Description
本発明の別の目的は、単一の又は任意の二次的エネルギ付与ステップにおいて、引き手部材(a pull handle)アセンブリを介してピンを保持する皿バネを取り除くことにより容器の内容物を加圧することを許容するパッチ状注入装置を提供することにある。
以下に説明される本装置の実施態様は、予め計られた投与量の、薬剤又は薬物のような物質を接着剤で取付けられている注入装置を介して使用者に送出する、便利な、パッチ状装置として使用され得る。該装置は、内蔵型であり、底面に配置されている接着剤により使用者の皮膚表面に取り付けられる。使用者により、一旦、適切に位置決めされ、起動されると、装置内の容器表面の解放された皿バネの圧力が、針マニホルドを介して1又はそれ以上の患者の顕微針を通して可撓性容器の内容物を空にすべく使用され得る。次に、容器内の物質は、装置に含まれる1又はそれ以上のバネで皮膚内に駆動される顕微針により使用者の皮膚を通って送出される。皿バネが本質的に機械的、電気的及び/又は化学的であり得る異なるタイプのエネルギ貯蔵装置と取り替えられる別の実施態様が可能であることが理解されるであろう。
図1から11に示される本発明に係る第1実施態様において、注入装置1000は、容器サブアセンブリ100を含んでいる。該容器サブアセンブリ100は、上部ハウジング110、容器ベース表面120、少なくとも1つの皿バネ130、保持ピン140、充填プラグ150、隔壁160及び容器用フィルム170を含んでいる。注入装置1000は、ハウジング・サブアセンブリ200をさらに含んでいる。該ハウジング・サブアセンブリ200は、低部ハウジング210、及び少なくとも1本の患者用針222及びマニホルド・フィルム224を有する患者用針マニホルド220を含んでいる。ハウジング・サブアセンブリ200は、針遮蔽体230、針遮蔽体駆動用バネ232及び調整可能な針用キャップ240をさらに含んでいる。接着層250は、低部ハウジング210の低部表面に配置され、取り外し可能なフィルム(不図示)及び引き手部材(a pull handle)260により覆われる。「E」形クリップのようなクリップ270が、保持ピン140を引き手部材260に固定するのに使用され得る。注入装置1000は、押しボタン・サブアセンブリ300をさらに含んでいる。該押しボタン・サブアセンブリ300は、少なくとも1つの患者用針マニホルド駆動用バネ310、押しボタン送り台320、少なくとも1つの隔壁用針330、隔壁用針遮蔽体340及び流体連通用チューブ350を含んでいる。ボタン前面360は、押しボタン・サブアセンブリ300を完成するために設けられ得る。以下の記載において、容器という用語は、容器サブアセンブリ100の組み立てられたもの及び容器アセンブリ100の個々の容器ベース表面120、充填プラグ150、隔壁160及び容器フィルム170について記載するのにしばしば用いられる。
図10Aに注入装置1000の容器サブアセンブリ100が示されており、該容器アセンブリ100は、1又はそれ以上の金属蒸着フィルムのような非膨張性であるが可撓性フィルム170と併せて使用される硬質部分120から構成され得る。容器アセンブリ100は、第1フィルムと第2フィルとの両方の間に物質が何種類でも入り得る。第1フィルム又は第2フィルムのどちらかは、また、硬質部分に対して配置され、あるいは、第1フィルムと硬質部分との間に配置される。
図6及び図10Aに示されるように、皿バネ130は、容器サブアセンブリ100の可撓性フィルム170にほぼ均一で一定の圧力を加えるように設けられ、可撓性フィルム170と硬質部分120の間の容器の内容物を圧縮し、図12に詳細に示されるように、該内容物を容器から1又はそれ以上の流体通路に強制的に通す。図12は、流体通路及び及び図10Aの容器サブアセンブリの部分的断面図を例示する。上述したように、図10Aの容器は、また、2又はそれ以上の可撓性で、非膨張性のフィルムで作られてもよい。この場合、内容物は、容器の内容物を圧縮し、加圧する硬質ベースを提供すべく少なくとも1枚のフィルムが硬質部分120に取り付けられるフィルム間に入れられ得る。容器サブアセンブリ100のさらに別の実施態様においては、流量は、初期の高流量から1段階以上減少された低流量まで自動的に調整される。流量調整に関するさらなる詳細は、「多段階流体送出装置及び方法」との名称で2003年3月26日に出願された米国特許出願第10/396,719号に議論されている。その内容全体は、本明細書に参照により組み入れられている。
