Description
A találmány tárgya nátrium/hidrogén csere inhibitorok alkalmazása protozoák által okozott betegségek kezelésére alkalmas gyógyszerkészítmények előállítására. A találmány közelebbről a celluláris nátrium/hidrogén csere inhibitorainak alkalmazására vonatkozik protozoák által embereknél és állatoknál okozott betegségek kezelésére alkalmas gyógyszerkészítmények előállítására.
A nátrium/hidrogén csere inhibitorokat az utóbbi években számos preklinikai kísérletben olyan anyagként jellemezték, amelyek a szív csökkent vérellátása esetén előnyösen alkalmazhatók a veszélyeztetett szívszövetek elpusztulásának megakadályozására. A szívszövetek védelme kiterjed a vérellátási zavarok által okozott szívkárosodások minden formájára, a szívritmuszavartól kezdve a szívizmok túlzott összehúzódásán és az ideiglenes funkcióhiányon keresztül a szívszövetek elhalásáig és az ezekkel járó tartós károsodásokig.
A nátrium/hidrogén csere inhibitorok hatásmechanizmusa azon alapszik, hogy csökkentik a nátriumionok fokozott beáramlását, ami a csökkent vérellátású szövetekben az intracelluláris savasodás és a nátrium/hidrogén csere azt követő aktiválódása hatására következik be. Mivel az emlősszövetekben egymással össze van kapcsolva a nátrium- és kalciumion-transzport, ezáltal megakadályozható a sejtek életveszélyes kalcium-túlterhelése is. Ez az egyedülálló mechanizmus viszonylag specifikus módon elsősorban a szívszövetekre érvényes.
Meglepő módon azt találtuk, hogy a nátrium/hidrogén csere inhibitorok képesek az emberekre és állatokra patogén hatást gyakorló protozoák elpusztítására, illetve a protozoák túlélőképességének jelentős korlátozására. Ezért a nátrium/hidrogén csere inhibitorok önmagukban vagy más hatóanyagokkal kombinálva felhasználhatók protozoák által okozott betegségek kezelésére.
A találmány tárgya tehát nátrium/hidrogén csere inhibitorok alkalmazása protozoák által okozott betegségek kezelésére vagy megelőzésére alkalmas gyógyszerkészítmények előállítására, melynek során az alábbi csoportból megválasztott vegyületet alkalmazunk: 3-trifluor-metil-5-N,N-dimetil-amino-benzoil-guanidin, 3-trifluor-metil-4-(3-piridil-oxi)-benzoil-guanidin, N-{4-[4-(2-amino-etil)-fenoxi]-3-metil-szulfonil-benzoil}-guanidin és ezek farmakológiailag alkalmazható sói.
Ilyen állatgyógyászati és humán patogén protozoák elsősorban az intracellulárisan aktív paraziták. Idetartoznak az Apicomlexa és Zoomastigophora osztályba tartozó paraziták, elsősorban a tripanoszómák, Plasmodien-félék (malária-kórokozók), Leishmania-félék, Babesia-félék és Theileria-félék, valamint a Cryptosporidiidae, Sarcocystidae, amőbák, Coccidiák és trichomonadok.
A nátrium/hidrogén csere inhibitorok különösen előnyösen alkalmazhatók a következő betegségek ellen:
Plasmodium falciparum által okozott malaria tropica, Plasmodium vivax és Plasmodium ovale által okozott malaria tertiana,
Plasmodium malariae által okozott malaria quartana,
Toxoplasma gondii által okozott toxoplazmózis,
Isospora belli által okozott coccidiosis,
Sarcocystis suihominis által okozott bélsarcosporidiosis,
Cryptosporidium parvum által okozott cryptosporidiosis,
Trypanosoma cruzi által okozott „Chagas-betegség”, kután vagy visceralis, vagy más Leishmaniosák által okozott fertőzés.
Alkalmazhatók továbbá állat patogén protozoák által okozott betegségek kezelésére:
szarvasmarháknál keleti-parti lázat okozó Theileria parva, szarvasmarháknál és bivalyoknál texasi lázat okozó Babesia begemina, európaiszarvasmarha-babesiosist, valamint kutyáknál, macskáknál és juhoknál babesiosist okozó Babesia bovis, juhoknál, szarvasmarháknál és sertéseknél sarcocystosist okozó Sarcocystidae, szarvasmarháknál és madaraknál cryptosporidiosist okozó Cryptosporidiidae, nyulaknál, szarvasmarháknál, juhoknál, kecskéknél és sertéseknél, valamint tyúkoknál és pulykáknál coccidiosist okozó Coccidiák ellen.
A humán és állat patogén protozoák által okozott betegségek kezelésére a nátrium/hidrogén csere inhibitorok adagolhatok orálisan vagy parenterálisan. A nátrium/hidrogén csere inhibitorok alkalmazhatók önmagukban vagy más hatóanyagokkal kombinálva. Mivel a nátrium/hidrogén csere inhibitoroknál egy új hatásmechanizmusról van szó, ezért ezek kombinálhatok olyan gyógyszerhatóanyagokkal, amelyeket az adott protozoabetegség kezelésére már használnak, melynek során előnyös additív hatás érhető el.
