本発明は、単球によって介在される病状を治療および診断するための方法に関する。より具体的には、単球に結合するリガンドと診断に使用される画像形成剤との複合体を形成させるか、あるいは、単球介在性疾患の治療のために使用される、単球機能を改変する免疫原、細胞毒素、または薬剤と前述リガンドとの複合体を形成させる。

哺乳類免疫系は、外来病原体の認識および排除のための手段を提供する。免疫系は、通常、外来病原体に対し防衛線を提供する一方、免疫反応それ自身が、病気の進行に関わる場合が数多くある。宿主自身の免疫反応が誘発したり、あるいは悪化させる病気の例としては、自己免疫疾患および、免疫反応が病気発生に寄与する、その他の疾患がある。例えば、マクロファージは、一般に、外来病原体に遭遇する最初の細胞であるため、この細胞は、免疫反応において重要な役割を果たすが、活性化されたマクロファージは、ある場合には病気の病理・生理機構にも貢献する。

葉酸受容体は、ビタミンの葉酸に高い親和度(<1 nM)をもって結合する、38KDのGPIアンカー型タンパク質である。受容体結合後、速やかなエンドサイトーシスによって該ビタミンは細胞内に輸送され、細胞内で、低pHのエンドソーム区画に落ち着く。重要なことは、葉酸と小型分子、タンパク質の共有結合、さらに、葉酸とリポソームの結合さえも、このビタミンの、葉酸受容体に対する結合能力を阻止しない。したがって、葉酸-薬剤結合体は、容易に細胞に輸送され、受容体介在エンドサイトーシスによって細胞内に進入することが可能である。

多くの細胞は、必要な葉酸を獲得するために、非特定的な還元型葉酸担体を用いるので、葉酸受容体の発現は数タイプの細胞に限られる。腎臓、脈絡叢、および胎盤を例外とすると、正常組織は、低レベルか、検出不能レベルの葉酸受容体しか発現しない。一方、多くの悪性組織、例えば、卵巣癌、乳癌、気管支癌、および脳の癌を含む悪性組織は、著しく高レベルの葉酸受容体を発現する。事実、全卵巣癌の95%は、葉酸受容体を過剰に発現すると推定されている。葉酸受容体の非上皮型異性形である葉酸受容体βは、活性化(ただし、休止期のものはそうではない)滑液マクロファージにおいて発現されることが報告されている。したがって、葉酸受容体は、ある一部のマクロファージ(すなわち、活性化マクロファージ)において発現される。

〈実施例14〉抹消血単球(pbmc)の単離
8-10 mlの全血サンプルを、EDTA抗凝固チューブに採取した。PBMCは、Ficoll-Paque Plus (Amersham Biosciences, Piscataway、ニュージャージー州)を用い、かつ、メーカーからのプロトコルに従って血液サンプルから単離した。簡単に言うと、血液サンプルは、平衡塩溶液(後述)で50:50に希釈した。8 mL のFicoll-Paque Plusを、50 mlの円錐形遠心チューブに加えた。この希釈血液サンプル(約16-20 ml)を、Ficoll勾配の上に重層した。このサンプルを、室温で30分400 x gで遠心した。遠心後、血漿層(最上部の透明層)をピペットを用いて取り出し、リンパ球/単球層をかく乱せずに残した。血漿層直下の濁った細胞層を取り除いた。その際、細胞層界面全体を取り除くが、Ficoll層の除去は最小となるように注意した。単離された細胞は、滅菌された50 mlの円錐形遠心チューブに移し、平衡塩溶液で3倍(容量/容量)に希釈した。得られた細胞溶液を穏やかに攪拌し、室温で10分100 x gで遠心して細胞をペレットとした。上清は除去し、細胞は、10%熱不活性化FBS、ペニシリン(100 IU/ml)、およびストレプトマイシン(100 μg/ml)を添加した、葉酸塩無添加RPMI 1640培養液に再縣濁した。細胞は、実験の必要性に応じて微小遠心チューブ、または顕微鏡チェンバーに撒かれた。

〈実施例32〉関節炎の動物モデル

X基はNSAIDを含み、前記組成物が、前記単球介在性病状を根絶し、
前記疾患は、変形性関節症、線維筋肉痛、シェーグレン症候群、糸球体腎炎からなる群から選択される、
ことを特徴とする使用方法。 - 前記NSAIDがインドメタシンおよびジクロフェナックから成る群から選択されることを特徴とする、請求項3に記載の使用方法。
- X基が、リポソームをさらに含むことを特徴とする、請求項1〜4のいずれかに記載の使用方法。
- 前記組成物が製薬学的に適用可能な担体をさらに含み、該担体が、液性担体であることを特徴とする、請求項1〜5のいずれかに記載の使用方法。
- 前記液性担体が、生理食塩水、グルコース、アルコール類、グリコール類、エステル類、アミド類、および、これらいずれかの組み合わせから成る群より選択されることを特徴とする、請求項6に記載の使用方法。
- 前記組成物が、非経口的投与のための形態をとっており、前記非経口的投与は、皮内投与、皮下投与、筋肉内投与、腹腔内投与、静脈内投与、および、髄膜投与から成る群から選択されることを特徴とする、請求項1〜7のいずれかに記載の使用方法。
- 前記組成物が、復元可能な凍結乾燥品であることを特徴とする、請求項1〜8のいずれかに記載の使用方法。
- 前記疾患が、変形性関節症であることを特徴とする、請求項1〜9のいずれかに記載の使用方法。
- 前記疾患が、線維筋肉痛であることを特徴とする、請求項1〜9のいずれかに記載の使用方法。
- 前記疾患は、シェーグレン症候群であることを特徴とする、請求項1〜9のいずれかに記載の使用方法。
- 前記疾患は、糸球体腎炎であることを特徴とする、請求項1〜9のいずれかに記載の使用方法。
- Ab基は、葉酸であることを特徴とする、請求項1〜13のいずれかに記載の使用方法。
