背景技术
现代诊断技术已经提供了具有空前的信息容量和处理量的高度敏感的工具。多种诸如帕帕尼古拉乌(Papanicolaou)染色法这样的技术,已经十分适合于自动化过程,这进一步改善了它们的一致性和可靠性。

具体实施方式
本发明提供了一种对细胞学样品分类的自动化方法。该方法包括用至少一个波长的光探测样品,以获得表示样品特性的结果。将该结果与适用于识别满足给定标准的样品的标准进行比较。然后将标识符附着于样品,从而反映该样品是否满足所述标准。自动标识系统允许快速确定样品是否适合于特定的化验。该方法可以和用于特定生物分子的探测样品基于分子的方法一起使用。

“互补”或“基本上互补”是指杂交的能力或核苷酸或核酸之间(例如,感受器肽核酸与靶多核苷酸之间)的碱基对。互补核苷酸通常是A和T(或A和U)、或C和G。当一个链的碱基与另一个链的至少约80%、通常至少约90%至95%、以及更优选约98%至100%的碱基最佳地对准并比较,并且具有适当的嵌入对或中间缺失对时,则认为两个单链多核苷酸或PNA基本上互补。
可替换地,当多核苷酸或PNA在选择性杂交条件下与其补体杂交时,存在基本的互补性。典型地,当在至少14至25个碱基的范围内存在至少约65%的互补、优选至少约75%、更优选至少约90%的互补时,将发生选择性杂交。参见,M.Kanehisa Nucleic AcidsRes.12:203(1984)。
“优选结合”或“优先杂交”是指与样品中的非互补多聚体相比,一种多核苷酸或PNA与样品中的其补体结合的增加的倾向。
杂交条件将典型地包括小于约1M的盐浓度,更通常小于约500mM,并且优选小于约200mM。在肽核酸或其它类似核酸与多核苷酸之间进行杂交的情况下,可以在含有很少盐或不含盐的溶液中进行该杂交。杂交温度可以低达5℃,但通常大于22℃,更通常大于约30℃,并且优选超过约37℃。对于特异性杂交,较长的片段可能需要较高的杂交温度。其它因素可能影响杂交的严谨性,这些因素包括碱基组成和互补链的长度、有机溶剂的存在及碱基错配的程度,并且所用参数的组合比任何单个参数的绝对量度更重要。可以控制的其它杂交条件包括:缓冲液类型和浓度;溶液pH;封闭剂或封闭蛋白溶液的存在和浓度,其中封闭剂用来降低诸如重复序列的本底结合;去污剂类型和浓度;诸如增加多核苷酸的相对浓度的多聚体的分子;金属离子及其浓度;螯合剂及其浓度;以及本领域中已知的其它条件。

可以对以任何方式获得的细胞学样品执行样品分类的自动化方法。适宜的技术在本技术领域是已知的。样品可以是任何来源的生物材料,其可以直接或间接地从生物机体获得,该生物机体包括细胞、组织或体液。样品的非限定性实例包括:血液,尿,精液,乳,痰,粘液,胸膜液,骨盆液,关节液(滑膜液),腹水液,体腔洗液,冲眼液,皮肤刮下物,颊拭子,阴道拭子,帕氏抹片,直肠拭子,抽吸物,针刺活组织检查、通过诸如外科手术或尸检获得的组织切片,血浆,血清,脊髓液,淋巴液,皮肤、呼吸道、肠道、以及泌尿生殖道的外分泌物,眼泪,唾液,肿瘤,器官,微生物培养物,病毒,以及体外细胞培养成分的样品。
可以将样品收集或放置在用于基于液体的细胞学的溶液中或介质中,其使细胞溶解并将部分或所有分子成分溶解在溶液中。在一个实施例中,样品可以包括防腐液,例如PreservCyt溶液(Cytyc公司)。