A nátrium/hidrogén csere inhibitorok különösen előnyösen alkalmazhatók a különböző maláriaformák, elsősorban malaria tropica kezelésére. Jelenleg is mintegy 300-500 millió ember szenved maláriabetegségben, és minden tizenkettedik másodpercben egy ember meghal az ilyen fertőzések következtében. A maláriás betegségek száma minden elővigyázat ellenére a következő években fokozódik, mivel hiányoznak a malária leküzdésére alkalmas hatékony módszerek. A malária ellen eddig alkalmazott hatóanyagok hatékonyságukat elvesztették. A malária kórokozói, a Plasmodium nembe tartozó protozoák rezisztenssé váltak. A kialakuló katasztrófa elkerüléséhez az Egészségügyi Világszervezet szerint sürgősen új hatóanyagokra van szükség a malária megelőzésére és gyógyítására. A malária ellen alkalmazható új gyógyszerek kifejlesztése azonban nagyon nehéz.
Különösen meglepőnek minősül ezért, hogy a nátrium/hidrogén csere inhibitorok nemcsak a klorokvinra érzékeny, hanem a klorokvinra rezisztens Plasmodium falciparum törzsekre is hatékonyak. A malária-kórokozók késői eritrocitastádiumának szokásos kezelése
HU 225 583 Β1 mellett a malária korai formáinak nátrium/hidrogén csere inhibitorokkal történő kezelésével a kórokozók már a májban előnyösen elpusztíthatok.
A nátrium/hidrogén csere inhibitorok protozoákkal szembeni hatékonyságát a következő kísérletekkel igazoljuk.
Plasmodium falciparum tenyészet
A humánmalária-parazita Plasmodium falciparum eritrocitastádiumait L-glutaminnal, 25 mmol/l HEPES-sel, 4 pg/ml gentamicinnel, 10% humán szérummal és 5% humán eritrocitával kiegészített RPMI 1640 közegben tenyésztjük, Trager és Jensen (1976) szerint. A tenyészetet Petri-csészékben (10 ml közeg 10 cm átmérőjű edényekben) tartjuk, és 92% nitrogén, 3% szén-dioxid és 5% oxigén összetételű légkörben 37 °C hőmérsékleten inkubáljuk. A tenyészközeget naponta cseréljük. Legfeljebb 10% parazitémia elérése után a tenyészetet hígítjuk. A tenyészetek szinkronizálásához a fertőzött eritrocitákat 5%-os szorbitoldattal mossuk Lambros és Vanderberg (1979) szerint. Ezt a kezelést csak a gyűrűs stádium éli túl.
IC50-érték meghatározása a különböző nátrium/hidrogén csere inhibitorokkal A különböző nátrium/hidrogén inhibitorok Plasmodium falciparum növekedése ellen mutatott 50%-os gátlókoncentrációját 96 mérőhelyes mikrotitrálólemezeken határozzuk meg a radioaktív hipoxantin beépülése alapján Desjardin és munkatársai (1979) szerint. Ennek során a parazitákat egy 48 órás teljes eritrocitaciklusban megfelelő nátrium/hidrogén csere inhibitor je5 lenlétében tenyésztjük.
A nátrium/hidrogén csere inhibitorokat dimetil-szulfoxidban oldjuk, és fiziológiás sóoldattal (150 mmol/l NaCI) hígítjuk. A mikrotitrálólemezekre a nátrium/hidrogén csere inhibitorokat tízszeresen koncentrált 20 μΙ térfogatú alikvot részekben visszük fel. Ezután minden mérőhelyhez 180 μΙ parazitaszuszpenziót adunk tenyészközegben. A paraziták a gyűrűs stádiumban találhatók, a parazitémia értéke 1 %. Ezután a mikrotitrálólemezeket 24 órán keresztül inkubáljuk. A tenyészkö15 zeg kicseréléséhez minden mérőhelyről 150 μΙ felülúszót eltávolítunk, és 135 μΙ új közeggel pótoljuk, ami 2 pCi/ml [3H]-hipoxantint tartalmaz. A nátrium/hidrogén csere inhibitorok koncentrációját tízszeresen koncentrált törzsoldat 15 μΙ alikvot részekben történő adagolá20 sával ismét beállítjuk. További 24 órán keresztül végzett inkubálás után a tenyészeteket zsugorított üvegszűrőn szűrjük, és a beépült radioaktivitást mérjük. A tríciumbeépülés %-os gátlását a nátrium/hidrogén csere inhibitor nélküli kontrolihoz viszonyítjuk.
A különböző nátrium/hidrogén csere inhibitorok IC50-értékét egy klorokvinra érzékeny törzsön (HB3) és egy klorokvinra rezisztens törzsön (Dd2) mérjük. Az eredményeket a következő táblázat tartalmazza.

- SZABADALMI IGÉNYPONTOK1. Nátrium/hidrogén csere inhibitorok alkalmazása protozoák által okozott betegségek kezelésére vagy megelőzésére alkalmas gyógyszerkészítmények előállítására, azzal jellemezve, hogy az alábbi csoportból megválasztott vegyületet alkalmazunk:3-trifluor-metil-5-N,N-dimetil-amino-benzoil-guanidin,3-trifluor-metil-4-(3-piridil-oxi)-benzoil-guanidin,N-{4-[4-(2-amino-etil)-fenoxi]-3-metil-szulfonil-benzoil}-guanidin és ezek farmakológiailag alkalmazható sói.
- 2. Nátrium/hidrogén csere inhibitorok 1. igénypont szerinti alkalmazása malaria tropica kezelésére vagy megelőzésére alkalmas gyógyszerkészítmények előállítására.
