Abstract
Un implante para uso en un procedimiento quirúrgico, comprendiendo el implante un substrato de metal y una capa superficial sobre el substrato de metal, incorporando la capa un metal biocida depositado a partir de una solución, caracterizado porque la capa superficial es integral con el substrato, de tal manera que a) o bien se desarrolla una capa a partir del substrato de metal por un proceso de anodización, de manera que es integral con el substrato; o b) se deposita una capa metálica por electrogalvanizado, y luego se somete a adhesión por difusión, de tal manera que la capa se vuelve integral con el substrato.

Esta invención se refiere a implantes de metal
para uso en procedimientos quirúrgicos y, en particular, para la
introducción de un material biocida dentro de tales implantes para
suprimir o controlar la infección.
Varios procedimientos quirúrgicos requieren el
uso de implantes. Por ejemplo, el hueso canceroso se puede retirar,
en cirugía protésica, se puede sustituir por un implante metálico.
Un implante de este tipo puede ser, por ejemplo de aleación de
titanio, que es muy robusto y relativamente ligero. Para asegurar
una superficie de desgaste dura, se ha sugerido la provisión de un
revestimiento de nitruro de titanio. Además, existe un riesgo de
introducción de infección cuando se implantan tales implantes de
metal y ha sido sugerido, por ejemplo, en el documento US 6 113 636,
que la plata metálica se puede galvanizar sobre los implantes
metálicos, siendo la placa un material biocida que puede controlar
la infección sin provocar efectos tóxicos al paciente. No obstante,
tales revestimientos, ya sean de nitruro de titanio o de plata, se
pueden rebajar debido a corrosión a partir de fluidos corporales,
de manera que el revestimiento se puede separar desde el implante,
que puede incrementar el desgaste y provocar daño en el tejido.
De acuerdo con la presente invención se
proporciona un implante para uso en un procedimiento quirúrgico,
comprendiendo el implante un substrato de metal y una capa
superficial sobre el substrato de metal, incorporando la capa un
metal biocida depositado a partir de una solución, caracterizado
porque la capa superficial es integral con el substrato, de tal
manera que
- a)
- o bien se desarrolla una capa a partir del substrato de metal por un proceso de anodización, de manera que es integral con el substrato; o
- b)
- se deposita una capa metálica por electrogalvanizado, y luego se somete a adhesión por difusión, de tal manera que la capa se vuelve integral con el substrato.
La invención proporciona también un método de
producción de un implante de este tipo, como en la reivindicación
5.
La anodización forma una capa de óxido adherente,
aunque si se realiza en ácido fosfórico, entonces se puede formar un
fosfato. Una capa de fosfato adherente puede ser modificada también
para formar una capa de hidroxiapatita, que puede estimular el
crecimiento óseo.
El material biocida debería ser efectivo con
preferencia durante al menos 6 semanas, con preferencia durante 6
meses después de la cirugía, y la tasa de liberación debería ser
baja para evitar efectos tóxicos sobre las células del cuerpo.
Además, la cantidad total de material biocida está limitada con
preferencia también para reducir al mínimo efectos tóxicos.
También es deseable que la superficie esté
altamente pulida antes de la producción de la capa superficial. Esto
se puede conseguir, por ejemplo, por electropulido.
En principio, se puede utilizar un rango de
materiales diferentes para el metal biocida. En particular, si la
capa es una capa de metal depositada por electrogalvanizado,
entonces debe ser claramente estable frente a la corrosión. Se
podrían utilizar potencialmente oro, platino, iridio y paladio,
aunque son costosos; se prefiere lata, ya que no es particularmente
soluble en fluidos corporales debido a la presencia de iones de
cloruro y a la baja solubilidad del cloruro de plata. Si la capa
superficial contiene el metal biocida en forma iónica, entonces
sería posible una gama más amplia de metales. Además de los
elementos ya mencionados, se podrían utilizar cobre, estaño,
antimonio, plomo, bismuto y cinc como iones combinados en una matriz
insoluble, por ejemplo, de óxido de metal o de fosfato de metal. La
tasa de liberación sería controlada, en este caso, principalmente
por la resistencia de la absorción de los iones de metal en la
matriz.
