Topiramat-Zubereitung
mit verzögerter
Wirkstofffreisetzung für
die orale Verabreichung an ein Säugetiersubjekt,
umfassend Topiramat als Wirkstoff, der aus der Zubereitung gemäß einem
vorgegebenen Freisetzungsprofil freigesetzt wird, wobei die Zubereitung
eine Komponente mit verlängerter
Freisetzung (XR) umfasst.
Claims (103)
- Topiramat-Zubereitung mit verzögerter Wirkstofffreisetzung für die orale Verabreichung an ein Säugetiersubjekt, umfassend Topiramat als Wirkstoff, der aus der Zubereitung gemäß einem vorgegebenen Freisetzungsprofil freigesetzt wird, wobei die Zubereitung eine Komponente mit verlängerter Freisetzung (XR) umfasst.
- Zubereitung nach Anspruch 1, wobei die XR-Komponente Topiramat in vivo in kontinuierlicher Weise freisetzt und 80% des Wirkstoffes innerhalb einer Zeitspanne freigesetzt werden, die ausgewählt ist aus nicht mehr als 24 Stunden, nicht mehr als 16 Stunden, nicht mehr als 12 Stunden, nicht mehr als 8 Stunden und nicht mehr als 4 Stunden.
- Zubereitung nach Anspruch 1, wobei die Zubereitung zusätzlich eine Komponente mit sofortiger Freisetzung (IR) enthält, bei der es sich gegebenenfalls um eine Zusammensetzung mit verstärkter sofortiger Freisetzung (EIR) handelt, die ein Komplexierungsmittel, ein Verstärkungsmittel oder beides umfasst.
- Zubereitung nach Anspruch 3, wobei die EIR-Zusammensetzung ein solches Freisetzungsprofil aufweist, dass 80% des Wirkstoffes innerhalb von nicht mehr als 30 Minuten gelöst werden.
- Zubereitung nach Anspruch 4, wobei die EIR-Zusammensetzung ein Freisetzungsprofil aufweist, das aus der Gruppe ausgewählt ist, die besteht aus: a) eine Auflösung von mindestens 50% des Wirkstoffes in nicht mehr als 10 Minuten; b) eine Auflösung von mindestens 70% des Wirkstoffes in nicht mehr als 10 Minuten; c) eine Auflösung von mindestens 25% des Wirkstoffes in nicht mehr als 5 Minuten; d) eine Auflösung von mindestens 40% des Wirkstoffes in nicht mehr als 5 Minuten und e) eine Auflösung von mindestens 55% des Wirkstoffes in nicht mehr als 5 Minuten.
- Zubereitung nach Anspruch 3, wobei es sich beim Komplexierungsmittel um ein Cyclodextrin handelt, das aus der Gruppe ausgewählt ist, die besteht aus Hydroxypropyl-beta-cyclodextrin, beta-Cyclodextrin, gamma-Cyclodextrin, alpha-Cyclodextrin und Derivaten davon.
- Zubereitung nach Anspruch 6, wobei es sich beim Komplexierungsmittel um Hydroxypropyl-beta-cyclodextrin handelt.
- Zubereitung nach Anspruch 3, wobei es sich beim Verstärkungsmittel um ein Mittel zur Verstärkung der Löslichkeit, ein Mittel zur Verstärkung der Auflösung, ein Mittel zur Verstärkung der Absorption, ein Mittel zur Verstärkung der Penetration, ein oberflächenaktives Mittel, einen Stabilisator, einen Enzyminhibitor, einen p-Glycoproteininhibitor, einen Multiarzneistoff-Resistenz-Proteininhibitor oder Kombinationen davon handelt.
- Zubereitung nach Anspruch 8, wobei es sich beim Verstärkungsmittel handelt um Vitamin E TPGS, Glutaminsäure, Glycin, Sorbit, Mannose, Amylose, Maltose, Mannit, Lactose, Saccharose, Glucose, Xylitose, Dextrine, Glycerin-polyethylenglykol-oxystearat, PEG-32-Glycerylpalmitostearat, Natriumlaurylsulfat, Polyoxyethylensorbitanmonooleat, Benzylalkohol, Sorbitan-monolaurat, Poloxamer 407, PEG3350, PVP K25, Ölsäure, Glycerylmonooleat, Natriumbenzoat, Cetylalkohol, Saccharosestearat, Crospovidon, Natriumstärkeglycolat, Croscarmellosenatrium, Carboxymethylcellulose, Stärke, vorgelierte Stärke, HPMC, substituierte Hydroxypropylcellulose, mikrokristalline Cellulose-natriumbicarbonat, Calciumcitrat, Natriumdocusat, Menthol oder Kombinationen davon.
- Zubereitung nach Anspruch 1 oder 3, wobei mindestens ein Teil des Wirkstoffes in Form von mikronisierten Teilchen vorliegt.
- Zubereitung nach Anspruch 10, wobei die Teilchen eine durchschnittliche Größe von etwa 2 μm bis etwa 100 μm aufweisen.
