、ガス不透過性容器に充填するか、ガス透過性プラスチック容器に充填し更にガス不透過性2次包材で包装することが好ましい。
また、本発明の体温低下抑制剤は、上記のように医薬品の形態で実施することができるだけでなく、医薬品の実施形態を参考にして飲食品へ含有させることにより飲食品への用途を容易に実施できる。
本発明の体温低下抑制剤は、体温低下を抑制する目的であればどのような症状・病態にも使用することが可能であるが、特に麻酔による体温低下を抑制する際に有用である。麻酔には、全身麻酔、局所麻酔がある。全身麻酔には、イソフルラン、セボフルラン、鎮痛、沈静、就眠作用のあるガス(笑気ガスなど)などによる吸入麻酔や、プロポフォールなどによる静脈麻酔がある。一方局所麻酔には、テトラカイン、ジブカインなどによる脊髄麻酔(腰椎麻酔)、リドカイン、メビバカイン、ブビバカインなどによる硬膜外麻酔、いわゆる局所麻酔(浸潤麻酔、表面麻酔)がある。本発明の体温低下抑制剤は、これらの麻酔のうち、特に外科手術時の全身麻酔による体温低下抑制に有用である。
さらにバリン、アスパラギン酸、グリシン及びシステインからなる群から選択される少なくとも1種のアミノ酸は、本発明の効果を奏する投与量範囲において、静脈麻酔剤(プロポフォール)の効果を減弱させることなく通常の全身麻酔を導入できる。
またバリン、アスパラギン酸、グリシン及びシステインからなる群から選択される少なくとも1種のアミノ酸は、手術中から手術終了後に発症する周術期合併症(悪寒・不快感、シバリング、頻脈・虚血性心電図変化、覚醒遅延、創部感染、創傷治癒遅延、免疫力低下、血液凝固障害等などの周術期合併症)の予防および/あるいは改善、更には手術侵襲からの回復の促進にも有用である。
手術時の全身麻酔によって、末梢血単核球からのTNF−αやIL−1βなどの炎症性サイトカインの産生・分泌抑制等、免疫系機能が低下することが知られている。また開胸手術や開腹手術といった侵襲によって、免疫系が抑制することが知られている。バリン、アスパラギン酸、グリシン及びシステインからなる群から選択される少なくとも1種のアミノ酸は、末梢血単核球のTNF−α産生低下を抑制することが、創部感染、創傷治癒遅延、免疫力低下などの周術期合併症の予防および/あるいは改善、手術侵襲からの回復の促進に関与していると考えられる。
本発明のバリン、アスパラギン酸、グリシン及びシステインからなる群から選択される少なくとも1種のアミノ酸を有効成分として含有することを特徴とする周術期合併症予防及び/または治療剤、手術侵襲回復促進剤、末梢血単核球TNF−α産生低下抑制剤、静脈投与用輸液の形態は何れも上記体温低下抑制剤の形態と同様に調製することが可能である。
本発明により、全身麻酔等による体温低下を抑制し、体温低下に伴う合併症を予防および/または改善する体温低下抑制剤、更には手術侵襲からの回復を促進する体温低下抑制剤が提供される。本発明の体温低下抑制剤を用いると、体温(核心温)の低下抑制をこれまでの周術期に投与されていた細胞外液組成の輸液に添加した製剤とすることで、従来の体温低下抑制方法に比較し、簡便に安価で行うことが出来る。また、従来の方法と組み合わせることによりさらに有効に体温低下の抑制を達成できる。
次に、実施例により本発明を説明する。
バリン液、アスパラギン酸液、グリシン液、システイン液は、それぞれのアミノ酸が0.1W/V%、1W/V%もしくは2W/V%となるように注射用蒸留水に溶解して調製した。アミノ酸混合液は、表1の配合量のアミノ酸を注射用蒸留水に溶解して調製した。各液は、投与前にろ過滅菌した。
(2)体温低下抑制効果測定1
評価に用いるラットは予め外頚静脈にカテーテルを挿入し背部にその先端を出しヘパリンでロックした状態で数日間回復させた。挿入カテーテルより生理的食塩液及び0.1W/V%、1.0W/V%のバリン液、アスパラギン酸液、グリシン液、システイン液、表1のアミノ酸混合液10.5ml/kgを麻酔剤(プロポフォール)の投与前60分より15分間隔で4回投与後、さらに麻酔剤投与開始と同時に42ml/kgで1時間持続投与した。各群の例数は4〜6例とした。麻酔剤の投与は、最初に15mg/kgをbolusにてカテーテルから静脈内投与し、同時に45mg/kg/hrの持続投与をも開始した。その1時間後からは投与速度を22.5mg/kg/hrにし、合計3時間の持続投与を行った。体温の測定は、直腸体温計にてアミノ酸液を投与する前に前値を測定し、麻酔剤投与開始から終了後の3時間までは30分間隔で測定した。