Los metales que se pueden utilizar para realizar
tales implantes protésicos son típicamente una forma de acero
inoxidable, una aleación de aluminio, o una aleación de cobalto /
cromo, aunque se podría utilizar zirconio. Las aleaciones estándar
para esta finalidad son 90% de titanio con 6% de aluminio y 4% de
vanadio (Norma Británica 7252), o cromo 26,5-30%,
molibdeno 4,5-7% y el resto cobalto (Norma Británica
7252, Parte 4).
Con preferencia, el implante es pulido
inicialmente para proporcionar una superficie muy lisa. Tanto la
aleación de acero inoxidable (cromo / hierro / níquel) como también
la aleación de cobalto / cromo se pueden
electro-pulir utilizando como electrolito una mezcla
de ácido fosfórico y glicerina, o una mezcla de ácido fosfórico y
ácido sulfúrico. La aleación de titanio puede ser
electro-pulida utilizando ácido acético, o una
mezcla de ácidos nítrico y fluorhídrico. De un a manera alternativa,
los implantes se pueden someter a una combinación de pasivación
anódica con pulido mecánico, que se puede referir como
electropulido, retirando este proceso el óxido que protege la
rugosidad superficial, siendo re-pasivada entonces
la superficie en ese punto electroquímicamente, produciendo de esta
manera un acabado liso como espejo. Varios electrolitos son
adecuados para este fin, incluyendo ácido nítrico mezclado con
ácido sulfúrico, hidróxido de sodio, fosfato de sodio o hidróxido de
sodio mezclado con nitrato de sodio.
Después del pulido de la superficie del metal,
puede tener lugar la deposición de la plata o la conversión de la
superficie. Considerando en primer lugar, la conversión de la
superficie, se puede formar una capa de óxido o fosfato de metal por
anodización en un electrolito adecuado, de manera que la capa de
óxido o fosfato se forma a partir de la superficie del metal. Los
iones de metal biocidas pueden ser absorbidos entonces a partir de
una solución de sal acuosa en la matriz < de óxido o fosfato, por
ejemplo los iones de Ag^{+} o de CU^{++}. Los cationes de
paladio, platino o incluso rutenio podrían ser absorbidos de una
manera similar. Si se desea, los iones depositados de plata, platino
o paladio pueden convertirse entonces en metal, o los iones
depositados de rutenio pueden convertirse en RuO_{2} insoluble,
dentro del revestimiento superficial de óxido o de fosfato, sien do
realizada esta reacción química o electroquímicamente o
por luz.

Considerando ahora la deposición de plata, el
revestimiento debería ser fino para prevenir efectos tóxicos. Se
puede asegurar un alto grado de adherencia al metal subyacente
retirando en primer lugar la capa de óxido de la superficie a
través de decapado anódico, seguido por una inversión breve de la
polaridad en presencia de iones adecuados, para cubrir la superficie
con un revestimiento fino de plata. Esto se puede repetir para
asegurar que no existen picaduras. El electrolito de revestimiento
puede incluir ácido clorhídrico o puede ser un electrolito de
electrogalvanizado de cianuro alcalino. Después de la deposición, el
revestimiento de plata debería adherirse por difusión para formar
una capa intermetálica, calentando el implante hasta una
temperatura elevada. Típicamente, debería calentarse hasta por
encima de 800ºC, con preferencia entre 810ºC y 950ºC, en una
atmósfera inerte, por ejemplo, de argón durante un periodo de tiempo
entre 1 y 6 horas. Esto elimina substancialmente el riesgo de
delaminación del revestimiento. No obstante, con implantes basados
en titanio, no debe excederse una temperatura de 850ºC, ya que el
titanio sería sometido a un cambio de fase de forma alfa a beta por
encima de esta temperatura.
En lugar de plata, se pueden
electro-depositar otros metales tales como platino o
paladio y luego se pueden tratar térmicamente de una manera similar
para formar una capa intermetálica.
A continuación se explicará la invención
adicionalmente y más en particular, solamente a modo de ejemplo.
