Description
Die Erfindung betrifft ein Element zur Implantation in
Knochengewebe, bestehend aus einem biologisch verträglichen,
porösen Material, welches das Auftreten von störenden elek
trischen Potentialdifferenzen und galvanischen Strömen im
Implantationsbereich verhindert, wobei das Element eine aus
Dellen bestehende Oberflächenstruktur aufweist, die Anlage
rungspunkte für aus terminalen Collagenfasergruppen, Kohle
hydratgruppen des Collagens und Collagenfasern mit Knochen
mineralien bestehende Grenzzonenbildner besitzt.
Bei einem bekannten Element dieser Art (DE-PS 837 294, Fig. 8/9),
bei dem allerdings die Porösität des Implantatmaterials
nicht erwähnt ist, weisen die Dellen schätzungsweise
einen Durchmesser in der Größenordnung von einem Millimeter
auf. Wie die Praxis gezeigt hat, ist der Verbund Knochen/
Implantat hierbei gänzlich unbefriedigend.
Bei anderen bekannten Implantatelementen (Chemie-Ing.-Techn.
47. Jahrg. 1975/Nr. 8, Seiten 327 bis 333) ist die Ober
flächenstruktur nicht von Dellen, sondern von Poren gebil
det, die einen Durchmesser zwischen 200 und 500 µm auf
weisen. Die offenen Poren bilden ein verzweigtes Kanalsystem,
in das das Knochengewebe hineinwächst, so daß das Implantat
schließlich von Knochengewebe durchwachsen ist. Wie Unter
suchungen der Scherfestigkeit ergeben haben, ist jedoch der
Verbund Knochen/Implantat noch verbesserungsfähig. Letzteres
gilt auch für ein bekanntes Implantatelement mit poröser
Oberflächenstruktur (US-PS 38 08 606), bei dem die Poren zu
einem Drittel einen Durchmesser von 50 bis 200 µm, zu einem
weiteren Drittel einen Durchmesser von 200 bis 800 µm und
zum letzten Drittel einen Durchmesser von 800 bis 3500 µm
aufweisen; die kleineren Poren sollen das Implantat nur am
Anfang fixieren helfen, bis die mittleren und großen Poren
durchwachsen sind.
Auch ist die Fachwelt der Ansicht, daß die Ausbildung von
regulärem Knochengewebe in den Porenräumen eines implantier
ten Werkstoffes nur dann vollständig abläuft, wenn die
Porendurchmesser einen bestimmten minimalen Wert von bei
spielsweise 150 µm nicht unterschreiten (DE-AS 22 42 867,
Spalte 2, Zeilen 9-24). Das steht in Übereinstimmung mit
den obigen Zahlenangaben.
Der Erfindung liegt die Aufgabe zugrunde, ein Element der
eingangs genannten Art anzugeben, welches verbesserte Eigen
schaften hinsichtlich der Annahme des Trägerelementes, ins
besondere eine verbesserte Haltbarkeit und ein gesundes Ein
wachsen aufgrund seiner biologischen Qualität verspricht.
Die erfindungsgemäße Lösung dieser Aufgabe besteht darin,
daß die Dellen einen Durchmesser aufweisen, der im Bereich
von 10 nm bis 1000 nm liegt und kleiner ist, als es dem
Zellendurchmesser des umgebenden Gewebes entspricht.
Nach bevorzugter Ausführungsform beträgt der Dellendurch
messer 10 m bis 300 nm. Als poröses Material eignet sich
Titan besonders gut. Hierbei empfiehlt es sich, Titan mit
einem Sauerstoffgehalt einzusetzen, der von einer die Dellen
aufweisenden Titanoxidoberfläche nach innen hin abnimmt.
Günstig ist es auch, wenn die Oberfläche des Elementes im
wesentlichen aus einer feinporigen Rutilschicht besteht.
Die Erfindung geht hierbei von der Erkenntnis aus, daß eine
Implantatoberfläche aus Dellen, d. h. muldenförmigen Ver
tiefungen, deren Durchmesser um Größenordnungen kleiner als
der der bisherigen Dellen und Poren ist, überraschenderweise
zu einer wesentlich besseren Implantatfixierung führt. Tat
sächlich bewirkt die Abstimmung der Dellen auf die Zellen
größe des Gewebes ein Aufwachsen des Gewebes auf das Implan
tat. Hierzu ist im einzelnen folgendes auszuführen:
Eine Analyse von Kiefereinpflanzungen in homo hat unerwar
teter Weise gezeigt, daß die erzielten Operationsergebnisse
merklich verbessert werden, wenn die Größe der Mikrodellen
an die Größenordnung des Zellendurchmessers im umliegenden
Gewebe herankommt. Die normale anwendbare Zellgröße liegt
im Bereich von 1000 nm. Das biooptimale Ergebnis wird jedoch
erzielt, wenn der Dellendurchmesser innerhalb des Bereiches
von 10 bis 300 nm gewählt wird. Material, das für Prothesen
von Verbindungsmaterial verwendet werden kann, muß daher
eine mit Mikrodellen versehene Oberfläche haben. Die Ober
fläche zwischen lebenden, einwachsenden Zellen und Zellaus
läufern soll nämlich im Verlauf des Mineralisierungspro
zesses, zusammen mit dem gefurchten Material und den Laminaten
mit Collagenfibrilen, eine solch feste und ausgedehnte
Grenzzone zu den Verbindungsmitteln bilden, daß dadurch eine
verstärkte und unlösbare Verankerung im lebenden Knochen
gewebe erzielt wird. So wird eine Lösung des Implantates aus
der Grenzzone unmöglich. Bei Experimenten wurde gefunden,
daß ein Bruch mit Abstand von den gebildeten Grenzzonen
stattfindet.
