Abstract

Description
Aparato para la reparación de válvulas del
sistema circulatorio.
La presente invención se refiere en general al
campo de la reparación de válvulas del sistema circulatorio. Más
particularmente, la presente invención se refiere a un aparato útil
en el campo de la reparación de válvulas cardiacas y
específicamente para la reparación de válvulas cardiacas mitrales,
para pacientes que padecen regurgitación mitral.
Existen cuatro válvulas en el corazón que sirven
para dirigir el flujo de sangre a través de los dos lados del
corazón en una dirección de avance. En el lado izquierdo, las
válvulas mitral y aórtica dirigen sangre oxigenada proveniente de
los pulmones, a través del lado izquierdo del corazón, hacia la
aorta para su distribución en el cuerpo. En el lado derecho, la
válvula tricúspide, situada entre la aurícula derecha y el
ventrículo derecho, y la válvula pulmonar, situada entre el
ventrículo derecho y la arteria pulmonar, dirigen sangre
desoxigenada proveniente del cuerpo, a través del lado derecho del
corazón, hacia la arteria pulmonar para su distribución en los
pulmones. La anatomía del corazón y la estructura y la terminología
de las válvulas cardiacas se describen e ilustran de forma
detallada en numerosas obras de referencia sobre anatomía y cirugía
cardiaca, incluyendo textos típicos tales como Surgery of the
Chest (Sabiston and Spencer, eds., Saunders Publ., Filadelfia)
y Cardiac Surgery de Kirklin y Barrett-Boyes,
Pathology and Abnormalities of Heart Valves.
Las cuatro válvulas cardiacas son todas ellas
estructuras pasivas por cuanto no gastan ninguna energía por sí
mismas y no realizan ninguna función contráctil activa. Constan de
"valvas" móviles que están diseñadas simplemente para abrirse
y cerrarse en respuesta a presiones diferenciales a cada lado de la
válvula. La válvula mitral tiene dos valvas y la válvula tricúspide
tiene tres. A las válvulas aórtica y pulmonar se les hace
referencia como "válvulas semilunares" debido al aspecto
exclusivo de sus valvas, a las que se denomina muy acertadamente
"cúspides" y tienen una forma algo similar a una media luna.
Los componentes del conjunto de válvula mitral incluyen el anillo
de la válvula mitral; la valva anterior; la valva posterior; dos
músculos papilares que están fijados por sus bases a la superficie
interior de la pared ventricular izquierda; y múltiples cuerdas
tendinosas, que acoplan las valvas de la válvula mitral a los
músculos papilares.
Los problemas que se pueden producir con las
válvulas se pueden clasificar en dos categorías: (1) estenosis, en
la que una válvula no se abre correctamente, o (2) insuficiencia, o
regurgitación, en la que una válvula no se cierra
correctamente.
La regurgitación mitral ("MR") viene
provocada por una disfunción del aparato subvalvular mitral o una
lesión directa en las valvas de la válvula. Múltiples etiologías
pueden conducir a una regurgitación mitral, siendo responsables de
cerca del 60% de los casos la degeneración mixomatosa de la válvula
y la enfermedad cardiaca isquémica. La reparación de la válvula
afectada requiere cirugía mayor en derivación cardiopulmonar para
permitir acceso a la válvula. Consecuentemente, algunos pacientes en
las primeras o últimas fases de la enfermedad no se consideran
candidatos apropiados debido al alto riesgo asociado a la operación.
Múltiples estudios han demostrado que la sustitución protésica de
la válvula mitral puede conducir a una disfunción ventricular
izquierda postoperatoria y con frecuencia requiere un tratamiento
con anticoagulantes de por vida. La reparación de la válvula
mitral, usando un anillo de anuloplastia posterior, ha demostrado
resultados mejorados con una mejor recuperación ventricular. Sin
embargo, estudios recientes realizados por los inventores (Umana et
al., Surg Forum 1997) han revelado que la anuloplastia de anillo
posterior provoca cambios en la geometría ventricular que conducen
a un movimiento paradójico de los músculos papilares normales,
deteriorando adicionalmente el rendimiento ventricular. En cambio,
la reparación en "pajarita" ("bow-tie") en
la que las valvas anterior y posterior de la válvula mitral se
fijan en oposición parece potenciar la contractilidad anular al
mismo tiempo que mantiene la arquitectura ventricular. Esto ha dado
como resultado una función ventricular postoperatoria mejorada casi
uniformemente.
La presente invención afronta las necesidades de
todos los pacientes con regurgitación mitral sin estenosis mitral,
incluyendo aquellos que, hasta el momento, pueden haber sido
excluidos debido a presentar una MR solamente moderada o a estar
demasiado enfermos como para ser candidatos para una cirugía
mayor.
La presente invención encuentra utilidad no
solamente para la reparación de válvulas mitrales sino para todas
las válvulas del sistema circulatorio, incluyendo válvulas aórticas,
válvulas tricúspide, y válvulas venosas.
