Hexadecy1phosphochol1 π er et kendt stof. I Pharmazie 37, 1982, side 706-707, angives herfor en lyserende og fusogen virkning.
Oet har nu vist sig, at denne forbindelse desuden har en udmærket antitumorvirkning, og sammenlignet med homologe forbindelser, der er beskrevet i europæisk patentansøgning nr.
5 108.565, udmærker sig ved følgende overraskende egenskaber:
Medens lignende forbindelser med en kortere alkylrest, som f.eks. tetradecylphosphocholin, praktisk taget ikke viser nogen antitumorvirkning (f.eks. in vitro i L 1210-koloniforsøg eller in vivo på autochthan methylnitrosourinstofinduceret mammakarci nom hos rotte), er de med en længere alkylrest, som jq f.eks. octadecylphosphocholin ganske vist antitumorvirksomme, men samtidig alt for giftige og derfor ikke anvendelige som lægemiddel. Ved en bestemmelse af den subakutte toksicitet under en behandling på 5 uger med en antitumorvirksom daglig dosis på 77 pmol pr. kg rotte oralt fandtes således en ekstrem høj mortalitet, der lå på 80% af alle dyrene. Trods en antitu-morvirkning var den mediane overlevelsestid sammenlignet med kontrollen hos octadecylphosphocholinbehandlede dyr, derfor forkortet 72%. I modsætning hertil var hos dyr behandlet med hexadecylphosphocholin mortaliteten halvt så stor, og som følge af mangel på en kronisk toksicitet skete der en højsignifikant stigning i den mediane overlevelsestid på 26% sammenlignet med kontrollen.

Hexadecylphosphocholin indtager altså inden for de homologe a 1ky1forb inde1 ser en overraskende særstilling, idet kun hexadecylphosphochol in har en i praksis udnyttelig god antitumorvirkning. Homologe med kortere alkylrester har ingen eller en alt for ringe antitumorvirkning. Homologe med længere alkylrester er ganske vist virksomme over for tumorer men samtidig alt for toksiske. Alene hexadecylphosphocholin udviser derfor en tilstrækkelig antitumorvirkning i ikke-toksi ske doser.

Opfindelsen angår et antitumorvirksomt lægemiddel, der er ejendommeligt ved, at det som virksomt stof indeholder hexadecylphosphocholin samt eventuelt sædvanlige farmaceutiske tilsætningsstoffer, bærestoffer og/eller fortyndingsmidler, og som egner sig især til behandling af tumorer. Endvidere angår opfindelsen fremgangsmåder til fremstilling 5 af antitumorvirksomme lægemidler, hvilke fremgangsmåder er ejendommelige ved det i den kendetegnende del af hvert af kravene 8-12 angivne. Sådanne lægemidler har en udpræget cytotoksisk virkning, der er blevet påvist både in vivo på kemisk inducerede mammakarcinomer hos rotte og også in vitro på leukæmiceller i cellekultur. Derudover blev ved en klinisk pilotundersøgelse hos kvindelige patienter med mammakarcinomer 10 hudmetastaser bragt til fuldstændig heling ved topisk anvendelse.
*n kendt, at der hidtil ikke har varet noget i enhver henseende tilfredsstillende legemiddel til rådighed til behandling af tumorer, især ondartede tumorer. Således haves der f.eks. til topisk behandling af hudmetastaser hos patienter med metastaserende tumorer kun 5-fluorouracil. Videreudviklin-15 ger af andre cytostatika er til denne applikationsmåde hidtil DK 172954 B1 3 ikke blevet fulgt op til den kliniske fase. På den anden side er ud fra et klinisk synspunkt et sådant koncept i den palliative terapianvendelse særlig ønskelig, da alternative behandlingskoncepter, såsom kirurgiske foranstaltninger, stråleterapi og systemisk kemoterapi, er forholdsvis aggresive terapimo-5 daliteter. Desuden foreligger der et betydeligt antal patienter som potentielle behandlingskandidater til en sådan topisk behandling. Således er f.eks. andelen af mammakarcinompatien-ter, der udviser hudangreb ca. 25 til 35%.
Forudsætningen for topisk behandling for så vidt angår det anvendte virksomme stof, er et det tåles på huden, har cytotok-sisk virkning over for tumorceller og en tilstrækkelig dybde-10 gennemtrængning.
Formålet med opfindelsen er derfor i første række at skaffe et lægemiddel, der er egnet til topisk behandling af tumorer. Et yderligere formål med opfindelsen er desuden at skaffe et generelt også i andre applikationsformer anvendeligt lægemiddel, der forbinder god virkning mod tumorer med ringe toksicitet og derfor er alment anvendeligt i tumorterapien.

