背景技术
在过去几十年里,已经发展了免疫疗法来克服免疫系统不能有效防范肿瘤或微生物的建立或有效排斥已建立的肿瘤或微生物的缺点。
在众多免疫疗法中,人们特别感兴趣的是基于细胞因子的免疫疗法。这些为可溶性分子的分子调节体液和/或细胞免疫。在这些分子中,人们更感兴趣的是IL-2、IL-7、IL-12和IL-15,这是由于这些分子诱导NK细胞的存活和/或增殖,因此其作为用于治疗感染或癌症的佐剂而被关注。
例如,已经证明人rIL-2在25-30%的转移性黑色素瘤或肾癌患者中导致肿瘤消退。间歇性IL-2疗法也与高活性抗逆转录病毒疗法联合用于HIV-感染患者中,该疗法恢复CD4+T淋巴细胞的持续的保护水平。

发明内容
现在,发明人已经惊奇的证实,与IL-15或IL-12任一个相比,他们的包含hIL-15氨基酸序列的分子(RLI)显示了非常不同的安全性,其中RLI的安全性更有利。最后,他们的研究结果表明RLI可用于对IL-2和IL-15而言是无法想象的治疗窗。
这种安全性使得高剂量RLI在治疗与不良预后相关的疾病中的应用和最低剂量RLI在治疗与正确或良好预后相关的疾病中的应用成为可能。
因此,第一方面,本发明涉及一种通过向受试者施用一定量的缀合物从而诱导天然杀伤细胞(NK细胞)增殖来治疗所述受试者的癌症、感染或免疫缺陷障碍的组合物,所述增殖与采用高剂量的白细胞介素-2(HDIL-2)得到的增殖相比相同或者更高,其中所述缀合物包含:
a)包含白细胞介素15或其衍生物的氨基酸序列的多肽,和
b)包含IL-15Rαsushi结构域或其衍生物的氨基酸序列的多肽。
第二方面,本发明涉及一种用于治疗受试者的癌症、感染或免疫缺陷障碍的方法,所述方法包括向有此需要的受试者施用一定量的缀合物从而诱导天然杀伤细胞(NK细胞)增殖的步骤,所述增殖与采用HDIL-2得到的增殖相比相同或者更高,其中所述缀合物包含:
a)包含白细胞介素15或其衍生物的氨基酸序列的多肽,和
b)包含IL-15Rαsushi结构域或其衍生物的氨基酸序列的多肽。

在第三优选的实施方案中,所述缀合物以诱导Treg细胞(FoxP3+CD4+CD25高)增殖的量施用于受试者,所述增殖比采用HDIL-2得到的增殖低。
第三方面,本发明涉及一种用于确定要向患有癌症、感染或免疫缺陷障碍的受试者施用的缀合物的治疗有效量的(体外)方法,所述方法包括以下步骤:
i)在使源自所述受试者的外周血单核细胞(PBMC)能够增殖的培养条件下,使所述PBMC与量渐增的如上所定义的缀合物接触;
ii)在使源自所述受试者的其它PBMC能够增殖的培养条件下,使所述其它PBMC与高剂量的白细胞介素-2(HDIL-2)接触;和
iv)筛选缀合物的治疗有效量,所述治疗有效量诱导所述PBMC的NK细胞增殖,所述增殖与采用HDIL-2得到的增殖相比相同或者更高。
进一步优选地,所述缀合物的量诱导的增殖NK细胞和/或CD8T细胞的百分数与Treg细胞的百分数的比率比采用HDIL-2得到的比率高至少25%;优选地高至少50%;和进一步优选地比采用HDIL-2得到的比率高至少75%。

本文所用术语“受试者”是指哺乳动物,例如啮齿动物、猫科动物、犬科动物或灵长类动物,且最优选的为人。
术语“缀合物”为其在本领域中的一般含义,是指共价或非共价复合物,其中优选的是指共价复合物,且最优选的是指融合蛋白。
术语“白细胞介素2”采用其在本领域中的一般含义(其核酸及氨基酸序列分别参见登录号NM_000586.3和NP_000577.2)。

优选地,本发明的组合的给药途径为肠胃外的;本文所用术语“肠胃外”包括静脉内、肌内、皮下、直肠、阴道或腹膜内给药。因此,所述药物组合物含有对于旨在被注射的剂型为药学上可接受的赋形剂。特别地,这些可以为等渗的、无菌的盐水溶液(磷酸二氢钠或磷酸氢二钠,氯化钠、氯化钾、氯化钙或氯化镁等,或这些盐的混合物),或干的、尤其是冷冻干燥的组合物,其根据情形加入无菌水或生理盐水时,允许构成可注射的溶液。合适的药物载体描述于E.W.Martin的“Remington'sPharmaceuticalSciences”中。
在这些肠胃外给药方式中,静脉内给药是最优选的。
所述缀合物可以溶解在缓冲液或水中,或者包含在乳液、微乳液、水凝胶(例如基于PLGA-PEG-PLGA三嵌段共聚物的水凝胶)、微球、纳米球、微粒、纳米粒(例如,聚(乳酸-共-乙醇酸)微粒(例如,聚乳酸(PLA);聚(丙交酯-共-乙醇酸)(PLGA);聚谷氨酸酯微球、纳米球、微粒或纳米颗粒)、脂质体或其它盖仑制剂中。在所有情形中,制剂必须是无菌的和注射可接受程度的液体。制剂必须在制备和贮存条件下稳定,并且必须防止例如细菌和真菌的微生物的污染作用。
分散体也可以在甘油、液体聚乙二醇及其混合物和油类中制备。在一般的储存和使用条件下,这些制剂含有防腐剂以防止微生物生长。
所述缀合物可以以中性或盐形式配制到组合物中。药学可接受的盐包括酸加成盐(与蛋白质的游离氨基形成),所述酸加成盐是与例如盐酸或磷酸的无机酸或例如乙酸、草酸、酒石酸、扁桃酸等的有机酸形成的。与游离羧基形成的盐也可以源自无机碱,例如氢氧化钠、氢氧化钾、氢氧化铵、氢氧化钙或氢氧化铁,以及有机碱例如异丙胺、三甲胺、组氨酸、普鲁卡因等。