- Zubereitung nach Anspruch 1, wobei die Zubereitung in einer Dosierungsform einer Tablette, einer Pille, einer Kapsel, eines Caplets, einer Pastille, eines Sachets, eines Kachets, eines Beutels oder von Streuseln vorliegt.
- Zubereitung nach Anspruch 12, wobei es sich bei der Tablette um eine mehrschichtige Tablette handelt, die mindestens eine Schicht, die die Komponente mit verlängerter Freisetzung umfasst, und mindestens eine Schicht umfasst, die die Komponente mit sofortiger Freisetzung umfasst, wobei es sich bei der Komponente mit sofortiger Freisetzung gegebenenfalls um eine Zusammensetzung mit verstärkter sofortiger Freisetzung handelt.
- Zubereitung nach Anspruch 1, wobei eine spezifische Menge der jeweiligen Komponenten gemäß dem Verabreichungszweck und dem vorgegebenen Freisetzungsprofil festgelegt wird und die Gesamtmenge an Topiramat in der Zubereitung 0,5 bis 3000 mg beträgt.
- Topiramat-Zubereitung mit verzögerter Wirkstofffreisetzung für die orale Verabreichung an ein Säugetiersubjekt, umfassend Topiramat als Wirkstoff, der aus der Zubereitung gemäß einem vorgegebenen Freisetzungsprofil freigesetzt wird, wobei die Zubereitung eine Komponente mit verlängerter Freisetzung und eine optionale Komponente mit sofortiger Freisetzung umfasst und wobei die Komponente mit verlängerter Freisetzung in mindestens einer Perlenpopulation bereitgestellt wird, die mit einem Überzug zur Steuerung der Freisetzung beschichtet ist und durch ihre eigene Freisetzungsgeschwindigkeit charakterisiert ist.
- Zubereitung nach Anspruch 15, wobei die Perlen einen inerten Träger, Topiramat, einen optionalen Verstärker und einen Überzug zur Steuerung der Freisetzung, der ein Beschichtungsmaterial und gegebenenfalls einen Porenbildner und andere Exzipientien umfasst, umfassen.
- Zubereitung nach Anspruch 16, wobei der inerte Träger aus der Gruppe ausgewählt ist, die aus Cellulosekügelchen, Siliciumdioxid, Stärke und Zuckerkügelchen besteht.
- Zubereitung nach Anspruch 16, wobei der Verstärker aus der Gruppe ausgewählt ist, die aus Löslichkeitsverstärkern, Auflösungsverstärkern, Permeabilitätsverstärkern, Stabilisatoren, Komplexierungsmitteln, Enzyminhibitoren, p-Glycoproteininhibitoren, Multiarzneistoff-Resistenz-Proteininhibitoren und Kombinationen davon besteht.
- Zubereitung nach Anspruch 16, wobei das Beschichtungsmaterial Ethylcellulose, Methylcellulose, Hydroxypropylcellulose, Hydroxypropylmethylcellulose, Celluloseacetat, Celluloseacetophthalat, Polyvinylalkohol, Polyacrylate, Polymethacrylate oder Copolymere davon umfasst; und wobei der Porenbildner Glucose, Fructose, Mannit, Mannose, Galactose, Sorbit, Pullulan, Dextran, wasserlösliche hydrophile Polymere, Hydroxyalkylcellulosen, Carboxyalkylcellulosen, Hydroxypropylmethylcellulose, Celluloseether, Acrylharze, Polyvinylpyrrolidon, vernetztes Polyvinylpyrrolidon, Polyethylenoxid, Carbowachse, Carbopol, Diole, Polyole, mehrwertige Alkohole, Polyalkylenglykole, Polyethylenglykole, Polypropylenglykole oder Blockpolymere davon, Polyglykole, Poly-(α-ω)-alkylendiole; anorganische Verbindungen; Alkalimetallsalze; Erdalkalimetallsalze oder Kombinationen davon umfasst.
- Zubereitung nach Anspruch 15, wobei eine spezifische Menge jeder Perlenpopulation gemäß dem vorgegebenen Freisetzungsprofil festgelegt wird.
- Zubereitung nach Anspruch 15, wobei das vorgegebene Freisetzungsprofil nach einer anfänglichen sofortigen Freisetzung eine Freisetzung mit verzögerter Geschwindigkeit umfasst.
- Zubereitung nach Anspruch 15, gekennzeichnet durch einen verringerten Grad mindestens einer unerwünschten ZNS-Nebenwirkung, verglichen mit der gleichen Menge an Topiramat, die als Zubereitung (BID) mit sofortiger Freisetzung verabreicht wird.
- Zubereitung nach Anspruch 15, geeignet für eine 1-malige tägliche orale Verabreichung.
- Zubereitung nach Anspruch 15, wobei die Zubereitung in einer Dosierungsform einer Tablette, einer Pille, einer Kapsel, eines Caplets, einer Pastille, eines Sachets, eines Kachets, eines Beutels oder von Streuseln vorliegt.