測定結果は、麻酔液投与3時間後の各々の投与前値に対する変化量(投与前値−投与後値)とし、平均±標準偏差で示した。統計学的な検定は、生理食塩液との2群間比較を
student t−testにて行った。
結果を表2に示す。バリン液、アスパラギン酸液、システイン液、グリシン液、及びアミノ酸混合液で体温(核心温)低下抑制効果が認められた。
(3)体温低下抑制効果測定2
投与タイミングの違い(以下のA〜D)により、バリンの体温低下抑制効果に差異がみられるか否か検討した。評価に用いるラット(7〜8週齢)は、予め外頸静脈にカテーテルを挿入し、背部にその先端をだしヘパリンでロックした状態で数日間回復させた。バリンは、
A:プロポフォール投与開始前2時間
B:プロポフォール投与開始前1時間+プロポフォール投与開始後1時間
C:プロポフォール投与開始と同時に2時間
D:プロポフォール投与開始後1時間の時点より2時間
の4通りの方法でカテーテルより投与を行った。A群には105mg/kg(1%バリン溶液を10.5mL/kg)を15分間隔で8回、B群には105mg/kg(1%バリン溶液を10.5mL/kg)を15分間隔で4回投与後、さらにその後420mg/kg/hr(1%バリン溶液を42mL/kg)で一時間持続投与、C群及びD群は420mg/kg/hr(1%バリン溶液を42mL/kg)で2時間持続投与した。各群(n=8〜9)のバリン総投与量は同じである。プロポフォール麻酔剤は最初に15mg/kgでカテーテルより投与し導入麻酔後、33.75mg/kg/hrで3時間持続投与した。
その結果、投与前から麻酔終了時までの体温低下は、A群が6.1±1.9℃、B群が4.3±1.2℃、C群が6.1±1.4℃、D群が6.9±1.0℃であり、B群の体温低下幅が最も小さく、次いで、A群及びC群が同程度であり、D群では体温低下抑制効果は認められなかった。また、麻酔投与終了時から麻酔投与終了後1時間までの体温の上昇は、B群(2.0±1.2℃)及びC群(2.0±0.9℃)が最も高く、次いでA群(1.1±0.5℃)、D群(1.0±0.6℃)であった。以上より,バリンはプロポフォール麻酔剤投与開始前または麻酔剤投与と同時に投与を行ったときに体温低下抑制作用が強く現れ,麻酔剤投与終了後の体温の上昇は,麻酔剤投与前後または麻酔剤投与と同時にバリンを投与したときに強く認められた。このように、麻酔剤投与前から麻酔剤投与中にバリンを投与することが好ましい。
(4)手術侵襲からの回復に対する効果
評価に用いるラット(7週齢)は、予め外頚静脈にカテーテルを挿入し背部にその先端を出しヘパリンでロックした状態で数日間回復させた。バリン投与群には、挿入カテーテルよりバリン105mg/kg(2W/V%バリン液を5.25ml/kg)をプロポフォール麻酔剤の投与前60分より15分間隔で4回投与後、さらにプロポフォール麻酔剤投与開始と同時に420mg/kg/hr(2W/V%バリン液を21ml/kg)で5時間持続投与した。対照群には、バリン液の代わりに生理食塩水を投与した。プロポフォール麻酔剤の投与は、最初に15mg/kgをbolusにてカテーテルから静脈内投与し、直後に45mg/kg/hrの持続投与をも開始した。その1時間後からは投与速度を33.75mg/kg/hrにし、5時間の持続投与を行った。各群の例数は8例とした。外科侵襲は、bolus麻酔後、ラットを開腹し盲腸部を腹腔から露出させ30分間空気暴露し、終了後盲腸部を腹腔内に戻し縫合した。体温の測定は、直腸体温計にてアミノ酸投与前に前値を測定し、プロポフォール投与後3時間に測定した。結果はPre値に対する低下した温度で示した。生存率は実験開始24時間後の生存数から求めた。
その結果、対照群の体温低下は、9.02±0.66℃(n=6、3時間測定まで2例死亡)であったのに対してバリン投与群では7.83±0.87℃(n=8)であり1.19℃の体温低下抑制が認められた。また、生存率(1日後)は、対照群が12.5%であったのに対しバリン投与群では87.5%であり、バリン投与により生存率の改善が見られた。
(5)末梢血単核球TNF−α産生に対する効果
