Claims (17)
- 標的血管の血管壁の内側または外側にアブレーション流体を周方向に送達する血管内神経アブレーションシステムであって、
3つまたは4つ以上の尖った注入ニードルと、
長手方向に延びる中心軸を有し流体注入ポートを有するカテーテル本体であって、前記流体注入ポートが、3つまたは4つ以上の尖った注入ニードル、3つまたは4つ以上のガイドチューブと流体連通し、各ガイドチューブが遠位部分および遠位チップを含み、前記遠位部分は、形状記憶金属を含み、かつ拘束が解除されると、外向きに付勢されて予め形作しておいたカーブを形成し、システムが第一閉ポジションを有し3つまたは4つ以上のガイドチューブおよび3つまたは4つ以上の尖った注入ニードルが半径方向に拘束された形態にあり、前記システムが第二開ポジションを有し、前記第二開ポジションは前記ガイドチューブが半径方向外向きに進めるようにして標的血管の血管壁内面と接触するようにし、前記システムが第三開ポジションを有し、前記第三開ポジションは3つまたは4つ以上の尖った注入ニードルが3つまたは4つ以上のガイドチューブの遠位チップを越えて外向きに進めるようにする、カテーテル本体と、
所定の注入深さで流体注入ルーメンからのアブレーション流体の周方向送達を行うように構成された3つまたは4つ以上の尖った注入ニードルのそれぞれの遠位端の近くに配置された注入出口と、
を備える血管内神経アブレーションシステム。 - 前記アブレーション流体の周方向への送達が、
前記標的血管の壁の中膜、
前記標的血管の壁の外膜、
前記標的血管の壁の外膜の外側のボリューム、
前記標的血管の壁の中膜および外膜、ならびに、
前記標的血管の外膜および前記標的血管の外膜の外側のボリュームからなる群から選択された組織の特定ボリュームへのものである請求項1に記載のシステム。 - 前記アブレーション流体が、前記標的血管内で下流に流れるのを阻止するための遠位バルーンをさらに備える請求項1に記載のシステム。
- 前記カテーテル本体が、その遠位端に取り付けられた固定ガイドワイヤを含む請求項1に記載のシステム。
- 別個のガイドワイヤの上から同軸的に進められるように構成された請求項1に記載のシステム。
- 遠位部分が半径方向外向きに進めるようにするため、前記第三開ポジションにリトラクトされるように構成された外側シースを備える請求項1に記載のシステム。
- 前記外側シースの遠位端に放射線不透過性マーカーをさらに含む請求項6に記載のシステム。
- 前記遠位部分が、ニチノール(登録商標)によって形成されている請求項1に記載のシステム。
- 前記アブレーション流体が、エタノール、フェノール、グリセロール、リドカイン、ブピバカイン、テトラカイン、ベンゾカイン、グアネチジン、およびボツリヌス毒素を含む群から選択されたアブレーション流体の少なくとも1つを含む請求項1に記載のシステム。
- 前記アブレーション流体が、加熱された流体組成物である請求項1に記載のシステム。
- 前記アブレーション流体が、冷却された流体組成物である請求項1に記載のシステム。
- 前記アブレーション流体が、前記カテーテル本体の注入ルーメンを通して注入される蒸気の形態である請求項1に記載のシステム。
- 3つまたは4つ以上の尖った注入ニードルの半径方向の進行を制限する貫通制限部材をさらに備える請求項1に記載のシステム。
- 3つまたは4つ以上の尖った注入ニードルのうちの第一の尖った注入ニードルが、3つまたは4つ以上のガイドチューブのうちの第一ガイドチューブ内に配置され、
3つまたは4つ以上の尖った注入ニードルのうちの第二の尖った注入ニードルが、3つまたは4つ以上のガイドチューブのうちの第二ガイドチューブ内に配置され、かつ
3つまたは4つ以上の尖った注入ニードルのうちの第三の尖った注入ニードルが、3つまたは4つ以上のガイドチューブのうちの第三ガイドチューブ内に配置されている請求項1に記載のシステム。 - 3つまたは4つ以上の尖った注入ニードルのうちの第一の尖った注入ニードルが、3つまたは4つ以上のガイドチューブのうちの第一ガイドチューブ内をスライドするように構成され、
3つまたは4つ以上の尖った注入ニードルのうちの第二の尖った注入ニードルが、3つまたは4つ以上のガイドチューブのうちの第二ガイドチューブ内をスライドするように構成され、かつ
3つまたは4つ以上の尖った注入ニードルのうちの第三の尖った注入ニードルが、3つまたは4つ以上のガイドチューブのうちの第三ガイドチューブ内をスライドするように構成されている請求項1に記載のシステム。 - 標的血管の血管壁に近接した組織のボリュームへアブレーション流体を周方向に送達する血管内神経アブレーションシステムであって、
長手方向に延びる中心軸を有するカテーテル本体と、
少なくとも3つのガイドチューブであって、各ガイドチューブが遠位端および中央ルーメンを有し、ガイドチューブが拘束を除去すると中心軸から半径方向外向きに進む遠位自己付勢部分を有し、前記遠位自己付勢部分が形状記憶金属および予め形作しておいたカーブを含む、少なくとも3つのガイドチューブと、
少なくとも3つの注入器チューブであって、各注入器チューブが注入ルーメンを有し、前記注入器チューブが前記ガイドチューブの内側に同軸に配置され、各注入器チューブがその遠位端に注入出口を有する尖った注入ニードルを有し、注入器チューブが前記ガイドチューブ内を長手方向にスライドするように構成された、少なくとも3つの注入器チューブと、
前記ガイドチューブの長手方向の動きをコントロールするように構成されたガイドチューブハンドルと、
アブレーション流体を注入するためのポートを含むカテーテル本体の近位端の近くに配置された注入チューブハンドルであって、前記ポートが前記注入器チューブと流体連結の状態にあり、注入ハンドルが注入器チューブの長手方向の動きをコントロールするようにさらに構成され、前記注入ハンドルおよび前記ガイドチューブハンドルが前記注入出口の標的血管の血管壁への貫通を制限するように構成された、注入チューブハンドルと、
を備えるシステム。 - 前記カテーテル本体の外側に同軸に配置されたシースをさらに含み、前記シースが閉ポジションおよび開ポジションを有し、前記開ポジションは、自己付勢ガイドチューブが、標的血管の壁の内面に対して外向きに進めるようにする請求項16に記載のシステム。
