Otra medida para cuantificar la asimetría “cuadrada vs redondeada” de la periferia 200 es una comparación entre las relaciones de radios adyacentes. En el compartimento 20 lateral más cuadrado, los pares de radios adyacentes definen relaciones grandes debido a que los radios de borde grandes (es decir, de arcos 208 borde anterolateral, arco 212 de borde lateral y arco 216 de borde posterolateral) son mucho más grandes que los radios de esquina adyacentes (es decir, de arco 210 de esquina anterolateral y arco 214 de esquina posterolateral). De otra parte, en el compartimiento 22 medial redondeado, los pares de radios adyacentes definen relaciones pequeñas (es decir, casi 1:1) porque los radios de los arcos mediales (es decir, arco 220 de esquina anteromedial, arco 222 de borde medial y arco 224 de esquina posteromedial) tienen magnitud similar.
En la realización ilustrada de la figura 3A, el arco 212 de borde lateral se considera un “borde” porque el arco 212 define una tangente 212A que es sustancialmente perpendicular al borde 202 anterior. Cuando una “esquina” se puede considerar que es la parte de la periferia 200 que hace una transición de anterior o posterior a medial o lateral, y el borde es esa parte de la periferia 200 que abarca el término anterior, posterior, medial o lateral de la periferia 200.
Del mismo modo, el arco 222 de borde medial define una tangente 222A que también es sustancialmente perpendicular al borde 202 anterior. El “borde” medial de la periferia 200 puede hacer parte del mismo arco que se extiende alrededor de la esquina anteromedial y/o la esquina anterolateral, cuando los arcos mediales son similares. De hecho, como se indica aquí, el compartimiento 22 medial puede tener un único arco que se extiende desde el borde 202 anterior hasta el borde 206 posterior medial.
La tabla 2 muestra una comparación entre las relaciones de radios adyacentes para compartimientos 20 y 22 mediales y laterales. Para cada par adyacente de radios, la diferencia entre las magnitudes de radios se expresa como un porcentaje de los radios más pequeños de par, como se indicó anteriormente.

Como se ilustra en la tabla 2, la periferia “cuadrada” del compartimiento 20 lateral aumenta a valores Δ-12L, Δ-23L, Δ34L y Δ-45L de disparidad que tienen por lo menos 42%, 48% o 59% y a lo sumo % 323, 337% o 362%. Se contempla que la disparidad entre un par de radios adyacentes en la periferia cuadrada del componente 20 lateral puede tener cualquier valor porcentual dentro de cualquier rango definido por cualquiera de los valores enumerados. También se contempla que la disparidad lateral valores pueden ser sustancialmente más mayores, según se requiera o desee para una aplicación particular.
Mientras tanto, la periferia “redondeada” del compartimiento 22 medial da aumento a valores Δ-12R y Δ-23R de disparidad que son más pequeños de 21%, 23% o 25% y no mayor de 61% y 62%, de 74%. Se contempla que la disparidad entre un par de radios adyacentes en la periferia redondeada del compartimento 22 medial puede ser cualquier valor dentro de cualquier rango definido por cualquiera de los valores enumerados. También se contempla que los valores de disparidad mediales pueden ser menores 21% y tan pequeño como cero %, sea se requiere o desea para una aplicación particular.
Más aún, la forma cuadrada del compartimiento 20 lateral y la forma redondeada del compartimento 22 medial también se demuestra mediante el número de arcos utilizados para definir la parte de la periferia 200 en los compartimientos

20, 22 medial y lateral. En el compartimento 20 lateral, se utilizan cinco arcos (es decir, arcos, 208, 210, 212, 204, 216) para definir la periferia lateral, que es indicador de “lados” anterior, lateral y posterior de una caja unida por transiciones relativamente agudas de arcos 210, 214 de esquina. De otra parte, el compartimiento 22 medial utiliza sólo tres radios (es decir, 220, 222, 224), que no dejan una clara definición de cualquiera de los “lados” cuadrados u otras transiciones. De hecho, se contempla que el compartimiento 22 medial puede unir el borde 202 anterior borde al 206 posterior medial mediante un único radio dentro de la extensión de la presente divulgación.
b. Área de superficie de compartimentos de placa base Medial y Lateral
Aún con referencia a la figura 3A, todavía otra caracterización de asimetría de la periferia 200 surge a parir de disparidades en el área de superficie para los compartimientos 20, 22 medial y lateral. Para los propósitos de la presente divulgación, al área de superficie del compartimento SAL lateral es esa área contenida dentro de la periferia 200 y en sobre el lado lateral del eje AH central. Del mismo modo, al área de superficie del compartimento 22 medial es esa área contenida dentro de la periferia 200 y sobre el lado medial del eje AH central.
