Description
本発明は動脈瘤及び類似の脈管異常部位を塞栓するための方法及び装置に関し、詳しくは、本発明は、動脈の、例えば瘤のような部位に挿通してその内部に塞栓を創出するための装置並びに、この装置を使用して動脈部位を塞栓する方法に関する。
(a)血管内でマイクロカテーテルを、該マイクロカテーテルの遠方端が目標血管部位内に導入されるように送ること、
(b)マイクロカテーテル内で脈管閉塞装置を、該脈管閉塞装置が目標血管部位の容積の一部分を充填する3次元形態を取るように目標血管部内に送ること、
(c)フィラメント状のキャリヤに非釈放自在に連結した拡張性の少なくとも1つの塞栓要素を含む血管塞栓装置を提供すること、
(d)マイクロカテーテル内で該血管塞栓装置を、該血管塞栓装置がマイクロカテーテルの遠方端を出て目標血管部位内に入るように送ること、
(e)単数あるいは複数の血管塞栓要素とキャリヤとの間の連結を維持しつつ、目標血管部位位置で該単数あるいは複数の血管塞栓要素を拡開させ、該目標血管部位の残部容積を血管塞栓要素で実質的に充填すること、
を含む血管部位を塞栓するための方法がある。
(a)血管内で血管内装置を、目標血管部位に隣り合う位置に配置すること、
(b)フィラメント状のキャリヤに非釈放自在に連結した少なくとも1つの拡張性の塞栓要素を含む血管塞栓装置を提供すること、
(c)血管内でマイクロカテーテルを、該マイクロカテーテルの遠方端が血管内装置を通して目標血管部位に送られるように送ること、
(d)マイクロカテーテル内で血管塞栓装置を、該血管塞栓装置がマイクロカテーテルの遠方端を出て目標血管部位入るように送ること、
(e)目標血管部位位置で、単数あるいは複数の血管塞栓要素とキャリヤとの間の連結を維持しつつ、該単数あるいは複数の血管塞栓要素を拡開させ、該目標血管部位の残部容積に血管塞栓要素を実質的に充填すること、
を含む血管部位を塞栓するための方法である。
この参考例の場合、血管内装置は米国特許第5,980,514号に記載される形式のものとすることができる。この血管内装置は、マイクロカテーテルにより動脈瘤その他との接合部に導入され、動脈瘤の狭窄部に隣り合うコイル形態を取る、繊維状要素を含む。
好ましい参考例における塞栓要素は、小さく、実質的に円筒状の、マイクロカテーテル内に貫入するに十分外径の小さい“マイクロペレット”形態の初期形状を有する。これらの塞栓要素は血管部位を実質的に一致する状態で充填する拡開形態へと親水的に拡張自在である。
図1、図2、図3には本発明に従う血管塞栓装置10が示される。好ましい実施例において血管塞栓装置10は、フィラメント状のキャリヤ14に沿って間隔を置いて位置付けた実質的に円筒状の“マイクロペレット”12として各々形成した複数の塞栓対を含む。塞栓要素12の数はキャリヤ14の長さに基づき、キャリヤ14の長さは血管の被塞栓部位の寸法に依存する。例えば血管部位が大きい場合は、8〜12個のマイクロペレットが使用され得るが、必要であれば更に多くのマイクロペレットを使用することができる。ある適用例(例えば非常に小さい動脈瘤)では、使用されるマイクロペレット数は1つあるいは2つといった少なさである。
