Description
Foreliggende oppfinnelse vedrører anvendelse av inhibitorer av cellulære natrium-hydrogen-utvekslere for fremstilling av et medikament for behandling eller profylakse av sykdommer i mennesker og dyr forårsaket av protozoer.
F.S. Buckner et al., Antimicrobial Agents and Chemotherapy, bind 40,1996, s. 2592-97 beskriver en teknikk for å undersøke legemidler med henblikk på aktivitet mot trypanosoma cruzi ved anvendelse av parasitter som uttrykker (3-galaktosidase, og en av forbindelsene som ble testet var 2-guanidino-benzimidazol, som er en inhibitor av glyceralde-hyd-3-fosfat dehydrogenase av beslektede organismer.
WO84/00875 (D2) beskriver en spesifikk klasse av guanidiner som har en fenylalkyl, fenylalkenyl, substituert fenylalkenyl eller en fenyl substituert med en 3- til 7-leddet ring bundet til acylgruppen, har antiprotozoisk aktivitet (side 2 og krav 1 i publikasjonen).
Inhibitorer av natrium-hydrogenutvekslere (NHE) er i løpet av de senere årene blitt karakterisert i en mengde prekliniske studier som forbindelser som er meget egnede ved lavere blodgjennomstrømning i hjertet for å beskytte skadet hjertevev mot ødeleggelse. Beskyttelse av hjertevevet omfatter følgelig alle fonner av hjerteskader frembragt ved utilstrekkelig blodgjennomstrømning, begynnende med hjerte-rytmeforstyrrelser over hyperkontraktur av hjertemuskelen og forbigående funksjonstap helt til død av hjertevev med derved forbundede varige skader.
Virkningsmekanismen til NHE-inhibitorene er slik at de reduserer den forsterkede inn-strømningen av natriumioner som oppstår på grunn av utilstrekkelig gjennomblødning av vev på grunn av intracellulær forsuring og påfølgende aktivering av NHE. På grunn av at det i pattedyrvev er en sammenkobling av natrium- og kalsiumionetransporten blir dermed også den livstruende kalsiumoverladningen til cellene redusert. Denne en-keltstående mekanismen er relativt spesifikk, og virker spesielt på hjertevev.
Det var følgelig overraskende at det ble oppdaget at NHE-inhibitorene er i stand til å drepe protozoer med patogen virkning på mennesker og dyr og i betydelig grad å be-grense overlevelsesevnen til protozoene. Følgelig egner inhibitorer av natrium-hydrogenutvekslere seg, alene eller i kombinasjon med andre virkestoffklasser, for behandling av sykdommer som er forårsaket av protozoer.
Slike veterinær- og humanpatogene protozer er fortrinnvis intracellulært aktive parasitter fra klassen Apicomlexa og Zoomastigophora, spesielt Trypanosomer, Plasmodier
(Malaria-fremkallere), Leishmanier, Babesier og Theilerierer, Cryptospoirdiidae, Sarcocystidae, amøber, Coccidien og Trichomonader.
NHE-inhibitorer egner seg fortrinnsvis for behandling av Malaria tropica, som er forårsaket av Plasmodium falciparum,
for behandling av Malaria tertiana, utløst av Plasmodium vivax og Plasmodium ovale, for behandling av Malaria quartana, forårsaket av Plasmodium malariae, samt for behandling av
Toxoplasmose, utløst av Toxoplasma gondii,
Coccidiose, utløst av Isospora belli,
Tarmsarcosporidose, forårsaket av Sarcocystis suihominis,
Cryptosporidose, forårsaket av Cryptosporidium parvum,
Chargas-sykdom, forårsaket av Trypanosoma cruzi,
kutane og viscerale samt andre former for Leishmanioser,
samt for behandling av dyr, som er blitt infisert med veterinærpatogene protozoer, som med
Theileria parva, som aktiverer østkystfeber i okser,
Babesia begemina, som aktiverer Texasfeber i okser og bøfler,
Babesia bovis, som aktiverer europeisk oksebabesiose, samt Babesioser i hund, katter og sauer,
Sarcocystidae, som aktiverer Sarkozystoser i sauer, okser og svin,
Cryptospiridiidae, som utløser Cryptosporidose i okse og fugler,
Coccidiener, som aktiverer Coccidioser i kaniner, okser, sauer, geiter og svin, spesielt høner og kalkuner.