- Zubereitung nach Anspruch 15, wobei die Zubereitung für eine maximale Plasmakonzentration im stationären Zustand (Cmax) von Topiramat sorgt, die im Bereich von 50 bis 125% der maximalen Plasmakonzentration liegt, die durch die gleiche Menge an Topiramat, das als Zubereitung (BID) mit sofortiger Freisetzung verabreicht wird, hervorgerufen wird.
- Zubereitung nach Anspruch 25, wobei der Cmax-Wert höher ist als die minimale, therapeutisch wirksame Konzentration, jedoch niedriger ist als die maximale Plasmakonzentration, die durch die gleiche Menge an Topiramat, das als Zubereitung (BID) mit sofortiger Freisetzung verabreicht wird, hervorgerufen wird.
- Zubereitung nach Anspruch 25, wobei der Cmax-Wert im Bereich von 80 bis 125% der maximalen Plasmakonzentration liegt, die durch die gleiche Menge an Topiramat, das als Zubereitung (BID) mit sofortiger Freisetzung verabreicht wird, hervorgerufen wird.
- Zubereitung nach Anspruch 25, wobei der Cmax-Wert im Bereich von 50 bis 100% der maximalen Plasmakonzentration liegt, die durch die gleiche Menge an Topiramat, das als Zubereitung (BID) mit sofortiger Freisetzung verabreicht wird, hervorgerufen wird.
- Zubereitung nach Anspruch 15, wobei die Zubereitung für einen relativen AUC-Wert im stationären Zustand im Bereich von 80 bis 125% des AUC-Werts der gleichen Menge an Topiramat, das als Zubereitung (BID) mit sofortiger Freisetzung verabreicht wird, sorgt.
- Zubereitung nach Anspruch 15, wobei die Zubereitung für einen Fluktuationsgrad im Bereich von 25 bis 90% des AUC-Werts der gleichen Menge an Topiramat, die als Zubereitung mit sofortiger Freisetzung (BID) verabreicht wird, sorgt.
- Zubereitung nach Anspruch 15, wobei mindestens eine Population von Perlen mit verlängerter Freisetzung von Topiramat zusätzlich eine Komponente zur sofortigen Freisetzung, die schichtförmig oben auf den Oberzug zur Steuerung der Freisetzung aufgebracht ist, umfasst.
- Zubereitung nach Anspruch 16, wobei der Verstärker in einer Schicht enthalten ist, die vom Überzug zur Steuerung der Freisetzung getrennt ist.
- Zubereitung nach Anspruch 15, umfassend mindestens ein Verstärkungsmittel, wobei das Mittel der Zubereitung in Form eines Pulvers oder in einer Population von Perlen, die gegebenenfalls eine gesteuerte Freisetzungsgeschwindigkeit aufweisen, einverleibt wird, und wobei das Verstärkungsmittel vom Wirkstoff getrennt ist.
- Topiramat-Zubereitung mit verzögerter Wirkstofffreisetzung für die orale Verabreichung an ein Säugetiersubjekt, wobei Topiramat aus der Zubereitung gemäß einem vorgegebenen Freisetzungsprofil freigesetzt wird, wobei die Zubereitung drei Populationen von Perlen mit einem Gehalt an Topiramat mit verlängerter Freisetzung, die als XR1, XR2 und XR3 bezeichnet werden, und eine Population von Perlen mit sofortiger Freisetzung (IR) umfasst, wobei die proportionalen Mengen der einzelnen Populationen der Zubereitung gemäß der folgenden Tabelle ausgewählt sind:

- Verfahren zur Behandlung oder Verhinderung eines pathologischen Zustands bei einem Säugetiersubjekt, umfassend die orale Verabreichung einer therapeutisch wirksamen Menge einer Zubereitung von Topiramat mit verzögerter Freisetzung an das Subjekt, wobei Topiramat aus der Zubereitung gemäß einem vorgegebenen Freisetzungsprofil freigesetzt wird und wobei die Zubereitung eine Komponente mit verlängerter Freisetzung und eine optionale Komponente mit sofortiger Freisetzung umfasst.
- Verfahren nach Anspruch 36, wobei die XR-Komponente Topiramat in vivo auf kontinuierliche Weise freisetzt und 80% des Wirkstoffes innerhalb einer Zeitspanne freigesetzt werden, die ausgewählt ist aus nicht mehr als 24 Stunden, nicht mehr als 16 Stunden, nicht mehr als 12 Stunden, nicht mehr als 8 Stunden und nicht mehr als 4 Stunden.