図10Aの容器サブアセンブリ100の硬質部分120は、容器の主室127に到達する、図12に示されるような少なくとも1つの流路128をさらに備えている。図12に示される実施態様において、流路128は、容器の主室127を出て、可撓性フィルム170を固定するために硬質部分120の外周を巡って設けられているヒート・シール領域の下又はそれを通って通過し、充填ヘッド栓150と隔壁160との間の室129に入り、容器の流体を容器から隔壁160まで運ぶことを可能とする。図12に示される実施態様において、流路128は、デッド・ボリューム(dead volume)を減少させるように構成されていることが好ましく、以下に詳細に述べられるような充填ヘッド受容形状を包含する。
図10A及び12に戻って、隔壁160と容器との間の室129は、また、充填用ヘッド栓150で閉じられ得る容器サブアセンブリ100に配置されている充填用ヘッド・ポート152を介してアクセスされ得る。充填用ヘッド栓150及び隔壁160は、同一の部品であってもよい。このことは製造における複雑さを減少させる。
図10Aに示されるように、円板バネ又は皿バネ130は、本質的に均一で一定の力を容器に加え、内容物を容器から強制排出させるために、装置1000内に含まれている。該バネ130は、以下、時に「一定力バネ(a constant force spring)」と称される。一定力バネ130は、装置の起動により解放されると使用時において容器を加圧するエネルギを蓄えるのに使用される。バネ130は、複数のバネ指の中心に位置しているピン140により撓んだ状態に保持される。そうすることで、バネは、蓄積中、容器サブアセンブリ100のフィルム170又は残りの装置の構成要素いずれにも応力を及ぼすことを妨げられている。
図10Bに戻って、底部すなわち下部ハウジング210は、上述した上部ハウジング110及び容器サブアセンブリ100と嵌合し得るように設けられている。下部ハウジング210は、全ての残りの構成要素を閉じ込め、収容すべく用いられ、構成要素及びハウジング部材を受け入れ取付けるためスナップ機能を提供し得る。下部ハウジング210は、また、以下でより詳細に記載されるように、ボタン送り台320及び患者用針マニホルド220を固定し、解放し、且つ誘導するために、1又はそれ以上の案内機能を含み得る。ユニット間、例えば上部ハウジングユニットと下部ハウジングユニット間の破断線が装置の垂直中心線に向けて配置され得る。装置は、以下に述べられる押しボタン・サブアセンブリが平板上の代わりに頑丈なハウジング内に逆さまにして積載され得るので、よりしっかりしたアセンブリを作り出す。上部ハウジング110及び下部ハウジング210は、それぞれ、互にスナップ結合又は超音波で結合され得る。
図10B及び10Cに戻って、開示される装置は、また、少なくとも1本の患者用針222すなわち顕微針を含んでいるが、例えば図10Cの押しボタン・サブアセンブリ300に示される3本の顕微針のように数本の針を含んでいてもよい。各顕微針222は、少なくとも31ゲージ又はそれより小さい、例えば34ゲージであることが好ましい。また、各顕微針222は、容器と流体連通状態で配置され得る患者用針マニホルド220内に固定されている。各顕微針は、1ポンド(約4.4N)以下の力でマニホルド220から分解されることを防止すべく固定されている。顕微針222は、また、1以上の針が装置に含まれる場合、異なる長さ又はゲージ、あるいは、異なる長さ及びゲージの組み合わせを持っていてもよい。そして、顕微針222は、本体の長さ方向に沿って、好ましくは針の先端近く又は該針が持っているのであればその先端斜面部の近くに、1又はそれ以上のポートを含んでいてもよい。
上述された実施態様において、非常に大きなカニューレ又は針を必要とする速やかな注射器による注射と典型的に関連するより長い期間にわたって注入が起こるので、容器内容物を送出するために複数の34ゲージ針の使用が実用的である。開示される実施態様において、皮内又は皮下の空間のどちらかを目標とするものであればどのような顕微針であっても使用され得る。しかしながら、図10Cに示される実施態様は、1〜4mmの間(すなわち、2mm)の露出長さを持つ顕微針を含み、特定の適用例が必要とする場合、針を何本でも含んでいてもよい。
