Mehrzw e ck-Blutverdünnungsmittel
Die Erfindung "bezieht sich auf ein zur Zählung und Größenbestimmung
von Blutkörperchen zu verwendendes Blutverdünnungsmittel, insbesondere auf ein Mehrzweck-Blutverdünnungsmittel,
das im Rahmen einer elektronischen Teilchenanalyse mit einem unter der Bezeichnung "Coulter-Gerät" für die hämatologische
Zählung der Blutkörperchen und Bestimmung der Hämoglobinkonzentration sowie der davon abzuleitenden Kennwerte verwendbar
ist.
Ein solches Verdünnungsmittel ist eine beständige wäßrige Lösung chemischer Salze, die eine elektrolytisehe Lösung ergeben,
welche die Großzahl der Erythrozyten und Leukozyten sowie anderer Blutbeständteile einer beigemengten Blutprobe verdünnen
und somit die Bestimmung, Zählung und Auswertung der gesuchten Parameter dieser Blutbestandteile ermöglicht.
Es ist in der Medizin allgemein üblich, für diagnostische Zwecke die Blutprobe eines Patienten zu analysieren und zu

untersuchen, um bestimmte charakteristische Blutwerte zu ermitteln.
Die Blutanalyse ist für den Arzt ein sehr wichtiges Hilfsmittel, und die hierbei gemeinhin sechs wichtigsten zu
ermittelnden Parameter sind:
die Erythrozytenzahl (BBC), der Hämatokrit-Index (HGT),
der Hamoglobingehalt (HGB), das mittlere Zellvolumen (MCV),
der mittlere Hamoglobingehalt (MCH), und die mittlere Hämoglobinkonzentration
(MCHC).
Ein aufschlußreicher siebenter Parameter ist die Leukozytenzahl.
Mit Hilfe eines bekannten Gerätes (DT-OS 1 773 226) lassen sich
diese Parameter aus einer Blutprobe des Patienten automatisch und kontinuierlich ermitteln. Die Beschreibung des bekannten
Gerätes enthält allgemein gültige Definitionen dieser Parameter
und beleuchtet auch die sich für die Aufbereitung der zu analysierenden Blutprobe im Zusammenhang mit dem Flüssigkeitssystem des bekannten Gerätes ergebenden Schwierigkeiten.
Für die Zählung und Analyse von Blutkörperchen gibt es auch
andere Geräte mit einfacherem Aufbau als das vorgenannte, die aber ebenfalls die Erythrozyten- und Leukozytenzahlen sowie
die davon abzuleitenden, obengenannten Kennwerte liefern. Ein
Mehrzweck-Verdünnungsmittel muß sich auch für diese Geräte eignen, denn die mit der Aufbereitung der Blutprobe verbundenen
Probleme bleiben die gleichen. Ein derartiges Blutverdünnungsmittel muß also mit allen auf dem "Coulter-Prinzip"
arbeitenden Teilchenanalysiergeräten verwendbar sein, die nachstehend auch pauschal als "Coulter-Counter" bezeichnet werden.
Zum besseren Verständnis der Erfindung sind bestimmte Eigenschaften
der Erythrozyten wie folgt zu erläutern.
Erythrozyten sind bikonkave scheibenförmige Zellen mit einer
zentralen Delle und elastischer Zwischenmembran. Im Zellinneren sind das Hämoglobin und andere Bestandteile enthalten. Die

