技术领域
本发明总体涉及调节性人工晶状体装置,更特别地,涉及被配置为用于植入受试者的眼睛的晶状体囊或沟中的调节性人工晶状体装置。
背景技术
随着技术进步允许复杂的干预解决各种各样眼科疾病,眼睛外科手术越来越多。在过去二十年中,患者接受度有所提高,因为此类手术已被证明一般是安全的,并且产生显著提高患者生活质量的结果。
白内障手术仍然是最常见的外科手术之一,全世界有1600多万例白内障手术正在进行。随着平均预期寿命持续增加,预计此数字将持续增加。白内障通常通过从眼睛中取出晶状体并且在其位置处植入人工晶状体(“IOL”)来进行治疗。由于常规IOL装置主要聚焦于远距视觉,所以它们未能矫正老花眼,并且仍然需要老花镜。因此,虽然经受标准IOL植入的患者不再经历白内障带来的混浊,但是他们不能适应或者不能使焦点从近到远、从远到近以及在它们之间的距离上变化。

本文描述的调节性人工晶状体(IOL)的各种实施例能够包括与基座晶状体相关的屈光力晶状体的屈光力变化特征。在一个可选的实施例中,IOL的屈光力变化特征由包括柔性膜和晶状体或光学器件的在屈光力晶状体的封闭体积内的流体光学器件驱动。此实施例的一个显著优点是,隔开柔性膜和晶状体的屈光力晶状体的封闭体积保持大致恒定的体积,并且因此避免与需要体积显著变化的流体光学IOL相关联的许多问题,即,流体从贮存器供给到腔体中。具有变化体积的流体光学器件表现出的许多缺点包括不均匀屈光力变化和屈曲。本文公开的IOL和屈光力晶状体的某些实施例能够通过保持大致恒定的体积,同时在屈光力变化的整个范围内保持良好的光学质量来避免或减轻此类问题。另外,包括如本文所述的屈光力晶状体和基座组件的两部分组件的IOL的各种实施例提供较小的递送轮廓、较小的植入物轮廓或两者,从而需要用于植入的显著较小的切槽尺寸。这继而能够具有伴随优点,即降低与递送和植入较大IOL相关联的生物相容性问题,并且能够导致较快的愈合和更稳定的屈光。
本文公开的IOL能够以任何数量的方式配置。一般来说,IOL能够包括基座组件和能够耦合在基座组件内的屈光力晶状体。在一个实施例中,施加在植入的IOL上的径向压缩力能够集中在屈光力晶状体的柔性膜上,以引起柔性膜的曲率变化。屈光力晶状体的晶状体或光学器件能够被配置成响应于柔性膜的曲率变化而朝向柔性膜轴向位移。此轴向位移能够通过以允许光学器件浮动的方式将光学器件耦合到IOL的周边边缘来促进。随着屈光力晶状体的柔性膜的曲率变化,流体粘附或表面张力能够将屈光力晶状体的光学器件或晶状体拉向柔性膜。在另一个实施例中,施加在屈光力晶状体上的径向压缩力能够集中在光学器件上,以引起光学器件轴向位移并且在与光学器件轴向位移相同的方向上推动柔性膜,从而实现柔性膜曲率的变化。在又一个实施例中,屈光力晶状体上的径向压缩力能够施加在光学器件和柔性膜上,以分别引起轴向位移和曲率变化。
在另一个实施例中,施加在屈光力晶状体上的径向压缩力能够施加至柔性膜和光学器件中的一个或两个上,以引起柔性膜的曲率变化,并且引起光学器件朝向柔性膜的轴向位移,同时在径向压缩期间保持恒定体积。

本文公开的两部分调节性IOL装置的某些实施例由于其单独的两部分配置能够提供许多优点。两部分IOL装置的植入能够需要显著减小的切槽尺寸,因为IOL装置的两个部分(例如,基座组件和屈光力晶状体)被单独植入,并且因此显著减小植入的递送轮廓。减小的切槽尺寸能够提供许多优点,包括不需要麻醉和缝合来封闭切槽部位和改善手术结果。在一个实施例中,植入两部分调节性IOL装置所需的切槽尺寸小于约5mm、小于约4mm、小于约3mm或小于约2mm。换句话说,两部分调节性IOL装置的每个部分能够以递送轮廓小于约5mm、小于约4mm、小于约3mm或小于约2mm的递送状态提供。两部分调节性IOL装置的每个部分的输送状态能够在每个部分被卷起、折叠或以其它方式压缩以减小其递送尺寸时提供。能够在基座组件和屈光力晶状体中的一个或两个上设置铰合部或预定折痕,以便于折叠或卷到递送状态。
另外,在外科手术期间,在调整IOL的尺寸和屈光力方面提供更大的控制。将基座组件植入晶状体囊将为医师提供关于患者的晶状体囊尺寸的印象,并且因此将有助于选择随后将被植入基座组件中的屈光力变化的晶状体的正确尺寸。

