Abstract
Description
脱落したFOLR1を検出するのに使用される幾つかの以前のアッセイはFOLR1に対して十分に特異的であるわけではない。たとえば、一部のアッセイは、FOLR1と他の受容体ファミリーメンバー(FOLR2、3及び4)との間を区別せず、総FBP(葉酸結合タンパク質)についての値を報告もしない。さらに、一部のアッセイは、ヒトの試料(たとえば、血漿)を軽い酸洗浄ステップで予備処理して葉酸を受容体から解離することを必要とする。一部のアッセイ結果は、抗体療法と診断用抗体の間での競合効果のために不正確さも有し得る。さらに、多数の市販のキットは従来、その試薬及びロット間の安定性の双方で頼りにならない。これらのキットの評価は非常に入り交じった結果を生じており、研究用途のみを対象としている。多数が、「マトリクス効果」による疑陽性の機会を減らすように分析の前にヒト試料を予備希釈することを求めている。従って、FOLR1に基づく治療法のための比較としてFOLR1の臨床的に関連するダイナミック・レンジを検出することができる感度の高い且つ正確な診断用アッセイに対する明らかなニーズがある。

患者の血漿試料にて循環する抗原(脱落抗原)のレベルを測定することは、たとえば、抗体メイタンシノイド複合体(AMC)治療に応答する可能性がより高い患者集団を特定するのに役立ち得る。高いレベルの脱落抗体は治療用抗体の薬物動態に顕著に影響することが報告されている(Tolcher,A.ら 20th Symposium on Molecular Targets and Cancer Therapeutics;October,21-24,2008;Geneva,Switzerland:EORTC-NCI-AACR,p163,#514;Baselga,Jら J.Clin.Oncol.14:737-744(1996))。患者の血漿試料に由来する脱落抗原のレベルは、たとえば、抗原の標的、疾患の適応及び疾患の経過に応じて可変であると思われる。現在、抗FOLR1免疫複合体IMGN853についての疾患の適応における脱落抗原のレベルは不十分にしか調べられていない一方で、固形腫瘍での発現との相関は限定されている。FOLR1の上昇が卵巣腺癌で報告されている一方で、データは、たとえば、小細胞肺癌のような他のFOLR1+腫瘍の適応ではそれは上昇しないことを示唆している(Mantovani,LTら Eur.J.Cancer,30A(3):363-9(1994);Basal,Eら PLoS ONE,4(7):e6292(2009))。本方法は、本明細書で提供され、脱落FOLR1のダイナミック・レンジを検出することが可能である抗体及びその抗原結合断片を用いて高い葉酸の存在下でFOLR1受容体の検出を可能にする。以前のアッセイはアッセイの設計にてMov19を利用していた。IMGN853はMov19を含有し、一実施形態では、本発明の標的療法であるので、Mov19の存在下または非存在下で方法がFOLR1を検出することは必須である。Mov19を利用する以前のアッセイは競合効果を有し、IMGN853治療を受けている患者にてFOLR1を有意に少なく検出するまたはまったく検出しないであろう。

FOLR1の検出について基質及び指示薬の使用が企図される。
性である茶色の最終生成物を生じる、HRPのような酵素の基質である。DABの酸化は重合も引き起こし、四酸化オスミウムと反応する能力を生じるので染色強度及び電子密度を高める。重合したDABの光学密度を強化するのに使用される幾つかの金属及び方法のうちで、硫化銀と組み合わせた塩化金が最も上手く行くと思われる。

