Description
また、本発明の基準義歯用位置合せ冶具は、患者(ヒト)の口腔内のみならず、上下無歯顎模型に対して用いることができる。本発明の基準義歯用位置合せ冶具を用いる義歯の作製方法は、上下無歯顎模型に適用することができ、患者(ヒト)に直接適用する場合は除かれる。なお、本願明細書においては、本発明の基準義歯用位置合せ冶具を用いる義歯の作製方法について、患者(ヒト)に直接適用する場合を含めて説明する。

以下、図面を参照しながら本発明を詳細に説明する。なお、以下の説明において、垂直、直交とは、角度90度である場合のみならず、80~100度の範囲を含むものとする。また、平行とは、完全な平行である場合のみならず、該完全な平行に対して-10~+10度の範囲を含むものとする。
また、柄の厚みは、1~3mmが好ましく、2mmである事が特に好ましい。このような範囲に柄の厚みを設定する事で、柄と上唇及び/又は下唇との位置関係により、前歯切縁位置を設定する事が出来る。
図8は、本発明の基準義歯用位置合せ冶具の使用状態を示す図である。
この基準義歯用位置合せ冶具101の上顎義歯保持溝15、及び下顎基準義歯保持溝17には、所定の咬み合わせが維持された状態で上顎基準義歯91、及び下顎基準義歯93がそれぞれ嵌合されて保持される。これにより、所定の咬み合わせが維持された状態で、一対の基準義歯が基準義歯用位置合せ冶具101に保持される。この上顎基準義歯91、及び下顎基準義歯93が保持された基準義歯用位置合せ冶具101は、ヒトの口腔内、又は上下無歯顎模型が固定された咬合器内に挿入される。その後、柄部11を外部から操作することによって、上顎基準義歯91、及び下顎基準義歯93の位置が調整される。
図3は、正中指標を有する本発明の基準義歯用位置合せ冶具の構造の一例を示す図であり、(a)は平面図、(b)は側面図、(c)は正面図である。なお、この基準義歯用位置合せ冶具において、底面図は平面図と実質的に同一であり、右側面図は左側面図と実質的に対称である。図2と同じ構成については、同じ符号を付してその説明を省略する。
図4は、スライド可能に設けられた正中指示用ピンを有する本発明の基準義歯用位置合せ冶具の構造の一例を示す図であり、(a)は平面図、(b)は側面図、(c)は正面図である。図3と同じ構成については、同じ符号を付してその説明を省略する。
柄部11にはスライド用溝31が形成されており、正中指示用ピン21はこの溝に沿って、図4(b)中の符号Cの範囲でスライド可能に配設されている。図4中、符号33はナットである。即ち、正中指示用ピン21は、ナット33を緩めることにより、溝31に沿ってスライド可能となる。ナット33を締めることにより、正中指示用ピン21は所期の位置に固定される。
図9(a)は、本発明の基準義歯用位置合せ冶具の使用状態を示す正面図であり、図9(b)は側面図である。
この基準義歯用位置合せ冶具102の上顎義歯保持溝15、及び下顎基準義歯保持溝17には、所定の咬み合わせが維持された状態で上顎基準義歯91、及び下顎基準義歯93がそれぞれ嵌合されて保持される。これにより、所定の咬み合わせが維持された状態で、一対の基準義歯が基準義歯用位置合せ冶具102に保持される。この際、各基準義歯の中切歯の正中の位置は、正中指標(正中指示用ピン)21に一致するように嵌合される。
この上顎基準義歯91、及び下顎基準義歯93が保持された基準義歯用位置合せ冶具102は、ヒトの口腔内、又は上下無歯顎模型が固定された咬合器内に挿入される。その後、柄部11を外部から操作することによって、上顎基準義歯91、及び下顎基準義歯93の位置が調整される。基準義歯用位置合せ冶具102は、正中指標21を有しているので、患者の口腔外、又は咬合器外から、この正中指標21を用いて位置合わせを容易に行うことができる。即ち、ヒトの顔の鼻頭と正中指標の一端と他端とが一直線上に存在するように調整すること等により、基準義歯の位置や左右の傾きを適切な位置に調整できる。また、正中指示用ピンは、口腔内又は咬合器内における基準義歯の奥行方向への位置を適切な位置に調整する指標にもなる。正中指示用ピンがスライド可能に設けられている場合、ヒトの顔や咬合器の大きさに応じて、正中指示用ピンの位置は適宜調整される。