所述载体还可以是包含例如水、乙醇、多元醇(例如甘油、丙二醇和液体聚乙二醇等)、它们合适的混合物及植物油的溶剂或分散介质。本发明的缀合物也可以被修饰,例如被聚乙二醇化,以提高其生物分布性(biodisponibility)。
例如,通过使用包衣(例如卵磷脂),在分散体的情况下通过维持所需的粒径和通过使用表面活性剂,可以保持适当的流动性。通过多种抗细菌剂和抗真菌剂,例如对羟基苯甲酸酯类、氯丁醇、苯酚、山梨酸、硫柳汞等,可以实现防止微生物的作用。在许多情况下,优选地包括等渗剂,例如糖或氯化钠。
可注射组合物的延长吸收可以通过在组合物中使用延迟吸收的试剂,例如单硬脂酸铝、明胶、多元醇、增长半衰期的共价和非共价制剂实现。
肽不稳定或降解的原因有许多,包括水解和变性。疏水性相互作用可引起分子集合到一起(即聚集)。可以加入稳定剂从而减少或防止这类问题。
稳定剂包括环糊精及其衍生物(参见美国专利No.5,730,969)。还可加入合适的防腐剂如蔗糖、甘露醇、山梨醇、海藻糖、葡聚糖和甘油以稳定最终制剂。可将选自离子型和非离子型表面活性剂、D-葡萄糖、D-半乳糖、D-木糖、D-半乳糖醛酸、海藻糖、葡聚糖、羟乙基淀粉、及其混合物的稳定剂添加至所述制剂中。碱金属盐或氯化镁的添加可以稳定肽。所述肽还可通过使其与选自葡聚糖、硫酸软骨素、淀粉、糖原、糊精和海藻酸盐的糖类接触而被稳定。其它可添加的糖类包括单糖、二糖、糖醇及它们的混合物(例如,葡萄糖、甘露糖、半乳糖、果糖、蔗糖、麦芽糖、乳糖、甘露醇、木糖醇)。多元醇可稳定肽,且是可与水混溶的或水溶性的。合适的多元醇可以是多羟基醇、单糖和二糖,包括甘露醇、甘油、乙二醇、丙二醇、三甲基二醇、乙烯基吡咯烷酮、葡萄糖、果糖、阿拉伯糖、甘露糖、麦芽糖、蔗糖及它们的聚合物。各种辅料也可稳定肽,包括血清白蛋白、氨基酸、肝素、脂肪酸和磷脂、表面活性剂、金属、多元醇、还原剂、金属螯合剂、聚乙烯吡咯烷酮、水解明胶和硫酸铵。
在本发明的上下文中,本文所用术语“治疗”指逆转、减缓、抑制应用该术语的障碍或病症或这样的障碍或病症的一个或多个症状的发展,或预防所述障碍或病症或这样的障碍或病症的一个或多个症状。
本文所用术语“治疗癌症”是指癌细胞的生长抑制。优选地,所述治疗还导致肿瘤生长的消退,即,可测量肿瘤的尺寸的减小。最优选地,这种治疗导致肿瘤的完全消退。
本文所用术语“治疗感染”是指微生物复制/增殖的抑制。
本文所用术语“治疗免疫缺陷障碍”是指NK细胞和/或T细胞的诱导。
缀合物的“有效量”是足以诱导肿瘤生长或微生物复制的消退的量。用于给药的剂量可作为多种参数的函数而采用,特别是作为使用的给药模式、相关病理学或可选地期望的治疗持续时间的函数而采用。自然地,药物组合物的形式、给药途径、剂量和治疗方案自然地取决于将被治疗的病症、疾病的严重程度、受试者的年龄、体重和性别等。下文中提供的有效剂量的范围不旨在限制本发明和代表优选的剂量范围。然而,如本领域技术人员理解和可确定,可为单独的受试者定制优选的剂量,而无需过多实验。
由于本发明的缀合物的非常重要的安全性,可以设想其给药在治疗癌症、感染和免疫缺陷障碍中具有非常重要的治疗窗,这一治疗窗远超限制的IL-2治疗窗,且也远超设想的IL-15的治疗窗。
这种安全性使得能够设想使用1)非常高剂量的RLI用于治疗不良预后的慢性疾病(如转移性肾腺癌或黑色素瘤)和2)低剂量的RLI用于治疗良好预后的疾病。
所述给药量还诱导CD8T细胞的增殖,所述增殖高于采用HDIL-2得到的增殖。
作为一个实例,所述至少一种缀合物的有效量高于40fmol/kg或0.2pmol/kg(1ng/kg或5ng/kg),优选地高于1pmol/kg或2pmol/kg(25ng/kg或50ng/kg),进一步优选地高于4pmol/kg(100ng/kg)、20pmol/kg(500ng/kg)或甚至高于40pmol/kg(1mcg/kg)。由于缀合物自身的分子量和活性可能影响剂量,因而其它剂量是可行的。本领域技术人员被认为很容易地确定落入所述范围内的适合的剂量,或如果需要,确定所述范围之外的适合的剂量。
作为另一个实例,所述至少一种缀合物的有效量对应于高于4fmol/ml(0.1ng/ml)、优选地高于40或80fmol/ml(1或2ng/ml)、和进一步优选地高于0.160pmol/ml(4ng/ml)的血药浓度。
有利地,所述至少一种缀合物的给药量低于2.4nmol/kg(60mcg/kg),优选地低于2nmol/kg(50mcg/kg)或1.2nmol/kg(30mcg/kg),和进一步优选地低于1.0nmol/kg(25mcg/kg)或甚至低于200pmol/kg(5mcg/kg)。
进一步有利地,所述至少一种缀合物的给药量对应于低于0.12nmol/ml(3,000ng/ml)、优选地低于80或40pmol/ml(2,000或1,000ng/ml)、和进一步优选地低于20pmol/ml(500ng/ml)或甚至低于12pmol/ml(300ng/ml)的血药浓度。
所述给药量还诱导CD8T细胞的增殖,所述增殖高于采用HDIL-2得到的增殖。

如本文所使用,与不良预后有关的疾病是其中中位生存期小于2年、优选地小于1年、和进一步优选地小于6个月的疾病。