En una realización de ejemplo, el área SALX de superficie lateral puede ser tan pequeña como 844 mm2 puede ser a lo sumo 1892 mm2 o puede ser cualquier área dentro del rango definida por los anteriores valores. En una realización de ejemplo, las áreas SAMX de superficie medial pueden ser tan pequeñas 899 mm2 o pueden ser a lo sumo 2140 mm2 o pueden tener cualquier área dentro del rango definido por los anteriores valores.
Las áreas de superficies SAL y SAM no incluyen ninguna de las áreas ocupadas por el corte 28 PCL, ya que cualquiera de dichas áreas no está dentro de la periferia 200. Sin embargo, la asimetría de las áreas SAL y SAM de superficie surge principalmente de las diferencias en la geometría y la ubicación de los arcos 208, 210, 212, 214, 216, 220, 222, 224 a diferencia de cualquier asimetría de corte 28 PCL. En las realizaciones ilustrativas de la figura 2A, por ejemplo, el corte 28X PCL es simétrico con respecto al eje AH central, pero se extiende posteriormente adicionalmente en el compartimiento 22 medial.
De esta manera, se contempla que la asimetría de las áreas SAL, SAM de superficies se cambian poco mediante la exclusión del corte 28 PCL del cálculo de área. Como se ilustra en la figura 3D, el corte 28 PCL se excluye efectivamente del cálculo al extrapolar la línea formada por el borde 204 posterior lateral y el borde 206 posterior medial hacia adentro para interceptar el eje AH central. En el compartimiento 20 lateral, dicha extrapolación coopera con el lateral del corte 28 PCL para definir área 80 de rellano lateral. En el compartimiento 22 medial, dicha extrapolación coopera con el lado medial del corte del 28 PCL para definir área 82 de relleno medial.
En la realización ilustrativa de la figura 3D, el área SALX' de superficie lateral puede ser tan pequeña como 892 mm2 puede ser a lo sumo 2066 mm2 o puede ser cualquier área dentro del rango definido por los anteriores valores. En una realización de ejemplo, el área SAMX' de superficie medial puede ser tan pequeñas como 986 mm2 o puede ser a lo sumo 2404 mm2 o puede tener cualquier área dentro del rango definido por los anteriores valores.
Las tablas 3 y 4 adelante ilustran las áreas SAMX de superficie medial que ocupan un mayor porcentaje del área total de superficie contenida dentro de la periferia 200X, independientemente de si el corte 28 PCL se incluye en el cálculo. Es decir, el área 82 de relleno medial es más grande que el área 80 de relleno lateral en aproximadamente la misma proporción que las áreas SAMX, SALX de superficie medial y lateral. En las realizaciones de ejemplo de la figura 3A, el área SAMX de superficie medial ocupa entre 52% y 53% del área de superficie total independientemente, mientras que el área SAMX de superficie lateral ocupa el resto. Si el corte PCL se excluye del cálculo como se muestra en la figura 3D, el área SAMX' de superficie medial ocupa entre 52% y 54% del área de superficie total, mientras que el área SAMX' de superficie lateral ocupa el resto. Con o sin el corte PCL incluido en el cálculo, se contempla que las áreas SAMX, SAMX' de superficies medial pueden ocupar tan poco como el 51% del área superficie total y a lo sumo el 60% del área de superficie total.

