ESTA INVENCION DESCRIBE UN IMPLANTE DE FUSION DE LA ESPINA DORSAL QUE ES AL MENOS CILINDRICO EN PARTE, HECHO DE UN MATERIAL APROPIADO PARA LA IMPLANTACION EN SERES HUMANOS Y QUE TIENE, PREFERIBLE, PERO NO NECESARIAMENTE, UN EXTREMO CERRADO Y UN EXTREMO QUE SE PUEDE CERRAR, DE MANERA QUE UNA CAMARA INTERNA PUEDE LLENARSE Y CONTENER CUALQUIER MATERIAL OSTEOCONDUCTOR, OSTEOINDUCTOR, OSTEOGENICO NATURAL O ARTIFICIAL O CUALQUIER OTRO QUE FACILITE LA FUSION. EL IMPLANTE PARCIALMENTE CILINDRICO, PARTICIPA DIRECTAMENTE Y ES INCORPORADO EN LA FUSION RESULTANTE. FINALMENTE, EL IMPLANTE DE ESTA INVENCION NO REQUIERE ROTACION PARA SU INSERCION Y PUEDE ASENTARSE CON UN AVANCE LINEAL.
Description
Implantes espinales intersomáticos por
fusión.
El presente invento hace referencia a implantes
espinales artificiales por fusión a colocar a través del espacio
intervertebral que queda una vez extraído un disco espinal dañado,
y de modo particular a un implante de fusión espinal, por lo menos
parcialmente cilíndrico, para la implantación allí donde dos
implantes cilíndricos roscados de la altura necesaria no cabrían
dentro de la anchura transversal de la espina dorsal.
En el pasado, Cloward, Wilterberger, Crock,
Viche, Bagby, Brantigan. Michelson y otros han enseñado varios
métodos comprendiendo el taladrado de orificios a través del espacio
discal entre dos vértebras adyacentes de la espina dorsal a fin de
conseguir una fusión espinal intersomática. Cloward enseñó la
colocación de una espiga ósea dentro del orificio taladrado al
objeto de cubrir el defecto y ser incorporada en la fusión. Viche
enseñó el roscado de dicha espiga ósea. Bagby enseñó la colocación
del injerto óseo dentro de un cubo metálico o de otro modo liso en
su superficie, salvo por las filas de orificios colocados
radialmente que comunican al interior del cesto y al injerto óseo.
El dispositivo de Bagby fue descrito como apto para ser utilizado en
un caballo. Brantigan enseñó el uso de bloques inertes hechos
preferiblemente de metal y con la superficie externa de dicho metal
a imitación de la porosidad del hueso. Brantigan teorizaba que la
espiga ósea podía ser completamente sustituida por un tapón de
metal, que, aun cuando no era en sí mismo activo en la fusión, sin
duda serviría para soportar las vértebras dentro del espacio discal
mientras permitía que se produjese la fusión a su alrededor.
La patente estadounidense núm. 3.848.601 emitida
para Ma y otro, el 19 de noviembre de 1974, enseña un método e
instrumentación para preparar espacios rectangulares a través del
espacio discal dentro de vértebras adyacentes, y para preparar un
injerto rectangular del propio hueso que se inserta dentro de los
espacios rectangulares.
La patente estadounidense núm. 4.743.256, emitida
para Brantigan el 10 de mayo de 1988, enseña el uso de un separador
artificial inerte en forma de un rectángulo en lugar de usar un
injerto óseo rectangular como describió Ma y otro.
La patente estadounidense núm. 4.878.915, emitida
para Brantigan el 7 de noviembre de 1989, enseña el uso de implantes
inertes completamente cilíndricos para usar en la fusión espinal
intersomática. Dichos implantes no participan en el proceso de
fusión ósea, pero actúan como separadores inertes y permiten el
crecimiento del hueso hasta las superficies externas de los
implantes.
La patente estadounidense núm. 4.834.757, emitida
para Brantigan el 30 de mayo de 1989, enseña un implante de fusión
espinal hueco, de forma rectangular, para utilizar en lugar de un
injerto óseo rectangular o el separador inerte artificial anterior
del mismo Brantigan.
La patente estadounidense núm. 5.015.247, emitida
para Michelson el 14 de mayo de 1991, enseña el uso de un implante
cilíndrico hueco, de paredes delgadas, muy perforado y roscado, que
está cerrado o puede cerrarse por ambos extremos, a fin de ser
cargado de manera comprimible con hueso u otros materiales que
fomenten la fusión. Adicionalmente, el dispositivo de Michelson
puede ser luego recubierto con un producto químico que induzca la
producción ósea, tal como hidrosiapatita. La patente de Michelson
también describe un método perfeccionado para taladrar orificios a
través del espacio discal y dentro de dos vértebras adyacentes y la
segura instalación de implantes cilíndricos, así como todo el
procedimiento quirúrgico que puede ser llevado a cabo a través de
un tubo cilíndrico hueco. El tubo cilíndrico hueco puede dejarse en
el lugar durante todo el procedimiento quirúrgico y sirve para
sostener las vértebras adyacentes en el lugar relativo entre las
demás, permite el taladrado protegido de los orificios a través del
espacio discal, y también permite la inserción del implante a
través del mismo tubo dentro del orificio taladrado a través del
espacio discal y dentro de las vértebras adyacentes.
