種々の外科的処置には、インプラントの使用が必要である。補綴外科手術において、例えば、癌性の骨は、除去して、金属製インプラントによって置換され得る。そのようなインプラントは、例えば、チタン合金でもよく、この合金は、非常に丈夫で比較的軽い。耐磨耗性の表面を保証するために窒化チタンコーティングが提案されてきている。さらに、そのような金属製インプラントを移植する場合、感染を導入する危険が存在し、また金属銀を金属製インプラント上に電気メッキし、この銀が、患者に毒作用を起こさずに感染を制御できる生物致死性材料であることが示唆されている。しかし、そのようなコーティングは、窒化チタン又は銀のいずれにせよ、体液からの腐食作用のため効果が低下し得、そのため、このコーティングは、インプラントから剥離するかもしれず、これは、摩耗を増加させ、また組織損傷を引き起こしうる。
本発明に従えば、外科的処理に使用されるインプラントが提供され、このインプラントは、金属支持体及びこの金属支持体と一体化している表面層を含み、この層は、溶液から析出した生物致死性金属を配合している。
そのように一体化した表面層は、金属自体からこの層を成長させることによって、例えば、陽極酸化法又は上記とは別に、この層を析出させることによって、例えば、電気メッキによってその後、拡散接合によって生成してもよく、故に、この層は、インプラントの金属と一体化する。陽極酸化によって、付着性の酸化物層が形成されるが、もしリン酸中で実施した場合、ホスフェートが形成されるだろう。さらにそのような付着性のホスフェート層は、変性されて、ヒドロキシアパタイト層を形成し得、これは、骨成長を刺激し得る。
生物致死性材料は、外科手術の後、好ましくは、少なくとも6週間、好ましくは、6ヶ月まで有効であり、また放出速度は、体細胞へ毒作用を避けるために低くすべきである。さらに、生物致死性材料の総量も、好ましくは、すべての毒作用を最小化するために制限されている。
さらに、表面が、表面層の形成の前に高度に研磨されていれば好ましい。これは、例えば電解研磨によって達成されうる。
基本的には、様々な異なる金属が、生物致死性金属に使用され得る。特に、この層が電気メッキによって析出された金属層であれば、それは明らかに腐食に安定であるにちがいない。金、白金、イリジウム及びパラジウムは、潜在的に好適であろうが、高価であり、銀は、塩化物イオンの存在及び塩化銀の低溶解性のため特に体液に溶解しないので好ましい。表面層が、イオン形態で生物致死性金属を含むならば、より広範囲の金属が見込まれるだろう。すでに記載した元素に加えて、銅、スズ、アンチモン、鉛、ビスマス及び亜鉛が、例えば金属酸化物又は金属ホスフェートの不溶性マトリックスに組込まれたイオンとして使用してよい。この場合、放出速度は、主にマトリックス中の金属イオン吸収の強度によって制御され得る。
好ましくは、インプラントには、最初に、研磨して、非常に滑らかな表面を提供する。ステンレス鋼(クロム/鉄/ニッケル)及びコバルト/クロム合金は、ともに、電解質としてリン酸及びグリセリンの混合物、又はリン酸及び硫酸の混合物を使用して、電解研磨され得る。チタン合金は、酢酸、又は硝酸及びフッ化水素酸の混合物を使用して電解研磨され得る。上記とは別に、インプラントは、陽極の不動態化と機械研磨との組合せに付してよく、これは、電気リニシング(electrolinishing)と言うことができ、この処理は、表面のでこぼこを保護する酸化物を除去し、その時点で、電気化学的に再不動態化され、鏡のように滑らかな仕上がりを提供する。種々の電解質は、この目的にとって好適であり、硫酸と混合した硝酸、水酸化ナトリウム、リン酸ナトリウム、又は硝酸ナトリウムと混合した水酸化ナトリウムを含む。
次に銀析出を検討すると、コーティングは、毒作用を妨げるために薄くすべきである。表面を銀の薄いコーティングで覆うために、下に位置する金属に対する高度な付着力は、まず表面酸化物層を陽極腐蝕法によって除去することにより保証され、その後に適当なイオンの存在下極性の短い逆転が続く。これは、塗膜の穴がないことを保証するために繰り返してよい。メッキ電解質は、フッ化水素酸を含んでよく、又はアルカリ性シアン化物電気メッキ電解質でもよい。析出の後、銀コーティングは、金属内(inter-metallic)層を形成するためにインプラントを高温で加熱することによって拡散接合すべきである。典型的に、それは、800℃より上で、好ましくは、810℃と950℃の間で、不活性雰囲気下、例えばアルゴンで、約1〜6時間加熱されるべきである。これは、実質的にコーティングの表層剥離の危険を排除する。しかし、チタンベースのインプラントで、チタンは、850℃を上回るとα型からβ型へ相変化するので、温度は上記温度を超えないようにすべきである。