Las técnicas para mejorar la eficacia de las
válvulas del cuerpo son conocidas. Por ejemplo, Laufer et al.,
patente U.S. n.º 5.609.598, describe un sistema de válvulas para el
tratamiento de la insuficiencia venosa crónica. El sistema presenta
limitaciones inherentes en términos de su efectividad para el
procedimiento descrito y su aplicabilidad, en caso de que la
hubiera, en otras válvulas, especialmente válvulas cardiacas.
El documento US 5.571.215 describe dispositivos
y métodos para realizar procedimientos quirúrgicos dentro de un
órgano o vaso.
El documento US 5.571.137 da a conocer un
sistema de instrumentos endoscópicos que incluye una parte de mango
con una empuñadura de tijera, un vástago que se extiende desde la
empuñadura de tijera y con una forma tal para extenderse a través
de una cánula, y una pluralidad de herramientas extremas
desechables, montables cada una de ellas en el extremo del vástago
accionable mediante el mango de tijera.
El documento US 5.403.326 describe una grapadora
médica o quirúrgica que comprende un cuerpo alargado que tiene un
conducto el cual está adaptado para recibir un componente médico tal
como un fórceps o fluido.
El documento US 5.904.153 se refiere a un
instrumento quirúrgico que comprende un miembro de fórceps flexible
alargado que tiene un diámetro suficientemente pequeño de manera que
el miembro de fórceps es insertable de forma deslizable en un canal
de biopsia que se extiende longitudinalmente a través de un miembro
endoscópico tubular flexible, estando provisto el miembro de
fórceps, en un extremo distal, de un par de mordazas de fórceps
formadas cada una de ellas sobre una superficie encarada hacia
dentro con una ranura que se extiende longitudinalmente.
El documento US 4.809.695 da a conocer un
conjunto suturador de tejidos y un método correspondiente que
conlleva grapas de sutura en forma de C incorporadas en un cartucho
formado por un soporte y mordazas de cartucho opuestas.
El documento US 5.542.949 da a conocer un
instrumento multifuncional aplicador de clips que incluye un cuerpo
de instrumento alargado que tiene un extremo proximal recibido en un
receptáculo de control, un extremo distal que tiene uno o dos pares
de mordazas de fórceps opuestas en separación por la acción de un
resorte, una pluralidad de clips quirúrgicos dispuestos en uno o dos
cargadores de clips insertados en un cartucho de clip recibido de
forma extraíble en el cuerpo del instrumento y un mango que coopera
con un receptáculo de control para hacer funcionar las mordazas del
fórceps con el fin de aplicar clips al tejido, coger y manipular
tejido cuando no hay ningún clip en las mordazas, hacer avanzar
clips de forma selectiva o automática hacia las mordazas y cortar
tejido de forma selectiva o automática después de la fijación de los
clips.
El documento US 3.378.010 se refiere a un clip
quirúrgico autoliberable adaptado para quedar retenido dentro del
cuerpo durante un periodo postoperatorio, que comprende mordazas
mutuamente opuestas, medios que interconectan las mordazas para
transmitir presión de sujeción entre ellas cuando las mordazas se
cierran sobre tejido corporal tras la instalación del clip,
comprendiendo dichas mordazas un material que queda expuesto a
fluidos corporales al producirse la instalación del clip y que es
relativamente resistente a la disolución en dichos fluidos
corporales, e incluyendo dichos medios de interconexión una parte
que comprende un material que queda expuesto a fluidos corporales
al producirse la instalación de un clip, siendo este último material
relativamente disoluble en dichos fluidos corporales.
El documento WO 96/21489 da a conocer un catéter
retrógrado de aplicación que incluye en su extremo distal un balón
configurado para ocluir el seno coronario del corazón de un
paciente, y tiene una longitud y una flexibilidad que permiten
posicionar el extremo distal en el seno coronario con el extremo
proximal extendiéndose transluminal a una vena periférica tal como
una vena yugular interna y fuera del cuerpo a través de una punción
en la misma.
El documento WO 96/26672 da a conocer un catéter
de biopsia dirigible para obtener una muestra de tejido de una
cavidad corporal de un paciente, que comprende un vástago de catéter
alargado, un alambre de dirección, un par de mordazas, y un alambre
de accionamiento.
La presente invención trata sobre un aparato
según la reivindicación 1. El aparato es útil en la reparación de
válvulas cardiacas que conlleve el uso de un dispositivo insertado o
dispositivo de agarre para sujetar y mantener juntas las valvas
anterior y posterior de la válvula, mediante inserción en el
ventrículo izquierdo a través del tórax derecho por medio de un
toracoscopio, a través de la vena yugular, o a través de la arteria
femoral. El dispositivo de agarre sujetará ambas valvas,
preferentemente después de que el corazón se haya detenido o
ralentizado farmacológicamente. La corrección del agarre inicial se
valora, por ejemplo, mediante ecocardiografía intraoperatoria, para
garantizar, por ejemplo, en el caso de la válvula mitral, que se
resuelve la regurgitación mitral. Si no, el dispositivo de agarre
podrá "ajustar" las valvas para permitir una mejor coaptación
o, si fuera necesario, volver a sujetar las valvas en una ubicación
diferente.
Inherente al dispositivo de agarre, en forma de
un componente fijado de modo enterizo, se introduce un dispositivo
de fijación y se despliega un fijador para mantener en posición de
forma segura las valvas después de que se haya liberado el
dispositivo de agarre. A continuación se retira la parte restante
del dispositivo.