Por ejemplo, en una familia de ejemplo de tamaños de prótesis, por lo menos 60% y a lo sumo 90% del cubrimiento de la superficie proximal extirpada está provista por la meseta 18 tibial de la placa base 12 tibial en donde la rotación se limita a +/-5 grados a partir del eje de AH central. En la mayoría de pacientes, dicha cubrimiento está entre 75-85%. El cubrimiento de hasta el 100% se puede alcanzar dentro del alcance de la presente divulgación, tal como al extender completamente el cubrimiento anterolateral y posteromedial de la meseta tibial (que deja intencionalmente espacio entre la meseta 18 tibial y la periferia de la tibia T como se indicó aquí).
El material posteromedial adicional de la meseta 18 tibial incluye un chaflán 32, descrito en detalle adelante con respecto al ensamble de la placa base 12 tibial para el componente 14 de soporte tibial. El chaflán 32 se forma en la pared 25 periférica, de tal manera que el chaflán 32 forma un ángulo α (figura 8) con la superficie 35 distal o que cubre el hueso de la meseta 18 tibial. En la realización ilustrada, el chaflán 32 define un perfil de sección transversal sagital sustancialmente lineal, con ángulo α entre aproximadamente 35 grados y aproximadamente 55 grados. Adicionalmente, se contempla que el chaflán 32 puede tener un perfil arqueado en un plano sagital, coronal y/o transversal y puede incluir curvaturas cóncavas o convexas según se requiera o desee para una aplicación particular.
2. Crecimiento periférico progresivo entre tamaños de implante
Además de la asimetría de cada tamaño individual/realización de placa base 12 de tibia, descrito anteriormente en detalle, la presente divulgación también proporciona asimetría en la forma periférica 200 que crece desde un hasta el siguiente. Ventajosamente, este crecimiento periférico asimétrico acomoda la tendencia de crecimiento observada en la tibia T de pacientes con diferente tamaño, mientras que también conserva el ajuste y cubrimientos óptimos proporcionados por la placa de base 12 y ofrece otras ventajas de diseño de acuerdo con la presente divulgación como se describe aquí.
En el crecimiento periférico simétrico, una placa base de gran tamaño es una versión a escala aumentada de un tamaño más pequeño y viceversa. En el crecimiento periférico asimétrico actual, en contraste, determinados parámetros de placa base 12 tibial crecen más rápidos que otros cuando el tamaño general de la placa base se hace más grande (es decir, de tamaño más pequeño 1/A a tamaños más grandes 9/J). De esta manera, se hacen componentes de diferente tamaño de acuerdo con la presente divulgación y no son proporcionales a otros en ningún aspecto, porque una prótesis tibial más grande no es proporcionalmente más grande que una prótesis tibial más pequeña en todos los aspectos.
Con referencia ahora a la figura 2B, la periferia 200X define un centroide CX, que se inclina medialmente con respecto al eje AH central debido a que el área SAM de superficie medial es más grande que el área SAL de superficie lateral (como se describió anteriormente en detalle). La distancia DMPX medial posterior se extiende desde el centroide CX hacia la “esquina” medial posterior de la periferia 200X (es decir, hacia el arco 224 de esquina medial posterior, mostrado en la figura 3A y descrito anteriormente) en un ángulo de 130 grados en sentido contra horario del eje AH. De manera similar, la distancia DLPX lateral posterior se extiende desde el centroide CX hacia la “esquina” posterolateral de la periferia 200X (es decir, hacia el arco 214 de esquina posterolateral, mostrado en la figura 3A y descrito anteriormente) en un ángulo de 120 grados a partir del eje AH central. Las esquinas posteromedial y posterolateral se definen en una forma similar que las esquinas anterolateral y anteromedial, descritas en detalle anteriormente. Más aún, aunque el crecimiento posterolateral y posteromedial asimétrico entre tamaños consecutivos se describe adelante con respecto a la distancia DLPX, DMPX, dicho crecimiento ocurre en el área completa ocupada por las esquinas postero medial y posterolateral.
Como se ilustra en la figura 2A y se muestra en la tabla 6 adelante, las distancias DLPX, DMPX posterolateral y posteromedial no crecen linealmente cuando el tamaño 1/A progresa entre tamaños consecutivos hasta alcanzar eventualmente el tamaño 9/J más grande. Por el contrario, las distancias DLPX, DMPX posterolateral y posteromedial exhiben un aumento en la magnitud de crecimiento cuando el tamaño progresa consecutivamente desde el tamaño 1/A hasta el tamaño 9/J. Este crecimiento asimétrico, no es lineal se ilustra en las gráficas de las figuras 2C y 2D y en la tabla 6 adelante.