銀の代わりに、他の金属、例えば、白金又はパラジウムは、金属内層を形成するために、電着され、及びその後類似の方法で熱的に処理してもよい。
本発明は、実施例のみによりさらにかつより具体的に記載される。
その後、インプラントを、攪拌された0.1M硝酸銀水溶液に浸し、2時間そのままにする。イオン交換の結果として、チタンホスフェートコーティング中にいくらかの銀ホスフェートが存在する。その後インプラントを移植する準備をする。体液への暴露の間、ホスフェート層から銀イオンがゆっくり浸出し、インプラントの直近にあるすべての細菌を殺す。従ってインプラントから生じる感染は、抑制される。
このチタン合金の実験サンプルを清浄化し、陽極酸化してチタンホスフェート層を形成し、その後イオン交換して銀を形成する。一つのサンプルを直接日光下に110時間置き、露出された表面は、この日光露出の結果、黒みを帯び、光反応によって金属銀の形成を示す。他のサンプルを4M硝酸及び0.5Mフッ化ナトリウムの混合物を含む溶媒(フッ化水素酸と等価である。)に浸漬し、コーティングを溶かす。暗灰色の表面コーティングを3分以内で完全に除去し銀白色仕上げを残した。得られた溶液を原子吸光分析法によって銀の存在について分析し、銀濃度が平均的表面配合量が73μg/cm2に相当すると分かった。
Claims (15)
- 外科的処置で使用するインプラントであって、前記インプラントは、金属支持体及び前記金属支持体上の表面層を含み、前記表面層が、陽極酸化処理によって前記金属支持体から成長させることで前記支持体と一体化しており、かつ前記表面層が、生物致死性金属が前記表面層から放出できるように、かつ生物致死性金属の量及びその放出速度が前記外科的処置後の感染を抑制するのに有効であるように、溶液から析出させたイオン形態の生物致死性金属を配合したことを特徴とするインプラント。
- 前記表面層が、金属酸化物及び金属ホスフェートからなる群から選択される材料を含む、請求項1に記載のインプラント。
- 前記生物致死性金属が、イオン交換により前記表面層内に吸収された金属イオンからなる、請求項1又は2に記載のインプラント。
- 前記生物致死性金属が、銀からなる請求項1〜3のいずれか一項に記載のインプラント。
- 前記生物致死性金属が、外科的処置後少なくとも6週間、感染を抑制するのに有効である、請求項1〜4のいずれか一項に記載のインプラント。
- 前記金属支持体が、90%のチタン、6%のアルミニウム及び4%のバナジウムからなるチタン合金を含む、請求項1〜5のいずれか一項に記載のインプラント。
- 外科的処置で使用するインプラントの製造方法であって、前記インプラントが、金属支持体を含み、かつ前記方法が、以下の一連の工程:
・陽極酸化処理により層を成長させることによって、前記金属支持体と一体化している表面層を前記支持体上に形成する工程;
・前記支持体上の前記表面層をすすぐ工程;及び
・前記表面層に生物致死性金属イオンが吸収されるように、生物致死性金属イオンを含有する溶液と前記支持体上の前記表面層を接触させる工程;
を含むことを特徴とする方法。 - 前記生物致死性金属イオンが、銀、銅、パラジウム、白金及びルテニウムイオンからなる群から選択される、請求項7に記載の方法。
- 前記生物致死性金属イオンが、銀イオンからなる、請求項8に記載の方法。
- 前記溶液が、硝酸銀水溶液である、請求項9に記載の方法。
- 前記表面層が、金属酸化物又は金属ホスフェートを含む、請求項7〜10のいずれか一項に記載の方法。
- 前記陽極酸化処理が、リン酸溶液を使用して行われる、請求項7〜11のいずれか一項に記載の方法。
- 前記表面層を形成する前に、前記インプラントを研磨する工程を含む、請求項7〜12のいずれか一項に記載の方法。
- 前記研磨が、前記インプラントを陽極の不動態化と機械研磨との組合せに付すことを含む、請求項13に記載の方法。
- 前記金属支持体が、チタン合金で作られている、請求項7〜14のいずれか一項に記載の方法。