- Verfahren nach Anspruch 36, wobei es sich bei der Komponente mit sofortiger Freisetzung um eine Zusammensetzung mit verstärkter sofortiger Freisetzung handelt, die Topiramat und ein Komplexierungsmittel und/oder ein Verstärkungsmittel umfasst, wobei es sich beim Komplexierungsmittel um ein Cyclodextrin handelt, das aus der Gruppe ausgewählt ist, die besteht aus Hydroxypropyl-beta-cyclodextrin, beta-Cyclodextrin, gamma-Cyclodextrin, alpha-Cyclodextrin und ein Cyclodextrinderivat; und wobei das Verstärkungsmittel aus der Gruppe ausgewählt ist, die besteht aus Mitteln zur Verstärkung der Löslichkeit, Mitteln zur Verstärkung der Auflösung, Mitteln zur Verstärkung der Absorption, Mitteln zur Verstärkung der Penetration, oberflächenaktiven Mitteln, Stabilisierungsmitteln, Enzyminhibitoren, p-Glycoproteininhibitoren, Multiarzneistoff-Resistenz-Proteininhibitoren und Kombinationen davon.
- Verfahren nach Anspruch 38, wobei die EIR-Zusammensetzung ein solches Freisetzungsprofil aufweist, dass 80% des Wirkstoffes innerhalb von nicht mehr als 30 Minuten gelöst werden.
- Verfahren nach Anspruch 39, wobei die EIR-Zusammensetzung ein Freisetzungsprofil aufweist, das aus der Gruppe ausgewählt ist, die besteht aus: a) eine Auflösung von mindestens 50% des Wirkstoffes in nicht mehr als 10 Minuten; b) eine Auflösung von mindestens 70% des Wirkstoffes in nicht mehr als 10 Minuten; c) eine Auflösung von mindestens 25% des Wirkstoffes in nicht mehr als 5 Minuten; d) eine Auflösung von mindestens 40% des Wirkstoffes in nicht mehr als 5 Minuten und e) eine Auflösung von mindestens 55% des Wirkstoffes in nicht mehr als 5 Minuten.
- Verfahren nach Anspruch 36, wobei die Komponente mit verlängerter Freisetzung in mindestens einer Perlenpopulation bereitgestellt wird, die mit einem Überzug zur Steuerung der Freisetzung beschichtet ist und ihre eigene Freisetzungsgeschwindigkeit aufweist.
- Verfahren nach Anspruch 41, wobei die Zubereitung für eine maximale Plasmakonzentration im stationären Zustand (Cmax) von Topiramat sorgt, die im Bereich von 50 bis 125% der maximalen Plasmakonzentration liegt, die durch die gleiche Menge an Topiramat, das als Zubereitung (BID) mit sofortiger Freisetzung verabreicht wird, hervorgerufen wird.
- Verfahren nach Anspruch 42, wobei der Cmax-Wert höher ist als die minimale, therapeutisch wirksame Konzentration, jedoch niedriger ist als die maximale Plasmakonzentration, die durch die gleiche Menge an Topiramat, das als Zubereitung (BID) mit sofortiger Freisetzung verabreicht wird, hervorgerufen wird.
- Verfahren nach Anspruch 42, wobei der Cmax-Wert im Bereich von 80 bis 125% der maximalen Plasmakonzentration liegt, die durch die gleiche Menge an Topiramat, das als Zubereitung (BID) mit sofortiger Freisetzung verabreicht wird, hervorgerufen wird.
- Verfahren nach Anspruch 42, wobei der Cmax-Wert im Bereich von 50 bis 100% der maximalen Plasmakonzentration liegt, die durch die gleiche Menge an Topiramat, das als Zubereitung (BID) mit sofortiger Freisetzung verabreicht wird, hervorgerufen wird.
- Verfahren nach Anspruch 41, wobei die Zubereitung für einen relativen AUC-Wert im stationären Zustand im Bereich von 80 bis 125% des AUC-Werts der gleichen Menge an Topiramat, das als Zubereitung (BID) mit sofortiger Freisetzung verabreicht wird, sorgt.
- Verfahren nach Anspruch 41, wobei die Zubereitung einen Fluktuationsgrad im Bereich von 25 bis 90% der gleichen Menge an Topiramat, das als Zubereitung mit sofortiger Freisetzung (BID) verabreicht wird, sorgt.
- Verfahren nach Anspruch 41, wobei die Perlen einen inerten Träger, Topiramat, einen optionalen Verstärker und einen Überzug zur Steuerung der Freisetzung, der ein Beschichtungsmaterial und gegebenenfalls einen Porenbildner und andere Exzipientien umfasst, umfassen.
- Verfahren nach Anspruch 48, wobei der inerte Träger aus der Gruppe ausgewählt ist, die aus Cellulosekügelchen, Siliciumdioxid, Stärke und Zuckerkügelchen besteht; und wobei das Beschichtungsmaterial aus der Gruppe ausgewählt ist, die besteht aus Ethylcellulose, Methylcellulose, Hydroxypropylcellulose, Hydroxypropylmethylcellulose, Celluloseacetat, Celluloseacetophthalat, Polyvinylalkohol, Polyacrylaten, Polymethacrylaten und Copolymeren davon.