Denne opgave løses ifølge opfindelsen med et lægemiddel, som er ejendommeligt ved, at det som virksomt stof indeholder he-xadecylphosphocholin.
Især til topisk applikation »en også til fremstilling som lægemiddel til andre applikationsmåder har det vist sig særligt . DK 172954 B1 4 gunstigt at anvende hexadecy1phosphocho1 i net sammen med mindst én a 1ky1 g 1ycerin med 3-12 carbonatomer i alkyl resten , der kan foreligge i form af en ethergruppe på en af de primære eller sekundære OH-grupper i glycerinet. Sådanne a 1ky1 g 1yceri ner 5 forager henholdsvis forbedrer virkningen af hexadecylphospho-cholin synergistisk. Fortrinsvis anvendes hertil alkylglyce-riner med 3-9 C-atomer alene eller i blanding.
Særligt gunstige virkninger har et synergistisk virkende lage-middel, der indeholder a) hexadecy1phosphocholin og
b) en eller flere alkylglyceriner med den almene formel I
15 H2C - 0 - Rl HC - O - R2 h2I: - oh hvori en af resterne Ri og R2 er en alkylgruppe med 3-12 C-atomer, og den anden rest betyder et H-atom, 20 samt eventuelt andre sædvanlige farmakologiske tilsætningsstoffer og for tynd ingsmid1 er.
En sådan blanding betegnes i det følgende også som kaskade.

Indholdet af hexadecylphosphocho 1 in i mg/ml kaskade betegnes med et efterstående index på en sådan måde, at f.eks. en kaskadeblanding, der indeholder 5 mg/ml hexadecy1phosphocho1 in, betegnes som kaskades og en blanding med 200 mg af forbindelsen hexadecy1phosphocho1 in pr. ml kaskade betegnes som kaska- 30 j 2 00
Fremstillingen af a 1ky1 glyceri nerne er kendt f.eks. fra DE-0S 33 43 530.8.
35 F.eks. foretrækkes a 1 ky1 glycerin-vand-b1 and i nger, der f.eks.
indeholder nonylglycerin, octylglycerin, hexyl glycerin, pen-tylglycerin, propylglycerin og ethylglycerin. Fortrinsvis indeholder sådanne vandige blandinger 3 af de nævnte glycerin- 5 DK 172954 B1 ethere nemlig en laver (ethyl, propyl), en mellem (pentyl, hexyl) og en højere (nonyl, octyl), idet vægtmængden af den lavere ether er ca. lige så stor som summen af vægtmængderne af de to andre glycerinethere. Vandmængden er ca. lig mængden af 5 den lavere glycerinether og er f.eks. halvdelen af den samlede mængde af de foreliggende glycerinethere. Eksempler på sådanne glycerinether-vand-blandinger er anført i det følgende:

For at behandle hudtumorer henholdsvis 20 hudmetastaser med dette lægemiddel bliver de angrebne hudom råder indgnedet to til tre gange om dagen med f.eks. kaskades til kaskade2o· Skadelige bivirkninger har ikke hidtil kunnet iagttages, heller ikke hos patienter der er blevet behandlet gennem et tidsrum på 3 måneder. Remissionen af hudmetastaserne 25 er ledsaget af en normalisering af huden, således som det kunne påvises entydigt ved hjælp af vævssnit. Flere kvindelige patienter med hudmetastaser blev behandlet på denne måde, og herved blev der iagttaget en fuldstændig forsvinden af mamma-karcinomhudmetastaserne.

Den topiske behandling med det ifølge opfindelsen foretrukne middel i sammensætningen kaskades til kaskade200 kan også anvendes til behandling af indre tumorer henholdsvis metastaser ved storfladet indgniden. Via resorptionen gennem huden opnås 35 herved et terapeutisk virksomt blodspejl. En fordel ved denne applikationsmåde ligger i, at præparaterne kaskades til kaska-de2oo tåles problemfrit af huden.

