Abstract
Description

このプロセスは、中心の縦軸からオフセットされている損傷の形成を可能にするニードルを使用するステップを含むことができる。手術は、図12のL5椎骨1101および図2Aのニードル103上で実施されるものとして説明される。本明細書で説明されているニードルの他の実施形態および/または他の腰椎は、説明されている手術またはそのバリエーションにおいて使用されうることは理解されるであろう。
このプロセスは、中心の縦軸からオフセットされている損傷の形成を可能にするニードルを使用するステップを含むことができる。手術は、図12のSIJの後枝1201上で実施され、図2Aのニードル103を使用するものとして説明される。本明細書で説明されているニードルの他の実施形態および/またはSIJの他の部分は、説明されている手術またはそのバリエーションにおいて使用されうることは理解されるであろう。
このプロセスは、ニードルの中心の縦軸からオフセットされている損傷の形成を可能にするニードルを使用するステップを含むことができる。関連する内側枝神経の高周波アブレーションを使用して胸郭z関節痛の治療を成功させるのは、横突間の空間内の内側枝の位置が食い違っているため困難な場合があるが、特にレベルT5〜T8においてそうである。フィラメントなしのニードルは、一般的に横突間の空間内の複数の場所に位置決めされ、内側枝神経切離を成功させられるように十分な組織アブレーションを行う。手術は、図14、および図2Aのニードル103を使用してT5からT8の胸椎の隣接する脊椎1301、1302間の横突間の空間上で実施されるものとして説明される。本明細書で説明されているニードルの他の実施形態および/または他の椎骨は、説明されている手術またはそのバリエーションにおいて使用されうることは理解されるであろう。
本明細書で説明されているニードルの実施形態(例えば、図2Aのニードル)は、C2/3頸部関節突起間関節(z関節)を含む、頸部関節突起間関節の完全な除神経に必要な組織アブレーションの体積を形成することができる。本明細書で説明されているニードルの実施形態を使用する頸部z関節の組織アブレーションは、単一の配置および単一の加熱サイクルを使用して達成することができる。そのような単一の配置および単一の加熱サイクルでは、フィラメントのないニードルの複数の配置からの不要な組織ダメージ、および過剰な損傷形成によって引き起こされる側副組織への意図しない傷害を回避することができる。アブレーションの領域は、手術が成功するのに十分な、かつ必要な組織凝固をもたらすように設計され、したがって、脊髄高周波神経切離を受けている患者の転帰を改善することが期待されうる。

Claims (15)
- 遠位端を有する細長部材と、
前記細長部材の前記遠位端に結合された先端部であって、前記細長部材の一方側の先端点を含むベベル部分を備える先端部と、
少なくとも部分的には前記細長部材内の第1の位置と、少なくとも部分的には前記細長部材の外側である第2の位置であって、前記細長部材の前記一方側の第2の位置との間で移動可能である複数のフィラメントであって、前記複数のフィラメントおよび前記先端部はプローブから高周波エネルギーを伝達し単極電極として動作するように構成される、複数のフィラメントと、
を備えることを特徴とする、ニードル。 - 前記ベベル部分は、約20°から約30°までの範囲内のベベル角度を有することを特徴とする、請求項1に記載のニードル。
- 前記複数のフィラメントのそれぞれは、前記先端部から見て外を向くベベルを備える遠位端を有することを特徴とする、請求項1または2に記載のニードル。
- 前記複数のフィラメントの前記ベベルは、約25°から約35°までの範囲内の角度であることを特徴とする、請求項3に記載のニードル。
- 単一のワイヤが、前記複数のフィラメントを含むことを特徴とする、請求項1〜4のいずれか一項に記載のニードル。
- 少なくとも部分的に前記先端点を覆う絶縁コーティングをさらに備えることを特徴とする、請求項1〜5のいずれか一項に記載のニードル。
- 前記先端部は、少なくとも部分的に前記細長部材内にステムを備え、前記ステムは第1のフィラメントルーメンと第2のフィラメントルーメンとを備えることを特徴とする、請求項1〜6のいずれか一項に記載のニードル。
- 前記ステムは、第3のルーメンを備え、前記ベベル部分は、前記第3のルーメンと連通する流体ポートを備えることを特徴とする、請求項7に記載のニードル。
- 前記細長部材は、近位端を有し、前記ニードルは、前記細長部材の前記近位端に結合された回転展開機構をさらに備え、前記回転展開機構は前記先端部に関して前記複数のフィラメントの部分的展開を示す印を備えることを特徴とする、請求項1〜8のいずれか一項に記載のニードル。
- 前記印は、聴覚的および触覚的戻り止めのうちの少なくとも1つを備えることを特徴とする、請求項9に記載のニードル。
- 前記細長部材は、近位端を有し、前記ニードルは、前記細長部材の前記近位端に結合されたフィラメント展開機構をさらに備え、前記フィラメント展開機構は、
前記複数のフィラメントに結合されたステムを備える前進ハブと、
螺旋状のトラックを備えるスピンカラーであって、前記前進ハブの前記ステムが少なくとも部分的に前記スピンカラーの内側にある、スピンカラーと、
前記螺旋状のトラックと連携するように構成された螺旋状のねじ山を備えるステムを具備するメインハブであって、前記メインハブの前記ステムが少なくとも部分的に前記スピンカラーの内側にあり、前記前進ハブの前記ステムが少なくとも部分的に前記メインハブの内側にある、メインハブとを備え、
前記フィラメントは、前記スピンカラーの回転後に、前記第1の位置と前記第2の位置との間で移動するように構成されることを特徴とする、請求項1〜8のいずれか一項に記載のニードル。 - 単一のワイヤが、前記複数のフィラメントを含み、前記ワイヤの近位端は、前記前進ハブの前記ステムに結合されることを特徴とする、請求項11に記載のニードル。
- 前記螺旋状のトラックおよび前記螺旋状のねじ山のうちの少なくとも一方は、前記複数のフィラメントの部分的展開または引き込みを示すように構成された複数の戻り止めを備えることを特徴とする、請求項11または12に記載のニードル。
- 前記第1の位置は、完全に引っ込められた位置であり、前記第2の位置は、完全に展開された位置であり、前記戻り止めは、前記フィラメントが前記第1の位置と前記第2の位置との間の第3の位置、前記第3の位置と前記第2の位置との間の第4の位置、および前記第4の位置と前記第2の位置との間の第5の位置にあるときに、使用者に聴覚的または触覚的フィードバックを送るように構成されていることを特徴とする、請求項13に記載のニードル。
- 前記複数のフィラメントは、前記第3の位置の前記先端点の近位にあり、前記複数のフィラメントは、前記第4の位置の前記先端点に実質的に縦方向に揃えられ、前記複数のフィラメントは、前記第5の位置の前記先端点の遠位にあることを特徴とする、請求項14に記載のニードル。