Los dispositivos accesorios necesarios para el
procedimiento incluyen instrumentos para abordajes toracoscópicos o
percutáneos. Aunque el método y el aparato preferidos descritos a
continuación en el presente documento se describen en referencia a
su uso con relación a la reparación de válvulas mitrales, se
contempla que el mismo aparato y la misma metodología u otros
sustancialmente similares también resultarían útiles en la
reparación de otras válvulas que se hallan en los sistemas
circulatorios humanos, particularmente otras válvulas cardiacas,
tales como, por ejemplo, válvulas venosas, válvulas aórticas y
válvulas tricúspide, entre otras.
Es un objetivo de la invención proporcionar un
aparato útil en un método para la reparación de válvulas cardiacas
con el fin de incrementar su eficacia.
Es otro objetivo de la invención proporcionar un
aparato útil en un método para la reparación de válvulas mitrales
con el fin de reducir la regurgitación mitral.
Es también un objetivo de la invención
proporcionar un aparato útil en un método para la reparación de las
válvulas mitrales el cual elimina la necesidad de cirugía de
derivación cardiopulmonar.
Es otro objetivo de la invención proporcionar un
aparato para su inserción percutánea en el corazón con el fin de
efectuar la reparación de una válvula cardiaca.
Es todavía otro objetivo de la invención
proporcionar un aparato para la reparación de una válvula mitral
mediante inserción percutánea de un dispositivo de agarre y fijación
en el corazón con el fin de reparar una válvula mitral y reducir o
eliminar la regurgitación mitral.
Estos y otros objetivos de la invención
resultarán evidentes para los expertos en la materia a partir de la
descripción más detallada que se proporciona a continuación.

La invención se puede apreciar quizás mejor
haciendo referencia a los dibujos. En la Fig. 1, se representa una
parte del corazón humano que muestra una válvula mitral 10, un
ventrículo izquierdo 12 y el extremo distal 14 de un aparato 16 de
agarre, que se ha insertado a través de una incisión 18 en el
ventrículo izquierdo 12. La incisión 18 se sutura holgadamente con
suturas 20 para sujetar de forma holgada el extremo distal 18 y para
evitar una hemorragia.
La válvula mitral 10 comprende la valva o
cúspide anterior 22 y la valva o cúspide posterior 24, así como dos
cúspides comisurales (no mostradas). La intención principal de la
invención en el presente documento es afianzar juntas o de forma
sustancialmente adyacente las secciones distales 26 y 28 de las
cúspides 22 y 24, respectivamente.
Tal como puede observarse en la Fig. 2, las
mordazas 30 del extremo distal 14 se separan y se posicionan de
forma exterior con respecto a las cúspides 22 y 24. A continuación,
tal como se muestra en la Fig. 3, las mordazas 30 se cierran
conjuntamente para conseguir que las secciones distales 26 y 28 de
las cúspides se aproximen entre sí. Una vez que se ha insertado un
cierre, tal como el cierre 32 de bucle de la Fig. 4, las mordazas
30 se abren ligeramente de manera que la sección distal 14 se pueda
retirar.
Los extremos distales de los medios de agarre
pueden variar notablemente. Se contempla que se pueda utilizar una
variedad de medios de agarre que presenten diferentes
configuraciones y elementos de agarre. Por ejemplo, se contempla
que los medios de agarre puedan ser del tipo en el que un lado del
dispositivo de agarre es fijo y el otro lado móvil.
Alternativamente, los medios de agarre podrían ser del tipo en el
que ambos lados son móviles en conjunción. Otra disposición
alternativa comprende unos medios de agarre que tienen múltiples
elementos de agarre para posibilitar que las valvas de la válvula
sean agarradas y sujetadas en múltiples posiciones. Se contempla
también que los propios elementos de agarre puedan comprender uno o
más elementos de succión para afianzar y mantener en posición las
valvas de las válvulas. Preferentemente, el dispositivo de agarre
tendrá la capacidad de ajustar las valvas de, por ejemplo, una
válvula mitral para obtener una coaptación óptima.
Adicionalmente, se contempla que el dispositivo
de agarre pueda comprender tecnología adicional para facilitar el
funcionamiento del dispositivo de agarre. Por ejemplo, el
dispositivo de agarre puede tener una sonda
eco-doppler o una tecnología de visualización
similar que permitiría una localización todavía mejor de las valvas
y una confirmación de una coaptación ideal.
La Fig. 5 representa el extremo 36 de agarre de
un aparato percutáneo 38 con mordazas 40 en la posición de
abertura. Las mordazas 40 del extremo 36 de agarre están acopladas
de forma móvil con respecto a la articulación 42 de tal manera que
las mordazas se pueden ser abiertas o cerradas de forma sencilla y
sin restricciones por el técnico del aparato percutáneo.