Denne foretrukne præparatform for lægemidlet ifølge opfindelsen i form af opløsningerne kaskades til kaskade2o egner sig også godt til fremstilling af suppositorier til rektal indføring . Også hermed kan indre tumorer henholdsvis indre metasta-5 ser behandles godt.
En anden anvendelsesmåde for lægemidlet ifølge opfindelsen består i instillation i forud dannede legemshulrum. Denne anvendelsesmåde egner sig især til pleurakarcinoser, malign as-j q cites, maligne per ikard-udstrømni nger og blærekarcinorner. I dette tilfælde anvendes hexadecylphosphocholi net enten alene eller i forbindelse med sædvanlige bærestoffer og fortyndings-midler, især også med kaskade.
Til systematisk applikation kan der f.eks. være tale om oral 1 5 eller intravenøs indgift.
Til den orale indgift anvendes hexadecy1phosphocho1 i net f.eks. i form af en drikkeopløsning. Som bærer egner sig f.eks. mælk, kakao, frugtsaft eller drikkevand. Fremstillingen af en sådan 20 drikkeopløsning kan f.eks. ske ved fortynding af en koncentreret alkoholisk opløsning af hexadecy1phosphocholin med vand eller et andet af de førnævnte midler. Hos rotter førte daglige doser på 20, 40 og 60 mg/kg legemsvægt af hexadecy1phospho-cholin til en fuldstændig remission af kemisk inducerede mam-25 makarci norner. Her viser hexadecy1phosphocho1 in sig som bedre virksomt og tåles bedre end f.eks. 1-octadecyl-2-methyl-rac-glycero-3-phosphocholin. Den til disse forsøg anvendte tumormodel er en såkaldt hård model. Dette betyder, at de på denne model konstaterede resultater også kan overføres til den hu-30 mane situation.
Til den i nt ravenøse indgift via den intravenøse infusionsterapi anvendes hexadecy1phosphocho1 i net hensigtsmæssigt i fysiologisk kogsaltopløsning. Også andre infusionsopløsninger kan 35 anvendes hertil. Dosis til mennesker af sådanne opløsninger er f.eks. 1-10 mg/kg legemsvægt.
Endelig kan flere applikationsmåder for lægemidlet ifølge op findelsen anvendes i komb i nation, idet den særlige topiske tå- DK 172954 B1 7 lelighed fører til, at en indgniden af huden kan anvendes kombineret med en af de andre applikationsformer.
En anden bærerbi ånding for hexadecy1phosphocho1 i net, der har 5 vist sig særlig god, består af en blanding af ca. 4 vægtdele vand, 4 vægtdele propylglycerin og 2 vægtdele af hver af he-xylglycerin og nony1glycerin.
Topisk anvendelse af lægemidlet ifølge opfindelsen i den særligt foretrukne præparatform kaskades til kaskade2oo gennem et tidsrum på flere måneder viste, at den lokale toksicitet begrænser sig til en forstærket afskalning af huden ligesom ved lokal anvendelse af acetylsalicylsyre.
Opfindelsen tilvejebringer derfor et hidtil ukendt lægemiddel 15 til behandling af tumorer og giver herved ikke blot overhovedet et antitumormiddel, men giver for første gang et middel, der kan påvises at være virksomt ved topisk anvendelse i kliniske forsøg. Herved åbnes nye muligheder for behandling af tumorpat i enter.