如本文所使用,与不良预后有关的疾病为晚期的(TNM第IV期)或转移性癌症。
在所述实施方案中,所述缀合物以诱导天然杀伤细胞(NK细胞)增殖的量施用,所述增殖比采用HDIL-2得到的增殖高至少20%;优选地高25%;和进一步优选地比采用HDIL-2得到的增殖高至少30%。

进一步惊奇的是,本发明的发明人证实采用调节性T细胞诱导得到的所述IL-15衍生的NK细胞诱导低于采用HDIL-2得到的NK细胞诱导。
在第三个优选的实施方案中,所述缀合物以诱导Treg细胞(FoxP3+CD4+CD25高)增殖的量施用于受试者,所述增殖低于采用HDIL-2得到的增殖。
有利地,所述缀合物以诱导Treg细胞增殖的量施用于受试者,所述诱导比采用HDIL-2得到的增殖低至少5%;优选地低至少10%或20%;和进一步优选地比采用HDIL-2得到的增殖低至少50%。
因此,由于最小的调节性T细胞诱导,采用IL-15衍生物得到的NK和CD8细胞诱导比由HKIL-2诱导的更高效。
优选地,所述缀合物以这样一种量施用于受试者,所述量的诱导的增殖NK细胞的百分数与诱导的增殖Treg细胞的百分数的比率比采用HDIL-2得到的比率高至少25%;优选地高至少50%;和进一步优选地比采用HDIL-2得到的比率高至少75%。
优选地,所述缀合物以这样一种量施用于受试者,所述量的诱导的增殖CD8T细胞的百分数与诱导的增殖性Treg细胞的百分数的比率比采用HDIL-2得到的比率高至少25%;优选地高至少50%;和进一步优选地比采用HDIL-2得到的比率高至少75%。
在上述剂量范围内缀合物的引入可以作为单一治疗或经过一系列治疗进行。事实上,尽管单一剂量提供益处且对于疾病的治疗/管理可被有效利用,但是一个优选的治疗过程可发生在几个阶段;最优选地,所述给药量对应于每天的给药量。这一量可以每天给药一次共1至20天,如1至10天,优选地2至5天,和最优选地2至4天。现在,所述给药量可以是一个长期持续的形式,从而形成具有相似的缀合物每日血药浓度的长期给药。
另一方面,本发明涉及一种用于确定要向患有癌症、感染或免疫缺陷障碍的受试者施用的缀合物的治疗有效量的方法,所述方法包括以下步骤:
i)在使源自所述受试者的外周血单核细胞(PBMC)能够增殖的培养条件下,使所述PBMC与量渐增的如上所定义的缀合物接触;
ii)在使源自所述受试者的其它PBMC能够增殖的培养条件下,使所述其它PBMC与高剂量的白细胞介素-2(HDIL-2)接触;和
iv)筛选缀合物的治疗有效量,所述治疗有效量诱导所述PBMC的NK细胞增殖,所述增殖与采用HDIL-2得到的增殖相比相同或者更高。
优选地,所述缀合物的治疗有效量诱导所述PBMC的CD8T细胞增殖,所述增殖与采用HDIL-2得到的增殖相比相同或者更高。
所述筛选的治疗有效量适用于治疗所述受试者的癌症、感染或免疫缺陷障碍。
进一步优选地,所述治疗有效量与诱导的增殖NK细胞和/或CD8T细胞的百分数与诱导的增殖性Treg细胞的百分数的比率有关,所述比率比采用HDIL-2得到的比率高至少25%;优选地高至少50%;和进一步优选地比采用HDIL-2得到的比率高至少75%。
现在,所述缀合物的治疗有效量诱导NK和/或CD8T细胞的增殖,所述增殖比采用不含缀合物(即也不含HDIL-2和IL-15)的培养基得到的增殖高至少50%。
在HDIL-2存在下,使PBMC能够增殖的培养条件是本领域技术人员公知的且描述于实施例中。
量渐增的缀合物对应于4fmol/ml至120pmol/ml(0.1至3,000ng/ml)、优选地40fmol/ml至80pmol/ml(1至2,000ng/ml)、和进一步优选地80fmol/ml至40pmol/ml(2至1,000ng/ml)的缀合物浓度。
HDIL-2是本领域技术人员公知的且对应于采用50IU/mL的PBMC培养(MURPHY,WELNIAK,BACK等,J.Immunol.,vol.170,p:2727–33,2003;ITOH等,CancerImmunol.Immunother.,vol.32(2),p:88-94,1990;ETTINGHAUSEN&ROSENBERG,CancerRes.,vol.46(6),p:2784-92,1986)。

在下文中,参照氨基酸序列、核酸序列和实施例更详细地描述本发明。然而,本发明不旨在受实施例的细节的限制。相反,本发明适于包括在本文实施例中未明确提及、但本领域技术人员无需过多努力可发现的细节的任何实施方式。
实施例
为了确定RLI效力,我们首选采用了对应于从源自人健康供体的人外周血单核细胞(PBMC)纯化得到的NK和CD8T细胞的体外模型。
1)通过RLI的人淋巴细胞增殖诱导
通过FICOLL-HYPAQUE梯度法(LYMPHOPREPTM;1.077g/mL)从健康志愿者分离外周血单核细胞(PMOLBC)。根据官方伦理协议获得供体血液。
简言之,用2,5μM的CFSE(羧基荧光素琥珀酰亚胺酯)标记PBMC5分钟,用NaCl洗涤。然后PBMC在37℃下在加湿的95%空气和5%CO2中培养3至7天。收集细胞并用抗-CD3、CD4、CD8、CD56和FixableAqua染色细胞从而筛选存活细胞。染色的细胞可在FACSCantoII流式细胞仪(BDBIOSCIENCES)上快速获得并且采用FLOWJO软件(TREESTAR)对其进行分析。