Das Material für das Verbindungsmaterial muß also eine
Oberflächenstruktur aufweisen, bei der der Durchmesser der
Dellen 10 bis 1000 nm beträgt, wobei die Obergrenze von
1000 nm dem Zelldurchmesser entspricht, und bevorzugt 10 bis
300 nm, wobei die Obergrenze von 300 nm einer Querdimension
eines Gewebezellenausläufers und eines Knochengewebefibrils
entspricht.
Die mineralisierten Bestandteile des Knochengewebes sind
insbesondere Hydroxyapatit mit einer Zusammensetzung von
Ca₁₀(PO₄)₆(OH)₂. Diese allgemeine Formel für die kleinste
übereinstimmende Moleküleinheit entspricht einer Einheits
länge von etwa 5 bis 20 nm. Die mineralisierten Teile des
Knochengewebes enthalten auch eine gewisse Menge an Mg2+,
Na⁺ und K⁺ sowie CO₃²2-, Cl⁻, F⁻ und Zitronensäure. Vom
mineralogischen Standpunkt aus betrachtet bestätigt das die
guten Erfahrungen bei der Operation von Implantaten mit
einem Dellendurchmesser von 10 bis 300 nm.
Die P-Atome in PO₄3- sind tetraedrisch angeordnet und bilden
ein dreidimensionales, deformiertes Gitter, in welchem die
PO₄3--Ionen zu einem Großteil durch Wasserstoffbrückenbin
dungen mit dem umgebenden Material mineralisierten und
lebenden Gewebes, bestehend aus Proteinen und anderen Mem
bransubstanzen, wie Kohlenhydrate, oder Fettsubstanzen, ver
bunden sind.
Die in der Größenordnung der Zelldurchmesser liegende Ober
grenze und die niedrigere, bevorzugte Dellendurchmesserober
grenze entspricht den Ausmaßen der Zellausläufer und Kno
chengewebsfibrilen mit den obigen Mineralkomponenten.
Die Bildung von neuem Knochengewebe findet von der Ober
fläche des Knochengewebes her durch das lebende Zellsystem
statt, wobei die interzelluläre Matrix aus Collagenfasern
und anorganischen Komponenten besteht.
Die anorganischen Komponenten im resultierenden Knochen
machen ²/₃ seines Gewichts aus. Davon macht Kalziumphosphat
85% und Kalziumcarbonat etwa 10% aus. Die Collagenfasern
tragen stark zur Festigkeit und Elastizität des Knochenge
webes bei. Alle Knochenzellen sind vom selben Grundtyp und
machen, entsprechend dem Mineralisierungsausmaß, Verände
rungen von Osteoblasten über Osteocyten zu Osteoklasten
durch. Osteoklasten variieren in ihrer Erscheinung von
quaderförmiger bis pyramidenförmiger Gestalt und sind in
kontinuierlichen Schichten orientiert. Das Zellplasma ist
gekennzeichnet durch die Gegenwart von Ribonukleoprotein,
welches Proteinkomponenten für die Knochenmatrix syntheti
siert. Die Osteoblasten enthalten das Enyzm alkalische
Phosphatase, welches nicht nur die Matrix bildet, sondern
auch für den Mineralisierungsprozeß verantwortlich ist. Die
Osteoblasten werden dadurch in Osteocyten, nämlich voll
entwickelte Knochenzellen, im Rahmen der Knochenmatrix über
geführt. Das Zellplasma der Osteoblasten enthält Fett, eine
gewisse Menge an Glycogen und feine Körnchen. Ein direkter
Kontakt zwischen benachbarten Osteoblasten erfolgt während
des Knochenbildungsprozesses. Der Prozeß konserviert ein
hochenwickeltes Kanalsystem für den Austausch von Meta
boliten zwischen den Knochenzellen und dem Gefäßbett (dem
Blutkreislauf).