En la Fig. 6 se representa una posible
configuración de un soporte 44 de fijación con un clip 46 de
fijación en funcionamiento, mantenido en la posición abierta para su
colocación sobre las valvas agarradas de una válvula mitral. El
soporte 44 de fijación y el clip 46 de fijación pueden ser enterizos
con un extremo del dispositivo de agarre según se muestra en la
Fig. 5 ó pueden ser independientes del mismo, en cuyo caso será
necesario proporcionar además unos medios percutáneos secundarios
para ser usados en la aplicación y la manipulación del soporte 44
de fijación y la liberación y fijación del clip 46 de fijación en la
posición correcta en torno a las hojas de una válvula mitral, una
vez que las mismas hayan sido agarradas correctamente por mordazas
40 del extremo 36 de agarre.
La Fig. 7 es una representación esquemática más
detallada del soporte 44 de fijación con sus mordazas 48 en su
posición abierta y el clip 46 de fijación en su posición, abierto
(línea de trazos). Se muestra también el clip 46 de fijación en su
posición de cierre, liberada. El clip 46 de fijación, que puede
tener un diámetro cerrado de entre aproximadamente 3 y 7 mm,
preferentemente de forma aproximada 5 mm, estará compuesto por un
material adecuado tal como acero inoxidable, nitinol, o titanio.
La Fig. 8 muestra una representación esquemática
detallada en sección transversal parcial de una realización
preferida del dispositivo de agarre de la presente invención, que
comprende el extremo 50 de agarre, mordazas móviles 52 que se
acoplan de forma móvil con respecto a la articulación 54, en la
posición abierta, en proximidad a las valvas 56 de la válvula. Cada
mordaza 52 tiene una superficie saliente 58 de agarre. No obstante,
la superficie 58 de agarre de una mordaza 52 está conectada de
forma operativa y deslizable a un miembro 60 de control para
posibilitar la alineación correcta de las valvas 56 de la válvula,
antes de la fijación.
En la Fig. 9, el dispositivo de agarre del
aparato de la invención mostrado en la Fig. 8 está en una posición
de cierre. Las mordazas móviles 52 tienen superficies salientes 58
de agarre, que se acoplan a valvas 56 de la válvula. Las valvas 56
se desplazan a una posición más óptima para su fijación mediante la
acción del miembro 60 de control sobre una de las superficies
salientes 58 de agarre, tal como se muestra en la Fig. 11. Además,
la barra 68 de accionamiento de la grapadora está conectada en este
momento de forma operativa al miembro 70 de control de la
grapadora.
La Fig. 10 es una representación esquemática de
una sección transversal del dispositivo de agarre ajustable
representado en la Fig. 9. Las mordazas comprenden superficies 58 de
agarre, un yunque superior 62 con rebaje 71, y un yunque inferior
64 dentro del cual está situado un fijador 66 de tipo grapa para
efectuar la fijación de las valvas de la válvula.
Tal como se muestra en las Figs. 9, 11 y 12, el
yunque inferior 64 tiene por lo menos un miembro 72 de superficie
inclinada. Cuando se obliga a que la barra 68 de accionamiento de la
grapadora se mueva distalmente contra el miembro 72 de superficie
inclinada, el fijador 66 de grapadora es obligado a desplazarse a
través de las valvas 56 hacia el yunque superior 62 para cerrar el
fijador 66 de grapadora.
En otra realización de la invención mostrada en
la Fig. 13, un dispositivo 80 de agarre comprende mordazas 82, 84.
La mordaza 82 está conectada de forma móvil a la barra 86 por el
punto 87 de pivotamiento, y la mordaza 84 está conectada de forma
móvil por el punto 88 de pivotamiento a la barra 90. La barra 92
está conectada de forma móvil a la mordaza 84 por el pivote 94. El
funcionamiento de las barras 90 y 92 provoca que las mordazas 82 y
84 se abran y cierran sobre las valvas 96 de la válvula. Axialmente
con respecto al dispositivo 80 de agarre se encuentra una vaina 98
que contiene un mecanismo 100 de accionamiento para hacer girar el
fijador helicoidal 102. El fijador helicoidal 102 avanza a modo de
espiral perforando valvas 96 en múltiples ubicaciones a medida que
la espiral 102 se hace avanzar hacia su posición final.
Las barras 86, 90, y 92 están conectadas
operativamente cada una de ellas a uno o más mecanismos de control
(no mostrados). Además, las mordazas 82, 84 de las secciones
distales pueden ser deslizables dentro de la vaina 81 del
dispositivo de agarre.
En las Figs. 14 a 16 se muestra otro dispositivo
110 de la invención, en el que las mordazas 112 están conectadas
operativamente a un mecanismo de mango (no mostrado). El dispositivo
110 comprende una vaina móvil 114 que contiene un fijador 116 de
cierre enderezado que es capaz de recuperar o adoptar una forma
circular para producir una coaptación de las valvas de la válvula
(no mostradas). El dispositivo 110 tiene una superficie 118 de
agarre de extrusión deslizable que está conectada operativamente al
mecanismo de mango.
Una vez que las mordazas 112 se han cerrado, la
punta distal de la vaina 114 se hace avanzar distalmente para
quedar contigua a la superficie 118 de agarre y su superficie 122 de
agarre, cooperante. Un empujador 124 obliga al fijador 116 a
avanzar fuera del extremo distal 126 de la vaina 114 para adoptar
una forma circular. El fijador 116, con esta forma, producirá una
cooptación de las valvas 120 de la válvula, tal como puede
observarse en la Fig. 17.