Til fremstilling af lægemidler bliver hexadecy1phosphocho1inet forarbejdet til farmaceutiske præparater med sædvanlige farmaceutiske bærestoffer og/eller fortyndingsmidler eller med øvrige hjælpestoffer eller bragt i en terapeutisk anvendelig 25 form. Dette sker f.eks. ved, at hexadecylphosphocholi net blandes eller homogeniseres med sædvanlige bærestoffer og/eller fortyndingsmidler henholdsvis hjælpestoffer ved temperaturer mellem 20 og 120*C, fortrinsvis mellem 30 og 100*C, den derved fremkomne blanding udstøbes i hule celler af passende størrel-30 se til fremstilling af præparater, som pr. doseringsenhed indeholder 5 til 2000 mg, fortrinsvis 10 til 500 mg, især 30 til 400 mg hexadecylphosphocholin, eller fyldes i kapsler af passende størrelse eller granuleres og derpå eventuelt under tilsætning af yderligere sædvanlige hjælpestoffer presses til 35 tabletter.
F.eks. ved at man blander hexadecylphosphocholin med et eller flere af følgende stoffer: stivelse, cellulose, lactose, for- 8 DK 172954 B1 ma1 i n-case in, modificeret stivelse, magniumstearat, calcium-hydrogenphosphat, højdispers kiselsyre, talkum, phenoxyetha-nol, granulerer den fremkomne blanding eventuelt med en vandig opløsning, der som bestanddel indeholder mindst gelatine, s t i -5 velse, po1yvi ny 1 pyrroli don, vi ny 1 pyrro 1 i don-vi ny 1 acetat-copo-lymerisat og/eller polyoxyethy1sorbitanmonooleat, homogeniserer granulatet eventuelt med et eller flere af de ovennævnte hjælpestoffer og presser denne blanding til tabletter eller fylder den i kapsler, idet sådanne tabletter eller kapsler pr.
10 doseringsenhed indeholder 5 til 2000 mg hexadecy1phosphocho-lin, eller at man suspenderer hexadecy1phosphocholin efter tilsætning af sojalecithin samt eventuelt 0,1 - 0,5 vægtdele phenoxyethano1 (beregnet på 1 vægtdel hexadecy1phosphochol in ) i smeltet hård fedt ved temperaturer mellem 33 og 371C og ho-15 mogeniserer og derpå udstøber blandingen i hule celler, idet doseringsenheden indeholder 5 til 2000 mg hexadecylphosphocho-lin samt eventuelt 0,1 - 0,5 vægtdele phenoxyethano1 (bereg net på 1 vægtdel hexadecy1phosphocholin), eller at man homogeniserer hexadecylphosphocholin ved en temperatur mellem 50 20 og 120eC, fortrinsvis 50 til 1001C, eventuelt i nærværelse af en eller flere emulgatorer og/eller 0,1 - 0,5 vægtdele pheno-xyethanol (beregnet på 1 vægtdel hexadecylphosphochol in) med mindst ét af følgende stoffer: paraffin, vaseline, alifatisk alkohol med 12 til 25 C-atomer, alifatisk monocarbonsyre med 25 15 til 20 C-atomer, sorbitanmonopalmitat, polyoxyethylenpoly- ol fedtsyreester og emulgerer den fremkomne blanding mellem 50 og 120“C med vand eventuelt under tilsætning af en polyvalent lavere alifatisk alkohol og/eller phenoxyethanol, eller at man opløser hexadecylphosphocholin i vand eller planteolie even-30 tuelt i nærværelse af 0,1 - 0,5 vægtdele phenoxyethanol (beregnet på 1 vægtdel hexadecy1phosphocholin) samt eventuelt i nærværelse af en emulgator ved temperaturer mellem 30 og 100®C og eventuelt supplerer den således fremkomne opløsning op med så meget vand eller planteolie, at den endelige opløsning in-35 deholder 0,05 til 10 vægt1, fortrinsvis 0,1 til 5 vægt1 hexadecylphosphochol in.
Som emulgatorer kan der f.eks. være tale om: i kke-i onogene emulgatorer og ionogene emulgatorer. Oe ikke-ionogene emulga- 9 DK 172954 B1 torer er f.eks. triglyceridblandinger af mættede plantefedt-syrer med Cg. C|o °9 C12 emulgatorer på basis af poly- additionsprodukter af ethylenoxid, som f.eks. alkyl- og acyl-substi tuerede polyadditionsprodukter af ethylenoxid, polyethy-5 lenglycol-fedtsyreester, omsætningsprodukter af ethylenoxid med ricinusolie henholdsvis hydreret ricinusolie, estere af hydreret ricinusoliefedtsyrer med oxyethyleret glycerin. Endvidere kan det dreje sig om emulgatorer på basis af fedtsyreamider eller fedtsyrekondensationsprodukter med hydrofile 10 grupper. Som ionogene emulgatorer kan der f.eks. være tale om emulgatorer på basis af fedtsyremonoestere af glycerin og andre polyvalente alkoholer (Lunacera alba).
