Description

Ein solches Implantat kann beispielsweise aus Titanlegierung bestehen, die sehr stark und relativ leicht ist. Um eine verschleißfeste Oberfläche sicherzustellen, wurde das Vorsehen eines Titannitrid-Überzugs vorgeschlagen. Es besteht ferner die Gefahr einer Infektion, wenn solche Metallimplantate eingesetzt werden, und es wurde z.B. in
US 6 113 636 vorgeschlagen, metallisches Silber auf Metallimplantate galvanisch aufzubringen, wobei das Silber ein biocidales Material ist, das Infektionen kontrollieren kann, ohne toxische Effekte auf den Patienten auszulösen. Solche Überzüge, ob aus Titannitrid oder Silber, können infolge Korrosion aus Körperflüssigkeiten unterhöhlt werden, so daß der Überzug sich vom Implantat ablösen kann, was die Abnutzung erhöhen und Gewebeschaden verursachen kann. - Erfindungsgemäß ist ein Implantat zur Verwendung in einem chirurgischen Verfahren vorgesehen, bei dem das Implantat ein Metallsubstrat und einen Oberflächenbelag auf dem Metallsubstrat sowie aus einer Lösung ausgeschiedenes biocidales Metall aufweist, dadurch gekennzeichnet, daß der Oberflächenbelag mit dem Substrat ein Ganzes bildet, wobei er entweder:
- a) ein aus dem Metallsubstrat durch ein Anodisierungsverfahren herausgewachsener Belag ist, so daß er mit dem Substrat ein Ganzes bildet, oder
- b) eine durch Galvanisierung aufgebrachte metallische Schicht ist und dann einer Diffusionsbindung unterworfen wird, so daß der Belag mit dem Substrat zu einem Ganzen wird.
- Die Erfindung sieht außerdem ein Verfahren zur Herstellung eines solchen Implantates nach Anspruch 5 vor.
- Die Anodisierung (Eloxierung) bildet eine anhaftende Oxidschicht, obwohl, wenn sie in Phosphorsäure ausgeführt wird, dann ein Phosphat gebildet werden kann. Eine solche anhaftende Phosphatschicht kann auch modifiziert werden, um eine Hydroxyapatitschicht zu bilden, die ein Knochenwachstum stimulieren kann.
- Das biocidale Material sollte vorzugsweise mindestens 6 Wochen lang, am besten bis 6 Monate nach chirurgischem Eingriff effektiv sein, und die Entlassungsrate sollte gering sein, um toxische Auswirkungen auf Körperzellen zu vermeiden. Außerdem ist die Gesamtmenge an biocidalem Material vorzugsweise ebenfalls begrenzt, um jegliche toxische Auswirkungen zu minimieren.
- Außerdem ist es erwünscht, wenn die Oberfläche vor Erzeugung des Oberflächenbelags hochglanzpoliert wird. Dies kann beispielsweise durch Elektropolieren geschehen.
- Im Prinzip kann eine Reihe unterschiedlicher Metalle für das biocidale Metall verwendet werden. Insbesondere dann, wenn der Belag eine durch Galvanisierung aufgebrachte Metallschicht ist, muß sie selbstverständlich korrosionsbeständig sein. Gold, Platin, Iridium und Palladium wären möglicherweise geeignet, wenn auch teuer; Silber ist vorzuziehen, da es nicht besonders in Körperflüssigkeiten löslich ist, und zwar infolge des Vorhandensein von Chloridionen und der geringen Löslichkeit von Silberchlorid. Wenn der Oberflächenbelag das biocidale Metall in ionischer Form enthält, dann würde ein weiterer Bereich von Metallen möglich sein. Zusätzlich zu den bereits erwähnten Elementen könnten Kupfer, Zinn, Antimon, Blei, Wismut und Zink als Ionen, kombiniert in eine unlösliche Matrix, z.B. aus Metalloxid oder Metallphosphat, verwendet werden. Die Entlassungsrate würde in diesem Fall primär durch die Stärke der Absorption der Metallionen in der Matrix kontrolliert.
- Die Metalle, die zur Herstellung solcher prothetischen Implantate verwendet werden können, sind typischerweise eine Form von Edelstahl, eine Titanlegierung oder eine Kobalt/Chrom-Legierung, wenn auch Zirkonium ebenfalls verwendet werden könnte. Die Standardlegierungen für diesen Zweck sind Titan 90% mit 6% Aluminium und 4% Vanadium (British Standard 7252) oder Chrom 26,5–30%, Molybdän 4,5–7% und der Rest Kobalt (British Standard, Teil 4).