当接部43、47はヒトの顔や咬合器の表面に当接させる部分であり、本発明において必須ではなく、ロッドの先端をヒトの顔や咬合器の表面に直接当接させても良い。また、当接部43、47は、ロッド41、45と一体となって成形されていても良い。
図10(a)は、本発明の基準義歯用位置合せ冶具の使用状態を示す正面図であり、図10(b)は側面図である。
この基準義歯用位置合せ冶具104の上顎義歯保持溝15、及び下顎基準義歯保持溝17には、所定の咬み合わせが維持された状態で上顎基準義歯91、及び下顎基準義歯93がそれぞれ嵌合されて保持される。この際、各基準義歯の中切歯の正中の位置は、正中指標(正中指示用ピン)21に一致するように嵌合される。これにより、所定の咬み合わせが維持された状態で、一対の基準義歯が基準義歯用位置合せ冶具104に保持される。
この上顎基準義歯91、及び下顎基準義歯93が保持された基準義歯用位置合せ冶具104は、ヒトの口腔内、又は上下無歯顎模型が固定された咬合器内に挿入される。その後、柄部11を外部から操作することによって、上顎基準義歯91、及び下顎基準義歯93の位置が調整される。このとき、上顎側の当接部43は、鼻下点又は咬合模型上の上顎前庭部最下点に当接させる。下顎側の当接部47は、オトガイ隆起又は模型上の下顎前庭部最下点に当接させる。基準義歯用位置合せ冶具104は、ロッド41、45を有しているので、患者の顔の一部又は咬合器にロッドの先端を当接させることにより、口腔内又は咬合器内における基準義歯の奥行方向への位置を適切な位置に調整することが容易となる。
図11(a)は、本発明の基準義歯用位置合せ冶具の使用状態を示す正面図であり、図11(b)は側面図である。
この基準義歯用位置合せ冶具105の上顎義歯保持溝15、及び下顎基準義歯保持溝17には、所定の咬み合わせが維持された状態で上顎基準義歯91、及び下顎基準義歯93がそれぞれ嵌合されて保持される。この際、各基準義歯の中切歯の正中の位置は、正中指標(正中指示用ピン)21に一致するように嵌合される。これにより、所定の咬み合わせが維持された状態で、一対の基準義歯が基準義歯用位置合せ冶具105に保持される。
この上顎基準義歯91、及び下顎基準義歯93が保持された基準義歯用位置合せ冶具105は、ヒトの口腔内、又は上下無歯顎模型が固定された咬合器内に挿入される。その後、柄部11を外部から操作することによって、上顎基準義歯91、及び下顎基準義歯93の位置が調整される。このとき、平面指標51の前後左右における傾きを確認することにより、口腔内又は咬合器内における基準義歯の咬合面の位置を適切な位置に調整することが容易となる。また、咬合器内における場合は、平面指標51と、上下無歯顎模型における仮想咬合平面と、を一致させることにより、基準義歯の咬合面の位置を適切な位置に調整することが容易となる。
本発明の部分床義歯作製キットは、前述の基準義歯用位置合せ冶具と、基準義歯と、からなり、裏装材を含んで構成されても良い。
上記の義歯作製キットを用いる義歯の作製方法としては、
先ず本発明の基準義歯用位置合せ冶具に所定の咬み合わせが維持された状態で基準義歯を保持し、
次いでこの基準義歯の内面側に裏装材を築盛し、
上記で説明したように、この基準義歯を保持した冶具をヒトの口腔内又は咬合器内に挿入して、基準義歯に築盛された裏装材に形状を印記する。これにより、ヒトの口腔内又は咬合器にセットされた模型の内面形状が裏装材に印記され、義歯が作製される。
マネキン(ニッシムタイプII、ニッシン製)に上下無歯顎模型を取り付け、通法に従い、以下の設定位置で上下総義歯を作製した。
(1)前歯部左右:無歯顎模型の正中と、総義歯の左右中切歯の接触点を一致させる(上下顎とも)。
