Description

Funda recargable para un sistema de catéter para el despliegue de dispositivos de oclusión vascular.
Antecedentes de la invención
Esta invención se refiere, en general, a dispositivos para tratamiento terapéutico quirúrgico o cirugía vascular para el tratamiento de anomalías en el sistema vascular y, más particularmente, consiste en un sistema para administrar dispositivos quirúrgicos intravasculares, tales como para el tratamiento de aneurismas.
Los dispositivos quirúrgicos vasculares, tales como los dispositivos vasooclusivos normalmente se colocan dentro del sistema vascular del cuerpo humano mediante el uso de un catéter. Los dispositivos quirúrgicos vasculares, tales como los stents, se pueden colocar dentro de un vaso ocluido para facilitar el flujo de sangre a través del vaso y, normalmente, los dispositivos vasooclusivos o bien se colocan dentro de un vaso sanguíneo para bloquear el flujo de sangre a través de un vaso, preparando esa parte del sistema vascular mediante la formación de una obstrucción, o se colocan dentro de un aneurisma que proviene del vaso para formar una obstrucción de este tipo dentro del aneurisma. Los stents pueden tener una gran variedad de configuraciones, pero en general se deben colocar en una posición deseada, dentro de un vaso sanguíneo, y posteriormente separar de ésta. Los dispositivos vasooclusivos usados para estas intervenciones quirúrgicas también pueden tener una gran variedad de configuraciones y los aneurismas se han tratado con grapas externas colocadas quirúrgicamente, con globos vasooclusivos separables y con dispositivos vasooclusivos generadores de obstrucciones, tales como una o más espirales obstructoras o vasooclusivas.
En el documento FR-A-2.631.835 se describe una funda flexible pensada para facilitar la sustitución de una prótesis vascular según el preámbulo de la reivindicación 1. La funda está formada por una parte de tubo de forma sustancialmente cilíndrica, con un diámetro parecido al de la prótesis que se va a sustituir. Preferentemente, este tubo está cortado sobre su longitud y dispuesto en el interior con un conducto cuya sección transversal es mucho más pequeña que la del tubo para recibir en el interior una varilla flexible usada para guiar las prótesis. La funda se aplica especialmente en la sustitución de sondas y otros catéteres endovasculares.
La administración de dispositivos vasooclusivos normalmente se ha llevado a cabo con una variedad de medios, que incluyen por medio de un catéter en el que un empujador para desplegar el dispositivo empuja el dispositivo a través de una abertura del extremo distal del catéter. Los dispositivos vasooclusivos se pueden producir de tal manera que pasarán a través del lumen de un catéter en forma lineal y adoptarán una forma compleja, respecto a la formada originalmente, una vez desplegados en la zona de interés, tal como un aneurisma.
Normalmente, los dispositivos vasooclusivos separables son espirales obstructoras fijadas a un extremo distal de un elemento de empuje flexible para la administración de espirales obstructoras y se pueden separar, mecánicamente, eléctricamente o por otros medios, del elemento de empuje flexible en la posición objetivo. Las espirales obstructoras separables se pueden administrar hasta la posición objetivo y separarlas si están dimensionadas y colocadas correctamente o se pueden retirar sin separarlas si las espirales no están dimensionadas correctamente, no están colocadas correctamente o si se pierde la posición del microcatéter. Algunos dispositivos vasooclusivos no se vuelven a usar durante una intervención quirúrgica del paciente si se sacan durante la intervención quirúrgica, debido a la incapacidad para volver a cargar el dispositivo dentro del microcatéter. Sería aconsejable proporcionar un sistema para volver a usar los dispositivos vasooclusivos durante una intervención quirúrgica clínica tras sacarlos de un dispositivo de introducción de microcatéteres. La presente invención satisface ésta y otras necesidades.
Resumen de la invención
Brevemente, y en términos generales, la presente invención proporciona una funda introductora para dispositivos vasooclusivos que se puede cargar en un ensamblaje de espiral obstructora vasooclusiva tras sacar el ensamblaje de espiral obstructora vasooclusiva de un microcatéter, para permitir que se vuelva a usar el ensamblaje de espiral obstructora vasooclusiva durante una intervención quirúrgica clínica y para reducir al mínimo la pérdida de un producto aceptable de espiral obstructora vasooclusiva de cualquier otro tipo.