- Verfahren nach Anspruch 48, wobei der Porenbildner ausgewählt ist aus der Gruppe, die besteht aus Glucose, Fructose, Mannit, Mannose, Galactose, Sorbit, Pullulan, Dextran, wasserlöslichen hydrophilen Polymeren, Hydroxyalkylcellulosen, Carboxyalkylcellulosen, Hydroxypropylmethylcellulosen, Celluloseether, Acrylharzen, Polyvinylpyrrolidon, vernetztem Polyvinylpyrrolidon, Polyethylenoxid, Carbowachsen, Carbopol, Diolen, Polyolen, mehrwertigen Alkoholen, Polyalkylenglykolen, Polyethylenglykolen, Polypropylenglykolen oder Blockcopolymeren davon, Polyglykolen, Poly-(α-ω)-alkylendiolen; anorganischen Verbindungen, Alkalimetallsalzen; Erdalkalimetallsalzen oder Kombinationen davon.
- Verfahren nach Anspruch 48, wobei der Verstärker aus der Gruppe ausgewählt ist, die aus Löslichkeitsverstärkern, Auflösungsverstärkern, Permeabilitätsverstärkern, Stabilisatoren, Komplexierungsmitteln, Enzyminhibitoren, p-Glycoproteininhibitoren, Multiarzneistoff-Resistenz-Proteininhibitoren und Kombinationen davon besteht.
- Verfahren nach Anspruch 41, wobei eine spezifische Menge jeder Perlenpopulation gemäß dem vorgegebenen Freisetzungsprofil festgelegt wird.
- Verfahren nach Anspruch 52, wobei das vorgegebene Freisetzungsprofil nach einer anfänglichen sofortigen Freisetzung eine Freisetzung mit verzögerter Geschwindigkeit umfasst.
- Verfahren nach Anspruch 36, wobei die Zubereitung 1-mal täglich oral verabreicht wird.
- Verfahren nach Anspruch 36, wobei die Zubereitung einen verringerten Grad an unerwünschten ZNS-Nebenwirkungen, verglichen mit der gleichen Menge an Topiramat, die als Zubereitung mit sofortiger Freisetzung (BID) verabreicht wird, aufweist.
- Verfahren nach Anspruch 36, wobei eine spezifische Menge der jeweiligen Komponenten gemäß dem Verabreichungszweck und dem vorgegebenen Freisetzungsprofil festgelegt wird und die Gesamtmenge an Topiramat in der Zubereitung 0,5 bis 3000 mg beträgt.
- Verfahren nach Anspruch 36, wobei die Zubereitung in einer Dosierungsform einer Tablette, einer Pille, einer Kapsel, eines Caplets, einer Pastille, eines Sachets, eines Kachets, eines Beutels oder von Streuseln vorliegt.
- Verfahren nach Anspruch 36, wobei der Zustand ausgewählt ist aus einer Gruppe, die besteht aus Epilepsie, Migräne, essentiellem Tremor, Gliedmaßen-Ruhelosigkeitssyndrom, Cluster-Kopfschmerzen, Neuralgie, neuropathischem Schmerz, Tourrette-Syndrom, infantilen Spasmen, perinataler Hypoxie-Ischämie und verwandten Schädigungen, Glaukom, Augenstörungen, Fettleibigkeit, Gewichtsverlust, Diabetes mellitus vom Typ II, diabetischer Retinopathie, beeinträchtigter oraler Glucosetoleranz, diabetischen Hautläsionen, diabetischer Neuropathie, erhöhten Blutglucosespiegeln, Syndrom X, erhöhtem Blutdruck, erhöhten Lipiden, bipolarer Störung, Demenz, Depression, Psychose, Manie, Angstzuständen, Schizophrenie, obsessiver-kompulsiver Störung, posttraumatischer Stress-Störung, ADHD, Impulskontrollstörungen, Borderline-Personalitätsstörung, Sucht, Autismus, Asthma, Autoimmunstörungen, chronischen neurodegenerativen Störungen, akuter Neurodegeneration, ALS, Schlafapnoe und Schlafstörungen.
- Verfahren zur Behandlung oder Verhinderung eines pathologischen Zustands bei einem Säugetiersubjekt, umfassend die orale Verabreichung einer wirksamen Menge einer Zubereitung mit verzögerter Freisetzung von Topiramat an das Subjekt, wobei Topiramat aus der Zubereitung gemäß einem vorgegebenen Freisetzungsprofil freigesetzt wird, wobei die Zubereitung drei Populationen von Perlen mit einem Gehalt an Topiramat mit verlängerter Freisetzung, die als XR1, XR2 und XR3 bezeichnet werden, und eine Population von Perlen mit sofortiger Freisetzung (IR) umfasst, wobei die proportionalen Mengen der einzelnen Populationen der Zubereitung gemäß der folgenden Tabelle ausgewählt sind:

- Verfahren zur Herstellung einer Topiramat-Zubereitung mit verzögerter Freisetzung, umfassend eine Komponente mit verlängerter Freisetzung (XR) und eine optionale Komponente mit sofortiger Freisetzung (IR), wobei mindestens die XR-Komponente in mindestens einer Perlenpopulation enthalten sind, die durch ihre eigene Freisetzungsgeschwindigkeit gekennzeichnet ist, wobei das Verfahren folgendes umfasst: das Bilden mindestens einer Population von Topiramat enthaltenden Perlen; das Beschichten jeder Population von Perlen mit einem eigenen Überzug zur Steuerung der Freisetzung; das Härten des Überzugs in einer Härtungsvorrichtung für eine Zeitspanne, die spezifisch für jede Perlenpopulation einen Überzug zur Steuerung der Freisetzung erzeugt; und das Einverleiben der Perlen der einzelnen Populationen in die Zubereitung in Mengen, die gemäß einem vorgegebenen Freisetzungsprofil festgelegt sind.