図10Cに押しボタン・サブアセンブリ300が示され、隔壁用針330、隔壁用針マニホルド322及び押しボタン送り台320を一体にしている。しかしながら、押しボタン・サブアセンブリ300の構成は、スナップ式の押しボタン前面板360を設け、2又はそれ以上のより簡単な成形ボタン部品を可能にすることで幾分簡素化され得る。押しボタン送り台320は、また、患者用針マニホルドを後退位置に固定し、装置が適切に起動したとき該マニホルドを解放する機構を提供する。
上述した装置は、薬物及び製薬剤を含むいろいろな物質を患者特に人間の患者に投与するのに用いるのに適している。本明細書で用いられるように、製薬剤は、体膜及び身体表面、特に皮膚を介して送出され得る生物活性を有する物質を含んでいる。以下により詳細に掲載される例は、抗生物質、抗ウイルス剤、鎮痛剤、麻酔薬、食欲抑制剤、関節炎治療薬、抗うつ剤、抗ヒスタミン剤、抗炎症剤、抗腫瘍性剤、DNAワクチンを含むワクチンなどを含んでいる。患者に皮内又は皮下に送出され得るその他の物質は、ヒト成長ホルモン、インスリン、プロテイン、ペプチド及びそのフラグメントを含んでいる。プロテイン及びペプチドは、自然界に存在し、化学的に合成され又は組み換え的に生成され得る。さらに、装置は、樹状細胞の皮内注入中のような細胞治療法に使用され得る。
不注意から起きる又は偶発的な針刺し及び装置の意図的再利用を避けるために、また、露出した針を遮蔽するために、ロッキング針安全機構が設けられ、皮膚表面から装置が取り除かれるとすぐに自動的に起動される。
図19A及び19Bに示される装置の第2実施態様は、装置の起動及びエネルギ付与は、単一の多機能/ステップ工程で達成される押しボタン・デザイン400である。図19Aは、側部押しボタンを使用するパッチ状注射器又は注入器システムの第2実施態様の分解斜視図であり、図19Bは、第2実施態様の患者用針/隔壁用針マニホルドシステムの断面図であり、図19Cは、エネルギ付与及び起動前の図19Aに示される第2実施態様の断面図であり、図19Dは、エネルギ付与及び起動後の図19Aに示される第2実施態様の断面図である。
図20A〜20Cに示される、装置の第3実施態様は、空気袋自体が患者の皮膚に向かって移動し、(反対方向に位置決めされている)患者用針と隔壁用針の両方を有するマニホルドに接触させ、患者用針を患者の皮膚内に押し込み、隔壁用針を隔壁内に押し込む押しボタン・デザイン500である。図20Aは、側部押しボタンを使用するパッチ状注射器又は注入器システムの第3実施態様の分解斜視図であり、図20Bは、エネルギ付与及び起動前を示す第3実施態様の断面図であり、図20Cは、エネルギ付与及び起動後を示す第3実施態様の断面図である。
図21A〜21Cに示される装置の第4実施態様は、押しボタンが装置の頂部外側の表面に配置され、使用者が、ボタンをその最も低い位置に押し下げることにより、エネルギを付与し、流体流れを起動する押しボタン・デザインである。図21Aは、頂部押しボタンを使用するパッチ状注射器又は注入器システムの第4実施態様の分解斜視図であり、図21Bは、エネルギ付与及び起動前の、図21Aに示される第4実施態様の部分断面図であり、図21Cは、エネルギ付与及び起動後の、図21Aに示される第4実施態様の部分断面図である。
好ましい装置の実施態様とその他の方法及び装置との間のいろいろな結果及び比較が図22〜26に示されている。図22のグラフは、その後の糖尿病のブタによる試行で使用される測定流量を確立すべく生体外での長い送出時間にわたる流量の一様性を示す。これは、本発明の実施態様にしたがって、圧力対送出容積データを示す図26にさらに例示される。可撓性容器に圧力を加える皿バネの使用は、結果として、上述した図1の実施態様を使って25時間の注入期間にわたってほぼ一定の送出量になる。
Claims (43)
- それらの間に流体室を画成する、対向する可撓性壁と硬質の壁を有する、患者に注入される流体を収容する流体容器と、
前記容器と流体連通状態にある少なくとも1本の患者用針と、
流体流路が完成されると、前記容器と前記患者用針との間の前記流体連通を提供する流体流路と、