Según la presente invención se proporciona una funda para un dispositivo vasooclusivo terapéutico, comprendiendo la funda: un elemento tubular alargado y hueco que tiene paredes superiores e inferiores opuestas, paredes laterales opuestas y un conducto interior longitudinal, y una ranura longitudinal formada en la pared superior del elemento tubular alargado y que se extiende por toda la longitud o gran parte de la longitud del elemento tubular alargado, teniendo la ranura laterales opuestos con superficies laterales interiores extendiéndose a través de la pared superior del elemento tubular alargado, conduciendo al conducto interior lo que permite introducir el dispositivo vasooclusivo en el conducto interior, caracterizada porque la pared superior del elemento tubular alargado, adyacente a la ranura, tiene una configuración en ángulo en la superficie exterior del elemento tubular alargado y hueco, de tal manera que las superficies en ángulo adyacentes a la ranura forman un ángulo interior. En una forma de realización preferente la configuración en ángulo en la superficie exterior del elemento tubular alargado y hueco comprende superficies exteriores opuestas que forman un ángulo interior, normalmente, de aproximadamente, 110º a 150º.
En otra forma de realización, la pared inferior del elemento tubular alargado y hueco tiene un grosor de, aproximadamente, 0,051 a 0,102 mm (0,002 a 0,004 pulgadas) para permitir que las paredes opuestas de la ranura del elemento tubular alargado y hueco se doblen hacia afuera para permitir que la ranura se abra para recibir el dispositivo vasooclusivo. El elemento tubular alargado y hueco puede estar formado a partir de un material termoplástico, tal como, por ejemplo, polietileno de alta densidad.
En una forma de realización, la funda puede incluir además elementos de ala extendiéndose hacia afuera de la configuración en ángulo en la superficie exterior del elemento tubular alargado y hueco para facilitar la inserción del dispositivo vasooclusivo en la funda. Los elementos de ala de la configuración en ángulo en la superficie exterior del elemento tubular alargado y hueco tienen superficies exteriores opuestas que normalmente forman un ángulo interior de aproximadamente 110º a 150º. En una variación de estas formas de realización, la funda se puede formar en combinación con el dispositivo vasooclusivo, comprendiendo el elemento tubular alargado una longitud sin ninguna ranura acoplada a un segmento del elemento de empuje flexible para facilitar el inicio de la carga del elemento de empuje flexible y de la espiral obstructora en la funda.
Estos y otros aspectos y ventajas de la invención resultarán evidentes gracias a la siguiente descripción detallada y a los dibujos adjuntos, que ilustran, a modo de ejemplo, las características de la invención.
Breve descripción de los dibujos
La Figura 1 es una vista en perspectiva de una primera forma de realización de la funda recargable para un dispositivo vasooclusivo, según la presente invención.
La Fig. 2 es una vista en perspectiva de una segunda forma de realización de la funda recargable para un dispositivo vasooclusivo, según la presente invención.
La Fig. 3 es una vista transversal que ilustra las dimensiones de la funda recargable de la Figura 1.
La Fig. 4 es una vista en corte longitudinal de una tercera forma de realización de la funda recargable en combinación con un dispositivo vasooclusivo acoplado permanentemente a un extremo de la funda recargable, que muestra el dispositivo vasooclusivo sustancialmente descargado de la funda recargable.
La Fig. 5 es una vista en corte longitudinal de la funda recargable de la Fig. 4, que muestra el dispositivo vasooclusivo totalmente cargado en la funda recargable.
Descripción detallada de las formas de realización preferentes
En los dibujos adjuntos se muestra una funda recargable 10 para un dispositivo vasooclusivo terapéutico 12.
El dispositivo vasooclusivo normalmente incluye un ensamblaje de un elemento de empuje flexible 14 y una espiral obstructora 16 (de la que sólo se muestra una parte) acoplada al elemento de empuje flexible, como se ilustra en las Figs. 5 y 6. Por ejemplo, el elemento de empuje flexible puede incluir una fibra óptica alargada que tiene un extremo distal 18 envuelta en un aro tubular 20 de un material con memoria de forma para retener la espiral obstructora en el extremo distal del elemento de empuje flexible. La fibra óptica se puede dimensionar para que sea bastante flexible y se doble lo suficiente como para seguir el lumen interior. Alternativamente, el empujador alargado se puede formar de materiales adecuados para conducir energía, tal como energía de radiofrecuencia, energía magnética, energía eléctrica o energía ultrasónica, por ejemplo, tal como un elemento de metal alargado, o de un tubo térmico, para conducir el calor desde una fuente de calor. Además, el empujador flexible puede comprender una construcción de alambres sólidos o tubulares con acoplamiento a una espiral obstructora para permitir, por ejemplo, el despliegue mediante medios mecánicos o hidráulicos.
