Description
Válvula percutánea y sistema de suministro
La presente descripción se refiere en general a aparatos, sistemas y métodos para su uso en el sistema vascular y, más en particular, a una válvula percutánea, un sistema y un método para su uso en el sistema de vasculatura.
Las válvulas pueden resultar dañadas y/o enfermas por una diversidad de razones. Las válvulas dañadas y/o enfermas se agrupan según qué válvula o válvulas estén involucradas, y de la cantidad de flujo sanguíneo que se interrumpe por la válvula dañada y/o enferma. Por ejemplo, las enfermedades más comunes de las válvulas
15 cardíacas ocurren en las válvulas mitral y aórtica. Las enfermedades de las válvulas tricúspide y pulmonar son bastante raras.
La válvula aórtica regula el flujo sanguíneo desde el ventrículo izquierdo hacia la aorta. La aorta es la principal arteria que suministra sangre oxigenada al cuerpo. Como resultado, las enfermedades de la válvula aórtica tienen un impacto significativo en la salud del individuo. Ejemplos de tales enfermedades incluyen regurgitación aórtica y estenosis aórtica.
La regurgitación aórtica se denomina también insuficiencia aórtica o incompetencia aórtica. Consiste en una condición en la que el flujo sanguíneo retrocede desde una válvula aórtica ensanchada o debilitada hacia el
25 ventrículo izquierdo del corazón. En su forma más severa, la regurgitación aórtica está causada por una infección que deja orificios en las hojuelas de la válvula. Los síntomas de regurgitación aórtica pueden no aparecer durante años. Cuando los síntomas aparecen, es porque el ventrículo izquierdo trabaja de forma más dura en relación a una válvula aórtica no comprometida para formar el flujo de retorno de la sangre. El ventrículo se hace eventualmente más grande y el fluido retrocede. La estenosis aórtica es un estrechamiento o bloqueo de la válvula aórtica. La estenosis aórtica ocurre cuando las hojuelas de la válvula resultan recubiertas con depósitos. Los depósitos cambian la forma de las hojuelas y reducen el flujo sanguíneo a través de la válvula. De nuevo, el ventrículo izquierdo tiene un trabajo más duro en relación con una válvula aórtica no comprometida para la formación del flujo sanguíneo reducido. Las realizaciones de la presente descripción están dirigidas a un aparato y un sistema para reemplazo y/o aumento de válvula percutánea. Por ejemplo, el aparato puede incluir una válvula que puede ser usada para reemplazar una válvula inservible (por ejemplo, una válvula aórtica, una válvula mitral, una válvula tricúspide, o una válvula pulmonar) o una vena en un lumen corporal. Los materiales biológicos adicionales a partir de los cuales se forman hojuelas 104 de válvula incluyen, aunque sin limitación, las venas explantadas, pericardio, lata facia, válvulas cardiacas cultivadas, vejiga, pared venosa, varios tipos de colágeno, elastina, sub-mucosa intestinal, y materiales de la membrana basal descelularizados, tal como sub-mucosa de intestino delgado (SIS), tejido amniótico, o cordón umbilical.
15 Alternativamente, las hojuelas 104 pueden estar formadas a partir de un material sintético. Los posibles materiales sintéticos incluyen, aunque sin limitación, politetrafluoretilieno expandido (ePTFE), politetrafluoretileno (PTFE), poliestireno-poliisobutleno-poliestireno (SIBS), poliuretano, poli(carbonato-uretano) segmentado, poliéster, polietileno (PE), polietileno tereftalato (PET), seda, uretano, rayón, silicona o similar. En una realización adicional, el material sintético puede incluir también metales, tales como acero inoxidable (por ejemplo, 316L) y nitinol. Estos materiales sintéticos pueden estar en un tejido, un tricotado, un molde u otras configuraciones físicas conocidas permeables o impermeables al fluido. Adicionalmente, se pueden incrustar metales chapeados con oro en los materiales de la hojuela 104 (por ejemplo, una configuración de sándwich) para permitir la visualización de las hojuelas 104 tras la colocación.