前記流体流路を完成させることにより、パッチ状注入装置の操作を開始させる押しボタン作動子と、
少なくとも前記患者用針から保護キャップを取り除くことを含む先行する操作が実行されるまで、前記押しボタン作動子の操作を阻止する保護体又は連結体と、
前記容器の可撓性壁に圧力を加え、流体を前記容器から前記患者用針まで流れさせるエネルギ貯蔵装置と、
を備えることを特徴とするパッチ状注入装置。 - 前記エネルギ貯蔵装置は、バネを含むことを特徴とする請求項1に記載のパッチ状注入装置。
- 前記バネは、皿バネを含むことを特徴とする請求項2に記載のパッチ状注入装置。
- 前記容器は、少なくとも部分的に透明であり、その内容物が視覚的に点検されることを可能とすることを特徴とする請求項1に記載のパッチ状注入装置。
- 前記可撓性壁は、プラスチック・フィルムからなることを特徴とする請求項1に記載のパッチ状注入装置。
- 前記プラスチック・フィルムは、線状低密度ポリエチレンの第1層、低密度ポリエチレンの第2層、アルミニウムの第3層、低密度ポリエチレンの第4層、及びナイロン及びポリエステルからなる群から選択される第5層を含んでいることを特徴とする請求項5に記載のパッチ状注入装置。
- 前記硬質の壁は、環状オレフィン共重合体(COC)層を備えることを特徴とする請求項1に記載のパッチ状注入装置。
- 前記注入装置が使用状態に置かれると、前記エネルギ貯蔵手段が前記容器の可撓性壁に前記圧力を加えることを開始させる作動手段をさらに備えることを特徴とする請求項1に記載のパッチ状注入装置。
- 前記エネルギ貯蔵装置は、中心開口を有する皿バネを備え、前記作動手段は、前記開口と係合可能な及び該開口から取り外し可能な保持器を備えることを特徴とする請求項8に記載のパッチ状注入装置。
- 前記保持器は、ピンを含むことを特徴とする請求項9に記載のパッチ状注入装置。
- 前記皿バネは、前記可撓性壁に圧力を加え、流体を前記容器から前記患者用針へ流れさせるための、中心開口を有し、
前記保持器は、前記注入装置が操作状態に置かれると、前記皿バネが前記圧力を加えることを開始させることを特徴とする請求項9に記載のパッチ状注入装置。 - 前記保持器はピンを含むことを特徴とする請求項11に記載のパッチ状注入装置。
- 前記保持器の前記開口からの取り外しは、前記注入装置の使用者に対する少なくとも1つの可聴の指示器及び触覚的指示器を提供することを特徴とする請求項11に記載のパッチ状注入装置。
- 前記保持器の前記開口からの取り外しは、使用者が前記注入装置の別の操作を実行すると自動的に起こることを特徴とする請求項11に記載のパッチ状注入装置。
- 前記操作は、前記押しボタン作動子を操作することを含むことを特徴とする請求項14に記載のパッチ状注入装置。
- 前記操作は、前記保護体又は連結体を取り除くことを含むことを特徴とする請求項14に記載のパッチ状注入装置。
- 前記流体流路は、前記容器からの流体流れを阻止する第1の突き刺し可能な隔壁と、前記第1の突き刺し可能な隔壁を突き刺し、前記流体を、前記流路を通って前記患者用針へ導く前記患者用針とは異なる隔壁用針とを備えることを特徴とする請求項1に記載のパッチ状注入装置。
- 前記流体流路は、前記隔壁用針と前記患者用針との間に介在する1本の可撓性チューブをさらに備えることを備えることを特徴とする請求項17に記載のパッチ状注入装置。
- 前記患者用針を運ぶ患者用針マニホルドをさらに備え、前記1本の可撓性チューブの一端は、前記マニホルドに接続されることを特徴とする請求項18に記載のパッチ状注入装置。
- 前記流体流路は、患者への流体注入量を制御する流れ制限器を含むことを特徴とする請求項17に記載のパッチ状注入装置。
- 前記患者用針は、前記隔壁用針より小さいゲージを有することを特徴とする請求項17に記載のパッチ状注入装置。
- 前記患者用針は、31ゲージ又はそれより小さいことを特徴とする請求項21に記載のパッチ状注入装置。
- 前記流体流路は、前記容器が流体で充填されることを可能とする充填ポートを含むことを特徴とする請求項17に記載のパッチ状注入装置。
- 前記充填ポートは、第2の突き刺し可能な隔壁を含み、前記第1の突き刺し可能な隔壁及び第2の突き刺し可能な隔壁は、互に離れて間隔をおいて配置され、それらの間に室を画成し、該室が前記流体流路の一部を形成することを特徴とする請求項23に記載のパッチ状注入装置。