La funda recargable está, generalmente, formada de un elemento tubular alargado y hueco 22 que tiene una pared superior 24 y una pared inferior opuesta 26, paredes laterales opuestas 28 y 30 y un conducto interior longitudinal 32. La pared superior del elemento tubular alargado incluye una ranura 34 ó hendidura con laterales interiores opuestos 36 que tienen superficies extendiéndose a través de la pared superior conduciendo al conducto interior, lo que permite introducir el dispositivo vasooclusivo en el conducto interior. La funda recargable puede tener las dimensiones de ejemplo ilustradas en la Fig. 3 y expuestas en la tabla que aparece a continuación. En la tabla, "A" indica una posible dimensión del conducto interior longitudinal (32) y "B" indica una posible dimensión del diámetro exterior de la funda.

Como se observa mejor en las Figs. 1 y 3 la pared superior del elemento tubular alargado, adyacente a la ranura, normalmente tiene una configuración exterior en ángulo o en forma de V en la superficie exterior del entubado, para facilitar la carga del dispositivo vasooclusivo en la funda recargable. La ranura puede, por ejemplo, tener superficies exteriores opuestas formando un ángulo interior de, aproximadamente, 110º a 150º. Normalmente, la pared inferior del entubado tiene un grosor de, aproximadamente, 0,051 a 0,102 mm (0,002 a 0,004 pulgadas) para permitir que los laterales opuestos de la ranura del elemento tubular alargado y hueco se doblen hacia afuera para permitir que la ranura se abra para recibir el dispositivo vasooclusivo. La configuración permite insertar el elemento de empuje flexible en la funda recargable colocando el elemento de empuje flexible y el ensamblaje de espiral obstructora sobre la ranura y ejerciendo una presión deslizante, tal como la que se realiza con el pulgar de una persona, a todo lo largo de la longitud del elemento de empuje flexible y del ensamblaje de espiral obstructora para introducir el elemento de empuje flexible y el ensamblaje de espiral obstructora en la funda. En esta forma de realización, la funda se puede extraer totalmente del elemento de empuje flexible.
En relación con la Fig. 2, en una segunda forma de realización, la presente invención proporciona una funda recargable 10' para un dispositivo vasooclusivo terapéutico 12, que normalmente incluye un ensamblaje de un elemento de empuje flexible 14 y una espiral obstructora 16 descritos anteriormente e ilustrados en las Figs. 4 y 5. La funda recargable 10' está, generalmente, formada de un elemento tubular alargado y hueco 22' que tiene una pared superior 24' y una pared inferior opuesta 26', paredes laterales opuestas 28' y 30' y un conducto interior longitudinal 32'. La pared superior del elemento tubular alargado incluye una ranura 34' con laterales interiores opuestos 36' que tienen superficies extendiéndose a través de la pared superior conduciendo al conducto interior, lo que permite introducir el dispositivo vasooclusivo en el conducto interior.
La pared superior del elemento tubular alargado, adyacente a la ranura, normalmente tiene una configuración exterior en ángulo o en forma de V en la superficie exterior del entubado, para facilitar la carga del dispositivo vasooclusivo en la funda recargable. La ranura puede, por ejemplo, tener superficies exteriores opuestas formando un ángulo interior de, aproximadamente, 110º a 150º. Normalmente, la pared inferior del entubado tiene un grosor de, aproximadamente, 0,051 a 0,102 mm (0,002 a 0,004 pulgadas) para permitir que los laterales opuestos de la ranura del elemento tubular alargado y hueco se doblen hacia afuera para permitir que la ranura se abra para recibir el dispositivo vasooclusivo. En esta forma de realización, las partes de la superficie superior en ángulo de la pared superior del elemento tubular alargado, adyacentes a la ranura, pueden estar formadas como paredes que se extienden hacia afuera o como elementos de ala 38 para facilitar la inserción del elemento de empuje flexible y del ensamblaje de espiral obstructora en la funda ranurada.