- Verfahren nach Anspruch 61, ferner das Einverleiben mindestens eines weiteren Bestandteils in die Zubereitung, der nicht in den Topiramat enthaltenden Perlen, die mit dem Oberzug zur Steuerung der Freisetzung beschichtet sind, enthalten ist.
- Verfahren nach Anspruch 61, wobei die Geschwindigkeit der Freisetzung durch die Zusammensetzung der Beschichtung zur Steuerung der Freisetzung, den Beschichtungsgrad, Verfahrensparameter der Beschichtung und der Härtung und Kombinationen davon festgelegt wird.
- Verfahren nach Anspruch 61, wobei die Härtungsstufe in Gegenwart eines Lösungsmittels durchgeführt wird.
- Verfahren nach Anspruch 64, wobei es sich bei dem Lösungsmittel um ein organisches Lösungsmittel handelt, das aus der Gruppe Alkohole, Ketone, Ether, Ester, Amide, Amine, Kohlenwasserstoffe, organische Säuren und Phenole; superkritische Flüssigkeiten; Ammoniak; Wasser; wässrige Lösungen und Kombinationen davon ausgewählt ist.
- Verfahren nach Anspruch 64, wobei die Härtungszeit von etwa 2 Wochen auf etwa mehrere Stunden verringert wird.
- Verfahren nach Anspruch 63, wobei die Freisetzungsgeschwindigkeit eine Funktion einer Gewichtszunahme während der Beschichtung ist.
- Verfahren nach Anspruch 62, wobei es sich beim zusätzlichen Bestandteil um die Komponente mit sofortiger Freisetzung, ein Verstärkungsmittel, ein Komplexierungsmittel oder eine Kombination davon handelt, wobei der zusätzliche Bestandteil der Zubereitung in Form eines Pulvers oder in Form einer Population von Perlen, die gegebenenfalls durch eine kontrollierte Freisetzungsgeschwindigkeit gekennzeichnet sind, einverleibt wird.
- Verfahren zur Herstellung einer Zubereitung mit verzögerter Freisetzung von Topiramat, umfassend eine Komponente mit verlängerter Freisetzung (XR) und eine Komponente mit sofortiger Freisetzung (IR), wobei Topiramat aus der Zubereitung mit verzögerter Geschwindigkeit gemäß einem vorgegebenen Freisetzungsprofil freigesetzt wird, wobei das Verfahren folgendes umfasst: das Festlegen des erwünschten Freisetzungsprofils; das Festlegen spezifischer Mengen der Komponente mit verlängerter Freisetzung und der Komponente mit sofortiger Freisetzung, die zur Erzeugung des vorgegebenen Freisetzungsprofils erforderlich sind; und das Einverleiben der spezifischen Mengen der Komponenten in die Zubereitung.
- Verfahren nach Anspruch 69, wobei es sich bei der Komponente mit sofortiger Freisetzung um eine Zusammensetzung mit verstärkter sofortiger Freisetzung (EIR) handelt, die zusätzlich mindestens ein Mittel umfasst, das aus Komplexierungsmitteln und Verstärkungsmitteln ausgewählt ist, wobei das Verstärkungsmittel aus der Gruppe ausgewählt ist, die besteht aus Mitteln zur Verstärkung der Löslichkeit, Mitteln zur Verstärkung der Auflösung, Mitteln zur Verstärkung der Absorption, Mitteln zur Verstärkung der Penetration, oberflächenaktiven Mitteln, Stabilisierungsmitteln, Enzyminhibitoren, p-Glycoproteininhibitoren, Multiarzneistoff-Resistenz-Proteininhibitoren und Kombinationen davon.
- Verfahren nach Anspruch 70, wobei die EIR-Zusammensetzung hergestellt wird, indem man einen hochgradig löslichen Komplex von Topiramat mit einem Komplexierungsmittel bildet, das ein Cyclodextrin umfasst, das aus einer Gruppe ausgewählt ist, die besteht aus Hydroxypropyl-beta-cyclodextrin, beta-Cyclodextrin, gamma-Cyclodextrin und alpha-Cyclodextrin oder einem Cyclodextrinderivat.
- Verfahren nach Anspruch 71 , wobei es sich beim Komplexierungsmittel um Hydroxypropyl-beta-cyclodextrin bei einem Gewichtsverhältnis des Komplexierungsmittel s zu Topiramat von 3:2 handelt.
