Description

本発明は、一般に睡眠障害の治療に関し、より詳細には、閉塞性睡眠時無呼吸症患者の治療のためのインプラントシステム、装置及び方法に関する。
閉塞性睡眠時無呼吸症（ＯＳＡ：Obstructive Sleep Apnea）は、気道の閉塞によって引き起こされ、通常は、睡眠中に喉の軟組織が倒れて閉鎖したときに発生する。Ｎａｔｉｏｎａｌ Ｉｎｓｔｉｔｕｔｅｓ ｏｆ Ｈｅａｌｔｈによれば、ＯＳＡには千二百万人を越える米国人が罹患している。無呼吸症状の間は毎回、患者は脳によって短時間刺激され、これによって呼吸の再開が起こる。しかしながらこの種の睡眠は、極めて断片化されており、質が低い。治療しないでおくと、ＯＳＡは、高血圧、心臓血管疾患、体重増加、性交不能症、頭痛、記憶障害、仕事への支障、及び／又は自動車事故を引き起こす場合がある。ＯＳＡの重大性にも関わらず、公衆及び医療専門家における認識の一般的な欠如により、大多数のＯＳＡ患者が未診断及び未治療のままとなっている。
ＯＳＡの治療を目的として多大な努力がなされている。例えば、いびき及び閉塞性睡眠時無呼吸症を治療するために軟口蓋を電気的に刺激するための装置が、米国特許第５，２８４，１６１号及び同第５，７９２，０６７号に開示されている。これら装置は、患者が使用レジメンを遵守することが必要であり、睡眠中患者に不快感を与え、患者が繰り返し目を覚ます結果となるので、これら装置の成果はまちまちであった。
別の治療として、持続的気道陽圧（ＣＰＡＰ）と一般に呼ばれるものは、特殊設計の鼻マスク又は枕により患者の気道に空気を供給するものである。患者が吸い込むときに空気の流れが陽圧を形成して、気道を開いたままに維持する。ＣＰＡＰは、いびき及び閉塞性睡眠時無呼吸症を緩和するために有効な非外科的治療であると多くの人々によって考えられているが、患者は、腫脹、鼻の乾燥、及びドライアイなど、マスク及びホースによる不快感についての不満を訴えている。その結果、ＣＰＡＰの患者コンプライアンスは約４０％に過ぎない。
ＯＳＡの治療には、外科的治療も使用されている。そのような治療の１つは口蓋垂口蓋咽頭形成術と呼ばれ、舌と咽頭壁との間で痙攣する軟口蓋の能力を低減するために、軟口蓋の後縁を約２ｃｍ除去するものである。別の手技としては、外科用レーザーを使用して軟口蓋表面に瘢痕組織を形成し、これが軟口蓋の柔軟性を低減させることにより、いびき及び／又は気道の閉鎖を低減させる。更に別の手技として、一般に焼灼支援口蓋硬化手術（ＣＡＰＳＯ）と呼ばれるものは、局所麻酔下で行われる診察室での手技であり、中央線に沿って軟口蓋粘膜を細長く除去し、その傷を治癒させることにより、弛緩した口蓋を硬化させる。
上述されたもののような外科的手技は、引き続き問題を有している。より具体的には、外科的に治療された組織領域（すなわち口蓋組織の除去又は口蓋組織の瘢痕）は、しばしば患者の状態を治療するのに必要な程度よりも大きい。加えて、上述の外科的手技はしばしば、長期間の不快な治癒期間に痛みがある。例えば、軟口蓋の瘢痕組織は、患者に持続的な刺激を与える場合がある。更に、上述の手技は、有害な副作用が生じた場合に元に戻すことができない。
ＯＳＡ治療のためのもう１つの外科的手技では、組織内にインプラントされる数個の編組ＰＥＴ円筒を用い、舌又は口蓋垂の組織をより堅く、屈曲しにくいものにする。Ｒｅｓｔｏｒｅ Ｍｅｄｉｃａｌ（Ｓｔ．Ｐａｕｌ，ＭＮ）から販売されているＰｉｌｌａｒ（商標）口蓋インプラントシステムは、編組ポリエステルフィラメントの円筒形エレメントからなり、軽度〜中程度のＯＳＡに罹患している患者において、気道閉塞の事象を減らすために、これを軟口蓋内に埋め込む。Ｐｉｌｌａｒ装置は数多くの有害副作用を伴い、これには円筒形エレメントの突出、感染症、及び患者の不快感が挙げられる。
ＩｎｆｌｕＥＮＴ（Ｃｏｎｃｏｒｄ，ＮＨ）によって商標「ＲＥＰＯＳＥ（商標）」として販売されている別のインプラントシステムは、口腔底の下顎骨の後面に挿入されるチタン製骨用ネジを使用する。縫合糸のループを舌根に通し、下顎骨ネジに取り付ける。Ｒｅｐｏｓｅ（商標）手技は、舌根の懸吊又はハンモックの状態を達成して、睡眠中に舌根が脱出しないようにする。しかしながら、覚醒状態の間に舌が活発に活動することに起因して、この装置の縫合要素は舌に対する「チーズカッター」としての働きをすることがあり、これは装置故障の原因となり、その後の除去を必要とする。
ＯＳＡ治療のもう１つの試みは、主気道の閉塞部分をバイパスするための補助気道の生成を伴う。同一出願人による米国特許出願第１２／１８２，４０２号（２００８年７月３０日出願）（この開示は参考として本明細書に組み込まれる）の１つの実施形態において、補助気道は、咽頭の咽頭壁の下に細長い導管を埋め込むことによって形成される。この細長い導管は、咽頭の第１領域と連通する近位端と、咽頭の第２領域と連通する遠位端と、咽頭の中咽頭領域にバイパスを形成するために咽頭壁の下に延在する中間部分とを有する。
ＯＳＡの治療には磁石も使用されている。例えば、同一出願人による米国特許出願第１２／１８３，９５５号（２００８年７月３１日出願）（この開示は参考として本明細書に組み込まれる）の１つの実施形態において、磁石インプラントには、骨アンカー、その骨アンカーに連結された第１磁石、舌アンカー、その舌アンカーに連結された第２磁石、及び、第２磁石を第１磁石から離れさせて骨アンカー方向へと向かわせる磁石間の反発力を生成するように、第１磁石と第２磁石とを整列させるための支持体が含まれる。この支持体は骨アンカーからある固定された距離に第１磁石を維持し、第１磁石を第２磁石と整列させ、かつ、第１磁石及び第２磁石の動きをガイドする。第１２／１８３，９５５号出願の１つ以上の実施形態において開示されている磁石インプラントは、ハードストップを有するインプラントを使用した場合に観察される「チーズカッター」効果を避けるために、ハードストップは有していない。