图6和图7描绘复曲面基座组件600、700的实施例。基座组件600、700包括基座晶状体610、710和大致圆形的触觉系统620、720,触觉系统620、720具有外周边和具有不同柔性的至少两个区域。因为至少两个区域具有比触觉系统620、720的其余区域更大的柔性640、730,所以在触觉系统620、720的外周边上施加径向压缩力导致触觉系统620、720在更大柔性区域640、730中的不对称变形。换句话说,更大柔性区域640、730比触觉系统620、720的其余区域在更大程度上被径向压缩或可压缩。这种不对称变形继而又能够在基座晶状体610、710和提供在触觉系统620、720内的屈光力晶状体中的一个或两个中提供复曲面屈光力变化。

图7A和图7B分别提供根据本公开的实施例的替代复曲面基座组件700的透视图和俯视平面图。复曲面基座组件700包括基座晶状体710和触觉系统720。在图7A和图7B所示的复曲面基座组件700中,除了具有切口740以在触觉系统720的某些区域中产生更大的柔性之外,触觉系统720还包括延伸超过由触觉系统720限定的圆周的两个延伸区域730。延伸区域730和切口740导致压缩力的不均匀分布,从而导致基座晶状体710和插入的屈光力晶状体中的一个或两个的复曲面变形。
调节性IOL能够设置有允许改变轮廓的基座组件。能够通过配置基座组件使得屈光力晶状体被放置得更远离基座晶状体(即,高轮廓)或更靠近基座晶状体(即,低轮廓)来提供变化的轮廓。

调节性IOL的低轮廓放大调节性IOL 900的屈光力变化,如将在下面参照图10更详细所述。
图10A至图10D描绘低轮廓调节性IOL 1000的另一个实施例,其中低轮廓基座组件1010被配置为将屈光力晶状体1020放置在基座组件1010内更深的位置,紧邻基座晶状体1030,以利用特定IOL配置来产生可能的更大范围的光学屈光力。低轮廓调节性IOL 1000包括基座组件1010和被配置为固定在基座组件1010内的屈光力晶状体1020。基座组件1010包括触觉部分1015,以用于将施加至触觉部分1015的周边的力平移到屈光力晶状体1020。
如在图10D的剖视图中能够看出,屈光力晶状体1020包括柔性膜1050和屈光力晶状体光学器件1040,而基座组件1010包括触觉部分1015,以用于将施加至触觉部分1015的周边的力平移到屈光力晶状体1020和基座晶状体1030。基座晶状体1030和屈光力晶状体光学器件1040彼此接近,并且调节性IOL的低轮廓放大屈光力晶状体1020的屈光力变化。调节性IOL 1000的屈光力变化主要是通过调节屈光力晶状体1020产生的。此外,通过向膜1050施加力而不是向屈光力晶状体光学器件1040施加力来最有效地产生屈光力晶状体1020的调节。如此,当施加至触觉件1015的周边的力被有效地平移到膜1050时,调节性IOL 1000的屈光力变化最有效。
低轮廓调节性IOL 1000以若干方式有效地将力从触觉件1015平移到膜1050。首先,将屈光力晶状体1020放置在触觉件1015内更深的位置处导致力从触觉件1015到屈光力晶状体1020(其包括膜1050)的更有效的平移。这至少部分地由于这样的事实,即当屈光力晶状体1020定位在触觉件1015内的更高位置时,施加在触觉件1015的周边上的力的更大比例部分被分配到基座晶状体1030,而不是屈光力晶状体1020。分配到基座晶状体1030的力的部分不会显著影响调节和/或屈光力变化,并且因此,不能被有效地用于实现调节性IOL1000的屈光力变化。通过将屈光力晶状体1020在触觉件1015内移动得更低,力的更大比例部分被分配到屈光力晶状体1020,而不是损失到基座晶状体1030。此外,将屈光力晶状体1020放置在基座晶状体1030附近允许屈光力晶状体1020更靠近铰合部分1090,该铰合部分1090使分配到基座晶状体1030的力的至少一部分平移到屈光力晶状体1020。
如上所述,即使力被施加至屈光力晶状体1020,当力被施加至膜1050而不是屈光力晶状体光学器件1040时,调节也被最大化。在屈光力晶状体1020的周边的下部上的切口1080导致力被平移到膜1050而不是屈光力晶状体光学器件1040。通过移除屈光力晶状体1020的周边的下部(切口1080),触觉件1015主要与屈光力晶状体1020的周边的上部1070接合,并且力主要施加在膜1050(其在屈光力晶状体1020的上部上)上,而不是施加在屈光力晶状体光学器件1040(其在屈光力晶状体1020的下部上)上,从而影响屈光力晶状体1020中的更大的屈光力变化。
在图8A至图8C所示的实施例中,与图9和图10所示的实施例相比,调节性人工晶状体能够具有更高的轮廓。换句话说,在高轮廓实施例中,屈光力晶状体被定位成更靠近触觉系统810的开口端并且更远离基座晶状体830(图8A至图8C),而在低轮廓实施例中,屈光力晶状体920、1020被放置成更远离开口端并且更靠近基座晶状体940、1030(图9A至图9C以及图10A至图10D)。在图8C和图10D中参照屈光力晶状体的周边边缘距触觉件的外周边的第一边缘的距离h1来表示屈光力晶状体的位置。

