Abstract
Description
Claims (29)
- ウィンドウを備えた側壁を有する中空部材と、
前記ウィンドウを交互に被覆し、かつ被覆を取るための前記中空部材に可動に接続された閉鎖部材とを含み、前記中空部材が、前記ウィンドウの縁部に沿って第1締め付け面を有し、前記閉鎖部材が、前記第1締め付け面と対向する第2締め付け面と、それに実質的に隣接した使い捨て用品とを有し、かつ
組織を互いに連結するために、前記第1締め付け面及び前記第2締め付け面との間に把持される前記組織に作用する、前記中空部材及び前記閉鎖部材の少なくとも一方に取り付けられる組織閉塞部品を含む外科用器具。 - 前記中空部材は、内視鏡の挿入部材を受けるためのチャネルを有し、前記中空部材は、前記チャネルに対して側方に位置するチャンバを含み、前記ウィンドウが前記チャンバと連通する請求項1に記載の器具。
- 前記閉鎖部材は、前記中空部材内のチャネルに、摺動可能に挿入される請求項2に記載の器具。
- 前記中空部材は、前記中空部材外側から前記チャンバの目視検査を可能にするための複数の光進入開口部を含む請求項2に記載の器具。
- 前記中空部材は、内視鏡器具の遠位端を前記チャンバに導入するための作業チャネルを含む請求項2に記載の器具。
- 前記側壁は、湾曲し、前記第1締め付け面及び前記第2締め付け面の少なくとも一方は、湾曲形状を有する請求項1に記載の器具。
- 前記閉鎖部材は、前記中空部材に摺動可能に接続され、前記第1締め付け面及び前記第2締め付け面は、前記閉鎖部材の開放及び閉鎖行程中に実質的に互いに平行なままでいる請求項1に記載の器具。
- 前記中空部材は、チャネルを備え、前記閉鎖部材は、前記チャネル内に部分的に配置され、前記ウィンドウは、前記チャネルと連通する請求項7に記載の器具。
- 前記中空部材は、縦軸を有し、前記第1締め付け面及び前記第2締め付け面は、前記軸に実質的に垂直に向けられた平面において伸長し、前記閉鎖部材は、前記軸に平行に可動である請求項7に記載の器具。
- 前記組織閉塞部材によって少なくとも部分的に形成された閉塞に遠位の組織塊を切り離すために、前記中空部材及び前記閉鎖部材の一方に操作可能に接続された組織切断部材を更に含む請求項1に記載の器具。
- 近位端で少なくとも部分的に開口し、前記近位端から間隔を置いたウィンドウを備えた側壁を有する、長手方向チャンネルを画定する中空体と、
有機組織が、前記ウィンドウを通して突出することを可能にするために前記ウィンドウから取り外し可能である、体内管腔内での中空体の位置決め中に、前記ウィンドウを被覆するために前記中空体に可動に取り付けられたシャッタ又は閉鎖部材とを含み、
前記中空体は、膨張部分によって形成された側壁を有し、前記ウィンドウは、前記膨張部分内に位置する外科用器具。 - 前記中空体は、前記膨張部分内に配置されたチャンバを有し、前記ウィンドウは、前記チャンバと連通し、前記中空部材は、前記チャネルを前記チャンバから隔てる隔壁を有する請求項11に記載の外科用器具。
- 前記チャネルは、内視鏡の挿入部材を受けるような寸法に作られる請求項11に記載の外科用器具。
- ウィンドウによって形成された側壁を有する中空体部材を含み、かつ組織閉塞部品を更に含む器具アセンブリを提供し、前記閉塞部品が一対の掴み具を画定し、前記掴み具の少なくとも一方が弓形の締め付け面を含むことと、
前記中空体部材を患者の体内管腔に挿入することと、
有機組織が、前記ウィンドウを通り前記中空体部材に突出するように前記中空体部材を操作することと、
前記ウィンドウを通る前記組織の突出後に、前記掴み具が、突出した組織の反対側に位置するように、前記閉塞部品を操作することと、
その後に、突出した組織を締め付けるために前記掴み具を閉鎖することと、
続いて前記突出した組織の一部を永久に収縮させるために、前記閉塞部品を操作することとを含む外科的方法。 - 前記閉塞部品は、組織切断装置を含み、前記閉塞部品によって少なくとも部分的に形成された閉塞に遠位の組織塊を切り離すために、前記組織切断装置を操作することを更に含む請求項14に記載の方法。
- ウィンドウを有するチャンバによって形成された側壁と、チャネルとを有する中空体部材を含む内視鏡器具アセンブリを提供し、内視鏡挿入部材が、前記チャネル内に部分的に配置されることと、
前記中空体部材と共に前記内視鏡挿入部材を患者の体内管腔に挿入することと、
有機組織が、前記ウィンドウを通り前記チャンバに突出するように前記内視鏡挿入部材と、前記中空体部材とを操作し、前記チャンバが、前記内視鏡挿入部材の側壁の側方に、又は隣接して配置されることと、
前記ウィンドウを通る前記組織の突出後に、外科手術を突出した組織上で行うために前記チャンバ内に伸長する少なくとも1つの器具を操作することとを含む外科的方法。 - 肛門鏡と、痔核閉塞装置とを含む痔核治療のための外科用器具アセンブリであって、