Die Osteoklasten sind ein Knochenzelltyp mit stark unter
schiedlichen Größen, welcher in der Knochenoberfläche gefun
den werden kann und aus sogenannten Zellhaufen, Riesenzellen
mit mehreren Kernen, bestehen. Das umliegende Knochengewebe
erscheint oft, als wäre es teilweise entmineralisiert, und
es wird daher angenommen, daß dieser Zelltypus eine Phase in
der Knochenresorption während der Wiederaufbauphase dar
stellt.
In der Knochenmatrix werden die Osteocollagenfasern mittels
einer im wesentlichen aus Mucopolysacchariden (Chondroitin
sulfat) bestehenden Zementsubstanzen zusammengehalten. Die
Mineralsubstanzen des Knochens sind in ihrem Vorkommen
ausschließlich auf den Zement zwischen den Fasern beschränkt
und bestehen im wesentlichen aus Kalziumphosphatkristallen
des Hydroxyapatitkristalltyps in Form von hochdichten Teil
chen, die senkrecht zu den Osteocollagenfasern angeordnet
sind. Die Knochenmatrix ist in Laminatform angeordnet, wobei
die Plättchen eine Dicke von 3 bis 7 µm haben. Die
Collagenfasern haben Querrillen in einem wiederkehrenden
Abstand von 68 mm zwischen den Fasern. Das bestätigt die
Richtigkeit der Anwendung des Materialsubstitutes, welches
als Trägerelement oder Ersatz für verletztes Gewebe auszu
wählen ist.
Die angewendeten mechanischen bzw. metallurgischen Produk
tionsverfahren sollten derart sein, daß sowohl die Anzahl an
Mikrodellen als auch der Dellendurchmesser im verwendeten
Material sogar eine möglichst große Gesamteinwachsoberfläche
zwischen Implantat und neu geformten Gewebe ergeben. Es
wurde auch als vorteilhaft befunden, das Implantat mit
großen Durchlöcherungen zu versehen, damit das Knochengewebe
durchwachsen kann.
Weiter sollen die Verbindungsmittel aus biologisch fehler
freiem Material hergestellt werden, welches nicht zu uner
wünschten Unterschieden im elektrischen Potential oder zu
galvanischen Strömen in der Implantationsgegend führen kann.
Es wurde gefunden, daß bestimmte Materialien, wie Titan,
Titanlegierung, keramisches Material oder mit keramischem
Material bedecktes Metall eine akzeptable biologische In
tegration gewährleisten.
Von den verschiedenen Materialien, die als Ausgangsmateria
lien für optimal integrierbare Verbindungsmittel gewählt
werden können, wird angenommen, daß Titan wegen seiner Nähe
zu Ca im Periodensystem und wegen seiner chemischen Trägheit
ein äußerst geeignetes Basismaterial, sowohl als Befesti
gungsmittel und Prothese, als auch als Ersatz für weiches
Gewebe darstellt. Experimente haben gezeigt, daß kein Titan
aus unlegiertem, in verschiedenen Gewebetypen operativ ein
gepflanzten Titan in das umliegende Gewebe eingedrungen ist.
Das für Osteointegration zu verwendende Titanmaterial sollte
vorzugsweise vollkommen unlegiert sein, zumindest in der
Grenzzone zwischen lebendem Gewebe und implantiertem Titan.
Die Bedeutung der Oberflächenstruktur des Titanmaterials ist
oben hervorgehoben, und bei Titan wird die Oberflächenstruk
tur durch Einkerben der Oberfläche mit einer Schneidege
schwindigkeit von weniger als 20 m/min unter Luft- oder
anderer Sauerstoffgaskühlung, wie O₂- oder stabilisierter
Wasserstoffsuperoxidkühlung, bewirkt. Die Schneidgeschwin
digkeit wird niedrig gehalten, um dem Prozeß genügend Zeit
für die Oxidierung des Arbeitsstückes zu geben. Die Oxidie
rung umfaßt die Bildung von Titanoxiden, die wegen ihrer
mineralogischen Eigenschaften die Bildung einer biologisch
geeigneten Oberflächenstruktur des Mikrodellentyps (Mond
oberflächentyps) fördern.
Selbstverständlich kann die Oberfläche auch auf andere Weise
erzeugt werden, wie durch Sintern der Metallaufdampfung
unter Inertgas. Die wirtschaftlichste Methode scheint jedoch
die Herstellung mit einem Schneidwerkzeug zu sein. Die Ober
fläche kann auch nur mit anderen biologischen, negativ ge
ladenen Ionen, wie Phosphaten (Phosphatieren), Phosphaten
+ O₂ und anderen Kombinationen mit kleinen Sulphat-,
Fluorid-, Chloridmengen und einer gewissen basischen Karbo
nisierung, keramisiert werden.