上述の先行技術にもかかわらず、長期的な結果をもたらし、患者コンプライアンスを高め、かつ患者の不快感を最小限に抑えるような、侵襲性が最小限のアプローチによるＯＳＡ治療の他のシステム、装置及び方法に対するニーズが依然として存在する。
一実施形態において、閉塞性睡眠時無呼吸症の治療方法は、中央領域、その中央領域の第１端から延びる第１アーム、及びその中央領域の第２端から延びる第２アームを有する、細長エレメントを提供することと、その細長エレメントの中央領域を舌内に埋め込むこととを含む。細長エレメントの中央領域を舌内に埋め込んだ後、第１アーム及び第２アームは好ましくは、下顎下筋肉組織に係合するまで舌内に押し進められる。本明細書で使用される用語「下顎下筋肉組織」は一般に、オトガイ舌骨筋、顎舌骨筋、二腹筋、及び翼状骨筋を指す。好ましくは、この第１アーム及び第２アームに張力をかけて、細長エレメントの中央領域を下顎下筋肉組織の方向へと引っ張り、これにより、舌の後部表面が、向かい合う咽頭壁表面から離れるよう動く。一実施形態において、張力をかけた後、第１アーム及び第２アームは望ましくは下顎下筋肉組織に固定され、これにより、舌の後部表面と向かい合う咽頭壁表面との間の空間が保持される。

一実施形態において、バットレスを形成するのに使用される材料には、例えば非再吸収性及び再吸収性ポリマーなどの、生体適合性材料が含まれ得る。好適な非再吸収性ポリマーには、シリコーン、ポリエチレンテレフタレート、ポリテトラフルオロエチレン、ポリウレタン及びポリプロピレンを挙げることができる。好適な再吸収性ポリマーには、ポリラクチド、ポリグリコリドコポリマー、ポリカプロラクトン、及びコラーゲンを挙げることができる。加えて、ニチノール、ステンレススチール、又は再吸収性合金（例えばマグネシウム若しくは鉄合金）などの材料が、バットレスを形成するのに使用され得る。一実施形態において、電気流動性又は磁気流動性材料の袋を、編組の中央内に配置することもできる。これらの材料の硬さは、軟口蓋又は咽頭後方壁などの望ましい位置で、磁界又は電界を配置することにより変えることができる。必要な磁界又は電界は、外部源から印加することができ、及び、電磁結合により材料に対して経皮的に伝達することができる。
一実施形態において、気道を開放する最適効果を提供するため、バットレスを有するインプラントを、舌内のさまざまな角度で配置することができる。一実施形態において、インプラントは舌の正中線に対して平行に配置することができる。
一実施形態において、閉塞性睡眠時無呼吸症の治療方法は、舌を通って延びる線維束の周囲を、細長エレメントで巻くことを含み、これにより線維束周囲に少なくとも１本のループが形成され、その少なくとも１本のループを用いて線維束を圧縮し、その細長エレメントにテザーを連結することができる。この方法は望ましくは、テザーの自由端を下顎下筋肉組織内に押し進めることと、ループ状になった細長エレメントを下顎下筋肉組織方向へ引っ張るためテザーに張力をかけることにより、舌の後部表面を、向かい合う咽頭壁表面から離れるよう動かすことと、そのテザーを下顎下筋肉組織に固定することとを含む。一実施形態において、このテザーは細長エレメントと共に一体形成される。

図１は、鼻腔Ｎと、硬口蓋ＨＰの骨Ｂと、軟口蓋ＳＰと、口Ｍと、舌Ｔと、気管ＴＲと、喉頭蓋ＥＰと、食道ＥＳと、後部咽頭壁ＰＰＷとを含む解剖学的構造を有するヒト頭部の断面図を示す。
人体において、鼻腔Ｎと喉頭ＬＸとの間の空気で満たされた空隙は、上気道と呼ばれる。睡眠障害を伴う上気道の最も重要な部分は咽頭ＰＸである。図２を参照すると、咽頭は３つの異なる解剖学的レベルを有する。上咽頭ＮＰは、鼻腔Ｎの後部に位置する咽頭の上部部分である。中咽頭ＯＰは、軟口蓋ＳＰ、喉頭蓋ＥＰ、及び舌Ｔの後部の曲線を含む、咽頭の中間部分である。下咽頭ＨＰは、中咽頭ＯＰの軟組織の下方に位置する咽頭の下部部分である。中咽頭ＯＰは、高率発生する、空気流のための空隙が少なくなる軟組織構造に起因して、倒れる可能性が最も高い咽頭部分である。下咽頭ＨＰは、喉頭の開口の下方かつ喉頭の後方に位置し、食道まで延びる。
当業者にはよく知られているように、軟口蓋及び舌は共に、柔軟性のある構造体である。軟口蓋ＳＰは、鼻腔Ｎと口Ｍとの間に障壁を提供する。多くの場合、軟口蓋ＳＰは必要以上に長く、舌Ｔの後部と後咽頭壁ＰＰＷとの間において相当の距離にわたって延在する。
睡眠中は体全体の筋肉が弛緩するが、呼吸器系の筋肉のほとんどは活性状態を維持する。吸入時に、横隔膜は収縮し、陰圧を引き起こして空気Ａを鼻腔Ｎ及び口Ｍの中まで引き込ませる。次いで、空気は咽頭ＰＸを通過して、気管ＴＲを介して肺内に流れる。陰圧は、上気道の組織をわずかに変形させ、これにより気道経路が狭くなる。無呼吸の患者では、軟口蓋ＳＰ、舌Ｔ、及び／又は喉頭蓋ＥＰは、後部咽頭壁ＰＰＷにぶつかって倒れ、気管の中への空気流を遮断する。気道が狭くなると、咽頭を通る空気流は乱流となり、これが軟口蓋ＳＰを振動させる原因となり、一般にいびきとして知られる音を生成する。
睡眠中、ヒトは、典型的には、空気流の短時間の閉塞及び／又は気管及び肺の中に入る空気流の量のわずかな低下を経験する。１０秒を超える空気流の閉塞は、無呼吸と見なされる。５０％を超える空気流の減少は、呼吸低下と見なされる。睡眠障害の重症度は、睡眠１時間の間に発生する無呼吸及び呼吸低下の回数で判断される。