El dispositivo 130 de la invención mostrado en
las Figs. 18 y 19 está destinado a formar un dispositivo de cierre
de tres piezas. Las mordazas 132 contienen, cada una de ellas, de
forma extraíble, un miembro 134 de cierre que tiene una superficie
136 de agarre. Ubicada axialmente con el dispositivo 130 se
encuentra una crimpadora 138 de cierre que está fijada de forma
extraíble en el extremo distal 140 de una barra 142 del
dispositivo. Cuando las mordazas 132 agarran las valvas 144 de la
válvula, la crimpadora 138 de cierre se hace avanzar distalmente
por medio de la barra 142 del dispositivo para encajar sobre los
extremos proximales de los miembros 134 de cierre. El cierre
formado se muestra en la Fig. 20.
Aunque una configuración típica de los medios de
agarre requeriría normalmente el uso de por lo menos un alambre de
control para accionar el(los) elemento(s) de agarre,
se contempla que también se podrían utilizar y manipular
eficazmente múltiples alambres de control independientes, desde el
extremo proximal del sistema para permitir el control preciso de
los elementos individuales de agarre.
Con respecto a los medios de fijación
utilizados, tal como se ha indicado anteriormente los medios de
fijación están constituidos en forma de un único aparato que
funciona en conjunción con los medios de agarre.
Aunque el fijador preferido representado está en
forma de un clip o grapa, se contempla también que los fijadores
utilizados para afianzar las valvas de la válvula puedan presentar
una variedad de diferentes configuraciones, cada una de las cuales
funcionaría con una eficacia mayor o menor dependiendo de las
condiciones de funcionamiento que predominasen. Además de los clips
o grapas, se contempla también que se puedan utilizar eficazmente
asimismo los siguientes tipos de fijadores: espirales, suturas,
fijadores de doble botón, fijadores de tipo gemelo, y
similares.
Fijadores de sutura adecuados incluirían
aquellos que puedan requerir un mecanismo apropiado para suturar
automáticamente tejido. Los fijadores helicoidales estarían
provistos en general de extremos afilados para permitir atornillar
estos fijadores en su posición enroscando el extremo afilado a
través del tejido de la valva de la válvula.
En referencia a las Figs. 21 a 23 que muestran
una representación secuencial del cierre de valvas de la válvula
usando unos medios de cierre preferidos, en la Fig. 22 se muestra un
cierre 150 de tipo clip que está siendo insertado a través de las
valvas 152 de la válvula. La Fig. 22 muestra el cierre 150 de tipo
clip en la posición fijada. La Fig. 23 es una vista en sección
transversal del cierre 150 de tipo clip representado en la Fig. 23.
Cada cierre 150 según se muestra en la Fig. 21 tendría un grosor de
entre aproximadamente 0,5 y 1,8 mm, de forma preferente
aproximadamente 1 mm, una anchura de entre aproximadamente 0,3 y 0,7
cm, de forma preferente aproximadamente 0,5 cm, y una longitud de
entre aproximadamente 0,6 y 1,4 cm, de forma preferente
aproximadamente 1 cm.
Las Figs. 24 y 25 son cada una de ellas una
representación esquemática de la inserción de otros medios de
cierre preferidos de la invención. Se inserta un cierre 156 de tipo
grapa a través de las valvas 158 de la válvula, y a continuación el
mismo se cierra, tal como se muestra en la Fig. 26. El cierre 156
tendría preferentemente una longitud total (incluyendo los lados) de
entre aproximadamente 1 y 4 cm, de forma preferente aproximadamente
3 cm, un diámetro efectivo de entre aproximadamente 0,1 y 0,5 mm, de
forma preferente aproximadamente 0,3 mm, y una abertura de entre
aproximadamente 0,5 y 1,3 cm, de forma preferente aproximadamente 1
cm.
Las Figs. 26 a 28 son cada una de ellas una
representación esquemática de la inserción de todavía otro cierre
preferido. Un cierre helicoidal espiral 160 se puede insertar a
través de las valvas 162 de la válvula de forma longitudinal,
latitudinal o transversal, mediante el uso, por ejemplo, del
dispositivo mostrado en la Fig. 13. Las espirales 160 tendrán
preferentemente extremos en punta y tendrán dimensiones externas que
comprenden una longitud de entre aproximadamente 3 y 7 cm, de forma
preferente aproximadamente 5 cm, y un diámetro de entre
aproximadamente 1 y 3 mm, de forma preferente aproximadamente 2
mm.
El diámetro total y/o las vueltas diferenciales
de la espiral 160 pueden ser uniformes o pueden variar. Por
ejemplo, el diámetro en cada extremo de la espiral 160 podría ser
igual, mayor, o menor que el diámetro de la parte central de la
espiral. De modo similar, la relación de las vueltas de la espiral
con respecto a la longitud, es decir, el paso, podría ser constante
o el paso podría ser mayor o menor en cada extremo de la espiral.
El diámetro del alambre de la espiral será preferentemente
constante.