免疫組織化学法(IHC)染色に好適である抗ヒトFOLR1モノクローナル抗体(本発明の抗体)を産生するハイブリドーマを16,000を超えるハイブリドーマから選択した。ハイブリドーマは、ヒトFOLR1、ヒトFOLR1/マウスIgG2a Fc組換えタンパク質及びヒトFOLR1組換えタンパク質を形質移入したホルマリン固定した300-19細胞を含む様々な抗原で野生型Balb/cマウスを免疫することによって作出した。固定した300-19細胞による免疫は、アジュバントの非存在下でのPBS中の形質移入した300-19細胞(5E6細胞/マウス/注射)の皮下注射によって行った。FOLR1組換えタンパク質による免疫は完全フロイントアジュバント(CFA)または追加免疫用の不完全フロイントアジュバント(Sigma)またはMagicマウスアジュバント(Creative Diagnostics)にて乳化したタンパク質の皮下注射によって行った。一般に、2週間間隔で5回マウスを免疫した後、融合の3日前に免疫源の腹腔内注射によって最後の追加免疫を行った。
イブリドーマのうち、FACSスクリーニングによって陽性である14のハイブリドーマが発見された。陽性ハイブリドーマはすべてヒトFOLR1/マウスIgG2aFc組換えタンパク質で免疫したマウスを起源とした。

FOLR1陽性の変性細胞上でFACSにより陽性の一次クローン(合計10クローン)をIHCによって評価した。クローン2つは融合352から得られた(クローン352.1及び352.2)。6つのクローンは融合353から得られ(クローン353.1、353.2、353.3、353.5、353.9、353.15)、2つのクローンは融合354から得られた(クローン354.1及び354.2)。培養したハイブリドーマ細胞からハイブリドーマ上清を回収し、解析に使用した。上清からマウスモノクローナル抗体を捕捉するための抗マウスL鎖特異的ポリクローナル抗体及び捕捉抗体を検出するための抗マウスFc特異的なポリクローナル抗体を用いたELISAによってハイブリドーマ上清における抗体濃度を測定し、既知の濃度のマウスモノクローナルIgG1試料をIgG濃度を算出する基準として用いた。細胞培養培地(非希釈)はIHC染色法を妨害しないことが示された(一次抗体の代わりに培地が使用された場合、バックグランド染色/非特異的染色がないことを確認した)。Leica抗体希釈液中10μg/mLまでの種々の濃度に希釈した10の上清(IMGN352.1、352.2、353.1、353.2、353.3、353.5、353.9、353.15、354.1、及び354.2)を、FOLR1の既知の陽性対照試料(ヒト正常肺、患者に由来する卵巣漿液性乳頭状腺癌及びKB細胞)を用いて染色し、評価して陽性の候補クローン(FOLR1陽性試料にて許容できる膜染色と特異性を示すクローン)を特定した。10のクローンのうち5つがFOLR1陽性試料にて許容できる膜染色及び良好な特異性を示した。5つの候補クローンのそれぞれについて好適な染色濃度が以下:353.1(0.7μg/mL)、353.2(2.3μg/mL)、353.3(2.3μg/mL)、353.5(2μg/mL及び10μg/mL)、及び353.9(2μg/mL及び10μg/mL)のように実験的に決定された。残りの5つのクローンのうち、クローン353.15(2μg/mL及び10μg/mLで染色した)はKB細胞及び正常肺組織で許容できる膜染色を示したが、細胞質の染色のみは調べた患者の卵巣癌組織にて観察された。クローン352.1、352.2及び354.1はどの試料でも目に見える染色を示さず、クローン354.2は明らかな非特異的細胞質染色しか示さず、すべて許容できないと見なされた。

実施例2にて上述したように、FACSスクリーニングによる一次確認に基づいて選択された14のハイブリドーマクローンのうち、10の一次クローンを免疫組織化学(IHC)解析によって解析した。10の一次クローン(すなわち、352.1、352.2、353.1、353.2、353.3、353.5、353.9、353.15、354.1、及び354.2)のうち、5つ(すなわち、353.1、353.2、及び353.3、353.5、及び353.9)がIHCによって陽性であり、5つすべてが同じ融合(353)に由来した。一次クローン353.2のサブクローン1つを選択して353.2-1(「2.1」)と命名した。一次クローン353.3のサブクローン1つを選択して353.3-8(「3.8」)と命名した。一次クローン353.5のサブクローン1つを選択して353.5-7(「5.7」)と命名した。一次クローン353.9のサブクローン2つを選択して353.9-20(「9.20」)及び353.9-21(「9.21」)と命名した。サブクローン9.20及び9.21を配列決定したが、予想どおり、双方は同じ配列を有した。加えて、クローンの2つ2.1及び9.20をそれぞれ2013年4月16日、それぞれPTA-120197及びPTA-120196としてATCCに寄託した。