無呼吸又は呼吸低下が１時間に５回を超えて発生する場合、ほとんどの医療関係者は、その個人が上気道の抵抗問題を有していると診断する。これらの患者の多くは、日中の眠気、抑うつ症、及び集中力の欠如などの睡眠障害に関連した症状を呈することが多い。
睡眠１時間の間に１０回以上の無呼吸又は呼吸低下の症状を発現する個人は、閉塞性睡眠時無呼吸症候群を有すると正式に分類される。気道が閉塞すると、個人は、無理に吸入するための試みを繰り返し行う。これらの症状の発現の多くは無音であり、個人が無理に空気を肺の中に引き込むときの腹部及び胸壁の動きによって特徴付けられる。典型的には、無呼吸の症状の発現は１分以上続く可能性がある。この間、血中の酸素濃度は低下する。最終的に、閉塞は、個人が大きな音のいびきを引き起こすことによって、又は息苦しい感覚によって目覚めることによって、克服される。
図２を参照すると、個人が起きている場合、舌Ｔの後部及び軟口蓋ＳＰは、舌Ｔの後部及び軟口蓋ＳＰのそれぞれの内部筋肉のために、それらの形状及び緊張を維持している。その結果、咽頭を通る気道Ａは開いたままであり、かつ遮るものはない。しかしながら、睡眠中、筋肉の緊張が低下し、そして舌の後部表面及び軟口蓋がより柔軟かつ膨張性となる。図３を参照すると、舌の後部及び軟口蓋の形状を維持し、かつ単独で又は群としてそれらを定位置に維持する正常な筋肉の緊張がなくなり、舌Ｔの後部表面、喉頭蓋ＥＰ、及び軟口蓋ＳＰは容易に倒れて気道Ａを遮断する傾向がある。
図４Ａを参照すると、一実施形態において、閉塞性睡眠時無呼吸症治療のために使用されるインプラント２０には、第１端２４及び第２端２６を有する有刺縫合糸などの細長エレメント２２が含まれる。この細長エレメント２２には好ましくは、その中央部分にあるバットレス２８と、バットレス２８と第１端２４との間にある第１アーム３０と、第１アーム３０の自由端に固定された第１針３２とが含まれる。細長エレメント２２には、好ましくは、バットレス２８と第２端２６との間に延在する第２アーム３４、及び第２アーム３４の自由端に固定された第２針３６も含まれる。一実施形態において、バットレス２８は、望ましくはこのインプラントの最も幅広の部分を形成する。
図４Ａ及び４Ｂを参照すると、一実施形態において、バットレス領域２８の中央には、望ましくは生体適合性エレメント３８がその中に配置されて含まれる。一実施形態において、生体適合性エレメント３８は楕円形を有し、前もって埋め込まれた細長エレメント内に配置することができるか、又は細長エレメントの中央に挿入してからその細長エレメントを組織内に埋め込むことができる。この細長エレメントは、非吸収性材料のみから形成することができるか、又は吸収性材料を含めることができる。非吸収性材料には、非再吸収性ポリマー、シリコーン、ポリエチレンテレフタレート、ポリテトラフルオロエチレン、ポリウレタン及びポリプロピレン、ニトニノール（nitninol）、ステンレススチール、並びに／又は複合材料などの高分子材料を挙げることができる。好適な再吸収性ポリマーには、ポリラクチド、ポリグリコリドコポリマー、ポリカプロラクトン、及び／又はコラーゲンを挙げることができる。生体適合性エレメント３８は更に、生体適合性金属又は合金を含むこともできる。
図４Ｃは、細長エレメント２２の第２アーム３４を示す。一実施形態において、第１アーム２４及び第２アーム３４はそれぞれ、好ましくは、柔軟性の芯４２から突出した複数の棘４０を含む。複数の棘４０は望ましくは、柔軟性の芯４２の長さ方向に沿って互いに間隔をあけて配置される。一実施形態において、連続して配置された棘４０の先端は、互いから約０．１５２ｃｍ（０．０６０インチ）である。一実施形態において、棘４０は、第１方向Ｄ１で組織内を引っ張られているときは内側に倒れ、第２方向Ｄ２に引っ張られているときは、第２アーム３４を定位置に保持するために組織に係合するよう適合されている。一実施形態において、棘の基底部は、アームエレメントの軸に沿って、部分的に向かい合うよう互い違いに配置されていてよく、又は、アームエレメントの軸に沿って、任意の２つの棘が直接向かい合わないようにすることができる。
図４Ａ〜４Ｃに示されている特定の実施形態は、それ自体から切り出された棘を有する単繊維である。しかしながら他の実施形態において、この細長エレメントには、棘のない編組エレメント、棘のある編組エレメント、棘あり若しくは棘なしの織布構造、及び／又は棘あり若しくは棘なしの環状編み構造が含まれ得る。特定の好ましい実施形態において、この細長エレメントには、同一出願人による米国特許出願公開第２００７０００５１０９号及び同第２００７０００５１１０号（これらは参考として本明細書に組み込まれる）に開示されている１つ以上の機能を組み込むことができる。
図５Ａを参照すると、一実施形態において、図４Ａ〜４Ｃに示されている細長エレメント２２は、患者の口腔内に埋め込まれる。図５Ａに示されているように、口腔には典型的に、舌Ｔの本体、顎舌骨筋５２、オトガイ舌骨筋５４、及びオトガイ舌筋５６が含まれる。顎舌骨筋５２は、下顎６０に固定された前方端５８と、舌骨（図示なし）に固定された後方端とを有する。図５Ａを参照すると、一実施形態において、このインプラントは好ましくは舌Ｔ内に配置され、これによりバットレス装着部分２８は、舌本体の中央に配置され、口腔の両側に向かって横方向に延在する。一実施形態において、バットレス領域２８は、舌Ｔの前方−後方軸（Ａ−Ｐとして図示されている）を横切る軸、又は実質的に垂直な軸に沿って延在する。中央バットレス装着部分２８は好ましくはインプラントを定位置に保持するための大きな表面積を有し、これにより、固定されたアンカー位置を備えたインプラントを使用した場合に起こる「チーズカッター」効果を避けることができる。インプラントの第１アーム３０は、望ましくはバットレス装着部分２８から、顎舌骨筋５２の前方端５８に向かって延在する。第２アーム３４も、望ましくはバットレス装着部分２８から、顎舌骨筋５２の前方端５８に向かって延在する。