样品可以包括适用于在环境温度下保存细胞和组织的防腐液。该溶液可以包括一种醇以及优选地包括一种缓冲剂,并且在环境温度下,在活组织检查以后、在染色或其它形式的分析之前,其可以用于哺乳动物细胞的体外保存。该溶液可以是于1993年10月26日授权给Hurley等人的美国专利第5,256,571号中所描述的溶液中的一种。防腐液可以包括一种水混溶的醇,以及优选地包括一种抗凝集剂和一种缓冲剂。醇组分以足以固定样品细胞或组织的量存在,同时仍然允许感受器对其靶标的可接受的结合。该醇典型地是低级烷(C1-6)醇,以及可以是C1-4醇,并且可以选自由甲醇、乙醇和异丙醇组成的组。醇的含量可以大于约40%并小于约60%,以及可以是约45%或更多,并且可以是约55%或更少。在另一变化实施例中,该醇的含量为溶液的至少约20%。抗凝集剂的含量可以是足以防止细胞在溶液中凝集。可以使用在醇防腐液中有效的任何适宜的抗凝集剂,该抗凝集剂可以是,例如,螯合剂,该螯合剂选自例如由乙二胺四乙酸(EDTA)及其盐(例如乙二胺四乙酸二钠、乙二胺四乙酸三钾和乙二胺四乙酸四钠)组成的组。被认为可用作抗凝集剂的其它制剂包括cuminin、肝素、链激酶、以及在溶解或抗凝剂组合物中发现的药剂。可以使用在使用期间能维持防腐液在期望pH值的任何缓冲剂。典型的缓冲剂包括PBS、三羟甲基氨基甲烷、乙酸钠、以及柠檬酸。EDTA及其盐也可以用作缓冲剂。该缓冲剂可以是这样的缓冲剂:其在保存期间将溶液的pH值维持在约4至约7之间的范围内。因此,优选的缓冲剂是乙酸盐缓冲剂,例如乙酸钠、乙酸镁、乙酸钙、及其组合。在溶液中也可以使用去污剂。该去污剂可以是非离子、阳离子、阴离子或两性离子的。也可以使用去污剂的混合物。典型种类的去污剂包括醇醚硫酸酯(盐)、醇的硫酸酯(盐)、链烷醇酰胺、烷基磺酸酯(盐)、氧化胺、两性去污剂、阴离子去污剂、甜菜碱衍生物、阳离子去污剂、二磺酸盐(酯)、十二烷基苯磺酸、乙氧基化醇、乙氧基化烷基酚、乙氧基化脂肪酸、甘油酯水溶助剂、月桂硫酸酯(盐)、单甘油酯和甘油二酯、非离子去污剂、磷酸酯、季盐类去污剂、以及脱水山梨糖醇衍生物。
样品的自动分类是本发明的关键特征。这允许保健护理者或实验室技术人员快速确定样品是否满足给定的标准,从而能够例如对样品进行另外的诊断试验而无需对要进行的化验折衷,例如帕帕尼古拉乌染色程序(帕氏染色法)或其变换形式。如果样品未能满足标准,该自动化方法还允许采取补救措施。
将用于自动化分析的样品提供在容器中,适宜的容器在本技术领域中是已知的。该容器可以是任何能够保持溶液的细胞学样品的容器,并且允许光学探测该溶液,以确定相对于一个或多个标准该样品是否适合。该容器可以是可密封的,并且可以是适于与ThinPrep2000处理器(Cytyc公司)一起使用的小细胞瓶(cellularvial)。容器的一个或多个表面和/或任何附件(例如,盖)对于用来探测样品的光足够透明,以便可以确定结果。
可以利用不同的探测方法相对于多个不同的标准对样品进行分类。可以利用不同波长的光、不同的光学技术、以及其组合来探测样品。可以探测样品的吸光度、透光度、荧光性、散射率等、以及其组合。在探测前可以对样品进行混合,并且可以手动地或自动地进行该混合;仪器可以结合有混合能力来执行此功能。
可以用光的各种波长的任意一个波长来探测样品,包括紫外线、可见光、红外线、近红外线、中红外线、远红外线、紫外线、近紫外线、远紫外线、极远紫外线、UV-A、以及UV-B。与滤光片结合的光源可以提供一种或多种精确波长的光,可以提供波长范围、或其组合,并且可以使用一个以上的不同光源。可以使用能够对感兴趣的标准产生可用于探测样品的波长的任何光源;在这种自动化仪器中可以使用多个光源。典型的光源包括:红外灯,例如,具有由绕制的堪塔耳合金或镍铬合金构成的灯丝并可选地连接于蓝宝石、硒化锌或氟化钙透射窗的灯,加热灯,发光二极管,纤维光源,可见光灯,脉冲可见光灯,氘灯;氙灯;连续波(cw)气体激光器,其包括但不限于:任何氩离子激光线(457nm、488nm、514nm等)或HeCd激光器;固态二极管激光器,例如基于GaN和GaAs(双倍)的激光器或基于YAG或YLF的双倍或三倍输出的激光器;以及脉冲激光器。
