技术领域
本发明涉及医疗装置,更特别是涉及一种具有旋转切割装置的活检装置。
背景技术
通常的活检装置包括:探针组件,该探针组件具有套管,该套管设置成具有试样凹口和组织取样腔室;以及相关联的组织切割机构。在活检处理过程中,可以使用真空辅助装置来帮助将组织穿过试样凹槽吸入取样腔室,并使得通过各试样获得的组织量最大。一些活检装置(通常称为单次插入、多次取样(或SIMS)的装置)利用试样获取和传送机构,该试样获取和传送机构能够从给定病变中获取多个试样,而并不在每次取样后都取出针头和重新插入针头。用于真空辅助的SIMS活检装置中的一种切割机构利用切刀相对于试样凹槽的旋转和线性运动,以便切断穿过试样凹槽被吸入组织取样腔室内的组织。施加真空以便将组织从取样腔室输送至试样收集篮。重复这样的处理,直到获得所需量的组织。

外部套管36的第一孔40和内部套管38的第二孔42中的至少一个具有切削刃44,该切削刃44被削尖至剃刀的尖锐度。例如,切削刃44可以形成于纵向边缘40-1、40-2、42-1和42-2的一个或多个上。具有切削刃44的该纵向边缘40-1、40-2、42-1和42-2的一个或多个的成角度的延伸范围有助于切断组织。

每次形成实际组织试样孔46时,通过真空源16而在内部套管20的第二管腔20-2内形成的负压将实际组织试样孔46附近的周围组织48拉入内部套管20中。槽或槽道(未示出)可以布置在内部套管20中,以便使得真空能够到达第二管腔20-2中的组织收集区域的两侧。然后,当实际组织试样孔46通过外部套管18和内部套管20的继续同时旋转而关闭时,外部套管18的第一孔22和内部套管20的第二孔28进行配合,以便切断被拉入内部套管20内的组织48。被这样切断的各组织试样通过负压而穿过内部套管20的第二管腔20-2被输送给组织试样容器49。
在图6A-6C所示的实施例中,参考图7,包括外部套管36和内部套管38的探针组件34可以安装在驱动器单元14上,且在一种实施方式中,外部套管36和内部套管38可以以不同转速沿相反旋转方向旋转,使得孔40和孔42周期性地对准,以便形成实际组织试样孔50,如图7中所示。不过,在图6A-7所示的实施例中,实际组织试样孔50的最大开口尺寸小于外部套管36的孔40和内部套管38的孔42中的任意一个的开口尺寸。

再参考图1-5,驱动器单元14包括第一驱动机构52、第二驱动机构54、马达56、控制器58和用户界面60。第一驱动机构52设置成与外部套管18可驱动地接合,以便使得探针组件12的外部套管18沿第一旋转方向62以第一转速旋转。第二驱动机构54设置成与探针组件12的内部套管20可驱动地接合,以便与外部套管18的旋转同时地使得内部套管20沿与第一旋转方向62相反的第二旋转方向64以与第一转速不同的第二转速旋转。

在步骤S108中,在已经获取了全部所需的组织试样之后,停止外部套管18和内部套管20的连续同时旋转。外部套管18和内部套管20的连续同时旋转的结束被选择为与第一侧部孔22和第二侧部孔28并不对准的最终位置82(见图5和9)重合,使得在从患者体内取出探针组件12之前,不再形成实际组织试样孔46。