Hvis hexadecylphosphocholinet ved den ovenfor anførte frem-stilling af lægemidlet anvendes i nærværelse af en glycerin- 1 b ether med formlen I eller en blanding af sådanne glycerinethe-re med formlen I, iagttages en synergistisk forøgelse af anti tumorvirkningen.
Til dette formål anvendes hexadecy1phosphochol i net sammen med 20 1 til 30, fortrinsvis 2 til 20 vægtdele (beregnet på 1 del he-xadecylphosphocholin), af mindst én giyceri nether med formlen I eller en blanding af sådanne glycerinethere samt eventuelt 0,5 - 30, fortrinsvis 1-20 vægtdele, vand (ligeledes beregnet på 1 vægtdel hexadecylphosphocholin). Denne blanding med 2 5 g 1 ycerinetherne kan ske ved begyndelsen af fremstillingen af lægemidlet, men eventuelt også på et senere fremstillingsstadium.
Hexadecy1phosphocholi net udviser f.eks. en god virkning på 30 7,12-dimethylbenzanthraceninduceret brystkirte 1 kræft hos rot te og ligeledes på methyl-nitrosour i ns tof induceret mammakarci-nom hos rotte.
F.eks. opnås ved ovennævnte forsøgsmetode med en dosis på 10 35 mg/kg legemsvægt af rotte en vækststandsning af tumoren og med højere doser også en fuldstændig forsvinden af svulsten.
Den laveste, men allerede virksomme dosis i ovennævnte dyreforsøg, er f.eks.
10 DK 172954 B1 5 mg/kg oral 5 mg/kg Intravenøs.
Som alment dosisområde for virkningen (dyreforsøg som ovenfor) 5 kan der f.eks. være tale om: 5-50 mg/kg oral, især 15 - 32 mg/kg 5-50 mg/kg intravenøs, især 15 - 32 mg/kg.
Virkningsretningen af forbindelserne ifølge opfindelsen er 10 sammenlignelig med virkningen af det kendte lægemiddel TAMOXIFEN, men der er dog især følgende forskelle: virkningen er stærkere og af længere varighed end af TAMOXIFEN.
Indikationer, hvortil forbindelserne ifølge opfindelsen kommer i betragtning, er brystkirtelkræft og andre menneskelige kræftformer.
De farmaceutiske præparater indeholder i almindelighed mellem 5 og 2000 mg, f.eks. 10 til 400 mg hexadecy1phosphocholin.
20 Indgiften kan f.eks. ske i form af tabletter, kapsler, piller, dragéer, suppositorier, salver, geler, cremer, pudder, støve-pulver, aerosoler eller i flydende form. Som flydende anvendelsesformer kan der f.eks. være tale om: olieagt i ge eller alkoholiske henholdvis vandige opløsninger samt suspensioner og 25 emulsioner. Foretrukne anvendelsesformer er tabletter, der indeholder mellem 40 og 400 ml, eller opløsninger der indeholder mellem 0,1 og 5% aktivt stof.
Enkeltdosen af hexadecy1phosphocho1 in kan f.eks. ligge 3 0 a) ved orale lægemiddelformer mellem 5 og 100 mg/kg legemsvægt, fortrinsvis 15 til 50 mg/kg legemsvægt, b) ved parenterale lægemiddelformer (f.eks. intravenøs, i n-tramuskulær) mellem 5 og 100 mg/kg legemsvægt, 35 c) ved lægemiddel former til lokal applikation på huden og slimhinden (f.eks. i form af opløsninger, lotioner, emul- , sioner, salver osv.) mellem 50 og 2000 mg, fortrinsvis 80 til 1500 mg.
n DK 172954 B1 F.eks. kan der anbefales 3 gange daglig 1 tablet med et indhold på 40 til 400 mg virksomt stof eller f.eks. ved intravenøs injektion 1 til 5 gange daglig en ampul på 1 til 5 ml indhold med 50 til 200 mg stof. Ved oral i ndgi ft er den minimale 5 daglige dosis f.eks. 120 mg. Den maksimale daglige dosis ved oral indgift skal ikke ligger over 100 mg/kg legemsvsgt.
Den akutte toksicitet af hexadecylphosphocholin på mus (udtrykt ved LD 50 mg/kg, metode ifølge Miller og Tainter: Proc.
Soc. Exper. Biol. a. Med. 57 (1944) 261) ligger f.eks. ved oral applikation mellem 200 og 450 mg/kg legemsvægt.
Lægemidlerne kan anvendes i humanmedicinen, veterinærmedicinen samt i landbruget alene eller i blanding med andre farmakologisk aktive stoffer.

