Abstract
Description
本発明は全体として癌ワクチンの分野に関する。
癌は毎年、全世界の全死亡人口の約13%を占める。既存の樹状細胞をベースとする治療戦略は、主として、患者の腫瘍の生検材料から単離される癌抗原を用いた活性化のための多くの細胞を生成するための、樹状細胞のエクスビボでの操作に基づく。これらのエクスビボの技術は侵襲的でありかつ費用がかかるので、外科的な生検およびエクスビボでの細胞操作に依らない樹状細胞をベースとする癌ワクチン戦略を開発することが緊急に必要である。
三次元(3D)足場は、樹状細胞(DC)に一時的な滞留を与えかつインサイチューでの宿主DCの輸送および活性化を効果的に調節すると共に、免疫系の寛容化している部分の上方制御を防ぎ、かつ、癌に対する治療的保護作用を提供する。例えば、本明細書に記載される癌ワクチンのシステムにおいて、DCの化学誘因物質(GM-CSF)、癌抗原(腫瘍溶解物)、および危険信号(CpGオリゴヌクレオチド)を負荷したマクロ孔質ポリ(ラクチドおよびグリコリド)(PLG)足場の移植が、足場への末梢DCの補充、補充されたDCへの癌抗原の負荷、および危険信号によるそれらの成熟を招いた(図1)。抗原提示成熟DCは、リンパ節に移動して、強力な細胞毒性Tリンパ球(CTL)反応を生じた。この方法では、本ワクチンシステムは、強い抗癌免疫応答のトリガーであり、癌の根絶を可能にした。このような一つのシステムは、抗原を生成するために治療される患者由来の腫瘍生検材料を用い、腫瘍組織のエクスビボでの操作およびプロセシングを必要とする。さらに、本システムは、各ワクチンが(その同一の患者からの腫瘍溶解物を用いて)治療される特定の個体のために製造されることを必要とする。本発明の方法はこれまでに記載されたシステムの改善を表す。
この方法で治療されるのに適した癌は、血流中に循環している原発性または転移性癌細胞が存在している、癌である。これらの細胞は、特定のケモカインの濃度勾配に反応しての遊走特性を特徴とする。従って、循環性癌細胞は、特定のケモカインが組み込まれている移植された足場に補充される。
●循環性癌細胞:様々な癌における転移性癌細胞、白血病細胞。
●癌細胞の化学誘因物質:癌のタイプに応じた様々なケモカイン(例えば、CCL-21、CCL-19、SDF-1、VEGF、IL-4など)。
●3D足場:生分解性多孔性ポリマー、多孔性無機材料、会合したナノ粒子、ナノチューブ、ナノロッドなどを含む、孔を持ってケモカインを負荷するように設計された様々なタイプの3D足場。
以下に記載する通り、足場への癌細胞の補充の他に、癌抗原を含有する溶解物をインサイチューにおいて生成するように癌細胞を補充した後に癌細胞を死滅させるために様々な外部刺激が用いられる。補充された癌細胞を死滅させるために、外部刺激は移植された足場に適用可能である。

○磁性ナノ粒子を負荷した足場のための交番磁界(AMF)。
○近赤外線(NIR)吸収ナノ粒子(例えば、金ナノロッド、金ナノシェル、金ナノケージ、その他の貴金属ナノ粒子、炭素ナノチューブ、炭素ナノ粒子、グラファイトなど)を負荷した足場のためのNIR照射。
癌細胞を死滅させるために用いられる信号はDC機能に対して負の影響を及ぼさないので、場合によっては、補充されたDCおよび癌細胞を分離することが望ましい。以下に記載する通り、この隔離は、各細胞タイプの補充の時間的な順序を通じての制御により、または足場の特定領域への外部刺激の適用を可能にする細胞の空間的な隔離により達成される。