En la figura 2C, la cantidad de crecimiento en DMPX se grafica contra tamaño no.X. Como se ilustra, la familia de placas base 12 tibiales ilustradas en la figura 2A presentan un aumento de crecimiento constante en DMPX, con casi aumento promedio del 20% en crecimiento de un tamaño hasta el siguiente tamaño consecutivo (como se representa por la pendiente de la línea de tendencia lineal que tiene la ecuación y = 0.1975x + 2.0225).
En la figura 2D, la cantidad de crecimiento en DLPX se grafica contra el tamaño no.X e ilustra un aumento de crecimiento aún positivo, pero más pequeño, a través de los tamaños de placa base. Más específicamente, la familia de placas base 12 tibiales ilustrada en la figura 2A presenta un aumento en promedio de casi el 4% en crecimiento de un tamaño al siguiente tamaño consecutivo (como se representa por la pendiente de la línea de tendencia lineal que tiene la ecuación y = 0.0392 x + 2.5508).
Como se utiliza aquí, una “familia” de prótesis se refiere a un grupo o kit de prótesis que comparten características de desempeño y/o geométricas comunes. Por ejemplo, la familia de nueve placas base tibial cuya periferia 200X se muestra en la figura 2A comparten una asimetría común como se describe aquí, de tal manera que cada placa base tibial se adapta para proporcionar cubrimiento sustancial tibial, facilita la rotación adecuada del implante y evita el impacto con diversos tejidos blandos de la rodilla. Normalmente, una familia de prótesis incluye una pluralidad de componentes de diferentes tamaños, con componentes de tamaños consecutivamente más grande/más pequeños para acomodar una variedad de huesos de diferente tamaño. En las realizaciones de ejemplo de la presente divulgación, una prótesis de tamaño “1” o “A” es la prótesis más pequeña de la familia, una prótesis de tamaño “9” o “J” es la prótesis más grande de la familia, y cada uno de los tamaños intermedios “2” o “B” a “8” o “H” son tamaños consecutivamente más grandes.
Ventajosamente, en la familia o kit de periferias de prótesis mostradas en la figura 2A, cada placa base 12 tibial (figura 1A) tiene una periferia 200X que proporciona un acoplamiento estrecho con un subconjunto particular de tibias T de paciente que tienen una única forma y tamaño. Se han diseñado características particulares de la periferia 200X con crecimiento no lineal que se calcula proporciona el ajusta más cercano posible para el número más gran de geometrías naturales particulares encontrados en las tibias T anatómicas, como se describe en detalle aquí. El ajuste estrecho permite el cubrimiento máximo de la periferia 200X tibial proximal extirpada, al acomodar los cambios no lineales que pueden ocurrir a través de tamaños periféricos tibiales anatómicos. Las distancias DLPX, DMPX posterolateral y posteromedial son parámetros de crecimiento no lineal de ejemplos encontradas en una familia de placas base 12 tibiales y reflejan el crecimiento no lineal en la extensión DMLX mediolateral y extensión DAPMX y DAPLX anteroposteriores a través de diversos tamaños.
3. Corte PCL alineado con técnica asociada y eje central
En la realización ilustrada, la meseta 18 tibial incluye un corte 28 PCL dispuesto entre los compartimientos 20, 22, como se describió anteriormente. El corte PCL deja el punto CP de unión PCL accesible, permitiendo por lo tanto al PCL pasar a través durante y después de implante de la placa base 12 tibial. El componente 14 de soporte tibial (figura 5) puede incluir del mismo modo el corte 30.