25 Según podrá apreciarse, la válvula 100 puede ser tratada y/o recubierta con cualquier número de tratamientos superficiales o materiales. Ejemplos de tales tratamientos incluyen, aunque sin limitación, agentes bioactivos, incluyendo los que modulan la trombosis, los que fomentan el crecimiento celular interno, crecimiento transversal, y endotelialización, los que resisten la infección y los que reducen la calcificación.
Las figuras 2A y 2B proporcionan un ejemplo de una válvula 200 no reivindicada por la presente invención. En una realización adicional, la válvula 300 puede incluir además un material 348 sellante posicionado en la periferia de la estructura de válvula 302. En una realización, una vez implantado el material 348 sellante, puede hincharse debido a la presencia de líquido para ocupar el volumen entre la estructura de válvula 302 y el tejido sobre el que se haya implantado la válvula 300 con el fin de impedir la pérdida de líquido alrededor de la pared exterior de la válvula
35 300.
Las realizaciones pueden incluir también un material sellante posicionado sobre una porción del borde 322 periférico de la estructura 311 de hojuela para sellar la estructura 311 de hojuela en la estructura de válvula 302.
Resulta factible una diversidad de materiales adecuados como material 348 sellante. Por ejemplo, el material 348 sellante puede ser seleccionado a partir de la clase general de materiales que incluyen polisacáridos, proteínas y geles biocompatibles. Ejemplos específicos de estos materiales poliméricos pueden incluir, aunque sin limitación, los derivados del óxido de polietileno (PEO), polietileno glicol (PEG), alcohol de polivinilo (PVA), polivinilpirrolidona (PVP), polietiloxazolina (PEOX), poliaminoácidos, seudopoliaminoácidos, y polietiloxazolina, así como copolímeros
45 de estos con cada uno de otros polímeros solubles en agua o de polímeros insolubles en agua. Ejemplos de polisacárido incluyen los derivados del alginato, ácido hialurónico, sulfato de condroitina, dextrano, sulfato de dextrano, heparina, sulfato de heparina, sulfato de heparán, quitosán; goma de gelano, goma de xantano, goma guar, derivados de celulosa solubles en agua, y carragenina. Ejemplos de proteínas incluyen las derivadas del gelano, colágeno, elastina, zeína y albúmina, si se producen a partir de fuentes naturales o recombinantes.
En una realización adicional, la válvula 300 de la presente descripción puede incluir miembros de anclaje unidos a la estructura de válvula 302 o a los miembros de válvula 306. Los miembros de anclaje pueden incluir púas, ganchos, etc.
55 Las realizaciones de la válvula descrita en la presente memoria, pueden ser usadas para reemplazar, suplementar o aumentar estructuras de válvula en el interior de uno o más lúmenes del cuerpo. Por ejemplo, las realizaciones de la presente invención pueden ser usadas para sustituir una válvula cardíaca defectuosa del corazón, tal como las válvulas aórtica, pulmonar y/o mitral del corazón. En una realización, la válvula cardíaca natural puede permanecer en su lugar o bien ser retirada (por ejemplo, por medio de un procedimiento de valvoplastia) con anterioridad a implantar la válvula cardíaca de la presente descripción.
Adicionalmente, posicionar el sistema que posee la válvula según se ha descrito en la presente memoria incluye introducir el sistema en el sistema cardiovascular del paciente usando técnicas percutáneas, transluminal, mínimamente invasivas. Por ejemplo, se puede posicionar un alambre de guiado en el interior del sistema 65 cardiovascular de un paciente que incluye la posición predeterminada. El sistema de la presente descripción, que incluye la válvula según se ha descrito en la presente memoria, puede ser posicionado sobre el alambre de guiado y
hacer avanzar el sistema con el fin de posicionar la válvula en, o adyacente a, la posición predeterminada. En una realización, se pueden usar marcadores radiopacos en el catéter y/o en la válvula, según se ha descrito en la presente memoria, para ayudar a localizar y posicionar la válvula.