将通过探测样品获得的结果与标准比较。设置标准以便允许基于探测的结果对样品进行符合要求的分类。该标准可以是吸光度、透光度、反射率、散射度、发光、荧光性、发射率、或背散射率,并且可以是在特定波长或选择的波长下。标准可以是绝对数值,并且可以以任何方式存储在装置中,包括存储在软件中、存储在固定存储器中、和/或存储在可编程存储器中。标准可以是相对值,该相对值通过仪器与一个或多个对照样品比较而获得。该标准可以是最小阈值或最大阈值。可以使用多个不同的标准以多种不同的方式对样品分类,并且可以同时地、顺序地、或以其组合方式加以应用。例如,该标准可以是对特定波长的光的吸光度。该标准可以选择性地用于特定的细胞类型,例如子宫颈内细胞,其在帕氏抹片样品中的存在表示宫颈的采样。为了确定样品中细胞的数目或浓度,可以使用在特定波长的光的吸光度;可以通过比较(需要确定其中是否存在细胞的样品的)溶液的吸收光谱,从经验上来确定适宜的波长(例如在可见光区或红外区)。可以通过适宜的方式来检测特定的样品成分(例如特定的细胞类型),例如利用竞争分析法,如其中一个成员连接于荧光基团而另一个成员连接于猝灭剂的结合对;对结合于结合对的一个成员的竞争可以从猝灭剂释放荧光基团,从而提供可以被检测的荧光信号。这可以利用对存在于感兴趣的细胞类型中的抗原的特异性抗体来完成。可以使用利用了荧光标记的麦胚凝集素(Oregon绿或四甲基若丹明;Molecular Probes)的竞争分析法来检测粘液,其中麦胚凝集素是一种特异性结合于在粘液中存在的N-乙酰神经氨酸(唾液酸)残基的凝集素。例如,在405~420nm的波长下,可以检测样品中血液的存在。
对于从光学探测获得的结果进行比较产生到样品的自动附着标识符。样品可以是正标志符或处理标识符。正标识符表明样品满足标准。例如,正标识符可以表明样品中有足够数目的细胞(其可以是所选择的类型)。正标识符可以表明样品中有足够的细胞,以允许对样品执行另外的方法,其超出了指定用于样品的预期的方法;例如,正标识符可以表明在对样品进行自动载玻片制备之前可以抽取等分量的样品用于分子测试。这是十分期望的,因为自动方法被认为易于受到样品之间的交叉污染的影响,因而从已经经历了自动处理的小瓶抽取的剩余样品被认为是不可靠的。细胞学家更希望在自动样品制备之前确定样品是否适合,以便可以移走未污染的样品并将其用于其它试验,例如分子HPV(人乳头状瘤病毒)试验。
处理标识符表示样品必须经过处理以使它适合于预期的试验。例如,处理标识符可以表示在样品中存在不足够的细胞、或不足够的特定类型的细胞,从而可以指定附加样品的获取。处理标识符可以表示必须对样品进行处理以使其可以接受。例如,在样品含有过量的血液而未能满足适合的血液水平的标准的情况下,可以将处理标识符附着于样品,表示要进行预处理步骤。预处理步骤可以是乙酸处理。在样品含有过量粘液的情况下,可以赋予其处理标识符,例如,以获得另一个样品,或用粘液还原剂(例如,诸如二硫苏糖醇的还原剂)处理样品。处理标识符可以表示其它处理,如样品的过滤、或样品的洗涤。
可以以任何方式将标识符附着于样品,可以物理附着,电子附着、以及以多种方式附着。物理附着可以通过以下方式来完成,例如,在样品容器上设置标识,或将反映标识的标记物附着于样品容器。一种仪器可以结合打印、雕刻、蚀刻或其它装置来直接对容器进行标记,并可以结合标记物打印机或标记物附着装置来将标记物附着于容器。可以将标识符电子附着于容器,例如在电子存储器中,可以通过标识符与样品号、样品位置、或在样品容器上的代码或其它标志的相关性将标识符附着于容器。