- Verfahren nach Anspruch 70, wobei die EIR-Zusammensetzung eine hochgradig lösliche Form von Topiramat umfasst, die durch Zugabe eines Löslichkeitsverstärkers hergestellt worden ist, der aus einer Gruppe ausgewählt ist, die besteht aus Natriumlaurylsulfat, D-alpha-Tocopherylpolyethylenglykol-1000-succinat, hydriertem Polyoxy-Rizinusöl, polyglycosylierten Glyceriden, Polyoxyethylen-sorbitanmonooleat, Poloxamer 407, PVP-K25, Saccharosestearat, Natriumdocusat und Menthol.
- Verfahren nach Anspruch 70, wobei die EIR-Zusammensetzung ein solches Freisetzungsprofil aufweist, dass 80% des Wirkstoffes in nicht mehr als 30 Minuten gelöst werden.
- Verfahren nach Anspruch 74, wobei die Komponente mit sofortiger Freisetzung ein Freisetzungsprofil aufweist, das aus der Gruppe ausgewählt ist, die besteht aus: a) eine Auflösung von mindestens 50% des Wirkstoffes in nicht mehr als 10 Minuten; b) eine Auflösung von mindestens 70% des Wirkstoffes in nicht mehr als 10 Minuten; c) eine Auflösung von mindestens 25% des Wirkstoffes in nicht mehr als 5 Minuten; d) eine Auflösung von mindestens 40% des Wirkstoffes in nicht mehr als 5 Minuten und e) eine Auflösung von mindestens 55% des Wirkstoffes in nicht mehr als 5 Minuten.
- Verfahren nach Anspruch 69, wobei das Freisetzungsprofil eine verzögerte Freisetzung nach einer anfänglichen sofortigen Freisetzung umfasst.
- Verfahren zur Herstellung einer Zubereitung mit verzögerter Freisetzung von Topiramat, umfassend eine Komponente mit verlängerter Freisetzung und eine Komponente mit verstärkter sofortiger Freisetzung, wobei Topiramat aus der Zubereitung gemäß einem vorgegebenen Freisetzungsprofil freigesetzt wird, wobei das Verfahren folgendes umfasst: das Festlegen des gewünschten Freisetzungsprofils; das Festlegen spezifischer Mengen der Komponente mit verlängerter Freisetzung und der Komponente mit sofortiger Freisetzung, die zur Erzeugung des vorgegebenen Freisetzungsprofils erforderlich sind; das Herstellen der Komponente mit verstärkter sofortiger Freisetzung durch Vereinigen von Topiramat mit mindestens einem Komplexierungsmittel oder Verstärkungsmittel; das Herstellen der Komponente mit verlängerter Freisetzung durch Bilden mindestens einer Population von Topiramat enthaltenden Perlen, die mit einem Mittel zur Steuerung der Freisetzung beschichtet sind; und das Einverleiben der spezifischen. Mengen der Komponenten in die Zubereitung.
- Verfahren nach Anspruch 77, wobei die Zubereitung mit verzögerter Freisetzung von Topiramat in eine Dosierungsform einer Pille, einer Kapsel, eines Caplets, einer Pastille, eines Sachets, eines Kachets, eines Beutels oder von Streuseln einverleibt wird.
- Verfahren nach Anspruch 77, wobei die Zubereitung mit verzögerter Freisetzung drei Populationen von Perlen mit einem Gehalt an Topiramat mit verlängerter Freisetzung, die als XR1, XR2 und XR3 bezeichnet werden, und eine Population von Perlen mit sofortiger Freisetzung (IR) umfasst, wobei die proportionalen Mengen der einzelnen Populationen der Zubereitung gemäß der folgenden Tabelle ausgewählt sind:

- Verfahren zur Herstellung von beschichteten Mikroteilchen, umfassend: das Bereitstellen eines Substrats von Mikroteilchen; das Beschichten der Mikroteilchen mit einer polymeren Beschichtungslösung; und das Härten der Beschichtung bei einer erhöhten Temperatur, wobei die Härtungsstufe in einer Härtungsvorrichtung in Gegenwart eines Lösungsmittels durchgeführt wird, das aus einer Gruppe ausgewählt ist, die besteht aus organischen Lösungsmitteln, die ausgewählt sind aus einer Gruppe, die Alkohole, Ketone, Ether, Ester, Amide, Amine, Kohlenwasserstoffe, organische Säuren und Phenole umfasst; superkritischen Flüssigkeiten, Ammoniak; Wasser; wässrigen Lösungen und Kombinationen davon.
- Verfahren nach Anspruch 81, wobei die Härtungszeit von etwa 2 Wochen auf etwa mehrere Stunden verringert wird.
- Verfahren nach Anspruch 81, wobei es sich bei den Mikroteilchen um Mikroteilchen mit einem Gehalt an einem pharmazeutischen Wirkstoff handelt.
- Verfahren nach Anspruch 83, wobei es sich bei den Mikroteilchen um Topiramat enthaltende Perlen handelt.