在各种实施例中,本文描述的基座组件能够提供部分屈光力矫正。结果,能够安装基座组件,并且能够选择要安装的适当的屈光力晶状体,以允许在屈光度的1/4内进行矫正。此外,在各种实施例中,基座组件的光学器件能够包括UV吸收器或UV吸收剂涂层,以吸收进入患者眼睛的UV。在各种实施例中,浦肯野(Purkinje)图像能够用于调整屈光力晶状体或基座组件,以确保它们在安装调节性IOL期间被正确安装。
在各种实施例中,通过使用屈光力晶状体和基座组件,诸如本公开中描述的基座组件,本文公开的两部分调节性人工晶状体允许更有效地编目产品。例如,需要4个基座组件和9个屈光力晶状体以在半屈光度步长下覆盖12屈光度至27.5屈光度的范围。使用单晶状体需要32个不同的晶状体。由于只需要4个基座组件和9个屈光力晶状体,因此只需要13个库存单元(以下被称为“SKU”)来储存和组织屈光力晶状体和基座组件,而不是32个不同的SKU。这减少错位的机会,并且使组织更容易。相对于复曲面晶状体,效率的这种增加甚至更大。例如,假设4种不同的复曲面屈光力,对于32种单晶状体类型,需要128个SKU来组织晶状体。然而,对于两部分调节性IOL,假设基座组件的4种不同的复曲面屈光力,只需要16个基座组件和9个屈光力晶状体,总共25个SKU,这显著小于单复曲面晶状体所需的128个SKU。
在各种实施例中,本文所论述的基座组件的突片和凸缘能够与屈光力晶状体不同地着色,以允许外科医生看到屈光力晶状体何时正确地插入基座组件中。在各种实施例中,能够对屈光力晶状体或屈光力晶状体的边缘进行着色,以进一步帮助屈光力晶状体插入基座组件中。在各种实施例中,基座组件的基座晶状体或触觉系统能够进行着色。着色能够通过向任何前述部件的外表面添加粗糙度或涂层来实现。
在各种实施例中,浦肯野图像能够用于帮助将屈光力晶状体调整并且放置到基座组件中。植入后,图像能够用于调整屈光力晶状体和基座晶状体,以确保最佳配合。这种方法通过瞄准器的目镜为外科医生提供即时而简单的反馈。例如,如果外科医生在通过瞄准器观察时能够看到一个完美的圆,则外科医生能够接收到这样的指示,即屈光力晶状体被正确地放置在基座晶状体中。如果圆变形或畸形,则外科医生将知道他们必须继续调整屈光力晶状体。
Claims (10)
1.一种调节性人工晶状体装置,包括:
基座组件,所述基座组件包括第一开口端、耦合到基座晶状体的第二端以及围绕中心腔体的触觉件,其中所述触觉件包括外周边、内表面以及第一边缘和第二边缘之间的高度;
屈光力晶状体,所述屈光力晶状体被配置为配合在所述中心腔体内,所述屈光力晶状体包括第一侧、第二侧、耦合所述第一侧和所述第二侧的周边边缘以及被配置为容纳流体的封闭腔体;
其中所述屈光力晶状体的所述第一侧定位在距所述触觉件的所述第一边缘预定距离处。
2.根据权利要求1所述的调节性人工晶状体装置,其中所述预定距离为约0mm至约0.75mm。
3.根据权利要求1所述的调节性人工晶状体装置,其中所述预定距离为约0.75mm至约1.5mm。
4.根据权利要求1所述的调节性人工晶状体装置,其中所述预定距离为所述触觉件的所述高度的约38%至约75%。
5.根据权利要求1所述的调节性人工晶状体装置,其中所述预定距离为所述触觉件的所述高度的约0.01%至约37%。
6.根据前述权利要求中任一项所述的调节性人工晶状体装置,其中所述内表面面向所述中心腔体并且包括多个间隔开的接触点,所述多个间隔开的接触点被配置为接合所述屈光力晶状体的所述周边边缘的一部分。
7.根据前述权利要求中任一项所述的调节性人工晶状体装置,进一步包括沿所述触觉件的所述高度的至少一部分延伸的外凹槽,所述外凹槽被配置为允许所述触觉件被径向压缩、径向扩张或两者兼有。
8.根据权利要求7所述的调节性人工晶状体装置,其中所述外凹槽与所述内表面接触点相对地设置在所述外周边中。
9.根据前述权利要求中任一项所述的调节性人工晶状体装置,其中所述触觉件进一步包括径向向内延伸到所述腔体中的多个突片,并且其中所述屈光力晶状体通过所述多个突片固定到所述基座组件。
10.根据权利要求9所述的调节性人工晶状体装置,其中所述触觉件进一步包括径向向内延伸到所述腔体中的多个台,并且其中在所述多个突片和所述多个台之间形成通道,以将所述屈光力晶状体固定到所述基座组件。
