Abstract
Description
Claims (15)
- 可撓性シャフトを備えたカテーテル装置と、
前記シャフトの先端に配置され、かつ、身体の標的血管内で展開するように構成可能であるバルーンと、
前記バルーンの壁によって支持された複数のアブレーション電極であって、前記アブレーション電極は、前記バルーンが展開形態にある場合に、標的血管の壁に近接した標的組織をアブレートするのに十分な電気エネルギーを与えるように構成されている、アブレーション電極と、
前記バルーンによって少なくとも部分的に包囲され、かつ、定常状態のアブレーション加熱が始まる位置が、標的血管壁における電極−組織界面から、前記界面から所定距離だけ離れた位置に移動するように、アブレーション中に少なくとも前記電極を冷却するように構成されている冷却装置と、を備える装置。 - 前記冷却装置は、定常状態のアブレーション加熱が電極から少なくとも約0.5mm離れた距離において始まるように、前記電極を冷却するように構成されている、請求項1に記載の装置。
- 前記冷却装置は、定常状態のアブレーション加熱が電極から少なくとも約0.5mm〜約1mm離れた距離において始まるように、前記電極を冷却するように構成されている、請求項1に記載の装置。
- 前記電極は、らせんパターンまたは1つ以上の周方向パターンを画定するようにバルーン壁上に構成されている、請求項1に記載の装置。
- 前記アブレーション電極の各々は、腎動脈壁の一部を圧迫し、かつ、圧迫された腎動脈壁部分を介して電気エネルギーを与えるように機能する組織接触面を画定する隆起部を備える、請求項1に記載の装置。
- 前記電極の各々は連続湾曲形状または複合湾曲形状を有する、請求項1に記載の装置。
- 前記電極は前記シャフトに沿って延びる導電体配列に接続されている、請求項1に記載の装置。
- 前記電極は前記バルーン内の導電性流体と、前記シャフトに沿って延び、かつ、前記導電性流体と電気的に導通している導電体とによってエネルギーを与えられる、請求項1に記載の装置。
- 前記冷却装置は、
液体冷却媒体を受容し、冷凍相転移から生じる使用済み気体を排出するように構成された相転移冷凍装置、
冷却された液体冷却媒体を受容し、使用済み液体冷却媒体を排出するように構成された熱交換装置、および
1つ以上の固体熱電冷却デバイスのいずれかを備える、請求項1に記載の装置。 - バルーン壁によって支持され、かつ、アブレーション中に腎動脈壁における温度または腎動脈に近接した温度を感知するように構成された1つ以上の温度センサーを備える、請求項1に記載の装置。
- 前記電極に隣接するバルーンの内壁の少なくとも一部は、アブレーション中に冷却装置と電極との間における熱エネルギー移動を高めるように構成された熱伝導性材料の層を備える、請求項1に記載の装置。
- 前記管腔配列は、ガイドワイヤを受容するような寸法に形成されたガイド管腔を備える、請求項1に記載の装置。
- 前記カテーテル装置の基端に接続された外部システムを備え、前記システムは、前記電極に与えられる電力および前記冷却装置に送達される冷却剤を制御するように構成されている、請求項1に記載の装置。
- 前記シャフトは、経皮的アクセス位置に関して、患者の腎動脈にアクセスするのに十分な長さを有し、かつ、
前記冷却装置は、定常状態のアブレーション加熱が始まる位置が、腎動脈壁における電極−組織界面から、前記界面から所定距離だけ離れた位置に移動するように、前記電極に隣接する血管周囲腎神経のアブレーション中に少なくとも前記電極を冷却するように構成されている、請求項1乃至13のいずれか1項に記載の装置。 - 前記シャフトは基端、先端、長さ、および前記基端と先端との間に延びる管腔配列を有し、前記シャフトの長さは、経皮的アクセス位置に対して、患者の腎動脈にアクセスするのに十分であり、
前記バルーンは、腎動脈内において展開するための寸法に形成されており、かつ、小さな外形の導入形態と、より大きな外形の展開形態との間で変形可能であり、前記バルーンは前記管腔配列に流体が流れるように結合されており、かつ、展開形態にある場合に、腎動脈の壁と接触するように構成された壁を備え、
前記複数のアブレーション電極は、前記バルーンが展開形態にある場合に、腎動脈に隣接する血管周囲腎神経をアブレートするのに十分な電気エネルギーを与えるように構成されており、
前記冷却装置は、定常状態のアブレーション加熱が始まる位置が、腎動脈壁における電極−組織界面から、前記界面から所定距離だけ離れた位置に移動するように、アブレーション中に少なくとも前記電極を冷却するように構成されている、請求項1乃至14のいずれか1項に記載の装置。
