Abstract
骨の間に移植され得る整形外科インプラントを提供す
る。 【解決手段】 脊椎の隣接する椎骨の間に移植するため
の整形外科インプラントは、概略環状のバッグ44と、
このバッグ44内の硬化したポリマ58とを備える。脊
椎の隣接する椎骨10を固定する方法は、隣接する椎骨
10の間の椎間板12の輪部26にアクセス孔を形成す
るステップと、輪部26により周囲を取り囲まれている
空洞を形成するために椎間板12内の核部を除去するス
テップと、空洞内に概略環状のバッグ44を配置するス
テップと、バッグ44にポリマ58を充填するステップ
と、環状のバッグ44により周囲を取り囲まれている空
洞に骨粒60を注入するステップと、ポリマ58を硬化
させるステップと、を含む。
Description
ントすなわち整形外科移植装置に関し、より詳細には、
脊椎固定(術)装置に関する。
骨の間の椎間板を除去してそれら椎骨の間にケージを設
置することによって、2つの隣接する椎骨の間を固定す
ることを必要とする。患者は前側及び後側(胃及び背
中)の両方を切開され、椎間板が2つの隣接する椎骨の
間から除去されることがある。椎間板には核部(髄核)
の周囲を取り囲む輪部(線維輪)が含まれている。輪部
を引き裂いたり、切開したり、若しくは椎骨の間から除
去したりして、さらに、より柔軟な核部を除去する。椎
骨の間の椎間板が除去された場所にケージを配置し、骨
粒を含んだ骨グラフト(骨移植体)をケージ内に詰め込
んで隣接する椎骨の間の終板の間に延在させる。さら
に、ネジがそれぞれのロッドに取り付られそれぞれの椎
骨内に延びた状態で、ロッドを脊椎の後側に配置しても
よい。
侵襲の外科手術手順で脊椎の隣接する椎骨の間に移植さ
れ得る整形外科インプラントを提供することである。

た整形外科インプラントを提供する。バッグはポリマを
充填され、バッグによって形成される中央空洞部は骨粒
及びポリマ基質を充填される。

に椎間板内の核部を除去するステップと、空洞内に概略
環状のバッグを配置するステップと、バッグにポリマを
充填するステップと、環状のバッグにより周囲を取り囲
まれている空洞に骨粒を注入するステップと、ポリマを
硬化させるステップと、を含む脊椎の隣接する椎骨を固
定する方法からなる。
腱が適正な張力をかけられ得ることである。他の利点
は、患者が手術後すぐにインプラントに荷重を加え始め
ることができることである。さらに別の利点は、インプ
ラントを単一の後部切開部位から移植することもできれ
ば、後部及び/又は前部切開部位から移植することもで
きることである。
施態様の以下の説明を参照することにより、本発明の上
述した特徴及び利点並びに他の特徴及び利点、それらを
実施する態様がさらに明らかとなり、本発明がより深く
理解されよう。複数の図面を通して対応する参照符号は
対応する部分を指している。本願に提示されている具体
例は本発明の好ましい一実施態様を一形態で示している
のであり、係る具体例が如何なる形でも本発明の範囲を
制限するものとして解釈されるべきではない。
隣接する椎骨を固定するための方法の実施態様を説明す
る。脊椎には複数の隣接する椎骨10が含まれており、
隣接する椎骨10の各対は椎間板12により分離されて
いる。椎間板12は幾つかの理由により損傷を受けた状
態になり、したがって、隣接する椎骨の固定を必要とす
ることがある。

は長手方向の各端部に逆側を向いている終板24を含ん

部(線維輪)26を含んでいる。

部14を形成し、それによって、脊髄が位置している領
域22を回避する。切開部14が形成された後に、ドリ
ルの刃先やメスのような公知の方法によって、選択され
た椎間板12の輪部26にアクセス孔30を形成する。
切開部14を患者の背中16に形成することから、通
常、アクセス孔30を椎間板12の後方側に形成する。
板12内から排出させる(図3)。核部28を除去する
ために、真空管34又は同種のものを使用することがで
きる。核部28の除去は、輪部26によって周囲を取り
囲まれている空洞42を椎間板12内に形成させる。核
部28の排出後に、空洞42及び切開部14から真空管
34を除去する。