FOLR1-陽性(Igrov-1、Ovcar-3、Caov-3、Wish、及びSkov-3)及びFOLR1-陰性(BxPC3、Panc-1、及びASPC1)の細胞株から調製した細胞溶解物のパネルによるウエスタンブロットによって、生成された抗体の特異性を解析した。アッセイについては、常法によって溶解物をSDSポリアクリルアミドゲル電気泳動にて泳動し、ニトロセルロース膜に移した。膜を本発明の抗FOLR1抗体と共にインキュベートし、形成された抗原/抗体複合体を西洋ワサビペルオキシダーゼ(HRP)に結合させた抗マウス二次抗体によって検出した(図3)。調べた抗FOLR1抗体はすべて高レベルのFOLR1発現を伴った細胞株(すなわち、Igrov-1及びWish)におけるFOLR1を認識した。低い発現の細胞株Ovcar-3、Caov-3及びSkov-3におけるFOLR1は抗FOLR1クローン2.1及び9.21によってのみ検出され、クローン3.8及び5.7はたぶん抗体の不十分な感度のためにこれらの細胞溶解物を染色しなかった。FOLR1陽性細胞株にてクローンによる追加の非特異的なバンドは検出されず、FOLR1陰性細胞株の染色も観察されなかった。
変性させた及び変性させない(ネイティブの立体構造)FOLR1に結合する抗FOLR1抗体の能力をFOLR1陽性細胞KB及びT47Dによる間接FACSによって解析した。Versineによって細胞を回収し、リン酸緩衝化生理食塩水(PBS)で洗浄した。変性させた細胞は、10%ホルムアルデヒドを含有するPBSにて細胞を4℃で一晩インキュベートし、その後、PBSで洗浄し、95℃で30分間インキュベートすることによって調製した。次いで変性させた及び変性させない細胞を氷上でFACS緩衝液(2%正常ヤギ血清で補完されたRPMI-1640培地)で希釈した抗FOLR1抗体と
共に2時間インキュベートした。細胞を遠心し、PBSで洗浄し、FITCを結合したヤギ抗マウスIgG抗体と共に40分間インキュベートした。細胞を再び遠心し、PBSで洗浄し、1%ホルムアルデヒドを含有する0.2mLのPBSで再懸濁させた。HTSマルチウェルと共にFACSCaliburフローサイトメトリー用いて細胞に関連する蛍光を測定し、CellQuest Pro(BD Biosciences,San Diego,US)を用いて解析した。図4に示すように抗FOLR1抗体はすべて変性させた及び変性させない細胞双方に結合した。
組換えヒトFOLR1/マウスFc2aタンパク質を抗原として用いたELISAによって抗FOLR1抗体の結合親和性を調べた。組換えタンパク質をマイクロタイタープレートに不動化し、抗体を様々な濃度でプレートに加えた。プレートを室温で2時間インキュベートし、0.05%のTween20で補完したPBSで洗浄し、HRP標識したヤギ抗マウス二次抗体と共に室温で1時間インキュベートした。プレートをPBS/Tween20で再び洗浄し、結合したHRP複合抗体をHRPの基質TMB(Bio-FX)を加えることによって検出した。代表的な結果を図5に示す。抗FOLR1抗体は0.5~0.9nMの50%効果濃度(EC50)にてヒトのFOLR1に対して類似の親和性を有した。
FOLR1は葉酸受容体ファミリーのメンバーである。抗FOLR1抗体の他のファミリーメンバーFOLR2及びFOLR3との交差反応性をELISAによって評価した。組換えタンパク質FOLR2-HisまたはFOLR3-His(R&D Systems)をNi-NTAプレート(QIAGEN)に不動化し、抗FOLR1抗体をプレートに加え、室温で2時間インキュベートした。FOLR2及びFOLR3のELISAの陽性対照として、それぞれポリクロナール抗FOLR2及び抗FOLR3抗体(R&D Systems)を用いた。形成された抗体/抗原複合体をHRP標識したヤギ抗マウス二次抗体によって検出した。図6に示すように、本発明の抗FOLR1抗体はFOLR2またはFOLR3に結合せず、対照抗体だけが相当する抗原を検出した。