5 La válvula puede ser desplegada desde el sistema en la posición predeterminada según cualquier número de formas, tal y como se describe en la presente memoria. En una realización, la válvula de la presente descripción puede ser desplegada y colocada en un número cualquiera de posiciones cardiovasculares. Por ejemplo, la válvula puede ser desplegada y colocada en el interior de una arteria principal de un paciente. En una realización, las arterias principales incluyen, aunque sin limitación, la aorta. Adicionalmente, las válvulas de la presente invención
10 pueden ser desplegadas y colocadas en el interior de otras arterias principales del corazón y/o en el interior del propio corazón, tal como en la arteria pulmonar para la sustitución y/o el aumento de la válvula pulmonar y entre el atrio izquierdo y el ventrículo izquierdo para el reposicionamiento y/o el aumento de la válvula mitral. También son posibles otras localizaciones.
15 Una vez implantada, la válvula puede proporcionar contacto suficiente con la pared del lumen corporal para impedir el flujo retrogrado entre la válvula y la pared del lumen corporal, y para posicionar la válvula de forma segura e impedir la migración de la válvula. La válvula descrita en la presente memoria presenta también flexibilidad y resistencia suficientes como para albergar cambios en el diámetro del lumen corporal, mientras se mantiene la colocación adecuada de la válvula. Según se describe en la presente memoria, la válvula puede encajar en el lumen
20 con el fin de reducir el volumen de flujo retrogrado a través y alrededor de la válvula. Se comprenderá, sin embargo, que se puede producir alguna pérdida de flujo entre la válvula y el lumen corporal y/o a través de las hojuelas de la válvula.
Claims (11)
- REIVINDICACIONES1.- Una válvula (100, 300), que comprende:5 una estructura de válvula (102, 302) que tiene un extremo (114) distal y un extremo (112) proximal y miembros (106) de estructura que definen un lumen (108, 308),una hojuela de válvula (104, 304) unida a una porción de la estructura de válvula (102, 302), en la que la hojuela de válvula (104, 304) tiene un extremo (118) distal y un extremo (116, 216) proximal, en la que una porción del extremo (118, 218) distal de la hojuela de válvula (104, 304) está unida de forma adyacente a un extremo (112, 312) proximal de la estructura de válvula (102, 302), yun miembro (110, 310) de transición de hojuela que está unido a al menos una porción de la hojuela de válvula (104, 304);15 caracterizada porque el miembro (110, 310) de transición de hojuela es una estructura de anillo que está acoplada al extremo (116) proximal de la hojuela de válvula (104, 304), en la que la hojuela de válvula (104, 304) y el miembro (110, 310) de transición de hojuela se extienden a distancia del lumen (108, 308) en una primera posición y están dispuestos para su transición a una segunda posición en la que la hojuela de válvula (104, 304) y el miembro (110, 310) de transición de hojuela están en el interior del lumen (108, 308) de la estructura de válvula (102, 302), en la que el miembro (110, 310) de transición de hojuela se expande desde un primer diámetro hasta un segundo diámetro para asegurar el miembro (110, 310) de transición de hojuela en la segunda posición.
- 2.- La válvula (100, 300) de la reivindicación 2, en la que, en la primera posición, la hojuela de válvula (104, 304) y el25 miembro (110, 310) de transición de hojuela están al menos parcialmente por fuera del lumen (108, 308) de la estructura de válvula (102, 302), extendiéndose el extremo (116) proximal de la hojuela de válvula (104, 304) a distancia tanto del extremo (114) distal como del extremo (112, 312) proximal de la estructura de válvula (102, 302), y en la que la hojuela de válvula (104, 304) y el miembro (110, 310) de transición de hojuela están dispuestos para una transición desde la primera posición hasta la segunda posición en la que el extremo (116) proximal de la hojuela de válvula (104, 304) y al menos una porción del miembro (110, 310) de transición de hojuela están en el interior del lumen (108, 308) de la estructura de válvula (102, 302).