Claims (27)
1.一种活检装置,包括:
探针组件,所述探针组件包括:
第一套管,所述第一套管具有限定第一管腔的第一侧壁,所述第一套管具有第一近端和第一远端,所述第一套管具有第一孔,所述第一孔在靠近所述第一远端处穿过所述第一侧壁延伸至所述第一管腔,所述第一套管具有纵向轴线;以及
第二套管,所述第二套管具有限定第二管腔的第二侧壁,所述第二套管具有第二近端和第二远端,所述第二套管具有第二孔,所述第二孔在靠近所述第二远端处穿过所述第二侧壁延伸至所述第二管腔,所述第二套管与所述第一套管同轴地布置;
所述第一孔和所述第二孔中的至少一个具有切削刃;以及
驱动器单元,所述驱动器单元设置成用于可释放地安装所述探针组件,所述驱动器单元可操作地设置成使得所述第一套管和所述第二套管沿相反的旋转方向以不同的转速同时旋转,使得所述第一孔和所述第二孔周期性地对准,以便形成实际组织试样孔。
2.根据权利要求1或2所述的活检装置,其中:所述第一套管是外部套管,所述外部套管具有位于所述第一远端处的针尖,所述第二套管是位于所述外部套管的所述第一管腔内的内部套管,所述活检装置还包括真空源,所述真空源与内部套管流体连通,以便在每次形成所述实际组织试样孔时通过负压而将组织拉入所述内部套管内,当通过所述外部套管和所述内部套管的继续旋转而关闭所述实际组织试样孔时,所述第一孔和所述第二孔配合以便切断被拉入所述内部套管内的所述组织。
3.根据权利要求1或2所述的活检装置,其中:在活检处理过程中,通过所述第一套管和所述第二套管的连续同时旋转而在相对于所述纵向轴线的多个角径向位置处形成所述实际组织试样孔。
4.根据权利要求3所述的活检装置,其中:所述第二套管位于所述第一套管的所述第一管腔内,所述活检装置还包括真空源,所述真空源与第二套管流体连通,以便在所述第二套管的所述第二管腔内提供负压,在活检处理过程中,每次形成所述实际组织试样孔,组织都被拉入所述第二套管的所述第二管腔内,然后,当通过所述第一套管和所述第二套管的所述连续同时旋转而关闭所述实际组织试样孔时,所述第一孔和所述第二孔配合以便切断被拉入所述内部套管内的所述组织,通过所述负压将被这样切断的各组织试样通过所述第二套管的所述第二管腔输送给组织试样容器。
5.根据权利要求4所述的活检装置,其中:所述驱动器单元包括控制器,所述控制器设置成执行速度分布图,所述速度分布图在所述第一套管和所述第二套管的所述连续同时旋转过程中提供用于所述第一套管和所述第二套管中的至少一个的可变转速。
6.根据权利要求5所述的活检装置,其中:在所述实际组织试样孔开始关闭以便切断所述组织时,所述速度分布图使得所述第一套管和所述第二套管中的至少一个的速度增大。
7.根据权利要求5所述的活检装置,其中:在所述实际组织试样孔开始关闭以便切断所述组织时,所述速度分布图提供用于所述第一套管和所述第二套管的相等的速度幅值。
8.根据权利要求5所述的活检装置,其中:所述速度分布图使得所述第一套管和所述第二套管中的至少一个的转速变化,以便限定下一次形成所述实际组织试样孔的下一个角径向位置。
9.根据前述任意一项权利要求所述的活检装置,其中:所述驱动器单元包括控制器,所述控制器设置成执行速度分布图,所述速度分布图在所述第一套管和所述第二套管的连续同时旋转过程中提供用于所述第一套管和所述第二套管中的至少一个的可变转速。
10.根据权利要求9所述的活检装置,其中:在所述实际组织试样孔开始关闭时,所述速度分布图使得所述第一套管和所述第二套管中的至少一个的转速增大。
11.根据权利要求9所述的活检装置,其中:在所述实际组织试样孔开始关闭时,所述速度分布图提供用于所述第一套管和所述第二套管的相等的速度幅值。
12.根据权利要求9所述的活检装置,其中:所述速度分布图使得所述第一套管和所述第二套管中的至少一个的转速变化,以便限定下一次形成所述实际组织试样孔的下一个角径向位置。
13.根据前述任意一项权利要求所述的活检装置,其中:所述驱动器单元包括:
第一驱动机构,所述第一驱动机构设置成用于与第一套管可驱动地接合,以便使得所述第一套管沿第一旋转方向以第一转速旋转;以及
第二驱动机构,所述第二驱动机构设置成用于与第二套管可驱动地接合,所述第二驱动机构设置成在所述第一套管旋转的同时使得所述第二套管沿与所述第一旋转方向相反的第二旋转方向以与所述第一转速不同的第二转速旋转。
14.根据权利要求13所述的活检装置,其中:所述第一驱动机构和所述第二驱动机构具有与它们相关联的马达,所述驱动器单元还包括控制器,所述控制器与所述马达通信连接,所述控制器设置成控制所述马达,以便使得所述第一套管根据第一速度分布图旋转和使得所述第二套管根据第二速度分布图旋转。
15.根据权利要求13所述的活检装置,其中:所述第一驱动机构具有第一马达,所述第二驱动机构具有第二马达,所述驱动器单元还包括控制器,所述控制器与所述第一马达和所述第二马达通信连接,所述控制器设置成控制所述第一马达以便使得所述第一套管根据第一速度分布图旋转,并控制所述第二马达以便使得所述第二套管根据第二速度分布图旋转。
16.根据权利要求1或2所述的活检装置,还包括:
第一齿轮,所述第一齿轮固定地附接在所述第一套管上,用于绕所述纵向轴线旋转;
