Abstract
Description
Claims (19)
- 近位端と、遠位端と、経皮アクセス位置に対して患者の腎動脈にアクセスするのに十分な長さと、を有する可撓シャフトを備えるカテーテルと、
前記シャフトの前記遠位端に設けられるとともに腎動脈内で展開するよう寸法形成された電極アレイであって、前記腎動脈の壁またはその近傍にある複数の腎動脈部位に配置可能であるとともに間隔をおいて配置された複数の電極を備える電極アレイと、
前記カテーテルに連結され、かつ患者から生理学的反応を引き出すが腎神経を切除するには不十分な刺激エネルギーを前記複数の電極に選択的に送達するように構成される外部システムであって、標的腎動脈部位に隣接する腎神経を切除するために、生理学的反応を引き出す前記標的腎動脈部位で高周波電気エネルギーを電極に送達するようにさらに構成される、外部システムと、
を備える装置。 - 前記電極アレイの電極が、個々にまたは選択された組み合わせで、作動可能または動作停止可能である請求項1に記載の装置。
- 前記電極アレイが前記シャフトの遠位端の周囲に配置される複数の長尺弾性部材を備え、各弾性部材が少なくとも1つの電極を支持する請求項1に記載の装置。
- 前記電極アレイが、前記複数の電極を支持する自己拡張型バスケット構造を備える請求項1に記載の装置。
- 前記電極アレイが、前記複数の電極を支持する自己拡張型バスケット構造を備え、
前記バスケット構造の遠位端および近位端のうち一方が前記シャフトに固定して搭載され、
前記バスケット構造の遠位端および近位端のうち他方が、前記バスケット構造の放射方向の拡張および収縮を促進するように前記シャフトに沿って長手方向に平行移動するように構成される摺動装着部材を備え、
制御素子が前記摺動装着部材に連結される遠位端と作動可能な近位端とを有する、請求項1に記載の装置。 - 前記電極アレイが前記複数の電極を支持する複数の自己拡張型バスケット構造を備え、
前記バスケット構造の遠位端および近位端のうち一方が前記シャフトに固定して搭載され、
摺動装着部材が、前記バスケット構造間に、かつ前記バスケット構造の遠位端および近位端のうち他方に位置し、
前記摺動装着部材が、前記バスケット構造の放射方向の拡張および収縮を促進するように前記シャフトに沿って長手方向に平行移動するように構成され、
制御素子が、作動可能な近位端と前記摺動装着部材のうち少なくとも1つに連結される遠位端とを有する、請求項1に記載の装置。 - 前記電極アレイを収容するよう寸法形成された内腔を有する外側シースを備える請求項1に記載の装置。
- 前記刺激エネルギーが、前記腎動脈部位に連続的に、あるいは前記腎動脈部位の組み合わせに同時に送達される請求項1に記載の装置。
- 前記刺激エネルギーが、生理学的反応の引き出しを強化する周波数、振幅、およびパターンのうち1つまたは複数を有する請求項1に記載の装置。
- 前記エネルギー送達システムが、腎神経切除の後に腎動脈に刺激エネルギーを選択的に送達して切除の有効性を判定するように構成される請求項1に記載の装置。
- 生理学的反応の引き出しを感知するように構成される1つまたは複数の植込み型センサまたは外部センサを備える請求項1に記載の装置。
- 生理学的反応が痛み、刺痛、熱、および圧力のうち1つまたは複数からなる、請求項1に記載の装置。
- 生理学的反応が、交感神経信号または結果として生じる化学的または電気的変化を含む一連の反応の変化、皮膚伝導性または発汗を含む生体指標、血圧、脈または呼吸の変化、血管緊張または筋緊張の変化、自発的胃腸活動、瞳孔反応、および心臓電気活動を備える請求項1に記載の装置。
- 前記標的部位での腎神経組織の切除が、前記電極アレイを再配置することなく標的部位の特定の完了後に実行される請求項1に記載の装置。
- 前記電極アレイが低プロファイル導入形態と展開形態との間で変形可能であり、
前記電極アレイが展開形態にあるときに、前記間隔をおいて配置された電極が前記複数の腎動脈部位に配置可能である、
請求項1に記載の装置。 - 刺激カテーテルであって、近位端と、遠位端と、経皮アクセス位置に対して患者の腎動脈にアクセスするのに十分な長さと、を有する可撓シャフトと、前記シャフトの前記遠位端に設けられるとともに複数の腎動脈部位に刺激手段を送達するように構成される刺激構造と、を備える刺激カテーテルと、
切除カテーテルであって、近位端と、遠位端と、経皮アクセス位置に対して患者の腎動脈にアクセスするのに十分な長さと、を有する可撓シャフトと、前記切除カテーテルのシャフトの前記遠位端に設けられる切除構造と、を備える切除カテーテルと、
前記刺激カテーテルに連結され、患者から生理学的反応を引き出すために複数の腎動脈部位に前記刺激手段を選択的に送達することを促進するように構成される第1の外部システムと、
前記切除カテーテルに連結され、標的腎動脈部位に隣接する腎神経を切除するために、生理学的反応を引き出す前記標的腎動脈部位に切除手段を送達することを促進するように構成される第2の外部システムと、
を備える装置。 - 前記刺激構造は、電気エネルギー、光エネルギー、音響エネルギー、機械力、振動、熱エネルギー、神経伝達物質、化学薬品または薬物、圧力変化、浸透圧変化、およびpH変化からなる1つまたは複数の刺激手段を送達するように構成され、
前記切除構造は、電気エネルギー、前記標的部位に隣接する腎神経を熱的に切除する光エネルギー、前記標的部位に隣接する腎神経組織内に微小気泡を形成して爆縮または破裂時に腎神経組織に含まれる神経線維と神経節を機械的に破壊する光エネルギー、前記標的部位に隣接する腎神経を熱的に切除する音響エネルギー、前記標的部位に隣接する腎神経組織内に微小気泡を形成して爆縮または破裂時に腎神経組織に含まれる神経線維と神経節を機械的に破壊する音響エネルギー、機械的荷重または圧縮力、寒冷エネルギー、凝固、変性または壊死を生じさせるのに十分な熱エネルギー、神経毒または毒液、および壊死を生じさせるのに十分な誘発pH変化、からなる1つまたは複数の切除手段を送達することを促進するように構成される、請求項16に記載の装置。 - 前記刺激構造が、痛みを引き起こす第1の刺激手段と痛みを遮断する第2の刺激手段とを選択的に送達するように構成され、
前記第1の外部システムが、生理学的反応を引き出す腎動脈部位を特定するために、前記第1の刺激手段と前記第2の刺激手段とを選択的に送達し、選択的に痛みを引き起こすおよび痛みを遮断するように構成される、請求項16に記載の装置。 - 前記刺激構造が、
動脈平滑筋細胞の神経活性化により一時的血管収縮を引き起こす近位腎動脈位置に第1の刺激手段を送達して、有意な血管周囲腎神経束の位置特定を促進し、
複数の遠位腎動脈に遮断薬を送達して遠心性神経位置の検出を促進する
ように構成される、請求項16に記載の装置。