- 3.- La válvula (100, 300) de las reivindicaciones 1 ó 2, en la que el miembro (110, 310) de transición de hojuela, en la segunda posición, esta acoplado a miembros (106, 306) de estructura.35 4.- La válvula (100, 300) de las reivindicaciones 1-3, en la que el miembro (110, 310) de transición de hojuela está dispuesto para expandirse desde un estado contraído en la primera posición, hasta un estado expandido en la segunda posición.
- 5.- La válvula (100, 300) de la reivindicación 4, en la que el miembro (110, 310) de transición de hojuela tiene forma de serpentín en estado contraído con el fin de expandirse desde un primer diámetro hasta un segundo diámetro en estado expandido.
- 6.- La válvula (100, 300) de las reivindicaciones 4-5, en la que la estructura de válvula (102, 302) incluye miembros45 (106, 306) de estructura que definen el lumen (108, 308) y el miembro (110, 310) de hojuela, en estado expandido, incluye al menos una porción del miembro (110, 310) de transición de hojuela que está dispuesta para ser expandida entre los miembros (106, 306) de estructura para asegurar el miembro (110, 310) de transición de hojuela en la segunda posición.
- 7.- La válvula (100, 300) de las reivindicaciones 1-6, en la que la hojuela de válvula (104, 304) incluye una estructura (111, 311) de hojuela acoplada a una porción de un borde (122, 322) periférico de la hojuela de válvula (104, 304).
- 8.- La válvula (100, 300) de la reivindicación 7, en la que la estructura (111, 311) de hojuela está acoplada al miembro (110, 310) de transición de hojuela.55 9.- Un sistema (324), que comprende:un catéter (326) de suministro alargado;un globo (328) expandible posicionado alrededor de al menos una porción del catéter (326) de suministro alargado;una funda (330) replegable posicionada alrededor de al menos una porción del catéter (326) de suministro alargado, en la que la funda (330) replegable está dispuesta para moverse longitudinalmente en relación con el catéter (326) de suministro alargado; y65 una válvula (100, 300) de acuerdo con cualquiera de las reivindicaciones anteriores, en la que una porción de la válvula (100, 300) está situada entre el catéter (326) de suministro alargado y la funda (330) replegable.
- 10.- El sistema (324) de la reivindicación 9, en el que la porción de la válvula (100, 300) posicionada entre el catéter(326) de suministro alargado y la funda (330) replegable es el miembro (110, 310) de transición de hojuela y una 5 porción de la hojuela de válvula (104, 304).
- 11.- El sistema (324) de las reivindicaciones 9-10, en el que la funda (330) replegable restringe al miembro (110, 310) de transición de hojuela en la primera posición.10 12.- El sistema (324) de las reivindicaciones 9-11, que incluye además un número de miembros (344) de empuje alargados, acoplados liberablemente al miembro (110, 310) de transición de hojuela, en el que los miembros (344) de empuje alargados están dispuestos para la transición del miembro (110, 310) de transición de hojuela y de la hojuela de válvula (104, 304) desde la primera posición hasta la segunda posición.15 13.- El sistema (324) de las reivindicaciones 9-12, en el que el miembro (110, 310) de transición de hojuela tiene forma de serpentín en la primera posición con el fin de expandir el miembro (110, 310) de transición de hojuela desde un primer diámetro hasta un segundo diámetro.
- 14.- El sistema (324) de las reivindicaciones 9-13, en el que la hojuela de válvula (104, 304) incluye una estructura 20 (111, 311) de hojuela acoplada a una porción del borde (122, 322) periférico de la hojuela de válvula (104, 304).
- 15.- El sistema (324) de la reivindicación 14, en el que la estructura (111, 311) de hojuela está abisagrada a una porción de la estructura de válvula (102, 302) para acoplar la estructura de válvula (102, 302) y la hojuela de válvula (104, 304).