En lo que concierne a este método para llevar a
cabo la fusión espinal intersomática utilizando implantes roscados
esencialmente cilíndricos, surge un especial problema (véase la
figura 1) cuando se intentan colocar dos implantes cilíndricos
(considerados la cantidad preferida puesto que es una construcción
mucho más estable y tiene mayor área superficial que un solo
implante colocado en el centro) lado a lado a través de un espacio
discal y dentro de dos vértebras adyacentes donde la altura del
espacio distal es tal que requiere un implante de un diámetro tan
grande para penetrar dentro y enlazar de manera significativa cada
una de las vértebras adyacentes que ya no es posible colocar dos de
tales implantes lado a lado y tenerlos aún contenidos dentro de la
anchura transversal de la espina dorsal. Si se intenta solucionar el
problema utilizando implantes de menor diámetro colocados lado a
lado de manera que ambos sean entonces capaces de entrar dentro de
la anchura transversal de la espina dorsal, entonces los implantes
tendrían una altura insuficiente para enlazar debidamente el hueso.
Si se intenta solucionar el problema abandonando la construcción de
doble implante lado a lado a favor de un solo implante colocado en
el centro, entonces el implante es lo bastante grande para ocupar
una porción suficiente del ancho transversal del espacio discal
para promover firme estabilidad, su altura vertical y excursión
dentro del las vértebras sería tan grave que si cualquiera de los
dos espacio discales consecutivos tuviera que volver a ser operado,
deberían cortarse las vértebras por la mitad.
La patente estadounidense núm. 5.055.104, emitida
para Ray el 8 de octubre de 1991 ("Patente de Ray"), describe
un implante que comprende una bobina espiral sin miembros de pared
que se monta una vez las bobinas están colocadas en el espacio
discal entre las vértebras, pudiendo supuestamente volver a sacarse
una vez las vértebras se han fundido entre sí. El implante de Ray
tiene defectos y no puede trabajarse dado que resulta imposible
usarlo de la manera que se describe, pues no puede insertarse
dentro de hueso duro una bobina helicoidal aislada sin que tenga
miembros de pared para soportar tal bobina, que podría
estructuralmente ser análoga a algo que se escabulle (Véase la
patente de Ray, figuras 1 y 7). Además, el implante de Ray es
indudablemente complejo, puesto que necesitaría la difícil, sino
imposible, tarea de montarlo dentro del espacio discal. La figura 3
de la patente de Ray pone claramente de manifiesto que Ray no
enseña el uso de un implante cilíndrico truncado, sino que
simplemente enseña el uso de una bobina helicoidal truncada que
parece un resorte aguzado en que falta por completo un miembro de
pared que pueda considerarse cilíndrico. Por tanto, Ray únicamente
enseña el uso de una rosca aislada que tan solo puede ser insertada
por rotación y no haciéndolo avanzar de manera lineal.
De superar los obstáculos de la imposibilidad de
insertar una rosca aislada sin miembros de pared y el problema de
montarla dentro del espacio discal, entonces el implante de Ray
todavía resultaría inseguro para el objeto que pretende puesto que
existiría un elevado riesgo de desmontaje espontáneo y fallo
mecánico. Además, existiría espacio insuficiente para girar con
seguridad un dispositivo para la inserción dado que la gran falta
de dicho espacio requiere el uso de un dispositivo que tenga una
anchura transversal decreciente.
Por consiguiente existe la necesidad de un
implante de fusión espinal que pueda ser insertado dentro de un
orificio taladrado a través del espacio discal entre dos vértebras
adyacentes y parcialmente dentro de las dos vértebras adyacentes de
manera que el implante de fusión espinal sea capaz de alojarse
dentro de la anchura transversal de la espina dorsal cuando se
coloca lado a lado junto a un segundo de su clase.
La patente EP-0.637.440A describe
un implante espinal tubular hueco con una sección generalmente
circular. El implante define dos superficies laterales sólidas y
superficies superior e inferior con aberturas. Unas nervaduras
anulares en el exterior evitan el movimiento de retroceso y de
rotación del implante cuando se inserta entre vértebras
adyacentes.
De acuerdo con el presente invento se proporciona
un implante de fusión espinal hecho de un material apropiado, por
lo menos en parte, para la implantación humana a través de la
altura corregida quirúrgicamente de un espacio discal entre cuerpos
de vértebras adyacentes, comprendiendo dicho implante: un miembro
que tiene un exterior y un eje longitudinal, teniendo dicho
exterior un extremo de entrada, un extremo de salida y una longitud
entre ambos, teniendo dicho exterior una sección transversal
parcialmente circular a lo largo del eje longitudinal del implante,
teniendo la sección transversal parcialmente circular porciones
curvadas opuestas entre sí, formando arcos del mismo circulo y
adecuadas para acoplarse por penetración en cada uno de los cuerpos
de vértebra adyacentes cuando son insertados entre los cuerpos
vertebrales y a través de la altura del espacio discal, de manera
que cada una de dichas porciones curvadas opuestas tiene por lo
menos una abertura pasante para permitir el crecimiento del hueso
del cuerpo vertebral adyacente al cuerpo vertebral adyacente,
caracterizado por el hecho de que la sección transversal
parcialmente circular incluye una porción cóncava adecuada para
adaptarse a una porción convexa de un segundo implante cuando es
implantado a través de la altura del espacio discal de manera que
la anchura combinada de los dos implantes es inferior a dos veces
el diámetro máximo de los dos implantes.
El presente invento es un implante de fusión
espinal intersomático que puede ser insertado en un orifico
taladrado a través del espacio discal entre dos vértebras
adyacentes y dentro de dos vértebras adyacentes de manera que el
implante de fusión espinal es capaz de alojarse dentro de la anchura
transversal de la espina dorsal cuando se coloca lado a lado cerca
de un segundo de su clase. El implante de fusión espinal del
presente invento puede comprender un cilindro o un cilindro parcial
de pared delgada con múltiples orificios hecho de un material
adecuado para la implantación humana y teniendo preferiblemente,
pero no necesariamente, un extremo cerrado y un extremo capaz de ser
cerrado, de manera que una cámara interna puede ser llenada y
mantener cualquier material natural o artificial osteocondutivo,
osteoinductivo, osteogénico u otro material para mejorar la fusión.
