Description
Erfindungsgemäß wird diese Aufgabe bei einem chirurgischen Klammerinstrument der eingangs beschriebenen
Art dadurch gelöst, daß der Stößelvorschub durch einen an dem äußeren Rahmen angeordneten
Anschlag zur Bestimmung der geringsten Entfernung zwischen Stößel und Amboß begrenzt ist.
Durch das erfindungsgemäße Gerät ist gewährleistet, daß auch bei richtiger Einstellung der Gewebelücke
sowohl ein Zerquetschen des zu nähenden Gewebes durch eine Überanpressung der eingebrachten Klammern
als auch ein zu lockeres Anlegen der Klammern verhindert wird. Die eingebrachten Klammern
werden immer gerade so weit gegen den Amboß geschoben, daß eine befriedigende Naht zustande

Eine gute Naht läßt sich insbesondere dann anbringen, wenn der Chirurg^ bei der Betätigung des
Gerätes keine äußersten Kräfte anwenden muß, sondern wenn der große Kraftanstieg zu Beginn des Ümbiegens
der Klammern durch eine geeignete Übersetzung in dem Gerät ausgeglichen wird. Eine bevorzugte
Ausführungsform der Erfindung sieht daher vor, daß die Stößelstange durch von einem beweglichen
Handgriff betätigten Hebel angetrieben ist, wobei der Hub des Handgriffs der zum Andrücken der
Klammern gegen den Amboß benötigten Kraft in der Weise angepaßt ist, daß den größeren Kräften zum
Beginn des Biegens der Klammern größere Wege des Handgriffs entsprechen.
In weiterer Ausgestaltung der Erfindung ist der um eine Achse schwenkbare Handgriff mit einer
Nockenscheibe verbunden, die zum Antrieb der Stößelstange auf mit dieser in Antriebsverbindung stehenden
Zapfen abgleiten. Dabei weist die Nockenscheibe eine Form auf, die der zum Anbringen der
Klammern benötigten Kraft entsprechenden Übersetzung angepaßt ist. Die Nockenscheibe ist so ausgebildet,
daß sie in dem Bereich die geringste Steigung aufweist, die dem Beginn des Biegens der Klammern
entspricht.
Zur Herabsetzung der Reibung zwischen den Zapfen und der diese antreibenden Nockenscheibe bestehen
die Zapfen zweckmäßigerweise aus Rollen.
Ausführungsbeispiele der Erfindung werden nachstehend
an Hand der Zeichnung näher erläutert. In der Zeichnung zeigt