Se fabricó un implante de cadera de aleación de
titanio (Ti/Al/V). El implante se limpió por ultrasonido utilizando
en primer lugar acetona como la fase líquida y luego una solución
acuosa 1 M de hidróxido de sodio, y luego se aclaró en agua
desionizada. El implante limpio se sumergió entonces en una solución
al 12% en peso de ácido fosfórico, y se anodizó en el transcurso de
2 horas a una tensión máxima de 10 V y una corriente máxima de 10
mA/cm^{2}, para formar un revestimiento superficial de fosfato de
titanio. Luego se aclaró de nuevo en agua desionizada. La
superficie, que era inicialmente gris pálida, se volvió gris mate
más oscuro como consecuencia dela anodización, con una tonalidad
ligeramente amarilla.
El implante fue sumergido entonces en una
solución acuosa agitada 0,1 M de nitrato de plata, y se dejó estar
en el transcurso de 2 horas. Como resultado del intercambio de iones
existía como consecuencia de ello cierta cantidad de fosfato de
plata en el revestimiento de fosfato de titanio. El implante estaba
entonces preparado para ser implantado. Durante la exposición a los
fluidos corporales, se produce una lixiviación lenta de los iones de
plata desde la capa de fosfato, de manera que se aniquilaron las
bacterias presentes en la proximidad inmediata del implante. Por lo
tanto, se suprimió la infección resultante del implante.
Muestras experimentales de esta aleación de
titanio fueron limpiadas, anodizadas para formar una capa de fosfato
de titanio, y luego fueron sometidas a intercambio de iones ara
formar fosfato de plata, seguido por el procedimiento descrito
anteriormente. Se colocó una muestra en luz diurna directa en el
transcurso de 110 horas; la superficie expuesta se oscureció como
resultado de esta exposición a la luz diurna, indicando la formación
de metal de plata por foto-reducción. La otra
muestra fue sumergida en un disolvente que contenía una mezcla de
ácido nítrico 4 M y fluoruro de sodio 0,5 M (equivalente a ácido
fluorhídrico) para disolver el revestimiento. El revestimiento de la
superficie gris oscura se eliminó completamente dentro de 3 minutos,
dejando un acabado gris plata. La solución resultante fue analizada
para determinar la presencia de plata por espectrometría de
absorción atómica, y se encontró que la concentración de plata era
equivalente a una carga media superficial
de 73 \mug/cm^{2}.

Claims (8)
1. Un implante para uso en un procedimiento
quirúrgico, comprendiendo el implante un substrato de metal y una
capa superficial sobre el substrato de metal, incorporando la capa
un metal biocida depositado a partir de una solución,
caracterizado porque la capa superficial es integral con el
substrato, de tal manera que
- a)
- o bien se desarrolla una capa a partir del substrato de metal por un proceso de anodización, de manera que es integral con el substrato; o
- b)
- se deposita una capa metálica por electrogalvanizado, y luego se somete a adhesión por difusión, de tal manera que la capa se vuelve integral con el substrato.
2. Un implante según la reivindicación 1, en el
que la capa superficial es desarrollada por un proceso de
anodización y comprende un fosfato metálico.
3. Un implante según la reivindicación 1 ó 2, en
el que la capa superficial se desarrolla por un proceso de
anodización y el material biocida comprende iones metálicos
absorbidos dentro de la capa superficial.
4. Un implante según la reivindicación 3, en el
que el material biocida comprende plata.
5. Un método de fabricación de un implante para
uso en un procedimiento quirúrgico, comprendiendo el implante un
substrato de metal y comprendiendo el método formar una capa
superficial sobre el substrato de metal, e incorporar un metal
biocida en la capa por deposición a partir de una solución,
caracterizado porque la capa se forma
- a)
- o bien desarrollando la capa superficial a partir del substrato de metal por un proceso de anodización, de manera que es integral con el substrato; o
- b)
- depositando la capa metálica por electrogalvanizado, y luego sometiendo la capa y el substrato a adhesión por difusión, de tal manera que la capa se vuelve integral con el substrato.
6. Un método según la reivindicación 5, en el que
la capa se forma por un proceso de anodización, y en el que se
incorporan iones de plata en la capa superficial por contacto con
una solución que contiene iones de plata.
7. Un método según la reivindicación 5, en el que
la capa se deposita por electrogalvanizado, y en el que antes de la
deposición de la capa, se somete la superficie a decapado
anódico.
8. Un método según una cualquiera de las
reivindicaciones 5 a 7, en el que la superficie del implante es
altamente pulida antes de la generación de la capa superficial.