El implante de fusión espinal del presente invento se basa en
rugosidades de la superficie externa para mejorar su estabilidad. El
implante de fusión espinal del presente invento incorpora en su
extremo posterior, medios de enganche para facilitar la inserción o
extracción del implante, preferiblemente en su extremo posterior.
El implante del presente invento puede estar hecho, llenado y/o
recubierto con sustancias que promueven la fusión. Además, el
implante de fusión espinal del presente invento no requiere
rotación para su inserción y puede colocarse mediante avance
lineal.
El implante de fusión espinal del presente
invento es generalmente eficaz, y es más seguro y más eficaz que
los implantes cilíndricos de la técnica anterior para el caso
especial de cuando se desean insertar dos implantes lado a lado
dentro de canales preparados cilíndricamente, y en que la altura del
espacio discal entre dos vértebras adyacentes es tan grande con
respecto a la anchura transversal de la espina dorsal que dos
implante de la altura requerida no se alojarían dentro de la
anchura transversal de la espina dorsal. La técnica anterior ha
enseñado a aquellos conocedores de la técnica de la cirugía
espinal, que la probabilidad de obtener una fusión espinal es
proporcional a tres factores: 1) el área superficial del implante,
2) la calidad y cantidad de material del injerto, y 3) la
estabilidad de la construcción de la fusión. El implante de fusión
espinal del presente invento aumenta cada uno de dichos tres
factores haciendo posible el uso de dos implantes lado a lado a
través del espacio discal que de otro modo le faltaría anchura
suficiente para admitir más de uno.
El implante de fusión espinal del presente
invento es más eficaz que la técnica anterior sobre la base de un
implante individual, por los siguientes motivos:
1. Mayor área superficial. El implante de
fusión espinal del presente invento, dadas sus rugosidades
superficiales, tiene una mayor área superficial para acoplarse con
las vértebras adyacentes que un implante con superficies externas
lisas. La presencia o ausencia de orificios no afecta esto
materialmente, mientras los orificios estén llenos con material que
efectivamente contribuya al área de contacto con la superficie. Las
porciones curvadas del implante prácticamente cilíndrico del
presente invento se hallan en contacto con las vértebras adyacentes
y proporcionan mayor área superficial de lo que es posible con una
porción plana de un implante no cilíndrico.
2. La cantidad y cualidad del material de
injerto presentado. Dado que el implante de fusión espinal del
presente invento no está atornillado, no debe construirse para que
resista la torsión asociado al mismo. Por tanto, el implante del
presente invento puede tener paredes más delgadas y en
consecuencia, para un determinado diámetro, posee un mayor volumen
interno. El implante de fusión espinal del presente invento tiene
porciones curvadas que hacen más fuerte el implante en compresión
que un implante al que le falten las superficies curvadas
superiores e inferiores de soporte, de manera que la pared del
implante puede ser relativamente más delgada que tales implantes.
Una pared delgada hace más fácil el crecimiento del hueso a través
de la misma. Asimismo, los puentes entre poros pueden ser más
pequeños debido a la mayor porosidad y por tanto mayor exposición
al material interior del injerto. Además, el implante de fusión
espinal del presente invento puede estar construido y/o recubierto,
y/o cargado con una variedad de materiales y/o substratos químicos
conocidos para que participen activamente en el proceso de fusión
del hueso. Dado que el implante de fusión espinal del presente
invento ofrece mayor área superficial y mayor volumen interno para
su diámetro exterior, proporciona la oportunidad de presentar una
mayor área superficial y volumen de dichos materiales de fusión.
3. El implante del presente invento ofrece
mayor estabilidad de que los implantes de la técnica anterior.
Los implantes menos estables son los implantes que les faltan
rugosidades superficiales. Los orificios en la superficie aumentan
la estabilidad del implante incrementado la interferencia del
implante con las superficies opuestas. El implante de fusión espinal
del presente invento es otra mejora sobre la técnica anterior por
el hecho de que las rugosidades superficiales del implante de fusión
espinal del presente invento resisten el movimiento en todas las
direcciones. Además, todos los implantes están sujetos a la
posibilidad de retroceder siguiendo el recorrido inverso al que
fueron insertados. Sin embargo, el implante de fusión espinal del
presente invento puede tener una superficie configurada para forzar
el implante de fusión espinal hacia adelante a fin de ofrecer una
mayor resistencia contra dicho movimiento de retroceso no deseado.
Además, las porciones curvadas del implante del presente invento
proporcionan una mayor área de soporte para distribuir mejor las
fuerzas de compresión a través de las vértebras.
El implante de fusión espinal del presente
invento es más fácil de usar dado que ocupa menos espacio, no
requiere roscado previo y puede ser insertado sin necesidad de
hacer girar un instrumento dentro de los estrechos confines de la
herida en la espina dorsal. Asimismo, el implante de fusión espinal
del presente invento es más fácil de instalar que los implantes que
les faltan las superficies curvadas de soporte superior e inferior
que son arcos del mismo circulo, cuyos implantes deben insertarse a
través del espacio discal y dentro de las vértebras adyacentes
puesto que es más fácil de preparar un orificio redondo que un
orificio cuadrado, ya que un orificio redondo puede taladrase un
una sola fase.