Cada espiral 160 tendría una longitud de entre
aproximadamente 3 y 7 cm, de forma preferente aproximadamente 5 cm,
con un diámetro de entre aproximadamente 1 y 3 mm, de forma
preferente aproximadamente 2 mm, y un diámetro del alambre de la
espiral de entre aproximadamente 0,2 y 0,4 mm. El devanado de la
espiral 160 debería ser de entre aproximadamente 5 y 10 vueltas/cm
en una condición no sometida a tensiones.
En las Figs. 29 y 30 un cierre 164 de tipo clip,
dentado, en forma de U, se aplica a la valva 166.
El dispositivo y los fijadores usados deben
estar compuestos por materiales biocompatibles, no inmunógenos. El
dispositivo de agarre está dispuesto preferentemente por materiales
rígidos tales como titanio, nitinol, acero inoxidable, o material
polimérico rígido tal como polietileno o poliuretano. Los clips,
grapas, espirales, etcétera, están compuestos preferentemente por
titanio, nitinol, o acero inoxidable. En algunos casos, también
pueden resultar útiles fijadores compuestos por material polimérico
moldeado.
Existen cuatro abordajes diferentes que se
pueden adoptar para efectuar una reparación de la válvula cardiaca
mitral usando el dispositivo según la invención:
Se podría emprender un procedimiento de este
tipo mientras el paciente está en derivación con el tórax abierto,
de forma o bien transapical o bien transauricular. Se realiza una
esternotomía media y el paciente se sitúa en derivación
cardiopulmonar canulando la aorta ascendente y la aurícula derecha.
A continuación, se aplica una sutura en bolsa de tabaco en el
vértice del ventrículo izquierdo y se realiza una incisión cortante
para insertar el instrumento que agarrará y fijará las valvas de la
válvula mitral. Una vez se ha obtenido una reparación adecuada de
la válvula, el instrumento se retira y el aire se evacúa del
ventrículo izquierdo a través de la incisión apical. A
continuación, el ventrículo se repara usando técnicas convencionales
de cierre de heridas.
Alternativamente, el dispositivo de agarre se
puede introducir a través de una incisión cortante similar
realizada sobre el techo de la aurícula izquierda. El dispositivo de
agarre atravesará la válvula y a continuación se manipulará para
darse la vuelta con el fin de agarrar las valvas desde el lado
auricular y aplicar el dispositivo suturador, exactamente tal como
se postula a partir del abordaje transventricular. Una vez que se ha
confirmado la adecuación de la reparación, el dispositivo se extrae
y la atriotomía se cierra usando técnicas convencionales de cierre
de heridas.
Este procedimiento se puede realizar
alternativamente con el paciente sin derivación, a través de o bien
una toracotomía izquierda o derecha o bien una incisión de
esternotomía. La técnica sería similar a la expuesta en líneas
generales para la reparación de la regurgitación mitral con
derivación cardiopulmonar. Después de abrir el tórax, el paciente
se pone bajo medicación (betabloqueante) para ralentizar el ritmo
cardiaco aproximadamente 40 latidos por minuto. Esto permite una
visualización ecocardiográfica adecuada de las valvas con el fin de
agarrarlas y fijarlas.
En tercer lugar, un procedimiento de este tipo
se puede emprender de forma toracoscópica. El paciente se intuba
selectivamente para colapsar el pulmón izquierdo, y se insertan
puertos percutáneos en el tórax izquierdo que permiten la
visualización del vértice del corazón o la aurícula izquierda. A
través de un puerto aparte, el dispositivo se introduce en la
cavidad torácica y posteriormente en el ventrículo izquierdo a
través del vértice. Previamente, una sutura en bolsa de tabaco o
triangular se había aplicado en torno a la punta del ventrículo
para controlar la hemorragia alrededor del sitio de entrada
ventricular. Las etapas posteriores de la reparación son idénticas
a las descritas para pacientes con tórax abierto, sin
derivación.
En el caso de que la operación requiriese que el
paciente se situase en derivación, esta se puede obtener
percutáneamente desde la ingle canulando la arteria y vena
femorales. Esta técnica podría resultar particularmente útil en las
primeras fases de desarrollo de la técnica, ya que el cirujano
podría operar sobre un corazón descomprimido y ralentizar o detener
el ritmo cardiaco según fuera necesario, sin ningún compromiso
hemodinámico.
Por último, con esta invención sería posible un
abordaje percutáneo para reparar la válvula mitral insertando el
dispositivo a través de o bien la arteria femoral o bien la vena
yugular. Cuando se use la primera, se llega al ventrículo izquierdo
aplicando el dispositivo a través de la válvula aórtica. Las valvas
se agarrarán girando la punta del instrumento aproximadamente 160º
con respecto al ángulo de entrada. Tal como se ha mencionado
previamente, las puntas del dispositivo de agarre se ajustan para
obtener una aposición óptima y también se aplica el dispositivo
suturador. Si se utiliza un abordaje transvenoso, se entra en la
aurícula izquierda a través del septo interauricular y las valvas
se manipulan tal como se describe para la técnica
transauricular.