すべての細胞を同時に補充するのではなく、先ず癌細胞が、補充され、かつ、溶解物を生成するために破壊される。その後、癌細胞は、外部刺激によってDCを損傷することなく、癌抗原の部位にDCを補充する。この目的のために、足場からDCおよび癌細胞への異なる化学誘因物質の放出特性を制御することが可能である。例えば、放出特性を制御するためにポリマーワクチン足場の調製において、異なる分解特性および/または各ケモカインに対する異なる分子親和性を持つポリマーの異なる組成物が用いられる。
足場はまた、一部の特定のコンパートメントに存在する癌細胞を特異的に死滅させるためにそのコンパートメントのみが外部刺激の影響を受けて、それによってその他のコンパートメント内の無傷および機能性のDCの維持が可能であるように、コンパートメント化される。この目的のため、足場の様々な構造上の変更が用いられる。癌細胞を死滅させるためのナノ粒子(NP)媒介性高体温の場合、NPは、足場の特定の部分に組み込まれ、NP部分における特異的な高体温を可能にし、DCが補充されるその他の部分に対しては高体温の影響は微々たるものである。
タンパク質薬物(例えば、サイトカインおよびモノクローナル抗体)を用いる免疫療法は癌管理のための一つのアプローチである。免疫療法の別の型である治療的な癌ワクチンは癌を治療するための別のアプローチである。癌ワクチンは強い抗腫瘍免疫活性を引き起こすように設計され、かつ、抗原特異的細胞毒性(CD8+)Tリンパ球(CTL)の誘導はそれらの機能の決定的な局面である。活性化されたCD8+ T細胞は、腫瘍細胞上に存在する特異的標識(抗原)を認識すると腫瘍細胞を死滅させ、かつ、この認識は、その抗原に特異的なT細胞受容体(TCR)への標識の結合に依存する。樹状細胞(DC)は最も重要な抗原提示細胞(APC)であり、CTL反応の開始において中心的な役割を果たす。
白血病細胞を補充するための金ナノロッド負荷癌ワクチン足場と、熱ショックタンパク質に結合した癌抗原を生成するためのNIR照射媒介性高体温によるそれらのその後の破壊との例を以下に記載する。足場が癌細胞を補充できることを実証するために、マウス白血病細胞(C1498)を、化学誘因物質としてCCL-21を用いたトランスウェルアッセイ法で調べた。C1498はCCL-21の濃度勾配に対して強い遊走を示した(図3)。
白血病細胞のインビボ補充は、GFP発現白血病細胞を用いて特徴決定された。いかなるケモカインも伴わないPLG足場(ブランク)、GM-CSFを負荷した足場(G)、GM-CSFおよびCCL-21を負荷した足場(G+C)をC57BL/6Jマウスの皮下に移植して、GFP-白血病細胞を4日目に尾静脈注入により血液中に注入した。足場を6日目に回収して、足場内の細胞を単離してFACSで分析した(図4)。さらに、蛍光NP標識した白血病細胞を注入したマウスから回収した足場を蛍光画像化装置(Xenogel)で画像化した(図5)。双方の結果により、足場から放出されたCCL-21が動物において白血病細胞の補充を増大させることを示された。
補充された癌細胞からの高体温媒介性抗原生成を達成するために、製造工程においてPLGマクロ多孔性足場に金ナノロッド(GNR)を組み込まれた(GNR-PLG足場)(図6)。GNR-PLG足場は250〜440umの孔を持ち、GNRは足場全体に組み込まれて、その結果、得られるPLG足場は暗色となった。細胞に対する毒性物質として公知であるGNR製造工程で用いられた元々の界面活性物質(臭化セチルトリメチルアンモニウム)から毒性を除去するために、GNRの表面をポリ(エチレングリコール)(PEG)で修飾した。