Uno de los objetos del presente invento es
proporcionar un implante de fusión espinal intersomático
perfeccionado de manera que sea posible colocar dos de tales
implantes lado a lado a través del espacio discal y dentro de dos
vértebras adyacentes cercanas una a otra, así como dentro de la
anchura transversal de la espina dorsal, allí donde la anchura
transversal de la espina dorsal resultaría insuficiente con
relación a la altura del implante que es necesaria para permitir la
acomodación de dos implantes roscados cilíndricos de la técnica
anterior.
Otro objeto del presente invento es proporcionar
un implante de fusión espinal que sea más fácil de insertar, y que
no requiera roscado antes de la implantación.
Todavía es otro objeto del presente invento
proporcionar un implante de fusión espinal que sea más seguro, en
el cual no sea necesario redondear roscas agudas cerca de
estructuras delicadas.
También es otro objeto del presente invento
proporcionar un implante de fusión espinal que sea más rápido de
implantar entre vértebras adyacentes a través de avance lineal en
lugar de avance por rotación.
Estos y otros objetos del presente invento se
pondrán de manifiesto a la vista de los dibujos adjuntos y a la
siguiente descripción detallada de los dibujos.
La figura 1 es una representación esquemática de
un segmento de la columna espinal humana comprendiendo varias
vértebras con diversos implantes roscados cilíndricos insertados a
través del espacio discal y dentro de dos vértebras adyacentes,
para mostrar los problemas presentados por dichos implantes;
La figura 2 es una vista en planta, desde arriba,

Haciendo referencia a la figura 1, se muestra una
representación esquemática de un segmento de la columna espinal
humana, indicada generalmente con la letra S. El segmento de la
columna espinal S comprende varias vértebras V y un espacio discal
D entre dos vértebras adyacentes V. Se han representado varios
implantes de fusión espinal roscados cilíndricos, dada uno de ellos
con diferentes diámetros, insertados a través del espacio discal
D.
Cuando la altura H_{s} del espacio discal D
tiene tal tamaño que dos implantes cilíndricos, tales como los
implantes de fusión espinal 10a y 10b, teniendo cada uno de ellos
un diámetro suficiente para atravesar el espacio discal D y
bastante para acoplarse dentro del hueso de las vértebras adyacentes
V, se colocan a través del espacio discal D, la anchura total
combinada de los dos implantes espinales 10b y 10b supera la
anchura transversal W_{s} de la columna espinal S. Como
consecuencia, una porción de cada implante 10a y 10b sobresale de
los costados de la columna espinal S y puede causar graves daños y
tal vez incluso la muerte al paciente dado que estructuras tan
delicadas y vitales se encuentran junto a aquella zona de la
columna espinal S que no sería deseable el uso de dos implantes de
fusión espinal cilíndricos 10a y 10b.
Si en lugar de dos implantes de fusión espinal
10a y 10b se utilizara un solo implante, tal como el implante de
fusión espinal 12a que tuviera un diámetro suficiente para
proporcionar estabilidad y fusión, entonces el implante penetraría
profundamente dentro de las vértebras adyacentes V. El implante de
fusión espinal 12a debería tener un diámetro que fuera
significativamente mayor que la altura H_{s} del espacio discal
D, de modo que las vértebras V tendrían que ser sustancialmente
perforadas para adaptar el gran diámetro del implante de fusión
espinal 12a. Como consecuencia se eliminaría una gran parte de las
vértebras V, con lo cual quedaría sustancialmente debilitada la
integridad estructural general de las vértebras V. Esto resulta
especialmente un problema cuando se coloca un segundo implante de
fusión espinal 12b idéntico al implante de fusión espinal 12a a
través del espacio discal D en el otro lado de la misma vértebra V
de manera que hay dos implantes de fusión espinal 12a y 12b
colocados a través de espacios discales D a cada costado de la
vértebra V. Como resultado, la vértebra V queda abierta en una
configuración "en mariposa" tal como la representada en la
figura 1, y la integridad estructural y resistencia de la vértebra
V aún se reduce más, de manera que la eficacia del proceso de fusión
espinal queda sustancialmente disminuido y las vértebras V
presentan un alto riesgo de devascularización y fractura.
Por el contrario, si se utilizan dos implantes
cilíndricos tales como los implantes de fusión espinal 14a y 14b,
teniendo cada uno de ellos un diámetro de tamaño suficiente para
que al ser colocados lado a lado en un espacio discal D, la anchura
toral combinada de los implantes de fusión espinal 14a y 14b llena
justo la anchura transversal W_{s} de la columna espinal S, el
diámetro de cada uno de los implantes de fusión espinal 14a y 14b
no bastaría para atravesar el espacio discal D a fin de acoplarse a
las vértebras V. Por tanto, mientras que los implantes de fusión
espinal 14a y 14b no sobresalgan de los costados de la columna
espinal S, los implantes de fusión espinal 14a y 14b no pueden
llegar y acoplarse al hueso de las vértebras V, y por tanto no
pueden funcionar para estabilizar las vértebras adyacentes V.