Ved behandling av sykdommer som er forårsaket av human- og veterinærpatogene protozoer, kan NHE-inhibitorer administreres både oralt og også parenteralt. Dermed anvendes NHE-inhibitorene som monoforbindelser og i kombinasjon med andre virkestof-fer. Da det ved NHE dreier seg om et nytt virkeprinsipp er kombinasjoner med legemidler foretrukket som allerede blir anvendt for behandling av hver av de protozoebetingede sykdommer idet det observeres en gunstig, additivt forsterket virkning.
Spesielt foretrukket er NHE-inhibitorer for behandling av Malaria-former, spesielt for behandling av Malaria tropica. For tiden har 300-500 millioner mennesker sykdommen Malaria og hvert 12. sekund dør et menneske på grunn av denne infeksjonssykdommen. Antall Malaria-tilfeller forventes å øke ytterligere i kommende år, idet virksomme me-toder for å bekjempelse av Malaria foreløpig mangler. Medikamenter som frem til nå er blitt anvendt mot Malaria, har i stor grad mistet sin virkning. Malaria-fremkalleren, protozoer fra slekten Plasmodium er blitt resistent. For å unngå kommende katastrofe, er det nødvendig for Verdens Helseorganisasjon å oppnå nye legemidler for profylaktisk og kurativ behandling av Malaria. Derimot har det vist seg å være meget vanskelig å utvikle nye medikamenter mot Malaria.
Det var dermed overraskende at NHE-inhibitorer har vist seg virksomme ikke bare mot kloroquin-sensitive, men også mot kloroquin-resistente Plasmodium falciparum-stammer. Ved siden av den hittil vanlige behandlingen av sene erytrocytære stadier av Malaria-utløsere, synes behandling med NHE-inhibitorer av tidlige stadier av Malaria for avlivning av sykdomsfremkalleren allerede i leveren å være spesielt fordelaktig.
Nedenfor blir virksomheten til NHE-inhibitorer mot protozoer beskrevet:
Kultur av Plasmodium falciparum
Erytrocytære stadier av humane Malaria-parasitter Plasmodium falciparum ble dyrket i RPMI 1640-medium, supplementert med L-glutamin, 25 mM HEPES, 4 ug/ml genta-mycin, 10% humant serum og 5% humane erytrozyter, ifølge Trager og Jensen (1976). Kulturene ble opprettholdt i petriskåler (10 ml pr. skål med 10 cm diameter) og inkubert ved en atmosfære omfattende 92% nitrogen, 3% karbodioksid og 5% oksygen ved 37°C. Kulturmediet ble utvekslet daglig. Ved en parasitemi på maksimalt 10% ble kulturene fortynnet. For å synkronisere kulturene ble de infiserte erytrocytene vasket i 5% sorbi-toloppløsning ifølge Lambros og Vanderberg (1976). Denne behandlingen overlever bare ringstadiene.
Bestemmelse av ICso-verdier av forskjellige NHE-inhibitorer
Halvmaksimal inhibitorisk konsentrasjon av forskjellige NHE-inhibitorer for formering av Plasmodium falciparum ble målt på 96 brønn-mikrotiterskåler ved tilsetning av ra-diaktivt hypoksantin ifølge Desjardins et al. (1979). Dermed ble parasittene dyrket i en 48 timer lang fullstendig erytrocytær syklus i nærvær av tilsvarende NHE-inhibitorer.
NHE-inhibitorene ble oppløst i DMSO og forfortynnet i fysiologisk saltoppløsning (150 mM NaCl). På mikrotiterskålen ble det tilsatt en fortynningsrekke av NHE-inhibitorene i 10 ganger konsentrerte 20 ul-alikvoter. Deretter ble det i hver brønn tilført 180 ul pa-rasittsuspensjon i kulturmediet. Parasittene befant seg i ring-stadiet ved en parasittemi på 1%. Deretter ble mikrotiterskålen inkubert i 24 timer. For å bytte kulturmediet, ble det fra hver brønn fjernet 150 ul supernatant og erstattet med 135 ul nytt medium, i til-legg supplementert med 2 uCi/ml [3H]-hypoksantin. Konsentrasjonen av NHE-inhibitoren ble innstilt ved tilførsel av 10 ganger konsentrert stamoppløsning i 15 (il alikvoter. Etter ytterligere 24 timers inkubasjon ble kulturen høstet ved filtrering gjen-nom glassfiberfilter og inkorporert radioaktivitet ble målt. Den prosentvise inhibisjonen av tritiuminkorporeringen ble vurdert relativt til en kontroll uten NHE-inhibitor.
ICso-verdiene til de forskjellige NHE-inhibitorene ble målt på en klorquin-sensitiv (HB3) og en kloroquin-resistent (Dd2) stamme.