De esta manera, la realización ilustrada de la prótesis 10 tibial se adapta para procedimiento quirúrgico de retención cruzada (CR), en el que el ligamento cruzado posterior no se extirpa durante el implante de la prótesis 10 tibial. Adicionalmente, como se indicó anteriormente, el eje AH central incluye la referencia al punto CP del punto de unión PCL cuando la placa base 12 tibial se monta sobre tibia T. Con el fin de facilitar la alineación del eje AH central con respecto a la placa base 12 tibial y la tibia T, el indicio de alineación 70A, 70P (figuras 4A y 4B) se pueden marcar en la pared sobre la superficie 34 proximal y/o la pared 25 periférica. Cuando la placa base 12 tibial se implanta (como

se describe adelante), se alinea el indicio 70A de alineación anterior (figuras. 4A y 4B) con el punto CA anterior en el “tercer medial” del tubérculo T tibial anterior, y el indicio 70P de alineación posterior se alinea con el punto CP de unión PCL natural de la tibia T.
Sin embargo, se contempla que se puede hacer una prótesis de acuerdo con la presente divulgación para un diseño en el que el ligamento cruzado posterior es extirpado durante la cirugía, tal como diseños "estabilizados posteriormente" (PS) o "ultra congruentes" (UC). Los diseños PS y UC pueden excluir el corte 30 PCL en el componente 14 de soporte, obviando por lo tanto la necesidad del corte 28 PCL en la placa base 12 tibial. En cambio, el material continuo puede ocupar el corte 28 (como se esquemáticamente muestra en la figura 3D). Más aún, se contempla que los cortes 28, 30 PCL pueden tener cualquier forma y/o tamaño dentro del alcance de la presente divulgación. Por ejemplo, los cortes 28, 30 PCL pueden ser asimétricos con respecto a un eje anteroposterior. Para propósitos de la presente divulgación “bisectar” un corte PCL asimétrico con un eje anteroposterior se refiere a dividir dicho recorte en dos áreas iguales para una sección anteroposterior dada del eje anteroposterior
4. Componente y habilitación de flexión profunda
Volviendo de nuevo a la figura 1A, el componente 14 de soporte tibial incluye la parte 39 lateral, parte 41 medial, superficie 36 inferior adaptada para acoplarse a la placa base 12 tibial y superficie 38 superior adaptada para articular con cóndilos de un componente femoral (tal como el componente 60 femoral mostrado en la figura 8 y descrito en detalle adelante). La superficie 38 incluye la superficie 40 articular lateral en una 39 parte lateral y superficie 42 articular medial en la parte 41 medial, con eminencia 44 (figura 5) dispuesta entre las superficies 40, 42 articulares. Con referencia a la figura 5, la eminencia 44 corresponde de manera general en forma y tamaño con la eminencia tibial natural de la tibia T antes de extirpación.
Con referencia ahora a la figura 1A, la meseta 18 tibial de la placa base 12 tibial incluye adicionalmente una superficie 35 de contacto ósea o distal y una superior 34 superior o proximal opuesta, con la superficie 34 superior que tiene un perímetro 24 elevado y mecanismos 26 de bloqueo formados entre los compartimientos 20, 22 medial y lateral. El perímetro 24 elevado y el mecanismo 26 de bloqueo cooperan para retener el componente 14 de soporte tibial sobre la placa base 12 tibial, como se describe en detalle adelante. Las Solicitudes de Patentes Provisionales de los Estados Unidos Números de serie 61/367,374 y 61/367,375, se describen mecanismos de bloqueo de placa base de ejemplo, titulados PRÓTESIS TIBIAL.
La superficie 36 interior del componente 14 de soporte tibial incluye la cavidad 46 en la periferia del mismo y un mecanismo de bloqueo de soporte tibial (no mostrado) se dispone entre las superficies 40, 42 articular medial y lateral. Las Solicitudes de Patentes Provisionales de los Estados Unidos Números de serie 61/367,374 y 61/367,375 se describen mecanismos de bloqueo de componentes, titulados PRÓTESIS TIBIALES. La cavidad 46 tiene un tamaño y se posiciona para que corresponda con el perímetro 24 elevado de la meseta 18 tibial, y el mecanismo de bloqueo de soporte tibial coopera con el mecanismo 26 de bloqueo de la meseta 18 tibial para fijar el componente 14 de soporte tibial a la placa base 12 tibial en una posición y orientación deseadas como se describe en detalle adelante. Sin embargo, se contempla que el componente 14 de soporte tibial se puede fijar a la placa base 12 mediante cualquier mecanismo o método adecuado dentro del alcance de la presente divulgación, tal como mediante un adhesivo, disposiciones de lengüeta/ranura, mecanismos de acción de presión, y similares.