第二齿轮,所述第二齿轮固定地附接在所述第二套管上,用于绕所述纵向轴线旋转;以及
所述驱动器单元包括:
第一齿轮驱动机构,所述第一齿轮驱动机构与所述第一齿轮接合;
第二齿轮驱动机构,所述第二齿轮驱动机构与所述第二齿轮接合;
马达,所述马达与所述第一齿轮驱动机构和所述第二齿轮驱动机构中的每一个可驱动地连接;以及
控制器,所述控制器与所述马达通信连接,所述控制器设置成控制所述马达,以便使得所述第一套管根据第一速度分布图旋转和使得所述第二套管根据第二速度分布图旋转。
17.根据前述任意一项权利要求所述的活检装置,其中:所述第一孔的纵向延伸范围平行于所述纵向轴线,所述第二孔的纵向延伸范围平行于所述纵向轴线,使得所述实际组织试样孔的最大开口尺寸等于所述第一孔和所述第二孔中每一个的相应开口尺寸中的较小一个。
18.根据权利要求17所述的活检装置,其中:至少所述第一孔为椭圆形,所述切削刃沿所述第一孔的所述纵向延伸范围形成。
19.根据权利要求1至16中任意一项所述的活检装置,其中:所述第一孔的纵向延伸范围沿第一方向相对于所述纵向轴线成角度,所述第二孔的纵向延伸范围沿第二方向相对于所述纵向轴线成角度,所述第二方向与所述第一方向交叉,使得所述实际组织试样孔的最大开口尺寸小于所述第一孔和所述第二孔中任一个的开口尺寸。
20.根据权利要求19所述的活检装置,其中:所述切削刃沿所述第一孔的所述纵向延伸范围形成。
21.一种活检装置,包括:
探针组件,所述探针组件包括:
第一套管,所述第一套管具有限定第一管腔的第一侧壁,所述第一套管具有第一近端和第一远端,所述第一套管具有第一孔,所述第一孔在靠近所述第一远端处穿过所述第一侧壁延伸至所述第一管腔,所述第一套管具有纵向轴线;以及
第二套管,所述第二套管具有限定第二管腔的第二侧壁,所述第二套管具有第二近端和第二远端,所述第二套管具有第二孔,所述第二孔在靠近所述第二远端处穿过所述第二侧壁延伸至所述第二管腔,所述第二套管与所述第一套管同轴地布置;
所述第一孔和所述第二孔中的至少一个具有切削刃;以及
驱动器单元,所述驱动器单元设置成用于可释放地安装探针组件,所述驱动器单元可操作地设置成使得所述第一套管根据第一速度分布图旋转和使得所述第二套管根据第二速度分布图旋转,以便在活检处理过程中通过所述第一套管和所述第二套管的连续同时旋转而使得所述第一孔和所述第二孔周期性地对准,从而在相对于所述纵向轴线的多个角径向位置处形成实际组织试样孔。
22.根据权利要求21所述的活检装置,其中:所述第一套管和所述第二套管沿相反的旋转方向旋转。
23.根据权利要求21所述的活检装置,其中:所述第二套管位于所述第一套管的所述第一管腔内,所述活检装置还包括真空源,所述真空源与第二套管流体连通,以便在所述第二套管的第二管腔内提供负压源,在所述活检处理过程中,每次形成所述实际组织试样孔,组织都被拉入所述第二套管的所述第二管腔内,然后,当通过所述第一套管和所述第二套管的所述连续同时旋转而关闭所述实际组织试样孔时,所述第一孔和所述第二孔配合以便切断被拉入所述内部套管内的所述组织,通过所述负压将被这样切断的各组织试样通过所述第二套管的所述第二管腔输送给组织试样容器。
24.一种用于在活检处理过程中控制活检装置的方法,所述活检装置具有:探针组件,所述探针组件具有外部套管和内部套管,所述外部套管具有远侧针尖,所述内部套管相对于纵向轴线与所述外部套管同轴地布置,所述外部套管具有第一侧部孔,所述内部套管具有第二侧部孔,所述第一侧部孔和所述第二侧部孔中的至少一个具有切削刃;以及真空源,所述真空源连接成与所述内部套管的管腔和组织试样容器流体连通,所述方法包括:
将所述外部套管和所述内部套管中的每一个定位在相应的初始旋转位置;
将所述探针组件插入患者要进行活检的区域中;
建立所述外部套管根据第一速度分布图和所述内部套管根据第二速度分布图的连续同时旋转,以使得所述第一侧部孔和所述第二侧部孔周期性地对准,从而在相对于所述纵向轴线的多个角径向位置处形成实际组织试样孔;
在所述内部套管的所述管腔内形成负压供给,使得每次形成所述实际组织试样孔,都将组织通过所述实际组织试样孔拉入所述内部套管的所述管腔内,然后,当通过所述外部套管和所述内部套管的连续同时旋转而使得所述实际组织试样孔关闭时,所述第一侧部孔和所述第二侧部孔配合,以便切断被拉入所述内部套管内的所述组织,通过所述负压将被这样切断的各组织试样通过所述内部套管的所述管腔输送给组织试样容器;以及
在已经获取所需全部的组织试样之后停止所述外部套管和所述内部套管的所述连续同时旋转。
25.根据权利要求24所述的方法,其中:所述外部套管和所述内部套管沿相反的旋转方向旋转。
26.根据权利要求24所述的方法,其中:所述外部套管和所述内部套管的所述相应的初始旋转位置被选择成使得所述第一侧部孔和所述第二侧部孔并不对准,使得在将所述探针组件插入所述患者体内之前并不形成所述实际组织试样孔。
27.根据权利要求24所述的方法,其中:所述外部套管和所述内部套管的所述连续同时旋转被选择成与最终位置重合,在所述最终位置,所述第一侧部孔和所述第二侧部孔并不对准,使得在将所述探针组件从所述患者体内取出之前不再形成所述实际组织试样孔。