試験は正常群,バリン投与群,対照群の3群で実施した。評価に用いるラット(7〜8週齢)は,予め外頸静脈にカテーテルを挿入し,背部にその先端をだしヘパリンでロックした状態で数日間回復させた。バリン投与群(n=6)には挿入カテーテルより105mg/kg(2W/V%バリン液を5.25mL/kg)をプロポフォール麻酔剤の投与前60分より15分間隔で4回投与後、さらにプロポフォール麻酔剤投与開始と同時に420mg/kg/hr(2W/V%バリン液を21mL/kg)で一時間持続投与した。対照群(n=6)にはバリンの代わりに生理食塩液を投与した。プロポフォール麻酔剤の投与は、最初に15mg/kgでカテーテルより投与し導入麻酔後、33.75mg/kg/hrで3時間持続投与した。開腹侵襲は、麻酔投与と同時にラットを開腹し、盲腸部を腹腔から露出させたのちただちに元に戻し、閉腹した。なお,正常群はカテーテル挿入、被験液投与及びプロポフォールの投与は行わなかった。麻酔投与終了と同時に腹部大動脈より血液を採取し、Nycoprep1.077Aを用いて比重法により末梢血単核球(PBMC)を分離・精製した。PBMCはlipopolisaccaraide(LPS)は1μg/mL存在下で10%FCS含有RPMI1640mediumにて24時間培養(37℃,95%空気,5%CO2)した。培養開始24時間後において培養上清を回収し、TNF−α測定まで凍結保存した。TNF−αの測定はELISA法にて行った。
その結果、TNF−α産生量は対照群(619.3±367.4pg/1×106cells)では正常群(1141.7±288.6pg/1×106cells)と比較して低値を示したが、バリン投与群(1389.8±415.8pg/1×106cells)は対照群と比較して有意に高値(p<0.05)を示し、正常群と同程度の値を示した。なお、麻酔投与終了時の体温は、バリン投与群が31.2±0.8℃、対照群が30.0±1.1℃であり、バリン投与群は対照群と比較して高値を示した。
表25の配合量に従い、塩化ナトリウム、L−乳酸ナトリウム、ブドウ糖を注射用蒸留水に溶解し、糖・電解質液を調製する。
(2)アミノ酸液の調製
L−バリンを5W/V%となるように注射用蒸留水に溶解し、アミノ液を調製する。
(3)プラスチック製容器への充填、滅菌、包装
調製した糖・電解質液とアミノ酸液の両液を無菌ろ過し、糖・電解質液は340mlを、隔壁で仕切られたプラスチック製容器(ダブルバッグ)の第1室に充填する。第1室を密封後、各プラスチック容器の第2室にアミノ酸液660mlを充填し、密封する。両液を充填・密封したプラスチック容器は高圧蒸気滅菌を行ない、冷却後に脱酸素剤とともに外包装材で包装する。
Claims (10)
- バリン、アスパラギン酸、グリシン及びシステインからなる群から選択される少なくとも1種のアミノ酸を、バリンについては0.01〜0.3W/V%、アスパラギン酸については0.02〜0.5W/V%、グリシンについては0.05〜0.4W/V%、システインについては0.01〜0.05W/V%含有し、かつこれ以外のアミノ酸を含有する輸液製剤の形態であり、総アミノ酸濃度が1.5〜5W/V%であることを特徴とする麻酔による体温低下の抑制剤。
- アミノ酸がバリンである請求項1記載の麻酔による体温低下の抑制剤。
- バリン、アスパラギン酸、グリシン及びシステインからなる群から選択される少なくとも2種のアミノ酸を組み合わせてなるアミノ酸混合物を有効成分として含有することを特徴とする請求項1記載の麻酔による体温低下の抑制剤。
- アミノ酸混合物がバリンを含有する請求項3記載の麻酔による体温低下の抑制剤。
- 更に電解質を含有することを特徴とする請求項1〜4のいずれか1項記載の麻酔による体温低下の抑制剤。
- バリン、アスパラギン酸、グリシン及びシステインからなる群から選択される少なくとも1種のアミノ酸を0.01〜1W/V%含有し、バリン、アスパラギン酸、グリシン及びシステイン以外のアミノ酸を含有しない輸液製剤の形態であることを特徴とする麻酔による体温低下の抑制剤。
- アミノ酸として、バリン、アスパラギン酸、グリシン及びシステインからなる群から選択される1種のアミノ酸のみを含有する請求項6記載の麻酔による体温低下の抑制剤。
- アミノ酸として、バリンのみを含有する請求項7記載の体温低下抑制剤。
- 輸液が静脈投与用である請求項1記載の体温低下抑制剤。
- 輸液が静脈投与用である請求項6記載の体温低下抑制剤。