図５Ｂを参照すると、一実施形態において、舌Ｔの組織を通って延びる１つ以上のアーム３０、３４は、好ましくは、柔軟性の芯４２と、その柔軟性の芯４２から外側に突出している複数の棘４０とを含む。棘４０は好ましくは、Ｄ１で示される方向にアームが引っ張られると、芯４２に向かって内側に倒れる。Ｄ２で示される方向にアームが引っ張られると棘４０は外側に突出し、アーム３０を舌組織内の定位置に保持する。本発明は特定の実施理論に制限されるものではないが、この棘は、組織内でのインプラントの固定を強化すると考えられる。
図６は、図４Ａ〜４Ｃの細長エレメントを中に埋め込んだ後の患者頭部の矢状方向断面図を示す。細長エレメント２２には、患者の舌Ｔ内に埋め込まれた、中央バットレス装着部分２８（この装置の第１アーム３０及び第２アーム３４よりも幅広）が含まれる。第１有刺アーム３０及び第２有刺アーム３４は、口腔の下端に向かって口腔組織内を通過して延在する。図６においては、矢状方向断面であるため、アーム３０及び３４のうち一方のみが見えている。針３２、３６は、第１有刺アーム３０、第２有刺アーム３４を組織内に押し進めるのに使用される。一実施形態において、第１有刺アーム３０及び第２有刺アーム３４は、舌骨６５の周囲でループ状になっていてよく、針３２、３６は患者の甲状腺軟骨ＴＣを通過することができる。一実施形態において、２本の有刺アーム３０、３４は好ましくは、舌骨又は甲状腺軟骨まで延在するよう適合されている。バットレス２８の中央は好ましくは舌Ｔの後方領域に埋め込まれるよう適合されており、有刺アーム３０、３４のそれぞれの遠位端は針に接続され、インプラント装置の配置及び固定を促進し得る。バットレス装着部分２８の中央は好ましくは、舌内に埋め込まれた時点で膨張する。この膨張は、有刺エレメント（例えば楕円形状の生体適合性エレメント）の芯内に生体適合性エレメントを配置することによってもたらされ得る。この生体適合性エレメントは、事前に埋め込まれた編組縫合糸内に配置することができるか、又は、編組プロセス中に挿入して装置を形成することができる。
図７を参照すると、一実施形態において、閉塞性睡眠時無呼吸症治療のためのシステムには、図４Ａ〜４Ｃに示され上述したものに類似の細長エレメントと、下顎下筋肉組織内に埋め込むよう適合された第２のバットレス１７４とが含まれる。
一実施形態において、インプラント（図４Ａ）の細長エレメントは、患者の舌の後方領域内に埋め込まれた第１バットレス、並びに、オトガイ舌骨及び／又は顎舌骨筋の中又は間に埋め込まれた第２バットレス１７４と共に、患者の口腔内に埋め込まれる。第２バットレス１７４は、二腹筋と顎舌骨筋との間に埋め込むこともできる。本発明は特定の実施理論に制限されるものではないが、下顎下筋肉組織内に埋め込みが可能な１つ以上の追加バットレスを提供することが、インプラント装置の安定性及び効果を向上させると考えられる。
図７を参照すると、一実施形態において、第２バットレス１７４には、第１端１７８、第２端１８０、及びその第１端１７８と第２端１８０との間に延在する中央開口部１８２を有する、円筒１７６が含まれる。円筒１７６には、第１放射状開口部１８４Ａ及び第２放射状開口部１８４Ｂが含まれる。第２バットレス１７４は好ましくは更に、円筒１７６の一方の側面から突出している第１フランジ１８６、及び円筒１７６の反対側の側面から突出している第２フランジ１８８を含む。第１フランジ１８６及び第２フランジ１８８は好ましくは多孔質であるか、又は多孔質フィルム若しくは布地で覆われている。
第２バットレス１７４は好ましくは更に、円筒１７６の中央開口部１８２内に受容される張力エレメント１９０を含む。一実施形態において、この張力エレメント１９０は好ましくは、円筒１７６の中央開口部１８２内で回転することができるスプールである。張力エレメント１９０は好ましくは、貫通可能な第１材料１９４を中に有する第１部分１９２と、貫通可能な第２材料１９８を中に有する第２部分１９６とを含む。この貫通可能な第１材料及び第２材料には、シリコーン、布地、織物、及び／又は中実のポリマー挿入物が挙げられ得る。一実施形態において、貫通可能な第１材料１９４及び第２材料１９８は、１つ以上の楔形スロットによって置き換えることができる。
張力エレメント１９０が円筒１７６の開口部１８２内に挿入されているとき、貫通可能な第１材料１９４及び第２材料１９８は、好ましくは、円筒１７６の第１開口部１８４Ａ及び第２開口部１８４Ｂと位置が揃っている。より詳しくは後述されるように、細長エレメントの第１アーム及び第２アームの自由端は、好ましくは、それぞれ第１開口部１８４Ａ及び第２開口部１８４Ｂ、並びに、貫通可能な第１材料１９４及び第２材料１９８を通り抜けて、インプラントの第１アーム及び第２アームを張力エレメント１９０と連結させる。張力エレメント１９０は次に、円筒１７６の細長い開口部１８２内で回転することができ、これにより、インプラントの第１アーム及び第２アームに張力をかけることができる。