Opfindelsen belyses af følgende eksempler.
Eksempe1 1.
Fremstilling af hexadecylphosphocholi η , H2O
20 a) Hexadecylphosphoethanolamin {phosphorylering, ringslutning og r i ngåbni ng).
Hexadecanol (1 mol, 243 g) og triethylamin (1,8 mol, 180 g) opløses i 1,5 liter THF (tetrahydrofuran) og sættes dråbevis 25 til en stærkt omrørt opløsning af phosphoroxychlorid (1,2 mol, 184 g) i 120 ml THF, således at temperaturen i reaktionsbeholderen (trehalset kolbe, 5 1, med skilletragt, termometer og omrører) ikke overst i ger 10* C. For at fremskynde reaktionen afkøles reaktionsbeholderen ned en blanding af is og kogsalt.
30 Umiddelbart efter inddrypningen er reaktionen afsluttet (påvisning via DSC i ether: Rf-værdi på 0,8 for udgangsproduktet, og på 0,0 for reaktionsproduktet efter hydrolyse med vand).
Man fjerner Isbadet, og tildrypper til reaktionsblandingen un-35 der stærk omrøring en opløsning af ethanolamin (1,5 mol, 92 g) og triethylamin (1,8 mol, ISO g) i l 1 dioxan, således at temperaturen 1 reaktionsbeholdtren stiger til 55 til 70’C. Derefter er ringdannelsen afsluttet (påvisning ved DSC i ether: Rf- DK 172954 B1 12 værdi på 0,2). Man filtrerer varmt fra udfældet triethy1 am in-hydroch1 or i d og sætter 1,5 1 2 N myresyre til filtratet ved 40 til 50 * C. Efter 15 minutter er ringåbningen afsluttet (påvisning ved DSC i ether: Rf-v*rdi 0,0, DSC i chloroform/methanol/ 5 eddikesyre/vand 100:60:20:5 pr. rumfang: Rf-værdi 0,8). Man afkøler til -20eC og filtrerer fra bundfaldet, der består af vidtgående rent hexadecylphosphoethanol ami η. I tilfælde af lette forureninger foretages en kromatografisk rensning (se eksempie 2). Mikroanalyse (MV 365,50): 10 beregnet (%) C 59,15 H 11,03 N 3,83 P 8,48 fundet (%) C 59,01 H 10,95 N 3,79 P 8,31 (Methylering af 1) 15 De ifølge eksempel 1 fremkomne krystaller bliver uden yderligere rensning optaget i 1,2 1 2-propanol og 0,4 1 dichlorme-than. Til suspensionen af krystallerne sætter man under stærk omrøring kaliumcarbonat (4 mol, 560 g) i 1 1 vand. Til den tofasede reaktionsblanding sættes di methyl sul fat (4 mol, 500 g) 20 dråbevis og under omrøring på en sådan måde, at temperaturen ikke overstiger 40eC. Reaktionen er afsluttet 60 minutter efter inddrypni ngen (påvisning ved DSC i chloroform/methanol/ 25% ammoniak 50:50:5 efter rumfang: Rf-værdi 0,3). Efter faseadskillelse ved 20eC indeholder den øvre fase produktet. Man 25 fjerner opløsningsmidlet på rotationsfordamper under vakuum og kromatograferer den viskose remanens på kiselgel (Merck Art.
7733, kiselgel 60, kornstørrelse 0,2 til 0,5 mm).
Kroma tograf i.