jeweils mit einem elektronischen Gerät nach dem Coulter-Prinzip
klinisch zu messenden Parameter schreiben die Eigenschaften des zu verwendenden Verdünnungsmittels zwingend vor.
Zum Beispiel läßt sich anhand des gemessenen Volumeninhalts der Erythrozyten und der ermittelten Erythrozytenzahl das
Sedimentvolumen bzw. der Hämatrokrit-Index errechnen. Das Verdünnungsmittel
muß daher ein Elektrolyt sein, der die Durchführung elektronischer Messungen nach dem Coulter-Prinzip ermöglicht
und das Volumen der Erythrozyten in der Blutprobe im Gleichgewicht halten und stabilisieren kann, damit das Kubikvolumen,
d.h. HCV, gemessen werden kann.
Wie bereits in Verbindung mit dem vorgenannten Gerät bekannt, erfordert das Zählen der Blutzellen mit einem Coulter-Counter
eine exakte und gute Verdünnung der in das Flüssigkeitssystem eingeführten Blutprobe. Eine derartige Analyse setzt für eine
erfolgreiche und exakte Punktion des Geräts bestimmte Eigenschaften
des Verdünnungsmittels voraus. Zum Beispiel muß das Vadünnungsmittel die chemische und physikalische Unversehrtheit
der Blutsellen vor und während des Analyseverfahrens gewährleisten.
Die Erythrozyten müssen in der verdünnten Lösung die gleichen physikalischen Eigenschaften wie in der unverdünnten
Probe haben. Zu diesem Zweck muß das Blut verdünnungsmittel
isotonisch, d.h. osmotisch gegenüber den in den Blutzellen befindlichen Lösungen ausgeglichen sein.
Die !Resistenz der Erythrozyten gegen die »um Zwecke der Hämoglobinbestimmung
durchzuführende Hämolyse darf durch das Blutverdünnungsmittel in keiner Weise beeinflußt werden. Das Blutverdünnungsmittel
darf auch in keiner Veise den Prozeß der Umwandlung des ausgetretenen Hämoglobins in eine mengenmäßig
charakteristische Verbindung, z.B. Zyanhämiglobin, beeinträchtigen, da sonst die Genauigkeit der Hämoglobinbestimmung in
Präge gestellt wäre.

Außerdem darf ein gutes Blutverdünnungsmittel keine Teilchen einer Fremdsubstanz enthalten, da etwa vorhandene Fremdteilchen
als Blutzellen oder deren Bestandteile gezählt werden würden. Aus dem Blutverdünnungsmittel müssen schon bei der Herstellung
alle Teilchen mit einem Durchmesser über 0.2 /u. ausgefiltert
werden. Darüber hinaus muß das Blutverdünnungsmittel auch bakteriostatisch wirken, um das Wachsen von Mikro-Organismen
nach erfolgter Verpackung des Verdünnungsmittels auszuschließen. Außerdem hat sich erwiesen, daß ein für Analysiergeräte
und Hämoglobinometer bestimmtes Blutverdünnungsmittel reaktionslos und osmotisch ausgeglichen sein muß, um
reproduzierbare, genaue Resultate erhalten zu können.
Derartige zur Zählung und Größenbestimmung von Blutzelten zu
verwendende elektrolytische Lösungen müssen daher in ihrer Konzentration derart beschaffen sein, daß der von den elektrolytischen
Ionen ausgeübte osmotische Druck dem der im Zellinneren enthaltenen Flüssigkeit entspricht. In einer Lösung
mit niedrigerem osmotischen Druck, d.h. in einer hypotonischen Lösung suspendierte Zellen nehmen Wasser auf und schwellen bis
zum Bersten an, so daß alle Zellflüssigkeiten in die Lösung
austreten. Werden Blutzellen in einer elektrolytischen Lösung mit höherem osmotischen Druck suspendiert, so wird ihnen die
Zellflüssigkeit durch die Lösung entzogen und sie schrumpfen zu sog. "Stechapfelformen" ein. Man muß zwar Konservierungsmittel
verwenden, um Bakterien- und Pilzwachstum zu verhindern, das verwendete Konservierungsmittel darf aber nicht zu
einer Vergrößerung des Zellvolumens der suspendierten Blutzellen führen.
Weiterhin wird zum Stand der Technik auf die US-Patentanmeldung
483,088 verwiesen.
Seit Jahren wird zwar ein unter der Warenbezeichnung "Isoton"
bekanntes Blutverdünnungsmittel erfolgreich in Coulter-Counters und Hämoglobinometer verwendet, es besitzt aber einige
unerwünschte Eigenschaften.