図８を参照すると、一実施形態において、図７に示され上述した第２バットレス１７４が、患者の下顎下筋肉組織内に埋め込まれる。一実施形態において、第２バットレス１７４は、二腹筋肉組織ＤＧとオトガイ舌骨筋肉組織（図示なし）との間に埋め込まれる。図８に示すように、二腹筋肉組織ＤＧは、患者の舌骨６５に連結されている。図４Ａ〜４Ｃに示すインプラントの第１アーム３０及び第２アーム３４は、円筒１７６の第１開口部１８４Ａ及び第２開口部１８４Ｂを通り抜ける。張力エレメント１９０は、回転して、第１アーム３０及び第２アーム３４に対して張力をかけることができる。第１アーム３０及び第２アーム３４に張力がかかると、舌内に埋め込まれた第１バットレス（図示なし）が第２バットレス１７４の方向に引っ張られ、これにより患者の舌根及び舌骨６５が前方に引っ張られ、ＯＳＡ症状の発生傾向を最小限に抑えることができる。
図９を参照すると、一実施形態において、ＯＳＡ治療のためのインプラントには、下顎下筋肉組織内に埋め込まれる生体適合性パッド２００が含まれる。生体適合性パッド２００は好ましくは、下顎下筋肉組織内に埋め込み可能な支持体基盤を提供する幅及び長さを有する。図９を参照すると、図４Ａに示す細長エレメントの第１アーム３０及び第２アーム３４は、顎舌骨筋肉組織５２、二腹筋ＤＧ、及び生体適合性パッド２００を通り抜けている。第１アーム３０及び第２アーム３４の自由端に張力がかかると、棘４０は内側に倒れ、組織及び生体適合性パッド２００を通り抜ける。アーム３０、３４に張力がかけられた後、棘４０は、第１アーム３０及び第２アーム３４が図示されているＤ２方向に戻るのを防ぐ。本発明は特定の実施理論に制限されるものではないが、棘４０の存在により、第１アーム３０及び第２アーム３４を介して張力をかけることが可能になり、これにより舌根を、向かい合う咽頭壁から離れるよう引っ張ることができると考えられる。
図１０を参照すると、一実施形態において、閉塞性睡眠時無呼吸症を治療するシステムには、舌Ｔの組織内に埋め込まれたバットレス２２８Ａを有する第１の細長エレメント２２２Ａが含まれる。この第１の細長エレメント２２８Ａには、バットレス２２８Ａから第２バットレス２７４Ａの方向へと突出している、第１アーム２３０Ａ及び第２アーム２３４Ａが含まれる。第２バットレス２７４Ａは、図７に示され上述した構造に実質的に類似する構造を有する。第１の細長エレメント２２２Ａの第１アーム２３０Ａ及び第２アーム２３４Ａの自由端は、第２バットレス２７４の張力エレメントに連結されている。第２バットレス２７４の張力エレメントは、回転して、第１の細長エレメント２２２Ａに張力をかけることができ、これにより舌Ｔの舌根を、向かい合う咽頭壁から離れるよう動かすことができる。
図１０に示すシステムはまた、好ましくは、第２バットレス２７４Ａに類似の構造を有する第３バットレス２７４Ｂを含む。このシステムは好ましくは、舌骨２６５の周囲でループ状になっている第２の細長エレメント２２２Ｂを含む。この第２の細長エレメント２２６Ｂは望ましくは、第３バットレス２７４Ｂに連結されたそれぞれの自由端を有する、第１アーム２３０Ｂ及び第２アーム２３４Ｂを含む。第３バットレス２７４Ｂの張力エレメントは回転して、第２の細長エレメント２２２Ｂの第１アーム２３０Ｂ及び第２アーム２３４Ｂそれぞれに張力をかけることができる。本発明は特定の実施理論に制限されるものではないが、第３バットレス２７４Ｂの張力エレメントを介して、第１アーム２３０Ｂ及び第２アーム２３４Ｂに張力をかけることは、舌骨２６５を前方方向に動かし、これによりＯＳＡ症状の発生傾向を最小限に抑えると考えられる。
図１１を参照すると、一実施形態において、ＯＳＡ治療のための埋め込み可能なシステムには、後方端３８６及び前方端３８８を有する細長い本体を有する第２バットレス３７４が含まれ得る。第２バットレス３７４は、本明細書に記載の細長いインプラントとともに使用され、図７に示され上述した第２バットレス構造の代わりに使用することができる。第２バットレス３７４は好ましくは、第１バットレス（例えば図４Ａの実施形態）と第２バットレス３７４との間に延在する第１アーム及び第２アームに対して張力をかけるため、張力エレメント３９０を受容するよう適合されている。張力エレメント３９０が回転すると、細長エレメントの第１アーム及び第２アームの自由端が第２バットレス３７４の方向へと引っ張られることにより第１アーム及び第２アームに張力がかかり、これによって細長エレメントの第１バットレスが、第２バットレス３７４の方向へ引っ張られる。一実施形態において、第２バットレス３７４はメッシュ３８５を含み、これによりインプラントの体内組織に対する固定を強化するための、組織の内部成長が可能になる。

図１３を参照すると、一実施形態において、ＯＳＡ治療のためのシステムには、複数のインプラント装置５２０Ａ、５２０Ｂ、５２０Ｃが含まれる。このインプラント装置５２０Ａ〜５２０Ｃは、図１２Ａ〜１２Ｃに示すインプラント装置又は本明細書に開示されている他のインプラント装置の任意のものに類似であり得る。各インプラント装置５２０Ａ〜５２０Ｃには、好ましくは、それぞれの中央バットレス５２８Ａ〜５２８Ｃと、その中央バットレスの相対する端から突出している一対の支持アーム５３０Ａ〜５３０Ｃとが含まれる。インプラント装置５２０Ａ〜５２０Ｃは、互いに平行であってよく、また互いに対して角度がつけられていてもよい。中央バットレスは、舌５５０中に好ましく配置され、アーム５３０Ａ〜５３０Ｃは好ましくは下顎下筋肉組織に固定される。