4)。より詳細には、バッグ44を折り畳んで、事前装
填管46内に挿入する。事前装填管46は輪部26に形
成されているアクセス孔30の内径よりも僅かに小さい
外径を有する。事前装填管46の端部48がアクセス孔
30を貫通して空洞42内に延びるように、内部にバッ
グ44が装填されている事前装填管46を切開部14及
びアクセス孔30に挿入する。次に、矢印50によって
示されているように、バッグ44を事前装填管46から
空洞42内へ滑動させる。事前装填管46の内径よりも
僅かに小さい外径を有し事前装填管46内に配置されて
いるプランジャ(不図示)を利用することによる方法の
ような適宜の方法で、バッグ44を事前装填管46から
駆出することが可能である。
周囲を概略充塞するような大きさ及び形状を有している
バッグ44が選択される(図5)。第1の充填ホース5
2及び第2の充填ホース54の各々がバッグ44に取り
付けられる(図5〜図11)。第1のホース52は、バ
ッグ44を貫通して延びており、バッグ44によって周
囲を取り囲まれている空洞42の部分で終端している
(図6)。他方、第2の充填ホース54は、バッグ44
内に延びており、バッグ44内で終端している。矢印5
6によって示されているように、第2の充填ホース54
を通してバッグ44内に高強度ポリマを注入する。バッ
グ44のこの第1充填段階の際には、ポリマ58を注入
して、バッグ44を膨張又は変形させることなく、予め
定められた第1の量までバッグ44に概略充填する。示
されている実施態様では、バッグ44は多孔性のもので
あり、ポリマ58は生体吸収性且つ硬化性のポリマーの
形態のものであり、その一部がバッグ44を透過する。
ポリマ58は例えばポリメチルメタクリレートなどであ
る。硬化は、熱エネルギや光エネルギやX線エネルギの
ようなエネルギの付加又は化学的触媒の添加によって行
われ得る。図6に示されているバッグ44の第1充填段
階の際には、好ましくは、ポリマ58は未硬化状態を維
持している。

を使用することもできることは了解されよう。示されて
いる実施態様では、骨粒60は合成骨代用物のような液
体内を浮遊した状態になっている。

されるまで、第1の充填ホース52を通して空洞42内
に骨粒及び懸濁液が注入される。その後、骨粒60を空
洞42内に保持して、追加でポリマ58をバッグ44内
に注入する。予め定められた第2の量までバッグ44内
にポリマ58を注入して、バッグ44を膨張させる。バ
ッグ44は(椎間板12に対して)軸線方向に膨張し
て、終板24に接触する。さらに、バッグ44は半径方
向内向き方向に膨張して、空洞42内において骨粒60
を半径方向に圧縮すると共に軸線方向に膨張させる。し
たがって、椎骨10を取り囲む靱帯及び腱に対して、そ
の間で骨粒60を軸線方向に膨張させることにより、張
力を再度かけることが可能となる。さらに、骨粒60及
びバッグ44が接触する領域の概略全てが荷重支持部材
を形成することから、固定領域は大きな接触面積を備え
ることとなる。
化合物又はポリマ混合物58並びに骨粒60を取り囲み
それを支持しているポリマを荷重支持状態へ硬化させる
こともできる。例えば、ポリマ化合物又はポリマ混合物
をX線エネルギ又は化学的触媒で硬化させることができ
る。したがって、最小限の侵襲(minimal evasive)で
あることに加えて、患者は、整形外科インプラント8内
でポリマを硬化させた後に、着座、起立などを通して迅
速に脊椎に荷重を付加することができる。第1の充填ホ
ース52及び第2の充填ホース54は図9に示されてい
るように整形外科インプラント8から切断され、切開部
14が適宜の閉鎖技術を用いて閉鎖される。
手術手順で椎間板12内に容易に移植され得る整形外科
インプラント8を提供することが分かるであろう。隣接
する椎骨10の間でバッグ内のポリマの硬化が急速に起
こり、椎骨の間に荷重の伝達のための大きな表面積と骨
の再成長のための安定した構造を提供する。

さらに改変することができる。したがって、本願は本発
明の総合的な原理を用いて本発明の任意の変化形態、用
途、適用形態を網羅することを意図するものである。さ

- 【請求項1】 隣接する椎骨の間の椎間板の輪部にアク
セス孔を形成するステップと、 前記輪部により周囲を取り囲まれている空洞を形成する
ために、前記椎間板内の核部を除去するステップと、 前記空洞内に概略環状のバッグを配置するステップと、 前記バッグにポリマを充填するステップと、 前記環状のバッグにより周囲を取り囲まれている前記空
洞の部分に骨粒を注入するステップと、 前記ポリマを硬化させるステップと、を含む脊椎の隣接
する椎骨を固定する方法。 - 【請求項2】 前記充填するステップが、予め定められ
た第1の量まで前記バッグに前記ポリマを充填する第1
のステップを含み、予め定められた第2の量まで前記バ
ッグに前記ポリマを充填する第2のステップをさらに含