Ein unerwünschtes Merkmal war z.B. durch die Verwendung von Hatriumazid als wirksames Bakteriostatikum "bedingt. Im übrigen
wurde ITatriumazid allgemein in isotonischen Blutverdünnungsmitteln
verwendet. Natriumazid ist eine verhältnismäßig hochtoxische Substanz, und bei seiner Verwendung muß berücksichtigt
werden, daß wäßrige Lösungen des Azids und Hydrazosäure-Dämpfe
möglicherweise zu gesundheitlichen Schädigungen der mit dem. Azid arbeitenden Laboranten führen. Bei Rohr systemen mit
Verrohrungen und Dichtungen aus Kupfer und Blei, durch welche die Azidlösungen abfließen müssen, sind sehr sorgfältige und
zweckentsprechende Spülungen erforderlich, um eine übermäßige
Ansammlung von Schwermetallaziden über eine längere Zeitdauer zu vermeiden.
Man war daher bestrebt, das Hatriumazid durch ein gleichwertig
wirksames Bakteriostatikum zu ersetzen, bisher ist dies aber nicht gelungen.
Im Zusammenhang mit den bekannten Blutverdunnungsmitteln hat
man auch Natriumchlorid, Kaliumchlorid und Natriumhydroxid verwendet. Auch die Verwendung eines Chelat bildenden Agens
ist bekannt, welches in Zusammenwirken mit dem verwendeten Phosphatsalz als Puffermittel zur Erzielung eines bestimmten
pH-Bereichs diente, innerhalb welchem das Verdünnungsmittel wirksam war. Ein solcher Chelatbildner war Ithylendiamintetraazetylsäure
(ÄDTA).
Der Erfindung liegt die Aufgabe zugrunde, eine elektrolytische Mehrzweck—Lösung zu schaffen, die in einem nach dem Coulter-Frinzip
selbsttätig arbeitenden elektronischen Teilchenanalysiergerät zur hämatologischen Zählung von Blutkörperchen sowie
zur Bestimmung der Hämoglobinkonzentration und anderer wichtiger Parameter einer zu untersuchenden Blutprobe verwendbar
ist und ohne Zusatz eines Azides die mit den bekannten Verdünnungsmitteln
dieser Art verbundenen Schwierigkeiten ausschließt.

Zur Lösung dieser Aufgabe sieht die Erfindung ein Mehrzweck-Blutverdünnungsmittel
vor, welches dadurch gekennzeichnet ist, daß es aus einer Metall-Halogenide, Metallphosphate, Natriumfluorid,
ein Konservierungsmittel und einen Chelatbildner enthaltenen osmotisch ausgeglichenen, wäßrigen elektrolytischen
Lösung besteht mit einem pH-Wert zwischen 7-0 und 8.0 und einer
Osmolalität O osmolality) zwischen 300 und 380 milliosmol.
Nach einem Merkmal der Erfindung wird in dem Blutverdünnungsmittel
statt Natriumazid vorteilhafterweise ein nicht-toxisches und verträgliches Bakteriostatikum verwendet, ohne daß seine
Wirksamkeit dadurch beeinträchtigt wird.
Das so erhaltene Verdünnungsmittel ist osmotisch ausgeglichen und reaktionslos und schließt dank des darin enthaltenen Fatri—
umfluorids eine Störung der für die korrekte Bestimmung der Hämoglobinkonzentration
erforderlichen Umwandlung des Hämoglobins in Zyanhämiglobin aus. Als Puffermittel enthält das Verdünnungsmittel
ein Antikoagulans mit einem Bakteriostatikum. Das Anti— koagulans verhindert eine Agglutination der Erythrozyten im i*al-Ie
etwaiger in der Blutprobe enthaltener anomaler Plasma-Proteine
sowie ein Ausfällen von Schwermet allsalzen in dem Verdünnungsmittel, die die Zählungen verfälschen wurden.
Somit läßt sich das Verdünnungsmittel nach der Erfindung mit
den bereits existierenden Coulter—Analysiergeräten verwenden
und ist mit deren zur Durchführung einer hämatologischen Analyse vorgegebenen Eichung vereinbar.
Fach einer bevorzugten Abstimmung kann das Blutverdünnungs—
mittel nach der Erfindung wie folgt zusammengesetzt sein;

Das oben spezifizierte Verdünnungsmittel wird durch das Puffermittel,
!DTA und Phosphatsalz auf einen pH-Wert von 7.2 bis gebracht; die Osmolalität (=» osmolality) liegt zwischen 320 und
340.
Das gewünschte osmotische Gleichgewicht ergibt sich durch Verwendung
sowohl von Natrium- als auch Kaliumchlorid. Hierdurch wird ein Puffern mit Natriumhydroxid vermieden.
Statt dem bisher verwendeten Natriumazid ist als Bakteriostatikum
2-Phenoxyäthanol enthalten, das sehr viel ungiftiger als Natriumazid ist und das Problem etwaiger toxischer Auswirkungen
für die Labortechniker völlig ausräumt. Außerdem geht das
2-Phenoxyäthanol keine bekannten gefährlichen Verbindungen mit
dem für gewöhnlich in Wasserleitungssystemen verwendeten Kupfer
oder Blei ein.
Das VerdünnungSMittel bleibt völlig reaktionslos und osmotisch
ausgeglichen, wie für die Bestimmung des mittleren Zellvolumens erforderlich, und die Pufferung wird gemeinsam durch die Phosphatealze
und die IDTA bewirkt. iDTA dient außerdem auch als
Antikoagulans, und das Natriumfluorid dient zur Stabilisierung des Erythrozytenvolumens und unterstützt die für Hämoglobinbestiamungen
erforderliche Umwandlung des Hämoglobins in Zyanhämiglobin.
Somit ergibt sich eine elektrolytische Lösung, die den Strom leitet und in der die Erythrozyten stabilisiert werden, so
daß deren Kubikvolumen exakt gemessen werden kann. Die Leukozyten werden durch das Verdünnungsmittel nicht beschädigt, so