NIR光がPLG足場の特定の部分においてGNRによって吸収されるために、PLG足場のGNR組み込み領域における局所的高体温は可能である。ハーフGNR-PLG足場およびハーフブランクPLG足場を物理的に接続し、かつ、足場全体をカバーするために大きなビームサイズを用いて808nmのレーザーを照射した結果、GNR-PLG足場側面にのみ特異的な加熱が生じた(図9)。この構成は異なるコンパートメント、即ち、癌細胞のためのコンパートメントおよびDCのためのコンパートメントを持つ、移植可能な足場を示す。癌細胞のためのコンパートメントのみが加熱されるが、DCのためのコンパートメントは正常なDC機能を可能にするように元のまま(加熱されていない)である。
GNRを組み込まれたコア部分および正常なシェル部分からなる単一のPLG足場(図10A)を癌特異的加熱のために作製した(図10B)。異なるコンパートメントおよび十分に相互接続した孔構造を持つこの単一のPLG足場(図10C)は高体温後のDCによる効率的な癌抗原の取り込みを可能にする(図10C)。
熱ショックが熱ショック癌細胞から産生されるHSPのアジュバント効果のためにより多くの免疫原性抗原を誘導することができるかどうかを調べるために、43℃の恒温水槽での熱ショック後に癌細胞培養物のHSPレベルを分析した(図11A)。一般的な溶解物生成方法である凍結融解法に比べて、熱ショック条件では細胞溶解物および細胞培養培地の双方において典型的なHPSであるHSP70がより多く得られた。熱ショック溶解物および凍結融解溶解物によるインビトロDC活性化は、熱ショック溶解物が高いHSPのために高いDC活性化特性を持つことを示した(図11B)。
GNR-PLG足場へのNIR照射が、足場に存在する癌細胞からHSPを誘導するかどうかを調べるために、GNR-PLG足場に白血病細胞を播種した後、808nmレーザーを照射した(図12A)。HSP70レベルを検討するために照射による様々な温度について調べ、45℃までの照射が最大のHSP70レベルを惹起することが示された(図12B)。さらに、ウェスタンブロットのデータは、45℃での照射後24時間のインキュベートが最も高いHSPを生成することを示した。
インビトロDC、例えば骨髄由来樹状細胞(BMDC)の、NIR照射後の45℃でのGNR-PLG足場からの細胞溶解物を用いた活性化は、活性化されたDCの代表的な細胞表面マーカーであるCCR7およびCD86に関して未照射細胞溶解物に比べて高いDCの活性化を示し(図13)、NIR照射が、GNR-PLG足場に存在する癌細胞からの高い免疫原性の癌抗原のインサイチュー生成を引き起こすことを示す。
NIR照射後のGNR-PLG足場におけるインビトロ癌細胞生存率について評価した。GNR-PLG足場に白血病細胞を播種して、温度を40、45、および50℃まで上昇させるためにNIR照射を行い、アラマーブルーアッセイ法により細胞の生存率を確認した(図14)。温度が高ければ高いほど、細胞の生存率は低かった。さらに、二回目の照射ではすべての条件においてさらに低い生存率となった。これらのデータは、GNR-PLG足場においてNIR照射によって惹起される高体温のために癌細胞が死滅したことを示す。
インビボ補充を模倣するために、白血病細胞を播種したGNR-PLG足場をC57BL/6Jマウスの皮下の組織内に移植し、足場を45および50℃までNIRレーザーで照射した。インビトロの実験と同様に、より高い温度の条件では細胞の生存率が低下し(図15)、NIR照射が、GNR-PLG足場において補充された癌細胞に対する熱ショックを誘発したことを示す。移植した足場装置への循環性癌細胞の補充、外力、例えば照射の適用による癌細胞の変質または破壊は、装置内での腫瘍抗原の有効性の向上をもたらす。癌における装置内での腫瘍抗原の有効性の向上はDC活性化の増大およびより有効な癌ワクチンをもたらす。