En relación con las Figs. 4 y 5, en una variación de la primera forma de realización, que es igualmente aplicable a la segunda forma de realización, la invención proporciona una funda en combinación con un dispositivo vasooclusivo que incluye un ensamblaje del elemento de empuje flexible 14 y una espiral obstructora 16 que está adaptada para insertarla en una parte de un sistema vascular para ocluir una parte del sistema vascular para uso en un tratamiento quirúrgico y en cirugía vascular. La funda se puede mantener acoplada a un segmento 40 del elemento de empuje flexible para facilitar el inicio de la carga del elemento de empuje flexible en la funda. La funda se puede sacar a todo lo largo de la ranura hasta alcanzar el segmento de la funda sin ranura, un segmento de longitud inferior a la longitud útil no usada del empujador cuando está totalmente cargado en el microcatéter, por ejemplo, aproximadamente, 10 cm. Posteriormente, la funda se puede enrollar y dejarla acoplada al extremo del elemento de empuje flexible mientras se carga el elemento de empuje flexible en un microcatéter (no se muestra). Si se extrae el elemento de empuje flexible sin separar la espiral obstructora, la funda se puede volver a cargar en el elemento de empuje flexible empezando en la posición conectada y avanzando hacia un extremo del elemento de empuje flexible. Posteriormente, se puede hacer avanzar la funda sobre la espiral obstructora para permitir que, con posterioridad, la espiral obstructora entre en el microcatéter.
En cada una de las formas de realización anteriores, el elemento tubular alargado que forma la funda recargable, normalmente, está formado a partir de un material termoplástico. El entubado puede estar formado a partir de un material termoplástico, tal como, por ejemplo, un polietileno de alta densidad. Además, también pueden ser adecuados otros materiales poliméricos similares, tales como poliuretano, nailon, poliéter-éter cetona (PEEK), politetrafluoretileno (PTFE), tereftalato de polietileno (PET) y similares.
Resultará evidente gracias a lo anterior que si bien se han ilustrado y descrito formas específicas de la invención, se pueden realizar diversas modificaciones sin apartarse del alcance de la invención. En consecuencia, no se pretende limitar la invención excepto por cuanto se refiere a las reivindicaciones adjuntas.

Claims (9)

1. Una funda (10) para un dispositivo vasooclusivo terapéutico (12), comprendiendo la funda (10):
un elemento tubular alargado y hueco (22) que tiene paredes superiores e inferiores opuestas (24, 26), paredes laterales opuestas (28, 30) y un conducto interior longitudinal (32) y
una ranura longitudinal (34) formada en la pared superior (24) del elemento tubular alargado (22) y que se extiende por toda la longitud o gran parte de la longitud del elemento tubular alargado (22), teniendo la ranura (34) laterales opuestos (36) con superficies laterales interiores extendiéndose a través de la pared superior (24) del elemento tubular alargado (22) conduciendo al conducto interior (32) lo que permite introducir el dispositivo vasooclusivo en el conducto interior, caracterizada porque la pared superior (24) del elemento tubular alargado (22), adyacente a la ranura (34), tiene una configuración en ángulo en la superficie exterior del elemento tubular alargado y hueco (22), de tal manera que las superficies en ángulo, adyacentes a la ranura (34), forman un ángulo interior.
2. La funda de la reivindicación 1, en la que la configuración en ángulo en la superficie exterior del elemento tubular alargado y hueco (22) comprende superficies exteriores opuestas que forman un ángulo interior de, aproximadamente, 110º a 150º.
3. La funda de las reivindicaciones 1 ó 2, en la que la pared inferior (26) del elemento tubular alargado y hueco (22) tiene un grosor de, aproximadamente, 0,051 a 0,102 mm (0,002 a 0,004 pulgadas) para permitir que los laterales opuestos (36) de la ranura (34) del elemento tubular alargado y hueco (22) se doblen hacia afuera para permitir que la ranura (34) se abra para recibir el dispositivo vasooclusivo (12).
4. La funda de cualquiera de las reivindicaciones precedentes, en la que el elemento tubular alargado y hueco (22) está formado a partir de un material termoplástico.
5. La funda de cualquiera de las reivindicaciones precedentes, en la que el elemento tubular alargado y hueco (22) está formado a partir de un polietileno de alta densidad.
6. La funda de cualquiera de las reivindicaciones precedentes, que comprende además elementos de ala (38) extendiéndose hacia afuera desde la configuración en ángulo en la superficie exterior del elemento tubular alargado y hueco (22) para facilitar la inserción del dispositivo vasooclusivo (12) en la funda (10).
7. La funda de la reivindicación 6, en la que los elementos de ala (38) de la configuración en ángulo en la superficie exterior del elemento tubular alargado y hueco (22) tiene superficies exteriores opuestas que forman un ángulo interior de, aproximadamente, 110º a 150º.
8. La funda de cualquiera de las reivindicaciones precedentes, en la que la ranura longitudinal (34) formada en la pared superior (24) del elemento tubular alargado (22) se extiende gran parte de la longitud del elemento tubular alargado (22) dejando, aproximadamente, hasta 10 cm sin ninguna ranura.