Para determinar la eficacia relativa del método
descrito en la obtención de la reparación de válvulas cardiacas,
tales como las válvulas mitrales, se realizaron varios
procedimientos sobre sujetos de estudio tanto animales como
humanos, de la manera siguiente:
Seis ovejas adultas fueron sometidas a ligadura
de OM2 y OM3 a través de una toracotomía izquierda para inducir MR
isquémica crónica. Después de 8 semanas, los animales se situaron
bajo derivación cardiopulmonar. Usando un abordaje posterior para
la aurícula izquierda, se realizó una reparación en pajarita. Como
control sirvió una anuloplastia de sutura posterior (DeVega). En
ambas reparaciones se aplicaron pinzas (snares) para
permitir un apriete alternado durante las mediciones. Diez cristales
piezoeléctricos de 2 mm se suturaron alrededor del anillo de la MV
y en las bases y puntas de los músculos papilares. Seis cristales se
afianzaron en el vértice (1), el septo (1), y el eje corto
epicárdico del ventrículo izquierdo (4) para una obtención de
imágenes por localización mediante matriz sonomicrométrica de 3
dimensiones (3D-SAL,
3-dimensional sonomicrometry array
localization). Se realizaron mediciones de la
3D-SAL después de retirar la derivación
cardiopulmonar en la línea basal y con cada tipo de reparación. Se
usó una ecocardiografía para medir la MR, el área de la MV, y la
fracción de acortamiento.
Tal como se muestra a partir de los resultados
presentados en la Tabla 1, la MR se redujo significativamente con
ambas reparaciones en comparación con la línea basal. Las mejoras
postoperatorias en la fracción de acortamiento fueron mayores en el
grupo de la pajarita pero no alcanzaron una significación
estadística. La MVA, medida por planimetría, se redujo más con la
reparación en pajarita; sin embargo, las áreas resultantes seguían
siendo sustanciales sin evidencias de un gradiente transvalvular.
La contractilidad anular de la válvula mitral (% cambio de área =
(área máxima - área mínima) / área máxima) por
3D-SAL aumentó desde 19,7% \pm 4,0% en la línea
basal hasta 21,5% \pm 3,2% después de la reparación en pajarita (P
= 0,026). La anuloplastia de sutura redujo la contractilidad anular
al 15,7% \pm 3,6% (P = 0,0011 con respecto a línea basal, y
P = 0,0001 con respecto a pajarita).
Los resultados obtenidos sugieren que las
técnicas actuales de reparación de válvulas mitrales en MR
isquémica pueden perjudicar adicionalmente el rendimiento
ventricular izquierdo al limitar la función sistólica del anillo y
la base del corazón. La técnica de reparación en pajarita, que es el
objeto de la presente invención, controla la MR y hace frente
directamente a la disfunción subvalvular dando como resultado una
función ventricular izquierda y anular mejorada.
Se revisaron los expedientes médicos de once
pacientes (cinco varones y seis mujeres) que fueron sometidos a
reparación valvular mitral conjuntamente con una sutura valvar
central (reparación en "pajarita"). Los pacientes se operaron
entre agosto de 1996 y abril de 1997. La edad media fue 68 años
(intervalo, 44 a 78). La etiología de la regurgitación mitral (MR)
era isquémica en nueve pacientes y degenerativa en dos. La
regurgitación mitral se atribuyó a la isquemia si se cumplía
cualquiera de los siguientes criterios propuestos por Radford et
al.: (1) rotura de una cuerda o cabeza del músculo papilar
(n=3); (2) infarto del músculo papilar en ausencia de patología
valvar (n=3); (3) historial limpio de comienzo reciente o
empeoramiento de regurgitación mitral después de infarto de
miocardio documentado (n=3).
El diagnóstico de MR se estableció por
ecocardiografía en 10/10 pacientes, y se clasificó
semicuantitativamente como severa (4+), moderada/severa (3+),
leve/moderada (2+), leve (1+), y tenue. El cateterismo cardiaco del
lado izquierdo confirmó la presencia de MR en nueve pacientes y la
presencia de enfermedad arterial coronaria (CAD) crítica que
involucraba invariablemente los territorios de la arteria
circunfleja y descendente posterior en todos los pacientes con MR
isquémica. En la Tabla 2 se muestran los diagnósticos preoperatorios
y la hemodinámica obtenidos durante el cateterismo. Todos los
pacientes se situaban en la clase III ó IV de la NYHA en el momento
de la cirugía.

Con el paciente bajo anestesia, se visualiza la
válvula en un ecocardiagrama transesofágico (TEE) y se determina el
modo probable de insuficiencia, poniendo un énfasis especial en la
presencia de prolapso valvar y el sitio y la dirección del chorro
regurgitante. Después de que se detuviera el corazón, se usa una
pera de goma con solución salina fría para distender el ventrículo
izquierdo y confirmar el modo de insuficiencia valvular.