在100μL的完全培养基(RPMOLI1640+10%热灭活的胎牛血清(FBS)+1%的L-谷氨酰胺+1%的非必需氨基酸+1%的丙酮酸钠+1%的青霉素-链霉素)中,在96-孔U型底板中培养2×105个PBMC/孔。然后,向培养物中加入100μL的2X培养基从而使在CHO细胞中产生的RLI(SEQIDNO:17或SEQIDNO:18)的最终浓度为2,5pg/ml、25pg/ml、250pg/ml、2,5ng/ml或25ng/ml。
作为阴性对照,用培养基培养PBMC。
作为阳性对照,用50IU/mL(3ng/mL)的人IL-2(PROLEUKIN,NOVARTISPHARMA)培养PBMC,所述量对人用途而言相当于高剂量的IL-2(MURPHY,WELNIAK,BACK等,J.Immunol.,vol.170,p:2727–33,2003;ITOH等,CancerImmunol.Immunother.,vol.32(2),p:88-94,1990;ETTINGHAUSEN&ROSENBERG,CancerRes.,vol.46(6),p:2784-92,1986)。另外作为阳性对照,我们采用了对应于与HDIL-2相同摩尔浓度的2.5ng/mL的重组人IL-15(CELLGENIX,PRECLINICAL)。
自第3天至第7天每天通过CFSE稀释来测定增殖NK细胞、CD8+T细胞和CD4+T细胞的百分数。
图1示出了RLI对人外周血单核细胞的体外剂量-效应(A)。在第0天用CFSE染色人PBMC,且之后用剂量递增浓度的RLI(2.5、25、250、2500和25000pg/mL)处理4天和7天。在第4天和第7天,通过荧光活化细胞分选(FACS)收集、染色和分析PBMC。未经处理的对照细胞同时在培养基中单独培养。在排除死细胞和重影(doublet)后,认为CD3-CD56+是NK细胞,认为CD3+CD8+细胞是CD8+T细胞,认为CD3+CD4+细胞是CD4+T细胞。增殖NK细胞(左侧小图)、CD8+T细胞(中间小图)和CD4+T细胞(右侧小图)的比例呈现在上侧小图1A中。图1B示出了RLI、rhIL-15和rhIL-2的增殖能力。用2.5ng/mL的RLI、2.5ng/mL的rhIL-15和50UI/mL(3ng/mL)的rhIL-2处理人PBMC4天和7天。

如表1至3和图1所示,结果显示低至25pg/mL(1fmol/ml)剂量的RLI能够诱导NK细胞的一定体外增殖,而低至250pg/mL剂量的RLI能够诱导CD8+T的一定体外增殖。
如表1和表2所示,第一天,剂量为25和250pg/mL的RLI分别诱导与2,500pg/ml的rhIL-15等量的NK细胞和CD8+T细胞的增殖。考虑第7天的增殖,剂量为250pg/mlRLI(是rhIL-15的10倍低)诱导的NK细胞增殖比rhIL-15高300%,且剂量为25pg/ml的RLI(是rhIL-15的100倍低)诱导的NK细胞增殖比rhIL-15高50%。在同一天,剂量为25pg/ml的RLI(是rhIL-15的100倍低)诱导的CD8+T细胞增殖比rhIL-15高100%。因此,参考这些参数,RLI的生物活性是rhIL-15的10至100倍。
相反,与3,000pg/mL的IL-2相比,剂量为250和2500pg/mL的RLI对于NK和CD8+T细胞分别显示出了更高的增殖能力(需注意,等摩尔剂量的IL-2为150至1500pg/ml,即3至30UI/mL)。对于NK细胞,结果显示,RLI诱导的NK细胞增殖比rhIL-2高30%,除了25pg/ml(是rhIL-2的100倍低)的剂量,而2.5pg/ml(是rhIL-2的1000倍低)时相同。对于CD8+T细胞,250pg/ml剂量的RLI的效力比hhIL-2高近400%,25pg/ml剂量的RLI的效力比hhIL-2高近100%,而2.5pg/ml剂量的RLI的效力与hhIL-2相同。因此,参考这些参数,RLI的生物活性是rhIL-2的至少2至10倍。
用等摩尔浓度的RLI、rhIL-2和rhIL-15重复相同的实验。
图12A和12B示出了RLI、rhIL-15和rhIL-2在第3、4、5、6和7天分别对NK细胞和CD8T细胞的增殖能力。
结果证明,自激活后的第3天直至第7天,RLI诱导NK细胞和CD8T细胞的增殖,所述增殖比采用等摩尔的rhIL-15和等摩尔的rhIL-2得到的增殖高。
2)人调节性T细胞和RLI
如在其它地方公开的(MIYARA等,Immunity,vol.30(6),p:899-911,2009)已经分析了Treg细胞。这一策略使得活化Treg(Foxp3高CD4+T细胞)、静息自然Treg(Foxp3低CD45RA+CD4+T细胞)和活化效应CD4+T细胞(Foxp3低CD45RA-CD4+T细胞)之间的辨别成为可能。