Haciendo referencia a la figura 2, en la misma se
muestra una vista en planta superior, tomada a lo largo de la línea
2-2 de la figura 1, con las vértebras superiores V
sacadas, de dos implantes cilíndricos roscados 10a y 10b colocados
a través del espacio discal D. Para que sean eficaces, los implantes
roscados 10a y 10b tienen una rosca exterior 11a y 11b que debe
tener una altura mínima que sea proporcional al tamaño del implante
roscado. La rosca 11a y 11b de los implantes roscados 10a y 10b
convierte el par de giro en un movimiento lineal, de modo que las
roscas 11a y 11b deben tener una altura suficiente para vencer la
resistencia del material, como el hueso, en que los implantes
roscados 10a y 10b están siendo insertados, siendo dicha resistencia
proporciona al área superficial y diámetro de cada uno de los
implantes roscados 10a y 10b. Así pues, la diferencia entre el
diámetro mayor (incluyendo las roscas) y el diámetro de la raíz
(menos las roscas) de cada implante roscado 10a y 10b es tal que
cuando se implantan dos implantes roscados 10a y 10b a través del
espacio discal D y dentro de vértebras adyacentes V, debe existir
una mínima distancia entre los dos implantes roscados 10a y 10b
para permitir la altura de las roscas 11a y 11b. Esto sería verdad
incluso si las roscas 11a y 11b fueran interdigitadas pues los
implantes roscados 10a y 10b todavía quedarían desplazado como
mínimo con la altura de la rosca de por lo menos uno de los
implantes roscados 10a y 10b. Dicha distancia mínima entre dos
implantes roscados 10a y 10b aumenta la anchura total combinada de
los dos implantes roscados 10a y 10b al ser insertados.
Por consiguiente, a fin de utilizar un implante
de fusión espinal cilíndrico en el proceso de fusión espinal en que
la altura H_{s} del espacio discal D entre dos vértebras
adyacentes V sea grande con relación a su anchura W_{s}, es
necesario tener un implante que pueda ser implantado junto a un
segundo de su clase, en un contacto más estrecho del que es posible
con implantes roscados, mientras todavía ofrezca una superficie de
implante que proporcione estabilidad mecánica al acoplarse con la
vértebra adyacente V. El uso de un implante cilíndrico es deseable
dado que es fácil de preparar el lugar receptor mediante el
taladrado de un orificio cilíndrico a través del espacio discal D y
dentro de las vértebras adyacentes V. La superficie curva de los
orificios cilíndricos taladrados en las vértebras V presenta una
mayor área superficial comparada con una superficie plana y también
ofrece la posibilidad de más estrecha congruencia cuando el
orificio cilíndrico se provee de un implante que tiene las
correspondientes porciones cilíndricas de un diámetro
equivalente.
Haciendo referencia a las figuras 3 a 19, en las
mismas se muestra una forma de realización del implante de fusión
espinal que se indica globalmente con el número 100. Aun cuando la
misma no está de acuerdo con el presente invento, presenta muchas
características en común con el mismo. El implante de fusión
espinal 100 tiene sustancialmente una configuración cilíndrica con
una delgada pared externa 112 que circunda una cámara interior 114
y un eje longitudinal central L. El exterior del implante de fusión
espinal 100 comprende rugosidades superficiales que proporcionan
una superficie adecuada para acoplarse a las vértebras V a fin de
estabilizar el implante de fusión espinal 100 a través del espacio
discal D y dentro de las vértebras adyacentes V una vez implantado
quirúrgicamente. En una forma de realización del implante de fusión
espinal 100, las rugosidades superficiales comprenden una serie de
enganches 120 a lo largo de la circunferencia de dicho implante de
fusión espinal. Cada uno de la serie de enganches 120 presenta un
borde de acoplamiento al hueso 122 y un segmento en ángulo 124.
Cada uno de la serie de enganches 120 tiene una
altura que es sustancialmente menor que la altura de una rosca
necesaria para un implante cilíndrico roscado del mismo tamaño.
Dado que una rosca es un dispositivo simple para convertir el par
de giro en un avance lineal, la altura necesaria de la rosca es
proporcional al área superficial y al diámetro del implante,
debiendo ser suficiente para tirar de un implante cilíndrico que
tiene un diámetro suficiente para atravesar el espacio discal D a
través de un material tan denso como el hueso. Por contra, los
enganches 120 tienen una altura que es notablemente menor que la
altura requerida de una rosca de un implante roscado del mismo
tamaño dado que el implante de fusión espinal 100 se implanta a
través del espacio discal D y dentro de las vértebras adyacentes V
mediante un avance lineal. El implante de fusión espinal 100 puede
ser empujado dentro del espacio discal cilíndrico D por medio de un
avance lineal directo dado que no precisa ninguna rosca para que
tire del mismo hacia adelante a través de la espina dorsal. Como no
se requiere ningún par de giro para hacer avanzar el implante de
fusión espinal 100 no existe ninguna altura mínima requerida de las
rugosidades superficiales. La única característica superficial
necesaria es que proporcione estabilidad al implante de fusión
espinal 100 una vez implantado.
Además, los enganches 120 pueden mirar en una
dirección, la dirección en que el implante de fusión espinal 100 se
inserta, y sirven para evitar que el implante de fusión espinal 100
retroceda saliendo del espacio discal D en una dirección opuesta a
la dirección de inserción una vez ha sido insertado entre dos
vértebras adyacentes V. Los enganches 120 empujan el implante de
fusión espinal 100 hacia adelante contra el hueso no sacado de la
vértebra V. Dado que generalmente los implantes tienden a
retroceder a lo largo del mismo camino en que han sido insertados,
debido al movimiento repetitivo del cuerpo del paciente con el paso
del tiempo, y que podría hacer que se soltara otro diseño de
implante (por ejemplo, hacer que un implante cilíndrico roscado
posiblemente se desenroscase), los enganches 120 tienden a forzar
el implante de fusión espinal 100 hacia adelante contra el hueso
sólido no sacado, además de resistir el desalojamiento y controlar
el movimiento resultante de una implantación excesivamente
estable.