インビボ樹状細胞補充およびレーザー照射を用いた活性化について評価した。106個のEG7.Ovaリンパ腫細胞をGM-CSFと共にGNR-PLG足場に負荷した。足場をC57BL/6Jマウスの皮下に移植した。移植後3日に足場の位置に45℃まで5分間、808nmのNIRレーザーを照射した。移植後7日に足場を回収して消化し、樹状マーカー(CD11c)および活性化マーカー(CD86)について細胞を分析した。図16A〜Bは、レーザー照射が、補充された樹状細胞のパーセントを有意(n=3、p<0.05)に増大させ(A)、足場内においてそれらを活性化する(B)ことを示す。
レーザー照射を用いた流入領域リンパ節におけるインビボ樹状細胞活性化について検討した。106個のEG7.Ovaリンパ腫細胞をGM-CSFと共にGNR-PLG足場に負荷した。足場をC57BL/6Jマウスの背部の皮下に移植した。移植後3日に足場の位置に45℃まで5分間、808nmのNIRレーザーを照射した。移植後7日に流入領域リンパ節(鼠径リンパ節を含む)を回収して消化し、樹状マーカー(CD11c)および活性化マーカー(CD86)について細胞を分析した。図17A〜Bは、レーザー照射が、流入領域リンパ節を有意(n=3、p<0.05)に肥大させ(A)、これが炎症後の反応であり、かつ、リンパ節内の活性化樹状細胞の数を増加させたこと(B)を示す。これらのデータは、補充されたDCが足場装置を離れて流入領域リンパ節(即ち、移植された装置の場所とは異なる解剖学的部位)に遊走する/移動したことを示す。
インビボ樹状細胞補充およびレーザー照射を用いた全PLGワクチンにおける活性化について評価した。106個のEG7.Ovaリンパ腫細胞を、GM-CSFと、DCを活性化するための危険信号として機能する濃縮CpG-ODNと共にGNR-PLG足場に負荷した。足場をC57BL/6Jマウスの皮下に移植した。移植後3日に足場の位置に45℃まで5分間、808nmのNIRレーザーを照射した。移植後7日に足場を回収して消化し、樹状マーカー(CD11c)および活性化マーカー(CD86)について細胞を分析した。図18A〜Dは、レーザー照射が補充されたDCのパーセント(A)および総数(B)をさらに増加させ(n=3、p<0.05)、足場の位置においてそれらを活性化すること(C〜D)を示す。このデータは、レーザー照射が免疫細胞活性化のためのさらなる危険信号として機能することを示す。
熱ショックB16細胞溶解物を用いたインビトロBMDC活性化について評価した。熱ショック細胞溶解物を調製するために、予め加温した恒温水槽において10x106個のB16黒色腫細胞に45℃で5分間、熱ショックを与えた。従来法の細胞溶解物は3サイクルの凍結融解手順を用いて作製した。調製した溶解物を106個のBMDCと共に18時間インキュベートした。BMDC活性化マーカーであるMHCIIおよびCD86を、フローサイトメトリーを用いて分析した。細胞にCD11c+DCに関してゲートを掛けた。図19は、熱ショック細胞から生成された溶解物が従来法の細胞溶解物よりもより多く(p<0.05)の活性化されたDCを生成できることから、熱ショック細胞から生成された溶解物がDCを活性化するための危険信号として機能することを示している。
高温はインビトロにおいてBMDCのLPSに対する反応性の低下を誘発する。106個/ウェルのBMDCに50℃で5分間、熱ショックを与えた。続いて、それらを、免疫細胞活性化を上方制御できる免疫アジュバントであるLPSを加えてまたは加えないで、インキュベートした。活性化マーカーであるCD86およびMHC-IIをフローサイトメトリーで分析した。図20A〜Dは、熱ショックがBMDCのLPS刺激に対する反応能力を抑制できることを示す。従って、補充されたDCを照射から保護するために代替となる足場構造が開発された。