図4から図7には血管塞栓装置10を使用する血管部位の塞栓方法の参考例が例示される。先ず図4を参照するに、既知の方法でマイクロカテーテル40が、その遠方端が目標血管部位(ここでは目標血管部位42)の内部に位置付けられるまで、血管内に挿通される。略述すると、この挿通は先ず、所望のマイクロカテーテル通路に沿ってカテーテルガイドワイヤ(図示せず)を導入し、次いで、このマイクロカテーテル40が動脈瘤のドーム部分の遠方側面に隣り合って位置決めされるまで、カテーテルガイドワイヤを越えて図4に示されるように送られる。次いでカテーテルガイドワイヤを除去し、図5及び図6に示されるように先に説明したように配置機器30の遠方端に取り付けた血管塞栓装置10を、配置機器30を使用してマイクロカテーテル40を通して軸方向に送り、この血管塞栓装置10がマイクロカテーテル40の遠方端を出て目標血管部位42内で完全に配置され(図6)て動脈瘤をその遠方側面から充填するまで押し出す。この配置手順は、先に説明したような放射線不透過部品により容易に実現されるところの血管塞栓装置10の可視化により容易化される。

Claims (21)
- 血管部位を塞栓するための装置であって、
細長の、フィラメント状の多重ループ構造のマイクロコイルと、
該マイクロコイルの表面上の所定位置に、マイクロコイルの長さの少なくとも一部分を包囲する状態で非釈放的に連結した拡張性の塞栓要素と、
を含む血管部位を塞栓するための装置。 - 塞栓要素が親水性のヒドロゲル発泡材料から形成される請求項1の装置。
- ヒドロゲル発泡材料が、整泡剤と、約10重量%までのマルチオレフィン機能架橋剤に架橋させた遊離基重合性のオレフィンモノマーのポリマーあるいはコポリマーとを含む微孔質の固形物として形成した水膨張性の発泡マトリクスを含む請求項2の装置。
- 塞栓要素が、ポリビニル発泡アルコール、発泡コラーゲン、ポリ(2−ヒドロキシエチルメタクリレート)から成る群から選択した材料から形成される請求項1の装置。
- 塞栓要素はその初期直径が約0.5mmより大きくなく、また、少なくとも約3.0mmの直径に拡張し得る請求項1の装置。
- 塞栓要素は、所定の初期容積を有し、該初期容積を少なくとも約25倍に拡大するべく拡張自在である請求項1の装置。
- 塞栓要素は第1の塞栓要素であり、塞栓装置は、マイクロコイル上の、前記第1の塞栓要素から離れた所定位置に機械的に連結した拡張性の第2の塞栓要素を少なくとも含んでいる請求項1の装置。
- マイクロコイル上で第1の塞栓要素と第2の塞栓要素との間に位置付けたマイクロコイルスぺーサを更に含む請求項7の装置。
- マイクロコイルが、多重ループ形態に形成される薄く且つ柔軟な金属製のワイヤを含む請求項1の装置。
- ワイヤが、良好な弾性記憶特性を有する、ニッケル及びチタンの合金から形成される請求項9の装置。
- マイクロコイルが、多重ループ形態に形成される薄いフィラメント状のポリマーを含む請求項1の装置。
- マイクロコイルが、弾性記憶を有する可撓性材料から形成され、初期において多重ループ形態に形成され、塞栓要素が、該マイクロコイルの長手方向に沿って間隔を置いて位置付けた複数の、拡張性の塞栓要素の1つである請求項1の装置。
- マイクロコイルが、拡張性の塞栓要素を位置付ける中間位置と、近接部分と、遠方部分とを有する請求項12の装置。
- マイクロコイルの中間位置が、概略第1の直径を有する少なくとも1つのループに形成され、近接部分が概略前記第1の直径を有する少なくとも1つのループに形成され、遠方部分が前記第1の直径よりも大きい概略第2の直径を有する少なくとも1つのループに形成される請求項13の装置。
- マイクロコイルの近接部分上に拡張性のリンク要素を更に含む請求項13の装置。
- 拡張性のリンク要素が塞栓要素と同じ材料から形成される請求項15の装置。
- 塞栓要素が親水性のヒドロゲル発泡材料から形成される請求項12の装置。
- ヒドロゲル発泡材料が、整泡剤と、約10重量%までのマルチオレフィン機能架橋剤に架橋させた遊離基重合性のオレフィンモノマーのポリマーあるいはコポリマーとを含む微孔質の固形物として形成した水膨張性の発泡マトリクスを含む請求項17の装置。
