Description
本発明のデバイス、システム、および方法は、腎臓の交感神経活動を変調(モデュレーション)することを対象としている。本発明の実施形態は、患者の腎動脈へ除神経療法を血管内送達するための装置に関する。様々な実施形態によれば、腎臓除神経療法の装置は、基端部と、先端部と、患者の体外の位置から少なくとも腎動脈に到達するのに十分な長さとを含んでなる、長尺状のガイドレールを備えている。螺旋状区域がガイドレールの先端部に提供される。螺旋状区域は腎動脈の直径にほぼ等しい直径を有する。治療装置は、患者の体外の位置から少なくとも腎動脈に到達するのに十分な長さと、長尺状のガイドレールを受承するように構成された長手方向に伸びるチャネルとを有する。該治療装置は、腎動脈に除神経療法を送達するように構成された治療要素を含んでなる。

高周波エネルギーは治療区域212/213の導体要素を通って流れ、イオンの撹拌を、したがって腎動脈の隣接組織内における摩擦を、引き起こす。この摩擦は、腎臓の神経を含む腎動脈の標的組織における温度上昇をもたらす。十分な温度に達した後、熱は数分以内に標的組織を死滅させる。
Claims (21)
- 基端部と、先端部と、患者の経皮的侵入位置に関して少なくとも腎動脈に到達するのに十分な長さと、を含んでなるカテーテル、
カテーテルの先端部に提供され、腎動脈内に展開配置するための寸法に形成された編組物であって、
腹大動脈から腎動脈内への編組物の展開配置を容易にする弾力性を有する材料と、
基端部と、先端部と、長さと、直径と、
編組物を少なくとも完全に1回転する、ほぼ螺旋形状を有する電導体パターンであって、高周波発電機と電気的に結合するように構成された電導体パターンと、
電導体パターンを欠いている編組物の領域を画成する絶縁部分と、を含んでなり、
軸方向の圧縮に応じて長さが減少しかつ直径が増大するように、また軸方向の伸長または弛緩に応じて長さが増大しかつ直径が減少するように構成された編組物、
編組物に連結され、カテーテルの基端部において駆動をなすことができる駆動部と、該駆動部は、編組物の基端部および先端部のうち少なくとも一方に連結され、かつ、編組物を長手方向に選択的に拡張および圧縮するように構成されることと、編組物の電導体パターンは、編組物の圧縮および弛緩に応じて、それぞれ腎動脈の内壁に向かうように、また腎動脈の内壁から離れるように促されることと、ならびに、
編組物の複数の箇所に提供または連結され、かつ、複数の編組物の箇所それぞれにおいて温度を感知するように構成された、センサ構成物であって、電導体パターンの長手方向に間隔を置いた箇所に提供された複数の温度センサを含んでなり、該複数の温度センサそれぞれが長手方向に間隔を置いた箇所のうち1箇所において温度を感知するように構成されることによって、編組物の電導体パターンの温度プロファイルを提供する、センサ構成物、
を含んでなる装置であって、
圧縮状態の編組物を用いて、かつ、発電機により電導体パターンに通電することによって、腎動脈に除神経療法が送達される、装置。 - 編組物材料は、腎動脈の内壁を冷却するために動脈血が編組物を通り抜けて流れるのを容易にする潅流構成物を画成する複数の空隙を含んでなる、請求項1に記載の装置。
- 電導体パターンの異なる領域は駆動部により逐次圧縮可能であり、集団として渦巻状の焼灼巣を形成する一連の熱傷スポットを形成するために電気的に活性化可能である、請求項1に記載の装置。
- センサ構成物は、複数の編組物の箇所それぞれにおいてインピーダンスを感知するように構成される、請求項1に記載の装置。
- 編組物に提供または連結された少なくとも1つのセンサを含んでなり、発電機は、除神経療法の送達の際に該少なくとも1つのセンサによって生じた信号に応じて編組物への電力送達を自動的に制御するように構成される、請求項1に記載の装置。
- 編組物は、ほぼ螺旋形状をなした電導体パターンの一部をそれぞれ含んでなる複数の編組物区域を含んでなる、請求項1に記載の装置。
- 複数の編組物区域は、該区域の独立した駆動を提供するためにそれぞれ複数の駆動部材のうち1つと連結される、請求項6に記載の装置。
- 複数の編組物区域は、該区域の独立した電気的活性化および不活性化を提供するためにそれぞれカテーテルの複数の電導体構成物のうち1つと連結される、請求項6に記載の装

- 基端部と、先端部と、患者の経皮的侵入位置に関して少なくとも腎動脈に到達するのに十分な長さと、を含んでなるカテーテル、
カテーテルの先端部に配置され、カテーテルのルーメンに流体が流れるように連結されたバルーンであって、腎動脈内に展開配置するために、かつ、ルーメンを介して熱伝達流体を受け入れるように、構成されたバルーン、ならびに、
バルーンの表面に提供された編組物であって、
弾力性を有する材料と、
基端部と、先端部と、長さと、直径と、
編組物を少なくとも完全に1回転する、ほぼ螺旋形状を有する電導体パターンであって、腎臓の除神経療法を送達するために高周波発電機と電気的に連結するように構成された電導体パターンと、
電導体パターンを欠いている編組物の領域を画成する絶縁部分と、を含んでなる編組物、ならびに、
編組物の複数の箇所に提供または連結され、かつ、複数の編組物の箇所それぞれにおいて温度を感知するように構成された、センサ構成物であって、電導体パターンの長手方向に間隔を置いた箇所に提供された複数の温度センサを含んでなり、該複数の温度センサそれぞれが長手方向に間隔を置いた箇所のうち1箇所において温度を感知するように構成されることによって、編組物の電導体パターンの温度プロファイルを提供する、センサ構成物、