- Vorzugsweise wird das Implantat zunächst poliert, um eine sehr glatte Oberfläche vorzusehen. Sowohl Edelstahl (Chrom/Eisen/Nickel) als auch eine Kobalt/Chrom-Legierung kann elektropoliert werden, wobei als Elektrolyt ein Gemisch aus Phosphorsäure und Glycerin oder ein Gemisch aus Phosphorsäure und Schwefelsäure verwendet wird. Titanlegierung kann unter Verwendung von Essigsäure oder einem Gemisch von Salpeter- und Flußsäure elektropoliert werden. Alternativ kann das Implantat einer Kombination von anodischer Passivierung mit mechanischem Polieren, das als Elektroglätten (electrolinishing) bezeichnet werden kann, unterzogen werden, wobei dieser Prozeß das Oxid beseitigt, das die Oberflächenrauheit schützt, wobei die Oberfläche an diesem Punkt dann elektromechanisch repassiviert wird, und somit eine spiegelglatte Oberflächenbeschaffenheit erzeugt wird. Verschiedene Elektrolyte sind für diesen Zweck geeignet, einschließlich Salpetersäure, gemischt mit Schwefelsäure, Natriumhydroxid, Natriumphosphat, gemischt mit Natriumnitrat.
- Nach dem Polieren der Oberfläche des Metalls kann entweder Silberniederschlag oder Oberflächenumwandlung stattfinden. Betrachtet man zunächst die Oberflächenumwandlung, so kann eine Schicht aus Metalloxid oder -phosphat durch Anodisierung (Eloxieren) in einem geeigneten Elektrolyt gebildet werden, so daß die Oxid- oder Phosphatschicht sich aus der Oberfläche des Metalls herausbildet. Biocidale Metallionen können dann aus einer wäßrigen Salzlösung in die Oxid- oder Phosphatmatrix absorbiert werden, z.B. die Ionen Ag+ oder Cu++. Kationen von Palladium, Platin oder gar Ruthenium könnten in ähnlicher Weise absorbiert werden. Falls erwünscht, könnten niedergeschlagene Silber-, Platin- oder Palladiumionen dann zu Metall umgewandelt werden oder niedergeschlagene Rutheniumionen zu unlöslichem RuO2 umgewandelt werden, und zwar innerhalb des Oxid- oder Phosphat-Oberflächenbelags, wobei diese Reaktion chemisch oder elektrochemisch oder durch Licht ausgeführt wird.
- Betrachtet man nun den Silberniederschlag, so sollte die Beschichtung dünn sein, um toxische Effekte zu vermeiden. Eine hochgradige Anhaftung am darunter liegenden Metall kann sichergesellt werden durch zunächst Beseitigung der Oberflächen- Oxidschicht mittels anodischer Ätzung, gefolgt von einer kurzen Umkehr der Polarität in Gegenwart entsprechender Ionen, um so die Oberfläche mit einer dünnen Silberbeschichtung zu bedecken. Dies kann wiederholt werden, um sicherzustellen, daß keine Nadellöcher vorhanden sind. Das Plattierungselektrolyt kann Flußsäure einschließen oder kann ein alkalisches Cyanid-Elektroplattierungs-Elektrolyt sein. Nach Ablagerung sollte der Silberüberzug diffusionsgebunden werden, um so eine intermetallische Schicht zu bilden, und zwar durch Erhitzung des Implantates auf eine erhöhte Temperatur. Im typischen Fall sollte es auf über 800°C, vorzugsweise zwischen 810°C und 950°C erhitzt werden, und zwar in einer inerten Atmosphäre, z.B. aus Argon, für eine Zeitdauer zwischen 1 und 6 Stunden. Dies eliminiert im wesentlichen die Gefahr einer Beschichtungs-Abblätterung. Jedoch darf bei Implantaten auf Titanbasis die Temperatur 850°C nicht überschreiten, da Titan eine Phasenänderung von der Alpha- zur Betaform oberhalb dieser Temperatur erleiden würden.
- Anstelle von Silber können andere Metalle, wie Platin oder Palladium, elektrodeponiert und dann thermisch in ähnlicher Weise behandelt werden, um eine Zwischenmetallschicht zu bilden.
- Die Erfindung wird nun weiter und mehr im Besonderen, aber nur beispielsweise beschrieben.
- Ein Hüftimplantat wird aus Titanlegierung (Ti/Al/V) hergestellt. Das Implantat wird mit Ultraschall gereinigt, wobei zunächst Azeton als flüssige Phase verwendet wird und dann eine 1-M-wässrige Lösung aus Natriumhydroxid, und wird dann in deionisiertem Wasser gespült. Das gereinigte Implantat wird dann in eine 12 Gew.-%ige Lösung von Phosphorsäure eingetaucht und 2 Stunden lang einer Maximalspannung von 10 V und einem Maximalstrom von 10 mA/cm2 anodisiert, um so eine Oberflächen-Beschichtung aus Titanphosphat zu bilden. Es wird dann wieder in deionisiertem Wasser gespült. Die Oberfläche, die anfänglich blaßgrau ist, verändert sich zu einem dunkleren Mattgrau als Folge der Anodisierung, mit einer leicht geblichen Nuance.