因此,这些方法可以和样品收集一起进行,从而提供明显的优点。在保健装置中可以执行特定的方法,以便于在患者离开保健机构之前确定样品是否适合,并且可以在采样时进行,以便于当时与获得样品一起进行这些过程。因此可以在当时获得另外的样品,而无需患者返回。
用于与本发明一起使用的自动化分类系统的一个实例包括激发源和探测器阵列,以及可选地包括:一个或多个滤光片,其用来减少样品所暴露于的波长数目和/或从样品观察到的波长数目;用于对来自样品的结果与标准进行比较的装置;用于将正标识符附着于样品的装置;以及用于将处理标识符附着于样品的装置。用于比较结果的装置可以包括电子设备,该电子设备可以是一种计算机,其可以接收来自探测器阵列的信号并将它与标准进行比较,所述标准可以是存储在存储器中的数值,而存储器可以是固定存储器或动态存储器,并且可以由硬件或软件提供,或者该数值可以通过与从对照样品获得的信号进行比较来获得。将标识符附着于样品的装置也可以包括电子设备,该电子设备可以是一种计算机,并且可以包括能够直接对小样品瓶做标记的打印机,或者可以印制附着于小瓶的标记物,或者可以电子指示对样品进行处理,例如通过将处理标识符附加于小瓶上已有的代码(例如条码),或者通过将处理标识符附加于样品在装置中所处的位置。术语“附着(附加)”包括物理附着于样品和/或其小瓶、电子附着于样品和/或小瓶的说明、或上述两者。用于比较的装置、用于附着正标识符的装置、以及用于附着处理标识符的装置都可以通过单个电子设备(例如单个计算机)来操作。
执行本发明的方法的仪器可以是独立的或者可以与能够对样品执行更多处理的附加部件结合。例如,可以在由自动样品处理器进行的标志之后对样品做进一步处理,并且可以通过自动成像系统进行成像。自动分类系统可以适用于将样品传送到自动样品处理器和/或成像器,并且可以可选地加入这样的系统。典型的自动化系统包括Cytyc公司的ThinPrep成像系统、TriPath FocalPointTM剖面仪、Chroma Vision Acis系统、CompuCyt iCyte成像系统、AppliedImaging Cyto VisionTM系统、以及Veracel Verasys成像系统。这些方法可以结合到诸如在美国专利第5,185,084号、第5,266,495号、第6,010,909号、第6,225,125号、以及第5,942,700号中所描述的样品处理装置中。
可以对样品执行样品分析的生化和/或分子方法,例如,利用结合伴侣(binding partner)作为感受器(sensor)分子来检测样品中靶标物种的方法。可以以结合光学探测进行的格式执行该方法,或者可以在分离部分样品之后执行该方法。该方法可以分析针对靶标的样品,其中所述靶标可以是期望要检测的样品的任何成分。
在靶标是细胞或细胞成分或产物的情况下,细胞可以是任何来源,包括原核来源、真核来源、或archea(一种古细菌)来源。细胞可以是活的或死的。如果细胞是从多细胞生物体获得,则该细胞可以具有任何细胞类型。细胞可以是培养的细胞系或最初分离物,细胞可以是哺乳动物细胞、两栖动物细胞、爬行动物细胞、植物细胞、酵母细胞、细菌细胞、螺旋体细胞、或原生动物细胞。细胞可以是正常细胞、突变细胞、基因操作细胞、肿瘤细胞等。靶标可以是传染剂、疾病或感染标记物、蛋白质、抗体、抗原等。
感受器可以是当存在于样品中时能够选择性结合于其靶标的任何物质。感受器的非限制性实例包括:如上所述的多核苷酸,其包括肽核酸和适体;以及如上所述的抗体。也可以使用不同感受器的组合,其可以便于检测和分析样品中的多个靶标。在一个变化的实施例中,感受器可以是特异性结合于靶多核苷酸的PNA,其中该靶多核苷酸被怀疑存在于样品中。不同的标记物可以用于对不同靶标特异性不同的次级感受器,其便于多重检测。