daß dieses auch als Elektrolyt für die Leukozytenzählung nach elektronischer Methodik verwendbar ist. Es hat auch
keinerlei Auswirkung auf die Blutplättchen und stört auch nicht die Umwandlung von Hämoglobin in das zur Hämoglobinbestimmung
dienende Zyanhämiglobin.
Die Herstellung des Verdünnungsmittels erfordert kein spezielles Verfahren noch eine besondere Reihenfolge der einzelnen,
dem Wasser zuzusetzenden Ingredenzien. Die Erfindung betrifft daher keine Methodik zur Abstimmung des Verdünnungsmittels.
Das Zusammenmdashen der Bestandteile erfolgt durch mäßiges
mechanisches Rühren über einen Zeitraum von 1-2 Stunden. Die Losung wird dann durch ein 0.2 /U Filter gefiltert und
kann dann sofort in Kunststoffbehälter abgefüllt werden.
Ungeachtet der vorstehend angegebenen bevorzugten Abstimmung kann der pH-Bereich und die Osmolalität für praktisches Zwek—
ke erweitert werden. Der pH-Bereich läßt sich zwischen 7.0 und 8.0 halten, und auch die Nutzbreite der. Osmolalität kann
zwischen 300 und 380 milliosmol liegen. Dies läßt sich durch Änderung der Menge der für diean Zweck angegebenen aktiven
Bestandteile erreichen.
Für den Fachmann ist ersichtlich, daß sich das Verdünnungsmittel nach der Erfindung auf die gleiche Weise verwenden
läßt wie das bekannte "Isoton"-Verdünnungsmittel, ohne mit
dessen nachteiligen Eigenschaften behaftet zu sein.

Claims (8)
- PatentansprücheΛΛ Mehrzweck-Blutverdünnungsmittel zur "Verdünnung einer auf —' eine Anzahl charakteristischer Parameter in einem elektronischen Teilchenanalysiergerät nach dem Coulter-Prinzip zu analysierenden Blutprobe, dadurch gekennzeichnet,daß es aus einera) Metall-Halogenide und Metall-Phosphateb) Natriumfluoridc) einem Konservierungsmittel undd) einem Chelatbildnerenthaltenden, osmotisch ausgeglichenen, wäßrigen elektrolytischen Losung besteht mit einem pH-Wert zwischen 7·0 und 8.0 und einer Osmolalität (osmolality) zwischen 300 und 380 milliosmol.
- 2. Blutverdünnungsmittel nach Anspruch 1, dadurch gekennzeichnet, daß als Metallhalogenide Natriumchlorid und Kaliumchlorid enthalten sind.
- 3. Blutverdünnungsmittel nach Anspruch 2, dadurch gekennzeichnet, daß der Chelatbildner Äthylendiamintetraazetylsäure enthält.
- 4. Blutverdünnungsmittel nach Anspruch 3» dadurch gekennzeichnet, daß als Metall-Phosphate Natriumdihydrogenphosphat und Dinatriumphosphat enthalten sind.
- 5. Blutverdünnungsmittel nach Anspruch 4, dadurch gekennzeichnet, daß das Konservierungsmittel 2-Phenoxyäthanol ist.
- 6. Blutverdünnungsmittel nach Anspruch 4, dadurch gekennzeichnet, daß 1 Liter der Verdünnungslösung ca. 7 g Natriumchlorid und ca. 0.4 g Kaliumchlorid enthält.- 2 609838/0931
- 7. Blutverdünnungsmittel nach Anspruch 1, gekennzeichnet durch einen pH-Bereich zwischen 7· 2 und 7·5·