- Das Implantat wird dann in eine 0,1 M wäßrige Lösung von Silbernitrat eingetaucht und 2 Stunden darin belassen. Als Folge von Ionenaustausch befindet sich schließlich einiges Silberphosphat im Titanphosphat-Überzug. Das Implantat ist dann fertig, um implantiert zu werden. Während es Körperflüssigkeiten ausgesetzt ist, wird es eine geringe Auslaugung von Silberionen aus der Phosphatschicht geben, so daß jegliche Bakterien in unmittelbarer Nähe des Implantates abgetötet werden. Eine Infektion vom Implantat her wird daher unterdrückt.
- Experimentelle Proben dieser Titanlegierung wurden gereinigt, anodisiert, um eine Schicht aus Titanphosphat zu bilden, und dann einem Ionenaustausch ausgesetzt, um Silberphosphat zu bilden, gefolgt von der oben beschriebenen Prozedur. Eine Probe wurde in direktem Tageslicht 110 Stunden lang plaziert; die belichtete Oberfläche dunkelte nach als Folge dieser Tageslicht-Einwirkung, ein Zeichen für die Bildung von Silbermetall durch Fotoreduktion. Eine andere Probe wurde in ein Lösungsmittel eingetaucht, das ein Gemisch aus 4 M Salpetersäure und 0,5 M Natriumfluorid (äquivalent zur Flußsäure), um die Beschichtung aufzulösen. Die dunkelgraue Oberflächenschicht wurde innerhalb 3 Minuten vollständig entfernt, wobei eine silbergraue Oberflächenbeschaffenheit übrig blieb. Die resultierende Lösung wurde auf Vorhandensein von Silber durch Atom-Absortions-Spektrometrie analysiert, und die Konzentration von Silber war, wie festgestellt wurde, äquivalent einer durchschnittlichen Oberflächen-Beschichtung von 73 μg/cm2.
Claims (8)
- Implantat zur Verwendung in einem chirurgischen Verfahren, wobei das Implantat ein Metallsubstrat und einen Oberflächenbelag auf dem Metallsubstrat und der Belag ein aus einer Lösung ausgeschiedenes biocidales Metall aufweist, dadurch gekennzeichnet, daß der Oberflächenbelag mit dem Substrat ein Ganzes bildet, wobei er entweder: a) ein aus dem Metallsubstrat durch ein Anodisierungsverfahren herausgewachsener Belag ist, so daß er mit dem Substrat ein Ganzes bildet, oder b) eine durch Galvanisierung aufgebrachte metallische Schicht ist und dann einer Diffusionsbindung unterworfen wird, so daß der Belag mit dem Substrat zu einem Ganzen wird.
- Implantat nach Anspruch 1, bei dem der Oberflächenbelag durch ein Anodisierungsverfahren gezüchtet ist und ein Metallphospat aufweist
- Implantat nach Anspruch 1 oder 2, bei dem der Oberflächenbelag durch ein Anodisierungsverfahren gezüchtet ist und das biocidale Material innerhalb des Oberflächenbelags absorbierte Metallionen aufweist.
- Implantat nach Anspruch 3, bei dem das biocidale Material aus Silber besteht.
- Verfahren zur Herstellung eines Implantates zur Verwendung in einem chirurgischen verfahren, wobei das Implantat ein Metallsubstrat aufweist, wobei das Verfahren die Bildung eines Oberflächenbelags auf dem Substrat sowie das Einverleiben eines biocidalen Metalls im Belag durch Niederschlag aus einer Lösung umfaßt, dadurch gekennzeichnet, daß der Belag gebildet wird entweder: a) durch Züchtung des Oberflächenbelags aus dem Metallsubstrat durch ein Anodisierungsverfahren, so daß der Belag mit dem Substrat ein Ganzes bildet; oder b) durch Niederschlagen des Belags mittels Galvanisierung und dann durch Aussetzten des Belags und des Substrats einer Diffusionsbindung, so daß der Belag mit dem Substrat ein Ganzes wird.
- Verfahren nach Anspruch 5, bei dem der Belag durch ein Anodisierungsverfahren (galvanisch) gebildet wird und bei dem Silberionen dem Oberflächenbelag durch Kontakt mit einer Silberionen enthaltenen Lösung einverleibt werden.
- Verfahren nach Anspruch 5, bei dem der Belag durch Galvanisierung aufgebracht wird und bei dem vor dem Niederschlagen des Belags die Oberfläche einem elektrolytischen Ätzen ausgesetzt wird.
- Verfahren nach einem der Anspruch 5 bis 7, bei dem die Oberfläche des Implantates vor Erzeugung des Oberflächenbelages hochglanzpoliert wird.