简言之,按照上文1)中所述获得CFSE标记的PBMC。用rhIL-15(2.5ng/mL)、rhIL-2(50IU/mL=3ng/mL)、RLI毕赤酵母(Pichia)(2.5ng/mL)或仅培养基在6孔板中培养源自健康志愿者的两百万的PBMC6天。然后收集细胞并用抗-CD3、抗-CD4、抗-CD8和FixableAqua染色细胞从而筛选存活细胞。在Foxp3固定/透化实验规程(EBIOSCIENCE)之后细胞被透化并用抗-Foxp3染色。用流式细胞仪立即获得标记的细胞。
图2A示出了增殖Foxp3-CD4+T细胞的比例。
图2B示出了增殖Foxp3+CD4+T细胞(左侧小图)、Foxp3低CD4+T细胞(中间小图)和Foxp3高CD4+T细胞(右侧小图)的比例。
结果表明RLI不诱导Foxp3+CD4+T细胞亚集的任何增殖。另一方面,rhIL-2诱导包含Foxp3高CD4+T细胞(高度抑制性Treg细胞)的Foxp3+CD4+T细胞亚集的强增殖。
=>为了更好地定义RLI的体内性质,我们决定采用以下两个互补的动物模型,所述模型对应于:
1)第一,猕猴,其是很好的用于研究药物活性和药代动力学的体内模型;和
2)第二,小鼠,其是很好的用于研究细胞因子副作用的体内模型,和更特别地由于猕猴不能用于预测人VLS,其是很好的用于研究血管渗漏综合征(VLS)的体内模型。
3)小鼠:RLI安全性论证
同时,我们希望测定与相似剂量的hIL-2和hIL-15相比RLI的安全性。基于此目的,我们采用小鼠作为免疫细胞激活和人VLS的动物模型。首先,我们测定在这一动物模型中RLI的活性。
a)RLI在体内模型小鼠中的生物活性
按照图3中呈现的实验规程,向从Harlan实验室获得的C57BL/6小鼠腹腔内(i.p)注射100μL的PBS作为阴性对照,注射rhIL-2(250000IU/小鼠)作为阳性对照,注射rhIL-15(1.2μg/小鼠)作为比较和注射RLI(2.5μg/小鼠)。
第4天时,通过颈椎脱臼法处死小鼠并取出脾。然后将脾在100μm细胞过滤器上用注射器的背部在单细胞混悬液中分解。然后用ACK溶液(铵-氯-钾)裂解血细胞。在完全培养基中洗涤脾细胞两次并采用KOVA载玻片对存活细胞进行计数。两百万的脾细胞被如下抗体染色:抗-CD3、抗-CD4、抗-CD8、NKp46和FixableAqua,从而筛选存活细胞。然后根据制造商的实验规程(EBIOSCIENCEFoxP3透化缓冲剂),脾细胞被透化并被抗-FoxP3和Ki67染色。Ki67的同种型用于鉴别阳性细胞。在FACSCantoII流式细胞仪上快速获得染色的细胞并且采用FLOWJO软件(TREESTAR)进行分析。NK细胞为CD3阴性的NKp46阳性细胞。分析CD8+T细胞,对CD3和CD8双阳性细胞设门。对于调节性T细胞的分析,实现了Foxp3的核内染色,以自CD4和CD3双阳性群中的效应T细胞中区分调节性T细胞。
图4示出在第4天增殖NK细胞、CD8+T细胞、Foxp3+CD4+T细胞和Foxp3-CD4+T细胞的比例。
图5示出了在第7天增殖NK细胞与Foxp3+T细胞(Treg)的比率。
结果表明,与IL-2和IL-15相比,RLI诱导效应细胞的强增殖而不诱导Treg的积累;在第4天和第7天,这些结果是相同的。
然后,我们测定了与IL-2和IL-15相比在这一动物模型中的由RLI的免疫细胞活化。
对于此,根据上文所述实验规程获得脾细胞。
对于分泌实验,将脾细胞在补充有5ng/mL的PMOLA(佛波醇12-肉豆蔻酸酯13-乙酸酯)和500ng/mL离子霉素的完全培养基中培养4小时。BrefelfinA溶液用于抑制蛋白质转运(EBIOSCIENCE)。然后收集细胞并用下列抗体染色细胞:抗-CD3、CD4、CD8、NKp46和FixableAqua,从而筛选存活细胞。表面染色后,根据制造商的实验规程(BDBIOSCIENCES,细胞内染色)固定且透化细胞。然后透化的细胞用抗-IFNγ抗体染色并在FACSCantoII流式细胞仪上快速获得。
对于体外细胞毒性实验,采用小鼠NK细胞分离试剂盒II(MILTENYIBIOTECH)富集NK细胞。通过流式细胞术控制纯度。2×104个YAC-1细胞与不同量的NK细胞(效应物:靶物比为(1:1)、(5:1)和(10:1))在96孔V-型底板中培养。最终体积为100μL每孔。4小时的共培养后,收集上清液并采用LDH细胞毒性检测试剂盒(RocheAppliedScience)测定释放的LDH。按照如下公式计算细胞毒性百分数:
细胞毒性(%)=[((“效应物:靶物”–“效应细胞对照”)–“低对照”)/(“高对照”–“低对照”)]×100。
图6示出了(A)测定NK细胞(左侧小图)、CD8+T细胞(中间小图)和CD4+T细胞(右侧小图)中IFNγ产生细胞的百分数,和(B)注射了PBS、IL-2、IL-15或RLI的小鼠中针对YAC-1细胞系(B)的NK细胞的细胞毒性。
结果表明,在小鼠体内对于NK细胞和CD8+T细胞,与等摩尔剂量的rhIL-15和IL-2的高剂量治疗方案(250,000IU/天,即15μg/天)相比,RLI显示出更强的生物活性。
因此,这些数据证实了体外人数据所显示的,每三天低至2μg/注射剂量的RLI比每天注射15μg的IL-2的生物活性高。
为了更好地评估RLI的安全性,我们同时在一个实验的体内肿瘤模型中比较了这些细胞因子疗法的抗肿瘤活性。