Fig.4 eine Seitenansicht der Antriebselemente der Stößelgruppe und
F i g. 5 eine Seitenansicht einer anderen Ausführungsform des Klammerinstruments.
Das in Fig. 1 in Seitenansicht dargestellte Instrument besteht aus einem äußeren Rahmen OF
(Fig.2), einem in diesem geführten inneren Rahmen
IF (F i g. 3) und aus einem in dem inneren Rahmen IF untergebrachten Antriebsmechanismus für die
Stößelgruppe P (F i g. 4). Der innere Rahmen IF wird
nach Aufnahme der Stößelgruppe P mit dem Antriebsmechanismus in den äußeren Rahmen eingepaßt,
wobei der vordere Teil des inneren Rahmens in einer U-förmigen Führung 1 des äußeren Rahmens
ruht. Der äußere Rahmen weist an seinem geschlossenen hinteren Ende einen Einschnitt 3 auf, der in
die Ringnut 4 der Flügelmutter 2 eingreift. Der innere Rahmen IF weist an seinem hinteren Ende
einen feststehenden Handgriff 32 und einen den Rahmen fortsetzenden Gewindebolzen 5 auf, auf den
die Flügelmutter aufgeschraubt ist, so daß durch Drehung der Flügelmutter der innere Rahmen in den
äußeren Rahmen in Längsrichtung verschieblich ist.
-Eine wegwerfbare KlammernpatroneC ist am vorderen
Ende des inneren Rahmens in der Weise montiert, daß der Stößel 42 mit der Vorderfront 92 seiner
Zähne 93 nach Betätigung des Handgriffs MH die Klammern 104 aus dieser austreibt. Als Widerlager
für die von dem Stößel 42 ausgetriebenen Klammern
104 dient der am äußeren Rahmen angeordnete Amboßö.
Die Stößelgruppe P ist in den inneren Rahmen eingepaßt, wobei die Stößelstange 41 in der Bohrung 38
des inneren Rahmens gleitend geführt ist. Die Stößelplatte 42 ist in den hohlen vorderen Kopf des inne-
ao ren Rahmens eingepaßt. Das hintere Ende der Stößelstange
41 ist in die Nockenplatte CP eingehängt, die durch die Schwenkbewegung des Handgriffs MH
angetrieben wird und ihre Antriebskraft auf die Stößelstange 41 überträgt. Die Nockenplatte CP besteht
aus zwei parallel zueinander angeordneten flachen Platten, in deren Zwischenraum die Rollen 52 auf
einer die Platten verbindenden Achse drehbar angeordnet sind. Die vordere Rolle 52 gleitet auf dem
Rahmenteil 19 des äußeren Rahmens, in dem die Nockenplatte CP geführt ist. Der bewegliche Handgriff
MH ist durch Niete oder auf andere Weise fest mit einer Kurvenscheibe 56 verbunden, die mit ihrer
Nockenfläche 62 bei Betätigung des Handgriffs MH auf der vorderen in der der Nockenplatte CP angeordneten
Rolle 52 abgleitet und die Nockenplatte und somit die Stößelgruppe antreibt. Der Handgriff
mit der Kurvenscheibe 56 ist um die Achse 60 schwenkbar, die in der Aussparung 20 b des inneren
Rahmens ruht und bei Verstellung des inneren Rahmens relativ zu dem äußeren durch die Flügelmutter
2 in der Nut 20 geführt ist.
Die Kraftübertragung von dem Handgriff MH auf
die Stößelgruppe P und somit die Klammern 104 hängt von der Steigung der Nockenfläche 62 der
Kurvenscheibe 56 ab. Die Steigung der Nockenfläche wird so gewählt, daß sie der zum Anbringen der
Klammern 104 benötigten Kraft auf Grund der sich daraus ergebenden Kraftübersetzung angepaßt ist.
Die Nockenscheibe 62 weist in dem Bereich die geringste Steigung auf, der dem Beginn des Biegens der
Klammern 104 entspricht. Der Stößelvorschub ist dadurch begrenzt, daß eine weitere Vorwärtsbewegung
nicht mehr möglich ist, nachdem die Nockenplatte CP mit ihrer Vorderfront gegen den im äußeren
Rahmen angeordneten Anschlag 19' aufgelaufen ist. Die zwischen Amboß 6 und Klammernpatrone C
gewünschte Gewebelücke wird durch Verstellung der Flügelmutter 2 eingestellt. Der Stößelvorschub ist unabhängig
von der Einstellung der Gewebelücke, da der Vorschub durch den im äußeren Rahmen angeordneten
Anschlag 19' begrenzt ist, gegen den die Nockenplatte mit ihrer Vorderfront 19" in ihrer vordersten
Stellung aufläuft. '
In Fig. 5 ist eine andere Ausführungsform des
Klammerninstruments dargestellt. Eine Stößelstangengruppe mit einer Stößelplatte 234 und einer Stößelstange
235 ist in den inneren Rahmen 217 geführt. Der innere Rahmen ist über die Flügelmut-
ter2, die mit: einer Ringnut in eine gabelförmige
Aussparung am hinteren Ende des äußeren Rahmens eingreift, über den mit dem inneren Rahmen verbundenen
Gewindebolzen 226 verstellbar. Der innere Rahmen 217 ist fest mit dem festen Handgriff 218
verbunden. In die Aussparung 233 des inneren Rahmens 217 ist die Achse des: schwenkbaren Handgriffs
239 einsetzbar. Der Handgriff weist oberhalb der Achse 239 einen freien Arm 240 auf, der an seiner
Vorderseite eine gekrümmte Nockenfläche aufweist. Der Arm 240 des beweglichen Handgriffs greift gabelförmig
in einen am hinteren Ende der Stößelstange 235 angeordneten Absatz ein und treibt die
Stößelstange. 235 durch Abgleiten seiner Nockenfläche auf den an der Stößelstange angeordneten
Bund 236 an. Die Form der an der Vorderseite des Arms 240 angeordneten Nockenfläche entspricht der
beim Stößelvorschub gewünschten Kraftübertragung. Im äußeren Rahmen ist ein Anschlag 252 angeordnet,
der den Stößelvorschub unabhängig von der eingestellten Gewebelücke begrenzt. Die Begrenzung
erfolgt dadurch, daß die Stößelstange 235 in ihrer vordersten Stellung mit ihrem Bund 254 gegen
den Anschlag 252 stößt. ■ · . : . ' .

Claims (6)
1. Chirurgisches Klammerinstrument mit ren Rahmen geführten Stößels ist durch Anschläge in