前記肛門鏡は、長手方向チャネルを画定する中空体を含み、前記中空体は、遠位端で閉鎖され、かつ近位端で少なくとも部分的に開放され、前記中空体は、少なくとも前記遠位端から間隔を置かれたウィンドウを備えた側壁を有し、
前記痔核閉塞装置は、遠位端に2つの掴み具を備えた器具シャフトを含み、前記掴み具の少なくとも一方は、前記ウィンドウを通って前記肛門鏡に突出した痔核組織を締め付けかつ閉塞するために、前記掴み具の他方から離れ、かつそれに向かって交互に可動な締め付け部材を含み、前記掴み具は、前記掴み具が前記ウィンドウを通って前記肛門鏡に突出した痔核組織の反対側に位置するように、前記肛門鏡に挿入可能であり、前記掴み具は、前記シャフトの一端で各々接続された一対の平行なロッドに取り付けられる外科用器具アセンブリ。 - 前記掴み具が、前記ロッドに実質的に垂直に向けられる平面において伸長する請求項17に記載の外科用器具アセンブリ。
- 前記掴み具の所与の一方が、前記ロッドに摺動可能に連結され、前記掴み具の他方が、前記ロッドに対して固定される請求項17に記載の外科用器具アセンブリ。
- 前記痔核閉塞装置は、前記掴み具の間に把持された組織に作用し、前記組織を互いに連結するために、前記掴み具に取り付けられた痔核閉塞部品を更に含む請求項17に記載の外科用器具アセンブリ。
- 前記肛門鏡は、肛門管内の肛門鏡の位置決め中に、前記ウィンドウを被覆するために、前記中空体に取り付けられたシャッタ部材を更に含み、前記シャッタ部材は、痔核組織が、前記ウィンドウを通って前記チャネルに突出することを可能にするために、前記ウィンドウの被覆を取るために、前記中空体に対して可動である請求項17に記載の外科用器具アセンブリ。
- 前記中空体は、縦軸を有し、前記側壁は、前記遠位端に向かって部分的に前記側壁の長さに沿って前記近位端から伸長し、かつ前記軸の片側に位置する膨張部分によって形成され、前記ウィンドウが、前記膨張部分内に位置する請求項17に記載の外科用器具アセンブリ。
- 前記掴み具が、前記掴み具の開放及び閉鎖行程中に互いに平行なままでいるように、前記軸に取り付けられる請求項17に記載の外科用器具アセンブリ。
- 前記組織閉塞部材によって少なくとも部分的に形成された閉塞に遠位の組織塊を切り離すために、前記閉塞装置に操作可能に接続された組織切断部材を更に含む請求項17に記載の外科用器具アセンブリ。
- 前記痔核閉塞装置は、ステープリング機構、硬化組成物の貯蔵器に接続可能な注入機構、及び電磁放射源に接続可能な放射誘導要素からなる群から選択される請求項17に記載の外科用器具アセンブリ。
- 肛門鏡と、閉鎖部材と、痔核閉塞装置とを含む痔核治療のための外科用器具アセンブリであって、
前記肛門鏡は、縦軸を有し、かつ長手方向チャネルを画定する中空体を含み、前記中空体は、遠位端で閉鎖され、かつ近位端で少なくとも部分的に開放され、前記中空体は、前記遠位端に向かって部分的に前記側壁の長さに沿って前記近位端から伸長し、かつ前記軸の片側に位置する膨張部分によって形成された側壁を有し、前記膨張部分が、少なくとも前記遠位端から間隔を置かれたウィンドウを備え、
前記閉鎖部材は、前記ウィンドウを交互に被覆し、かつ被覆を取るために、前記膨張部分内で前記中空体に摺動可能に接続可能であり、
前記痔核閉塞装置は、遠位端に2つの掴み具を備えた器具シャフトを含み、前記掴み具の少なくとも一方は、前記ウィンドウを通って前記肛門鏡に突出した痔核組織を締め付けかつ閉塞するために、前記掴み具の他方から離れ、かつそれに向かって交互に可動な締め付け部材を含み、前記掴み具は、前記掴み具が前記ウィンドウを通って前記肛門鏡に突出した痔核組織の反対側に位置するように、前記肛門鏡に挿入可能である外科用器具アセンブリ。 - 前記痔核閉塞装置が、前記掴み具の間に把持された組織に作用し、前記組織を互いに連結するために、前記掴み具に取り付けられた痔核閉塞部品を更に含む請求項26に記載の外科用器具アセンブリ。
- 前記痔核閉塞部品は、ステープリング機構、硬化組成物の貯蔵器に接続可能な注入機構、及び電磁放射源に接続可能な放射誘導要素からなる群から選択される請求項27に記載の外科用器具アセンブリ。

前記痔核閉塞装置は、遠位端に2つの掴み具を備えた器具シャフトを含み、前記掴み具の少なくとも一方は、前記ウィンドウを通って前記肛門鏡に突出した痔核組織を締め付けかつ閉塞するために、前記掴み具の他方から離れ、かつそれに向かって交互に可動な締め付け部材を含み、前記掴み具は、前記掴み具が前記ウィンドウを通って前記肛門鏡に突出した痔核組織の反対側に位置するように、前記肛門鏡に挿入可能であり、
前記掴み具は、前記掴み具の一方が、突出した痔核組織と、前記肛門鏡の内部又は遠位端との間で、前記ウィンドウを通って前記肛門鏡に突出した痔核組織の遠隔又は遠位側に位置するように、かつ前記掴み具の他方が、突出した痔核組織と、前記肛門鏡の外部又は近位端との間で、突出した痔核組織の近接又は近位側に位置するように、前記肛門鏡に挿入可能である外科用器具アセンブリ。