図１４を参照すると、一実施形態において、ＯＳＡ治療のためのシステムには、互いに交差する少なくとも２本のインプラント装置６２０Ａ、６２０Ｂが含まれる。各インプラント装置には、好ましくは、それぞれの中央バットレス６２８Ａ、６２８Ｂと、その中央バットレスの相対する端から突出している一対の支持アーム６３０Ａ、６３０Ｂとが含まれる。線維との係合を最大にするため、舌６５０を通って延びる線維の面が交差するように、インプラント装置が交差している。
図１５Ａを参照すると、一実施形態において、ＯＳＡ治療のためのシステムには、第１端７２４及び第２端７２６を有する細長エレメント７２２を含んだ有刺縫合糸７２０が含まれる。細長エレメント７２２の第１端７２４には閉ループ７２５があり、これは、細長エレメント７２２の反対側の第２端７２６を受容するよう適合されている。図には示されていないが、他の実施形態において、細長エレメントには、その長さ方向に沿って設けられた、２つ又はそれ以上の閉ループが含まれていてよい。インプラント７２０には好ましくは、細長エレメント７２２の第２端７２６に固定された組織貫通エレメント７３２が含まれ、これにより細長エレメントを組織内に押し進める。細長エレメント７２２は望ましくは、それ自体から突出している少なくとも１群の棘７４０を含む。
図１５Ｂを参照して、一実施形態において、細長エレメント７２２は、舌骨７６５の周囲に細長エレメント７２２を通過させ、組織貫通エレメント７３２及び１つ以上の棘７４０を、細長エレメント７２２の第１端にある閉ループ７２５に通すことによって、ＯＳＡ治療のために使用することができる。細長エレメント７２２の第２端７２６に張力をかけ、これにより、舌骨７６５を前方及び／又は下方に動かすよう引っ張ることができる。図１５Ｂは、舌骨の周りでループ状になった細長エレメントを示しているが、咽頭壁の後方部分から舌根及び／又は舌骨を引き離すようにするため、この細長エレメントは、口腔内又は近隣にある他の構造の周りでループ状になっていてもよい。
本発明の一実施形態において、ＯＳＡ治療のためのシステムには、舌を通って延びる線維（筋線維など）の周りに巻き付けられた細長エレメントが含まれる。一実施形態において、この線維は好ましくは、舌を通って全体的に垂直方向に延在する筋線維であり、例えばオトガイ舌筋線維である。本実施形態において使用される用語「垂直」とは、人体の上端及び下端に対する方向を示す。細長エレメントは、好ましくは、筋線維の周囲に少なくとも１周ループ状になっており、これによりループ内に筋線維が捕捉される。ループ状になった細長エレメントは、垂直に延びる線維に対して実質的に水平面に延在し得る。一束の筋線維をループ状になった細長エレメント内に捕捉すると、筋線維は望ましくは、細長エレメントによって圧密又は圧縮される。一実施形態において、細長エレメントの自由端に張力をかけることができ、これにより向かい合う咽頭壁から離れるよう舌を動かすことができる。細長エレメントの自由端は、舌を前方にシフトした位置に維持するために下顎下筋肉組織に固定することができ、これにより睡眠中に、舌の後部が、向かい合う咽頭壁に対して倒れないようにすることができる。
図１６を参照すると、一実施形態において、ＯＳＡ治療のためのインプラントシステムには、患者の口腔内に埋め込まれる細長エレメント８２２が含まれる。口腔には典型的に、舌Ｔの本体、顎舌骨筋８５２、オトガイ舌骨筋８５４、及びオトガイ舌筋８５６が含まれる。顎舌骨筋８５２は、下顎８６０に固定された前方端８５８と、舌骨（図示なし）に固定された後方端とを有する。細長エレメント８２２は、好ましくは、開口部８２５を備えた第１端８２４、その開口部８２５に隣接したバットレス８２８、第２端８２６、及び棘８４０を有する。棘８４０は好ましくは、細長エレメントの第２端８２６とバットレス８２８との間で、細長エレメント８２２から突出している。一実施形態において、細長エレメントの第２端８２６は開口部８２５を通過して、舌Ｔを通って延びる線維の周りにループを形成し、この細長エレメント８２２の第２端８２６が下顎下筋肉組織に向かって引っ張られる。一実施形態において、ループ８４５内にある線維を更に圧縮し、舌根を前方及び／又は下方方向に動かすために、細長エレメント８２２の第２端８２６に張力がかけられる。細長エレメント８２２の第２端８２６は望ましくは、下顎下筋肉組織に固定され、これにより舌の位置を向かい合う咽頭壁から離して維持する。一実施形態において、この細長エレメントはバットレスを有していなくともよい。
図１７を参照すると、一実施形態において、ＯＳＡ治療のためのインプラントシステムには、第１端９２４、第２端９２６、及びその第１端と第２端との間に配置されたバットレス９２８を有する細長エレメント９２２が含まれる。一実施形態において、バットレス９２８は、望ましくはこのインプラントの最も幅広の部分を形成する。細長エレメント９２２には、好ましくは、バットレス９２８と第１端９２４との間に位置する第１アーム９３０、及びバットレス９２８とその第２端９２６との間に延在する第２アーム９３４が含まれる。組織貫通エレメント（図示なし）は、第１アーム９３０及び第２アーム９３４のそれぞれの自由端に固定され得る。一実施形態において、この細長エレメントは、図４Ａ〜４Ｃに示され上述したインプラントに見られる１つ以上の機能を有し得る。