Los bordes que se acoplan al hueso 122 de los
enganches 120 tienen una altura en el punto más alto medido desde
el diámetro de la raíz del implante de fusión espinal 100 que es
aproximadamente de 0,25 mm. De este modo, el implante de fusión
espinal 100 puede colocarse al lado de un segundo de su clase a una
distancia de separación de aproximadamente 0,7 mm, e incluso más
cerca si está desviado, reduciendo sustancialmente la anchura total
combinada de los dos implantes de fusión espinal 100 una vez
implantados quirúrgicamente. Los enganches 120 pueden tener una
altura del orden de 0,25 a 1,5 mm, siendo la altura preferida del
orden de 0,35 a 0,75 mm.
Haciendo referencia a la figura 5, se muestran
dos implantes de fusión espinal 100a y 100b insertados a través del
espacio discal D que tiene las mismas dimensiones que el espacio
discal D representado en la figura 2. Los dos implantes de fusión
espinal 100a y 100b tiene una anchura total combinada más reducida
en comparación con los dos implantes de fusión espinal roscados
colocados lado a lado que han sido descritos previamente y
representados en la figura 2. La anchura total combinada más
reducida de los dos implantes de fusión espinal 100a y 100b es la
diferencia entre los diámetros de la raíz y mayor de los implantes
de fusión espinal 100a y 100b, y se consigue utilizando rugosidades
superficiales tales como los enganches 120 para conseguir
estabilidad. Las rugosidades superficiales permiten a los dos
implantes de fusión espinal 100a y 100b acercarse considerablemente
más uno al otro y precisan menos anchura transversal total para su
inserción de la que es posible para dos implantes cilíndricos
roscados que tuvieran idénticos diámetros de raíz debido a la
necesaria altura de rosca de tales implantes roscados. La reducción
del desplazamiento entre implantes permite usar implantes de mayor
diámetro que entonces pueden alojarse dentro de la anchura
transversal W_{s} de la columna espinal y conseguir más
acoplamiento sustancial dentro de las vértebras adyacentes V.
Con referencia a la figura 7, en la misma se
muestra una sección transversal del implante de fusión espinal 100
en que la pared 112 tiene aberturas 128 que la atraviesan para
comunicar con la cámara interna 114. La cámara interna 114 puede
ser llenada con material óseo o cualquier material de injerto óseo
natural o artificial o bien material que promueva la fusión de
manera que se produzca el crecimiento de las vértebras V a través de
las aberturas 128 al material dentro de la cámara interna 114.
Mientras las aberturas 128 han sido representadas circulares en los
dibujos, se observará que las aberturas 128 pueden tener cualquier
forma, tamaño o conformación apropiada para utilizar en un implante
de fusión espinal sin apartarse del ámbito del presente invento.
Asimismo, el número de aberturas puede variar e incluso pueden no
existir aberturas en el implante de fusión espinal.
Haciendo referencia a las figuras 8 y 9, el
implante de fusión espinal 100 tiene un tapón 130 con una rosca que
se acopla por roscado a un extremo del implante de fusión espinal
100. Una vez el tapón 130 fijado al implante de fusión espinal 100,
el borde 136 actúa como un enganche adicional 120 para estabilizar
más el implante de fusión espinal 100 una vez está implantado a
través del espacio discal D.
El tapón 130 puede sacarse para tener acceso a la
cámara interna 114, de manera que dicha cámara interna puede ser
llenada y mantener en ella cualquier material natural o artificial
osteoconductivo, osteoinductivo, osteogénico u otro material que
mejore la fusión. Algunos ejemplos de dichos materiales son hueso
cosechado del paciente, o material que induzca el injerto óseo tal
como, pero sin quedar limitado a ellos, hidroxiapatita, fosfato
tricálcico de hidroxiapatita; o proteína morfogénica ósea. El tapón
130 y/o el propio implante de fusión espinal 100 está hecho de un
material apropiado para implantaciones humanas, tales como titanio
y/o puede estar hecho de, y/o llenado y/o recubierto con un
material que induzca el crecimiento del hueso, tales como, pero sin
quedar limitado a ellos, hidroxiapatita, fosfato tricálcico de
hidroxiapatita o cualquier otro material osteoconductivo,
osteoinductivo, osteogénico u otro material que mejore la
fusión.
Haciendo referencia a la figura 4,
alternativamente el tapón 130a puede tener forma de "bala"
para facilitar la inserción. El tapón 130a tiene en su mayor
diámetro un diámetro igual al diámetro de raíz del implante de
fusión espinal 100 de manera que no hay formados enganches
adicionales 120.
Haciendo referencia a la figura 10, el implante
de fusión espinal 100 tiene medios de acoplamiento en un extremo en
forma de una ranura rectangular 140 para acoplar un instrumento de
accionamiento que posea medios de acoplamiento desmontables para
acoplarse íntimamente a la ranura rectangular 140. Una porción
roscada del instrumento de accionamiento, que en una forma de
realización se extiende como una barra a través de un miembro
tubular hueco y puede controlarse de manera giratoria, se atornilla
dentro de una abertura roscada 142 en la ranura 140 y une el
implante 100 y el instrumento de accionamiento entre sí. Una vez
fijado el instrumento de accionamiento del implante, el implante de
fusión espinal 100 puede ser introducido a través de un tubo
cilíndrico hueco y accionado dentro del orificio cilíndrico que ha
sido taladrado a través del espacio discal D. Luego el instrumento
de accionamiento del implante puede ser golpeado con un mazo, o un
dispositivo similar, para hacer avanzar linealmente el implante de
fusión espinal 100 a través del espacio discal D. Cuando el
implante de fusión espinal 100 está insertado a través del espacio
discal D, los enganches 120 se acoplan al hueso de las vértebras V
y se retira el instrumento de accionamiento del implante de fusión
espinal 100. El procedimiento para taladrar los orificios a través
del espacio discal D y la instrumentación necesaria para ello se
describen en la patente WO-94/28824.