ヒトのFOLR1は69位、161位及び201位(UniProt)にてN-グリコシル化の3つの可能性のある部位を有し、文献で報告されたように、3つの部位すべてがグリコシル化される。本明細書で記載される抗FOLR1抗体によって認識されるエピトープの性質を特徴付けるために、脱グリコシル化した受容体及び未処理の受容体で結合実験を行った。配列データに基づいてクローンは同じエピトープに関連し、結合すると思われるので、生成された抗FOLR1クローンのうちでクローン2.1のみを試験で使用した。クローン2.1に加えて、他の2つの抗FOLR1抗体:huMov19(WO2011/106528)及びクローンBN3.2(Leica)を含めた。FOLR1を脱グリコシル化するために、組換えヒトFOLR1またはFOLR1陽性のKB細胞若しくはIgrov-1細胞の溶解物を製造元のプロトコールに従って脱グリコシル化酵素(Enzymatic DeGlycoMX Kit,QA-bio)の混合物で処理した。次いで処理した及び未処理のFOLR1をELISA及びウエスタンブロットの解析に用いた。ELISAについては、脱グリコシル化した及び未処理のFOLR1をELISAプレート(Immulon)に不動化し、抗FOLR1抗体FRIHC2-1(「2.1」)またはhuMov19を加えた。2時間のインキュベート後、HRP標識したヤギ抗ヒト(huMov19について)または抗マウス(2.1について)二次抗体によって抗体/抗原複合体を検出した(図7)。ウエスタンブロット解析については、脱グリコシル化した及び未処理の溶解物またはhuFOLR1組換えタンパク質の試料をSDSポリアクリルアミドゲル電気泳動によって分離し、常法によってニトロセルロース膜に移した。膜を抗FOLR1抗体2.1、huMov19またはBN3.2と共にインキュベートし、西洋ワサビペルオキシダーゼを結合させた適当な抗マウスまたは抗ヒト二次抗体によって抗原/抗体複合体を検出した(図8)。図7及び図8に示すように、抗体2.1のグリコシル化したFOLR1への結合対未処理のFOLR1への結合が有意に低下したということは、抗体がグリコ依存性のエピトープに結合することを示唆している。対照的に、他の2つの抗FOLR1抗体:huMov19及びBN3.2が脱グリコシル化した受容体及び未処理の受容体に同様に結合するということは、(i)FOLR1タンパク質は脱グリコシル化手順の間に損傷されなかった及び(ii)huMov19及びBN3.2はFOLR1のタンパク質エピトープを認識することを示している。
製造元のプロトコールに従ってRNeasyキット(QIAgen)を用いて実施例1で記載されたFOLR1ハイブリドーマの5×106個の細胞から細胞性の全RNAを調製した。その後、SuperScript II cDNA合成キット(Invitrogen)を用いて全RNAから8つのサブクローン(2.1、2.12、3.8、3.9、5.7、5.10、9.20、及び9.21)についてcDNAを合成した。

抗FOLR1抗体のそれぞれについて得られた可変領域cDNA配列を生殖細胞系列の定常領域配列と組み合わせて完全長の抗体cDNA配列を得た。次いで重鎖及び軽鎖の分子量をcDNA配列の翻訳から算出し、精製されたマウス抗FOLR1抗体のLC/MS解析によって得られた分子量と比較した。LC/MSは抗体を脱グリコシル化し、還元して完全な軽鎖及び重鎖ペプチドを単離することによって行った。重鎖のそれぞれについて観察された分子量は予想と一致したが、軽鎖のそれぞれはおよそ85Da違っていた。軽鎖断片のLC/MSによるその後のペプチド断片化の解析は軽鎖リーダーペプチドの最終的なセリンが成熟軽鎖に実際保持されており、予想MWに約87Daを加えたので、FOLR1抗体のそれぞれについてcDNA配列は確認された。