基于此目的,将106个B16F10黑色素瘤细胞注射入小鼠背部真皮的上部。在肿瘤接种后的第6天根据之前所述的治疗方案治疗,在这一时间点的肿瘤结节的尺寸为≈50-55mm3,是清晰可见且可触知的。使用卡尺以两个垂直的直径测量可触知肿瘤,且半径通过将平均直径除以2估算得到。假定肿瘤球形生长,肿瘤体积使用公式4/3(πr3)计算。
图9(A)示出了取决于细胞因子治疗方案的肿瘤体积的演变。图9(B)示出了在用所示试剂处理的小鼠中皮下肿瘤生长的曲线下面积(AUC)。数据代表两个独立的实验。
如图9A和9B所示,与PBS组相比,LDRLI将原发性肿瘤生长降低47%,这与将原发性肿瘤生长降低46%的HDRLI相似。LDIL-2无治疗效果,而LDIL-15具有非常适度的治疗效果(对原发性肿瘤生长-9%)。有趣的是,HDIL-2和HDIL-15对原发性肿瘤呈现出适度的但显著的治疗效果(分别为-22%和-28%)。尽管对CD8T和NK细胞具有相似的效果,但是IL-15/IL-15Rα-Fc将原发性肿瘤生长降低37%,这低于采用LD和HDRLI所降低的百分数。然而,采用IL-15/IL-15α-Fc的CD8/CD4Treg和NK/CD4Treg的比率不如采用RLI有利。尽管对CD8T和NK细胞具有相似的效果且CD8/CD4Treg和NK/CD4Treg的比率不如采用RLI有利,IL2/602mAb将原发性肿瘤生长降低51%,这稍高于采用LD或HDRLI所降低的百分数。这表明,与CD8T和NK细胞与CD4Treg的相对比率相比,控制B6F10原发性肿瘤生长的最重要的免疫驱动子更与CD8T和NK细胞的数量扩增相关。然而,许多研究涉及小鼠和人癌症中的CD4Treg的发展和活性,所述CD4Treg作为有利于通过免疫逃逸的肿瘤发展的关键免疫抑制细胞。
进一步地,转移性肾细胞癌(Renca)的体内实验证实了IL-15和IL-2对原发性肿瘤生长的适度但显著的治疗效果,而在第1-4天每天腹腔内注射2μgRLI观察到了强肺转移性发展抑制。
b)血管渗漏综合征(VLS)
富集的CFSE标记的Ly5.1+CD8+T细胞被转移至野生型小鼠中,随后4天每天注射PBS、1.5μg(低剂量,LD)或15μg(高剂量,HD)的重组人细胞因子(包括LDIL-2、LDIL-15、HDIL-2和HDIL-15)、1.5μg细胞因子加抗-人细胞因子抗体(IL-2/602)、1.5μgIL-15加可溶的IL-15Rα-Fc(IL-15/sIL-15Rα也被称为IL-15非共价复合物)和2.25μgRLI(LDRLI)或15μgRLI(HDRLI)。基于所述HDIL-2安全性(即可接受的风险-收益平衡),这一IL-2剂量可被认为是VLS诱导的最高限制剂量。
在第5天,对脾细胞进行了(A)供体Ly5.1+CD8+细胞、宿主CD44高CD122高记忆-表型CD8+T细胞(MPCD8+)、CD4+CD25+调节性T细胞(Treg)和CD3-NK1.1+天然杀伤细胞(NK)的CFSE分布分析。
图7示出了计算供体细胞、MPCD8+T细胞和NK细胞的总细胞数。数据代表两个独立的实验。
结果显示,LDRLI诱导了转移的Ly5.1+CD8+T细胞的强的增殖和扩增,富集了CD122+CD44+细胞(效应和中央记忆CD8T细胞),为89%的增殖细胞相对于HDRLI组中97%的增殖细胞。因此,LDRLI或HDRLI诱导靶细胞拟相似的药理作用,其这意味着达到最大药理作用无需RLI的这种极高浓度。
与等摩尔的LDIL-2(12%)、LDIL-15(13.5%)和甚至HDIL-2(52%)或HDIL-15(62%)相比,LDRLI诱导更强的转移的CD8T细胞增殖。
此外,LDRLI和HDRLI与IL-2(IL2/602mAb;98%)和IL-15(IL-15/IL-15Rα-Fc;98%)的超激动剂非共价复合物吻合良好。
结论是,RLI对扩增NK和CD8T细胞是高效的,且对CD4Treg的扩增具有更有限的效力,从而表明在所有测试的试剂和治疗方案中,其在将免疫调节平衡朝向免疫细胞毒性移动而不扩大免疫抑制方面具有最好的能力。
对于测定血管渗漏综合征(VLS),我们评估了与湿组织与干组织重量之间的比率相关的肺水肿。将小鼠在麻醉下放血。收集肺,立即称重并在50℃下干燥处理2天。通过由肺的湿重减去干重得到水侵量。
图8示出肺水肿(高于虚线)作为小鼠总重量的百分比。虚线代表生理背景水平。数据代表两个独立的实验。
与PBS组的正常肺湿重(PWW)相比,LDIL-15和LDIL-2分别诱导7.3%和17.9%的适度的PWW增长。HDIL-15和HDIL-2分别增长PWW约54.5%和120%。HDIL-2诱导一种非常重要的PWW增长,这与出现在用HDIL-2治疗的某些患者中的血管渗漏综合征一致。对于HDIL-15,尽管PWW增长并非不显著,但该PWW增长远低于由HDIL-2所诱导的增长。
令人惊讶的是,结果显示,与VLS的最高限量相比,以低剂量和高剂量的RLI诱导的VLS看起来是可接受的,而通过RLI的NK和CD8+细胞诱导比通过hIL-15和hIL-2得到的诱导高(高于3倍)。