Foreliggende oppfinnelse omfatter følgelig anvendelse av en inhibitor av Na^VtT" utveks-leren for fremstilling av et medikament for behandling eller profylakse av sykdommer som forårsakes av protozoer, hvor det anvendes en forbindelse som er valgt fra gruppen bestående av forbindelsene
3-trifluormetyl-5-N,N-dimetylaminobenzoylguanidin,
3- trifluormetyl-4-(3-pyridyloksy)benzoylguanidin,
4- [4-(2-aminoetyl)fenoksy]-3-metylsulfonylbenzoylguanidin
og deres farmasøytisk tålbare salter.
Ifølge foretrukne utførelsesformer av oppfinnelsen anvendes inhibitoren av Na<+>/H<+> ut-veksleren for fremstilling av et medikament for behandling eller profylakse av Malaria tropica, for behandling eller profylakse av Malaria tertiana, for behandling eller profylakse av Malaria quartana, for behandling eller profylakse av toksoplasmose, for behandling eller profylakse av kokkidose, for behandling eller profylakse av tarmsarkosporidose, for behandling eller profylakse av kryptosporidose, for behandling eller profylakse av Chargas-sykdom, for behandling eller profylakse av kutan og visceral samt andre former for Leishmanioser og endelig for behandling eller profylakse av sykdommer i dyr som er infisert med veterinærpatogene protozoer.
Claims (11)
1.
Anvendelse av en inhibitor av Na<+>/H<+->utveksleren for fremstilling av et medikament for behandling eller profylakse av sykdommer som forårsakes av protozoer, hvor det anvendes en forbindelse som er valgt fra gruppen bestående av forbindelsene
3- trifluormetyl-5-N,N-dimetylaminobenzoylguanidin,
3 -trifluormetyl-4-(3 -pyirdyloksy)benzoylguanidin,
4- [4-(2-aminoetyl)fenoksy]-3-metylsulfonylbenzoylguanidin
og deres farmasøytisk tålbare salter.

Anvendelse av en inhibitor av Na<+>/H<+->utveksler ifølge krav 1 for fremstilling av et medikament for behandling eller profylakse av Malaria tropica.

Anvendelse av en inhibitor av Na<+>/H<+->utveksler ifølge krav 1 for fremstilling av et medikament for behandling eller profylakse av Malaria tertiana.

Anvendelse av en inhibitor av Na<+>/H<+->utveksler ifølge krav 1 for fremstilling av et medikament for behandling eller profylakse av Malaria quartana.

Anvendelse av en inhibitor av Na<+>/H<+->utveksler ifølge krav 1 for fremstilling av et medikament for behandling eller profylakse av toksoplasmose.

Anvendelse av en inhibitor av NaVH+-utveksler ifølge krav 1 for fremstilling av et medikament for behandling eller profylakse av kokkidiose.