(2)臼歯部左右:無歯顎模型の正中と、総義歯の左右第一大臼歯中心窩を結ぶ線の中点を一致させる(上下顎とも)。
(3)前歯部上下:上顎総義歯の前歯部において、床縁から中切歯切縁までの距離を22mm(ノギスで測定)とする。
(4)前歯部上下:下顎総義歯の前歯部において、床縁から中切歯切縁までの距離を20mmとする。
(5)前歯部上下:上下総義歯を咬合させた状態において、上顎前歯部床縁から下顎前歯部床縁までの距離を40mmとする(上下顎の中切歯が2mm垂直的に被蓋する)。
(6)臼歯部上下:鼻下点から耳珠点を結んだ平面(カンペル平面)と、下顎中切歯切縁と左右下顎第二大臼歯遠心頬側咬頭頂を連なる平面を平行とする。
※下顎に平板を載せ、カンペル平面との傾きの差を分度器で測定
(7)前後:上顎無歯顎模型の切歯乳頭中心点から上顎中切歯切縁までの距離を9mmとする(写真を撮影し、投影図における距離を測定)。
10名の被験者に、上下基準義歯と硬質裏装材(製品名:トクヤマリベースIII)、及び位置合せ冶具を用いて総義歯を作製させた。得られた総義歯の位置(上記パラメータ)を測定した。各パラメータを参考例1で作製した総義歯と比較し、以下の基準で○×判定した。各パラメータにおいて○の人数を表1に示した。
用いた位置合せ冶具は、図2に記載する構造の物である。この基準義歯保持部に基準義歯をセットし、基準義歯内面に硬質裏装材を盛り付け、マネキンの口腔内に挿入し、裏装材に粘膜面形状を印記、硬化させることで義歯を作製させた。
位置合せ冶具として、図2に記載する構造の物の柄部に正中指示ラインが示されているものを用いた以外は実施例1と同様に義歯を作製させた。なお、作製の際には、正中指示ラインとマネキンの正中を合わせるように指示した。
位置合せ冶具として、図3に記載する構造の物を用いた以外は実施例1と同様に義歯を作製させた。正中指示用ピンの長さは40mmである(柄を中心に上下20mmずつ、計40mm)。なお、作製の際には、正中指示用ピンの上端とマネキンの鼻下点とをあわせ、正中指示ピンの下端をマネキンのオトガイ隆起(水平的な最豊隆部)を合わせるように指示した。
実施例3の位置合せ冶具に、図6に記載する平面指標を追加した以外は実施例3と同様に義歯を作製させた。なお、作製の際には、平面指標を、マネキンの鼻下点と耳珠点を結ぶカンペル平面と平行とする事を指示した。
位置合せ冶具として、図6に記載する構造の物を用いた以外は実施例1と同様に義歯を作製させた。基準義歯を位置合せ冶具に取り付け、マネキンの口腔内に挿入させた。
マネキンの下唇と位置合せ冶具の基準義歯固定部とが軽く接触する位置で前後位置が固定されるよう、正中指示ピンをスライドさせ、ロッドがマネキンのオトガイ隆起と接触する位置で固定させた。その状態で口腔外に基準義歯と位置合せトレーを取り出した。その後は、実施例4と同様に義歯を作製させた。
位置合せ冶具を用いずに、10名の被験者に、上下基準義歯と硬質裏装材(製品名:トクヤマリベースIII)を用いて総義歯を作製させた。得られた総義歯の位置を実施例1と同様に測定した。

前記基準義歯用位置合せ冶具の前記ロッドの先端を、上下無歯顎模型における上顎前庭部最下点に当接させるとともに、前記基準義歯に築盛された裏装材に形状を印記することを特徴とする義歯作製方法。 - 請求項7又は8に記載の基準義歯用位置合せ冶具に基準義歯を保持し、
前記基準義歯の内面側に裏装材を築盛し、
前記基準義歯用位置合せ冶具の前記ロッドの先端を、上下無歯顎模型における下顎前庭部最下点に当接させるとともに、前記基準義歯に築盛された裏装材に形状を印記することを特徴とする義歯作製方法。 - 請求項10~12の何れか1項に記載の基準義歯用位置合せ冶具に基準義歯を保持し、
前記基準義歯の内面側に裏装材を築盛し、
前記基準義歯用位置合せ冶具の前記平面指標と、上下無歯顎模型における仮想咬合平面と、を一致させるとともに、前記基準義歯に築盛された裏装材に形状を印記することを特徴とする義歯作製方法。