- Orale Dosierungsform, umfassend eine Zubereitung mit verzögerter Freisetzung von Topiramat, wobei ein Wirkstoff aus der Zubereitung gemäß einem vorgegebenen Freisetzungsprofil freigesetzt wird und wobei die Zubereitung eine Komponente mit verlängerter Freisetzung (XR) und eine optionale Komponente mit sofortiger Freisetzung (IR) umfasst.
- Orale Dosierungsform nach Anspruch 85, wobei spezifische Mengen von Topiramat in den XR- und IR-Komponenten durch das vorgegebene Freisetzungsprofil festgelegt werden und die Gesamtmenge in der Zubereitung 0,5 bis 3000 mg beträgt.
- Dosierungsform nach Anspruch 85, ausgewählt aus einer Gruppe, die besteht aus einer Tablette, einer Pille, einer Kapsel, einem Caplet, einer Pastille, einem Sachet, einem Kachet, einem Beutel und Streuseln.
- Dosierungsform nach Anspruch 85, wobei mindestens die XR-Komponente in mindestens einer Perlenpopulation enthalten ist, die mit einem Überzug zur Steuerung der Freisetzung beschichtet ist und durch ihre eigene Freisetzungsgeschwindigkeit gekennzeichnet ist, wobei eine Menge von Perlen einer jeden Population gemäß einem vorgegebenen Freisetzungsprofil festgelegt wird.
- Dosierungsform nach Anspruch 88, hergestellt durch ein Verfahren, das die folgenden Stufen umfasst: das Bilden mindestens einer Population von Topiramat enthaltenden Perlen; das Beschichten jeder Population von Perlen mit einem eigenen Überzug zur Steuerung der Freisetzung; das Härten des Überzugs in einer Härtungsvorrichtung für eine Zeitspanne, die spezifisch für jede Perlenpopulation einen Überzug zur Steuerung der Freisetzung erzeugt; und das Einverleiben der Perlen der einzelnen Populationen in die Zubereitung in Mengen, die gemäß einem vorgegebenen Freisetzungsprofil festgelegt sind.
- Dosierungsform nach Anspruch 89, wobei die Härtungsstufe in Gegenwart eines Lösungsmittels durchgeführt wird, das aus einer Gruppe ausgewählt ist, die besteht aus organischen Lösungsmitteln, die aus einer Gruppe ausgewählt sind, die besteht aus Alkoholen, Ketonen, Ethern, Estern, Amiden, Aminen, Kohlenwasserstoffen, organischen Säuren und Phenolen; superkritischen Flüssigkeiten; Ammoniak; Wasser; wässrigen Lösungen und Kombinationen davon.
- Orale Dosierungsform nach Anspruch 88, umfassend eine Komponente zur sofortigen Freisetzung, bei der es sich um eine Zusammensetzung mit verstärkter sofortiger Freisetzung handelt.
- Orale Dosierungsform nach Anspruch 91, wobei die Zubereitung mit verzögerter Freisetzung drei Populationen von Perlen mit einem Gehalt an Topiramat mit verlängerter Freisetzung, die als XR1, XR2 und XR3 bezeichnet werden, und eine Population von Perlen mit sofortiger Freisetzung (IR) umfasst, wobei die proportionalen Mengen der einzelnen Populationen der Zubereitung gemäß der folgenden Tabelle ausgewählt sind:

- Dosierungsform nach Anspruch 92, bei der es sich um eine Kapsel handelt.
- Orale Dosierungsform nach Anspruch 85, geeignet zur einmaligen täglichen Verabreichung.
- Orale Dosierungsform nach Anspruch 87, wobei es sich bei der Tablette um eine mehrschichtige Tablette handelt, die mindestens eine Schicht umfasst, die die Komponente mit verlängerter Freisetzung umfasst, sowie mindestens eine Schicht, die die Komponente mit sofortiger Freisetzung umfasst.
- Zubereitung nach Anspruch 1, ferner umfassend einen zusätzlichen pharmazeutischen Wirkstoff in Kombination mit Topiramat.
- Verfahren nach Anspruch 36, wobei die Zubereitung ferner einen zusätzlichen pharmazeutischen Wirkstoff in Kombination mit Topiramat umfasst.
- Orale Dosierungsform nach Anspruch 85, ferner umfassend einen zusätzlichen pharmazeutischen Wirkstoff in Kombination mit Topiramat.
- Zubereitung nach Anspruch 15, wobei mindestens ein Teil des Wirkstoffes in Form von mikronisierten Teilchen vorliegt.
- Zubereitung nach Anspruch 100, wobei die Teilchen eine durchschnittliche Größe von etwa 2 μm bis etwa 100 μm aufweisen.
- Verfahren nach Anspruch 36, wobei mindestens ein Teil des Wirkstoffes in Form von mikronisierten Teilchen vorliegt.
- Verfahren nach Anspruch 102, wobei die Teilchen eine durchschnittliche Größe von etwa 2 μm bis etwa 100 μm aufweisen.