Haciendo referencia a las figuras 11 a 14, en las
mismas se muestra una forma de realización alternativa de un
implante de fusión espinal, indicado generalmente con el número de
referencia 200. A pesar de que esta forma de realización no está de
acuerdo con el presente invento, posee muchas características
comunes con el mismo. El implante de fusión espinal 200 es similar
al implante de fusión espinal 100 salvo por el hecho de que las
aberturas 228 están bisectadas por el borde de acoplamiento al hueso
22 de la serie de enganches 220. De este modo, los bordes de
acoplamiento al hueso quedan interrumpidos por las aberturas 228 a
fin de proporcionar un borde "a modo de diente" que engancha
el hueso de las vértebras V y crea un ajuste de interferencia para
evitar el retroceso del implante 200 una vez insertado. Hay que
tener en cuenta que el número de aberturas 228 y el número de bordes
de acoplamiento al hueso 222 pueden variarse, y que la abertura 228
puede colocarse en cualquier orientación con respecto a los
enganches 220 u otra rugosidad superficial, sin apartarse del ámbito
del presente invento.
Haciendo referencia a las figuras 15 a 22, en
ellas se muestra una forma de realización alternativa del implante
de fusión espinal, indicado generalmente con el número de
referencia 300. De nuevo, esta forma de realización aun no estando
de acuerdo con el presente invento presenta muchas características
comunes al mismo. El implante de fusión espinal 300 tiene
sustancialmente configuración cilíndrica con rugosidades
superficiales para estabilizar el implante 300 dentro del espacio
intervertebral D. Tal como puede verse en la figura 18, las
rugosidades superficiales comprenden un estriado superficial 320
tal como, pero sin quedar limitado a ella, el modelo en forma de
diamante para acoplarse al hueso. El implante de fusión espinal 300
puede tener un estriado superficial 320 por toda la superficie
externa del implante de fusión espinal 300, en solo una porción de
la superficie externa o cualquier combinación de ellas, sin
apartarse del ámbito del presente invento. En aquellas
circunstancias donde no existe hueso sin taladrar en el espacio
discal D antes del implante de fusión espinal 300 para resistir más
avance del implante, se prefiere el estriado superficial 320 puesto
que produce un ajuste de interferencia excesivamente alta con el
hueso de las vértebras V y resiste el movimiento por igual en todas
las direcciones sin tendencia a avanzar por sí mismo.
Haciendo referencia a la figura 17, dos implantes
de fusión espinal 300a y 300b puede colocarse lado a lado a través
del espacio discal D que tenga las mismas dimensiones que el
espacio discal D represando en la figura 2, de modo que los dos
implantes de fusión espinal 300a y 300b se toquen uno al otro para
reducir así la anchura total combinada de los dos implantes
espinales 300a y 300b a la mínima distancia posible con un implante
sustancialmente cilíndrico que tiene una superficie rugosa. De esta
manera, dos implantes de fusión espinal cilíndricos 300a y 300b que
tienen un diámetro suficiente para atravesar la altura H_{s} del
espacio discal D pueden colarse a través del espacio discal D sin
exceder la anchura transversal W_{s} de la columna espinal S. Los
implantes de fusión espinal 300a y 300b se insertan mediante el
avance lineal tal como se ha descrito antes para el implante de
fusión espinal 100. Por consiguiente, dado que no se necesita
ningún roscado para la inserción de los implantes de fusión espinal
300a y 300b, se requiere la presencia de poco o ningún espacio entre
los implantes de fusión espinal 300a y 300b, en comparación con el
espacio que se necesitaría para una rosca caso de utilizar
implantes roscados. Así pues, los implantes de fusión espinal 300a y
300b pueden colocarse juntos para reducir sustancialmente la
anchura total combinada de dos de tales implantes.
Haciendo referencia a la figura 23, se muestra
una segmento de la columna espinal S con una forma de realización
alternativa de dos implantes de fusión espinal 900a y 900b
insertados a través del espacio discal D_{1}. El implante de
fusión espinal 900a tiene una superficie cóncava 902 que tiene la
forma correspondiente para recibir la superficie convexa 904 del
implante de fusión espinal 900b. Cuando los dos implantes de fusión
espinal 90a y 900b se colocan lado a lado, la superficie cóncava
902 se apareja con la superficie convexa 904 de tal modo que la
anchura total combinada de los dos implantes de fusión espinal es
inferior a dos veces el diámetro máximo M de dichos implantes. Como
resultado, las ventajas de colocar dos implantes que son
parcialmente cilíndricos, con respecto a la porción que se acopla a
las vértebras V, lado a lado a través del espacio discal D puede
obtenerse sin exceder la anchura W_{S} de la
columna espinal.

Haciendo todavía referencia a la figura 23, se
muestra una forma de realización alternativa del implante de fusión
espinal del presente invento que comprende un simple implante de
fusión espinal 1000 insertado a través del espacio discal D_{2}
de la columna espinal S. El implante de fusión espinal 1000
comprende una primera porción cilíndrica 1010 y una segunda porción
cilíndrica 1012, y puede tener cualquiera de las rugosidades
superficiales descritas antes con referencia a las formas de
realización presentadas con anterioridad. En la forma de realización
preferida, el implante de fusión espinal 1000 se inserta por medio
de avance lineal dentro de dos orificios cilíndricos solapados que
han sido taladrados a través del espacio discal D_{2}.