Como se ve mejor en las figuras 1B, 5 y 8, la periferia externa del componente 14 de soporte tibial corresponde generalmente con la periferia externa de la meseta 18 tibial, excepto por la extensión posteromedial de la meseta 18 cuando se compara con el componente 14 de soporte tibial. La “esquina” del componente 14 de soporte tibial define el radio R3 (figura 5) que tiene un centro generalmente común con radio R2L placa base 12 en un plano transversal, es decir, los radios R2L y R3 son sustancialmente coincidentes en una vista de plano. Del mismo modo, la “esquina” anteromedial del componente 14 de soporte tibial define un radio R4 que tiene un centro generalmente común con radio R1R de placa base 12 en un plano transversal, es decir, radios R1R y R4 que son sustancialmente coincidentes en una vista de plano.
R3 define una longitud radial ligeramente más pequeña cuando se compara con R2L y R4 define una longitud radial ligeramente más pequeña cuando se compara con R1R, de tal manera que la parte anterior de la pared 54 de perímetro del componente 14 de soporte tibial se fija hacia detrás de la parte anterior de la pared 25 periférica (es decir, del borde 202 anterior y los arcos adyacentes, como se describió anteriormente) de la placa base 12 tibial. En cuanto a la comparación descrita anteriormente entre los radios R2L y R1R, el radio R4 anteromedial es sustancialmente mayor que el radio R3 anterolateral.
Dada la parte 41 medial del componente 14 de soporte tibial tiene una extensión menos anteroposterior en comparación con el compartimiento 22 medial de la meseta 18 tibial, la parte 41 medial debe estar inclinada anteriormente con el fin de que las “esquinas” anteromediales del componente 14 de soporte tibial en la meseta 18 tibial coincida como se muestra en la figura 5. En vista de esta inclinación anterior, se puede decir que el componente 14 de soporte tibial se orienta asimétrica sobre la meseta 18 tibial. Mas particularmente, aunque la superficie 40 articular lateral se centra generalmente con respecto al compartimiento 20 lateral de la meseta 18 tibial, la superficie 42 articular medial se sesga anteriormente con respecto al compartimento 22 medial de la meseta 18 tibial con el fin

de dejar el chaflán 32 expuesto a la esquina posterolateral. Este montaje asimétrico del componente 14 de soporte tibial sobre la meseta 18 tibial asegura una interacción articular deseada entre la prótesis 10 tibial y el componente 60 femoral, como se describe en detalle adelante.
La meseta 18 tibial de la placa base 12 tibial se desvía de la periferia del componente 14 de soporte tibial en la parte posteromedial de cada componente, dejando la parte 41 incongruente con el compartimiento 22 medial de la placa base 12 tibial. Más particularmente, la meseta 18 tibial se extiende posteromedialmente hasta cubrir sustancialmente la superficie extirpada proximal de la tibia T, como se muestra en la figura 5 y como se describió anteriormente, aunque el componente 14 de soporte tibial no se extiende más allá posteromedialmente del terminal superior del chaflán 32 (es decir, el componente 14 de soporte tibial no “sobresale” del chaflán 32). Adicionalmente, el componente 14 de soporte tibial incluye el chaflán 50 formado en la pared 54 periférica, con el chaflán 50 que tiene una disposición geométrica y de perfil que corresponde con el chaflán 32 de la meseta 18 tibial. Mas particularmente, cuando el componente 14 de soporte tibial se ensambla con la placa base 12 tibial como se muestra en las figuras 1B y 8, la orientación anterior o “sesgo” de la parte medial del componente 14 de soporte tibial (como se describió anteriormente) alinea los chaflanes 32, 50, que a su vez cooperan para crear un chaflán sustancialmente continuo que se extiende desde la tibia T hasta la superficie 42 articular medial. Con referencia a la figura 8, los chaflanes 32, 50 cooperan adicionalmente para definir el vacío 52 formado entre el fémur F y la meseta 18 tibial cuando la prótesis 10 tibial está en una orientación de flexión profunda. En la realización ilustrada de la figura 8, la orientación de flexión profunda se define por el ángulo β entre el aje AT de tibia anatómico y el eje AF femoral anatómico de hasta aproximadamente 25 grados a aproximadamente 40 grados, por ejemplo (es decir, aproximadamente 140 grados a 155 grados de flexión o más).