- 前記患者用針を支持し、マニホルド入口から前記患者用針へ流体を導く、前記患者用針と流体連通状態にあるマニホルドをさらに備え、
前記マニホルドは、硬い低部部分の頂部で開いた上方に面した内部空洞を画成する硬い低部部分であって、その下面から突出する前記患者用針を有する低部部分を備え、前記マニホルドは、前記空洞を閉じ、それにより、前記マニホルド入口と流体連通する前記マニホルド内の内部室を画成する、前記硬い低部部分の頂部に接着される可撓性フィルムをさらに備えることを特徴とする請求項1に記載のパッチ状注入装置。 - 前記硬い低部部分は、アクリル・ポリマーからなることを特徴とする請求項25に記載のパッチ状注入装置。
- 前記可撓性フィルムは、金属蒸着プラスチック積層体からなることを特徴とする請求項25に記載のパッチ状注入装置。
- 前記、可撓性フィルムは、ヒート・シールにより前記硬い低部部分の頂部に接着されることを特徴とする請求項25に記載のパッチ状注入装置。
- 前記空洞は、前記マニホルド内の流体容積を最小限にするように構成されていることを特徴とする請求項25に記載のパッチ状注入装置。
- 複数の患者用針が設けられ前記マニホルドは、前記空洞を前記患者用針に接続する流体経路を含むことを特徴とする請求項25に記載のパッチ状注入装置。
- 前記患者用針は、線形パターンに配列されていることを特徴とする請求項30に記載のパッチ状注入装置。
- ハウジングと、
前記注入装置の使用後、前記患者用針を覆う、前記ハウジングによって収容される回転可能な安全遮蔽体と、
をさらに備え、
前記流体容器は、前記ハウジング内に設けられ、
少なくとも1つの前記患者用針は、前記ハウジングによって収容されることを特徴とする請求項1に記載のパッチ状注入装置。 - 前記安全遮蔽体は、前記ハウジングの患者接触表面にほぼ平行な軸の周りに回転可能であることを特徴とする請求項32に記載のパッチ状注入装置。
- 前記安全遮蔽体は、前記ハウジング内の引込位置と突出位置との間で前記軸の周りに回転可能であり、安全遮蔽体は、前記患者接触表面より下に突出し、前記患者用針を覆うことを特徴とする請求項33に記載のパッチ状注入装置。
- 前記注入装置の使用後、前記安全遮蔽体を前記突出位置に回転させるバネをさらに備えることを特徴とする請求項32に記載のパッチ状注入装置。
- 前記少なくとも1本の患者用針は、前記ハウジングから突出可能であり、
前記流体流路は、前記注入装置が完全に組み立てられて後、前記容器が充填されることを可能とする、前記ハウジングの外からアクセス可能な充填ポートを含むことを特徴とする請求項32に記載のパッチ状注入装置。 - 前記充填ポートは、突き刺し可能な隔壁を含むことを特徴とする請求項36に記載のパッチ状注入装置。
- 前記先行する操作は、前記流体容器を加圧することを含むことを特徴とする請求項1に記載のパッチ状注入装置。
- 前記患者用針は、前記押しボタン作動子の操作に応答して、遮蔽された位置から注入位置へ移動することを特徴とする請求項1に記載のパッチ状注入装置。
- ハウジングと、
前記ハウジング内にあり、そこから前記流体を受け取る、前記流体容器と流体連通状態にある共通室を画成する患者用針マニホルドと、
前記共通室から前記流体を受け取る、前記マニホルドによって収容できる複数の患者用針と、
をさらに備え、
前記流体容器は、前記ハウジング内に設けられ、
前記患者用針マニホルドは、前記ハウジング内であって、前記患者用針が前記ハウジング内に遮蔽されている第1位の位置と、患者の皮膚内への注入のために前記患者用針が前記ハウジングから突出する第2の位置との間で移動可能であることを特徴とする請求項1に記載のパッチ状注入装置。 - 前記患者用針は、前記マニホルドが前記第2の位置にある時、前記ハウジングの底面の開口を通って突出することを特徴とする請求項40に記載のパッチ状注入装置。
- 前記患者用針の各々は、31ゲージまたはそれより小さく、露出される長さが4mm以下であることを特徴とする請求項40に記載のパッチ状注入装置。
- 前記患者用針マニホルドを前記第1の位置から前記第2の位置に移動させる押しボタン作動子をさらに備えることを特徴とする請求項40に記載のパッチ状注入装置。