夹层技术可以用来检测靶标与感受器的结合。例如,在感受器是对靶标特异性抗体的情况下,可以使用并不干扰感受器抗体结合的第二标记抗体,以允许检测靶标的结合。类似地,也可以使用结合于部分靶标或部分感受器:靶标复合物而不破坏该复合物的多核苷酸。可用于检测感受器:靶标复合物的标记物包括在感受器结合于靶标以后与存在于样品中的靶标相关的、可以直接或间接被检测的任何物质。典型的标记物包括发色团、发光基团、荧光基团、色素原、半抗原、抗原、放射性同位素、磁性粒子、金属纳米粒子(例如金或银纳米粒子)、酶、以及结合对的一个成员。
Claims (24)
1.一种对细胞学样品分类的自动化方法,包括:
提供在容器中的溶液中的细胞学样品;
用至少一个波长的光光学探测所述溶液;
将所述探测的结果与标准进行比较;
如果所述结果满足所述标准,则将正标识符附着于所述样品;以及
如果所述结果不满足所述标准,则将处理标识符附着于所述样品。
2.根据权利要求1所述的方法,其中,所述正标识符表示所述样品适合于进行预期的化验。
3.根据权利要求1或2所述的方法,其中,所述预期的化验包括由所述样品制备载玻片。
4.根据权利要求3所述的方法,其中,如果所述样品含有足够的细胞,则所述样品是符合要求的。
5.根据权利要求4所述的方法,其中,所述细胞具有期望的类型。
6.根据权利要求1所述的方法,其中,所述正标识符表示所述样品适合于自动载玻片制备。
7.根据权利要求1所述的方法,其中,所述正标识符表示所述样品足以允许在进行预期的化验之前抽取部分所述样品。
8.根据权利要求1至7中任一项所述的方法,其中,所述处理标识符指示获取另外的样品。
9.根据权利要求1至7中任一项所述的方法,其中,所述处理标识符指示对所述样品的处理。
10.根据权利要求9所述的方法,其中,所述处理包括将乙酸加入所述样品。
11.根据权利要求9所述的方法,其中,所述处理包括将还原剂加入所述样品。
12.根据权利要求1至11中任一项所述的方法,其中,所述标准指示细胞在所述样品中的浓度。
13.根据权利要求1至11中任一项所述的方法,其中,所述标准指示特定类型的细胞在所述样品中的浓度。
14.根据权利要求13所述的方法,其中,所述细胞是子宫颈内细胞。
15.根据权利要求1至11中任一项所述的方法,其中,所述标准指示所述样品中的粘液的水平。
16.根据权利要求1至11中任一项所述的方法,其中,所述标准指示所述样品中的血液的水平。
17.根据权利要求1至11中任一项所述的方法,其中,所述标准指示在所述样品中血液的水平。
18.根据权利要求1至17中任一项所述的方法,其中,在光学探测所述溶液之前混合所述样品。
19.根据权利要求1至11中任一项所述的方法,其中,所述正标识符包括在所述容器上的标识。
20.根据权利要求19所述的方法,其中,所述正标识符包括在电子存储器中的标志。
21.根据权利要求1至20中任一项所述的方法,其中,所述处理标识符包括在所述容器上的标识。
22.根据权利要求1至20中任一项所述的方法,其中,所述处理标识符包括在电子存储器中的标志。
23.根据权利要求1至22中任一项所述的方法,其中,所述样品是选自由血液,尿,精液,乳,痰,粘液,胸膜液,骨盆液,关节液,腹水液,体腔洗液,冲眼液、皮肤刮下物,颊拭子,阴道拭子,帕氏抹片,直肠拭子,抽吸物,针刺活组织检查,组织切片,血浆,血清,脊髓液,淋巴液,皮肤、呼吸道、肠道、或泌尿生殖道的外分泌物,眼泪,唾液,肿瘤,器官,微生物培养物,以及体外细胞培养成分组成的组。
24.根据权利要求1至23中任一项所述的方法,其中,所述样品包括有效量的水溶性醇,以保持所述溶液对至少一种污染物的无菌性。