8つのサブクローンについての抗体配列の配列比較は、4つの元々のハイブリドーマのうち3つが密接に関連するが、独特の抗体を産生したことを示した。予想通り、4つの姉妹サブクローン対のそれぞれは同一だった。加えて、サブクローンの2セットも同一であり、3つの独特の抗体配列(配列番号27~32)(2.1、5.7及び9.20)を生じた。これら3つの独特の抗体の軽鎖及び重鎖の可変フレームワーク配列は密接に関連するが、各抗体は体細胞アミノ酸置換の結果と思われる独特のCDRを含有する(以下の表14を参照のこと)。マウスの抗FOLR1抗体のこれらCDR変異体は機能的に同一であることが見いだされたので、それらは本発明の抗FOLR1抗体のCDRの配列の自由度に何らかの構造的な洞察を提供する。軽鎖のCDR2及び3が抗体のそれぞれにおいて同一であったということは、これらのしっかりと保存されたCDRはFOLR1結合についての一貫した構造的な基礎を提供することができることを示唆している。その一方、残っているCDR、特に重鎖CDR2及び3におけるアミノ酸置換はこれらの位置がこれら抗体の親和性及び特異性の改善に決定的であることを示唆している。これらのCDRの位置における特定の残基の置換はまたこれら抗体の操作された型の中に組み込まれ得る残基の例も提供する。表14は本発明の抗FOLR1抗体から集められた複合CDR配列のリストを提供する。本明細書で特定される複合CDRは本発明の抗FOLR1抗体の機能的な特質を保存するように期待される組換え抗体の設計に使用することができる。
たとえば、参照によってその全体が本明細書に組み入れられるRoguskaら,Proc.Natl.Acad.Sci.,USA,91(3):969-973(1994)及びRoguskaら,Protein Eng.9(10):895-904(1996)にて以前記載された表面再構成法に従ってFRIHC2-1抗体をヒト化した。表面再構成には軽鎖及び重鎖の双方における可変領域フレームワークの表面残基の特定及びヒト同等物によるその置き換えが関与する。表面再構成された抗体ではマウスのCDRは保存される。FRIHC2-1抗体の例となるCDRは表14で示されたように定義される。CDRに入るリジンを結合する影響についての懸念をできるだけ抑えるために、マウスFRIHC2-1抗体の軽鎖CDR1におけるリジン24及びリジン27をヒト化バージョン1.0(イタリック体で示す)についてはアルギニンで置き換えるので、LCのCDR1の双方のバージョンが与えられる。表面再構成に採用されるAbM重鎖CDR2の定義に加えて、表はFRIHC2-1抗体のマウス型及びヒト型の双方についての例となるKabat定義の重鎖CDR2を提供する。下線の配列は表面再構成のためのCDRとは見なされなかったKabat重鎖CDR2の部分に印をつける。
353-2.1抗体を用いたIHCによってFOLR1の発現について、卵巣癌(n=63)、肺腺癌(n=104)及び子宮内膜腺癌(n=58)を代表とするヒト腫瘍試料を評価した。FOLR1染色の強度及びスコアの分布を以下の表16にて要約する。図15は353-2.1抗体による卵巣癌及び肺腺癌の組織の染色の例を示す。これらの結果は、FOLR1を標的とする作用物質(たとえば、IMGN853)による治療法の可能性がある候補としての患者を特定するIHCアッセイで使用するための特異的で且つ感度の高い抗体としての353-2.1の有用性を明らかにしている。
Claims (21)
- 患者から得た癌性試料における葉酸受容体1(FOLR1)発現の検出からFOLR1発現スコアを決定する方法であって、該検出が、FOLR1に特異的に結合する抗体またはその抗原結合断片を用いて行われ、該抗体またはその抗原結合断片が、以下:
(i)それぞれ、配列番号3~8;
(ii)それぞれ、配列番号9~14;および
(iii)それぞれ、配列番号15~20
からなる群より選択されるVH CDR1~3とVL CDR1~3のポリペプチド配列を含む、方法。 - 前記検出が免疫組織化学法(IHC)を用いて行われる、請求項1に記載の方法。
- 前記スコアは、前記患者が抗FOLR1抗体またはその抗原結合断片を含む活性作用物質の投与から利益を得るであろうことの指標である、請求項1または2に記載の方法。