Til 2 kg kiselgel sættes chloroform/methanol/25% ammoniak (20/ 15/1 efter rumfang) og det fyldes på en kromatograf i søj1e. Man opløser den viskose olie i 800 ml af ovennævnte opløsningsmiddelblanding og sætter råproduktet på søjlen (uopløselige bestanddele frafiltreres på forhånd). Man eluerer med løbemidler 3 ζ med stigende polaritet, indtil forureningerne er udvasket. Produktet elueres til slut med chloroform/methanol/25% ammoniak (50/50/5 efter rumfang). Oe forenede eluater inddampes på 13 DK 172954 B1 roterende fordamper, og resterende vand fjernes med toluen. Remanensen optages i 600 ml dlchlormethan og der tilsættes 4 1 acetone. De ved -20*C udskilte krystaller vaskes med kold acetone, derpå med pentan og tørres i vakuum. Udbyttet af ren he-5 xadecylphosphocholin er 250 g (ca. 70¾ beregnet på hexadecyl-glycerin).

Eksempler på farmaceutiske præparater.
Eksempel på en opløsning: 1 5 25 g 1-n-propyloxy-2,3-propandiol, 12,5 g 1 -n-hexy1oxy-1,3 - propandiol, 12,5 g l-n-nonyloxy-2,3-propandiol, 44 g vand og 1 g phenoxyethanol blandes, og 5 g hexadecy1phosphocho 1 in opløses i denne blanding. Opløsningen befris for synlige partikler ved filtrering over et egnet filter.
20 1 g opløsning indeholder 50 ml hexadecylphosphocholin.
Eksempel på en salve.
5 g af stoffet hexadecylphosphocholin suspenderes i 35 g tykt-25 flydende paraffin, 30 g emulgerende cety1 steary1 a 1 kohol og 30 g hvid vaseline tilsættes og smeltes. Smelten omrores indtil afkøling. En homogen fordeling af det virksomme stof opnås ved bearbejdning af den afkølede smelte ved hjælp af et egnet homogeniseringsapparat (f.eks. en treval semøl 1e).
30 1 g af den hydrofile salve indeholder 50 mg hexadecylphosphochol i n .
Eksempel på en emulsion: 3 5 11,83 g l-n-propyloxy-2,3-propandiol, 5,91 g 1-n-hexyloxy-2,3-propandiol, 5,91 g l-n-nonyloxy-2,3-propandiol, 20,35 g vand og 1,0 g phenoxyethanol blandes, og 5 g hexadecylphosphochol in 14 DK 172954 B1 opløses i denne blanding. På et vandbad smeltes 30 g hvid vaseline, 15 g cetylalkohol og 5 g sorbitanmonopalmitat, opvarmes til 70* C og den ligledes til 70 * C opvarmede opløsning af virksomt stof emulgeres i fedtfasen ved hjælp af et hurtigt-5 gående dispergeringsapparat. Derefter afkøles cremen til 30®C under omrøring.
1 g vand-i-olie-creme indeholder 50 ml hexadecy1phosphocho 1 in . Eksempel på kapsler.
10 1,25 kg hexadecylphosphocholin opløses i 5 kg chloroform, og i denne opløsning suspenderes 1,25 kg aerosil. Bagefter aftrækkes opløsningsmidlet i vakuum. Den tørre masse føres gennem en 1 mm sigte og tørres igen i vakuum ved 30°C for at fjerne de 15 sidste rester af opløsningsmiddel. Dette granulat bliver på kendt måde på en egnet kapse 1 mask ine fyldt i hårde gelatine-kapsler af størrelsen 00 indtil 500 mg.
En kapsel indeholder 250 mg hexadecy1phosphocho 1 in.

Eksempel på et lyofilisat: I 3 1 vand til injektionsformål bliver under skylning med nitrogen opløst 500 g mannit, 50 g hexadecy1phosphocholin dis-pergeres ved hjælp af et hurtigtgående homogeniseringsapparat 25 og med vand til injektionsformål suppleres op til 4 1. Denne mælkeagtige dispersion bliver ved ultralydbehandling eller ved hjælp af en spaltehomogenisator omdannet til et let opalise-rende kolloiddisperst system. Under aseptiske betingelser bliver nu sterilf iltreret over et membranfilter med en porevidde 30 på 0,22 pm, og der fyldes 40 ml i 100 ml injektionsf1 asker un der skylning med nitrogen. Flaskerne forsynes med frysetørringspropper og lyofiliseres i et egnet anlæg. Efter tørring skylles med steril tør nitrogen, og flaskerne lukkes i anlæg get. Propperne sikres med en ombertlet kappe.