Mientras el presente invento ha sido descrito con
detalle respecto a formas de realización preferidas, hay que tener
en cuenta que pueden idearse otras variantes del presente invento
que no se aparten del ámbito del presente invento, tal como viene
definido en las reivindicaciones adjuntas.
Claims (20)
1. Un implante de fusión espinal hecho de un
material apropiado, por lo menos en parte, para la implantación
humana a través de la altura corregida quirúrgicamente de un
espacio discal (D_{1}) entre cuerpos de vértebras adyacentes (V),
comprendiendo dicho implante:
un miembro (900a) que tiene un exterior y un eje
longitudinal, teniendo dicho exterior un extremo de entrada, un
extremo de salida y una longitud entre ambos, teniendo dicho
exterior una sección transversal parcialmente circular a lo largo
del eje longitudinal del implante, teniendo la sección transversal
parcialmente circular porciones curvadas opuestas entre sí, formando
arcos del mismo circulo y adecuadas para acoplarse por penetración
en cada uno de los cuerpos de vértebras adyacentes (V) cuando son
insertados entre los cuerpos vertebrales (V) y a través de la
altura del espacio discal(D_{1}), de manera que cada una de
dichas porciones curvadas opuestas tiene por lo menos una abertura
pasante (128; 228; 328) para permitir el crecimiento del hueso del
cuerpo vertebral adyacente al cuerpo vertebral adyacente,
caracterizado por el hecho de que la sección transversal
parcialmente circular incluye una porción cóncava (902) adecuada
para adaptarse con una porción convexa (904) de un segundo implante
(900b) cuando es implantado a través de la altura del espacio
discal (D_{1}) de manera que la anchura combinada de los dos
implantes es inferior a dos veces el diámetro máximo de los dos
implantes.
2. El implante de fusión espinal de acuerdo con
la reivindicación 1, en que dicho miembro tiene un interior hueco
capaz de retener material para promover la fusión.
3. El implante de fusión espinal de acuerdo con
la reivindicación 2, en que dicho implante tiene por lo menos un
tapón desmontable (130; 230; 330) para cerrar por lo menos un
extremo de dicho interior hueco.
4. El implante de fusión espinal de acuerdo con
una cualquiera de las reivindicaciones anteriores, comprendiendo
además una serie de enganches anulares (120; 220) en el exterior de
dicho miembro (900a) para acoplarse los cuerpos vertebrales (V) a
fin de mantener dicho implante en su sitio, encontrándose dichos
enganches anulares en la periferia del citado miembro.
5. El implante de fusión espinal de acuerdo con
la reivindicación 4, en que dichos enganches (120; 220) miran en
una dirección.
6. El implante de fusión espinal de acuerdo con
la reivindicación 4 ó 5, en que dichos enganches (120; 220)
incluyen un segmento en ángulo (124; 224) que termina en un borde
para acoplarse al hueso (122; 222) en la periferia del citado
miembro.
7. El implante de fusión espinal de acuerdo con
una cualquiera de las reivindicaciones 4, 5 ó 6, en que dicha por lo
menos una abertura (228) interrumpe los mencionados enganches
(220).
8. El implante de fusión espinal de acuerdo con
una cualquiera de las reivindicaciones 4, 5, 6 ó 7, comprendiendo
además una serie de aberturas (128) situadas, por lo menos en
parte, entre dichos enganches.
9. El implante de fusión espinal de acuerdo con
una cualquiera de las reivindicaciones 1 a 3, comprendiendo además
estrias (320) en el citado exterior de dicho miembro para
engancharse a los cuerpos vertebrales (V) a fin de mantener dicho
implante en su sitio.
10. El implante de fusión espinal de acuerdo con
una cualquiera de las reivindicaciones anteriores, en que dicho
implante comprende un material para el crecimiento óseo.
11. El implante de fusión espinal de acuerdo con
la reivindicación 10, en que dicho implante comprende un material
de crecimiento óseo distinto al hueso.
12. El implante de fusión espinal de acuerdo con
una cualquiera de las reivindicaciones anteriores, en que dicha por
lo menos una abertura (12(, 228; 328) es capaz de retener material
promotor de la fusión.
13. El implante de fusión espinal de acuerdo con
una cualquiera de las reivindicaciones anteriores, en que dicho
implante está combinado con un material promotor de la fusión.
14. El implante de fusión espinal de acuerdo con
la reivindicación 13, en que dicho material promotor de la fusión
incluye por lo menos un hueso, proteína morfogenética ósea,
hidroxiapatita y fosfato tricálcico de hidroxiapatita.
15. El implante de fusión espinal de acuerdo con
una cualquiera de las reivindicaciones anteriores, en que dicho
implante está hecho de un material artificial.
16. El implante de fusión espinal de acuerdo con
la reivindicación 15, en que dicho material artificial incluye un
metal de calidad implantable.
17. El implante de fusión espinal de acuerdo con
una cualquiera de las reivindicaciones anteriores, en combinación
con un segundo implante de fusión (900b).
18. El implante de fusión espinal de acuerdo con
una cualquiera de las reivindicaciones 1 a 12, en combinación con un
material de injerto.
19. El implante de fusión espinal de acuerdo con
una cualquiera de las reivindicaciones anteriores, en combinación
con un impulsor de implante configurado para insertar dicho
implante de fusión espinal dentro de la espina dorsal.
20. El implante de fusión espinal de acuerdo con
una cualquiera de las reivindicaciones anteriores, en combinación
con un tubo hueco que tiene una abertura para proporcionar acceso
protegido al espacio discal (D) y los cuerpos vertebrales
adyacentes (V) y para guiar la inserción de dicho implante de fusión
espinal dentro de la espina dorsal.