図１７を参照すると、一実施形態において、細長エレメントのバットレス領域９２８は、舌Ｔを通って延びる線維（例えば筋線維）の周囲に巻き付けられ、線維周囲を取り囲むループ９４５を形成する。この線維はオトガイ舌筋線維であり得る。ループ９４５が形成されると、第１アーム９３０及び第２アーム９３４それぞれの自由端９２４、９２６は、舌組織内を下顎下筋肉組織の方向へと進められる。第１アーム及び第２アームの自由端９２４、９２６に張力をかけることにより、ループ９４５内の線維束が圧縮される。一実施形態において、第１アーム９３０及び第２アーム９３４の自由端に張力をかけることにより、舌を前方及び／又は下方方向に動かし、ＯＳＡ症状の発生傾向が最小限に抑えられる。第１アーム及び第２アームの端は好ましくは、本明細書に開示されている任意の実施形態の１つ以上の特徴を用いて、下顎下筋肉組織に固定される。
一実施形態において、生体適合性エレメントが線維（例えば舌を通って延びるオトガイ舌線維）の周囲にループ状になっている。生体適合性エレメントは、一方の側に開口部を有するリング様装置を含み得、これにより線維をリング様構造内に配置させることが可能になる。線維をこのリング様構造内に配置した後、リング様構造の開放端を閉じて、線維周囲を締め付け、リング様構造内の線維を圧縮することができる。テザー又は細長エレメントを、このリング様構造に連結することができる。テザーの遠位端を下顎下筋肉組織に向かって前進させ、テザーに張力をかけてリング様構造を前方及び／又は下方方向に引っ張ることができる。テザーは、下顎下筋肉組織に固定し、舌を向かい合う咽頭壁から離れるよう動かすことができる。
上述の実施形態は特定の実施理論に制限されるものではないが、舌内の筋線維は、舌の上方粘膜表面近くに至る際、全般に垂直方向に延在していることが理解される。このように、水平方向に延びるバンド又はループを、垂直に延在する線維の束の周囲に固定し、このバンド又はループを前方及び／又は下方方向に引っ張って、舌の位置を動かすことができる。テザー又は細長エレメントは、このバンド又はループと連結させることができ、テザー又は細長エレメントの下端を下顎下筋肉組織に固定した状態で、舌を前方にシフトした位置に保持することができ、これにより舌の後部は、向かい合う咽頭壁から間隔を開けた状態のままとなる。
明解さのため、図に示されている実施形態の多くでは、単繊維の細長エレメントが描かれている。ただし１つ以上の実施形態において、この細長エレメントには、棘あり若しくは棘なしの編組エレメント、棘あり若しくは棘なしの織布構造、及び／又は棘あり若しくは棘なしの環状編み構造が含まれ得る。特定の好ましい実施形態において、細長エレメントは、同一出願人による米国特許出願公開第２００７０００５１０９号及び同第２００７０００５１１０号（これらは参考として本明細書に組み込まれる）に開示されている１つ以上の機能を組み込み得る。
一実施形態において、患者には、局所麻酔又は全身麻酔を用いた手術準備が行なわれる。有刺縫合糸の第１アーム３０（図４Ａ）を、舌の後方部分を通る横方向に進めて、有刺縫合糸の中央バットレス装着部分２８が舌５０の中央になるようにする。第１支持アーム３０の端にある針は、好ましくは、舌の後方部分から、全体に前方及び下方方向へと舌内を通過して、下顎下筋肉構造に係合する。この針は、第１支持アームを舌Ｔの組織内に押し進めやすくする。第２支持アーム３４は、第２支持アーム３４の自由端に取り付けられている針を用いて舌組織内を少しずつ進められる。
一実施形態において、支持アームの遠位端又は自由端は、舌骨と下顎との間にある軟組織に取り付けられるよう適合される。一実施形態において、細い直径のトロカールを、舌根近くの口底を通って押し進めることができる。支持アームそれぞれを保持するため、好ましくはトロカールの管腔内に係蹄が導入される。支持アームは好ましくはトロカールを通して引っ張られ、トロカールは除去される。外科医は、舌の後部表面が前方方向へと動くまで支持アームの遠位端を引っ張ることができ、これにより、咽頭の後側壁に対して密閉が形成されにくくなる。支持アームの遠位端は、下顎下領域の軟組織に取り付けることができ、これにより舌が新しい位置に固定される。支持アームの遠位端は、装置の棘、糊、縫合糸、若しくはこれらの任意の組み合わせの使用により、又は支持アームの自由端２本を互いに結び合わせることにより、取り囲んだ筋肉組織を捕捉して、例えばオトガイ舌骨筋などの軟組織又は筋肉組織に取り付けることができる。
一実施形態において、有刺縫合糸の自由端は、舌骨及び／又は甲状腺軟骨に取り付けられることが意図されている。この実施形態において、細い直径のトロカールは、舌骨のすぐ下の皮膚の水平しわ近くにあらかじめ配置された切開部を通して、舌根まで押し進められる。有刺縫合糸の各自由端を保持するため、好ましくはトロカールの管腔内に係蹄が導入される。装置の自由端は好ましくはトロカールを通して引っ張られ、トロカールは除去される。外科医は、咽頭の後側壁に対して密閉が形成されにくくなるよう、又は形成できなくなるよう、舌の後部表面がわずかに前方へ動くまで、有刺縫合糸の自由端を引っ張ることができる。
一実施形態において、さまざまな組織部位に配置される２つ、３つ、又はそれ以上のバットレス装着部分を有するインプラントを使用することができる。この実施形態において、外科医は、皮膚のしわ内に小さな切開部を形成し、望ましい位置にバットレス構成要素を最小限の侵襲度で配置することができる。システム全体を、セルフロック装置又は機構を用いることによって定位置にロックすることができ、これにより外科医は、各バットレスを独立に、又はセルフロックナットの使用により、調節することができる。一実施形態において、中実のポリマー製装置の代わりに、メッシュ様のチューブを使用することができる。複数バットレスの概念は、中央取り付け装置なしで独立に使用することもできる。複数バットレスの端を結び合わせて、バットレスの反対側とは独立に、３本の端すべてを固定することができる。