除具有较低的治疗功效外,IL-15/IL-15Rα-Fc增长PWW76.4%,这是由LDRLI诱导的VLS的两倍。尽管具有类似的治疗功效,IL-2/602mAb增长PWW62.6%,这比由LDRLI诱导的VLS高39%。此外,有趣的是我们注意到,尽管对NK细胞和CD8T细胞具有拟相似的药理作用和拟相似的治疗功效,HDRLI增长PWW96.7%,而LDRLI组增长PWW38.2%。因此,尽管就NK细胞和CD8T细胞的活性和治疗活性而言,LDRLI和HDRLI吻合良好(这意味着采用该LD治疗方案即能达到效力稳定状态),增加RLI剂量可诱导更高VLS,这意味着这一毒性效应与涉及治疗功效的机制无关。
为了表示RLI相对于IL-2和IL-15的安全性(即高效性和低毒性),图11给出了分别注射PBS、IL-2、IL-15和RLI的小鼠的VLS随着NK细胞和CD8+T细胞变化。
最后,我们的结果表明,与IL-2和IL-15的安全性相比(尽管IL-15和RLI可能使用相同的信号通路),RLI具有非同寻常的安全性,其中RLI的安全性比IL-15和IL-2的安全性更有利得多。所以,RLI与IL-15和IL-2相比呈现改善的剂量幅度和治疗窗,从而将效应免疫细胞影响为诱导无副作用的治疗效果。相比之下,采用IL-2,甚至是采用IL-15来实现免疫系统的正确激活而不迅速诱导VLS现象看起来是困难的。
c)对NK细胞扩增相对于毒性(VLS)的剂量-反应效应
根据前述的实验规程评估RLI对NK细胞扩增相对于在肺和肝中的VLS的剂量范围效应。自第0天至第3天,小鼠接受4天的RLICHO的注射,其中每次腹腔内注射0.2μg、0.5μg、1μg或2μg,然后在第4天处死小鼠。
图13示出RLI对NK细胞扩增的剂量-反应效应。
结果表明,RLI治疗诱导脾中NK细胞的剂量依赖性扩增,其中启动效应从首剂0.2μg开始直至2μg,稳定状态开始于1μg。
平行于NK扩增,评估了处理过的小鼠相对于对照组小鼠(PBS)的肺和肝中的VLS。
图14示出了RLI对小鼠中肺和肝中的VLS的剂量-反应效应。
结果表明,与未处理的小鼠的PWW相比,RLI的实验剂量不诱导显著的肺和肝中的VLS。现在,在最高剂量2μg时,可认为出现了VLS的一个新兴信号。
再次,根据前述的实验规程采用两个新的剂量评估RLI对NK细胞扩增相对于在肺中的VLS的剂量范围效应。自第0天至第3天,小鼠接受4天RLICHO的注射,其中每次腹腔内注射0.2μg、0.5μg、1μg、2μg、5μg或25μg,然后在第4天处死小鼠。
图15示出了RLI对肺中的VLS的剂量-反应效应。
再次,剂量为0.2μg至2μg的RLI未诱导肺中的VLS,而剂量为5μg和25μg的RLI诱导了具有相似的和最大强度的强VLS。
对于NK细胞,图16示出了RLI对NK细胞扩增的剂量反应,而图17示出了RLI对Treg扩增的剂量反应。
再次,每次注射剂量为0.2至5μg的RLI诱导脾中NK细胞的剂量依赖性扩增,而剂量为25μg时,RLI失去活性且被认为是有害的。平行地,剂量为0.2至2μg的RLI不增加Treg的百分数,而与对照组相比,这些治疗方案诱导Treg的数量的相似增长。相比之下,最高剂量的RLI(5μg和25μg)增长CD4Treg的百分数和数量。
图18示出了RLI对CD4Treg相对于NK细胞的百分数配给的剂量-反应效应,结果证明RLI对NK细胞增殖呈现特定的剂量依赖活性,而对CD4Treg无特定活性。此外,0.2和2μg之间的剂量幅度表现为有效且安全,这反映出刺激细胞毒性免疫细胞如NK细胞而不引起VLS的可能性。这一药物幅度范围的存在对掌握在患者中的疗效和安全性是至关重要的。
图19概括了在健康小鼠中,通过RLI治疗诱导的相对于VLS的NK细胞和CD8T细胞刺激之间的比较,以及在Renca细胞转移发展中的细胞治疗效果。
最后,这些结果证明了与IL-15和IL-2相比,RLI的非常高的安全性,且证明了RLI可用于对IL-2和IL-15而言无法想象的治疗窗:极高剂量用于不良预后患者和极低剂量用于良好预后患者。
采用猕猴的药代动力学
向4岁大的约3至4kg的食蟹猕猴(Cynomolgusmacaques)以不同剂量(2个猕猴以20mcg/kg;2个猕猴以3.5mcg/kg;猕猴)注射RLI(静脉推射,15min)。
本研究是在符合如OECD文件“实验室良好实施规范(principlesofgoodlaboratorypractice)”(1997年修订)中所述的现行GLP法规的前提下进行的。这一实验规程由EthicsCommitteeofVETAGROSup(法国)修订且批准号为1162。所有的实验将根据2001年2月13日公开在法国官方公报的欧盟指令86/609/EEC进行。
药代动力学特征的分析通过实施针对不同的时间点的血清中的RLI的ELISA生物测定进行:t-72h记为t0、t+10min、t+30min、t+1h、t+4h、t+6h、t+24h和t+72h。采用两室模型分析基于所测定的浓度的实验曲线,根据一般方程所述两室模型具有零级静脉注射:拟合浓度==IF(time<tinf;InfR*A*(1-EXP(-_lbd1*time))+InfR*B*(1-EXP(lbdz*time));InfR*A*(EXP(_lbd1*time))*EXP(-_lbd1*time)+InfR*B*(EXP(lbdz*time))*EXP(-lbdz*time))。(拟合浓度公式(EXCEL,拟合分析,具有零级静脉输液的两室模型))。