Bei Lagerung nimmt Titan (und auch niedriglegiertes Titan
nach Bearbeitung) rasch eine oxidierte Oberfläche an, die
einer kristallisierten Oberfläche von Rutil, TiO, gleich
kommt, worauf der Sauerstoffgehalt des Titans von der Ober
flächenschicht nach innen sinkt, bis der Sauerstoffgehalt
einen Wert von 0 erreicht.
Der deformierte Knochenapatit tendiert zur Bildung von star
ken Bindungen über seine monoklin und tetraedrisch (hexago
nal) facettierte Struktur mit dem feinporigen Rutilüberzug
des Titanmaterials, da das kubische Gitter des Rutils und
das hexagonale Gitter des Knochenapatits hervorragend zu
sammenpassen.
Die experimentelle Erfahrung bestätigt die Notwendigkeit
einer solchen mit Mikrodellen versehenen Oberfläche auf dem
Knochenersatzmaterial, wobei in dem Dellennetz der Dellen
durchmesser so klein zu sein hat, daß dreidimensionale
Ablagerungspunkte für a) terminale Collagenfasergruppen, b)
Kohlenhydratgruppen des Collagens und c) Collagenfasern mit
Knochenmineralien gebildet werden, während gleichzeitig
kovalente Bindung und intermolekulare Bindekräfte in einem
Netz bzw. Raummuster entstehen, welches so eng wie möglich
ist (5 bis 500 nm). Die Voraussetzungen für eine dauerhafte
Bindung zwischen lebendem Gewebe und einem Ersatz für
lebendes Gewebe beruhen auf dem entsprechenden Wissen, das
über das Collagenfasersystem der verschiedenen Gewebetypen
mit darüber verteilten, auf die biologische Funktion jedes
Gewebes spezialisierten Mineralien gesammelt wurde.
Aus obigem geht klar hervor, welche biologischen Erforder
nisse in Hinblick auf die Errichtung einer Grenzzone
zwischen lebendem Gewebe und Implantat, welche so fest wie
möglich verankert ist, erfüllt werden müssen.
Nach vielen Jahren experimenteller Arbeit wurden starke
Verbindungen mit auf verschiedene Weise hergestellten Mate
rialien erzielt, wobei die Oberfläche des Verbindungsmittels
durch Riefelung und Gewindebohrung desselben so gestaltet
ist, daß ein verstärkter Oberflächenkontakt besteht.
Durch sorgfältiges Gewindebohren am Befestigungsmaterial
wird eine gesunde und kräftige Knochenoberfläche mit exakter
anatomischer Hilfsübereinstimmung mit der Implantatober
fläche erhalten, was eine kontrollierte Verdichtung des
Knochengewebes liefert. Die Erfahrung zeigt, daß ein sorg
fältiges Gewindeschneiden an Bohrlöchern im Knochengewebe
und am korrespondierenden Implantat unter Verwendung von
Schneidwerkzeugen aus Titan, die mit geringer Drehzahl ar
beiten, die beste Dichtung und eine weitaus optimierte und
erhöhte Haftung und Heilungsoberfläche zwischen dem Implan
tat und dem für die Operation vorbereiteten lebenden Gewebe
liefert, ohne Schnitte im Gewebe zu verursachen, beispiels
weise durch Metallteilchen, die von anderen, schneller
arbeitenden Werkzeugen verursacht werden können.
Claims (5)
1. Element zur Implantation in Knochengewebe, bestehend aus
einem biologisch verträglichen, porösen Material, welches
das Auftreten von störenden elektrischen Potentialdiffe
renzen und galvanischen Strömen im Implantationsbereich ver
hindert, wobei das Element eine aus Dellen bestehende Ober
flächenstruktur aufweist, die Anlagerungspunkte für aus
terminalen Collagenfasergruppen, Kohlehydratgruppen des
Collagens und Collagenfasern mit Knochenmineralien be
stehende Grenzzonenbildner besitzt, dadurch ge
kennzeichnet, daß die Dellen einen Durchmesser
aufweisen, der im Bereich von 10 nm bis 1000 nm liegt und
kleiner ist, als es dem Zellendurchmesser des umgebenden
Gewebes entspricht.
2. Element nach Anspruch 1, dadurch gekennzeichnet, daß der
Dellendurchmesser 10 nm bis 300 nm beträgt.
3. Element nach Anspruch 1 oder 2, dadurch gekennzeichnet,
daß das poröse Material aus Titan besteht.
4. Element nach Anspruch 3, dadurch gekennzeichnet, daß das
Material aus Titan mit einem Sauerstoffgehalt besteht, der
von einer die Dellen aufweisenden Titanoxidoberfläche nach
innen hin abnimmt.
5. Element nach einem der Ansprüche 1 bis 4, dadurch gekenn
zeichnet, daß die Oberfläche des Elementes
aus einer feinporigen Rutilschicht besteht.