本発明は、閉塞性睡眠時無呼吸症候群及び呼吸低下を治療するのに用いられる従来技術の方法及び装置に優る多くの利点を提供する。まず、本明細書に開示される方法、システム及び装置は、侵襲が最低限の単純な外科的手技を提供する。典型的には、本明細書に開示される方法、システム及び装置は、外来処置の間に利用することができる。加えて、本明細書に開示される方法、システム及び装置は、閉塞性睡眠時無呼吸症候群及び呼吸低下の治療の即効性及び長期的結果の両方を提供する。更に、本明細書に開示される方法、システム及び装置は、かなり高い患者コンプライアンスを必要としない。
加えて、本発明は舌の後側部分を不動の硬構造（例えば下顎）に固定せず、好ましくは軟組織内及び又は軟組織に対してのみ固定を行う。よって、本発明は嚥下又は発語に影響する可能性が大幅に低くなり、これにより、先行技術の装置、システム及び方法に対して大きな改善をもたらす。本発明はまた、好ましくは、長期的な生物適合性を有する材料を用いる。
本明細書に開示した様々な実施形態はヒトでの使用に関連しているが、全ての哺乳動物、及び気道を有する全ての動物内で使用できることが想定される。更に、本明細書に開示された方法、システム及び装置は、生体適合性である任意の材料、並びに装置が埋め込まれた後の拒絶反応を最小限にする、組織の内部成長を促進する、粘膜層の形成を促進する、及び身体による装置の許容を改善する任意の溶液又は構成成分を組み込むことができる。
本明細書に使用される項目は整理目的のものであり、本明細書又は請求項の範囲を制限するために使用することを意図したものではない。本出願全体に使用される用語「〜得る（may）」は、許可の意味（すなわち、その可能性を有するという意味）で使用されるものであり、命令の意味（すなわち、しなければならないという意味）ではない。同様に、「含む（include、including、及びincludes）」は、それらを含むが限定されないことを意味する。理解を促進するため、可能な場合には、図に共通の同様要素を指定するために、同様の参照番号が使用される。

該細長エレメントの該中央領域を舌内に埋め込む工程と、
該第１アーム及び該第２アームの自由端が下顎下筋肉組織に係合するまで、該第１アーム及び該第２アームを該舌内に押し進めることと、
該第１アーム及び該第２アームに張力をかけて、該舌の後方表面を、向かい合う咽頭壁表面から離れるよう動かすことと、
該第１アーム及び該第２アームを、該下顎下筋肉組織に固定することと、を含む、閉塞性睡眠時無呼吸症の治療の方法。
（２） 前記舌を通って延びる筋線維の周囲に前記細長エレメントを巻き付けることにより、該筋線維周囲に少なくとも１本のループを形成することと、
少なくとも１本の該ループを使用して、該細長エレメントに巻かれた該筋線維を圧縮することと、を更に含む、実施態様１に記載の方法。
（３） 前記固定工程が、前記第１アーム及び第２アームの少なくとも一方を前記下顎下筋肉組織に固定するために、縫合糸、クリップ、ステープル、棘、又は接着剤を使用することを含む、実施態様１に記載の方法。
（４） 前記細長エレメントの前記中央領域が、該細長エレメントのより幅広の部分を画定するバットレスを含む、実施態様１に記載の方法。
（５） 前記埋め込み工程後に、前記バットレスが前記舌の前方−後方軸を横断する軸に沿って延在する、実施態様４に記載の方法。
（６） 前記下顎下筋肉組織内に第２バットレスを埋め込むことと、前記第１アーム及び第２アームを該第２バットレスに連結することと、を更に含む、実施態様４に記載の方法。

（１９） 前記細長エレメントの前記中央領域が、該細長エレメントのより幅広の部分を画定するバットレスを含む、実施態様１６に記載のインプラント。
（２０） 前記バットレスが前記舌の前方−後方軸を横断する軸に沿って延在する、実施態様１９に記載のインプラント。
（２１） 前記下顎下筋肉組織に埋め込まれ、前記第１及び第２アームが連結された、第２バットレスを更に含む、実施態様１９に記載のインプラント。
（２２） 前記第２バットレスが、前記細長エレメントの前記第１及び第２アームに張力をかけるための張力エレメントを含む、実施態様２１に記載のインプラント。

Claims (8)

  1. 舌に埋め込み可能な中央領域、該中央領域の第１端から延びる第１アーム、及び該中央領域の第２端から延びる第２アームを含む、細長エレメントと、
    該舌を通って延在し、下顎下筋肉組織に固定される、該細長エレメントの該第１及び第２アームと、を含む、閉塞性睡眠時無呼吸症治療のためのインプラント。
  2. 前記舌を通って延びる筋線維の周囲に巻き付けられ、これにより該筋線維の周囲に少なくとも１本のループを形成する前記細長エレメントを更に含み、少なくとも１本の該ループが、該細長エレメントに巻かれた該筋線維を圧縮する、請求項１に記載のインプラント。
  3. 前記第１及び第２アームの少なくとも一方が、縫合糸、クリップ、ステープル、棘、及び接着剤からなる群から選択される締結具を使用して前記下顎下筋肉組織に固定される、請求項１に記載のインプラント。
  4. 前記細長エレメントの前記中央領域が、該細長エレメントのより幅広の部分を画定するバットレスを含む、請求項１に記載のインプラント。
  5. 前記バットレスが前記舌の前方−後方軸を横断する軸に沿って延在する、請求項４に記載のインプラント。