Claims (5)
1. Antitumorvirksomt lægemiddel, kendetegnet ved, at det indeholder hexade-cylphosphocholin som virksomt stof samt eventuelt sædvanlige farmaceutiske tilsætnings- 5 stoffer, bærestoffer og/eller fortyndingsmidler.
2. Lægemiddel ifølge krav l.kendetegnet ved, a) at det som virksomt stof indeholder hexadecylphosphocholin og b) at det endvidere indeholder en eller flere alkylglyceriner med den almene formel H2C - O - R,
10 I (I) HC - O - R2 H2C - OH hvori den ene af resterne R, og R2 betyder en alkylgruppe med 3 til 12 C-atomer, og den 15 anden rest er et H-atom samt eventuelt sædvanlige farmaceutiske tilsætningsstoffer, bærestoffer og/eller fortyndingsmidler.

Fremgangsmåde til fremstilling af et (antitumorvirksomt) lægemiddel, kende-t egnet ved, at man blander hexadecylphosphocholin med et eller flere af følgende 20 stoffer: stivelse, cellulose, lactose, formalin-casein, modificeret stivelse, magnesiumste-arat, calciumhydrogenphosphat, højdispers kiselsyre, talkum og phenoxyethanol, granulerer den fremkomne blanding, eventuelt med en vandig opløsning, der som bestanddel i det mindste indeholder gelatine, stivelse, polyvinylpyrrolidon, vinylpyrrolidon-vinylacetat-copolymerisat og/eller polyoxyethylensorbitanmonooleat, eventuelt homoge-25 niserer granulatet med et eller flere af de ovennævnte hjælpestoffer og presser denne DK 172954 B1 blanding til tabletter eller fylder den i kapsler, idet tabletterne eller kapslerne pr. doseringsenhed indeholder 5 til 2000 mg hexadecylphosphocholin.

Fremgangsmåde til fremstilling af et (antitumorvirksomt) lægemiddel, kendetegnet ved, at man ved temperaturer mellem 33 og 37°C suspenderer hexade- 5 cylphosphocholin sammen med sojalecithin samt eventuelt 0,1 til 0,5 vægtdele phenoxy-ethanol (beregnet på 1 vægtdel hexadecylphosphocholin) i smeltet hårdt fedt og homogeniserer og derefter udstøber blandingen i hule celler, idet doseringsenheden indeholder 5 til 2000 mg hexadecylphosphocholin.
11. Fremgangsmåde til fremstilling af et (antitumorvirksomt) lægemiddel (salve, creme, 10 emulsion), kendetegnet ved, at man ved en temperatur mellem 50 og 120°C homogeniserer hexadecylphosphocholin, eventuelt i nærværelse af enelier flere emulgatorer og/eller 0,1 til 0,5 vægtdele phenoxyethanol (beregnet på 1 vægtdel hexadecylphosphocholin), med mindst et af følgende stoffer: paraffin, vaseline, alifatisk monocarboxylsyre med 15 til 20 C-atomer og polyoxyethylenpolyolfedtsyreester, hvor-15 efter man eventuelt emulgerer blandingen under tilsætning af en polyvalent lavere alifatisk alkohol.
12. Fremgangsmåde til fremstilling af en (antitumorvirksom) opløsning, kendetegnet ved, at man ved temperaturer mellem 30 og 100°C opløser hexadecylphosphocholin, eventuelt i nærværelse af 0,1 til 0,5 vægtdele phenoxyethanol (beregnet på 1 20 vægtdel hexadecylphosphocholin) samt eventuelt i nærværelse af en emulgator, hvorefter man eventuelt supplerer den således fremkomne opløsning op med så meget vand eller planteolie, at den endelige opløsning indeholder 0,1 til 5% hexadecylphosphocholin. 13. Fremgangsmåde til fremstilling af et lægemiddel ifølge et eller flere af de foregående krav 8-12, kendetegnet ved, at man ved fremstillingen yderligere anvender en alkylglycerin med formlen H2C - O - R,
5 I HC - O - R2 (I) H2C - OH hvori en af resterne R, og R2 er en alkylgruppe med 3 til 12 C-atomer, og den anden 10 rest betyder et H-atom, eller en blanding af sådanne alkylglyceriner samt eventuelt vand, idet man anvender 1 til 30 vægtdele alkylglycerin med formlen I henholdsvis en tilsvarende alkylglycerinblanding og eventuelt 1 til 30 vægtdele vand, hver især beregnet på 1 vægtdel hexadecylphosphocholin. 15