Generalmente se realiza una reparación convencional usando un anillo
de anuloplastia y la válvula se vuelve inspeccionar con inyección
de solución salina. Si los bordes de la valva no se oponen entre sí
en un círculo concéntrico paralelo al anillo de anuloplastia, y se
observa una regurgitación continuada, entonces se inicia una
reparación en "pajarita". Si la reparación se realiza desde la
exposición transventricular o transaórtica, se aplica una única
sutura de prolene 4-0 en ocho sin detectar
coaptación de las valvas. Usando una sutura de prolene
4-0, la valva anterior se fija a la valva posterior
correspondiente en el lugar de la aposición deficiente. La sutura en
ocho se aplica a través de cada valva exactamente cuando el borde
se dobla hacia abajo para fijarse a las cuerdas principales.
Habitualmente, este es el sitio más craneal en el que se tocarían
las 2 valvas durante la sístole y crea la mayor área de coaptación
posible.
En ese momento la sutura está muy próxima a una
comisura y el resultado es un estrechamiento del orificio único de
la válvula. Más comúnmente, la sutura está más próxima al centro de
la válvula y se crea una válvula de orificio doble que recuerda a
una "pajarita". Después de confirmar visualmente que la
reparación es satisfactoria con inyección de solución salina fría,
la aurícula se cierra, al paciente se le retira la CPB, y se usa un
TEE intraoperatorio para confirmar la adecuación de la reparación.
Se realizaron ecocardiagramas transtorácicos convencionales así
como de esfuerzo antes de la descarga para establecer la competencia
de la reparación en "pajarita" así como la ausencia de un
gradiente significativo a través de la válvula.
Seis pacientes se operaron electivamente por
empeoramiento de la MR que condujo a una insuficiencia cardiaca
congestiva resistente al tratamiento o angina inestable. Cuatro
pacientes fueron sometidos a operación urgente debido a un
empeoramiento agudo de la MR secundario a rotura isquémica del
músculo papilar anterior (n=2), MI agudo con shock cardiogénico que
requirió balón de contrapulsación intraaórtico, MR severa y
arritmias malignas (N=1), y empeoramiento agudo de MR degenerativa
crónica (n=1). Un paciente presentó MR moderada (3+) en asociación
con insuficiencia aórtica crítica. El grado medio de MR
preoperatoria por ecografía fue 3,5 \pm 0,7, con una fracción de
eyección (EF) media del 42% \pm 17%. Nueve pacientes fueron
sometidos a cateterismo cardiaco preoperatorio. La presión de
enclavamiento capilar pulmonar media fue 23 mmHg \pm 8 mmHg, con
una onda v auricular media de 39 mmHg \pm 25 mmHg; el CO medio,
medido por técnica de termodilución, fue 3,9 1 l/min (intervalo de
2,4 a 4,5 1 l/min) (Tabla 2). Los procedimientos concomitantes
realizados en el momento de la MR incluyeron injerto de derivación
aortocoronaria (CABG) en ocho pacientes. De los dos pacientes con
una etiología degenerativa de la enfermedad valvular, uno requirió
sustitución de la válvula aórtica, mientras que el segundo fue
sometido a una resección cuadrangular de la valva posterior y a
anuloplastia. Dos pacientes, no incluidos en esta serie, con
insuficiencia cardiaca congestiva (CHF) en fase terminal, secundaria
a dilatación ventricular, tuvieron reparaciones en "pajarita"
durante una ventriculectomía izquierda parcial. Nueve pacientes
tuvieron una anuloplastia de anillo posterior como procedimiento
principal para el tratamiento de la MR (Tabla 3). Un paciente
requirió reparación del defecto septal ventricular (VSD) isquémico a
través de una ventriculotomía, lo cual hizo que no resultase
práctico insertar un anillo de anuloplastia. La válvula mitral de
este paciente se reparó satisfactoriamente solo con una
"pajarita". Un segundo paciente se presentó con MR aguda
secundaria a rotura de la cabeza anterior del ppm. La reparación del
músculo papilar se realizó usando parches pericárdicos. Debido a la
falta de dilatación anular y a la persistencia de la MR, se aplicó
una sutura en "pajarita" sin anillo de anuloplastia. El control
de la MR valorado intraoperatoriamente mediante inyección directa
de solución salina fría y TEE resultó satisfactorio en todos los
pacientes.

Se observará así que los objetivos antes
expuestos, entre los puestos de manifiesto a partir de la
descripción anterior, se obtienen de forma eficaz y, debido a que en

Claims (5)
1. Aparato, que se puede insertar
percutáneamente, para la reparación de una válvula cardiovascular
que tiene valvas, que comprende un dispositivo de agarre capaz de
sujetar y producir la coaptación de las valvas de la válvula, un
dispositivo de fijación desplegable fijado de forma enteriza al
dispositivo de agarre, medios para hacer funcionar el dispositivo
de fijación independientemente de los medios de agarre y por lo
menos un alambre de control capaz de accionar el dispositivo de
agarre.
2. Aparato de la reivindicación 1, en el que el
dispositivo de agarre tiene una mordaza deslizable para ajustar las
valvas de la válvula con el fin de obtener una coaptación
óptima.
3. Aparato de la reivindicación 1, en el que el
aparato es capaz de ser insertado a través de un abordaje
transvenoso.
4. Aparato de la reivindicación 1, en el que el
aparato es capaz de ser insertado a través de la arteria
femoral.
5. Aparato de la reivindicación 1, en el que el
aparato es capaz de ser insertado a través de la vena yugular.