Ventajosamente, el vacío 52 coopera con el “retiro” o borde 206 posteromedial incongruente y esquina 224 posteromedial, cuando se compara con una periferia lineal típica (descrito anteriormente), para permitir la orientación de flexión profunda que se va a alcanzar sin el impacto del componente 60 femoral y/o fémur F sobre a meseta 18 tibial y/o el componente 14 de soporte tibial. Los tejidos blandos en la región de vacío 52 se acomodan por lo tanto también con poco o ningún impacto en los componentes circundantes.
Adicionalmente, el tamaño relativamente grande de la meseta 18 tibial (que cubre una gran parte de la superficie proximal extirpada de la tibia T) también permite que el componente 14 que tiene sea relativamente grande, de tal manera que el componente 14 de soporte tibial proporciona un área de superficie suficiente no articular en los chaflanes 32, 50 y alrededor de la periferia las superficies 40, 42 articular medial y lateral para permitir transiciones redondeadas, de radio relativamente grande, entre las superficies 40, 42 articulares y la pared 54 periférica del componente 14 de soporte tibial. Estas transiciones de radio grande, graduales evitan la fricción indebida entre la prótesis 10 tibial y cualquier tejido blando circundante que puede permanecer en el lugar después de implante de la prótesis, tal como la banda iliotibial (IT).
En determinados rangos de articulación de prótesis, por ejemplo, la banda iliotibial (IT) humana (puede tomar la “esquina” anterolateral, es decir, la parte del componente 14 de soporte tibial que tiene el radio R3. Debido a que la extensión anterolateral del componente 14 de soporte tibial sigue a la extensión anterolateral de la meseta 18 tibial (como se describió anteriormente), la transición entre la superficie 40 articular lateral y la pared 54 periférica en el punto de contacto entre una banda IT y el componente 14 de soporte tibial puede tener una parte convexa relativamente grande mientras aun deja suficiente espacio cóncavo para la superficie 40 articular. Esta parte convexa grande resulta en un área grande de contacto si la banda IT hace contacto con el componente 14 de soporte tibial, que en cambio resulta en presiones relativamente bajas sobre la banda IT. Adicionalmente, el “retiro” anterolateral o incongruencia entre el arco 210 de esquina anterolateral de la periferia 200 y una periferia tibial típica, descrita en detalle anteriormente, permite a la esquina anterolateral correspondiente del componente 14 mantener la separación de la banda IT a través de un amplio rango de flexión y presiones de bajo contacto en donde ocurre el contacto.
Sin embargo, para cualquier contacto entre la banda IT y el componente 14 de soporte tibial se puede evitar o minimizar al designar la periferia 200 de tal manera que el arco 210 de la esquina anterolateral y/o y el arco 212 de borde lateral se coloca lejos de la periferia esperada de una tibia T típica (como se calcula a partir de datos anatómicos, descritos anteriormente). Esta separación extra diseñada en la periferia 200 proporciona espacio extra para la banda iliotibial. Adicionalmente, esta holgura extra asegura que la proporción substancial de pacientes posibles que carecen del tubérculo de Gerdy, que es una eminencia ubicada en la parte anterolateral de la tibia T, no experimentarán ninguna “saliente” de la meseta 18 tibial más allá de la periferia anatómica de la tibia T extirpada.
De esta manera, en términos generales, la prótesis 10 tibial puede ser considerada “amigable con los tejidos blandos” porque los bordes del componente 14 de soporte tibial y la meseta 18 tibial, incluye chaflanes 32, 50, son suaves y redondeados, de tal manera que cualquier tejido blando que entre en contacto con estos bordes probablemente sean menos propensos a achaflanarse o raspar.
Ventajosamente, el área de superficie inferior/distal relativamente grande de la meseta 18 tibial facilita una gran cantidad de crecimiento óseo en donde el material de crecimiento óseo se proporciona en la placa base 12 tibial. Por ejemplo, la placa base 12 también se puede construir de o se puede recubrir de un biomaterial altamente poroso.