图10A示出取决于随着时间变化的注射剂量,观察到的和模型化的RLI浓度的演变。数据代表每一剂量的两个不同猕猴。
每一实验的第二隔室的半衰期(t1/2β)约为3小时。拟合曲线可用于评估在延长的时间中整个血流中RLI的剩余浓度,这表示在图10(B)中。
结果表明,对于20mcg/kg组,在6h和12h时的血液浓度分别为28.67和6.02ng/ml。对于3.5mcg/kg组,在6h和12h时的血液浓度分别为0.55和0.03ng/ml,即分别为550pg/ml和30pg/ml。
根据表1和表2,如此低和非常低的浓度对于分别刺激人类NK细胞和CD8T细胞是高度有效的。现在,所述NK细胞和CD8T细胞的增殖诱导在猕猴中得到证实,同时,未观察到对Treg的作用(数据未示出)。
为了更好地将动物剂量向人类等效剂量转化,存在基于BSA的理论模型。
人类等效剂量(HED)的测定可以基于以下公式获得:
HED(mg/kg)=动物剂量(mg/kg)×(动物Km÷人类Km)
在这一公式中,Km是反映体重和体表面积之间关系的校正因子。
对于一个典型的成人(体重为60lb.,体表面积为1.6m2),Km为37。
对于最常用的实验动物物种的平均Km,小鼠为3、兔为6、猕猴为12、狗为20、成人为37(参见“CenterforDrugEvaluationandResearch,CenterforBiologicsEvaluationandResearch.(2002)Estimatingthesafestartingdoseinclinicaltrialsfortherapeuticsinadulthealthyvolunteers,U.S.FoodandDrugAdministration,Rockville,Maryland,USA”)。
在所述要素的基础上,参照小鼠的实验,我们可以估算一些RLI的最大人类等效剂量。所述剂量汇总于表4。

有趣的是,我们已经表明,高剂量(25ng/ml)和非常低的剂量(2.5pg/ml或25pg/ml)均能够刺激离体人PBMC的人NK细胞和CD8T细胞(表1和表2,图1)。在猕猴中,这种弱或非常弱的浓度可以在峰值血清水平(0.25小时)(图10A和B)时达到。例如,根据实验曲线的拟线性回归分析,当食蟹猕猴的剂量为0.01mcg/kg时,最大血药浓度可达到约62.5pg/ml(图10B)。如表1所示,在诱导人NK细胞增殖方面25pg/ml的RLI仍然优于HDIL-2。在这些猕猴血液中血药浓度的基础上和基于之前所述的公式,我们确定了不同浓度的RLI人类等效剂量(HEDRLI),所述HEDRLI汇总于表5。

Claims (29)
1.一种通过施用一定量的缀合物从而诱导天然杀伤细胞(NK细胞)增殖来治疗受试者的癌症、感染或免疫缺陷障碍的药物组合物,所述增殖与采用高剂量的白细胞介素-2(HDIL-2)得到的增殖相比相同或更高,其中所述缀合物包含:
a)包含白细胞介素15或其衍生物的氨基酸序列的多肽,和
b)包含IL-15Rα的sushi结构域或其衍生物的氨基酸序列的多肽;
最终与药学上可接受的载体相关联。
2.根据权利要求1所述的药物组合物,其中所述缀合物还以诱导CD8T细胞增殖的量施用,所述增殖高于采用HDIL-2得到的增殖。

25.一种用于确定要向患有癌症、感染或免疫缺陷障碍的受试者施用的缀合物的治疗有效量的方法,优选地一种体外方法,所述方法包括以下步骤:
i)在使源自所述受试者的外周血单核细胞(PBMC)能够增殖的培养条件下,使所述PBMC与量渐增的如上所定义的缀合物接触;
ii)在使源自所述受试者的其它PBMC能够增殖的培养条件下,使所述其它PBMC与高剂量的白细胞介素-2(HDIL-2)接触;和
iv)筛选缀合物的治疗有效量,所述治疗有效量诱导所述PBMC的NK细胞增殖,所述增殖与采用HDIL-2得到的增殖相比相同或者更高。
26.根据权利要求25所述的方法,其中所述缀合物的治疗有效量诱导所述PBMC的CD8T细胞增殖,所述增殖与采用HDIL-2得到的增殖相比相同或更高。
27.根据权利要求25或26中任一项所述的方法,其中所述缀合物的治疗有效量对应于诱导的增殖NK细胞和/或CD8T细胞的百分数与诱导的增殖Treg细胞的百分数的比率,所述比率比采用HDIL-2得到的比率高至少25%;优选地高至少50%;和进一步优选地比采用HDIL-2得到的比率高至少75%。
28.根据权利要求25至27中任一项所述的方法,其中所述量渐增的缀合物对应于4fmol/ml至120pmol/ml(0.1至3,000ng/ml)、优选地40fmol/ml至80pmol/ml(1至2,000ng/ml)、和进一步优选地80fmol/ml至40pmol/ml(2至1,000ng/ml)的缀合物浓度。
29.根据权利要求25至28中任一项所述的方法,其中本发明所述的方法进一步包括以下步骤:
iii)在使源自所述受试者的外周血单核细胞(PBMC)能够增殖的培养条件下,使所述PBMC与渐增的与所述缀合物相比等摩尔量的IL-15接触。