補充されたBMDCの直接の死滅は避けるが、補充された癌細胞の熱ショックは可能にするように、GNR足場の代替構造であるコアシェル型の足場が工学的に操作された。内部コアの足場は癌を補充するケモカインおよびGNRを負荷するように設計され(色は負荷されたGNRのために暗色である);外側シェルの足場はDCを補充するためにGM-CSFのみを負荷する。圧縮試験は、この足場スキームが従来法の足場スキームよりも低いヤング率を持つことを示す(図21A〜C)。この設計では、内部コアの足場に補充される癌細胞のみがレーザー照射の熱ショックに供されて、外側シェルの足場の補充されたDCは熱ショックを回避する。

Claims (18)
- プロセシングされた腫瘍抗原をインサイチューにおいて産生するための生検不要の方法であって、癌と診断された対象に、癌細胞の化学誘因物質を含む多孔性三次元足場を投与する工程、該足場を、循環性癌細胞を蓄積するのに十分な期間、インサイチューにおいて維持して、癌細胞含有足場を得る工程、および、該細胞含有足場を細胞毒性因子または細胞溶解性因子に接触させて、プロセシングされた腫瘍抗原を産生する工程を含む、方法。
- 前記細胞毒性因子が、前記細胞含有足場への外部からの熱、超音波、レーザー照射、またはγ線照射の適用を含む、請求項1記載の方法。
- 前記レーザー照射が紫外線または近赤外線のレーザー照射を含む、請求項2記載の方法。
- 前記足場が高体温誘発組成物をさらに含む、請求項1記載の方法。
- 前記高体温誘発組成物が磁性ナノ粒子または近赤外線(NIR)吸収ナノ粒子を含む、請求項4記載の方法。
- 前記ナノ粒子が磁性であり、かつ、前記方法が、インサイチューにおいて局所的高体温を誘発するために磁性ナノ粒子を交番磁界に接触させる工程、それによって前記癌細胞を崩壊させる工程、および、プロセシングされた腫瘍抗原を産生する工程をさらに含む、請求項5記載の方法。
- 前記NIRナノ粒子が、金ナノロッド、金ナノシェル、金ナノケージ、貴金属ナノ粒子、炭素ナノチューブ、炭素ナノ粒子、およびグラファイトナノ粒子からなる群より選択され、かつ、前記方法が、インサイチューにおいて局所的高体温を誘発するために該NIRナノ粒子をNIR照射に接触させる工程、それによって前記癌細胞を崩壊させる工程、および、プロセシングされた腫瘍抗原を産生する工程をさらに含む、請求項5記載の方法。
- 前記癌細胞の化学誘因物質が、CCL-21、CCL-19、SDF-1、VEGF、およびIL-4からなる群より選択されるケモカインを含む、請求項1記載の方法。
- 前記癌が循環性腫瘍細胞を特徴とする、請求項1記載の方法。
- 前記対象が転移性癌状態または白血病と診断されている、請求項1記載の方法。
- 多孔性ポリマー、癌細胞のための化学誘因物質、および細胞毒性誘発組成物を備える、腫瘍抗原プロセシング装置。
- 前記細胞毒性誘発組成物が高体温誘発粒子を含む、請求項11記載の装置。
- 前記細胞毒性誘発組成物が金ナノ粒子または金ナノロッドを含む、請求項11記載の装置。
- 免疫細胞補充組成物をさらに備える、請求項11記載の装置。
- 前記免疫細胞補充組成物が顆粒球マクロファージコロニー刺激因子を含む、請求項14記載の装置。
- 前記化学誘因物質、前記細胞毒性誘発組成物、および前記免疫細胞補充組成物が前記多孔性ポリマー全体に散在する、請求項14記載の装置。
- 前記多孔性ポリマーが、前記化学誘因物質および前記細胞毒性誘発組成物を含む第一の領域、ならびに、前記免疫細胞補充組成物を含む第二の領域を含む、請求項14記載の装置。
- 前記第一の領域がコアとして構成され、かつ、前記第二の領域がシェルとして構成される、請求項15記載の装置。