- 前記細胞の25%以上における少なくとも2のFOLR1発現スコアは、前記癌を前記活性作用物質に応答する可能性があるものとして特定する、請求項3に記載の方法。
- 前記細胞の25%以上における少なくとも3のFOLR1発現スコアは、前記癌を前記活性作用物質に応答する可能性があるものとして特定する、請求項3に記載の方法。
- 前記細胞の75%超における少なくとも2のFOLR1発現スコアは、前記癌を前記活性作用物質に応答する可能性があるものとして特定する、請求項3に記載の方法。
- 前記細胞の75%超における少なくとも3のFOLR1発現スコアは、前記癌を前記活性作用物質に応答する可能性があるものとして特定する、請求項3に記載の方法。
- 前記癌が、卵巣癌、脳腫瘍、乳癌、子宮癌、子宮内膜癌、膵臓癌、腎臓癌、肺癌および腹膜の癌からなる群から選択される、請求項1~7のいずれか一項に記載の方法。
- 前記癌が、卵巣癌および腹膜の癌からなる群から選択される、請求項1~7のいずれか一項に記載の方法。
- 前記卵巣癌が上皮性卵巣癌である、請求項9に記載の方法。
- 前記癌が、白金耐性、再発性または難治性である、請求項10に記載の方法。
- 前記活性作用物質の抗FOLR1抗体またはその抗原結合断片が、配列番号45のアミノ酸配列を含む重鎖可変領域と、配列番号47のアミノ酸配列を含む軽鎖可変領域とを含む、請求項1~11のいずれか一項に記載の方法。
- 前記活性作用物質が、リンカーを介して前記活性作用物質の抗FOLR1抗体またはその抗原結合断片に連結した細胞毒性物質を含む、請求項1~12のいずれか一項に記載の方法。
- 前記リンカーが、N-スクシンイミジル 4-(2-ピリジルジチオ)-2-スルホブタノエート(スルホ-SPDB)である、請求項13に記載の方法。
- 前記細胞毒性物質が、メイタンシノイドである、請求項13または14に記載の方法。
- 前記メイタンシノイドが、N(2’)-デアセチル-N(2’)-(4-メルカプト-4-メチル-1-オキソペンチル)-メイタンシン(DM4)である、請求項15に記載の方法。
- 前記活性作用物質が、N-スクシンイミジル 4-(2-ピリジルジチオ)-2-スルホブタノエート(スルホ-SPDB)リンカーを介して前記活性作用物質の抗FOLR1抗体またはその抗原結合断片に連結したN(2’)-デアセチル-N2’-(4-メルカプト-4-メチル-1-オキソペンチル)-メイタンシン(DM4)を含む、抗体メイタンシノイド複合体であり、ここで、前記活性作用物質の抗FOLR1抗体またはその抗原結合断片が、配列番号45のアミノ酸配列を含む重鎖可変領域および配列番号47のアミノ酸配列を含む軽鎖可変領域を含む、請求項1~16のいずれか一項に記載の方法。
- 前記抗体またはその抗原結合断片が、以下:
(i)それぞれ、配列番号27~28;
(ii)それぞれ、配列番号29~30;および
(iii)それぞれ、配列番号31~32
からなる群より選択される配列を含む可変重鎖(VH)および可変軽鎖(VL)ポリペプチドを含む、請求項1~17のいずれか一項に記載の方法。 - 試料におけるFOLR1を検出する方法であって、前記方法は、前記試料を、以下:
(i)それぞれ、配列番号3~8;
(ii)それぞれ、配列番号9~14;および
(iii)それぞれ、配列番号15~20
からなる群より選択されるVH CDR1~3とVL CDR1~3のポリペプチド配列を含む抗体またはその抗原結合断片を接触させるステップを包含する、方法。 - 前記試料が癌性試料である、請求項19に記載の方法。
- 抗葉酸受容体1(FOLR1)活性作用物質を用いる治療に感受性である癌を特定する方法であって、前記方法は、以下:
(i)それぞれ、配列番号3~8;
(ii)それぞれ、配列番号9~14;および
(iii)それぞれ、配列番号15~20
からなる群より選択されるVH CDR1~3とVL CDR1~3のポリペプチド配列を含む抗体またはその抗原結合断片を用いる免疫組織化学法(IHC)を用いて、前記癌に由来する細胞を含む生物学的試料におけるFOLR1発現を検出するステップを包含し、ここで、前記試料における前記FOLR1の検出は、前記癌が前記活性作用物質に対して感受性であることの指標である、方法。
